2. Adherence to antiretroviral therapy and factors associated with non-adherence: a cohort study at two referral services in Brazil.

This cohort study evaluated non-adherence to antiretrovirals at referral services in Pernambuco, Brazil, 2016/2017, through self-report. A generalized mixed-effects model for binary outcomewas used. We assessed 542 participants with an adherence rate of 85.50%. A greater chance of non-adherence was associated with:a low/moderate level of nicotine dependence (OR = 2.79, p = 0.00, IC = 1.44-5.41); ≥7 tablets/day (OR = 6.14, p = 0.00, IC = 3.42-11.02); LPV/r (OR = 1.49, p = 0.6, IC = 0.98-2.26), ddI (OR = 3.34, p = 0.03, IC = 1.12-9.97), ABC (OR = 4.02, p = 0.05, IC = 1.01-16.03), RAL (OR = 2.49, p = 0.01, IC = 1.32-4.70) and DTG (OR = 4.65, p = 0.01, IC = 1.42-15.16); 6–10 year seropositive diagnosis (OR = 2.17, p = 0.01, IC = 1.20-3.92) and symptoms of depression (OR = 1.55, p = 0.03, IC = 1.03-2.33). Protective factors for non-adherence weres: ≥50 years (OR = 0.67, p = 0.06, IC = 0.45-1.01), secondary/higher education (OR = 0.48, p = 0.00, IC = 0.34-0.70), embarrassment at health service (OR = 0.49, p = 0.04, IC = 0.24-0.97), good understanding of antiretrovirals (OR = 0.62, p = 0.03, IC = 0.40-0.96), adverse event (OR = 0.74, p = 0,06, IC = 0.54-1.01), use of TDF (OR = 0.62, p = 0.01, IC = 0.43-0.90), NVP (OR = 0.41, p = 0.05, IC = 0.71-1.00) and EFZ (OR = 0.48, p = 0.01, IC = 0.29-0.80) and good knowledge of HIV/AIDS/ART. Objective: to analyze the access of women living with HIV to health care services. Method: qualitative, participant research, mediated by interview and observation with 14 women in the health services of a city in southern Brazil. Content analysis was developed, based on the theoretical categories of the structural and procedural elements of access according to Starfield. Results: the access of women living with HIV to Specialized Services and Primary Health Care occurred at the woman's own initiative. The link occurred mainly in the specialized service that best embraced and met their needs, regardless of the psychosocial relationships established, waiting time for care and geographic location of health services. Final considerations: several factors interfere positively or negatively in women's access to health policy, highlighting the lack of efficient communication between professionals from different services, which affects women's access to information and procedures. Counseling is a tool for prevention, risk assessment and emotional support, recommended in the field of HIV/AIDS and Sexually Transmitted Infections (STIs). This study aimed to identify and analyze the scientific production about the practice of counseling on HIV/AIDS and other STIs in Primary Health Care (PHC) in Brazil. A narrative review of the literature was carried out in the Scielo, Pepsic and Lilacs databases, using the descriptors “counseling”, “hiv”, “aids”, “primary care” and “primary care”. Twelve articles were selected according to inclusion criteria. The studies are characterized by qualitative approaches, carried out with primary care professionals, in the period 2006-2018. All studies point to difficulties, such as the limitation of practice with emphasis on the technical part of the rapid test, insecurity of professionals and the need for permanent education. Some also point out potentialities such as the recognition of the importance of counseling and matrix support. We emphasize the need for studies exploring the perspective of users of the health system. [ABSTRACT FROM AUTHOR]

Objective: to analyze the knowledge of nursing students from different undergraduate grades in relation to HIV/ AIDS and syphilis. Method: descriptive-exploratory research, developed with 65 nursing students from a university in southern Brazil. Data were collected by an instrument adapted from the World Health Organization. KruskalWallis test and fisher significant minimum difference was applied for comparison. Results: there was a significant difference in knowledge between the series in specific aspects of infections, such as: HIV immunological window, aids pathophysiology, mode of transmission of syphilis, undetectable method = non-transferable of HIV and characterization of syphilis phases. Conclusion: the acquisition of knowledge about HIV/AIDS and syphilis among nursing students tends to evolve throughout graduation, since the last grades presented better scores. However, the previous knowledge of the students of the initial grades was evident and, at times, it was not inferior in relation to the other ones. [ABSTRACT FROM AUTHOR]

TB-HIV is a challenge for the public health system, especially in regions in socioeconomic development. The purpose of the study is to identify social, health-disease and health care aspects of people with TB-HIV coinfection who underwent tuberculosis treatment. It is a cross-sectional study carried out in two large Brazilian municipalities located, both, in the state of Paraíba. It had as a population all patients coinfected with TB-HIV with diagnosis and treatment of tuberculosis in 2016. From cadastral sampling, 35 people participated. The data were from July to October 2017, from semi-structured interviews, with a questionnaire that included social, health-disease and health care variables. The bivariate analysis was performed using SPSS v.13.0. There was a predominance of males in the young-adult phase, low schooling, with exposure to violence. It was also evidenced the impediment of carrying out occupational activities after becoming ill, late diagnoses of tuberculosis, lack of antituberculostatic and antiretroviral drugs. Exposure to violence, withdrawal from occupational activities and lack of medication during treatment were unpublished findings and were configured as challenges that probably discourage therapeutic adherence and make it difficult to control both ailments. These results reveal the need for intersectoral actions with the purpose of developing strategic actions that contribute to the reduction of social inequalities and, consequently, to the control of grievances. Objective: to analyze the barriers to access HIV rapid test from the perspective of the discourse of managers of Primary Health Care services. Method: exploratory, qualitative research conducted in a health district of João Pessoa, Paraíba, Brazil. Data collection was performed in September 2017 with 13 health managers. The empirical material was based on the theoretical-methodological contribution of the Discourse Analysis of the French matrix. Results: the discursive block "Barriers of user access in the health system to perform the HIV rapid test in Primary Health Care" was identified. The discourses revealed barriers of access tied to the dimensions: geographical accessibility, availability, feasibility and acceptability. Final considerations: the units have the potential to strengthen the performance of the HIV rapid test, however, they need political support aimed at strategies to strengthen the performance of the rapid test with scope to reduce access barriers. Objectives: There is a paucity of data on cardiovascular disease (CVD) among people living with HIV (PLHIV) in resource-limited countries. We assessed factors associated with CVD and the impact of prevalent CVD on all-cause mortality in PLHIV on antiretroviral therapy in Brazil.Methods: Competing risk regression to assess factors associated with CVD and all-cause mortality in the HIV-Brazil Cohort Study between 2003 and 2014.Results: Among 5614 patients, the rate of CVD was 3.5 (95% confidence interval [95% CI] 2.9-4.3) per 1000 person-years. CVD was associated with older age (adjusted hazard ratio [aHR] 6.4 for ≥55 years vs. <35 years, 95% CI: 2.5-16.3, P < 0.01), black race (aHR 1.8 vs. white race, 95% CI: 1.0-3.1, P = 0.04), past CVD (aHR 3.0 vs. no past CVD, 95% CI: 1.4-6.2, P < 0.01), hypertension (aHR 1.8 vs. no hypertension, 95% CI: 1.0-3.1, P = 0.04), high-grade dyslipidemia (aHR 9.3 vs. no high-grade dyslipidemia, 95% CI: 6.0-14.6, P < 0.01), ever smoking (aHR 2.4 vs. never, 95% CI: 1.2-5.0, P = 0.02) and low nadir CD4 cell count (aHR 1.8 for 100-250 cells/mm3 vs. >250 cells/mm3 , 95% CI: 1.0-3.2, P = 0.05). The rate of death was 16.6 (95% CI: 15.1-18.3) per 1000 person-years. Death was strongly associated with having had a past CVD event (aHR 1.7 vs. no past CVD event, 95% CI: 1.1-2.7, P = 0.01).Conclusions: Traditional and HIV-specific factors associated with CVD among PLHIV in Brazil are similar to those identified among PLHIV in high-income countries. PLHIV in Brazil with a history of CVD have a high risk of death. CVD care and treatment remain priorities for PLHIV in Brazil as this population ages and antiretroviral therapy use expands. Introduction: Human immunodeficiency virus (HIV) infection due to its severity, mainly due to the evolution of opportunistic diseases and its pandemic character, represents a current health problem. In this context, religion and spirituality collaborate to confront this health problem. Objectives: To analyze the stigmas that involve religious beliefs and people living with HIV (PLHA). Methodology: A cross - sectional, qualitative descriptive study that used a semi - structured interview script in 17 patients undergoing treatment at the Immunology outpatient clinic of a University Hospital of Rio de Janeiro. The qualitative analysis of the speeches used the Content Analysis method, aiming at interpreting the meaning of the narratives of the subjects interviewed. Discussion: The stigmas surrounding disease and prejudice promote behavioral changes in the individual. However, Religious Beliefs and Spirituality influence the breaking of paradigms and minimize the suffering of these patients, consolidating a network of solidarity involving religion and patient, which provides improvements in the quality of life of these individuals. This study seeks to identify challenges in the implementation of post-exposure prophylaxis for HIV, based on an analysis of actions taken by healthcare professionals in the state-run health sector in Porto Alegre, Brazil. Based on a qualitative approach that included ethnographic observations and in-depth interviews, we found that contextual, institutional, and individual factors represented challenges to the implementation of postexposure prophylaxis for HIV. Barriers to implementation included the historical context structuring healthcare services and practices, the lack of training and/or continued education in health, and certain attitudes on the part of healthcare professionals (ideas regarding both the strategy itself as well as the individuals that seek PEP). We conclude that there is a need for greater attention to specialized services for STI/HIV/AIDS as well as the professionals that provide these services, in order to guarantee greater effective access to this strategy at the local level. Objective: To characterize the scientific production on the HIV/AIDS vulnerability in the elderly. Method: Integrative revision, from the guiding question how is HIV/AIDS vulnerability being approached in Brazilian literature? The search for scientific production was conducted on MEDLINE, BDENF and LILACS data base, employing the HIV, aged and health vulnerability descriptors. Results: 19 articles divided into two categories: vulnerability and HIV in elderly people were found; And the diagnosis and intervention in elderly with HIV. Conclusion: The stigma of the elderly population, prejudice and belief that aging is related to the loss of sexual desire causes an increase in the number of elderly people with HIV/AIDS. It is necessary for the studies to address the issues of the social representation of HIV to the elderly, in addition to specific interventions to this age group to improve the quality of life of this population. Objective: To learn the perceptions of technical and administrative workers from a nursing course at a public university in the northern region of Brazil on HIV/AIDS. Methodology: Descriptive exploratory study with a qualitative approach. Data were collected by means of semi-structured interviews from February to May 2015, with the participation of 30 technical and administrative workers. The corpus was submitted to thematic analysis. Results: Perceptions are multidimensional because they are based on three dimensions: knowledge of common sense (popular dimension); feelings (subjective dimension); and HIV/AIDS - a deadly disease (imagetic dimension). Conclusions: The perceptions of technical and administrative professionals remain associated with the knowledge shared in their social groups, receiving little influence from the scientific information circulating within the nursing course. Introduction: The prevention of HIV or AIDS must consist in a strategy that takes into account the biomedical, behavioral and structural approaches to the population that is directed; it is what is called combined prevention (CP). Objectives: This research aims to know if generalist media talk about prevention or PC when they deal with information related to HIV or AIDS and the city of Barcelona. Methodology: The study is an analysis of quantitative content of all the information, regardless of the medium, that speak about HIV or AIDS and Barcelona in the period between September 1, 2017 and September 1, 2018; for this issue, the Kantar Media analysis and monitoring platform has been used from a series of key words. Results: Most of the information does not explicitly talks about prevention (13,82%) and that biomedical approaches prevail, being structurally nonexistent. On the other hand, the analysis of the data shows that the informants do not handle the CP concept. Conclusion: We believe that the media should talk more frequently about prevention, handle the CP concept and deal more often with behavioral and structural approaches. [ABSTRACT FROM AUTHOR]

Introduction: This paper argues the role of Discourse Analysis - DA (Michel Pêcheux, Eni Orlandi) as a theoretical-methodological contribution to the evaluation of health campaigns. Objectives: It shows how DA can be applied to identify and understand discourses of resistance to health promotion campaigns. Methodology: It analyses the HIV prevention campaign of the Brazilian Ministry of Health designed for the carnival of 2010, as well as individual interviews with two people living with HIV. Results: By comparing the discourses mapped in the campaign and those present in the interviews, some dissonance of senses, understood as forms of resistance, are identified. Discussion: Such results also lead to the discussion regarding biopower issues (Michel Foucault) and the relationship between power and resistance in historical processes of subjectivation. Conclusion: It is concluded that, before trying with campaigns to change individual's practices towards behaviors that promote health, institutional practices must be changed in order to prioritize listening to people when designing such campaigns, by applying Discourse Analysis's theoretical-methodological contributions. Hegemonic masculinity has distanced heterosexually active men from the responses to the HIV epidemic. However, with the combined prevention paradigm, postexposure prophylaxis offers new ways to manage the risks of HIV infection, while at the same time bringing about new challenges. The aim of this paper is to discuss - through the lens of gender and masculinities - how heterosexually active men perceive their HIV risk and how they manage prevention methods and strategies, including post-exposure prophylaxis. Employing qualitative methods, 16 heterosexually active men in five health services from different Brazilian cities were interviewed. The analysis indicates that condom use - the main prevention method - was conditioned by subjective and contextual factors, and its failure stands out as the primary reason for seeking out post-exposure prophylaxis. We argue that ideals of hegemonic masculinity permeate perceptions and risk management discourses in the current context of the HIV/AIDS epidemic. [ABSTRACT FROM AUTHOR]
