Climate change and development projects towards the Brazilian Amazon have affected traditional livelihoods and imposed transformations on these cultures. In this context, this study aimed to understand the inner workings of the social learning processes in the riverside community 'São Carlos do Jamari' triggered by its historical flood. An in-depth case study was jointly constructed, and semi-structured interviews and focus groups were developed with residents of the community, leaders of the Movement of People Affected by Dams (MAB) and workers of the local civil defense. Our findings indicate that the main shifting elements that fostered and were driven by social learning processes are related to Community-Community relationships, Community-Territory relationships, and the Community-State relationships. MAB's role as an educational agent is highlighted, while public authorities hinder social participation in shared management. The proposals of social learning framework are taken as a possible way of overcoming local vulnerabilities and expanding social participation.

Purpose: The purpose of this paper is to characterize the trends for educational innovation in higher education in Brazil, constructing a conceptual model of innovation trends in the sector. Design/methodology/approach: A preliminary profile online was done with 76 experts in university education, and 17 were contacted for in-depth perceptions. The analysis of the content was made of all material and, as a result, the critical analysis of the results, which culminated in the development of a conceptual model and characterization of trends, dimensions and subdimensions to innovation in higher education. Findings: The dimension universal design of accessibility and learning had major considerations, contributing to implementation of new innovative practices for higher education. Some subdimensions emerged, namely, governance, risk management, curricular extension and affirmative policies. Research limitations/implications: The difficulty in performing the deepening of all dimensions involved in terms of plurality of specialties involved. Practical implications: The use of the model and characterization of trends could serve as tools to support the strategic planning of HEI, and the trends allow planning innovation practices, favoring improvements of higher education institutions (HEI), students, employees and community to learning organization. Social implications: The identification of trends for higher education, highlighting innovation indicators or successful practices, and the characterization of the dimensions and subdimensions trends, and undeniable contribution to measure the educational innovation in higher education. Originality/value: Encourages researchers, in partnership with institutions, to develop scientific projects with other institutions and researchers, to meet interests not only of HEI as a whole but also of countries that prioritize education with quality, to reach the real educational objectives.

Camping in Malaysia, New Zealand, England, Brazil, and the United States is compared. The basic function of camp, providing a safe place for fun and discovery, seems universal. Falls are among the top 10 causes of years lived with disability in people aged 75 and over. Preventive programs like case management (CM) are crucial. Objectives: To evaluate the effects of a multifactorial fall prevention program based on CM on physical performance, the presence of pain, and the risk of falls and fractures in older people who have suffered falls. Methods: This randomized, single-blind clinical trial with parallel groups, Intervention Group (IG) and Control Group (CG), was composed of 55 older people with a history of falling, living in the community. All participants underwent an initial assessment via video call (containing anamnesis, timed up-and-go test, falls risk score, short physical performance battery, and clinical frax). The IG underwent CM, the physical exercise protocol, and the cognitive stimulation protocol. The CG was monitored through telephone calls and received general health and fall guidance. Results: No significant results were found in the physical capacity, the presence of pain, the risk of falls, or the fractures between the Intervention and Control Groups and between assessments. Conclusion: This program was not effective in improving functional performance, but it was important for characterizing pain and the probability of fracture in the next 10 years in this population. The high costs of environmentally responsible and socially just approaches are some of the factors that hinder the implementation of sustainable projects in the industrial sector. However, upstream collaborative integration in the corrugated cardboard industry in southeastern Brazil is a reality. In this context, this study aimed to highlight the structure of a collaborative sustainable project in the context of the corrugated cardboard industry, to analyze ESG risk management in the corrugated cardboard industry in the Southeast Region of Brazil, the relationship between ESG risk management and social responsibility in corrugated cardboard industries, the strengths and weaknesses of risk management models in use by the consulted participants, and to propose innovations regarding risk analysis, applicable in sustainable projects with a collaborative bias. The methodology adopted was the case study, with consultation with managers in professional activity in the industrial sector, corroborated by document analysis and systematic review of the literature. We identified preliminary risk analysis (PRA) and failure mode analysis (FMEA) as key practices used by managers in the corrugated paper industry in the region. These practices are insufficient in the convergence of objectives, engagement with the community, and extension of the lifecycle of products and processes. Our findings show that the sustainability of a project may be an extension of the risk analyses carried out in the initial phase, mainly about strategic decision-making and the operationalization of investments. Without bias to the need for risk analysis, the life cycle analysis is also an effective way of highlighting the critical factors, weaknesses, and strengths of a sustainable project. The study indicates the so-called GUT matrix approach (gravity – urgency – tendency) as the most suitable for innovation in risk analyses oriented towards sustainable projects in the analyzed sector. This study investigates the influence of risk management on collaborative innovation and the intervention of communication intensity in agricultural cooperatives. A survey was conducted with managers of Brazilian agricultural cooperatives listed in the Organization of Cooperatives of Brazil. The structural equation modeling technique was applied to a sample of 103 valid answers. The results show that risk management positively influences collaborative innovation and communication intensity, and that communication intensity positively influences collaborative innovation. Furthermore, a mediating effect of communication intensity was observed in the relationship between risk management and collaborative innovation. Theoretical implications arise from the observation of the intervention of communication intensity in this relationship, mainly by providing insights to expand research in this direction. Practical implications also arise from observing the intervention of communication intensity in this relationship, such as alerts to the managers of these organizations about the relevance of aligning communication and risk management to the purpose of collaborative innovation. In any society, the relationship between economy and politics is characterized by controversy, enigmas and mysteries. Economic performance affects political results, and the political environment and its process produce financial results. Brazil was hit by economic and political crises from 2012 to 2016. Such crises made it possible to redesign the correlation network between the assets negotiated on the B3 (Brazil Stock Exchange and Over‐the‐Counter Market). This paper aimed to analyse the main aspects related to the topological structure of the assets negotiated on the B3 and its volatility between the pre‐and post‐2014 electoral periods. Results showed the hierarchical clusters and the evolution of the systemic risk, with the banks leading the concentration in the post‐electoral period. The closeness centrality indices for the minimum variance portfolios were modified by approximately 80% between the pre‐electoral and post‐electoral periods. It was concluded that the political events significantly changed the structure, the risk and the possibility of selecting assets for portfolios in the Brazilian market. [ABSTRACT FROM AUTHOR]

Background: Nowadays, we find ourselves in very unexpected and challenging circumstances facing the COVID-19 pandemic. The impact of the new coronavirus pandemic probably affected everyone's mental health, and people with pre-existing mental disorders may have an aggravated disease condition, leading to a suicide attempt. Pandemic also increased the use of direct-to-consumer telemedicine (TM) exponentially, and consequently, it was expected that cases of attempted suicide could be evaluated remotely. Some TM centers have adapted safety protocols from psychiatric guidelines for managing these patients. However, there is a lack of evidence of the effectiveness of follow-up by TM for patients at high risk for suicide, and there is no consensus on what action should be taken vis-à-vis the patient who requests immediate help remotely. Case presentation: Here, we reported a case of a TM evaluation of a patient's suicidal ideation in a direct-to-consumer telemedicine emergency center, describing the conduct taken in the face of this situation. We also discuss the importance of planning the emergency telemedicine center for situations of risk of suicide. Conclusions: Telemedicine centers should be prepared for direct consumer assessment of suicidal ideation. Current management suggestions include recognizing the risk profile through institutional training and software skills and immediate referral for face-to-face assessment, encouraging continuous monitoring until the admission and active recruitment of family members or closest friends. The objective of this article is to present a proposed application for systematic risk assessment considering the dependence between risks. The proposal relies on a systematic literature review (SLR) as the initial phase, in which the risk classes, management phases and the tools that can be applied to the risk assessment are identified, considering the dependence between them. For this, the system adopted includes the identification and later evaluation of the risks. The evaluation involves the analytic network process (ANP), Monte Carlo Simulation and conditional probability by means of Bayes' theorem. The identification and evaluation of the risks were applied to two links of a piped gas supply chain in Brazil, identified as company X and Y, where six specialists were interviewed in each company in the managerial areas. The ANP indicted that the most critical risk in the links is the demand risk. From this, it was possible through Monte Carlo Simulation to identify the probability of occurrence of events with connection to demand risk: demand (X) / demand risk (Y), with probability of 10%; price risk (X) / demand risk (Y), with probability of 0.64%; and risk of supply (Y) / demand risk (X), with a probability of 0%. This indicates that the highest risk is the risk of demand of firm Y, and therefore mitigation strategies should focus on this risk, as it represents the true cause of supply chain vulnerability, generating risk with the highest probability. Introduction: The frequent occurrence of adverse events during hospital admission demands proactive means of risk management, including checking trackers/triggers. Objective: To verify the factors associated with triggers and adverse events in pediatric hospitalization. Material and Methods: Cross-sectional research based on the Institute for Healthcare Improvement (IHI) methodology, through the application of the Pediatric Trigger Tool (PTT) to a sample (n= 194) from medical records of pediatric patients from a hospital in the Center-West of Brazil. Descriptive, inferential statistical analysis and Poisson regression were performed. Results: More than half (n=107; 55.15%) of patients had at least one trigger upon admission. 204 triggers were identified, with the highest occurrence of a drop in hemoglobin/hematocrit (9.80%), a drop in oxygen saturation (9.80%) and an increase in kidney function markers (9.20%). Of the total triggers, 64 (31.37%) adverse events were confirmed, which were mostly classified as temporary damage requiring patient support (65.62%). The length of stay (p-value=0.004) and the nature of the hospitalization (p-value<0.001) were variables associated with the occurrence of triggers. Character of hospitalization and admissions from other institutions were predictors of the occurrence of triggers and adverse events. Discussion: The study found 31.37% of triggers resulting in harm to the patient, early detection is essential in pediatric patient safety, prolonged hospitalizations are linked to infections and adverse events, patient transfers require rigorous and effective safety measures. In asset management for energy portfolios, quantitative methodologies are typically employed. In Brazil, the NEWAVE computational model is universally used to generate scenarios of hydraulic production and future prices, which result in revenue distributions. These distributions are then used to estimate the portfolio's revenue and assess its risk. Although this is a well-established analysis, it has some shortcomings that are not always considered. The validity of the revenue series constructed by NEWAVE, especially in long-term analysis, is a real problem for agents concerning the acquisition of assets such as power plants. Another issue is the disregard for other objectives that are important for the operationality of the management task and are often ignored, such as operational risk. To address these limitations, this work combines the areas of multicriteria decision making under uncertainty and risk management and presents a methodology for evaluating the acquisition of long-term energy assets, as well as a practical application of the proposed method. Investment alternatives are evaluated in multiple developed scenarios, so it is possible to measure how robust a given option is. By analyzing several scenarios simultaneously, a larger region of uncertainties can be covered, and therefore, decision making becomes more secure. The proposed methodology includes six objectives, designed to address a wider range of stakeholder needs. This approach is applied to an illustrative portfolio, producing results that allow for a more comprehensive understanding of decision attributes. Therefore, this work not only addresses the current limitations in the field but also adds an original contribution by considering simultaneously several scenarios and integrating multiple objectives in a robust and secure decision-making framework. The main objective of this study was to identify the main characteristics of risk management in the automotive supply chain in Brazil, considering companies in the first and second tiers. This objective was achieved through the proposition of three hypotheses that were tested based on a survey of 211 auto parts producers scattered throughout the country, from which 77 completed forms were received. This research enabled us to conclude that the risks perceived in tier 2 are higher than those observed in tier 1. Also, it was possible to verify that the performance of companies in tier 2 (higher risk) is lower than those in tier 1. Contrary to what was observed in the literature, this research could not find evidence to confirm that risk management tools are prioritized to reduce the risk of companies in the supply chain. Therefore, this research contributes to both theory and practice: this is the first study that performs an analysis of risk management in supply chains with tier 1 and 2 companies in the Brazilian automotive industry, becoming an important complement to recent studies on this topic. For practitioners and business executives who manage risks in automotive supply chains in Brazil, this paper shows the reality of the issue in first- and second-tier companies, which can provide them with important contexts for their decision making in the day-to-day business of the automotive sector. Objectives. To develop and test a framework to assess the potential of public health systems to maintain a resilient performance. Methods. Quantitative data from public databases and qualitative data from technical reports of Brazilian health authorities were used to develop the framework which was assessed and modified by experts. Fuzzy logic was used for the mathematical model to determine scores for four resilient abilities -- monitoring, anticipation, learning, and response -- and an aggregated coefficient of resilient potential in health care. The coefficient measures used data from before the coronavirus disease 2019 (COVID-19) pandemic. These were compared with measures of the actual performance of health systems in 10 cities in Brazil during the pandemic. Results. The coefficient of resilient potential in health care showed that the cities most affected by COVID-19 had lower potential for resilient performance before the pandemic. Some local health systems had adequate response capabilities, but other abilities were not well developed, which adversely affected the management of the spread of COVID-19. Conclusions. The coefficient of resilient potential in health care is useful to indicate important areas for resilient performance and the different types of resilience capacities that can be considered in different contexts and levels of public health systems. Regular assessment of the potential of health systems for resilient performance would help highlight opportunities for continuous improvement in health system functions during chronic stress situations, which could strengthen their ability to keep functioning in the face of sudden disturbances. Objective: to evaluate nursing professionals and patient safety culture during the professional performance in the care of suspected or infected patients with COVID-19. Method: a cross-sectional study carried out with 90 professionals from critical care units of two teaching hospitals. An instrument for sociodemographic characterization and health conditions was used, in addition to the constructs "Nursing professional and patient safety" and the Hospital Survey on Patient Safety Culture. Univariate analyzes were performed between the diagnosis of COVID-19 and the characteristics of Nursing professionals, applying Kendell's correlation between the constructs. Results: the COVID-19 diagnosis presented a significant statistical difference between nursing professionals that worked for more than six years at the critical care unit (p=0.020) and the items of the construct "Nursing professional and patient safety" regarding the doubts about how to remove the personal protective equipment (p=0.013) and safety flow (p=0,021). The dimensions 2 (p=0.003), 3 (p=0.009), 4 (p=0.013), 6 (p<0.001), and 9 (p=0.024) of the Hospital Survey on Patient Safety Culture were associated with the accomplishment of training. Conclusion: a higher professional nursing experience time was associated with non-infection by COVID-19. Purpose: The purpose of this paper was to present the stages of the Brazilian startups' organizational life cycle (OLC), identifying and describing the main factors related to the entrepreneurial and organizational perspectives, as well as the factors related to external relationship agents and the associated risks at each OLC stage. In addition, the variables that comprised the three perspectives above were used for a descriptive reading of the evolutionary process from the perspective of intellectual capital during the stages. Design/methodology/approach: The type of research used was quantitative with a descriptive character. For data collection, the authors have used the survey method and two complementary questionnaires were used as research tools. The measurement scale used in these instruments was the 11-point Likert scale. In total, 100 Brazilian startups registered in the STARTUPBASE and residents in São Paulo comprised the sample of this research. For the data processing, it was used the exploratory factor analysis techniques, to identify the analysis factors, and the cluster analysis, to identify the OLC stages. Findings: Based on the results obtained, four stages were identified and described to build the OLC model of this research: Stage I – conception and development; Stage II – organization and traction; Stage III – growth and scale; and Stage IV – consolidation and transition. In addition, the authors described the main lines of evidence found at each stage. Practical implications: This research contributes to academic studies of the OLC in startups and the evolution of risks that originate from the relationship between external agents in the business environment and startups. Thus, a management map is built, which helps entrepreneurs and managers construct these businesses since such a management map allows identifying the risks and challenges that a startup needs to overcome to grow and develop. Originality/value: The originality of this research lies in the adaptation of the OLC approach, which is widely used in studies analyzing the growth and development of mature organizations. The authors used this adaptation to analyze the growth and development of startups in Brazil. In addition, the identification and analysis of external agents that make up the business environment, as well as the analysis of the risks, originated from the relationships between the startup and these agents, brought original and essential results and discussions, both for OLC studies and for risk analysis studies in startups. Dillemma: After assuming as the Procurement Manager of the East University of Brazil (ULB), the public manager Roberto, together with students of Public Management, has the dilemma to improve the processes of his area, trying to get a better governance to public procurement. Educational objective: The case allows to develop competences to the interpretation of the general concepts of bureaucracy, public procurement, public governance and risk management in public sector. Contextualization: The trajectory of Roberto with the students is mediated by professor Franco, in the context of Roberto's role as assistant professor in the Organization, Systems and Methods class and as student of the Public Management Graduation course of ULB. The preconceptions and challenges are presented from an inside view of the public sector. This paper aims to present the BowTie methodology as a practical tool for the Risk Management of the spin maneuver during flight instruction, identifying hazards, controls and consequences related to the loss of control in-flight (LOC-I). Exploratory research was carried out regarding its objectives, through a literature review. Initially, a comparison is made between the Brazilian and international training programs, then the particularities of this training in Brazil was analyzed. Objective: to identify the patient safety strategies adopted by the multidisciplinary team in high-risk pregnancy. Method: descriptive, qualitative study conducted with the multidisciplinary team responsible for specialized care for high-risk pregnancy in a city in southern Brazil. The interviews took place in March 2020 and the results were grouped into thematic categories. Results: two themes were highlighted: safety in embracement and flow of care to high-risk pregnant women, represented by the role of primary and secondary care in assistance to pregnant women, early identification of risk factors and/or aggravations to pregnancy and guidelines for the safety of high-risk pregnant women, identifying the need for individualized multiprofessional care to the needs of pregnant women. Risk and disaster management involves a large amount of data that needs to be collected, processed, and analyzed to become information that results, therefore, in measures to reduce risk and damage. Therefore, this study aims to describe how the macro processes of risk and disaster management are supported using Geographic Information Systems in the Web platform (SIG Web). More specifically, the use of SIG Web VICON as an alternative to improve risk and disaster management actions, especially in the context of disaster risk assessment and communication by the protection and civil defense agencies in Brazil. The contribution of information that the tool provides for the identification, monitoring of threats, response actions, and characterization of vulnerable populations, makes VICON an instrument with high potential to support reducing the impacts caused by disasters. The aim of this study is to identify the main sources of risk and how different members in an international supply chain manage risk, considering contingency effects. A case study was conducted in an international mango supply chain, using multiple-unit embeddedness. Interviews were conducted with the staff of companies that represent four nodes in the mango export supply chain from Brazil to the USA. All interviews were also recorded and transcribed literally in NVivo software, and a coding system was developed based on the literature. The main results indicate that the exchange rate and dependence on a single transport mode are the main sources of risk for the mango supply chain. There are also differences in SCRM concerning the mitigation strategies adopted by the different members. Exporters and importers have better developed mitigation strategies compared to those of logistics operators and suppliers. The main mitigation strategy observed is collaboration. The traditional role of compounding pharmacies is to make drugs prescribed by physicians for patients with needs that cannot be met by commercially available drugs. Medication errors have attracted attention of health authorities since they compromise the patient's assistance, enhance morbidity rates and increase the healthcare costs. This study analyzed medication errors that occurred in a compounding pharmacy school in order to identify types and periodicity and to outline strategies in the service delivery process to mitigate such errors. This is a retrospective descriptive study carried out from March to June of 2018 and based on the analysis of occurrences recorded by the service sector of a magistral pharmacy school in Rio de Janeiro. The errors were classified according to the stage in the pharmaceutical assistance process and reached 124 records, with an average of 1.03 occurrence/day. The main causes were prescription errors (95 occurrences or 76.60%), administering (12 occurrences or 9.68%), labeling (7 occurrences or 5.65%), dispensing (7 occurrences or 5.65%) and handling (3 occurrences or 2.42%). The errors in the prescription stage, the most frequent ones, were potential but intercepted and cleared before they resulted in a harmful outcome. It is argued that potential risk, understood as the possibility of occurrence of an event, is the most adequate definition of risk for Health Surveillance. This integrative review analyzes the definitions of risk and identifies the purposes of using potential risk within the scope of Health Surveillance. Searches were carried out on a legislation database and on four other databases. A theory and a typology of risk supported the categorization. 14 normative acts and 20 articles were selected. The 70 definitions of risk were grouped into categories: probability of an event; probability and scenarios, consequences or severity of these; uncertainty; possibility of loss; event or consequence and uncertainty about the objectives. The limitations of some definitions were pointed out, such as the difficulty of calculating the probability of events. There are divergences between articles and normative acts regarding the terminology adopted to refer to risk. The definitions are composed of diverse terms, which cannot always be differentiated. Only one article investigates the meanings that professionals attribute to risk. Potential risk, an expression that has different definitions in Brazil and Russia, is used mainly to standardize risk assessment. A convergent effort is needed to intensify the use of the Brazilian theoretical definition attributed to potential risk and to minimize the variety of terms used in the definitions of risk. Social projects are an important mean to reduce social problems and they have an increased relevance in contexts of high social inequalities, as it is the case of Brazil. However, the existence of these projects may not be enough, they need to be properly managed, including projects risks. In this context, this paper aims to evaluate the application of risk management in social projects in Brazil. A survey was performed with social project managers and data was analyzed through TOPSIS (Technique for Order of Preference by Similarity to Ideal Solution), to rank the items application levels, according to the ISO 31,000's guidelines. Results provide an overview of Brazilian situation. According to the findings presented, the items related to projects context analysis, and responsibility assignments in the projects were the items better applied, while the items related to the understanding of team members regarding risks in social projects and to the amplitude of risk management in the projects were the items with the worst rank positions. Objective: To present the implementation of instances or nuclei for Patient Safety in Primary Health Care, from the perspective of innovation centered on the implementation of a culture of safety through safe care for the user. Methods: This is an experience report that occurred in a pilot project proposed by the National Council of Health Secretaries in partnership with the Collaborating Center for Health Care Planning in the municipality of Uberlândia - Minas Gerais. Results: the experience has shown that although these instances or nuclei are more common in hospital care, building this narrative for Primary Care is possible and necessary to enable a health system committed to the definition that health care with attributes of quality, focused on the safe user, offered at the right time, effective, effective and equitable. The aim of this study was to analyze the perception of the members of the risk committees of federal universities in Brazil regarding the challenges in the adoption of risk management in those institutions. Currently, federal universities are obliged by law to manage their risks. This is a recent process that presents them with considerable challenges, which have scarcely been explored. Studying the challenges in adopting risk management enables federal universities to gradually improve their overall management, with the aim of adopting the process in the best way possible. This study contributes to the professional and academic areas by proposing a set of actions within the operational context of the universities to improve the maturity level of the risk management of those institutions. The procedure adopted was a survey covering 68 federal universities in operation in 2019. The quantitative study was based on a questionnaire sent to the public servants on their governance, risk, and control committees, which had a 73% response rate. The data were analyzed using descriptive statistics and position and dispersion measures. Perception was analyzed regarding the challenges arising from the adoption of risk management, in which a lack of process mapping, the need for staff engagement and training, the emergence of divergences concerning the treatment of risk, and excess demands on staff were highlighted. The evidence indicated that risk management can guarantee and facilitate compliance with laws, regulations, norms, and standards, as well as the identification of external scenarios that can influence the occurrence of events that negatively impact not only the universities but the whole community. The present project is part of a planning study for structuring resilience in disaster risk reduction in cities. The objective of the article is to evaluate the capacity of municipal managers in terms of preparation for the occurrence of disasters. The article opted for an exploratory study, with the application of 10 interviews with specialists in risk management and disaster protection and civil defense. Ten municipalities were adopted in Paraná, in the south of Brazil, which had critical occurrences recorded between August/2016 and August/2017: hailstorms, storm tides (floods), runoffs and gale. The article provides how different municipalities deal with disaster preparedness, with a clear need for greater social and cultural involvement in their activities. Each municipality has its own characteristics in risk and disaster management. Therefore, researchers are encouraged to apply the methodology in other cities in Paraná. The document includes implications for the development of indicators that allow public managers to monitor risk and disaster management in communities, minimizing the negative impacts suffered by the local population. The sustainability of the Brazilian Supplementary Health System has been frequently debated, since the number of operators has decreased considerably in recent years after bankruptcy records. In this context, risk transfer mechanisms are presented as options for financial loss mitigation and asset protection. Reinsurance is one of them, but, due to legal understandings, its adoption has been restricted to insurance companies since 2009 and forbidden for other types of operators, such as cooperatives, self-management, and philanthropic enterprises. Using industry microdata and applying the collective risk theory, implemented by the classic ruin model (Cramér-Lundberg), this study aimed to counterfactually verify how reinsurance would impact operators solvency in a hypothetical long-term scenario in which all of them had access to these tools. By introducing quota share, with retentions of 60% and 80% of the aggregate expenses, and Stop-Loss treaties, with retention limits of 80% and 95%, it was verified that the ruin probability decreased in the first case by around 40% and 20%. However, the adoption of Stop-Loss for aggregate expenses increased the need for solvency capital, as a result of its higher premiums, becoming viable when directed to higher cost events. Background: The risks of the childbirth assistance process are still very high, both for mothers and babies. According to the WHO, birth-related asphyxia accounts for 23% of all 3.3 million annual neonatal deaths and an even larger number of survivors with disabilities. On the other hand, maternal mortality is still a global challenge, affecting 17 mothers per 100 000 births in the USA. This is associated with the use of outdated technologies and a lack of well-defined processes in monitoring labour and early recognition of maternal clinical deterioration., Method: This study used Lean methodology to map the care flow for pregnant women in a Brazilian maternity hospital (Hospital Israelita Albert Einstein) in order to identify the risks within this process and a set of actions to minimise them. The work team consisted of 29 individuals, including local medical and nursing leaders, as well as healthcare professionals. The What-if tool was used to categorise the levels of risks, and the proportion of severe and catastrophic adverse events was evaluated before and after the implementation of changes., Results: After the implementation of the actions, 100% of the extreme risks (28 risks) and 8% of the high risks (4 risks) were eliminated. This led to a reduction in the interval between severe/catastrophic events from 126 to 284 days, even with an increase in the average monthly number of visits from 367 to 449. Consequently, the weighted value of events decreased from 7.91 to 3.29 per 1000 patients treated, resulting in an annual cost savings of R$693 646.80 (US$139 000.00)., Discussion: The construction of a process based on Lean methodology was essential for mapping the involved risks and implementing a set of actions to minimise them. The participation of the healthcare team and leadership seemed to be important in choosing the measures to be adopted and their applicability. Objective: Verify the knowledge of the surgical team on the performance of the safe surgery checklist in operating room. Method: Quantitative, cross-sectional, exploratory and descriptive research. Data obtained through a structured script interview, applied to hospital operating room professionals in northeastern Rio Grande do Sul. Results: The study included 123 professionals, 43 nursing technicians, 13 nurses, 18 anesthetists and 49 surgeons. 100% of participants reported knowing the checklist, 65% received training, 98% considered it important for patient safety, and 75% claimed to check the checklist information before signing. Regarding the number of steps in the checklist 82% were correct, and regarding the knowledge about the steps, the one that had the most hits was step 1 (82% hits), followed by step 3 which had 77.35% hits. Conclusion: The professionals surveyed have knowledge about the checklist, however there are steps that are not clear, denoting the need for training and professionalization of care to improve the quality of patient care. Shellfish are among the most at‐risk foods of the occidental culture, requiring special control along all the production chain to provide safe products to the population. Codex Alimentarius regulations offer orientations to the control and guarantee of shellfish safety, but the practical application of their indications shall be hampered at the local level by several elements: political, logistic and geographic, as examples. In this article, we provide a comprehensive review of the Brazilian legislative framework on shellfish safety in comparison to those proposed by other trading blocs, mainly the European Community and the USA. Our analysis points out how the simple application of international guidelines on shellfish safety designed for non‐tropical areas, the lack of political interest, and scientific orientation, and the concentration of reference laboratories in the extreme south of Brazil, hamper the development of safe and legal shellfish farming and commerce in the rest of the country. Purpose: The purpose of this paper is to identify the risks to which agrifood supply chains are exposed and to analyze how these risks impact the degree of coordination of the chain. Design/methodology/approach: The present investigation was executed in two steps. Initially, a literature review and interviews with slaughterhouse managers were carried out to identify the main risks to which agrifood supply chains are exposed. The second step consisted of a survey involving 66 Brazilian slaughterhouses to identify how the perception of risks influences the degree of coordination in the examined chains. Findings: The study revealed that risks, transaction costs and creation of collaborative advantages are determining factors in defining the degree of coordination in the analyzed agrifood supply chains. Practical implications: The results allow slaughterhouse managers to more easily recognize the risks to which the supply chains are exposed and evaluate in more detail strategies for relationships with their suppliers. These strategies may be able to avoid conflict and create value for the supplier by stimulating longer relationships and facilitating animal purchase transactions for slaughter. This can lead to quality improvements, lower costs and reduced risk. Originality/value: Studies of risks in agrifood supply chains are rare in comparison with those developed in other sectors. The present investigation is innovative in identifying the main risks specific to agrifood supply chains and associating those risks with a degree of coordination that minimizes them. The internationalization of supply networks brings opportunities and challenges for companies. Among these challenges is the need to manage supply risks to reduce vulnerability and increase resilience. Over the past four years Brazil has been shaken by large corruption and money laundering scandals involving public firms, as revealed by Operation Car Wash, a criminal investigation carried out by the Federal Police, exposing companies' weaknesses in managing internal controls and risk management. In this study we identify the main characteristics of risk management and internal controls disclosed in reference forms of six firms traded on B3, involved in corruption and money laundering crimes and indicted by the Federal Public Ministry. Reference forms published between 2013 and 2016 were analyzed. Among other deficiencies, several of the firms have not yet (or only very recently) adopted a formal risk management policy. This paper describes perceptions of civil servant about risk management implementation process in federal government secretariat of Brazil, with regard to their understanding of this management model, as well as the difficulties and possible gains related to its implementation. This is qualitative field research, cross-cutting, with content analysis that use to consolidate the information obtained through a questionnaire with open questions about risk management, applied to a non-probabilistic convenience sample of a civil servants of the executive branch. Despite the initial stage of the implementation of risk management, the servers perceive more possible gains than difficulties in adopting the model. Possible earnings the are related to the focus on improving processes and achieving results, preventive action to events, increased engagement of those involved, institutional strengthening and compliance control recommendations. The analysis identified four difficulties: risk management policy's limitations, server engagement's lack, structural limitations for risk management and support's lack from top management. An integrative review was performed to identify and analyze national studies on barriers to patient safety incident reporting by health professionals within Brazilian health services. A search in the Virtual Health Library (BVS) Portal, PubMed and Web of Science was performed in January 2017 for papers published in the last ten years. One thousand and seven publications were identified and, following application of inclusion and exclusion criteria, eight papers were analyzed, five of which were qualitative and three quantitative. All research was conducted in hospitals, exclusively with nursing professionals, and 75% was conducted in Southeast Brazil. Most studies showed an under-reporting of incidents, and the main reasons were fear about reporting, reporting focused on more severe incidents, lack of knowledge about the subject or how to report and, registered nurse-centered reporting. While study of this theme is still incipient in Brazil, this review found important weaknesses in the process and barriers to incident reporting by professionals, revealing a need for encouraging their participation, eliminating or reducing such barriers with a view to strengthening patient safety. The main purpose of this research is to analyze the relationship between characteristics of hiring firms and the perception of auditors/accountants that provide services to them regarding complexity in the elaboration/disclosure of sensitivity analyses and risk management. We seek to analyze whether characteristics of firms that have a better level of disclosure could also be associated with the complexity perceived by the professionals that provide services to such firms concerning the information to do with sensitivity analyses and risk management. Information about risk management arouses considerable debate in the area of accounting and finance; nevertheless, how it is addressed, from the point of view of the professionals, still deserves more attention. The results of this study may lead to an improvement, or even a revision, of the standards related with sensitivity analyses and risk management involving financial instruments. The study sample was composed of respondents from South America, namely accountants and auditors from Brazil and Chile. The perception of complexity was evaluated through a structured questionnaire, which was applied using electronic forms. Regarding the explanatory variables, the quantitative model considers the following characteristics of hiring firms: auditor type; firm size; industry; listed firms; transparency; exporting firms. The main advancement provided by this research lies in it revealing that some characteristics of companies usually associated with better levels of disclosure are not necessarily reflected in the perception by accountants/auditors of lower levels of complexity in the elaboration/disclosure of sensitivity analyses and risk management. In conjunction with aims to reduce infant and maternal mortality, antenatal care in Brazil is based on risk assessment and monitoring. Exploring the reliance of pregnancy management policies on a distinction between 'low' and 'high-risk' pregnancies, I conducted two ethnographic studies of pregnant women from a working-class background in Recife and Rio de Janeiro, Brazil. In addition to in-depth interviews with women and professionals, observations were made in public maternity wards and neighbourhood health units. With an approach focused on notions of governance and subjectivity, my data show that risk becomes not only a technique for regulating pregnancies, but also an important dimension of the reproductive experience of women. However, tensions appear between these two dimensions (regulation and experience) of risk. An analysis of my data sheds light on three tensions in particular. Firstly, the notion of risk appears to be a cultural resource for health professionals to transmit norms of parenthood to certain categories of women. Secondly, it allows for distinctions between two registers of care: medicalisation and healthisation. While on one hand the risk approach is aligned with a process of technicisation and pathologisation of pregnancies, it also introduces behavioural and psycho-social perspectives on pregnancy. Thirdly, the risk approach promotes an individualising tendency in the management of pregnancy. Women's experiences lead them to make pragmatic adjustments to regulations. Women circumvent certain norms and easily articulate the registers of medicalisation and healthisation according to the situation, simultaneously incorporating their experience of risk in a social network of mutual help. This article thus shows that the analysis of pregnancy risk management must take into consideration the difference between expectations regarding the effect of regulations implemented by institutions and the de facto experience of women in a specific socio-cultural context. Aim: To develop and validate a predictive model for falls in hospitalized adult clinical and surgical patients, assessing intrinsic (i.e. patient‐related) and extrinsic factors (i.e. care process‐related). Background: To identify factors predictive of falls and enable appropriate management of fall risk it is necessary to understand patient and environmental factors, along with care delivery processes. Design: A matched case–control study. Methods: This study was conducted in the medical and surgical wards of a Brazilian teaching hospital. The sample included 536 patients, with data collected in 2013–2014. Data analysis included descriptive statistics and conditional logistic regression. Cases of patients aged 18 years or older who fell while hospitalized were included. One patient who did not fall during hospitalization, matched by sex, ward and admission date, was selected as a control for each included case. Results: The SAK Fall Scale (Severo‐Almeida‐Kuchenbecker) was developed and validated. The scale includes seven variables: disorientation/confusion, frequent urination, walking limitations, lack of caregiver, postoperative status, previous falls and number of medications administered within 72 hr prior to the fall. This scale showed acceptable predictive accuracy. Conclusions: The newly developed SAK Fall Scale includes five intrinsic and two extrinsic variables and differs from other predictive scales for falls. The findings of this study are broad and the scale, which is easy to apply, can be used worldwide by nurses in health services. In advanced practice, the testing of a new model for fall risk contributes to preventive interventions and thus has an impact on patient safety. [ABSTRACT FROM AUTHOR]
