Purpose: This study examines the value of self-reported health (SRH) as an indicator of underlying health status in a developing country setting. Design and Methods: Logistic regression methods with adjustments for multistage sampling are used to examine the factors associated with SRH in 2,921 men and women aged 50 and older in rural Bangladesh. Results: SRH incorporates multiple dimensions of health status (including physical disability assessed by measured physical performance; self-reported limitations in activities of daily living, or ADLs; self-reported chronic morbidity; and self-reported acute morbidity), severity, comorbidity, and trajectory in a similar fashion for both men and women and for different age groups. Older individuals are more likely to report poor SRH than their younger counterparts, and women report significantly worse SRH than their male peers at each age group. In both cases, this disadvantage can be fully accounted for by differences in measured physical performance, ADL limitations, and chronic and acute morbidity. Implications: Among older Bangladeshis, SRH is an easily recorded, multifaceted, nuanced indicator of underlying health status that is significantly associated with measured physical performance. Moreover, SRH appears to be independent of age- and gender-related norms.

Why do activist groups form alliances and why do some alliances later fall apart? This article asks these questions in the context of a popular mobilisation against resource extraction in Bangladesh. It focuses on the dynamics of a strategic alliance between a locally organised community mobilisation against a British mining company and an urban radical activist group, known for its anti-capitalist activism, to explore the subsequent collapse of the alliance and the demobilisation of one group. Based on the qualitative analysis of in-depth interviews with activists and organisational documents, the article probes the underlying causes of rupture. Although several individual and organisational factors are identified, it is argued that Bangladesh's confrontational political culture and its authoritarian party system played a critical role, with local activists vulnerable to co-optation or being silenced by powerful political actors. The article contributes to social movement scholarship by emphasising that specific political cultures can undermine efforts to build strategic alliances between diverse social movement organisations. Arsenic crosses the placenta, possibly increasing the risk of adverse reproductive outcomes. We aimed to examine the association between maternal arsenic exposure and fetal/neonatal survival using data from a prospective cohort study of 1,616 maternal-infant pairs recruited at a gestational age of ≤16 weeks in Bangladesh (2008–2011). Arsenic concentration in maternal drinking water was measured at enrollment. Extended Cox regression (both time-dependent coefficients and step functions) was used to estimate the time-varying association between maternal arsenic exposure and fetal/neonatal death (all mortality between enrollment and 1 month after birth). In a sensitivity analysis, we assessed gestational arsenic exposure using maternal urine samples taken at enrollment. We observed 203 fetal losses and 20 neonatal deaths. Higher arsenic exposure was associated with a slightly decreased mortality rate up to the middle of the second trimester, and then the mortality rate switched directions around 20 weeks' gestation. In the step function model, the hazard ratios for combined mortality (fetal loss and neonatal death) per unit increase in the natural log of drinking water arsenic concentration (μg/L) ranged from 1.35 (95% CI: 1.08, 1.69) in weeks 25–28 to 0.81 (95% CI: 0.65, 1.02) in weeks 9–12. This nonlinear association suggests that arsenic may exert survival pressure on developing fetuses, potentially contributing to survival bias, and may also indicate that arsenic toxicity differs by fetal developmental stage. Drawing on social movement scholarship, this paper analyses subaltern struggles against a multinational mining company. The Phulbari coal mine is the centre of contention between the mining company and local/national activists. Local concerns about the dispossession of lands and livelihoods and environmental destruction have been merged with a Leftist political agenda on the growing vulnerability of the state and national sovereignty in the Global South. A close examination of the movement's discourses suggests that a broader political struggle against resource plunder and energy imperialism has been strengthened by local community resistance to an environmentally destructive coal mine. Based on in-depth qualitative interviews, I analyse how activists have created new meanings of the conflict to confront and delegitimize hegemonic discourses of capitalist development and modernity. Bangladesh’s energy sector institutionalized neoliberal policies in the early 1990s after a decade long implementation of structural adjustment policies suggested by the World Bank. These policies strengthened the role of foreign private capital and reduced the role of public enterprises. In spite of the country’s success in exploring and developing petroleum resources, the World Bank pushed for policy reforms to reduce the role of state-owned companies. Bangladesh signed several production sharing contracts with multinational energy corporations during 1993–1998. This resulted in the development of a counter-hegemonic social movement, the National Committee. Its activists made the energy sector the most contested national policy domain. Its direct action programs and other mobilization tactics transformed Bangladesh’s public sphere vis-à-vis energy politics. The National Committee caused the government to reverse its decision in some cases, but is far from achieving its main goal: increasing the capability of national institutions to maintain full ownership in natural resource management. Notwithstanding this limitation, the National Committee is a fascinating case of a counter-movement in the Global South contesting neoliberal resource appropriation. [ABSTRACT FROM AUTHOR]

ObjectiveThe goal of the present study was to examine the influence of community environment on the nutritional status (weight-for-age and height-for-age) of children (aged 0–59 months) in Bangladesh. In addition, we tested the association between specific characteristics of community environments and child nutritional status.DesignCross-sectional survey.SettingThe data are from the nationally representative 2004 Bangladesh Demographic and Health Survey.SubjectsRespondents were ever-married women (aged 15–49 years) and their children (n 5731), residing in 361 communities. Child nutritional outcomes are physical measurements of weight-for-age and height-for-age in sd units. We considered the following attributes of community environments potentially related to child nutrition: (i) community water and sanitation infrastructure; (ii) availability of community health and education services; (iii) community employment and social participation; and (iv) education level of the community.ResultsMultilevel regression analysis showed that the spatial distribution of maternal and child covariates did not entirely explain the between-community variation in child nutritional status. The education level of the community emerged as the strongest community-level predictor of child height-for-age (highest v. lowest tertile, β = 0·18 (se 0·07)) and weight-for-age (highest v. lowest tertile, β = 0·21 (se 0·06)). In the height-for-age model, community employment and social participation also emerged as being statistically significant (highest v. lowest tertile, β = 0·13 (se = 0·06)).ConclusionsThe community environment influences child nutrition in Bangladesh, and maternal- and child-level covariates may fail to capture the entire influence of communities. We have gone from lamenting health professional out-migration from the developing world to welcoming it, recognizing that there is substantial potential for promoting health sector development in the developing world via the transfer of financial and social capital from health professional diasporas. In the case of Bangladesh, actualizing this potential requires changes both abroad and at home. Diaspora health professional organizations have to significantly expand their membership base and systematically identify members with appropriate skill sets and match them with Bangladeshi institutional partners. On the Bangladeshi side, what is required is (i) strategies to change prevailing attitudes about brain drain and significantly increase the size of the health professional diaspora and (ii) a clear articulation of gaps and needs, coupled with the active recruitment of diaspora professionals by the provision of positive incentives and the removal of bureaucratic and legal hurdles that constrain effective cooperation and collaboration. This study uses high quality longitudinal data on kin availability, proximity, and marital status from the Matlab surveillance area in rural Bangladesh to explore the impact of kin members on the survival of the elderly over a six year period. The results -- from discrete time hazard models- suggest that the presence of a spouse, sons, and brothers substantially improves survivorship, but with differing effects by the sex of the elderly and the number of sons and brothers. This study offers little support of any of the following as mechanisms by which kin affect the survival of the elderly: changes in the economic status of the elderly as proxied by land holdings; improved access to instrumental support as proxied by the marital status of sons; decreases in social isolation as proxied by proximity of kin.
