The Australian Study of Health and Relationships is a large national population-representative survey of sexual behavior and attitudes conducted every decade. We describe experiences of sexual difficulties lasting at least a month among Australians surveyed in 2012-2013 and identify changes since the previous survey in 2001-2002. Computer-assisted telephone interviews were completed by 20,091 people aged 16-69 years (participation rate 66%) of whom 16,897 people had had sex with a partner in the previous year. We asked how long each difficulty lasted, whether it was a problem, and whether they sought treatment. Half (48%) the men and 68% of women reported at least one difficulty. Lack of interest in having sex was common (28% men, 52% women); 21% of men reported coming to orgasm "too quickly" and women reported inability to reach orgasm (25%) and trouble with vaginal dryness (22%). Women were more likely than men not to find sex pleasurable and to have physical pain during intercourse. Some differences by age group were also apparent. Many difficulties were not seen as problems, especially lacking interest and reaching orgasm too quickly. People with erection/dryness problems, or with pain in intercourse, were more likely to seek treatment, as were people with multiple difficulties. Between 2001-2002 and 2012-2013, there was little change for men, but among women rates of all sexual difficulties fell by 4-10 percentage points. This change accompanied a drop in frequency of sex among people in ongoing relationships and an increase in masturbation and use of pornography. One explanation might be that, over time, fewer women were agreeing to "service sex" when they were not in the mood. Background Few studies have examined smoking and female sexual difficulties. The aim of this study was to investigate the association between current tobacco smoking and sexual difficulties in Australian men and women.Methods: Data for this study came from the Second Australian Study of Health and Relationships (2012-13), which includes a representative sample of 18427 sexually active Australian adults (aged 16-69 years). The main study and outcome measures were tobacco smoking and sexual difficulties. A multiple logistic regression analysis was conducted to adjust for potential confounders.Results: Male heavy smokers (>20 cigarettes per day) were significantly more likely than non-smokers to have trouble keeping an erection [adjusted odds ratio (AOR) 4.14, 95% confidence interval (CI) 1.87-9.20; P<0.0001], lack interest in having sex (AOR 2.18, 95% CI 1.20-3.97; P=0.011), have anxiety about performance (AOR 2.46, 95% CI 1.24-4.86; P=0.010) and be unable to come to orgasm (AOR=2.81, 95% CI 1.23-6.42; P=0.015). Female smokers were also significantly more likely than non-smokers to not find sex pleasurable (AOR 1.48, 95% CI 1.05-2.07; P=0.025); and light female smokers were significantly more likely than non-smokers to be unable to come to orgasm (AOR=1.44, 95% CI 1.05-1.98; P=0.025).Conclusions: Current tobacco smoking was associated with sexual difficulties in both men and women. There are societal concerns that looking at pornography has adverse consequences among those exposed. However, looking at sexually explicit material could have educative and relationship benefits. This article identifies factors associated with looking at pornography ever or within the past 12 months for men and women in Australia, and the extent to which reporting an “addiction” to pornography is associated with reported bad effects. Data from the Second Australian Study of Health and Relationships (ASHR2) were used: computer-assisted telephone interviews (CASIs) completed by a representative sample of 9,963 men and 10,131 women aged 16 to 69 years from all Australian states and territories, with an overall participation rate of 66%. Most men (84%) and half of the women (54%) had ever looked at pornographic material. Three-quarters of these men (76%) and more than one-third of these women (41%) had looked at pornographic material in the past year. Very few respondents reported that they were addicted to pornography (men 4%, women 1%), and of those who said they were addicted about half also reported that using pornography had had a bad effect on them. Current information about numbers of other-sex partners, experiences of different heterosexual behaviours and the recent heterosexual experiences among a representative sample of Australian adults is needed. It is not known whether these practices have changed between 2001-02 and 2012-13. Methods: Computer-assisted telephone interviews were completed by a representative sample of 9963 men and 10 131 women aged 16-69 years from all states and territories. The overall participation rate among eligible people was 66.2%. Results: Men reported more sexual partners than women, although the lifetime number of heterosexual partners reported by women increased significantly between 2001-02 and 2012-13. In 2012-13, 14.7% of men and 8.6% of women reported two or more sexual partners in the last year. Reporting multiple partners was significantly associated with being younger, being bisexual, living in major cities, having a lower income, having a blue-collar occupation and not being married. The proportion of respondents reporting ever having had oral sex or anal intercourse increased significantly since the last survey. At the last heterosexual encounter, 91.9% of men and 66.2% of women had an orgasm, oral sex was reported in only approximately one in four encounters and anal intercourse was uncommon. Conclusion: There were increases between 2001-02 and 2012-13 in partner numbers among women and in the lifetime experience of oral and anal sex. In a national telephone-based survey, information on sexual behaviour and outcomes were collected from 20091 randomly selected Australians in 2012-13. Data were weighted and analysed to determine the proportion of those who had received school-based sex education and to examine the associations between sex education and sexual health outcomes, specifically a history of a sexually transmissible infection (STI), early pregnancy, contraception use at first sex, and level of STI knowledge. Just over half the respondents (53%; n=19836) reported receiving sex education that included information about condoms and contraception. Using logistic regression analysis after adjusting for age, education and non-English-speaking background, we found that sex education was strongly associated with increased odds of using contraception at first vaginal intercourse (odds ratio (OR)=1.57; 95% confidence interval (CI) 1.44-1.71; P<0.001) and higher levels of STI knowledge (OR=1.75; 95% CI 1.46-2.12; P<0.001). We analyzed data from the Australian Study of Health and Relationships, a national telephone survey (2001-2002) of sexual behavior and attitudes among a representative sample of 19,307 Australians aged 16 to 59. Respondents were asked whether at their last sexual encounter they gave or received manual stimulation and oral sex, had vaginal intercourse or anal intercourse, and whether they had an orgasm. Most encounters (95%) included vaginal intercourse. Of the 64 possible combinations of practices, 13 accounted for 93% of encounters; vaginal intercourse alone (12%), intercourse and manual stimulation of the man's and/or woman's genitals (49%), and intercourse, manual, and oral (32%). Men had an orgasm in 95% of encounters and women in 69%. Women were more likely to reach orgasm in encounters including more practices, especially cunnilingus. The results from a telephone survey in 2001–02 of a probability sample of Australian households including 10,173 men aged 16–59 (response rate 69.4%) are used to assess the prevalence of circumcision across social groups in Australia and examine lifetime history of sexually transmissible infection (STI), sexual difficulties in the last year, sexual practices including masturbation, and sexual attitudes. More than half (59%) of the men were circumcised. Circumcision was less common among younger men (32% aged <20) and more common among the Australian born (69%). After correction for age, circumcision was unrelated to reporting STI, but appeared to protect against penile candidiasis. Circumcision was unrelated to most sexual difficulties, but circumcised men were less likely to report physical pain during intercourse or trouble keeping an erection; reasons for this are unknown. There were no significant differences in practices at last sexual encounter with a female partner or in masturbation alone. Circumcised men had somewhat more liberal sexual attitudes. Neonatal circumcision was routine in Australia until the 1970s. It appears not to be associated with significant protective or harmful sexual health outcomes. Purpose: To compare the estimates of HIV-related risk derived from a sample drawn through random digit dialing and that component of the sample drawn from households listed in a telephone directory. Methods: The prevalence of key outcomes, and their 95% confidence intervals, was estimated for the total sample and for that component of the sample drawn from households listed in a telephone directory. Results: On all outcome measures the sample derived from listed households was more conservative. With few exceptions, the estimates derived from the overall sample and from listed households were not significantly different. The aim of this study was to assess whether the lives of Australian people living with HIV/AIDS (PLWHA) have improved to the extent hoped for following the introduction of new antiretroviral (ARV) drugs for HIV. In 1997, 925 Australian PLWHA completed the first national survey of the social impacts of HIV/AIDS. In 1999, 924 Australian PLWHA were recruited for a repeat of the survey. Study participants completed an anonymous self-administered questionnaire. The data revealed that although new ARV drugs have improved the clinical profiles of many PLWHA, there have not been corresponding improvements in the physical well-being, levels of employment, or financial circumstances of many PLWHA. Nevertheless, PLWHA express favourable attitudes toward ARV drugs. The objective of this study was to describe the medical, attitudinal and cultural correlates of antiretroviral uptake amongst people living with HIV/AIDS (PLWHA) in Australia. Stratified purposive sampling produced a sample of 925 PLWHA, which represents 8.3% of the current population of PLWHA in Australia. Respondents completed a self-administered questionnaire which revealed that 78% of respondents were using antiretroviral drugs for HIV/AIDS. Logistic regression revealed that PLWHA were more likely to use antiretroviral drugs if they had more favourable attitudes toward antiretroviral drugs, if they had been diagnosed with an AIDS-defining illness, and if they had ever had a CD4/T-cell count below 400 copies/ml blood. Women were less likely than men to use antiretroviral drugs, and logistic regression revealed different predictors of antiretroviral drug use amongst men and women. Given the importance of attitudes toward antiretroviral drugs, it is likely that if the current confidence in antiretroviral drugs were to change, this would be reflected in an equally rapid cessation of treatment amongst many PLWHA. To understand safe sex behavior in two countries which have been differentially affected by the HIV/AIDS epidemic, the present study compared the AIDS-related knowledge, attitudes, and behaviors of 920 heterosexual undergraduate students in Australia and 228 heterosexual undergraduate students in South Africa. South African students were found to have significantly less knowledge about HIV/AIDS, and significantly less favorable attitudes toward safe sex behavior than their Australian counterparts. They were also more likely to report that they have avoided various groups of people for fear of contracting AIDS. Experience from Australia over the period 1986-1995 suggests that significant improvements in the AIDS-related knowledge, attitudes, and behaviors of South African undergraduates are achievable. Objective: To document the use of contraception by a representative sample of Australian women aged 16-49 years and compare it with 2001-2002.Methods: Women were asked about their use of contraception and method used or reason for non-use during computer-assisted telephone interviews in 2012-2013. Women were sampled by random digit dialling of landline and mobile phones (participation rate 67.2%).Results: Of a weighted sample of 5654 heterosexually active women interviewed 81% were using a method of contraception including sterilisation; this amounts to 66% of all women aged 16-49. Of those who were not using a method, 42% were pregnant or wanted a baby, 25% said they or their partners were infertile, 5% were currently not having intercourse, 3% were past menopause and 25% were apparently at risk of unintended pregnancy. Of those who used a method, 33% used oral contraceptives, 30% condoms and 19% sterilisation as their primary method. Use of condoms, intrauterine devices, implants and emergency contraception has increased since 2002, and use of sterilisation has fallen. Method used varied by age group, location, occupational group, relationship status and parity. A third of women had ever used emergency contraception, with the highest rate among women in their 20s.Conclusion: Australian women have access to a wide range of effective contraceptive methods.Implications: Given the high levels of use, most unintended pregnancies in Australia are likely to be attributable to method failure or inconsistent use. Objective: To enumerate pregnancy outcomes for a representative sample of women in Australia surveyed in 2012-2013 (primary aim) and compare these with women surveyed in 2001-2002 (secondary aim)., Methods: Computer-assisted telephone interviews with over 10,000 women aged 16-69 years (participation rate 68.4%). Results are weighted for chance of selection and to reflect the population as a whole., Results: Of women with experience of vaginal intercourse, 75.1% had ever been pregnant, 18.4% reported difficulties getting pregnant and 10.0% had had fertility treatment. Of those who had been pregnant, 91.3% had ever had a live birth, 34.3% a miscarriage, 22.8% an abortion and 2.3% a stillbirth; 0.9% had relinquished a child for adoption. The proportion first pregnant in their 30s was 11% among women aged 60-69 and 26% among those aged 40-49. Fewer older women reported difficulties getting pregnant. Of the 21,882 pregnancies reported, 70% led to live births and 10% were terminated. Compared with our 2001-2002 survey, fewer women reported ever having been pregnant. Giving up newborns for adoption has become very rare., Conclusions: Falling fertility since the 1960s reflects greater access to contraception and abortion and higher opportunity costs of childbearing. Intoxication can be a factor in unwanted sex, but research on the extent of the issue in both women and men is limited. We assessed the prevalence, correlates, and 10-year time-trends of unwanted sex due to intoxication among a representative sample of 4,279 women and 3,875 men aged 16-69 years in Australia and considered how these vary by gender. In 2012-13, 16% of women and 10% of men reported ever having had a sexual experience when they "did not want to because they were too drunk or high at the time." For both women and men, this was associated with younger age, bisexual activity, and reports of lifetime injection drug use, sexually transmitted infections, and forced sex. Among women only, it was associated with drinking above guideline levels and ever having terminated a pregnancy. Among men only, it was associated with current tobacco smoking, elevated psychosocial distress, and poor general health. Compared with 2001-02 data, fewer men reported unwanted intoxicated sex, while there were no changes for women as a whole. Interpreting these findings through an intersectional assemblage framework supports stronger understanding of the multiple factors influencing sexuality and substance use with implications for promoting equity, safety, and sexual health.

Introduction: People with sexual interests in bondage and discipline, "sadomasochism" or dominance and submission (BDSM) have been seen by many professionals as damaged or dangerous., Aim: To examine sexual behavior correlates of involvement in BDSM and test the hypothesis that BDSM is practiced by people with a history of sexual coercion, sexual difficulties, and/or psychological problems., Methods: In Australia in 2001-2002, a representative sample of 19,307 respondents aged 16-59 years was interviewed by telephone. Weighted data analysis used univariate logistic regression., Main Outcome Measures: Self-reported demographic and psychosocial factors; sexual behavior and identity; sexual difficulties., Results: In total, 1.8% of sexually active people (2.2% of men, 1.3% of women) said they had been involved in BDSM in the previous year. This was more common among gay/lesbian and bisexual people. People who had engaged in BDSM were more likely to have experienced oral sex and/or anal sex, to have had more than one partner in the past year, to have had sex with someone other than their regular partner, and to have: taken part in phone sex, visited an Internet sex site, viewed an X-rated (pornographic) film or video, used a sex toy, had group sex, or taken part in manual stimulation of the anus, fisting or rimming. However, they were no more likely to have been coerced into sexual activity, and were not significantly more likely to be unhappy or anxious-indeed, men who had engaged in BDSM scored significantly lower on a scale of psychological distress than other men. Engagement in BDSM was not significantly related to any sexual difficulties., Conclusion: Our findings support the idea that BDSM is simply a sexual interest or subculture attractive to a minority, and for most participants not a pathological symptom of past abuse or difficulty with "normal" sex.

It is well established that, compared to other women, women who have been forced or frightened into unwanted sexual activity have poorer psychological, physical, and sexual health. However, it is not clear whether particular experiences of sexual coercion, such as younger age when coerced or number of times coerced, are more likely to lead to poorer health status. This study of a representative sample of Australian women aged 16-59 years was designed to examine such associations. Of the 9134 women recruited for the study, 885 (21%) had been sexually coerced, which was defined as being forced or frightened into unwanted sexual activity. This study showed that any experience of sexual coercion was associated with poorer psychological, physical, and sexual health. The data revealed no consistent associations between health status and particular characteristics of sexual coercion, such as the age when women were coerced, the number of times they had been coerced, or the time since being coerced. Two of the few significant findings were that women who had been coerced more than once reported significantly greater psychosocial distress, and that women first coerced when aged 13-16 reported poorer physical well-being than women first coerced at younger or older ages. There was no significant association between whether women had consulted a psychologist or other professional counselor and better health status. The absence of consistent associations between health status and particular characteristics of sexual coercion indicates a need for care and support services for all women who have been sexually coerced.

Many studies have examined the influence on sexual attitudes and behavior of religious belief (i.e., religious denomination) or religiosity (e.g., attendance at services, subjective importance of religion). However, few studies have examined the combined effects of religion and religiosity on sexual attitudes and behavior. This study examined such effects in a representative sample of 19,307 Australians aged 16-59 years (response rate, 73.1%). The study compared members of four religious groups (Protestant, Catholic, Buddhist, Muslim) and two levels of frequency of attendance at religious service (less than monthly, at least monthly). Religious participants were compared to their non-religious peers in analyses adjusted for potential confounding by demographic variables. The outcomes were five sexual behaviors and five corresponding measures of sexual attitudes. The study revealed inconsistent patterns of association between religion/religiosity and a range of sexual behaviors and attitudes. In general, greater attendance at religious services was associated with more conservative patterns of behavior and attitudes. However, religious people who attended services infrequently were more similar to their non-religious peers than their more religious peers. The results of this study highlight the importance of considering not only religion or religiosity, but the intersection between these two variables.

To facilitate the development of interventions to reduce health risk behavior among young people, we designed this study to compare risk behavior among young people and older people, to compare risk behavior profiles between young men and women, and to identify sociodemographic correlates of risk behavior among young people. Computer-assisted telephone interviews with a representative sample of 19,307 Australian men and women (response rate 73.1%) assessed alcohol consumption, cigarette smoking, injection drug use, and unprotected intercourse. Respondents aged 16 to 24 reported less healthy behavior than older people. Although men and women aged 16 to 24 had similar profiles of health risk behavior, correlates of these behaviors differed for men and women. There were few consistent sociodemographic correlates of different risk behaviors. The results suggest that young women are now as important a priority as young men for interventions. Young people remain an important target group for health promotion, with nonheterosexual young people a particular high-risk group.

Objective: To describe numbers of opposite-sex partners, experiences of different heterosexual behaviours, and recent heterosexual experiences among a representative sample of Australian adults., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years from all States and Territories. The response rate was 73.1% (69.4% among men and 77.6% among women)., Results: Men reported more sexual partners than women over their lifetime, in the past five years and in the past year. 15.1% of men and 8.5% of women reported multiple sexual partners in the past year. Reporting multiple opposite-sex partners was significantly associated with being younger, identifying as bisexual, living in major cities, having a lower income, having a blue-collar occupation, and not being married. All but a handful of respondents' most recent heterosexual encounters involved vaginal intercourse and condoms were used in one-fifth of these sexual encounters. Anal intercourse was very uncommon during respondents' most recent heterosexual encounters., Conclusion: Patterns of heterosexual experience in Australia are similar to those found in studies of representative samples in other countries., Implications: There may be a need for interventions targeted at people with multiple sexual partners to promote safer sexual behaviour and to reduce the likelihood of transmission of HIV and other sexually transmitted infections.

Objectives: To document the use of contraception by a representative sample of Australian women aged 16-59 years., Method: Between mid-2001 and mid-2002, computer-assisted telephone interviews were completed by 9,134 women aged 16-59 years from all States and Territories selected by modified random-digit dialling of households (response rate 77.6%). Respondents were asked about contraceptive methods used in their current regular relationship(s) and during recent sexual encounters, or in general, or their reasons for non-use. Responses were allocated to 14 categories., Results: 70.8% of respondents were using a method of contraception, over 95% of those apparently at risk of pregnancy. Most common reasons for non-use were not having intercourse (41.6%) and being past menopause (21.6%). Among those apparently at risk of unplanned pregnancy (i.e. who were heterosexually active and fertile but not pregnant or trying to become pregnant; 13.0% of non-users), the most common reasons for non-use were experience of side-effects or contra-indications (23.0%), leaving it to chance (20.2%), forgetting/not caring (18.9%), breast feeding (16.5%) and believing it unnatural/unhealthy (13.7%). No women cited religious objections or lack of access to services. The most used methods were oral contraceptives (33.6% of users), tubal ligation/hysterectomy (22.5%), condom (21.4%) and vasectomy of partner (19.3%). Tubal ligation rates were higher and condom use lower in regional and remote areas., Conclusion: Given the high levels of use and knowledge and lack of evidence of unmet service need, most unplanned pregnancies in Australian adults are likely to be attributable to method failure or inconsistent use.

Objective: To document the reproductive experiences of a representative sample of Australian women aged 16-59 years., Method: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years from all States and Territories. The overall response rate was 73.1% (69.4% among men, and 77.6% among women). Women were asked the number of times they had experienced a live birth, a still birth, a miscarriage and a termination of pregnancy., Results: Of the women surveyed, 15.5% reported having experienced difficulty in becoming pregnant and 76.1% had been pregnant at least once. Nearly all the women who had been pregnant reported experiencing a live birth. Substantial minorities of women reported having experienced a miscarriage (33.4%) or a termination of pregnancy (22.6%). The percentage of women who reported becoming pregnant the first time as a teenager declined from 22.8% among women aged 50-59 to 16.9% among women aged 20-29. Of those who had had vaginal intercourse, 19.2% had used emergency contraception, 53.3% of them only once., Conclusion: There was clear evidence of substantial changes in the fertility of Australian women over the past 40 years.

Objective: Some blood-borne and sexually transmissible agents may be spread by using non-sterile injecting equipment and by unprotected sexual intercourse. The objective of this study was to describe the prevalence of these risk factors in the general population of Australia., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years. The overall response rate was 73.1% (69.4% men, 77.6% women)., Results: 3% of respondents had ever injected non-prescription drugs; men were more likely than women to have done so. Predictors of injecting in men and women included being aged 20-29, homosexual or bisexual identity, and lower levels of education and income. Injecting was not more common in cities than elsewhere. Among people who had injected in the past year, 12.4% had shared needles and 43.0% had shared other paraphernalia. Sharing was significantly related to lower income. Unprotected sex with casual partners was more common in heterosexual activity than in male homosexual activity. Among heterosexually active respondents, 3.3% reported unprotected vaginal sex with casual partners (59% of those with such partners) and among homosexually active males 2.1% reported unprotected anal sex with casual partners (12% of those with such partners). Predictors of unprotected sex included indices of lower socio-economic status., Conclusion: Sexual and injecting risks are reported by substantial minorities of the Australian population and are associated with indices of lower socio-economic status and bisexual identity. Programs are needed to address vulnerabilities to these infections.

Objective: To describe selected characteristics of Australian adults' regular or ongoing sexual relationships., Method: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years. The overall response rate was 73.1% (69.4% men, 77.6% women). Respondents indicated how often they had sex in the past four weeks, contraceptive use, their own and their partners' expectations about having sex with other people during their relationship, whether they had discussed these expectations with their partners and whether they had an explicit agreement about sex with other people., Results: 85.3% of men and 89.5% of women were in a regular heterosexual relationship, among whom 81.4% of men and 89.3% of women reported contraceptive use. Men and women who had a regular partner for the past 12 months had had sex with their partners an average of 1.84 times per week in the four weeks before interview; younger people had sex more often. Most respondents expected themselves and their partners to not have sex with other people, although men were less likely than women to have discussed these expectations with their partner. Bisexually identified men and women were significantly less likely than heterosexually identified men and women to support having sex only with their regular partner. Only 4.9% of men and 2.9% of women in regular heterosexual relationships had concurrent sexual partners in the past 12 months., Conclusion: Australians' attitudes to not having sex with people while in a regular relationship are highly consistent with their behaviour.

Objective: To describe knowledge related to sexually transmitted infection (STI) and blood-borne virus (BBV) infection in a representative sample of Australian adults., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years from all States and Territories. The overall response rate was 73.1% (69.4% among men and 77.6% among women). Respondents were read a series of statements about STIs and BBVs and asked to say if they believed they were true or false. Predictors of knowledge were examined., Results: Respondents were more likely to answer correctly questions about hepatitis C than questions about herpes, gonorrhoea, genital warts and chlamydia. Women had better knowledge than men. Other predictors of better knowledge included speaking English at home, homosexual or bisexual identity, higher educational levels, higher income, higher occupational level and a previous STI diagnosis., Conclusion: Knowledge of transmission routes and health consequences of the most common STIs was poor. Although knowledge is only one prerequisite for effective disease prevention, the findings suggest that improving public knowledge of STIs is an urgent task in reducing the morbidity associated with these common infections., Implications: Education campaigns to increase knowledge of the transmission and health consequences of STIs are urgently required if the substantial morbidity associated with these common infections is to be reduced.

Objective: To describe the lifetime and recent history of STIs and BBV, including place of seeking treatment, in a representative sample of Australian adults., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years from all States and Territories. The overall response rate was 73.1% (69.4% among men and 77.6% among women)., Results: Overall, 20.2% of men and 16.9% of women had ever been diagnosed with an STI or BBV, and 2.0% and 2.2% respectively had been diagnosed in the past year. The participant's usual general practitioner was the most common location of treatment. Sexual health clinics accounted for a small proportion of treatment locations. Predictors of recent STI or BBV diagnosis in men included homosexual or bisexual identity, a history of sex work as a worker or client, a history of injecting drugs and having more than one partner in the past year. In women, predictors included bisexual identity, history of sex work as a worker, injecting drug use, and having more than one partner in the past year. Around 40% of men and women had been tested for HIV and in homosexually identified men, 77% had been tested., Conclusion: STIs and BBVs are common infections in Australia and care is mostly received from general practitioners. Although a variety of predictors, including homosexual or bisexual identity, injecting drug use and sex work were related to STI diagnosis, STIs were not uncommon among people without these risk factors., Implications: General practitioners in Australia require a high level of expertise to recognise, offer testing, and manage common STIs and BBVs.

Objective: To provide reliable estimates of the prevalence, correlates and consequences of sexual coercion among a representative sample of Australian adults., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years. The response rate was 73.1% (69.4% men, 77.6% women)., Results: Overall, 4.8% of men and 21.1% of women had experienced sexual coercion, i.e. being forced or frightened into unwanted sexual activity, and 2.8% of men and 10.3% of women had been coerced when aged 16 or younger. Although women were significantly more likely than men to have been sexually coerced, correlates of sexual coercion were similar for men and women and were not limited to effects on sexual behaviour. People who had been coerced reported greater psychosocial distress, were more likely to smoke, were more anxious about sex, and more likely to have acquired a sexually transmitted infection. Few people had talked to others about their experiences of sexual coercion and fewer had talked to a professional., Conclusion: Sexual coercion is an unacceptably common experience. Sexual coercion has detrimental effects on various aspects of people's lives. It most commonly occurs at the ages at which people become sexually active and women are more likely than men to be sexually coerced., Implications: There may be need for more readily accessible services that are better able to minimise the detrimental effects of sexual coercion. This is in addition to a more general need to reduce the incidence of sexual coercion.

Objective: To review the content, method and process of the Australian Study of Health and Relationships (ASHR)., Results: ASHR achieved a large sample, a high response rate and documented the sexual life histories and current sexual health-related knowledge, attitudes and practices of the Australian population aged 16-59. Its cross-sectional nature limited our ability to partition observed variation between age and time despite clear evidence of both age-related and cohort-related changes in sexual practice. Similarly, its reliance on a sample of individuals reporting on their sexual experiences rather than a sample of sexual relationships or encounters and their participants limits our ability to understand the dynamics of those relationships and encounters. Finally, our understanding of sexuality in Australia could have been improved through qualitative studies with a subsample of ASHR participants., Conclusion: ASHR represents a significant contribution to our understanding of sexual health-related knowledge, attitudes and practices in Australia.

Objectives: To describe the prevalence of a range of autoerotic and other non-coital sexual practices among Australians., Method: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years (response rate 73.1%). Respondents were asked whether in the past year they had: masturbated, engaged in various other autoerotic activities, or engaged in any of six other non-coital or esoteric practices., Results: Half of the respondents (65% men, 35% women) had masturbated in the past year. Nearly half (48%) of the men and 25% of the women had masturbated in the past four weeks, among whom men had done so a mean of 5.8 times and women 3.3 times. About a quarter of all respondents had watched an X-rated film (37% men, 16% women), 12% of men and 14% of women had used a sex toy, and 17% of men and 2% of women had visited an Internet sex site. 17% of men and 14% of women had engaged in digital-anal stimulation with a partner. Phone sex, role play or dressing up, bondage and discipline, sadomasochism or dominance and submission (BDSM-DS), fisting (rectal or vaginal, insertive or receptive) and rimming (oral-anal stimulation) were all engaged in by less than 5% of the sample., Conclusion: Most of the practices studied were engaged in by more men than women. A range of autoerotic activities are both substitutes for partnered sex and additional sources of pleasure for people with sexual partners.

Objective: To describe the prevalence of same-sex and opposite-sex attraction and experience in Australia and the prevalence of different sexual identities., Method: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years from all States and Territories of Australia. The overall response rate was 73.1% (men, 69.4%; women, 77.6%). Men and women were asked about their experience of same-sex and opposite-sex attraction and experience along with their sexual identity. The agreement and disagreement between sexual attraction and sexual experience were explored., Results: Among men, 97.4% identified as heterosexual, 1.6% as gay or homosexual and 0.9% as bisexual. Among women, 97.7% identified as heterosexual, 0.8% as lesbian or homosexual and 1.4% as bisexual. Among men, 91.4% reported only opposite-sex attraction and experience, as did 84.9% of women. Thus, some same-sex attraction or experience was reported by 8.60% of men and 15.1% of women. Of men, 4.2% reported sexual attraction and sexual experience that was inconsistent, as did 8.2% of women. Factors associated with this agreement or disagreement included age group, non-English-speaking background, education and socio-economic status., Conclusion: Relatively few Australians reported a sexual identity other than heterosexual. However, both same-sex attraction and homosexual experience are more common than homosexual or bisexual identity would suggest. Reporting same-sex attraction or experience was associated with poorer mental health and is likely to reflect responses to homophobia in Australian society.

Objective: Condom use is a central part of strategies to prevent the transmission of human immunodeficiency virus and other sexually transmissible infections. The objective of this study was to provide reliable estimates of the prevalence and correlates of condom failure among a representative sample of Australian men., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men aged 16-59 years from all States and Territories. The response rate was 69.4%., Results: Among men who used condoms in the year prior to being interviewed, 23.8% experienced at least one condom breakage in the past year and 18.1% experienced at least one condom slippage in the past year. Experience of condom breakage in the past year was significantly related to younger age, having a blue-collar occupation, and using more condoms. Neither condom slippage nor condom failure was significantly related to use of water-based lubricants or oil-based lubricants., Conclusion: Condom failure is related to certain characteristics of individuals and is not randomly distributed across all condom users. Lubricant use did not affect condom failure., Implications: There may not be a need to promote lubricant use for vaginal intercourse, as there was no association between lubricant use and condom failure.

Objectives: The Australian Study of Health and Relationships reports on sexual behaviours in a representative sample of Australian adults., Method: Between May 2000 and June 2001, computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16 to 59 years (response rate 73.1%). Respondents were asked whether in the past year they had for a period of at least one month experienced a range of sexual difficulties., Results: The most common sexual difficulty was lack of interest in having sex (24.9% men, 54.8% women). Women were more likely than men to report being unable to come to orgasm (28.6% vs. 6.3%), not finding sex pleasurable (27.3% vs. 5.6%), physical pain during intercourse (20.3% vs. 2.4%) or worrying during sex about their body looking unattractive (35.9% vs. 14.2%). Men were more likely to report coming to orgasm too quickly (23.8% vs. 11.7%). Men (16.0%) and women (17.0%) were equally likely to have felt anxious about their ability to perform sexually. Erectile difficulties and lack of interest in sex were higher among the over-40s. Anxiety about performance was highest among men under 20. Experience of sexual difficulties correlated with self-reported ill health., Conclusion: The assumption that everyone should be interested in sex, should want to have it and should enjoy having it is a socially normative prescription and not a description of the actual state of affairs. Women reported less interest in sex, less pleasure and more pain than men.

Objective: To describe the prevalence and features of homosexual experience and recent homosexual encounters among a representative sample of Australian adults., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years from all States and Territories. The overall response rate was 73.1% (69.4% among men and 77.6% among women)., Results: Overall, 8.6% of women and 5.9% of men reported some homosexual sexual experience in their lives (p<0.001); these figures fell to 5.7% and 5.0% respectively (p=0.106) when non-genital sexual experience was excluded. 1.9% of men and 1.5% of women reported homosexual experience in the past year. Men who reported homosexual experience reported more same-sex partners than did women (means 31.6 and 3.2, p<0.001), and men and women who identified as homosexual or bisexual reported more sexual partners in total than those who identified as heterosexual. Respondents reporting homosexual experience were significantly more likely to be from an English-speaking background, have higher levels of education, live in a major city, and report a white-collar or managerial/professional occupation. Women who reported homosexual experience were less likely to have a middle or high income. In the most recent homosexual encounter, genital touching was the most commonly reported practice, and oral sex was reported much more frequently than in heterosexual encounters., Conclusion: Homosexually and bisexually identified men and women had higher total numbers of partners than heterosexuals. Improved societal attitudes towards homosexuality are likely to lead to further increases in estimates of the prevalence of homosexual experience in the community.

Objective: To describe the characteristics of Australian adults' first vaginal intercourse and contraceptive use on that occasion, as well as first oral sex., Method: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years. The overall response rate was 73.1% (69.4% men, 77.6% women). Respondents indicated their age at first vaginal intercourse (if ever), the partner's age, their relationship to their partner, the duration of the relationship before first intercourse, and what contraception (if any) was used. Respondents also indicated their age when they first had oral sex., Results: For men, the median age at first intercourse declined from 18 among men aged 50-59 to 16 for men aged 16-19. For women the decline in median age was from 19 to 16. For women born between 1941 and 1950 the difference in median age at first vaginal intercourse and subsequent first oral sex was six years but for women born between 1981 and 1986 the difference in medians was one year. For men, a similar convergence was observed. Contraceptive use at first intercourse has increased significantly, from less than 30% of men and women in the 1950s to over 90% in the 2000s. For men, first intercourse before age 16 was significantly associated with greater numbers of lifetime and recent sexual partners, ever having paid for sex and having had a sexually transmitted infection (STI), and for women, with greater numbers of lifetime and recent sexual partners, ever having been paid for sex and ever having had an STI., Conclusion: Sex education should be given so that all young people have information about contraception and disease prevention before they begin their sexual careers.

Objective: To describe social attitudes towards sex of Australian adults and correlates of a scale of sexual liberalism., Methods: Computer-assisted telephone interviews were completed by a representative sample of 10,173 men and 9,134 women aged 16-59 years. The overall response rate was 73.1% (69.4% men, 77.6% women). Respondents were asked about their agreement with nine attitude statements. Factor analysis and examination of internal consistency resulted in a six-item scale of liberalism. Correlates of attitude statements and the liberalism scale were examined., Results: Most people agreed that premarital sex was acceptable, that oral sex was considered 'sex', that sex was important for a sense of well-being and that extramarital sex was unacceptable. Men were more likely (36.9%) to disapprove of sex between two men than women were to disapprove of sex between two women (25.1%). Higher levels of education were associated with increased liberalism for men and women, as was speaking English at home, identifying as homosexual or bisexual, vaginal intercourse before age 16, having had more than one sexual partner in the year before interview, having had heterosexual anal intercourse, having no religion or faith, smoking tobacco, and drinking more alcohol., Conclusion: Sexual attitudes of Australians largely support a heterosexual paradigm with no sex outside the relationship. High levels of approval of premarital sex are consistent with decreasing age of first intercourse in Australia. Higher levels of liberalism were associated with greater sexual adventurism and health risk taking.

Objectives: To document sexual and emotional satisfaction with their relationships and desired and actual frequency of sex among a representative sample of Australian adults., Method: Between mid-2001 and mid-2002, computer-assisted telephone interviews were completed by 10,173 men and 9,134 women aged 16-59 years from all States and Territories selected by modified random-digit dialling of households (response rate 73.1%). Respondents in a regular relationship were asked how physically pleasurable they found sex with the partner to be and how emotionally satisfying the relationship was. All respondents were asked how often they would ideally like to have sex and how often they had sex in the past four weeks., Results: Most people in heterosexual relationships found sex very or extremely pleasurable (90.3% men, 79.1% women) and the relationship emotionally satisfying (87.5% men, 79.2% women); men were more satisfied with both. Physical pleasure in sex was correlated with emotional satisfaction. One person in four had had no sex in the past four weeks; most people had had sex less than twice a week. Most people wanted ideally to have sex more often than they did. However, 24.3% of men but only 8.3% of women said they ideally wanted sex daily or more often., Conclusion: Men on average express higher levels of relationship satisfaction and of sexual interest, but the overlap between men and women is large.

The Australian Study of Health and Relationships (ASHR), a survey of 19,307 people aged 16-59 years which had a broad focus across many aspects of sexual and reproductive health. Our primary goal in presenting these data is to describe the overall prevalence of key outcomes and the ways in which they are patterned with respect to a range of sociodemographic factors. The socio-demographic factors used include age, gender, language spoken at home, highest level of education completed, region of residence, household income and occupational category. Where appropriate we also include stated sexual identity. Our choice of these factors reflects the fact that they are major elements in the structure of the Australian population and Australian society. For the purpose of reporting the initial results, we have grouped these outcomes into 18 papers. They are bracketed by an introductory methods paper which provides a detailed account of the design and execution of the study, and a concluding paper reflecting on the key themes emerging from the findings, the strengths and limitations of our approach and recommendations for future research.
