Background: Current clinical care for common bacterial STIs (Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG) and Mycoplasma genitalium (MG)) involves empiric antimicrobial therapy when clients are symptomatic, or if asymptomatic, waiting for laboratory testing and recall if indicated. Near-to-patient testing (NPT) can improve pathogen-specific prescribing and reduce unnecessary or inappropriate antibiotic use in treating sexually transmitted infections (STI) by providing same-day delivery of results and treatment., Methods: We compared the economic cost of NPT to current clinic practice for managing clients with suspected proctitis, non-gonococcal urethritis (NGU), or as an STI contact, from a health provider's perspective. With a microsimulation of 1000 clients, we calculated the cost per client tested and per STI- and pathogen- detected for each testing strategy. Sensitivity analyses were conducted to assess the robustness of the main outcomes. Costs are reported as Australian dollars (2023)., Results: In the standard care arm, cost per client tested for proctitis, NGU in men who have sex with men (MSM) and heterosexual men were the highest at $247.96 (95% Prediction Interval (PI): 246.77-249.15), $204.23 (95% PI: 202.70-205.75) and $195.01 (95% PI: 193.81-196.21) respectively. Comparatively, in the NPT arm, it costs $162.36 (95% PI: 161.43-163.28), $158.39 (95% PI: 157.62-159.15) and $149.17 (95% PI: 148.62-149.73), respectively. Using NPT resulted in cost savings of 34.52%, 22.45% and 23.51%, respectively. Among all the testing strategies, substantial difference in cost per client tested between the standard care arm and the NPT arm was observed for contacts of CT or NG, varying from 27.37% to 35.28%., Conclusion: We found that NPT is cost-saving compared with standard clinical care for individuals with STI symptoms and sexual contacts of CT, NG, and MG., Competing Interests: Declaration of Competing Interest CSB and CKF were supported by an Australian National Health and Medical Research Council Leadership Investigator Grant (GNT1173361 and GNT1172900, respectively). Background: With sexually transmitted diseases (STDs) no longer on the decline, it is prudent to assume that all cases of non specific urethritis and cervicitis are caused by sexually transmitted infection. Specific infections of the urethra and cervix due to chlamydia and other non gonococcal organisms are common and because they are often not apparent clinically, are underdiagnosed., Objective: The aim of this paper is to provide a realistic approach to the management of urethritis and cervicitis by encouraging the performance of investigations and the instigation of treatment at the first consultation., Discussion: A knowledge of the patient's sexual history combined with a working knowledge of the new DNA based technologies should provide for the early assessment of and intervention with patients who are at risk of at least one STD infection.

Background: The leukocyte esterase (LE) test has a limited role in determination of empiric therapy for male patients screened for urethritis because of its poor positive predictive value in low (< 5%) prevalence settings. The recent advent of nucleic acid amplification testing of first-void urine (FVU) has dramatically increased the ease with which widespread screening for Chlamydia trachomatis and Neisseria gonorrhoeae can be performed, but the costs of such testing may be prohibitive. The LE test may therefore have a role in management of urethritis because of its high negative predictive value., Objectives: To determine the sensitivity, specificity, and positive and negative predictive value of LE testing for the diagnosis of N. gonorrhoeae and C. trachomatis in male FVU specimens in a low-prevalence urban setting using a commercial polymerase chain reaction (PCR) as the gold standard., Methods: Data were obtained on men presenting to an urban sexually transmitted disease clinic over a 16-month period. Patients were included if an FVU had been tested for the presence of LE using a rapid dipstick, read by an automated urine analyzer, and the sample (either an FVU or urethral swab) had then been processed for the detection of N. gonorrhoeae and C. trachomatis by PCR., Results: Of 301 assessable patients, there were 14 cases of gonorrhoea, 21 cases of chlamydia, and 1 case of dual infection detected by PCR. Most men (245/301; 81.4%) were asymptomatic, of whom 12 of 245 (4.9%) had an infection detected compared with 24 of 56 (42.9%) in the symptomatic men (P < 0.001). Using a "< or = trace" cutoff, the overall value for the sensitivity of the LE test was 77.8% (95% confidence interval, 60.4-89.3), specificity 80.8% (75.4-85.2), positive predictive value 35.4% (25.2-47.1), and negative predictive value 96.4% (92.8-98.3)., Conclusions: The negative predictive value of the LE test may be of use in determining which patients should proceed to specific diagnosis by nucleic amplification methods (e.g., PCR or ligase chain reaction). By limiting testing to patients with positive LE results, cost savings may be made, enabling the technology to be used in a wider community setting. The value of the LE test in higher prevalence populations with access to nucleic amplification testing remains to be established.
