Background: With the increasing availability of information, patients are becoming more informed about radiology procedures and requesting imaging studies. This qualitative study aims to explore factors that influence general practitioners' (GPs) decisions to fulfil patient requests for imaging studies during clinical consultation. Methods: Semi‐structured interviews were conducted with 10 GPs working across five private medical centres in Northwest Sydney. Conventional content analysis was used with emergent themes to identify GPs perspectives. Results: Six themes stood out from the interviews with GPs fulfilling patient requests for imaging studies. They included four pertaining to patient factors: patient expectations, 'therapeutic scans', 'impressive labels' and entitled. Two further themes pertained to the GP perspective and included defensive medicine, and 'new patients'. Requests are fulfilled from anxious or health‐obsessed patients, with GPs worrying about litigation if they refuse. However, GPs decline requests from patients with entitlement attitudes or during first visits. Discussion: The findings suggest that GPs struggle to balance their responsibilities as gatekeepers of imaging with patients' expectations of request fulfilment. Clear guidelines on the appropriate use of diagnostic imaging and its limitations could help patients understand its proper use and ease anxiety. Additionally, education and training for GPs could help them manage patient expectations and provide appropriate care. Patient Contributions: Patients, service users, caregivers, people with lived experiences or members of the public were not directly involved in the design, conduct, analysis or interpretation of the study. However, our study was conducted in primary care facilities where the GPs were interviewed about patients' requests for diagnostic imaging based on their own initiatives. GPs' perspectives in managing patient expectations and healthcare utilisation were explored within the Australian Medicare system, where medical imaging and image‐guided procedures come at little to no cost to the individual. The study findings contribute to a better understanding of the challenges faced by GPs in dealing with patient consumerism and requests for diagnostic imaging, as well as factors influencing request fulfilment or denial. Background: Traditionally, general practitioners (GPs) have initiated the need for, and ordered, radiological tests. With the emergence of consumer-centred care, patients have started to request scans from doctors on their own initiative. Consumeristic health care has shifted the patient–doctor dyadic relationship, with GPs trending towards accommodating patients' requests. Methods: A mixed method analysis was conducted using a survey instrument with open ended questions and concurrent interviews to explore participants' responses from their requests for radiological studies from GPs. Themes emerging from both qualitative and quantitative methodologies were mapped onto the Andersen Newman Model (ANM). Results: Data were analysed for 'predisposing,' 'need' and 'enabling' elements of the ANM model and were correspondingly mapped to patient's requests for radiological referrals according to the elements of the ANM. Participants expressed anxiety about their health, were confident in the types of radiological scans they desired and typically indicated the need for evidence of good health. Their desire for such requested scans was often enabled through prior exposure to health information and the experience of specific symptoms. Requests came with the expectation of validation, and if these requests were denied, participants indicated that they would seek another doctor who would oblige. Conclusions: In our modest study of Australian patients, participants were well informed about their health. Exposure to information seems to create a sense of anxiousness prior to visiting the doctor. Individuals sought visual proof of wellness through imaging, and doctors in return often accommodated patient requests for radiological studies to appease patients' needs and to maintain workflow. With increased availability of online health information, individuals are more educated about managing their health care. Our study investigates patients requesting their own radiological scans rather than relying on their doctors' expertise. We explore participant responses to clinical encounters where they have initiated a radiological study from their general practitioner, and their self-reported responses and outcomes from such requests. Introduction. Histopathological assessment of liver biopsies is the current "gold standard" for diagnosing graft dysfunction after liver transplantation (LT), as graft dysfunction can have nonspecific clinical presentations and inconsistent patterns of liver biochemical dysfunction. Most commonly, post-LT, graft dysfunction within the first year, is due to acute T-cell mediated rejection (TCMR) which is characterised histologically by the degree of portal inflammation (PI), bile duct damage (BDD), and venous endothelial inflammation (VEI). This study aimed to establish the relationship between global assessment, which is the global grading of rejection using a "gestalt" approach, and the rejection activity index (RAI) of each component of TCMR as described in revised Banff 2016 guidelines. Methods. Liver biopsies (n = 90) taken from patients who underwent LT in 2015 and 2016 at the Australian National Liver Transplant Unit were identified from the electronic medical records. All biopsy slides were microscopically graded by at least two assessors independently using the revised 2016 Banff criteria. Data were analysed using IBM SPSS v21. A Fisher–Freeman–Halton test was performed to assess the correlation between the global assessment and the RAI scores for each TCMR biopsy. Results. Within the cohort, 60 (37%, n = 164) patients underwent at least 1 biopsy within 12 months after LT. The most common biopsy outcome (total n = 90) was acute TCMR (64, 71.1%). Global assessment of TCMR slides strongly positively correlated with PI (p value <0.001), BDD (p value <0.001), VEI (p value <0.001), and total RAI (p value <0.001). Liver biochemistry of patients with TCMR significantly improved within 4 to 6 weeks post-biopsy compared to the day of the biopsy. Conclusion. In acute TCMR, global assessment and total RAI are strongly correlated and can be used interchangeably to describe the severity of TCMR. [ABSTRACT FROM AUTHOR]

Introduction: The aim of this work is to report diagnostic reference levels (DRLs) for hybrid positron emission tomography and x-ray computed tomography (PET/CT) exams in Australia (AU) for Queensland (QLD) and Western Australia (WA) (AU QLD/WA) and New Zealand (NZ).Methods: Two-structured booklets were designed to collect dose information, patient demographics, equipment details and acquisition protocols for fluoride-18 fluorodeoxyglucose (18 F-FDG) PET/CT procedures, and any additional diagnostic CT routinely performed as part of 18 F-FDG whole-body examination. The DRL was reported based on the 75th percentile and achievable dose for 18 F-FDG, CT dose index volume (CTDIvol ) and dose length product (DLP). The effective dose and total effective dose was reported for 18 F-FDG whole-body PET/CT examination. Also, the effective dose was reported separately for identified additional diagnostic CT.Results: The findings of this study show that the current DRL for 18 F-FDG in AU QLD/WA and NZ was 333.75 MBq and 332.87 MBq, respectively. The reported AU QLD/WA CTDIvol and DLP associated with 18 F-FDG whole-body PET/CT examinations from vertex to thigh (VT) was 4.41 mGy and 474 mGy.cm. In NZ, the reported VT CTDIvol and DLP was 13.07 mGy and 1319.05 mGy.cm. The effective dose for 18 F-FDG and CT component was 5.6 mSv and 4.7 mSv for AU QLD/WA. For NZ, the effective dose was 5.7 mSv and 10.9 mSv for 18 F-FDG and CT component. The total effective dose delivered from the 18 F-FDG whole-body scan from the AU QLD/WA PET/CT centres (10.44 mSv) were lower than the radiation doses delivered from the NZ (16.65 mSv).Conclusions: The current DRLs were proposed for AU QLD/WA and NZ for 18 F-FDG whole-body PET/CT examinations. Variations existed in the current practice of AU QLD/WA and NZ PET/CT examinations. There is a need to optimize the radiation doses delivered from PET/CT examinations. [ABSTRACT FROM AUTHOR]

Low-lying coral reef islands are considered highly vulnerable to climate change, necessitating an improved understanding of when and why they form, and how the timing of formation varies within and among regions. Several testable models have been proposed that explain inter-regional variability as a function of sea-level history and, more recently, a reef platform size model has been proposed from the Maldives (central Indian Ocean) to explain intra-regional (intra-atoll) variability. Here we present chronostratigraphic data from Pipon Island, northern Great Barrier Reef (GBR), enabling us to test the applicability of existing regional island evolution models, and the platform size control hypothesis in a Pacific context. We show that reef platform infilling occurred rapidly (~4-5 mm yr) under a 'bucket-fill' type scenario. Unusually, this infilling was dominated by terrigenous sedimentation, with platform filling and subsequent reef flat formation complete by ~5000 calibrated years BP (cal BP). Reef flat exposure as sea levels slowly fell post highstand facilitated a shift towards intertidal and subaerial-dominated sedimentation. Our data suggest, however, a lag of ~1500 yr before island initiation (at ~3200 cal BP), i.e. later than that reported from smaller and more evolutionarily mature reef platforms in the region. Our data thus support: (1) the hypothesis that platform size acts to influence the timing of platform filling and subsequent island development at intra-regional scales; and (2) the hypothesis that the low wooded islands of the northern GBR conform to a model of island formation above an elevated reef flat under falling sea levels. A meeting of the Australasian Gastro-Intestinal Trials Group (AGITG) was held to develop a consensus statement defining when a patient with pancreatic cancer has disease that is clearly operable, is borderline, or is locally advanced/inoperable. Key issues included the need for multidisciplinary team consensus for all patients considered for surgical resection. Staging investigations, to be completed within 4 weeks of presentation, should include pancreatic protocol computed tomography, endoscopic ultrasound, and, when possible, biopsy. Given marked differences in outcomes, the operability of tumours should be clearly identified by categories: those clearly resectable by standard means (group 1a), those requiring vascular resection but which are clearly operable (group 1b), and those of borderline operability requiring vascular resection (groups 2a and 2b). Patients who may require vascular reconstruction should be referred, before exploration, to a specialist unit. All patients should have a structured pathology report with standardised reporting of all seven surgical margins, which identifies an R0 (no tumour cells within a defined distance of the margin) if all surgical margins are clear from 1 mm. Neo-adjuvant therapy is increasingly recommended for borderline operable disease, while chemotherapy is recommended as initial therapy for patients with unresectable loco-regional pancreatic cancer. The value of adding radiation after initial chemotherapy remains uncertain. A small number of patients may be downstaged by chemoradiation, and trimodality therapy should only be considered as part of a clinical trial. Instituting these recommendations nationally will be an integral part of the process of improving quality of care and reducing geographic variation between centres in outcomes for patients. Purpose – Recovery approaches to healthcare are now an important feature of the mental health policies and plans of many western countries. However, there are continuing challenges to the operationalisation of these approaches. The purpose of this paper is to consider how to overcome these challenges, using insights gained from health managers and practitioners who have been involved in the process of implementation. Design/methodology/approach – The analysis is undertaken through a descriptive single-case embedded study of the implementation of Recovery into a public mental health service in Australia. The unit of analysis that features in this paper is the perceptions of the implementation of Recovery-oriented services, of health managers and practitioners. Findings – The analysis suggests that although health service managers followed many of the recommendations that can be found in the research literature to support achievement of Recovery-oriented services, there was a need to go further. For example, practitioners in the case study context were educated about the principles of Recovery and provided with new processes of clinical documentation to support their work, however these practitioners felt they were ill-equipped to address complex issues of practice, including the management of clinical risk and professional accountability issues. This raises questions about the content of the education and training provided, and also about the ongoing support provided to practitioners who work within a Recovery-oriented framework. Originality/value – The descriptive single-case embedded study of the implementation of Recovery is the first of its kind in Australia. A model that resolves reef island formation in relation to both reef platform substrate development and mid-Holocene sea-level change is presented for Bewick Island, northern Great Barrier Reef, based on morphostratigraphic analysis and radiometrically dated island sediments and reef corals. On Bewick Island, microatolls record reef-flat development at higher sea level (+1.5 m) by 6500 yr B.P. Island building began on a partially emergent reef flat 5000-4000 yr B.P., when sea level was 0.5 m above present. As sea level fell to its present level, the reef platform process window closed and the island core stabilized. Results present the first unequivocal evidence of island building directly over a reef flat comprising microatolls, and the first detailed model of island formation from the Great Barrier Reef. The model demonstrates that the interplay of sea level and reef surface elevation can vary between sites but their convergence is critical for island initiation. Future trajectories of island change will vary, dependent on the unique relative sea level and substrate depth conditions that govern island formation. [ABSTRACT FROM AUTHOR]

Clinicopathological data regarding pancreatic solid pseudopapillary tumors (SPT) in a multiethnic country are limited. The aim of the present study was to characterize pancreatic SPT in Australia. Clinicopathological features, treatment, immunohistochemical findings and outcome data of 34 patients (79% Caucasian, 12% Asian, 6% South Pacific Islander and 3% African) with pancreatic SPT were reviewed. The most presenting complaint was abdominal pain. Median diameter of tumors was 60 mm (range: 20-220); predominantly located in the pancreatic tail (tail : body : head = 23:3:8). All tumors were resected and patients underwent surgery, including a liver resection for metastasis, all patients were alive after a median follow up of 70 months (IQR: 48-178). Two patients underwent repeated surgery for local recurrences with liver metastases after 8 and 18 months, which were successfully managed by surgical resection. Completeness of excision, perineural spread, vascular space invasion, mitotic rate and cellular atypia did not predict recurrence. In all cases, there was aberrant nuclear staining of beta-catenin and a loss of membranous expression of E-cadherin with aberrant nuclear localization of the cytoplasmic domain. Most pancreatic SPT were also strongly positive for CD10 (96%), progesterone receptor (79%), cytokeratin (28%), synapthophysin (26%) and chromogranin (15%). Pancreatic SPT occur in all races and are uniformly indolent. Given complete resection of a pancreatic SPT is usually curative and recurrences can be treated with re-operation, correct diagnosis is important. [ABSTRACT FROM AUTHOR]

Complex relationships exist between tropical reef ecology, carbonate (CaCO3) production and carbonate sinks. This paper investigated census-based techniques for determining the distribution and carbonate production of reef organisms on an emergent platform in central Torres Strait, Australia, and compared the contemporary budget with geological findings to infer shifts in reef productivity over the late Holocene. Results indicate that contemporary carbonate production varies by several orders of magnitude between and within the different reef-flat sub-environments depending on cover type and extent. Average estimated reef-flat production was 1.66 ± 1.78 kg m-2 year-1 (mean ± SD) although only 23% of the area was covered by carbonate producers. Collectively, these organisms produce 17,399 ± 18,618 t CaCO3 year-1, with production dominated by coral (73%) and subordinate contributions by encrusting coralline algae (18%) articulated coralline algae, molluscs, foraminifera and Halimeda (<4%). Comparisons between the production of these organisms across the different reef-flat zones, surface sediment composition and accumulation rates calculated from cores indicate that it is necessary to understand the spatial distribution, density and production of each major organism when considering the types and amounts of carbonate available for storage in the various reef carbonate sinks. These findings raise questions as to the reliability of using modal production rates in global models independent of ecosystem investigation, in particular, indicating that current models may overestimate reef productivity in emergent settings. A field experiment was conducted at Warraber Island, Torres Strait, Australia to investigate spatial and temporal variations in wave characteristics and energy across a mesotidal coral reef platform. Measurements of water depth were obtained using five pressure sensors deployed across a 2.7-km section of reef flat from July 3–5, 2001. The reef surface was uneven and consisted of an outer reef flat, a central reef flat depression, an inner reef ramp, a palaeo-reef surface and the shoreline. Water levels decreased landward across the platform with tide ranges at the shoreline being almost 50% lower than at the outer reef flat. Rising and falling tides were characterised by a bimodal energy distribution with both short-period (0–3 s) and wind (3–8 s) waves present. Higher water levels were dominated by wind waves. The highest waves occurred at high tides associated with nocturnal tidal cycles with Hs decreasing from 0.5 to 0.2 m from the outer reef flat to the shoreline. Wave energy at swell (8–20 s) and infragravity (>20 s) frequencies was negligible across the reef platform although there was evidence of wave groups at higher water levels.Reef geometry and changes in water level determine the magnitude of wave energy on the reef platform. Up to 85–95% of incident wave energy is attenuated by the central reef flat depression at high and low tide, respectively, and strong linear relationships exist between Hs and h at all locations. Both wave height and wave type are strongly depth dependent. Critical reef rim depths required to produce Hs of a given size vary spatially across the reef rim due to variations in reef topography. A distinct depth related threshold exists at which short-period and wind wave dominance reverses. Over a 14-day spring-neap tidal cycle, the time of occurrence of wave action diminishes across the reef platform to the shoreline. Larger waves (Hs=0.2 m) occur for only 9% of time at the outer reef flat and for less than 0.5% over the remaining reef platform. This implies that on mesotidal reef platforms, sediment entrainment and transport are severely constrained under normal wave energy conditions and significant change is likely restricted to extreme events. Over the past 30 years there has been a wealth of research examining the geomorphology of Australian estuaries. This paper reviews the major regional controls on estuarine geomorphic development and discusses the focus of research efforts to understand estuarine evolution, configuration and processes controlling geomorphic development and change. The presence and position of estuaries along the Australian coastline is controlled by large-scale climate-led changes in sea-level, the antecedent structure of the coast and tectonic activity. The configuration of Australia’s estuaries is controlled by a number of environmental factors identified by Jennings & Bird (1967) including climate, oceanographic regime, sediment availability, structure and mineralogy, and tectonics. Interaction of these factors produce a range of estuarine configurations around the Australian continent from wave-dominated, microtidal, bar-built estuaries in the south to low energy macrotidal estuaries in the north which have produced extensive low-lying coastal plains. The principle focus of geomorphologists during the past 30 years has been understanding the medium to long-term development of estuaries in response to fluctuating sea-level over the past 125 000 years. Classifications and models of estuarine development and associated biological community response to estuary development have been identified based on lateral and vertical sedimentary sequences. Results have provided essential information on the physical resources and characteristics of estuarine systems including the distribution of sediment facies that host a variety of different ecological communities. There has been a paucity of research examining contemporary processes controlling geomorphic change in Australia’s estuaries. Tropical Cyclone (TC) Yasi (Category 5) was a large (~700 km across) cyclone that crossed Australia's Queensland coast on the 3rd of February 2011. TC Yasi was one of the region's most powerful recorded cyclones, with winds gusting to 290 km/h and wave heights exceeding 7 m. Here we describe the impacts of TC Yasi on a number of nearshore, turbid-zone coral reefs, that include several in the immediate vicinity of the cyclone's landfall path (King Reef, Lugger Shoal and Dunk Island), as well as a more distally located reef (Paluma Shoals) ~150 km to the south in Halifax Bay. These reefs were the focus of recent (between 2006 and 2009) pre-Yasi studies into their geomorphology, sedimentology and community structure, and here we discuss data from a recent (August 2011) post-Yasi re-assessment. This provided a unique opportunity to identify and describe the impacts of an intense tropical cyclone on nearshore reefs, which are often assumed to be vulnerable to physical disturbance and reworking due to their poorly lithified framework. Observed impacts of TC Yasi were site specific and spatially highly heterogeneous, but appear to have been strongly influenced by the contemporary evolutionary stage and ecological make-up of the individual reefs, with site setting (i.e. exposure to prevailing wave action) apparently more important than proximity to the landfall path. The most significant ecological impacts occurred at King Reef (probably a result of freshwater bleaching) and at Paluma Shoals, where widespread physical destruction of branched Acropora occurred. New coral recruits are, however, common at all sites and colony re-growth clearly evident at King Reef. Only localised geomorphic change was evident, mainly in the form of coral fracturing, rubble deposition, and sediment movement, but again these impacts were highly site specific. The dominant impact at Paluma Shoals was localised storm ridge/shingle sheet deposition, at Lugger Shoal major offshore fine sediment flushing, and at Dunk Island major onshore coarse sand deposition. There was little geomorphic change evident at King Reef. Thus whilst small-scale and taxa specific impacts from Cyclone Yasi are clearly evident, geomorphological changes appear minor and ecological impacts highly variable between sites, and there is no observed evidence for major reef structural change. The study suggests that the vulnerability of reefs to major physical disturbance events can be extremely site specific and determined by interacting factors of location relative to storm path and pre-event geomorphology and ecology. Optimal nutrition care is important in the management of cystic fibrosis (CF). This paper summarises the '2017 Nutrition Guidelines for Cystic Fibrosis in Australia and New Zealand (NZ)'. CF dietitians formulated 68 practice questions which were used to guide a systematic literature search and review of the evidence for nutrition in CF. Identified papers underwent quality and evidence assessment using the American Dietetic Association quality criteria checklist and the National Health and Medical Research Council of Australia (NHMRC) rankings. Evidence statements, graded recommendations and practice points were developed covering core nutrition topics (assessment and nutrition interventions including oral, enteral and micronutrient supplementation); nutrition-related co-morbidities (including pancreatic insufficiency, CF-related diabetes, bone health and distal intestinal obstruction syndrome); and key new topic areas (genetic modulator therapies, overweight/obesity and complementary therapies). Abstract Background Best practise care optimises survival and quality of life in patients with pancreatic cancer (PC), but there is evidence of variability in management and suboptimal care for some patients. Monitoring practise is necessary to underpin improvement initiatives. We aimed to develop a core set of quality indicators that measure quality of care across the disease trajectory. Methods A modified, three-round Delphi survey was performed among experts with wide experience in PC care across three states in Australia. A total of 107 potential quality indicators were identified from the literature and divided into five areas: diagnosis and staging, surgery, other treatment, patient management and outcomes. A further six indicators were added by the panel, increasing potential quality indicators to 113. Rated on a scale of 1–9, indicators with high median importance and feasibility (score 7–9) and low disagreement (<1) were considered in the candidate set. Results From 113 potential quality indicators, 34 indicators met the inclusion criteria and 27 (7 diagnosis and staging, 5 surgical, 4 other treatment, 5 patient management, 6 outcome) were included in the final set. Conclusions The developed indicator set can be applied as a tool for internal quality improvement, comparative quality reporting, public reporting and research in PC care. [ABSTRACT FROM AUTHOR]

Abstract Aims Ductal adenocarcinoma is a rare variant of prostate cancer, and as such clinical outcomes and best management are not well defined. This series demonstrates the atypical presentation and unusual clinical behaviour of ductal adenocarcinoma and proposes management guidelines to assist clinicians. Materials and methods A retrospective review of pure (nine patients) and mixed (18 patients) ductal adenocarcinoma of the prostate referred to the Departments of Radiation Oncology of the Sydney Cancer Centre, Royal Prince Alfred Hospital and Northern Sydney Cancer Centre, Royal North Shore Hospital, between 2000 and 2015. Results Twenty-seven patients were treated with definitive radiotherapy, nine patients (33%) with pure ductal and 18 (67%) with mixed ductal-acinar adenocarcinoma. The median follow-up was 38 months. Four patients (15%) failed locally, all of whom received less than 80 Gy, or no brachytherapy boost. Five patients (19%) failed distantly, four with biopsy-proven lung metastases. All distant failures occurred with a prostate-specific antigen (PSA) < 3 ng/ml. Conclusion This series shows the atypical clinical presentation of this entity, as well as its propensity to metastasise to unusual sites. Relapse may occur at low absolute PSA values and is often asymptomatic. Ductal cancer should not simply be regarded as a high Gleason grade cancer. We propose management guidelines, including regular computed tomography examinations (rather than relying solely on PSA levels) as part of the follow-up for patients with any component of ductal adenocarcinoma. Background: Dietary intervention in cystic fibrosis (CF) has historically focused on high-energy diets to address malnutrition, with little attention on diet quality. With increased survival, CF complications such as impaired glucose tolerance (IGT) and cystic fibrosis related diabetes (CFRD) have increased in prevalence. In the absence of consensus on the management of IGT, the role of dietary intake, specifically carbohydrate quality, requires consideration., Aims: The aims of this study were to: 1) determine nutritional quality of dietary intake at an adult CF clinic and compare this to the Australian Dietary Guidelines 2) explore relationships between dietary intake, including glycaemic index (GI) and glycaemic load (GL), and glucose response variables using continuous glucose monitoring (CGM)., Methods: Adults attending a Sydney hospital were recruited to undergo CGM for five-seven days and record dietary intake using a food record over the CGM period. The relationship between variables of dietary intake, including GI and GL and variables of glycaemic response, including mean amplitude of glycaemic excursions (MAGE), percentage of time in hyperglycaemic and euglycaemic range, were determined., Results: Eighteen participants completed the study with 87 full days of dietary and CGM data. Dietary intake was higher than recommendations in the Australian Dietary Guidelines in relation to grains and protein foods and only slightly higher in saturated fat. Bivariate correlations showed dietary GI was significantly positively associated with percentage of time in hyperglycaemic range. Dietary GL was significantly associated with SD, MAGE and percentage of time in euglycaemic range on CGM. Results remained significant when controlled for energy intake in partial correlation analyses., Conclusions: This study suggests GI and GL may be important dietary factors influencing glucose metabolism in CF.
