Objective: To investigate symptomatic and preventive medication use according to age and frailty in Australian and Japanese nursing homes (NHs). Methods: Secondary cross-sectional analyses of two prospective cohort studies involving 12 Australian NHs and four Japanese NHs. Frailty was measured using the FRAIL-NH scale (non-frail 0–2; frail 3–6; most-frail 7–14). Regular medications were classified as symptomatic or preventive based on published lists and expert consensus. Descriptive statistics were used to compare the prevalence and ratio of symptomatic to preventive medications. Results: Overall, 550 Australian residents (87.7 ± 7.3 years; 73.3% females) and 333 Japanese residents (86.5 ± 7.0 years; 73.3% females) were included. Australian residents used a higher mean number of medications than Japanese residents (9.8 ± 4.0 vs 7.7 ± 3.7, p < 0.0001). Australian residents used more preventive than symptomatic medications (5.5 ± 2.5 vs 4.3 ± 2.6, p < 0.0001), while Japanese residents used more symptomatic than preventive medications (4.7 ± 2.6 vs 3.0 ± 2.2, p < 0.0001). In Australia, symptomatic medications were more prevalent with increasing frailty (non-frail 3.4 ± 2.6; frail 4.0 ± 2.6; most-frail 4.8 ± 2.6, p < 0.0001) but less prevalent with age (< 80 years 5.0 ± 2.9; 80–89 years 4.4 ± 2.6; ≥ 90 years 3.9 ± 2.5, p = 0.0042); while preventive medications remained similar across age and frailty groups. In Japan, there was no significant difference in the mean number of symptomatic and preventive medications irrespective of age and frailty. Conclusions: The ratio of symptomatic to preventive medications was higher with increasing frailty but lower with age in Australia; whereas in Japan, the ratio remained consistent across age and frailty groups. The novel domestic cat hepadnavirus (DCH), a member of the Hepadnaviridae, was first detected in Australia and has recently been identified in more countries. In this study, we explored the DCH genome using next-generation sequencing of a plasma sample from a cat with a fever of unknown cause. Nucleotide sequence analysis showed the virus to be relatively genetically distant from the first reported DCH in Australia, showing 89% homology. Then we conducted an epidemiological survey by PCR of plasma samples collected from 203 cats that visited a veterinary hospital for diagnosis and treatment. Two of the 203 surveyed cats a were positive for DCH. One of the two positive cases had elevated liver enzymes of unknown etiology, and the other had hepatocellular adenoma. Our study indicated that DCH infection was observed in domestic cats in the Tokyo area of Japan as well as other reported areas in the world. Purpose: A sudden unexpected death has significant negative impacts on patients, family caregivers, and medical staff in hospice/palliative care. This study aimed to clarify the incidence and associated factors of sudden unexpected death according to four definitions in advanced cancer patients. Methods: We performed a prospective cohort study in 23 inpatient hospices/palliative care units in Japan. Advanced cancer patients aged ≥18 years who were admitted to inpatient hospices/palliative care units were included. The incidence and associated factors of sudden unexpected death were evaluated in all enrolled patients according to four definitions: (a) rapid decline death, defined as a sudden death preceded by functional decline over 1–2 days; (b) surprise death, defined if the primary responsible palliative care physician answered "yes" to the question, "Were you surprised by the timing of the death?"; (c) unexpected death, defined as a death that occurred earlier than the physicians had anticipated; and (d) performance status (PS)‐defined sudden death, defined as a death that occurred within 1 week of functional status assessment with an Australia‐modified Karnofsky PS ≥50. Results: Among 1896 patients, the incidence of rapid decline death was the highest (30‐day cumulative incidence: 16.8%, 95% CI: 14.8–19.0%), followed by surprise death (9.6%, 8.1–11.4%), unexpected death (9.0%, 7.5–10.8%), and PS‐defined sudden death (6.4%, 5.2–8.0%). Male sex, liver metastasis, dyspnea, malignant skin lesion, and fluid retention were significantly associated with the occurrence of sudden unexpected death. Conclusion: Sudden unexpected death is not uncommon even in inpatient hospices/palliative care units, with range of 6.4–16.8% according to the different definitions. [ABSTRACT FROM AUTHOR]

Globally, many parts of fire emissions are driven by deforestation. However, few studies have attempted to evaluate deforestation and vegetation degradation fires (DDF) and predict how they will change in the future. In this study, we expanded a fire model used in the Community Land Model to reflect the diverse causes of DDF. This enabled us to differentiate DDFs by cause (climate change, wood harvesting, and cropland, pastureland, and urban land‐use changes) and seasonality. We then predicted the state of fire regimes in the 2050s and 2090s under RCP 2.6 and RCP 6.0 scenarios. Our results indicate that the area affected by global total fires will decrease from the current 452 to 211–378 Mha yr−1 in the 2090s under RCP 6.0 and to 184–333 Mha yr−1 under RCP 2.6, mainly due to socioeconomic factors such as population and economic growth. We also predict that DDF will decrease from the current 73 million hectares per year (Mha yr−1) to 54–66 Mha yr−1 in the 2090s under RCP 6.0 and 46–55 Mha yr−1 under RCP 2.6. The main contributor to these decreases in DDF burned area was climate change, especially the increasing of precipitation. The impact of future land use change on future DDF was similar or slightly lower than present‐day. South America, Indonesia, and Australia were identified as high‐risk regions for future DDF, mainly due to the expansion of wood harvest and pastureland. Appropriate land and fire management policies will be needed to reduce future fire damage in these areas. Plain Language Summary: Global fire activities are affected by climate, land use, and socioeconomic factors. In order to represent changes in future fire activity, we model worldwide burned area with a particular focus on land‐use change and predict future changes. We find that total fires could decrease in the future. The most obvious factor is that socioeconomic change (i.e., population and GDP growth) will decrease future fire activity. Climate change might cause a reduction or increase in total fire. The impact of future land‐use changes on fires is similar to that of present‐day. The land use changes due to wood harvest in tropics and pasture expansion in temperate regions increased regional deforestation or vegetation degradation fires in future. And we expect that these land‐use change will increase fires in South America, Indonesia, and Australia, suggesting the need for adaptation to reduce the impacts of fires in these regions. Ensembles of climate change projections created by general circulation models (GCMs) with high resolution are increasingly needed to develop adaptation strategies for regional climate change. The Meteorological Research Institute atmospheric GCM version 3.2 (MRI-AGCM3.2), which is listed in the Coupled Model Intercomparison Project phase 5 (CMIP5), has been typically run with resolutions of 60 km and 20 km. Ensembles of MRI-AGCM3.2 consist of members with multiple cumulus convection schemes and different patterns of future sea surface temperature, and are utilized together with their downscaled data; however, the limited size of the high-resolution ensemble may lead to undesirable biases and uncertainty in future climate projections that will limit its appropriateness and effectiveness for studies on climate change and impact assessments. In this study, to develop a comprehensive understanding of the regional precipitation simulated with MRI-AGCM3.2, we investigate how well MRI-AGCM3.2 simulates the present-day regional precipitation around the globe and compare the uncertainty in future precipitation changes and the change projection itself between MRI-AGCM3.2 and the CMIP5 multiple atmosphere–ocean coupled GCM (AOGCM) ensemble. MRI-AGCM3.2 reduces the bias of the regional mean precipitation obtained with the high-performing CMIP5 models, with a reduction of approximately 20% in the bias over the Tibetan Plateau through East Asia and Australia. When 26 global land regions are considered, MRI-AGCM3.2 simulates the spatial pattern and the regional mean realistically in more regions than the individual CMIP5 models. As for the future projections, in 20 of the 26 regions, the sign of annual precipitation change is identical between the 50th percentiles of the MRI-AGCM3.2 ensemble and the CMIP5 multi-model ensemble. In the other six regions around the tropical South Pacific, the differences in modeling with and without atmosphere–ocean coupling may affect the projections. The uncertainty in future changes in annual precipitation from MRI-AGCM3.2 partially overlaps the maximum–minimum uncertainty range from the full ensemble of the CMIP5 models in all regions. Moreover, on average over individual regions, the projections from MRI-AGCM3.2 spread over roughly 0.8 of the uncertainty range from the high-performing CMIP5 models compared to 0.4 of the range of the full ensemble. Ottelia, a pantropical genus of aquatic plants belonging to the family Hydrocharitaceae, includes several narrowly distributed taxa in Asia. Although the Asian species have received comparatively more research attention than congeners in other areas, various key taxonomic questions remain unaddressed, especially with regards to apparent cryptic diversity within O. alismoides, a widespread species complex native to Asia, northern Australia and tropical Africa. Here we test taxonomic concepts and evaluate species boundaries using a phylogenetic framework. We sampled five of the seven species of Ottelia in Asia as well as each species endemic to Africa and Australia; multiple samples of O. alismoides were obtained from across Asia. Phylogenetic trees based on five plastid DNA markers and the nuclear ITS region shared almost identical topologies. A Bayesian coalescent method of species delimitation using the multi-locus data set discerned one species in Africa, one in Australia and four in Asia with the highest probability. The results lead us to infer that a population sampled in Thailand represents a hitherto unrecognised cryptic taxon within the widespread species complex, although the apparent lack of unambiguous diagnostic characters currently precludes formal description. Conversely, no molecular evidence for distinguishing O. cordata and O. emersa was obtained, and so the latter is synonymised under the former. Two accessions that exhibit inconsistent positions among our phylogenetic trees may represent cases of chloroplast capture, however incomplete lineage sorting or polyploidy are alternative hypotheses that ought to be tested using other molecular markers. The Dixon Island Formation of the coastal Pilbara Terrane, Western Australia is a 3.2 Ga volcanic-sedimentary sequence influenced by syndepositional hydrothermal activity formed in an island-arc setting. We documented lateral variations in stratigraphy, hydrothermal alteration, and biological activity recorded in the sedimentary rocks (over several kilometers), with the aim of identifying areas of biological activity and related small-scale structures. The Dixon Island Formation comprises volcaniclastics, black chert, and iron-rich chert within seven tectonic blocks. Based on detailed geological mapping, stratigraphic columns, carbon isotope composition, and organic carbon (Corg) content, we found lateral (>5 km) variations in stratigraphy and carbon isotope compositions in a black chert sequence above the Mesoarchean seafloor with hydrothermal activity. Two felsic tuff layers are used as stratigraphic marker beds within a black chert sequence, which was deposited on altered volcanic rocks. The black chert sequence in each tectonic block is 10-20 m thick. Thickness variations reflect topographical undulations in the paleo-ocean floor due to faulting. Early-stage normal faults indicate extensional conditions after hydrothermal activity. Black chert beds in the topographically subsided area contain higher Corg contents (about 0.4 wt%) than in areas around the depression (<0.1 wt%). Carbon isotope compositions for the black chert vary from −40 to −25‰, which are similar to values obtained for a black chert vein within the komatiite-rhyolite tuff sequence (underlying the black chert sequence). Those for other rock types in the Dixon Island Formation are −33 to −15‰. Results indicate that deformation occurred soon after the final stages of hydrothermal activity. After this early-stage deformation, organic-rich sediments were deposited over an area several kilometers across. The organic-rich sediments indicate stagnant anoxic conditions that resulted in the deposition of siliceous and organic matter from hydrothermal vein systems. When hydrothermal activity terminated, normal faulting occurred and organic matter was deposited from the sea surface and silica from the seafloor. [ABSTRACT FROM AUTHOR]

Using a variety of time‐series data, we compare how changes in risk perception following the attacks impacted domestic and international demand. This study explored variations in the primary service and clinical outcomes of a state-wide advanced practice physiotherapist-led service embedded in public medical specialist orthopaedic and neurosurgical outpatient services across Queensland, Australia. An audit of the service database over a six-year period was taken from 18 service facilities. The primary service and clinical outcomes were described. Variations in these outcomes between facilities were explored with a regression analysis adjusting for known patient- and service-related characteristics. The findings showed substantial positive impacts of the advanced practice model across all facilities, with 69.4% of patients discharged without a need for medical specialist review (primary service outcome), consistent with 68.9% of patients reporting clinically important improvements in their condition (primary clinical outcome). However, 15 facilities significantly varied from the state average for the primary service outcome (despite only three facilities varying in the primary clinical outcome). While this disparity in the primary service outcomes appears to be influenced by potentially modifiable differences in the service-related processes between facilities, these process differences only explained part of the variation. The mining industry makes up a large portion of the gross domestic product (GDP) in Australia, although securing human resources remains a problem in that field. The aim of this paper is to identify Australian university mining students' preferences, considering it as potential employees' preferences, for labour conditions at mining sites by means of a discrete choice experiment to promote efficient improvements in labour conditions in the mining industry. The data of 93 respondents analysed in this paper was collected by survey carried out in two universities in Australia. The result of the study showed that students have preferences on several factors such as wage, fatality rate, working position, commuting style, and company. Students having specific sociodemographic characters were found to show specific preferences on labour conditions. Spirometra erinaceieuropaei is a diphyllobothriid cestode whose adult stage occurs mainly in cat-like carnivores, but occasionally in canids and humans. Although it is generally accepted that the distribution of S. erinaceieuropaei is cosmopolitan, it is controversial as to whether all of S. erinaceieuropaei reported are the same species. This study determined partial sequences of the CO1 gene from several isolates in Asian countries and compared them to sequence data from the GenBank/EMBL/DDBJ nucleotide sequence database. Then intraspecific variation of S. erinaceieuropaei and its phylogenetic relationship with Diphyllobothrium were evaluated. The level of nucleotide variation in the CO1 gene sequences within S. erinaceieuropaei was less than 2.6%. Although it was a little larger than that within each species of Diphyllobothrium (0.1-1.0%), it was much smaller than the interspecific variation within the genus Diphyllobothrium (6.2-14%). These facts indicate that all isolates of S. erinaceieuropaei used in this study, which were collected from Asia, Australia and New Zealand, belong to the same species. Based on CO1 gene sequences, genus Spirometra is clearly separate from the genus Diphyllobothrium. It seems that the genus Spirometra is not a synonym of the genus Diphyllobothrium. The phylogenetic relationship between S. erinaceieuropaei and Sparganum proliferum inferred from the CO1 gene clearly confirm the previous opinion that S. proliferum is a distinct species from S. erinaceieuropaei.
