Background: Through the filtering of information, the creation, and reinforcement of stereotypes, media moulds attitudes and set agendas on critical social issues including public policy and disability. Methods: This study explored Australian media representations of the care of people with intellectual disability during a crucial period in disability policy change: the National Disability Insurance Scheme (NDIS) rollout (2013-2018). Search criteria identified 168 news stories, examined via content analysis and news framing. Results: Four major issues were identified: Roles and responsibilities of government; housing; mistreatment of persons with intellectual disability, and responsibility of care for families. Stories tended to be presented negatively, however, regional and local/community metropolitan stories were more balanced or positive compared with major metropolitan stories. Conclusion: Despite significant disability policy change, media presentations continue to reinforce stereotypes of people with intellectual disability and position the government as one continuing to fall short in critical areas of funding, safety, and support.

Purpose: The present article provides a narrative review of Australia's approach toward acquired brain injury (ABI) and proposes how Australia and the United States can collaborate to improve service delivery for persons with ABI and their families with epidemiology, healthcare, prevention, research, and training. Method: A narrative review of journal articles, government documents, and websites was completed to present a broad overview of Australia's approach toward meeting the needs of persons with ABI. Results: The narrative review and synthesis of publications were summarized into the following categories: a) overview of ABI in Australia, b) long-term care government programs, c) services and advocacy, d) research and training, and e) recommendations for Australian-U.S. collaborations. Conclusion: The current time presents an opportunity for Australia and the United States to collaboratively address areas of common ABI need by dialogue, collaboration, and academic engagement, which may lead to better outcomes for persons with ABI by the sharing of research findings, service approaches, advocacy efforts, and rehabilitation counselor training. Rehabilitation counselors in Australia and the United States should lead the process of collaboration and engagement around common areas of ABI need.

Background: This study synthesises current literature concerning the barriers to the National Disability Insurance Scheme (NDIS) faced by people with intellectual disability and/or complex support needs who are involved with the criminal justice system, or at risk of (re)involvement. Method: Through targeted searches of key databases, 41 publications were identified from the scholarly and grey literature. Results: Collected literature raises three main barriers to the NDIS. First, there are (mis)identification and diagnostic tensions hindering eligibility. Second, there are problems with the planning process, including the impracticality of "planning for" chaotic situations. Third, there are market and workforce deficits. Conclusions: Research into the NDIS experiences of justice-involved people with intellectual disability and/or complex support needs is nascent and developing alongside an evolving NDIS landscape. Future research should focus on the effects of forthcoming developments designed to reduce the barriers facing these populations, including the Complex Support Needs Pathway, and workforce capabilities and market strategies.

Introduction: This paper reports the findings of a study of risk management in public libraries. The focus of the research was to determine whether the libraries had a risk management and disaster plan for major disasters. Method: A qualitative study was done to investigate risk management and disaster recovery in public libraries in South Australia. Seven personal interviews were conducted with library managers and librarians at four public libraries. Analysis: The qualitative results emerging from the interviews were analysed through hand coding using grounded theory. Results: Participants confused risk management and disaster recovery with the practice of work (occupational) health and safety. None of the participating libraries have a risk management or disaster plan. Conclusions: The library managers do not rate the risk of disaster as high, believing that their library is located in a low-risk disaster area. They also do not regard any part of their collections to be of great value. Loss of a collection is perceived as an opportunity to refresh that collection. The participants do not consider risk management and disaster recovery as an important part of their business. The National Disability Insurance Scheme (NDIS) was introduced in Australia in 2013 to address the inadequacies of a fragmented and underfunded service system for people with disabilities. For young children with developmental disabilities, NDIS services fall under the early childhood early intervention (ECEI) approach, the impact of which is yet to be documented. Considering the critical role of families in supporting their children with a disability, the aim of the present study was therefore to examine their experiences of the ECEI approach in the State of Victoria. Seventeen parents participated in interviews and focus groups, which generated information clustered around five themes: (1) accessing the NDIS and the ECEI; (2) plan development and implementation; (3) choice and control; (4) family and community life; and (5) parental distress. While highlighting the potential of the NDIS funding and its long-term benefits, participating parents reported numerous challenges in their pursuit of supports under the ECEI. It is concluded that for the NDIS to fulfil its promise and potential, substantive revisions of its systems, policies and practices will be required to be relevant and sensitive to the needs of young children with developmental disabilities and their families.

In Australia, the National Disability Insurance Scheme (NDIS) was rolled out in pilot sites in 2013. This article reports a formative study conducted with qualified preschool teachers in one site in a context of change. The NDIS has been hailed in the media as a bold move to deliver more choice of services to people with disabilities and incorporates the delivery of early intervention (EI) services into its portfolio. As established EI agencies became providers for the scheme, local referral networks changed substantially. Children needed to establish eligibility for the scheme prior to families' entry. Fourteen preschool teachers responded to an online survey, and four opted in for this study, which used positioning theory and a constructivist approach. Online surveys and interviews were conducted in 2 months at the end of the 3-year pilot phase. Findings suggest that teachers detecting developmental delays (DDs) in 4-year-old children needed to reposition themselves away from the rights and duties of the developmental expert to develop timely collaborative relationships with parents if the dominant storylines of early detection, medical confirmation, and the benefits of EI for families were to remain in the foreground.

Background: Planning is a key mechanism by which the Australian National Disability Insurance Scheme (NDIS) ensures individuals with disability have choice and control over supports. People with intellectual disability will comprise the largest NDIS participant group and many will need assistance to engage in planning. In order to respond effectively, NDIS planners must understand the decision-making support required by individuals. Method: Focus groups were conducted with 9 adults with intellectual disability living in an NDIS trial site to explore their experiences of NDIS planning. Results: Thematic analysis identified 6 themes related to good planning experiences for people with intellectual disability: preparation, learning from mistakes, personal growth, and having a credible, consistent, and disability-aware planner. Conclusions: Participants who developed a trusting relationship with a planner used planning to increase independence and social participation. Planner skills, particularly communication and sector knowledge, and attributes such as warmth and openness created trust.

In Australia and Europe, government agencies and not-for-proft organisations (NFPOs) have had long involvement in the funding and provision of community disability services. Signifcant change has occurred in Australia over the past two decades in the way government funds are expended, with marketplace mechanisms increasingly being used. As a consequence of economic and governance imperatives, funding of services via NFPOs has changed signifcantly with a move away from the provision of grants to the contracting of these organisations for the provision of services. In 2013, a new national policy, the National Disability Insurance Scheme (NDIS), was introduced that has impacts for the provision of disability services for children and their families. In particular, Indigenous families are likely to experience barriers in accessing services. This paper reviews the impact of international changes in policy and associated funding models and considers the impacts and research implications of Australia's initial experience of implementation of the NDIS.

Background: Planners will engage with people with cognitive disability and complex support needs in the Australian National Disability Insurance Scheme, but the specific skills needed to build sustainable plans with this group are not yet known. Method: A qualitative study was conducted to explore the barriers and facilitators to planning with people with cognitive disability and complex support needs. Focus groups were held with 99 planning practitioners across metropolitan and regional locations in New South Wales, Australia. Results: Thematic analysis showed planners need to build a partnership based on mutual trust and respect with a person with complex support needs and harness a range of skills to respond to individual support needs, learning capacity, systemic hurdles and life challenges. Conclusions: Planner skills can be used to address barriers to planning for people with cognitive disability and complex support needs. Gaining skills took professional support and personal commitment.

This paper explores the changing terrain of disability support policy in Australia. Drawing on a critical disability framework of policy sociology, the paper considers the policy problem of access to education for people with disabilities under recent reform by means of the National Disability Insurance Scheme (NDIS), which commenced full roll-out across the country from July 2016. The paper reviews NDIS reports, legislation and associated literature to consider how eligibility to scheme participation and education services are shaped, and how education is positioned in the development and implementation of the NDIS. The analysis highlights tensions that exist for people with disabilities and their families who both access the scheme and who might draw on its provision to support their education, because of the way the policy is oriented towards pathological categorisation, standardised outcomes and service delineation rather than integrated support and informed involvement. The paper concludes by arguing that despite the policy priority across Organisation for Economic Co-operation and Development countries of increasing lifelong learning opportunities, fragmented NDIS policy in Australia prevents people with disabilities from achieving this ideal.

This study sought to analyze and evaluate the accessibility, availability and quality of online information regarding the National Disability Insurance Scheme (NDIS) and hearing loss. The most common search engine keyword terms a caregiver may enter when conducting a web search was determined using a keyword search tool. The top websites linked with each keyword term were then evaluated using a subjective quality rating tool. It was found that only 8% of the websites evaluated referred users directly to the NDIS. The majority of websites (70%) scored in the fair range for overall quality with 15% of websites scoring poorly and 15% of websites scoring highly. However, the hierarchy in which websites were presented on Google when a search string was typed into the search bar was not necessarily associated with the highest quality website appearing first. Overall, whilst the quality of information regarding the NDIS was of a fair quality, the accessibility to this information by caregivers was found to be poor.

The author of this conceptual article was awarded a Churchill Fellowship to travel to the United States, Ireland, and United Kingdom to meet with researchers, practitioners, and community agencies and review how support for people with intellectual disability is provided. A specific focus was upon provision of care for individuals who were either ageing or requiring palliative care. The project took place from June to August 2013 and involved face-to-face interviews and site visits with key academics, government representatives, and community providers in Los Angeles, Chicago, New York, New Jersey, Dublin, Edinburgh, London, Oxford, Cambridge, Bath, Bristol, Birmingham, and Stoke-on-Trent. A priority was placed upon examining how the key features of leading practice models could be implemented within the Australian environment featuring a National Disability Insurance Scheme (NDIS). On the basis of the observations of practice, key points are discussed regarding the potential for improvements to the support of people with intellectual disability within the Australian context of the NDIS.

Using 1980/81 and 1990/91 census data from Australia, Canada, and the United States, we estimate the effects of time in the destination country on male immigrants' wages, employment, and earnings. We find that total earnings assimilation is greatest in the United States and least in Australia. Employment assimilation explains all of the earnings progress experienced by Australian immigrants, whereas wage assimilation plays the dominant role in the United States, and Canada falls in between. We argue that relatively inflexible wages and generous unemployment insurance in countries like Australia may cause assimilation to occur along the quantity rather than the price dimension. In the five years leading up to 2002 there were many significant changes in the insurance industry in Australia that brought about a range of training needs. These training needs arose from matters as diverse as mergers, increased competition, corporate failures, and legislative changes. This study includes findings from a survey of the insurance industry in Australia in the period 2000-2002 as a means of exploring the importance of the environment (marketplace) in predicting aspects of training needs. The findings demonstrate that an environmental analysis approach to training needs analysis can predict the type of training that organisations will subsequently need to provide, and thus has the potential to produce a more accurate assessment of training needs in the future. It also has the potential to contribute to the economic success of businesses operating in the financial services industry.

Introduction: This study examines the impact of funding provided to support the well‐being of rural health practitioners and their practice staff following the 2019–2022 bushfires. Objective: To assess the benefits and implications of grant funding for rural practices to aid recovery following bushfires in NSW, Australia. Design: An explanatory sequential mixed method design consisted of a survey and a thematic analysis of semi‐structured Interviews. Findings: Five key themes emerged from analysis: (1) the disasters altered the role of the practice and therapeutic relationships; (2) the funding had a positive impact on access to professional development; (3) the training had a positive impact on staff well‐being and resilience; (4) the professional development had a positive impact on rural practitioner's sense of capability; and (5) important elements of future grant opportunities. Discussion: The findings indicate the importance of recovering funding to facilitate access to professional development for rural health practioners during natural disasters which improved their mental health and wellbeing, capability and support to clients. Conclusion: Recovery funding facilitated improvement in workforce capability, professional resilience, mental health and well‐being in the face of natural disasters and emergencies. There was a link between training and maintaining the capability of rural health care practitioners and their practice staff. Objectives: With elimination of the financial burden of dental services, one can expect an increase in utilization of dental services. This study aimed to investigate the effective mechanisms of financial barriers to the utilization of dental services in an Australian adult population. Methods: South Australian survey data from the Dental Care and Oral Health Study (2015) were analysed. Following the flexible mediation approach, the direct effect of income and indirect effect of income through mediators (insurance, concession card and service sector) on the outcomes (visit avoidance and treatment prevention due to the cost) were calculated. Results: Findings showed that around half of the low‐income people and one‐third of the high‐income South Australians experienced a financial burden on receiving a dental visit or service. The indirect effect of income on both outcomes of financial burden was negligible, while the direct effect was significant. By changing the potential outcome distribution to their counterfactual exposure distribution and if the mediators are drawn from their counterfactual exposure (lower/higher income) distribution, the odds of visit avoidance and treatment prevention due to the cost were almost twice (Odds Ratio: 2.13, 95% CI 1.72–2.60) and 98% (Odds Ratio: 1.98, 95% CI 1.67–2.35) than in the lower‐income individuals, respectively. Conclusions: It can be concluded that the level of household income, directly and regardless of insurance status, concession card ownership and whether the service sector was public or private, affected the financial burden on utilization of dental services. This study reviews perceptions over flood insurance and the purchase of this measure among residents living in Southeast Queensland, Australia. The decision of householders living in eleven suburbs in Brisbane to take up flood insurance is analysed by an exploratory model that regresses a polar dependent variable with 22 independent variables directly associated with flood insurance experience. The main findings, based on a questionnaire personally delivered to 1796 householders (homeowners and renters) randomly selected and representing a survey return rate of 26.16% (N = 469), are (i) the need to review and include a clear definition of 'flooding' in insurance policies to minimize unexpected surprises by policyholders claiming coverage for this hazard; (ii) the neutral influence that insurers' recommendations exert on householders' decisions to engage in residential flood risk reduction measures; (iii) the self-reliance of householders living in risky areas even in the absence of financial relief provided by public flood insurance schemes; and (iv) the objection of householders to increased insurance premiums based on updates of flood hazard maps. Controversy surrounding recommendations for supplemental screening (ultrasound and magnetic resonance screening) in women with dense breasts exists, as the long-term benefits from these additional modalities may not outweigh the harms. This study aimed to examine factors associated with supplemental screening intentions following a hypothetical breast density notification in a population of women who have not been routinely notified. Australian women of breast screening age participated in an online randomised experimental study where they were presented with one of two breast density notifications (with or without health literacy-sensitive information) and asked their screening intentions. After adjusting for covariates in multivariable analyses, women in both groups (n = 940) who indicated higher levels of breast cancer worry, had private health insurance, had a family history of breast cancer, and had a greater number of times previously attending mammography screening had higher intentions for supplemental screening. Understanding women's supplemental screening intentions following notification of dense breasts has important implications for health systems with breast screening considering the impacts of widespread notification. Personal, clinical and psychological factors should be considered when discussing both the benefits and harms of supplemental screening with women with dense breasts. [ABSTRACT FROM AUTHOR]

The expansion of genetic and genomic testing in clinical practice and research, and the growing market for direct-to-consumer genomic testing has led to increased awareness about the impact of this form of testing on insurance. Genetic or genomic information can be requested by providers of mutually rated insurance products, who may then use it when setting premiums or determining eligibility for cover under a particular product. Australian insurers are subject to relevant legislation and an industry led standard that was updated in 2019 to introduce a moratorium on the use of genetic test results in life insurance underwriting for policies

Purpose: This qualitative study conducted in Queensland, Australia aimed to explore various stakeholders' perspectives on (1) the barriers and facilitators of Return to Work (RTW) for injured persons following minor to serious Road Traffic Injuries (RTI) in a fault-based scheme, and to investigate the changes needed to better support RTW following RTI. Methods: The study was performed using the Interpretive Description methodological approach. Data were collected during interviews (n = 17), one focus group (n = 4), and an open-ended survey (n = 10) with five categories of stakeholders: treating health providers, workplace representatives, legal representatives, rehabilitation advisors, and insurers. Participants were eligible to participate if they had at least one year of employment history in their respective profession in Queensland, Australia, and were experienced in assisting the RTW of people with RTI. Thematic analysis was used to analyse the data. Results: Seven themes were extracted reflecting the barriers and facilitators of RTW along with stakeholders' recommendations to address these barriers. These themes were: (1) knowledge is power; (2) stakeholder expertise; (3) early and appropriate treatment matters; (4) insurers could do better; (5) necessity of employers' support; (6) fix the disjointed system; (7) importance of individual factors pre- and post- injury. The main barriers identified were stakeholders' insufficient communication and knowledge on RTW process following RTI. Conclusions: Individual and system barriers identified in this study suggest that RTW after RTI occurs in a complex system requiring the commitment of all stakeholders. This is particularly important for managing knowledge-related barriers by provision of high quality and easily accessible information about the RTW process, disability schemes, and the nature of RTI. Introduction Australia has achieved universal health insurance for its population since 1975 - a major step forward for increasing access to primary care (PC). Nevertheless, there are reports of several multi-layered challenges, including inequity, that persist. This analysis aims to undertake a scoping review of the success, explanatory factors, and challenges of Primary Health Care (PHC) in Australia guided by the World Health Organization (WHO)-defined key characteristics of good PC. Methods We searched PubMed, Embase, Scopus and Web of Science using key terms related to PHC principles, attributes, system functioning and health care delivery modalities. We also used key PC terminologies used to assess key characteristics of good PC developed by WHO and key terms and attributes from Australia's health care landscape. We then integrated our search terms with the PHC Search Filters developed by Brown, L., et al. (2014). We restricted the search from 2013 to 2021. Two authors independently assessed study eligibility and performed a quality check on the extracted data. We presented findings according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Results We identified 112 articles on primary health care (PHC), represented from all Australian states and territories. Overall, Australian PHC has achieved comprehensiveness, access and coverage, quality of care, patient / person centeredness and service coordination indicators with exemplary evidence-base practice/knowledge translation and clinical decision-making practices at the PC settings. Yet, we identified complex and multilayered barriers including geographic and socio-economic berries and inequality, staff dissatisfaction/turn over, low adoption of person-centred care, inadequate sectoral collaboration, and inadequate infrastructure in rural and remote primary care units. Conclusion Primary health care in Australia, which has evolved through major reforms, has been adapting to the complex health care needs of the socio-culturally diversified nation, and has achieved many of the PC attributes, including service diversity, accessibility, acceptability, and quality of care. Yet, there are persistent gaps in service delivery to socio-economically disadvantaged populations, including indigenous people, culturally and linguistically diverse (CALD) populations, and rural- and remote-residents. These challenges could be mitigated through system-wide and targeted policy-level intervention to further improve service delivery through effective and functional local health service coordination, sectoral integration, and improving health care providers' cultural competence. The comparative efficiency, equity, and simplicity attributes of different details of a reform package to replace current conveyance duty, narrow base land taxes and special taxes on insurance imposed by the states are assessed. Option details for the replacement annual property tax include land or improved value as tax base, flat or progressive tax rate schedule, and transition path. To minimise changes in the distribution of the tax burden while reaping most of the efficiency gains, the article argues for a land base, progressive rate schedules which differ for owner occupied, rental and commercial property categories, and a transition path along the lines of the Australian Capital Territory phased switch to the replacement tax or provide some credit for recent payers of stamp duty. Background: Hysteroscopy is a safe procedure which allows both diagnosis and management of cervical and endometrial pathology. Improving Australian women's access to outpatient hysteroscopy would improve cost efficiency and allow women a quicker recovery, negating the need for a general anaesthetic. Increasing the Medicare renumeration for outpatient hysteroscopy could incentivise provision of outpatient hysteroscopy. Aim: We sought to review the trend and current uptake of outpatient diagnostic hysteroscopy in Medicare Benefits Scheme (MBS)‐funded clinics within Australia. Materials and Methods: A retrospective review of Australian MBS data from 1 January 1993 to 31 December 2020. Results: Over the past 27 years, 1 319 909 hysteroscopies have been claimed from Medicare in Australia, with 39 958 (3.1%) claimed as an outpatient diagnostic procedure. Australian outpatient diagnostic hysteroscopy MBS item number use peaked in 1994 (5871 cases) representing 18.2% of all hysteroscopies claimed through the MBS that year. Uptake of the outpatient hysteroscopy item number rapidly declined after 1994 and in 2010, it represented 0.8% of all hysteroscopies claimed (426 of 49 618) and has remained below <0.5% from 2010 to 2020. Conclusions: The lower Medicare rebate and lack of recognition of the importance of outpatient hysteroscopy has likely been a driving factor in continuing inpatient hysteroscopy. Incentivised government funding has been successfully utilised in the UK to improve outpatient hysteroscopy access. This MBS data suggests that Australia has not progressed in outpatient hysteroscopy access and support a change in the current funding model to assist in supporting the uptake of outpatient access. To extend the authors' previous research by investigating the maintenance of work performance over time and the long-term experience of employment following traumatic brain injury (TBI). Semi-structured interviews were conducted with a sample of 72 individuals with predominantly severe TBI, up to 10-years post-injury. Thematic analysis of transcribed interviews was conducted. Four key themes were identified: brain injury influences on work performance and long-term employment; factors optimising long-term employment; and need for compromise and acceptance, which all impacted the final theme, quality of life outcome. While TBI-related difficulties such as fatigue, cognitive, and emotional difficulties still impacted work many years post-injury, work modifications and employer and vocational rehabilitation (VR) support were important factors optimising long-term employment. The timing of return to work (RTW) was also an important and potentially modifiable factor influencing work performance, which appeared to have long-term effects on the maintenance of employment. Support for acceptance of altered career goals is needed. Findings support the need for early VR to facilitate employer contact, to maximise adjustment and preparation time before RTW, to educate both employees and employers and to implement work modifications to support continuing employment after TBI. The maintenance of employment following TBI is of critical importance for financial independence, psychological and social well-being, and improved quality of life. Injury-related difficulties still impact work performance many years after injury requiring individuals with TBI to continue working in modified roles and for reduced hours over the long-term. Ongoing VR and employer support is needed to facilitate long-term work modifications and compensatory strategies to support work performance. Early VR involvement to facilitate employer contact and maximise adjustment and preparation time before returning to work is necessary to avoid the negative and potentially dire consequences of returning too early. Objective: This study set out to present data on out-of-pocket payments for Medicare mental health services provided by general practitioners (GP), psychiatrists, clinical psychologists and other psychologists, to explore how much is spent on out-of-pocket payments for mental health; if any trends could be seen; and what variations exist across regions. Methods: We performed secondary analysis of publicly available data on Medicare-subsidised GP, allied health and specialist health care across Australia. We merged and interrogated data covering the period 2013–19 and 2019–21 to create a data set covering eight full years of Medicare mental health services, arranged by profession and by region. Results: Out-of-pocket payments for mental health care in Australia have been rising consistently over the period 2013–21, at a considerably faster rate than overall expenditure on mental health care. There is wide variation in out-of-pocket payments depending on where you live. Conclusions: The impact of out-of-pocket payments on community access to mental health care is growing. This has implications, especially in poorer communities, for access to care. This should be an important consideration taken as the Australian Government considers next steps in national mental health reform, including the Better Access Program, currently under evaluation. What is known about the topic? A$35 m is spent on Medicare mental health services every week but there has been little research on the out-of-pocket payments charged to Australians. What does this paper add? Looking at Medicare data from 2013 to 2021, this paper answers three main questions: how much are out-of-pocket payments for mental health in Australia; what trends can be seen; and what variations exist? What are the implications for practitioners? 'Insurantial imaginaries' describes the shared structures of comprehension and action that establishes the social value of insurance and its practical influence. As insurance is a bricolage of economic, legal, political, cultural and moral elements that vary across time and space, for insurance to be intelligible and functional, these imaginaries render this dynamic complexity into stable and discrete forms. In this paper, we explore one such imaginary constituted through the intersection of housing and insurance norms, vis a vis house and contents insurance, in home-owning democracies. Drawing on the cases of Australia and the United Kingdom, we observe how a rise in renting in these countries is resulting in an increase in underinsurance, with renters less likely to insure their assets. This is refashioning a long-standing insurantial imaginary shaped by socio-cultural aspirations for home-ownership, liberal norms of individual autonomy, and links between home-ownership and democracy, including participation in voting. In observing that this imaginary valorises home-ownership and insurance, and problematises renting and underinsurance, we draw attention to the covert power of insurance in co-constituting everyday practices and identify a novel spatialised insurantial imaginary. In grounding our analysis within home-owning democracies, we also make a broader contribution to economic and financial geography by extending understanding of the complex spatial nature of financial mechanisms and processes, and how market-based economies appear to be reconfiguring democracy. We examine how health shocks impact on households' decision to save, and how different socioeconomic and ethnic groups respond to health shocks in Australia. We find that health shocks are associated with a substantial reduction in individual savings. We do not find any substitution of labor supply by a partner in response to an individual's health shocks. We extend previous work by examining whether access to formal health insurance or government benefits mitigate the economic consequences of a health shock. We find evidence that negative health shocks are associated with an increase in receipts of public transfers and benefits in the following years. There is evidence that the fall in savings is greater for low‐income individuals. Individuals who have private health insurance are more affected by health shocks, compared with individuals who do not have private health insurance, suggesting a role for selection into private health insurance. Across the world, cities are growing, blurring lines between urban and rural. In Australia, peri-urban areas are undergoing demographic shifts and extensive development. In the literature, these shifts are characterised by differences in the risk perceptions and hazard experiences between established and incoming residents. In this paper, we illustrate how some of these differences are perceived by focusing on house and contents insurance in the bushfire-prone City of Whittlesea on the fringes of Greater Melbourne. This location captures the complex relationship between growing population and high bushfire risk, and is the site of the country's deadliest bushfire event, Black Saturday, in 2009. Through in-depth interviews, we observe that residents perceive insurance as playing a role in peri-urban change. Specifically, underinsurance is understood to be a challenge faced by many impacted by the Black Saturday fires, and contributes to feelings of uncertainty regarding the capacities of changing communities to work together to prepare for and recover from future fires. Our focus on insurance is informed by the need to better understand the social qualities of this dimension of disaster preparedness and recovery, and how perceptions of insurance amid peri-urban change may help produce social patterns and trends. The article examines the enforcement of policy terms in insurance contracts at common law and the legislative measures that have been deployed in some common law countries, including Nigeria, the United Kingdom and Australia, to ensure fairness as well as to balance the inequality in the bargaining power of the contracting parties. In as much as the principle of freedom of contract will generally be honoured by the court, the paper argues that through legislative intervention in policy terms in these countries, the principle is being discountenanced with, in appropriate cases, in order to effectuate the just and reasonable expectation of the insured. The focus of this article is harm that may be caused to consumers arising out of use of AI- and Big Data-powered analytics in terms of discrimination, exclusion, and unfair prices. We analyse insurance-specific rules currently in place, including recent reforms. We focus on anti-discrimination laws, insureds’ duty of disclosure, and insurers’ obligations (including the duty of utmost good faith), and consider whether they sufficiently address the potential harms that using decision-making models may cause to consumers. The National Disability Insurance Scheme is the new consumer-controlled funding system for people with disability in Australia, and is expected to enhance participation outcomes of people with disability. This research explored participation opportunities for people with disability during the formative period of transition to the scheme, through stakeholder accounts of changes in allied health service contexts. Qualitative data were generated during interviews, workshops and meetings with industry, policy, practice and education stakeholders involved in scheme services. Inductive coding explored key themes within the data. The International Classification of Functioning model was then applied as a deductive coding framework to illuminate how the scheme was perceived to be impacting participation opportunities for recipients of scheme funding. Using the International Classification of Functioning helped us illuminate whether changes resulting from scheme transition posed participation opportunities or barriers for scheme recipients. Research participants often framed these changes negatively, even when examples suggested that changes had removed participation barriers for scheme recipients. Some participants viewed changes as obstructing equitable and quality professional practice. We explore potential opportunities to resolve tensions that also optimise the participation outcomes of individuals who receive services through individualised funding. The introduction of individualised funding has removed barriers to participation for many National Disability Insurance Scheme recipients. Efforts must be made to build the trust of stakeholders involved in National Disability Insurance Scheme service provision regarding how fee-for-service funding can lead to good participation outcomes for scheme recipients. Transparency around the shared processes of clinical governance and equitable service access operating in Australia's individualized disability funding scheme are suggested to build trust. In most of the United States, insurance companies may use gender to determine car insurance rates. In addition, several studies have shown that women over the age of 25 generally pay more than men for car insurance. Then, we investigate whether the distributions of claims for women and men differ in location, scale and shape by means of the GAMLSS regression framework, using microdata provided by U.S. and Australian insurance companies, to use this evidence to support policy makers' decisions. We also develop a parametric-bootstrap test to investigate the tail behavior of the distributions. When covariates are not considered, the distribution of claims does not appear to differ by gender. When covariates are included, the regressions provide mixed evidence for the location parameter. However, for female claimants, the spread of the distribution is lower. Our research suggests that, at least for the contexts analyzed, there is no clear statistical reason for charging higher rates to women. While providing evidence to support unisex insurance pricing policies, given the limitations represented by the use of country-specific data, this paper aims to promote further research on this topic with different datasets to corroborate our findings and draw more general conclusions., (© 2024. Purpose: This study aimed to explore individuals' experiences of return to work (RTW) following minor to serious road traffic injury (RTI) in Queensland, Australia; seek their recommendations if any, on how to provide support for RTW after RTI; and identify the strategies and resources used to return and remain at work after their RTI., Methods: The interpretive description methodological approach was used. Semi-structured interviews were conducted with eligible participants ( n  = 18) aged 18-65 y who had experienced a minor to serious RTI at least 6 months earlier. Thematic analysis was used to analyse the data., Results: Five themes emerged: (1) physical and mental consequences of RTI negatively impact RTW; (2) money matters; (3) RTW support makes a difference; (4) feeling alone and confused in the RTW process; and (5) several strategies and resources helped with return/stay at work after RTI. Regular contact and cooperation with employers and insurers, job modifications, and using social media to obtain information and social support were helpful RTW strategies. Participants recommended timely and appropriate medical care, financial assistance, and educational support., Conclusions: Policy changes to reduce financial stress, increase employer support, and improve injured individuals' knowledge following a RTI are recommended in jurisdictions operating a fault-based scheme.IMPLICATIONS FOR REHABILITATIONThis study identified several factors that can influence return to work (RTW) following minor to serious road traffic injuries (RTIs) in a jurisdiction operating a fault-based compensation scheme.Legislative changes that provide financial assistance to all injured people regardless of their fault-status could reduce financial stress arising from reduced work ability following a road traffic injury.Increasing employer' awareness of the importance of return to work for those with road traffic injuries and reimbursement for possible expenses of providing RTW support for these individuals could increase employability of injured people following RTI.Improving injured individuals' knowledge about return-to-work processes after a road traffic injury could accelerate recovery and return to work.

Objective: The aim of this study is to explore policy paths towards private health insurance (PHI) reform that might reduce out-of-pocket costs, restore public confidence and allow insurers to finance value-based care. Methods: This study used thematic analysis of semi-structured interviews with informed opinion holders, including a politician, three former senior public servants and an industry lobbyist. Critical analysis of peer reviewed and grey literature was also conducted. Results: PHI regulation is contributing to unexpected out-of-pocket expenses and low-value care. Modification of existing tax incentives would be incapable of significantly increasing PHI coverage. Regulatory reform could restore confidence among policy holders by promoting value-based care, wherein health outcomes are measured and incentivised with remuneration. Conclusions: Targeted relaxation of out-of-hospital restrictions should be explored to promote value-based competition and facilitate bundled payments for chronic disease management and community services. To address out-of-pocket diagnostic and procedural costs, insurers should have more responsibility for private specialist's fees, including by financing the entire provider's bill through insurance and redirecting the Medicare Benefits Schedule fee to the insurer. Home and contents insurance is framed as key to Australia's national strategy for disaster resilience. Despite the perceived importance of financial indemnity, ambiguity characterises the ways in which it facilitates both short‐ and long‐term everyday household recovery when disaster strikes. Addressing such ambiguity, this article explores how insurance impacts upon households' capacities to rebuild and recover after disastrous bushfire. In‐depth interviews conducted with residents in the Blue Mountains of New South Wales four years after they were affected by the October 2013 bushfires show that insurance was fundamental to recovery. Swift claims handling and payouts allowed participants to access rental accommodation, material necessities, and sufficient finances to repair, rebuild, and refurnish their houses—essentials in restoring everyday routines and a sense of normalcy. Yet, many people were underinsured and used alternative strategies to stretch "adequate" insurance coverage, which had practical and emotional implications. Importantly, while insurance reduced post‐disaster stress and trauma, the length and depth of recovery were more closely determined by the impacts of personal and situational circumstances than by insurance. On that basis, analysis points to the conclusion that while insurance is a crucial tool for disaster resilience, greater levels of psychosocial support are needed to improve households' short‐ and longer‐term recovery. In many countries, health technology assessment (HTA) organizations determine the economic value of new drugs and make recommendations regarding appropriate pricing and coverage in national health systems. In the US, recent policy proposals aimed at reducing drug costs would link drug prices to six countries: Australia, Canada, France, Germany, Japan, and the UK. We reviewed these countries' methods of HTA and guidance on price and coverage recommendations, analyzing methods and guidance documents for differences in (1) the methodologies HTA organizations use to conduct their evaluations and (2) considerations they use when making recommendations. We found important differences in the methods, interpretations of HTA findings, and condition-specific carve-outs that HTA organizations use to conduct evaluations and make recommendations. These variations have ethical implications because they influence the recommendations of HTA organizations, which affect access to the drug through national insurance and price negotiations with manufacturers. The differences in HTA approaches result from the distinct political, social, and cultural contexts of each organization and its value judgments. New cost-containment policies in the US should consider the ethical implications of the HTA reviews that they are considering relying on to negotiate drug prices and what values should be included in US pricing policy. While causes for underinsurance have been widely researched, changes to Australian building regulations in the last decade has established a new source of insurance miscalculations. Bushfire Attack Level (BAL) ratings can inflate rebuilding costs by 20% or more, yet BAL ratings remain obfuscated to homeowners and are notoriously confusing to navigate. After the October 2013 bushfires in New South Wales, the Blue Mountains Local Recovery Steering Group found that 'information on the BAL process, the guidelines, the expected costs, the consulting experts and a property's bushfire-prone status is literally all over the place'. This paper aims to provide clarity on the subject, tracing the precise socio-technical means through which disaster risk is perceived and assessed. The paper conceptualises insurance and risk ratings as calculative devices that provide both a technical solution to reduce financial losses and a philosophical tool for risk rationalisation. It then builds on interviews conducted with residents in the Blue Mountains affected by the 2013 bushfires, to ascertain how such calculative devices practically affect communities at risk. The paper concludes by outlining potential solutions to a confusing and costly problem in Australia, highlighting critical public awareness issues surround BAL ratings, which have profound insurance and wellbeing implications for people rebuilding and recovering from bushfire. Australian health policy has prioritized efforts to close the ten-year life expectancy gap between indigenous and nonindigenous Australians, a disparity largely driven by cardiovascular disease and diabetes. Because out-of-pocket spending poses a barrier to accessing medications for chronic conditions, in 2010 the Australian government reduced or eliminated medication copayments for indigenous people with chronic disease or risk factors for chronic disease. In this quasiexperimental study we found that the copayment reductions were associated with a 39 percent relative increase in the use of medications and a 61 percent reduction in out-of-pocket spending. Among indigenous Australians who qualified for the largest copayment reductions, overall use of medications increased by 156 percent--including increases of 26-109 percent in the use of lipid-lowering, hypertension, and diabetes medications. These findings suggest that Australia's novel strategy of targeted copayment reductions improved access to prescription medications among indigenous Australians, a population with a high burden of chronic conditions and marked social disadvantage.
