Background and aims: The impact of the COVID‐19 pandemic on gambling participation and levels of gambling harm across populations during the pandemic is now addressed in a well‐established body of empirical literature. This study aimed to measure the longer‐term implications of COVID‐19 on gambling participation and levels of gambling harm. Design: Population‐based cohort study using group‐based trajectory modelling. Setting: Australia, using gambling participation, problem gambling risk, sociodemographic and psychosocial data from 2019 (pre COVID‐19), 2020, 2021 (during COVID‐19) and 2023 (post COVID‐19). Participants: A population representative survey of Australian adults, including four waves collected in April 2019 (n = 2054), November 2020 (n = 3029), October 2021 (n = 3474) and January 2023 (n = 3370), with a subset (n = 3160) of the sample having longitudinal data available. Measurements Participants were asked which gambling activities they participated in over the past 12 months for money. Problem gambling risk was measured by the nine‐item Problem Gambling Severity Index (PGSI). Findings There was an overall reduction in gambling participation during COVID‐19 and return to pre‐pandemic levels for most gambling activities by 2023. The longitudinal analysis yielded four trajectories of gambling participation from 2019 to 2023, including individuals who (1) never gambled (25.0% of the longitudinal sample; n = 789); (2) engaged in non‐problematic gambling (59.8%; n = 1888); (3) ceased gambling during COVID‐19 and started again post pandemic (10.7%; n = 337); and (4) engaged in high risk gambling (4.6%; n = 146), with particular demographic and psychosocial profiles and patterns of participation in specific gambling activities related to these trajectories. Conclusions: Although overall gambling participation rates decreased at the population level in Australia during COVID‐19, by 2023 participation in gambling appeared to have nearly returned to pre‐pandemic levels. Objectives, Purpose, or Aim: The study aimed to decrease noise levels in the ICU, anticipated to have adverse effects on both patients and staff, by implementing enhancements in acoustic design. Background: Recognizing ICU noise as a significant disruptor of sleep and a potential hindrance to patient recovery, this study was conducted at a 40-bed ICU in Fiona Stanley Hospital in Perth, Australia. Methods: A comprehensive mixed-methods approach was employed, encompassing surveys, site analysis, and acoustic measurements. Survey data highlighted the importance of patient sleep quality, emphasizing the negative impact of noise on work performance, patient connection, and job satisfaction. Room acoustics analysis revealed noise levels ranging from 60 to 90 dB(A) in the presence of patients, surpassing sleep disruption criteria. Results: Utilizing an iterative 3D design modeling process, the study simulated significant acoustic treatment upgrades. The design integrated effective acoustic treatments within patient rooms, aiming to reduce noise levels and minimize transmission to adjacent areas. Rigorous evaluation using industry-standard acoustic software highlights the design's efficacy in reducing noise transmission in particular. Additionally, cost implications were examined, comparing standard ICU construction with acoustically treated options for new construction and refurbishment projects. Conclusions: This study provides valuable insights into design-based solutions for addressing noise-related challenges in the ICU. While the focus is on improving the acoustic environment by reducing noise levels and minimizing transmission to adjacent areas. It is important to clarify that direct measurements of patient outcomes were not conducted. Young citizens growing up in different societies experience multiple socialization processes that help to shape their values and attitudes toward the political life of their societies. In this cross-national study, researchers asked students directly about their views of what "good" citizens do, how they saw themselves participating in their political communities in the future, and what their attitudes were to rights for different groups in the community. This article highlights the conceptions of citizenship that students, as young citizens, have in three societies--Australia, Hong Kong, and the United States. The authors are specifically interested in how students in these three societies, with distinctly different histories and cultures, viewed citizenship at a time when the global community was increasingly interconnected but in a context of strong nation-states. In particular, how young people in these three societies conceived the rights and responsibilities of a good citizen in contexts that were globally connected but politically separated. For individuals with tetraplegia, regaining upper limb function forms the highest priority for improving quality of life. Use of nerve transfers to reconstruct upper limb function is increasing, however little is known about individual's decision to have and experience of the surgery and associated rehabilitation outcomes. This qualitative study aimed to understand the experience of surgery on the lives of individuals with tetraplegia 18 months post-surgery. In-depth, semi-structured interviews were conducted with five purposively selected individuals who have undergone upper limb nerve transfers at a metropolitan health service, Melbourne, Australia, specializing in spinal cord injury rehabilitation. Collaizi's phenomenological framework guided data analysis, resulting in an essence statement describing the individuals' experience. An essence statement comprising three themes; Deciding on Surgery, Facing Challenges: Surgery to Recovery and Evaluating Surgical Outcomes, was developed. The study suggests that for individuals with tetraplegia, hope to regain lost upper limb function forms a core consideration in the decision to have surgery. For clinicians supporting patient's decision, balancing hope with the realities of surgery is important. Even small changes in upper limb function had an important influence on participant's confidence in social situations through enhanced participation in a range of everyday activities. In making a decision to have surgery, individuals with tetraplegia benefit from two way discussions with the healthcare team and others who have already undergone surgery. Healthcare teams need to help prepare individuals for the challenges of surgery including: expectations of pain, hospital stay, initial loss of independence and the time it may take to see re-innervation of target muscles and subsequent functional changes. Surgery should be routinely considered as individuals' report that even small changes in upper limb function positively increases participation in everyday tasks and confidence in social situations. This study presents estimates of multidimensional household inequality in Australia from 2001 to 2017. Earnings inequality declines through the sample, while disposable income, non-durable consumption expenditures, food expenditures and net worth inequality exhibit relatively flat trends. The relatively flat trend for non-durable expenditures inequality, even over the life cycle, suggests households insure consumption against idiosyncratic shocks. Standard regression estimates of consumption growth against income shocks confirm this finding. Quantile regression estimates indicate households experiencing negative (positive) consumption growth have more sensitivity to negative (positive) income shocks than households with positive (negative) consumption growth, but coefficient estimates have small magnitudes, confirming standard tests. This paper assesses the performance of momentum strategies in Australia, and how they were affected by the GFC. This paper is the first to address the issue of the dollar capacity of momentum strategies in the Australian market. We find evidence of a strong momentum effect in Australia amongst the S&P/ ASX200 constituents. We find that momentum portfolios suffered during the GFC, but the effect was not persistent. Finally, we show that the capacity of the momentum effect in Australia is large enough in dollar terms to reject the assertion that momentum is more of a theoretical than a practical construct. The factors leading individuals to immigrate to developed contexts are widely studied, but comparatively less is known about those who emigrate from them. In this paper, we use data from a nationally representative cohort of Australian adults to develop longitudinal measures of emigration and to assess how social ties and individual economic position predict emigration. Cox proportional hazards models indicate that the propensity to emigrate is particularly pronounced for those with relatively little social connectedness in Australia. Specifically, our results show that first-generation Australians, especially those with relatively short durations in the country, have substantially higher emigration rates than later-generation Australians. Similarly, having a partner with deeper generational roots in Australia strongly reduces the likelihood to emigrate. At the same time, our analysis also shows that economic position matters, with the not employed having higher risks of emigration. Perhaps most interestingly, estimates from our models reveal that those with university degrees are much more likely to emigrate than individuals with lower levels of education, a finding that is true for both first- and later-generation Australians. Atkinson et al. (J. Econ. Lit. 49( 1):3-71, 2011) survey an important new literature using income-tax-based data to measure the share of income held by top income groups. But changes in tax legislation that expand the tax base to include income sources (e.g. capital gains, dividends, etc.) disproportionately held by these groups will conflate such an expansion with an increase in the share of income they hold. We provide a cautionary tale from Australia of how comprehensive tax reform legislation in 1985 substantially altered Australian top income series, especially those that do not separate taxable realized capital gains from other taxable income. Drawing on the Household, Income and Labour Dynamics in Australia (HILDA) Survey we then estimate the size and distribution (across income groups) of taxable realized capital gains in 2006 and 2009, and compare these results with those using accrued capital gains, finding substantially different distributions. More importantly, we find substantial differences across our measures in how capital gains changed between 2006 and 2009. Our results suggest that yearly taxable realized capital gains, often included in studies of top incomes, might be a poor proxy for the theoretically more appropriate yearly accrued capital gains. Objective: The objective of this study was to identify factors associated with poor dietary intake (less than four servings of fruit and vegetables daily) in a large nationally representative sample of adults with psychotic disorders. Methods: The sample comprised 1286 adults aged 18-64 years who took part in the second Australian national survey of psychosis. Dietary information was obtained using a standardised questionnaire; all participants provided fasting blood samples. Variables that may be related to diet and nutritional intake were investigated; these included demographics, physical health outcomes, physical activity, substance use, symptom severity and financial difficulty. Dietary status was explored by sex, age and body mass index using univariate analyses, while a multivariate analysis was performed to identify predictors of low nutritional intake. Results: Approximately 74% of participants ate less than four servings of fruit and vegetables daily. This was associated with a lower body mass index (p<0.05), lower levels of physical activity (p<0.05), sedentary behaviour (p<0.05), substance use (p<0.001), more negative symptoms (p<0.05), eating less frequently (p<0.001), consuming whole fat milk compared to low fat milk (p<0.05), adding salt to food (p<0.05) and financial difficulty (p<0.05). Male sex and younger age (18–34 years) were also associated with lower fruit and vegetable intake (p<0.001). A multivariate regression analysis showed that current smoking (p<0.001) and alcohol (p<0.01) and cannabis abuse (p<0.05) were risk factors for lower fruit and vegetable intake. Conclusion: The findings suggest that poor diet in people with psychosis, as reflected by less than four servings of fruit and vegetables daily, is accompanied by other unhealthy behaviours, which has important implications for the development of effective interventions. Aim This report describes the results from the last international prevalence measurement of care problems in the Netherlands, Austria and Switzerland, including the course of the prevalence rates during the past 4 years. Background Basic care problems such as pressure ulcers, malnutrition and falls occur frequently in healthcare organizations. Measuring these care problems provides insight into their occurrence, and, while a measurement is included of the prevention, treatment and structural quality indicators, this gives institutions the possibility of improving their care regarding these care problems. Design An annual cross-sectional multicentre study. Method The prevalence measurement of care problems is conducted annually on one specific day in different healthcare settings, among which are hospitals and care homes. Data are collected by means of a comprehensive, standardized questionnaire that comprises three levels: institutional, ward/department and patient level. Results Besides general characteristics of patients, results are presented for prevalence rates, prevention, treatment and quality indicators regarding each care problem for each country. Objective The Royal Perth Hospital ( RPH; Perth, Australia) has been the receiving facility for burns patients in two separate disasters. In 2002, RPH received 28 severely injured burns patients after the Bali bombing, and in 2009 RPH received 23 significantly burnt patients as a result of an explosion on board a foreign vessel in the remote Ashmore Reef Islands (840 km west of Darwin). The aim of this paper is to identify the interventions developed following the Bali bombing in 2002 and review their effectiveness of their implementation in the subsequent burns disaster. Methods A comparative case study analysis using a standardised approach was used to describe context with debrief reports and ED photographs from both disasters used for evaluation. Results The implementation of regular ED disaster response planning and training, early Code Brown notification of the entire hospital with regular updates, early clearing of inpatient beds, use of Short Message Service to communicate regularly with ED staff, control of the public and media access to the ED, visual identification of staff within the ED, early panendoscopy to ascertain intubation needs, and senior clinical decision makers in all areas of the ED were all acknowledged as effective based on the debrief reports. There was a reduction in ED length of stay (150 to 55 min) and no deaths occurred; however, quantitative analysis can only be suggestive rather than a direct measure of improvement given the likelihood of other system changes. In situ measurements of soil moisture are invaluable for calibrating and validating land surface models and satellite-based soil moisture retrievals. In addition, long-term time series of in situ soil moisture measurements themselves can reveal trends in the water cycle related to climate or land cover change. Nevertheless, on a worldwide basis the number of meteorological networks and stations measuring soil moisture, in particular on a continuous basis, is still limited and the data they provide lack standardization of technique and protocol. To overcome many of these limitations, the International Soil Moisture Network (ISMN; http://www.ipf.tuwien.ac.at/insitu) was initiated to serve as a centralized data hosting facility where globally available in situ soil moisture measurements from operational networks and validation campaigns are collected, harmonized, and made available to users. Data collecting networks share their soil moisture datasets with the ISMN on a voluntary and no-cost basis. Incoming soil moisture data are automatically transformed into common volumetric soil moisture units and checked for outliers and implausible values. Apart from soil water measurements from different depths, important metadata and meteorological variables (e.g., precipitation and soil temperature) are stored in the database. These will assist the user in correctly interpreting the soil moisture data. The database is queried through a graphical user interface while output of data selected for download is provided according to common standards for data and metadata. Currently (status May 2011), the ISMN contains data of 19 networks and more than 500 stations located in North America, Europe, Asia, and Australia. The time period spanned by the entire database runs from 1952 until the present, although most datasets have originated during the last decade. The database is rapidly expanding, which means that both the number of stations and the time period covered by the existing stations are still growing. Hence, it will become an increasingly important resource for validating and improving satellite-derived soil moisture products and studying climate related trends. Abstract: Gradients in the sediment fauna comprising groundwater (GW) and hyporheic taxa were investigated in the sand/silt-bottomed Marbling Brook in Western Australia. The structure of sediment invertebrate assemblages from Marbling Brook sediments and the adjacent GW were studied at five sites over 1 year and hydrological interactions were characterized using a suite of abiotic factors. Although all five stream sites were upwelling, the sites differed in the degree of hydrological interactions between GW and surface water. Sediment fauna taxa abundances were not correlated with any of the abiotic factors investigated and did not change gradually with depth. Faunal assemblages in the stream sediments were distinct from faunal assemblages in alluvial GW. While water exchanged between alluvial GW and sediment water, as shown by abiotic factors, the distinct differences in faunal assemblages indicated an unpredicted complexity in the catchment with fundamentally different hydrogeological situations on the decimetre scale. Sampling in sandy sediments needs to take this small-scale variability into account. [Copyright &y& Elsevier]

Older people form a large and growing segment of our population, experience disproportionately more illness and require more use of health services than any other group. This differential is largely due to falls, which are the leading cause of injury for those aged 65 plus. The North Coast ‘Stay On Your Feet’ programme is a 4-year multi-strategic, community-based intervention to address this problem among 80000 older residents. This paper presents key results of the first 18 months of the programme. It demonstrates potential achievements of this type of intervention and examines some barriers. Programme effect was measured quasi-experimentally by monitoring indicators of awareness, knowledge, attitudes and risk factors via a telephone survey with random cohorts in intervention and control areas. After allowing for baseline covariates, the intervention was significantly associated with: raised awareness both of the problem of falling and its preventability; improved knowledge of the risk factors for falling; and a higher self-rated risk of falling. As expected, there is as yet no population change in falls rate. Initial changes shown in risk factors for falling raise interesting challenges. A reduction in physical activity may indicate that older people, now more aware of risk, are being advised to restrict their activities. An increase in proportion of older people taking medications which may cause unsteadiness also presents a challenge. However, a concomitant decrease in reported dizziness may indicate that medications are now better managed. This evaluation shows that in 18 months, a well-funded and managed community-based falls prevention programme can achieve changes in awareness, knowledge and attitudes but that continued intervention is required to substantially change behavioural risk factor profiles and the likelihood of an older person falling. SUMMARY An imaging protocol with a multi-slice CT scanner that allows comprehensive assessment of patients with haematuria is described. This protocol allows evaluation of the kidneys, ureters and bladder in a single examination using CT. This approach should streamline the diagnostic work-up of patients with haematuria. Objectives: To characterize the specific autonomic disturbances underlying the cold-induced sweating syndrome (CISS), and to describe a novel genetic variant of this rare recessive disorder. The two not previously reported patients had similar dysmorphic features: abnormal facial appearance, high arched palate, low set rotated ears, flexion deformities of elbows and fingers and scoliosis. Most noticeable were their paradoxical sweat responses: cold ambient temperature induced a profuse sweating over the face, arms and trunk but not over the lower limbs; while in the heat very little sweating occurred primarily on the legs. Testing of autonomic functions demonstrated normal cardiovascular reflexes and postganglionic sympathetic efferent functions. Sural nerve morphology and number of unmyelinated fibers was normal and skin biopsies showed normal appearing eccrine sweat glands. MRI scans revealed no structural brain abnormalities. Oral clonidine, prescribed in one patient, completely suppressed cold-induced sweating. Observed clinical features matched those of two sisters reported from Israel and of two brothers reported from Norway. All six cases presented a similar phenotype. The Norwegian, Israeli and Canadian cases were homozygous or compound heterozygous, respectively, for mutations in the CRLF1 gene on chromosome 19p12 (CISS1). The Australian case, however, had no pathogenic sequence variants in the CRLF1 gene, but was compound heterozygous for mutations in the CLCF1 gene on chromosome 11q13.3 (CISS2)., Conclusion: The rare cold-induced sweating syndrome is genetically heterogeneous and is probably caused by central and peripheral impairment of sudomotor functions. This is the first detailed report on the clinical consequences of mutations in the CLCF1 gene in humans. Directions for medical therapies are outlined to achieve long term symptom control.
