Background Assessment for vitamin C deficiency (VCD) is rarely undertaken in an acute hospital setting in high-income countries. However, with growing interest in VCD in community settings, there is emerging evidence investigating the prevalence and impact of VCD during hospitalization. Objectives In this scoping review, the prevalence of VCD in adult hospitalized patients is explored, patient characteristics are described, and risk factors and clinical outcomes associated with VCD are identified. Methods A systematic scoping review was conducted in accordance with the PRISMA-ScR framework. The Ovid MEDLINE, Ovid Embase, Scopus, CINAHL Plus, Allied and Complementary Medicine Database, and the Cochrane Library databases were searched for interventional, comparative, and case-series studies that met eligibility criteria, including adult hospital inpatients in high-income countries, as defined by the Organization for Economic Co-operation and Development, that reported VCD prevalence using World Health Organization reference standards. These standards define VCD deficiency as plasma or serum vitamin C level <11.4 µmol/L, wholeblood level <17 µmol/L, or leukocytes <57 nmol/108 cells. Results Twenty-three articles were included, representing 22 studies. The cumulative prevalence of VCD was 27.7% (n = 2494; 95% confidence interval [CI], 21.3-34.0). High prevalence of VCD was observed in patients with severe acute illness and poor nutritional status. Scurvy was present in 48% to 62% of patients with VCD assessed in 2 studies (n = 71). Being retired (P  = 0.015) and using excessive amounts of alcohol and tobacco (P  = 0.0003) were independent risk factors for VCD (n = 184). Age was not conclusively associated with VCD (n = 631). Two studies examined nutrition associations (n = 309); results were inconsistent. Clinical outcomes for VCD included increased risk of frailty (adjusted odds ratio, 4.3; 95%CI, 1.33–13.86; P  = 0.015) and cognitive impairment (adjusted odds ratio, 2.93; 95%CI, 1.05–8.19, P  = 0.031) (n = 160). Conclusions VCD is a nutritional challenge facing the healthcare systems of high-income countries. Research focused on early identification and treatment of patients with VCD is warranted. Summary: Climate change is threatening our current food systems and countries around the world are seeking alternative food sources to safeguard against potential food insecurity and nutrient deficiency concerns. Various traditional foods used by Aboriginal groups of Australia have previously been shown to be highly nutritious, contain beneficial functional properties and can thrive in the harsh climatic conditions found across Australia. As interest in these traditional foods grows, Aboriginal groups are looking at developing a range of foods for modern markets. However, for many of these foods, there is still little known about their nutritional and functional properties. In this study, we report the nutrition composition and antioxidant properties of a native Australian grain species that is currently being developed for commercial markets by a collective of Aboriginal Traditional Custodians and compare its properties to a commonly consumed wheat variety. Compared with wheat, the native grain species contained 2‐fold greater protein and total fats, and higher levels of essential minerals and trace elements, including 8‐fold iron levels and >2.5‐fold calcium, magnesium, zinc, copper and manganese levels. Functionally, the native grain contained 2.4‐fold greater polyphenol content and displayed greater antioxidant potential in exposed human monocyte cultures. Importantly, the native grain was also found to contain a very low gluten content. Altogether, this shows that the native grain is nutritionally and functionally superior to commonly consumed wheat for the measured parameters and could potentially serve as a grain alternative or be used to fortify current wheat‐based and gluten‐free products. Background and aims: Proteaceae are a prominent plant family in south-western Australia. Most Proteaceae are 'calcifuge', occurring exclusively on old phosphorus (P)-impoverished acidic soils, with a few 'soil-indifferent' species also found on young P-richer calcareous soils. Calcium (Ca)-enhanced P toxicity explains the calcifuge habit of Proteaceae. However, previous research has so far been focused exclusively on the roles of Ca and P in determining Proteaceae distribution, and consequently there is little knowledge on how other soil-based strategies influence this distribution. We aimed to study the effects of young calcareous soils on four soil-grown Proteaceae and assess differences between calcifuge and soil-indifferent Proteaceae to better understand their natural distribution. Methods: Two calcifuge and two soil-indifferent Proteaceae from south-western Australia were grown in six contrasting soils, including young calcareous, and old acidic soils. Results: When grown in calcareous soils all species showed root growth inhibition, micronutrient deficiency, Ca-enhanced P toxicity, and negative impacts on physiology. Calcifuge species were more sensitive to calcareous soils than soil-indifferent ones, although this varied between genera. Soil-indifferent species tended to produce more cluster roots, release more carboxylates per root mass, and allocate less Ca to their leaves, compared with calcifuges; they also had smaller seeds and were less sensitive to Ca-enhanced P toxicity. Conclusion: We surmise that a combination of these traits allows soil-indifferent species to tolerate calcareous soils. Background: Pulses are cultivated across a range of soil and climatic conditions that often have many abiotic constraints to production. Whilst water stress has been identified as the greatest constraint in Australian pulse cultivation, other limitations include heat, frost, nutrient deficiency, and a variety of soil physiochemical constraints. Several studies on various pulses have examined shoot and root architecture and their ability to mitigate specific abiotic constraints. However, these studies have been conducted independently of each other, and there is a lack of amalgamated information combining both shoot and root responses that can address production constraints. Scope: This review examines the shoot and root system architecture of key pulses used for human consumption grown in Australia including chickpea, lentil, faba bean, field pea and lupin; and where possible it provides a comparison with relevant research from other crops, especially cereals. It examines research conducted on adaptations to drought, waterlogging, temperature extremes, soil chemical toxicities and high soil strength. Conclusion: The review utilises a physiological framework to identify trait combinations that define theoretical ideotypes of pulse crops that would be better able to mitigate abiotic constraints currently limiting Australian pulse productivity. This framework can be extended directly to other similar environments globally or be used to develop new ideotypes that are better adapted to a wider range of regions within Australia. Agronomic biofortification of crops is a promising approach that can improve the nutritional value of staple foods by alleviating dietary micronutrient deficiencies. Iodine deficiency is prevalent in many countries, including Australia, but it is not clear what foliar application strategies will be effective for iodine fortification of grain. This study hypothesised that combining adjuvants with iodine in foliar sprays would improve iodine penetration in wheat, leading to more efficient biofortification of grains. The glasshouse experiment included a total of nine treatments, including three reference controls: 1) Water; 2) potassium iodate (KIO3) and 3) potassium chloride (KCl); and a series of six different non-ionic surfactant or oil-based adjuvants: 4) KIO3 + BS1000; 5) KIO3 + Pulse® Penetrant; 6) KIO3 + Uptake®; 7) KIO3 + Hot-Up®; 8) KIO3 + Hasten® and 9) KIO3 + Synerterol® Horti Oil. Wheat was treated at heading, and again during the early milk growth stage. Adding the organosilicon-based adjuvant (Pulse®) to the spray formulation resulted in a significant increase in grain loading of iodine to 1269 µg/kg compared to the non-adjuvant KIO3 control at 231µg/kg, and the water and KCl controls (both 51µg/kg). The second most effective adjuvant was Synerterol® Horti Oil, which increased grain iodine significantly to 450µg/kg. The Uptake®, BS1000, Hasten®, and Hot-Up® adjuvants did not affect grain iodine concentrations relative to the KIO3 control. Importantly, iodine application and the subsequent increase in grain iodine had no significant effects on biomass production and grain yield relative to the controls. These results indicate that adjuvants can play an important role in agronomic biofortification practices, and organosilicon-based products have a great potential to enhance foliar penetration resulting in a higher translocation rate of foliar-applied iodine to grains, which is required to increase the iodine density of staple grains effectively. Nutrition is recognized as one of the leading factors influencing the growing incidence of noncommunicable diseases. Despite society experiencing a global rise in obesity, specific populations remain at risk of nutrient deficiencies. The food industry can use health claims to inform consumers about the health benefits of foods through labeling and the broader promotion of specific food products. As health claims are carefully regulated in many countries, their use is limited due to considerable investments required to fulfill the regulatory requirement. Although health claims represent a driving force for innovation in the food industry, the risk of misleading of consumers need to be avoided. The health claim scientific substantiation process must be efficient and transparent in order to meet the needs of companies in the global market, but should be based on strong scientific evidence and plausible mechanisms of actions, to ensure highest level of consumer protection. The objective of this review is to compare the possibilities for using health claims on foods in the European Union, the USA, Canada, and Australia and New Zealand. In particular, we focused on differences in the classification of claims, on the scientific substantiation processes and requirements for health claims use on foods in the selected regions. Reduction of disease risk (RDR) claims are associated with relatively similar procedures and conditions for use, whereas several notable differences were identified for other types of claims. In all cases, RDR claims must be approved prior their introduction to the market, and only a few such claims have been authorized. Nutrient deficiencies are considered a reason for commercial yields of wheat (Triticum aestivum L.) and canola (Brassica napus L.) in the high-rainfall zone (HRZ) of southern Australia being well below predicted potential yields. With the aim of developing soil-test interpretation guidelines suitable for HRZ conditions, nutrient-response experiments, 15 with wheat and 12 with canola, were conducted between 2015 and 2018. These experiments quantified responses to nitrogen (N), phosphorus (P), potassium (K), sulfur (S), copper (Cu) and zinc (Zn) in pre-sowing soil tests. The highest yielding treatment of the wheat experiments averaged 7.1 t/ha (range 2.6–10.8 t/ha), and of the canola experiments 4.2 t/ha (range 0.7–6.2 t/ha). The most frequent responses were to N and P, followed by S and K. There were no significant positive responses to Cu or Zn. Across the experiments, the 95% critical value for Colwell P in wheat was 52 mg/kg, with a 95% confidence range of 39–68 mg/kg. For canola, the critical value was 59 mg/kg, with a range of 38–139 mg/kg. These values are higher than from lower rainfall regions of Australia. Critical values for K and S were also higher than from drier regions of Australia. The Sprengel–Lieberg Law of the Minimum overestimated yield where there were multiple nutrient limitations, whereas an equivalent Law of the Product underestimated yield under these conditions. These higher critical values based on evidence from the HRZ are expected to assist in closing the yield gap for wheat and canola in the region. We used a series of 27 nutrient-response experiments to derive soil-test critical values for wheat and canola in the high-rainfall zone of southern Australia. Applying lime to ameliorate soil acidity has been observed to induce manganese (Mn) deficiency in canola (Brassica napusL.) crops grown on acid sandy soils near Albany and gravelly acid sands of the Great Southern Districts of southwestern Australia. These soils were often Mn-deficient in patches for wheat (Triticum aestivumL.) production when they were newly cleared for agriculture requiring application of Mn fertilizer to ensure grain yields were not reduced by the deficiency. Since then, these soils have acidified and in the 1990s, canola started to be grown on these soils in rotation with wheat and lupins (Lupinus angustifoliusL.). These limed soils may now have become marginal to deficient in Mn for canola production. The effect of liming may change the effectiveness of fertilizer Mn. In addition, the effect of liming on the residual value of Mn fertilizer applied to these soils for canola production is unknown. Therefore, a glasshouse experiment was conducted using Mn deficient sand. Three levels of finely-powdered calcium carbonate were added and incubated in moist soil for 42 days at 22±2°C to produce 3 soils with different pH values [1:5 soil:0.01 M calcium chloride (CaCl2)]: 4.9 (original soil), 6.3, and 7.5. Five Mn levels, as solutions of Mn sulfate, were then added and incubated in moist soil for 0, 50, and 100 days before sowing canola. To estimate the residual value (RV) of incubated Mn for canola production, the effectiveness of the incubated Mn was calculated relative to the effectiveness of Mn applied just before sowing canola (freshly-applied Mn). The RV of the incubated Mn was determined using yield of dried canola shoots, the Mn application level required to produce 90% of the maximum shoot yield, and Mn content in dried shoots (Mn concentration in shoots multiplied by yield of dried shoots). As measured using both yield of dried shoots and Mn content of dried shoots, the residual value of Mn decreased with increasing soil pH and with increasing period of incubation of Mn with moist soil. The critical Mn concentration, for 90% of the total yield of dried canola shoots, was (mg Mn kg−1) ∼17 in youngest mature growth (apex and youngest emerged leaf, YMG), and ∼22 for the rest of dried shoots. These values were similar to current critical values for un-limed soils suggesting critical Mn concentrations remain the same for limed soils. Plant testing of canola is recommended if soils are to be limed to ameliorate soil acidity. When plant tests indicate a high likelihood of Mn deficiency, foliar Mn sprays need to be applied to that crop to ensure Mn deficiency does not reduce grain production that year, and fertilizer Mn needs to be re-applied to the soil when sowing the next crop to reduce the likelihood of Mn deficiency for subsequent crops. Abstract: In Australia, 60% of children (9-16 years) are not meeting the estimated average requirement (EAR) for calcium, and milk is an important dietary source of calcium. We hypothesized that milk drinking (plain or flavored) would be associated with higher milk and nutrient intake among Australian children. Nutrient intakes reported via 24-hour recalls and anthropometric measurements collected from the 2007 Australian National Children''s Nutrition and Physical Activity Survey were analyzed. Children and adolescents (cross sectional, n = 4487, 2-16 years) were classified into 4 categories according to milk intake: (i) exclusively plain milk drinkers, (ii) flavored (flavored and plain) milk drinkers, (iii) nondrinkers of milk (no plain or flavored milk as beverage), and (iv) nonconsumers of milk (total milk intake is 0 g). Total daily milk, energy and nutrient intake, body mass index, and waist circumference were compared between categories of milk drinking by analysis of variance models. Results of this study show that milk drinkers (plain or flavored) had a significantly higher total milk intake than did nondrinkers of milk. Milk drinking was associated with higher intakes of calcium, phosphorus, magnesium, potassium, and iodine in comparison with nondrinkers of milk and nonconsumers of milk (P < .05). Milk drinkers (exclusively plain or flavored) were more likely to meet the EAR for calcium than nondrinkers and nonconsumers of milk, whereas flavored milk drinkers were more likely to meet the EAR for calcium than exclusively plain milk drinkers. In particular, among children aged 9 to 16 years, flavored milk drinkers were 1.7 times more likely to meet the EAR for calcium than plain milk drinkers (P < .001). Although flavored milk drinkers had higher total sugar and energy intakes than did exclusively plain milk drinkers and nonconsumers of milk (P < .05), no between-group differences were observed in body mass index, waist circumference, or physical activity level. In conclusion, drinking milk was associated with higher total milk and micronutrient intake, and drinking both plain and flavored milk may help children meet their calcium requirements without negatively impacting body weight measures. To compare the dietary intake and nutritional status of users and non-users of proton pump inhibitor medications. Cross-sectional study of subacute rehabilitation inpatients. Forty-nine patients (nine men and 40 women) without dementia fulfilled the study criteria. Information was collected on admission and included medical history, duration of proton pump inhibitor medication use, nutritional status and cognitive scores. Dietary data were collected by food frequency questionnaire and blood samples were analysed for vitamin B, homocysteine and methylmalonic acid concentrations. Age was 80.4 ± 7.7 (mean ± SD) years and body mass index was 26 ± 6.7 kg/m. Twenty-one (of 49) subjects had subclinical vitamin B deficiency, which was defined as serum vitamin B <148 pmol/L or 148-258 pmol/L and methylmalonic acid >0.30 µmol/L or tHcy >13 µmol/L (women) and >15 µmol/L (men). Subjects were stratified according to proton pump inhibitor use and vitamin B status. The presence of subclinical deficiency was similar between the groups (χ-test P= 0.17). Proton pump inhibitor users had higher dietary protein and calcium intakes (but not supplement calcium intakes) compared with non-users (93 vs 81 g/day, P= 0.002 and 968 vs 742 mg/day, P= 0.038, respectively). Subjects using proton pump inhibitor medications did not have lower vitamin B status, but had higher dietary protein intakes suggesting higher intakes of meats, eggs and dairy foods may reduce the risk of developing vitamin B deficiency whilst taking proton pump inhibitor medications. Although the adverse health consequences of zinc deficiency are becoming increasingly well known internationally, few studies have investigated zinc status in Australia and New Zealand. The aim of this review was to identify which population groups, if any, in Australia and New Zealand are likely to be at risk of zinc deficiency. A literature review was conducted to identify papers on zinc status in Australasia using electronic databases (MEDLINE, BIOSIS Previews, CINAHL, EMBASE, Web of Knowledge, ProQuest and Cochrane Library). Government databases and reference lists from peer-reviewed journal articles were also accessed. Thirty-one Australasian studies that measured dietary zinc intake and/or serum zinc were identified. Many were based on a small number of self-selected subjects, and did not use the recommended procedures for evaluating the prevalence of inadequate zinc intakes and low serum zinc concentrations. Existing data suggest that at-risk groups in Australasia are toddlers, adolescents (especially those of Pacific and Aboriginal ethnicities), institutionalised elderly and possibly people with diabetes. Data on the zinc status of selected population groups in Australasia are limited. Randomised controlled zinc supplementation trials that include the recommended indicators and other clinical outcomes are needed to confirm the existence of zinc-related health outcomes among the at-risk groups identified. Abstract: Most mutations related to aberrant splicing occur in conserved splice acceptor and donor sites. Some exonic mutations also affect splicing. We identified and characterized a point mutation (c.1124A>G) in an Australian patient (GK43) with mitochondrial acetoacetyl-CoA thiolase (T2) deficiency. GK43 is a homozygote of c.1124A>G, which activates a cryptic splice donor site 5 bases upstream from c.1124A>G within exon 11, causing aberrant splicing in most transcripts. The aberrant splicing results in c.1120–1163 (44-base) deletion, causing a frameshift in T2 mRNA. A mini-gene splicing experiment confirmed that the c.1124A>G substitution was responsible for this aberrant splicing. This cryptic splice site has a Shapiro and Senapathy score (70.0) in a normal sequence but if mutated, the score (84.3) becomes higher than the one in the authentic splice donor site of intron 11 (81.4). This is an example in which a point mutation activates a cryptic splice donor site motif that is used preferentially over a downstream authentic splice site. [Copyright &y& Elsevier]

Phosphorus (P) is an important determinant of plant productivity, particularly in the tropical grasslands of Australia, which contain both C3 and C4 species. Few studies have compared the responses of such species to P deficiency. Previous work led us to hypothesise that C3 photosynthesis and the three subtypes of C4 photosynthesis have different sensitivities to P deficiency. To examine their dynamic response to P deficiency in more detail, four taxonomically related tropical grasses (Panicum laxum (C3) and Panicum coloratum, Cenchrus ciliaris and Panicum maximum belonging to the C4 subtypes NAD-ME, NADP-ME and PCK, respectively) were grown under contrasting P supplies, including P withdrawal from the growing medium. Changes in photosynthesis and growth were compared with leaf carbohydrate contents and metabolic fingerprints obtained using high-resolution proton nuclear magnetic resonance (1H-NMR). The response of CO2 assimilation rates to leaf contents of inorganic phosphate ([Pi]) was linear in the C3 grass, but asymptotic for the three C4 grasses. Relative growth rate was affected most by low P in the C3 species and was correlated with the leaf content of glucose 6-phosphate more than with carbohydrates. Principal component analysis of the 1H-NMR spectra revealed distinctive profiles of carbohydrates and amino acids for the four species. Overall, the data showed that photosynthesis of the three C4 subtypes behaved similarly. Compared with the C3 counterpart, photosynthesis of the three C4 grasses had a higher P use efficiency and lower Pi requirement, and responded to a narrower range of [Pi]. Although each of the four grass species showed distinctive 1H-NMR fingerprints, there were no differences in response that could be attributed to the C4 subtypes. [ABSTRACT FROM AUTHOR]

Background: An adequate total body pool of ascorbate is essential for optimum health in humans. Requirements for ascorbate are increased in peritoneal dialysis (PD) patients most likely due to a combination of poor nutrition and increased dialysate losses. Methods: We measured serum ascorbate levels in 45 clinically stable PD patients to assess the prevalence of ascorbate insufficiency (level between 2 and 4 mg/L) and deficiency (level <2 mg/L). We also assessed the efficacy of subsequent supplementation and patients' adherence to the prescribed supplementation. All patients were advised on commencement of dialysis to take a multivitamin tablet containing 100–120 mg ascorbate. Results: Eighteen (41%) PD patients were regularly taking ascorbate-containing multivitamins, while 27 (59%) patients did not take ascorbate supplements. Serum ascorbate levels ranged from <0.2 to 41 mg/L, with wide variations in serum ascorbate at any given intake level. Ascorbate deficiency was present in 1/3 of the current PD population (44% of patients not taking supplements and in 16% of those on supplements), although none of the patients demonstrated clinical manifestations of scurvy. Targeted supplementation of ascorbate insufficient patients increased the median serum ascorbate level from 1.7 mg/L (IQR 1.2–2.2) to 22.5 mg/L (IQR 16.7–32.9). Conclusion: Our results show that, in PD patients, ascorbate deficiency is common and can readily be identified with serum ascorbate measurements. Oral supplements in the form of inexpensive multivitamin preparations restore adequate serum ascorbate levels in the majority of these patients. We therefore suggest measurement of ascorbate levels in all PD patients at the commencement of dialysis with a target level in the normal range (4–14 mg/L). [ABSTRACT FROM AUTHOR]

The aim of this study was to determine the incidence of lactose malabsorption in healthy, adult Australian Aborigines. Malabsorption of lactose was measured in 45 full-blood Aboriginal subjects and 37 non-Aboriginal, multiracial controls using the breath hydrogen method. Eighty-four percent of the Aboriginal subjects were found to be lactose malabsorbers and 64% developed abdominal pain or diarrhea. In the control subjects, 20% were found to be lactose malabsorbers and all of these developed symptoms of diarrhea. The results provide strong evidence that Australian Aborigines, in common with most human adults, are lactase deficient.
