One-hundred Polish soldiers of a contingent in Afghanistan in 2019 were screened for Enterobacterales resistant to newer-generation β-lactams at their departure and return. Seventeen percent were colonized in the gut at the departure, whereas 70% acquired carriage in Afghanistan. The commonest organisms were extended-spectrum β-lactamase (ESBL)-producing Escherichia coli (ESBL-Ec; 96.6%). All isolates were sequenced and were clonally diverse overall, even within the same sequence type, indicating that independent acquisitions mainly. ESBL-Ec were often multi-drug-resistant. Soldiers stationing in certain regions are at high risk of acquiring resistant bacteria that may cause endogenous infection, be transmitted to vulnerable individuals, and spread resistance genes. Covering war conflicts may compromise the psychological and physical health of journalists because chronic exposure to these environments has been related to depression, memory dissociative processes, and post-traumatic stress disorder; however, acute effects have not been studied yet. Thus, a combat simulation was carried out replicating actual warfare scenarios, including personnel and equipment. Psychophysiological response, memory, and information-processing were analysed of 40 professional soldiers (21 males and 19 females) and 19 journalists (12 males and 7 females) with international experience in current conflict areas such as Afghanistan, Iraq, Bosnia and Herzegovina, and Kosovo, in relation to their experience of a combat intervention. A significant increase (p < 0.05) in metabolic, muscular, cardiovascular, and cortical and psychological anxiety response, as well as a decrease in memory accuracy directly after and 24 h and 72 h post-combat were found; these modifications were modulated by the nature of the stimulus. Journalists presented higher cognitive and memory impairment than soldiers, resulting in a press reporting of real events accuracy of only 27%. [ABSTRACT FROM AUTHOR]

Objective: To describe and characterize the lifetime history of traumatic brain injury (TBI) among active duty soldiers returning from deployment to Afghanistan or Iraq. Method: Data were extracted from a larger parent study that was conducted at two large United States Army bases between 2009 and 2014 during Post-Deployment Health Assessment. The sample included 1,060 soldiers who sustained at least one TBI during their lifetime. Results: The Ohio State University TBI-Identification Method interview was administered to characterize individuals' total lifetime history of TBI. Soldiers reported sustaining a median of 2 lifetime TBIs. Slightly more than half of the sample without a most recent deployment-related TBI still reported a history of a lifetime TBI (some of which occurred during previous deployments). Most lifetime injuries reported were of mild severity; however, 6% of individuals reported a history of moderate/severe TBI. Blast was the most frequent mechanism associated with recent deployment-related mild TBIs. Conclusion: Findings suggest that soldiers who screened positive, as well as those who screened negative, for a history of TBI during their recent deployment still endorsed a lifetime history of TBI. Future research is needed to explore the functional impact of multiple TBIs over one's lifetime to help inform screening, assessment, and treatment among military personnel. Although soldiers in this sample mostly reported mild traumatic brain injuries (TBIs), 6% of individuals reported lifetime moderate to severe TBIs. Moreover, a number of soldiers reported a history of nondeployment TBIs. A history of such injuries may contribute to persistent post-TBI symptoms, as well as postmilitary functional challenges. Humanitarian wars are a primary means of globally affirming a specific model of humanity, built according to the cultural, moral, and economic standards of Western democracies. How are forms of humanity produced in the context of humanitarian war in Afghanistan? How are notions of freedom mobilized? How does the idea of a prospective humanity relate to the use of military force? In an attempt to reflect on the different configurations of freedom and humanity that emerged in the context of recent Afghanistan conflicts and international interventions, this article addresses the perspective of two key figures: the Taliban and the humanitarian soldier. Building on narratives such as poetry, interviews, and conversations conducted during fieldwork, this angle allows us to observe the complexity of the Afghan humanitarian theatre in a way that goes beyond mere assessments of political and economic interests, revealing a fragment of global contemporaneity that is crucial for understanding how processes of producing humanity combine with war and humanitarian efforts. Background: Attentional bias (AB) to threat is thought to play a key role in the development and maintenance of posttraumatic stress symptomatology (PTS). Empirical evidence though is inconsistent. Some studies report associations between AB towards, threat and PTS; other studies report associations between AB away from threat and PTS; yet other studies fail to find any association. We propose that prospective-longitudinal study of AB as a dynamic process, expressed from moment to moment in time, may help to understand these mixed findings and the role of AB in PTS.Methods: We tested cross-sectional and prospective-longitudinal associations between AB and PTS among German soldiers from pre- to post-deployment in Afghanistan (n = 144). AB to threat and positive emotion stimuli (angry/happy faces) was measured using the dot-probe task. PTS was assessed by the PTSD Checklist. The number of traumatic experiences was assessed using CIDI-traumatic experience lists for military.Results: We found that AB dynamics (i.e., towards, away, temporal variability) at pre- and post-deployment, with respect to angry and happy faces, predicted higher levels of PTS after deployment as a function of number of intermediate traumatic experiences. Traditional aggregated mean bias scores did not similarly prospectively predict PTS post deployment.Conclusions: Findings indicate that AB to emotionally arousing stimuli may play an important function in the development and maintenance of PTS. We argue that mixed and null findings appear to be due to failure to model the within-subject temporal variability in AB expression. Objective: New trajectories of PTSD symptoms have recently been identified in war exposed army veterans. The aim of this army veterans study was to examine whether pre-deployment cognitive ability is associated with the risk of developing PTSD symptoms or non-resilient PTSD trajectories.Method: Follow up study in 428 Danish soldiers, deployed to Afghanistan in 2009, who were assessed at six occasions from pre-deployment to three years post-deployment. Pre-deployment vulnerabilities, deployment and homecoming stressors were measured. Pre-deployment cognitive test scores on Børge Priens Prøve (based on logical, verbal, numerical and spatial reasoning) were converted to a mean of 100 and with a standard deviation of 15.Results: Higher pre-deployment cognitive ability scores were associated with lower risk of PTSD symptoms as assessed by the Posttraumatic Stress Disorder Checklist-Civilian Version (PCL-C) 2.5 years post-deployment (OR=0.97; 95% CI 0.95-1.00) after adjustment for educational length, baseline PCL-C score and perceived war-zone stress. Compared to a resilient trajectory, a non-resilient relieved-worsening trajectory (high baseline mental symptoms, being symptom free during deployment and a drastic increase in PTSD symptoms at the final assessments of PTSD symptoms) had significantly lower cognitive scores by a mean difference of 14.5 (95% CI 4.7-24.3). This trajectory (n=9) comprised 26.5% of soldiers with moderate-severe PTSD symptoms 2.5 years post-deployment.Conclusion: We confirmed an inverse association between pre-deployment cognitive ability and risk of PTSD symptoms, and observed significantly lower mean pre-deployment cognitive scores in one non-resilient PTSD trajectory. Understanding the development of posttraumatic stress disorder (PTSD) implicates research regarding factors besides the preceding traumatic event. This study investigated the influence of predisposing personality traits on development of PTSD in a group of Danish Soldiers deployed to Afghanistan (N = 445). Using a prospective design data was collected using questionnaires including the NEO Five Factor Inventory and the Posttraumatic Stress Disorder Checklist. The results showed a PTSD-prevalence of 9.2% in the total sample 2.5 years after homecoming. Using Kruskal-Wallis, Mann-Whitney U, and Spearman's rho significant relationships were identified between pre-existing personality traits of neuroticism and agreeableness with development of PTSD symptoms 2.5 years after homecoming, however, a number of additional cofounders were identified. The article reports on research which was conducted to investigate the influence training frequency had on the perceived health status and fitness levels of deployed National Guard soldiers. Researchers evaluated 88 soldiers. They found that soldiers who trained three or more times per week experienced better fitness levels and perceived health during deployments.

Abstract: Although the DSM-IV recognizes that events can traumatize by evoking horror, not just fear, the role of disgust in the development of posttraumatic stress disorder (PTSD) has received little research attention. In a study of soldiers deployed to Afghanistan, we examined whether reports of peritraumatic disgust and trait disgust vulnerability factors (disgust propensity and disgust sensitivity) predict PTSD-symptoms, independently of peritraumatic fear, neuroticism, and anxiety sensitivity. Participants (N =174) enrolled in this study before deployment, and were retested around 6 months (N =138; 79%) and, again, 15 months (N =107; 62%) after returning home. The results showed that (1) greater peritraumatic disgust and fear independently predicted PTSD-symptom severity at 6 months, (2) greater disgust propensity predicted more peritraumatic disgust, but not PTSD-symptom severity, and (3) disgust sensitivity moderated the relationship between peritraumatic disgust and PTSD-symptom severity. Implications of these findings for broadening the affective vulnerabilities that may contribute to PTSD will be discussed. [Copyright &y& Elsevier]

Objectives: To analyse differences in uptake of the insecticide permethrin in wearers of non-impregnated and permethrin impregnated battle dress uniforms (BDU) in Afghanistan and Germany., Methods: In two separate studies, in April 2003-January 2004 (study I, n=549) and in February-April 2005 (study II, n=195), healthy female and male members of the German Federal Armed Forces were equipped with permethrin impregnated BDU (two sub-cohorts in Germany and one in Afghanistan) while members equipped with non-impregnated uniforms served as a control group. Human biomonitoring was conducted before, during and after wearing the uniforms by measuring permethrin metabolites in urine samples via GC-MS., Results: Subjects of the Afghan and German control groups had permethrin levels in the range of the German general population. In contrast, subjects wearing impregnated BDU had about 200-fold higher exposure levels. Within this group, subjects located in Afghanistan and smokers had significantly higher exposure levels. Internal exposure decreased with increasing duration of use of impregnated BDU., Conclusions: There is no evidence for a higher background permethrin contamination in military bases located in Afghanistan compared to Germany. Daily use of permethrin impregnated BDU is associated with significantly higher permethrin uptake compared to the general population. Hand-mouth contact by smoking can increase uptake which also seems to be influenced by the duration of use of impregnated BDU.
