Background: Falling on stairs is a major health hazard for older people. Risk factors for stair falls have been identified, but these are mostly examined in controlled biomechanics/gait laboratory environments, on experimental stairs with a given set of step dimensions. It remains unknown whether the conclusions drawn from these controlled environments would apply to the negotiation of other domestic staircases with different dimensions in real houses where people live., Objectives: The aim of this paper is to investigate whether selected biomechanical stepping behavior determined through stair gait parameters such as foot clearance, foot contact length and cadence are maintained when the staircase dimensions are different in real houses., Methods: Twenty-five older adults (>65 years) walked on a custom-made seven-step laboratory staircase. Older adults were classified into two groups (fallers and non-fallers) based on recent fall history. Among the 25 participants, 13 people had at least one fall, trip, or slip in the last six months and they were assigned to the fallers group; 12 people did not experience any fall in the last six months, so they were assigned to the non-fallers group. In addition, these participants walked on the stairs in three different real exemplar houses wearing a novel instrumented shoe sensor system that could measure the above stair gait parameters. MATLAB was used to extract fall risk parameters from the collected data. One-way ANOVA was used to compare fall risk parameters on the different staircases. In addition, the laboratory-based fall risk parameters were compared to those derived from the real house stairs., Results: There was a significant difference in selected stair-fall biomechanical risk factors among the house and laboratory staircases. The fall risk group comparisons suggest that high-risk fallers implemented a biomechanically riskier strategy that could increase overall falling risk., Conclusions: The significant differences due to the main effects of the fallers and non-fallers groups were obtained. For example, when ascending, the fallers group had less foot clearance on the entry ( p = 0.016) and middle steps ( p = 0.003); in addition, they had more foot clearance variability on the entry steps ( p = 0.003). This suggests that the fallers group in this present study did not adopt more conservative stepping strategies during stair ascent compared to low-risk older adults. By showing less foot clearance and more variability in foot clearance, the risk for a trip would be increased.

Trips and slips are significant causal perturbations leading to falls on stairs, especially in older people. The risk of a trip caused by a toe or heel catch on the step edge increases when clearance is small and variable between steps. The risk of a slip increases if the proportion of the foot area in contact with the step is reduced and variable between steps. To assess fall risk, these measurements are typically taken in a gait lab using motion-capture optoelectronic systems. The aim of this work was to develop a novel smart shoe equipped with sensors to measure foot placement and foot clearance on stairs in real homes. To validate the smart shoe as a tool for estimating stair fall risk, twenty-five older adults' sensor-based measurements were compared against foot placement and clearance measurements taken in an experimental staircase in the lab using correlations and Bland-Altman agreement techniques. The results showed that there was a good agreement and a strong positive linear correlation for foot placement (r = 0.878, p < 0.000) and foot clearance (r = 0.967, p < 0.000) between sensor and motion analysis, offering promise for advancing the current prototype into a measurement tool for fall risk in real-life staircases.

Stair falls are a major health problem for older people. Most studies on identification of stair fall risk factors are limited to staircases set in given step dimensions. However, it remains unknown whether the conclusions drawn would still apply if the dimensions had been changed to represent more challenging or easier step dimensions encountered in domestic and public buildings. The purpose was to investigate whether the self-selected biomechanical stepping behaviours are maintained when the dimensions of a staircase are altered. Sixty-eight older adults (>65 years) negotiated a seven-step staircase set in two step dimensions (shallow staircase: rise 15 cm, going 28 cm; steep staircase: rise 20 cm, going 25 cm). Six biomechanical outcome measures indicative of stair fall risk were measured. K-means clustering profiled the overall stair-negotiating behaviour and cluster profiles were calculated. A Cramer's V measured the degree of association in membership between clusters. The cluster profiles revealed that the biomechanically risky and conservative factors that characterized the overall behaviour in the clusters did not differ for the majority of older adults between staircases for ascent and descent. A strong association of membership between the clusters on the shallow staircase and the steep staircase was found for stair ascent (Cramer's V: 0.412, p < 0.001) and descent (Cramer's V: 0.380, p = 0.003). The findings indicate that manipulating the demand of the task would not affect the underpinning mechanism of a potential stair fall. Background: Older adults are at greater risk of falls while descending stairs. Cognitive deficits can further influence one's ability and mild cognitive impairments (MCI) specifically affect visual attention and dual tasking behavior. The present study aimed at comparing the attentional costs at different points during the approach to and descent of a staircase between older adults with and without MCI., Methods: Eleven older adults with MCI and twenty-three healthy older individuals without cognitive impairments were recruited. Neuropsychological tests were carried out. In addition, participants approached and descended a 5-step staircase while a simultaneous visual Stroop dual-task was randomly introduced during the approach, transition or steady state descent phases across trials. Three-dimensional kinematics and accuracy on the Stroop task were analyzed and dual task costs were calculated., Results: The MCI group showed deficits for visuo-spatial attention, memory and multi-tasking abilities, as well as balance and decreased confidence for falls efficacy, but not for daily activity scores. Despite such changes, this group of community-dwelling individuals with MCI presented a functional capacity to descend stairs even during divided visual attention. However, there were subtle, but significant, group differences for movement fluidity and performance on the simultaneous cognitive task, particularly during the approach and transition to descent phases. The MCI group also tended to descend slower while using the handrails more than healthy older adults., Conclusion: The present cohort of community-dwelling older adults with MCI were functional, but appeared to prioritize locomotor demands over the simultaneous cognitive task in a possible "posture first" strategy to descend stairs. Stair falls, especially during stair descent, are a major problem for older people. Stair fall risk has typically been assessed by quantifying mean differences between subject groups (e.g. older vs. younger individuals) for a number of biomechanical parameters individually indicative of risk, e.g., a reduced foot clearance with respect to the stair edge, which increases the chances of a trip. This approach neglects that individuals within a particular group may also exhibit other concurrent conservative strategies that could reduce the overall risk for a fall, e.g. a decreased variance in foot clearance. The purpose of the present study was to establish a multivariate approach that characterises the overall stepping behaviour of an individual. Twenty-five younger adults (age: 24.5 ± 3.3 y) and 70 older adults (age: 71.1 ± 4.1 y) descended a custom-built instrumented seven-step staircase at their self-selected pace in a step-over-step manner without using the handrails. Measured biomechanical parameters included: 1) Maximal centre of mass angular acceleration, 2) Foot clearance, 3) Proportion of foot length in contact with stair, 4) Required coefficient of friction, 5) Cadence, 6) Variance of these parameters. As a conventional analysis, a one-way ANOVA followed by Bonferroni post-hoc testing was used to identify differences between younger adults, older fallers and non-fallers. To examine differences in overall biomechanical stair descent behaviours between individuals, k-means clustering was used. The conventional grouping approach showed an effect of age and fall history on several single risk factors. The multivariate approach identified four clusters. Three clusters differed from the overall mean by showing both risky and conservative strategies on the biomechanical outcome measures, whereas the fourth cluster did not display any particularly risky or conservative strategies. In contrast to the conventional approach, the multivariate approach showed the stepping behaviours identified did not contain only older adults or previous fallers. This highlights the limited predictive power for stair fall risk of approaches based on single-parameter comparisons between predetermined groups. Establishing the predictive power of the current approach for future stair falls in older people is imperative for its implementation as a falls prevention tool., (Crown Copyright © 2019. People with diabetes display biomechanical gait alterations compared to controls and have a higher metabolic cost of walking (CoW), but it remains unknown whether differences in the vertical displacement of the body centre of mass (CoM) may play a role in this higher CoW. The aim of this study was to investigate vertical CoM displacement (and step length as a potential underpinning factor) as an explanatory factor in the previously observed increased CoW with diabetes. Thirty-one non-diabetic controls (Ctrl); 22 diabetic patients without peripheral neuropathy (DM) and 14 patients with moderate/severe Diabetic Peripheral Neuropathy (DPN), underwent gait analysis using a motion analysis system and force plates while walking at a range of matched speeds between 0.6 and 1.6 m/s. Vertical displacement of the CoM was measured over the gait cycle, and was not different in either diabetes patients with or without diabetic peripheral neuropathy compared to controls across the range of matched walking speeds examined (at 1 m/s: Ctrl: 5.59 (SD: 1.6), DM: 5.41 (1.63), DPN: 4.91 (1.66) cm; p > 0.05). The DPN group displayed significantly shorter steps (at 1 m/s: Ctrl: 69, DM: 67, DPN: 64 cm; p > 0.05) and higher cadence (at 1 m/s: Ctrl: 117 (SD1.12), DM: 119 (1.08), DPN: 122 (1.25) steps per minute; p > 0.05) across all walking speeds compared to controls. The vertical CoM displacement is therefore unlikely to be a factor in itself that contributes towards the higher CoW observed recently in people with diabetic neuropathy. The higher CoW in patients with diabetes may not be explained by the CoM displacement, but rather may be more related to shorter step lengths, increased cadence and the associated increased internal work and higher muscle forces developed by walking with more flexed joints., (Copyright © 2018 The Author(s). The Achilles tendon (AT) has the capacity to store and release elastic energy during walking, contributing to metabolic energy savings. In diabetes patients, it is hypothesized that a stiffer Achilles tendon may reduce the capacity for energy saving through this mechanism, thereby contributing to an increased metabolic cost of walking (CoW). The aim of this study was to investigate the effects of diabetes and diabetic peripheral neuropathy (DPN) on the Achilles tendon and plantarflexor muscle-tendon unit behavior during walking. Twenty-three nondiabetic controls (Ctrl); 20 diabetic patients without peripheral neuropathy (DM), and 13 patients with moderate/severe DPN underwent gait analysis using a motion analysis system, force plates, and ultrasound measurements of the gastrocnemius muscle, using a muscle model to determine Achilles tendon and muscle-tendon length changes. During walking, the DM and particularly the DPN group displayed significantly less Achilles tendon elongation (Ctrl: 1.81; DM: 1.66; and DPN: 1.54 cm), higher tendon stiffness (Ctrl: 210; DM: 231; and DPN: 240 N/mm), and higher tendon hysteresis (Ctrl: 18; DM: 21; and DPN: 24%) compared with controls. The muscle fascicles of the gastrocnemius underwent very small length changes in all groups during walking (~0.43 cm), with the smallest length changes in the DPN group. Achilles tendon forces were significantly lower in the diabetes groups compared with controls (Ctrl: 2666; DM: 2609; and DPN: 2150 N). The results strongly point toward the reduced energy saving capacity of the Achilles tendon during walking in diabetes patients as an important factor contributing to the increased metabolic CoW in these patients. NEW & NOTEWORTHY From measurements taken during walking we observed that the Achilles tendon in people with diabetes and particularly people with diabetic peripheral neuropathy was stiffer, was less elongated, and was subject to lower forces compared with controls without diabetes. These altered properties of the Achilles tendon in people with diabetes reduce the tendon's energy saving capacity and contribute toward the higher metabolic energy cost of walking in these patients.

Aim: To investigate lower limb biomechanical strategy during stair walking in patients with diabetes and patients with diabetic peripheral neuropathy, a population known to exhibit lower limb muscular weakness., Methods: The peak lower limb joint moments of twenty-two patients with diabetic peripheral neuropathy and thirty-nine patients with diabetes and no neuropathy were compared during ascent and descent of a staircase to thirty-two healthy controls. Fifty-nine of the ninety-four participants also performed assessment of their maximum isokinetic ankle and knee joint moment (muscle strength) to assess the level of peak joint moments during the stair task relative to their maximal joint moment-generating capabilities (operating strengths)., Results: Both patient groups ascended and descended stairs slower than controls (p<0.05). Peak joint moments in patients with diabetic peripheral neuropathy were lower (p<0.05) at the ankle and knee during stair ascent, and knee only during stair descent compared to controls. Ankle and knee muscle strength values were lower (p<0.05) in patients with diabetic peripheral neuropathy compared to controls, and lower at knee only in patients without neuropathy. Operating strengths were higher (p<0.05) at the ankle and knee in patients with neuropathy during stair descent compared to the controls, but not during stair ascent., Conclusion: Patients with diabetic peripheral neuropathy walk slower to alter gait strategy during stair walking and account for lower-limb muscular weakness, but still exhibit heightened operating strengths during stair descent, which may impact upon fatigue and the ability to recover a safe stance following postural instability., (Copyright © 2016 The Authors. People with diabetes walk slower and display biomechanical gait alterations compared with controls, but it remains unknown whether the metabolic cost of walking (CoW) is elevated. The aim of this study was to investigate the CoW and the lower limb concentric joint work as a major determinant of the CoW, in patients with diabetes and diabetic peripheral neuropathy (DPN). Thirty-one nondiabetic controls (Ctrl), 22 diabetic patients without peripheral neuropathy (DM), and 14 patients with moderate/severe DPN underwent gait analysis using a motion analysis system and force plates and treadmill walking using a gas analyzer to measure oxygen uptake. The CoW was significantly higher particularly in the DPN group compared with controls and also in the DM group (at selected speeds only) compared with controls, across a range of matched walking speeds. Despite the higher CoW in patients with diabetes, concentric lower limb joint work was significantly lower in DM and DPN groups compared with controls. The higher CoW is likely due to energetic inefficiencies associated with diabetes and DPN reflecting physiological and biomechanical characteristics. The lower concentric joint work in patients with diabetes might be a consequence of kinematic gait alterations and may represent a natural strategy aimed at minimizing the CoW., (Copyright © 2016 the American Physiological Society.)

Diabetic patients have an altered gait strategy during walking and are known to be at high risk of falling, especially when diabetic peripheral neuropathy is present. This study investigated alterations to lower limb joint torques during walking and related these torques to maximum strength in an attempt to elucidate why diabetic patients are more likely to fall. 20 diabetic patients with moderate/severe peripheral neuropathy (DPN), 33 diabetic patients without peripheral neuropathy (DM), and 27 non-diabetic controls (Ctrl) underwent gait analysis using a motion analysis system and force plates to measure kinetic parameters. Lower limb peak joint torques and joint work done (energy expenditure) were calculated during walking. The ratio of peak joint torques and individual maximum joint strengths (measured on a dynamometer) was then calculated for 59 of the 80 participants to yield the ‘operating strength’ for those participants. During walking DM and DPN patients showed significantly reduced peak torques at the ankle and knee. Maximum joint strengths at the knee were significantly less in both DM and DPN groups than Ctrls, and for the DPN group at the ankle. Operating strengths were significantly higher at the ankle in the DPN group compared to the Ctrls. These findings show that diabetic patients walk with reduced lower limb joint torques; however due to a decrement in their maximum ability at the ankle and knee, their operating strengths are higher. This allows less reserve strength if responding to a perturbation in balance, potentially increasing their risk of falling.

Objective: Although patients with diabetic peripheral neuropathy (DPN) are more likely to fall than age-matched controls, the underlying causative factors are not yet fully understood. This study examines the effects of diabetes and neuropathy on strength generation and muscle activation patterns during walking up and down stairs, with implications for fall risk., Research Design and Methods: Sixty-three participants (21 patients with DPN, 21 diabetic controls, and 21 healthy controls) were examined while walking up and down a custom-built staircase. The speed of strength generation at the ankle and knee and muscle activation patterns of the ankle and knee extensor muscles were analyzed., Results: Patients with neuropathy displayed significantly slower ankle and knee strength generation than healthy controls during stair ascent and descent (P < 0.05). During ascent, the ankle and knee extensor muscles were activated significantly later by patients with neuropathy and took longer to reach peak activation (P < 0.05). During descent, neuropathic patients activated the ankle extensors significantly earlier, and the ankle and knee extensors took significantly longer to reach peak activation (P < 0.05)., Conclusions: Patients with DPN are slower at generating strength at the ankle and knee than control participants during walking up and down stairs. These changes, which are likely caused by altered activations of the extensor muscles, increase the likelihood of instability and may be important contributory factors for the increased risk of falling. Background: Stair descent is a frequent daily activity that poses great risks for injury due to falling. Very little is understood about the attentional demands of stair descent and their changes with aging. The present study compared combined locomotor and cognitive functioning during different phases of stair descent between healthy young and older individuals., Methods: Sixteen young and sixteen healthy older subjects walked down a 5-step staircase, performing a simultaneous visual Stroop task (i.e., a dual task) during the approach, transition or steady-state descent phases in some trials. Three dimensional kinematics of trunk and foot motion were recorded along with the accuracy and dual task costs (DTCs) for responses to the Stroop stimuli., Results: Dual tasking influenced both gait and cognitive performance for all subjects, and older adults generally walked slower with higher foot clearances and had greater DTCs. Specific age differences were found at stair transition where older adults showed more attentional effects., Conclusions: Healthy, active older adults showed changes to attention and planning due to normal aging specifically associated with a crucial point of fall risk during stair descent., (Copyright © 2013 Elsevier Inc. In this study, we aimed to establish the joint moment and joint range of motion requirements of stair descent and the demands relative to maximal capacities in elderly and young adults. Participants descended a custom-built standard dimension four-step staircase, at their self-selected speed in a step-over manner. Kinetic data were acquired from force platforms embedded into each of the steps and into the floor at the base of the stairs. A motion analysis system was used to acquire kinematic data and joint moments were calculated using the kinematic and kinetic data. Maximum capacities (joint moment and joint range of motion) were assessed using a dynamometer. During stair descent the elderly generated lower absolute ankle joint moments than the young, which enabled them to operate at a similar relative proportion of their maximal capacity compared to young adults (75%). The knee joint moments during stair descent were similar between groups, but the elderly operated at a higher proportion of their maximal capacity (elderly: 42%; young: 30%). Ankle plantarflexion-dorsiflexion angle changes were similar between groups, which meant that the elderly operated at a higher proportion of their maximal assisted dorsiflexion angle. These results indicate that the elderly redistribute the joint moments in order to maintain the task demands within 'safe' limits.

In the present study we investigated in vivo length changes in the fascicles and tendon of the human gastrocnemius medialis (GM) muscle during walking. The experimental protocol involved real-time ultrasound scanning of the GM muscle, recording of the electrical activity of the muscle, measurement of knee- and ankle-joint rotations, and measurement of ground reaction forces in six men during walking at 3 km h(-1) on a treadmill. Fascicular lengths were measured from the sonographs recorded. Musculotendon complex length changes were estimated from anatomical and joint kinematic data. Tendon length changes were obtained combining the musculotendon complex and fascicular length-change data. The fascicles followed a different length-change pattern from those of the musculotendon complex and tendon throughout the step cycle. Two important features emerged: (i) the muscle contracted near-isometrically in the stance phase, with the fascicles operating at ca. 50 mm; and (ii) the tendon stretched by ca. 7 mm during single support, and recoiled in push-off. The behaviour of the muscle in our experiment indicates consumption of minimal metabolic energy for eliciting the contractile forces required to support and displace the body. On the other hand, the spring-like behaviour of the tendon indicates storage and release of elastic-strain energy. Either of the two mechanisms would favour locomotor economy
