Background: The management of most solid tumors of the anterior mediastinum involves complete resection. Because of their location near mediastinal structures, wide resection is not possible; therefore, surgeons must use subjective visual and tactile cues to determine disease extent. This clinical trial explored intraoperative near-infrared (NIR) imaging as an approach to improving tumor delineation during mediastinal tumor resection.Methods: Twenty-five subjects with anterior mediastinal lesions suspicious for malignancy were enrolled in an open-label feasibility trial. Subjects were administered indocyanine green (ICG) at a dose of 5 mg/kg, 24 hours before resection (via a technique called TumorGlow). The NIR imaging systems included Artemis (Quest, Middenmeer, the Netherlands) and Iridium (VisionSense Corp, Philadelphia, Pennsylvania). Intratumoral ICG uptake was evaluated. The clinical value was determined via an assessment of the ability of NIR imaging to detect phrenic nerve involvement or incomplete resection. Clinical and histopathologic variables were analyzed to determine predictors of tumor fluorescence.Results: No drug-related toxicity was observed. Optical imaging added a mean of 10 minutes to case duration. Among the subjects with solid tumors, 19 of 20 accumulated ICG. Fluorescent tumors included thymomas (n = 13), thymic carcinomas (n = 4), and liposarcomas (n = 2). NIR feedback improved phrenic nerve dissection (n = 4) and identified residual disease (n = 2). There were no false-positives or false-negatives. ICG preferentially accumulated in solid tumors; this was independent of clinical and pathologic variables.Conclusions: NIR imaging for anterior mediastinal neoplasms is safe and feasible. Objective: Patients frequently have an “enlarged thymus” incidentally identified on imaging. We sought to determine whether thymectomy is appropriate in patients with diffusely enlarged thymus glands. Methods: A retrospective review was conducted of patients undergoing thymectomy without myasthenia gravis at 1 institution over 15 years. Results: Of 117 patients undergoing thymectomy, 109 patients had complete data. Thirty-six had a gland judged by the surgeon to be diffusely enlarged, and 73 had a discrete mass. Of the 36 diffusely enlarged thymus glands, 18 (50%) occurred in patients with no symptoms referable to the thymus. No patient (0/18; 0%) with an asymptomatic diffusely enlarged thymus gland had a pathologic diagnosis that would have required resection (8 normal; 10 “hyperplasia”). Of the 18 symptomatic patients with diffusely enlarged glands, 4 (22.2%) harbored lymphoma, but none harbored thymoma or other tumor (P < .05; symptomatic vs asymptomatic). Of the 73 patients with discrete masses, 45 (61.6%) were symptomatic, and both the symptomatic and asymptomatic patients had a high rate of pathologic diagnoses that represented an indication for resection (53.3% and 42.8%, respectively, harbored thymoma or other tumor). Of the 25 (of 109) patients initially having a diagnosis of thymic hyperplasia, only 3 (12%) had true follicular hyperplasia on re-review of the pathologic condition. Interestingly, an autoimmune disorder developed in 2 (67%) of these 3 patients on long-term follow-up. Conclusions: Asymptomatic patients with diffusely enlarged thymus glands can be followed up expectantly because they have a negligible incidence of significant thymic disease; symptomatic patients with diffusely enlarged thymus glands may have lymphoma, so biopsy is appropriate. Half of patients with a discrete mass have tumors requiring resection; imaging advances would be useful to better differentiate among patients within this group. [ABSTRACT FROM AUTHOR]

Context.-Sarcomatous pulmonary neoplasms are a rare and diagnostically challenging group of tumors. Primary pulmonary sarcomas must be distinguished from the more frequent occurrence of sarcoma metastatic to the lung, primary pulmonary sarcomatoid carcinoma, and diffuse malignant mesothelioma involving the lung. In current practice, the general availability of ancillary diagnostic techniques, such as immunohistochemistry and molecular analysis, can reliably classify many sarcomatoid lesions. Objective.-To review the literature and report on additional primary material about the application of immunohistochemistry and molecular analysis in the differential diagnosis pulmonary sarcomatous neoplasms. Data Sources.-Literature review of relevant articles indexed in PubMed (National Library of Medicine) between 1961 and 2007 and primary material from the author's institution. Conclusions.-This review discusses specific criteria for the diagnosis of primary lung sarcomas and offers a practical approach to excluding other sarcoma-like lesions involving the lung. The pathologist has an essential role in evaluating these tumors and will often be the first to suggest an unusual, alternative diagnosis, which may have significant implications for patient care, therapy, and prognosis. Mucoepidermoid carcinoma of the subglottis is infrequently reported in the literature. This tumor subtype is more commonly associated with the major and minor salivary glands. Mucoepidermoid carcinoma of the larynx can be difficult to diagnose, and there is no consensus as to appropriate management. We report a case of a low-grade mucoepidermoid carcinoma that was confined to the subglottis and treated with organ-preservation surgery. A conservative surgical approach was taken because of the tumor's low-grade histology and its location and in order to preserve the patient's laryngeal function. At follow-up 15 months postoperatively, the patient remained diseasefree, and laryngeal function was intact. [ABSTRACT FROM AUTHOR]

Summary Based on studies in mouse tumor models, granulocytes appear to play a tumor-promoting role. However, there are limited data about the phenotype and function of tumor-associated neutrophils (TANs) in humans. Here, we identify a subset of TANs that exhibited characteristics of both neutrophils and antigen-presenting cells (APCs) in early-stage human lung cancer. These APC-like “hybrid neutrophils,” which originate from CD11b + CD15 hi CD10 − CD16 low immature progenitors, are able to cross-present antigens, as well as trigger and augment anti-tumor T cell responses. Interferon-γ and granulocyte-macrophage colony-stimulating factor are requisite factors in the tumor that, working through the Ikaros transcription factor, synergistically exert their APC-promoting effects on the progenitors. Overall, these data demonstrate the existence of a specialized TAN subset with anti-tumor capabilities in human cancer. [ABSTRACT FROM AUTHOR]
