Existing quantitative imaging biomarkers (QIBs) are associated with known biological tissue characteristics and follow a well-understood path of technical, biological and clinical validation before incorporation into clinical trials. In radiomics, novel data-driven processes extract numerous visually imperceptible statistical features from the imaging data with no a priori assumptions on their correlation with biological processes. The selection of relevant features (radiomic signature) and incorporation into clinical trials therefore requires additional considerations to ensure meaningful imaging endpoints. Also, the number of radiomic features tested means that power calculations would result in sample sizes impossible to achieve within clinical trials. This article examines how the process of standardising and validating data-driven imaging biomarkers differs from those based on biological associations. Radiomic signatures are best developed initially on datasets that represent diversity of acquisition protocols as well as diversity of disease and of normal findings, rather than within clinical trials with standardised and optimised protocols as this would risk the selection of radiomic features being linked to the imaging process rather than the pathology. Normalisation through discretisation and feature harmonisation are essential pre-processing steps. Biological correlation may be performed after the technical and clinical validity of a radiomic signature is established, but is not mandatory. Feature selection may be part of discovery within a radiomics-specific trial or represent exploratory endpoints within an established trial; a previously validated radiomic signature may even be used as a primary/secondary endpoint, particularly if associations are demonstrated with specific biological processes and pathways being targeted within clinical trials. KEY POINTS: • Data-driven processes like radiomics risk false discoveries due to high-dimensionality of the dataset compared to sample size, making adequate diversity of the data, cross-validation and external validation essential to mitigate the risks of spurious associations and overfitting. • Use of radiomic signatures within clinical trials requires multistep standardisation of image acquisition, image analysis and data mining processes. • Biological correlation may be established after clinical validation but is not mandatory., (© 2021. Objective: To determine whether a simulated low-dose metal artifact reduction (MAR) CT technique is comparable with a clinical dose MAR technique for shoulder arthroplasty evaluation., Materials and Methods: Two shoulder arthroplasties in cadavers and 25 shoulder arthroplasties in patients were scanned using a clinical dose (140 kVp, 300 qrmAs); cadavers were also scanned at half dose (140 kVp, 150 qrmAs). Images were reconstructed using a MAR CT algorithm at full dose and a noise-insertion algorithm simulating 50% dose reduction. For the actual and simulated half-dose cadaver scans, differences in SD for regions of interest were assessed, and streak artifact near the arthroplasty was graded by 3 blinded readers. Simulated half-dose scans were compared with full-dose scans in patients by measuring differences in implant position and by comparing readers' grades of periprosthetic osteolysis and muscle atrophy., Results: The mean difference in SD between actual and simulated half-dose methods was 2.42 HU (95% CI [1.4, 3.4]). No differences in streak artifact grades were seen in 13/18 (72.2%) comparisons in cadavers. In patients, differences in implant position measurements were within 1° or 1 mm in 149/150 (99.3%) measurements. The inter-reader agreement rates were nearly identical when readers were using full-dose (77.3% [232/300] for osteolysis and 76.9% [173/225] for muscle atrophy) and simulated half-dose (76.7% [920/1200] for osteolysis and 74.0% [666/900] for muscle atrophy) scans., Conclusion: A simulated half-dose MAR CT technique is comparable both quantitatively and qualitatively with a standard-dose technique for shoulder arthroplasty evaluation, demonstrating that this technique could be used to reduce dose in arthroplasty imaging.

Background: Cardiac computed tomography (CT) is emerging as an adjunctive modality to echocardiography in the evaluation of infective endocarditis (IE) and surgical planning. CT studies in IE have, however, focused on its diagnostic rather than prognostic utility, the latter of which is important in high-risk diseases like IE. We evaluated the associations between cardiac CT and transesophageal echocardiography (TEE) findings and adverse outcomes after IE surgery., Methods: Of 833 consecutive patients with surgically proven IE during May 1, 2014 to May 1, 2019, at Cleveland Clinic, 155 underwent both preoperative ECG-gated contrast-enhanced CT and TEE. Multivariable analyses were performed to identify CT and TEE biomarkers that predict adverse outcomes after IE surgery, adjusting for EuroSCORE II (European System for Cardiac operative Risk Evaluation II)., Results: CT and TEE were positive for IE in 123 (75.0%) and 124 (75.6%) of patients, respectively. Thirty-day mortality occurred in 3 (1.9%) patients and composite mortality or morbidities in 72 (46.5%). Pseudoaneurysm or abscess detected on TEE was the only imaging biomarker to show independent association with composite mortality or morbidities in-hospital, with odds ratio (95% CI) of 3.66 (1.76-7.59), P =0.001. There were 17 late deaths, and both pseudoaneurysm or abscess detected on CT and fistula detected on CT were the only independent predictors of total mortality during follow-up, with hazards ratios (95% CI) of 3.82 (1.25-11.7), P <0.001 and 9.84 (1.89-51.0), P =0.007, respectively., Conclusions: We identified cardiac CT and TEE features that predicted separate adverse outcomes after IE surgery. Imaging biomarkers can play important roles incremental to conventional clinical factors for risk stratification in patients undergoing IE surgery.

OBJECTIVE. The purpose of this study was to compare a combined dual-energy CT (DECT) and single-energy CT (SECT) metal artifact reduction technique with a SECT metal artifact reduction technique for detecting lesions near an arthroplasty in a phantom model. MATERIALS AND METHODS. Two CT phantoms with a cobalt chromium sphere attached to a titanium rod, simulating an arthroplasty, within a background of soft-tissue attenuation containing spherical lesions (range, 10-20 mm) around the head and stem of different attenuations from the background (range of attenuation, 10-70 HU) were scanned with a single CT scanner individually (unilateral) and together (bilateral) with the following three dose-equivalent techniques: the currently used clinical protocol (140 kVp, 300 Reference mAs); 100 kVp; and DECT (100 kVp and 150 kVp with a tin filter). Three radiologists reviewed the datasets to identify lesions. Nonparametric AUC was estimated for each reader with each technique. Multireader ANOVA was performed to compare AUCs. Multiple-variable logistic regression analysis was used to identify factors affecting sensitivity and specificity. RESULTS. Accuracy was lower ( p < 0.001) for the DECT 130-keV technique than for the 100-, 70-, and 140-kVp techniques. Sensitivity was higher with unilateral arthroplasties ( p = 0.037), with greater contrast differences from background ( p < 0.001), and with the SECT 100-kVp technique versus other techniques ( p < 0.001). The difference in specificities of modalities was not statistically significant ( p = 0.148). CONCLUSION. Combining DECT and SECT techniques does not provide additional benefits for lesion detection as opposed to using SECT alone.

Purpose To compare the diagnostic yield and complication rates of electromagnetic navigational bronchoscopic (ENB)-guided and computed tomography (CT)-guided percutaneous tissue sampling of lung nodules. Materials and Methods Retrospectively identified were 149 patients sampled percutaneously with CT guidance and 146 patients who underwent ENB with transbronchial biopsy of a lung lesion between 2013 and 2015. Clinical data, incidence of complications, and nodule pathologic analyses were assessed through electronic medical record review. Lung nodule characteristics were reviewed through direct image analysis. Molecular marker studies and pathologic analyses from surgical excision were reviewed when available. Multiple-variable logistic regression models were built to compare the diagnostic yield and complication rates for each method and for different patient and disease characteristics. Results CT-guided sampling was more likely to be diagnostic than ENB-guided biopsy (86.0% [129 of 150] vs 66.0% [99 of 150], respectively), and this difference remained significant even after adjustments were made for patient and nodule characteristics (P < .001). Age, American Society of Anesthesiologists class, emphysema grade, nodule size, and distance from pleura were not significant predictors of increased diagnostic yield. Intraprocedural time for physicians was significantly lower with CT-guided sampling (P < .001). Similar yield for molecular analyses was noted with the two approaches (ENB-guided sampling, 88.9% [32 of 36]; CT-guided sampling, 82.0% [41 of 50]). The two groups had similar rates of major complications (symptomatic hemorrhage, P > .999; pneumothorax requiring chest tube and/or admission, P = .417). Conclusion CT-guided transthoracic biopsy provided higher diagnostic yield in the assessment of peripheral pulmonary nodules than navigational bronchoscopy with a similar rate of clinically relevant complications. © RSNA, 2017 Online supplemental material is available for this article.

Objective: The objective of this study was to compare reader accuracy detecting lesions near hardware in a CT phantom model at different radiation exposures using an advanced metal artifact reduction (MAR) algorithm and standard filtered back projection (FBP) techniques and to determine if radiation exposure could be decreased using MAR without compromising lesion detectability., Materials and Methods: A CT phantom manufactured with spherical lesions of various sizes (10-20 mm) and attenuations (20-50 HU) embedded around cobalt-chromium spheres attached to titanium rods, simulating an arthroplasty, was scanned on a single CT scanner (FLASH, Siemens Healthcare) at 140 kVp and 0.6-mm collimation using clinical-dose (300 Quality Reference mAs [Siemens Healthcare]), low-dose (150 Quality Reference mAs), and high-dose (600 Quality Reference mAs) protocols. Images reconstructed with iterative MAR, advanced modeled iterative reconstruction (ADMIRE), and FBP with identical parameters were anonymized and independently reviewed by three radiologists. Accuracies for detecting lesions, measured as AUC, sensitivity, and specificity, were compared., Results: Accuracy using MAR was significantly higher than that using FBP at all exposures (p values ranged from < 0.001 to 0.021). Sensitivity was also higher for MAR than for FBP at all exposures. Specificity was very high for both reconstruction techniques at all exposures with no significant differences. Accuracy of low-dose MAR was higher than and not inferior to standard-dose and high-dose FBP. MAR was significantly more sensitive than FBP in detecting smaller lesions (p = 0.021) and lesions near high streak artifact (p < 0.001)., Conclusion: MAR improves reader accuracy to detect lesions near hardware and allows significant reductions in radiation exposure without compromising accuracy compared with FBP in a CT phantom model.

Background: Accurate and consistent regional lymph node classification is an important element in the staging and multidisciplinary management of lung cancer. Regional lymph node definition sets-lymph node maps-have been created to standardize regional lymph node classification. In 2009, the International Association for the Study of Lung Cancer (IASLC) introduced a lymph node map to supersede all preexisting lymph node maps. Our aim was to study if and how lung cancer specialists apply the IASLC lymph node map when classifying thoracic lymph nodes encountered on CT scans during lung cancer staging., Methods: From April 2013 through July 2013, invitations were distributed to all members of the Fleischner Society, Society of Thoracic Radiology, General Thoracic Surgical Club, and the American Association of Bronchology and Interventional Pulmonology to participate in an anonymous online image-based and text-based 20-question survey regarding lymph node classification for lung cancer staging on CT imaging., Results: Three hundred thirty-seven people responded (approximately 25% participation). Respondents consisted of self-reported thoracic radiologists (n = 158), thoracic surgeons (n = 102), and pulmonologists who perform endobronchial ultrasonography (n = 77). Half of the respondents (50%; 95% CI, 44%-55%) reported using the IASLC lymph node map in daily practice, with no significant differences between subspecialties. A disparity was observed between the IASLC definition sets and their interpretation and application on CT scans, in particular for lymph nodes near the thoracic inlet, anterior to the trachea, anterior to the tracheal bifurcation, near the ligamentum arteriosum, between the bronchus intermedius and esophagus, in the internal mammary space, and adjacent to the heart., Conclusions: Use of older lymph node maps and inconsistencies in interpretation and application of definitions in the IASLC lymph node map may potentially lead to misclassification of stage and suboptimal management of lung cancer in some patients., (Published by Elsevier Inc.)

Background: Optimal imaging triage for intervention for large vessel occlusions remains unclear. MR-based imaging provides ischemic core volumes at the cost of increased imaging time. CT Alberta Stroke Program Early CT Score (ASPECTS) estimates are faster, but may be less sensitive., Objective: To assesses the rate at which MRI changed management in comparison with CT imaging alone., Methods: Retrospective analysis of patients with acute ischemic stroke undergoing imaging triage for endovascular therapy was performed between 2008 and 2013. Univariate and multivariate analyses were performed. Multivariate logistic regression was used to evaluate the effect of time on disagreement in MRI and CT ASPECTS scores., Results: A total of 241 patients underwent both diffusion-weighted imaging (DWI) and CT. Six patients with DWI ASPECTS ≥6 and CT ASPECTS <6 were omitted, leaving 235 patients. For 47 patients, disagreement between the two modalities resulted in different treatment recommendations. The estimated probability of disagreement was 20.0% (95% CI 15.4% to 25.6%). In a multivariate logistic regression, CT ASPECTS >7 (p=0.004) and admission National Institutes of Health Stroke Scale (NIHSS) score <16 (p=0.008) were simultaneously significant predictors of agreement in ASPECTS. The time between modalities was a marginally significant predictor (p=0.080)., Conclusions: The study suggests that patients with NIHSS scores at admission of <16 and patients with CT ASPECTS >7 have a higher likelihood of agreement between CT and DWI based on an ASPECTS cut-off value of 6. Additional MRI for triage in patients with NIHSS at admission of >16, and ASPECTS of 6 or 7 may be more likely to change management. Purpose To compare the diagnostic accuracy and image quality of computed tomographic (CT) enterographic images obtained at half dose and reconstructed with filtered back projection (FBP) and sinogram-affirmed iterative reconstruction (SAFIRE) with those of full-dose CT enterographic images reconstructed with FBP for active inflammatory terminal or neoterminal ileal Crohn disease. Materials and Methods This retrospective study was compliant with HIPAA and approved by the institutional review board. The requirement to obtain informed consent was waived. Ninety subjects (45 with active terminal ileal Crohn disease and 45 without Crohn disease) underwent CT enterography with a dual-source CT unit. The reference standard for confirmation of active Crohn disease was active terminal ileal Crohn disease based on ileocolonoscopy or established Crohn disease and imaging features of active terminal ileal Crohn disease. Data from both tubes were reconstructed with FBP (100% exposure); data from the primary tube (50% exposure) were reconstructed with FBP and SAFIRE strengths 3 and 4, yielding four datasets per CT enterographic examination. The mean volume CT dose index (CTDIvol) and size-specific dose estimate (SSDE) at full dose were 13.1 mGy (median, 7.36 mGy) and 15.9 mGy (median, 13.06 mGy), respectively, and those at half dose were 6.55 mGy (median, 3.68 mGy) and 7.95 mGy (median, 6.5 mGy). Images were subjectively evaluated by eight radiologists for quality and diagnostic confidence for Crohn disease. Areas under the receiver operating characteristic curves (AUCs) were estimated, and the multireader, multicase analysis of variance method was used to compare reconstruction methods on the basis of a noninferiority margin of 0.05. Results The mean AUCs with half-dose scans (FBP, 0.908; SAFIRE 3, 0.935; SAFIRE 4, 0.924) were noninferior to the mean AUC with full-dose FBP scans (0.908; P < .003). The proportion of images with inferior quality was significantly higher with all half-dose reconstructions than with full-dose FBP (mean proportion: 0.117 for half-dose FBP, 0.054 for half-dose SAFIRE 3, 0.054 for half-dose SAFIRE 4, and 0.017 for full-dose FBP; P < .001). Conclusion The diagnostic accuracy of half-dose CT enterography with FBP and SAFIRE is statistically noninferior to that of full-dose CT enterography for active inflammatory terminal ileal Crohn disease, despite an inferior subjective image quality. Objective: The purpose of this study was to compare iterative metal artifact reduction (iMAR), a new single-energy metal artifact reduction technique, with filtered back projection (FBP) in terms of attenuation values, qualitative image quality, and streak artifacts near shoulder and hip arthroplasties and observer ability with these techniques to detect pathologic lesions near an arthroplasty in a phantom model., Materials and Methods: Preoperative and postoperative CT scans of 40 shoulder and 21 hip arthroplasties were reviewed. All postoperative scans were obtained using the same technique (140 kVp, 300 quality reference mAs, 128 × 0.6 mm detector collimation) on one of three CT scanners and reconstructed with FBP and iMAR. The attenuation differences in bones and soft tissues between preoperative and postoperative scans at the same location were compared; image quality and streak artifact for both reconstructions were qualitatively graded by two blinded readers. Observer ability and confidence to detect lesions near an arthroplasty in a phantom model were graded., Results: For both readers, iMAR had more accurate attenuation values (p < 0.001), qualitatively better image quality (p < 0.001), and less streak artifact (p < 0.001) in all locations near arthroplasties compared with FBP. Both readers detected more lesions (p ≤ 0.04) with higher confidence (p ≤ 0.01) with iMAR than with FBP in the phantom model., Conclusion: The iMAR technique provided more accurate attenuation values, better image quality, and less streak artifact near hip and shoulder arthroplasties than FBP; iMAR also increased observer ability and confidence to detect pathologic lesions near arthroplasties in a phantom model.

Rationale and Objectives: Tumor volume change has potential as a biomarker for diagnosis, therapy planning, and treatment response. Precision was evaluated and compared among semiautomated lung tumor volume measurement algorithms from clinical thoracic computed tomography data sets. The results inform approaches and testing requirements for establishing conformance with the Quantitative Imaging Biomarker Alliance (QIBA) Computed Tomography Volumetry Profile., Materials and Methods: Industry and academic groups participated in a challenge study. Intra-algorithm repeatability and inter-algorithm reproducibility were estimated. Relative magnitudes of various sources of variability were estimated using a linear mixed effects model. Segmentation boundaries were compared to provide a basis on which to optimize algorithm performance for developers., Results: Intra-algorithm repeatability ranged from 13% (best performing) to 100% (least performing), with most algorithms demonstrating improved repeatability as the tumor size increased. Inter-algorithm reproducibility was determined in three partitions and was found to be 58% for the four best performing groups, 70% for the set of groups meeting repeatability requirements, and 84% when all groups but the least performer were included. The best performing partition performed markedly better on tumors with equivalent diameters greater than 40 mm. Larger tumors benefitted by human editing but smaller tumors did not. One-fifth to one-half of the total variability came from sources independent of the algorithms. Segmentation boundaries differed substantially, not ony in overall volume but also in detail., Conclusions: Nine of the 12 participating algorithms pass precision requirements similar to what is indicated in the QIBA Profile, with the caveat that the present study was not designed to explicitly evaluate algorithm profile conformance. Change in tumor volume can be measured with confidence to within ±14% using any of these nine algorithms on tumor sizes greater than 10 mm. No partition of the algorithms was able to meet the QIBA requirements for interchangeability down to 10 mm, although the partition comprising best performing algorithms did meet this requirement for a tumor size of greater than approximately 40 mm., (Copyright © 2015 AUR. Background: Preprocedural 3-dimensional CT imaging of the aortic annular plane plays a critical role for transcatheter aortic valve replacement (TAVR) planning; however, manual reconstructions are complex. Automated analysis software may improve reproducibility and agreement between readers but is incompletely validated., Methods: In 110 TAVR patients (mean age, 81 years; 37% female) undergoing preprocedural multidetector CT, automated reconstruction of the aortic annular plane and planimetry of the annulus was performed with a prototype of now commercially available software (syngo.CT Cardiac Function-Valve Pilot; Siemens Healthcare, Erlangen, Germany). Fully automated, semiautomated, and manual annulus measurements were compared. Intrareader and inter-reader agreement, intermodality agreement, and interchangeability were analyzed. Finally, the impact of these measurements on recommended valve size was evaluated., Results: Semiautomated analysis required major correction in 5 patients (4.5%). In the remaining 95.5%, only minor correction was performed. Mean manual annulus area was significantly smaller than fully automated results (P < .001 for both readers) but similar to semiautomated measurements (5.0 vs 5.4 vs 4.9 cm(2), respectively). The frequency of concordant recommendations for valve size increased if manual analysis was replaced with the semiautomated method (60% agreement was improved to 82.4%; 95% confidence interval for the difference [69.1%-83.4%])., Conclusions: Semiautomated aortic annulus analysis, with minor correction by the user, provides reliable results in the context of TAVR annulus evaluation., (Copyright © 2015 Society of Cardiovascular Computed Tomography. Objective: Iterative metal artifact reduction (IMAR) is a sinogram inpainting technique that incorporates high-frequency data from standard weighted filtered back projection (WFBP) reconstructions to reduce metal artifact on computed tomography (CT). This study was designed to compare the image quality of IMAR and WFBP in total shoulder arthroplasties (TSA); determine the optimal amount of WFBP high-frequency data needed for IMAR; and compare image quality of the standard 3D technique with that of a faster 2D technique., Materials and Methods: Eight patients with nine TSA underwent CT with standardized parameters: 140 kVp, 300 mAs, 0.6 mm collimation and slice thickness, and B30 kernel. WFBP, three 3D IMAR algorithms with different amounts of WFBP high-frequency data (IMARlo, lowest; IMARmod, moderate; IMARhi, highest), and one 2D IMAR algorithm were reconstructed. Differences in attenuation near hardware and away from hardware were measured and compared using repeated measures ANOVA. Five readers independently graded image quality; scores were compared using Friedman's test., Results: Attenuation differences were smaller with all 3D IMAR techniques than with WFBP (p < 0.0063). With increasing high-frequency data, the attenuation difference increased slightly (differences not statistically significant). All readers ranked IMARmod and IMARhi more favorably than WFBP (p < 0.05), with IMARmod ranked highest for most structures. The attenuation difference was slightly higher with 2D than with 3D IMAR, with no significant reader preference for 3D over 2D., Conclusions: IMAR significantly decreases metal artifact compared to WFBP both objectively and subjectively in TSA. The incorporation of a moderate amount of WFBP high-frequency data and use of a 2D reconstruction technique optimize image quality and allow for relatively short reconstruction times.

Rationale and Objectives: New tests are typically assessed by estimating their technical and diagnostic performance through comparisons with a reference standard. A valid reference standard, however, is not always available and is not required for assessing the interchangeability of a new test with an existing one., Materials and Methods: To show interchangeability of a new test with an existing test, one compares the differences in diagnoses between the new and existing tests to differences between diagnoses made with the existing test on several occasions. We illustrate the test for interchangeability with two studies. In a transcatheter aortic valve replacement study, we test whether semiautomated analysis can be used interchangeably with manual reconstructions from three-dimensional computed tomography (CT) images. In patients with femoroacetabular impingement, we test whether magnetic resonance imaging (MRI) can replace CT to measure acetabular version., Results: Although the semiautomated method agreed often with the manual measurement of aortic valve size (87.6%), interchanging the semiautomated method with manual measurements by an expert would lead to a 1.7%-12.2% increase in the frequency of disagreement. Interchanging MRI for CT to measure acetabular version would lead to differences in angle measurements of 2.0° to 3.1° in excess of the differences we would expect to see with CT alone., Conclusions: Testing for agreement or correlation between a new and an existing test is not sufficient evidence of the performance of a new test. Purpose: To compare images acquired with 50% tube exposure with a dual-source computed tomographic (CT) scanner and reconstructed with sinogram-affirmed iterative reconstruction (SAFIRE) with 100% exposure images reconstructed with filtered back projection (FBP) for reader ability to detect stones, reader confidence, and findings outside the urinary tract., Materials and Methods: In this HIPAA-compliant, institutional review board-approved study, imaging examinations in 99 patients with urolithiasis were assessed. Data from both tubes were reconstructed with FBP; data from the primary tube only were reconstructed with SAFIRE. Seven readers evaluated randomized studies for calculi in nine regions. Reader confidence was scored by using a five-point scale. Ancillary findings were noted. Nonparametric methods for clustered data were used to estimate the area under the receiver operating characteristic curves with 95% confidence intervals to test for noninferiority of 50% exposure with SAFIRE., Results: Calculi were found in 113 locations (pyelocalyceal ureter, 86; proximal ureter, seven; midureter, four; distal ureter, 15; bladder, one) and not found in 752 locations. Mean area under the receiver operating characteristic curve for FBP was 0.879 (range, 0.607-0.967) and for SAFIRE, 0.883 (range, 0.646-0.971; 95% confidence interval: -0.025, 0.031). The SAFIRE images were not significantly inferior to FBP images (P = .001). Reader confidence levels for images with stones were similar with FBP and SAFIRE (P = .963). For the 52 patients who had extraurinary findings, readers reported them correctly in 74.4% (271 of 364) and 72.0% (262 of 364) of cases (P = .215) for FBP and SAFIRE, respectively. For the nine patients with potentially important findings per the reference standard, the detection rates were 44% (28 of 63) and 33% (21 of 63, P = .024), respectively. For the 43 patients with unimportant or likely unimportant findings, the false detection rates were 15% (44 of 301) and 14% (43 of 301, P = .756), respectively., Conclusion: The 50% tube exposure CT images reconstructed with SAFIRE were not inferior to 100% exposure images reconstructed with FBP for diagnosis of urolithiasis, without decreases in reader confidence.

Purpose: To measure the effect of reduced radiation exposure on low-contrast low-attenuation liver lesion detection in an anthropomorphic abdominal phantom by using filtered back projection (FBP) and sinogram-affirmed iterative reconstruction., Materials and Methods: Eighteen radiologists blinded to phantom and study design interpreted randomized image data sets that contained 36 spherical simulated liver lesions of three sizes and three attenuation differences (5-mm diameter: 12, 18, and 24 HU less than the 90-HU background attenuation of the simulated liver insert; 10- and 15-mm diameter: 6, 12, and 18 HU less than the 90-HU background attenuation) scanned with four discrete exposure settings and reconstructed by using FBP and sinogram-affirmed iterative reconstruction. Response assessment included region-level lesion presence or absence on a five-point diagnostic confidence scale. Statistical evaluation included multireader multicase receiver operating characteristic curve analysis, with nonparametric methods and noninferiority analysis at a margin of -0.10., Results: Pooled accuracy at 75% exposure for both FBP and sinogram-affirmed iterative reconstruction was noninferior to 100% exposure (P = .002 and P < .001, respectively). Subsequent exposure reductions resulted in a significant decrease in accuracy. When the smallest (5-mm-diameter) lesions were excluded from analysis, sinogram-affirmed iterative reconstruction was superior to FBP at 100% exposure (P = .011), and sinogram-affirmed iterative reconstruction at 25% and 50% exposure reduction was noninferior to FBP at 100% exposure (P ≤ .013). Reader confidence was greater with sinogram-affirmed iterative reconstruction than with FBP for 10- and 15-mm lesions (2.94 vs 2.76 and 3.62 vs 3.52, respectively)., Conclusion: In this low-contrast low-attenuation liver lesion model, a 25% exposure reduction maintained noninferior diagnostic accuracy. However, detection was inferior with each subsequent exposure reduction, regardless of reconstruction method. Sinogram-affirmed iterative reconstruction and FBP performed equally well at modest exposure reduction (25%-50%). Objective: The purpose of this article is to evaluate the effect of sinogram-affirmed iterative reconstruction (SAFIRE) on contrast-to-noise ratio (CNR) compared with filtered back projection (FBP) and to determine whether SAFIRE improves low-contrast object detection or conspicuity in a low-contrast object phantom and in the liver on full- and low-dose examinations., Subjects and Methods: A low-contrast object phantom was scanned at 100%, 70%, 50%, and 30% dose using a single-source made of a dual-source MDCT scanner, with the raw data reconstructed with SAFIRE and FBP. Unenhanced liver CT scans in 22 patients were performed using a dual-source MDCT. The raw data from both tubes (100% dose) were reconstructed using FBP, and data from one tube (50% dose) were reconstructed using both FBP and SAFIRE. CNR was measured in the phantom and in the liver. Noise, contrast, and CNR were compared using paired Student t tests. Six readers assessed sphere detection and conspicuity in the phantom and liver-inferior vena cava conspicuity in the patient data. The phantom and patient data were assessed using multiple-variable logistic regression., Results: The phantom at 70% and 50% doses with SAFIRE had decreased noise and increased CNR compared with the 100% dose with FBP. In the liver, the mean CNR improvement at 50% dose with SAFIRE compared with FBP was 31.4% and 88% at 100% and 50% doses, respectively (p < 0.001). Sphere object detection and conspicuity improved with SAFIRE (p < 0.001). However, smaller spheres were obscured on both FBP and SAFIRE images at lower doses. Liver-vessel conspicuity improved with SAFIRE over 50%-dose FBP in 67.4% of cases (p < 0.001), and versus 100%-dose FBP, improved in 38.6% of cases (p = 0.085). As a predictor for detection, CNR alone had a discriminatory ability (c-index, 0.970) similar to that of the model that analyzed dose, lesion size, attenuation difference, and reconstruction technique (c-index, 0.978)., Conclusion: Lower dose scans reconstructed with SAFIRE have a higher CNR. The ability of SAFIRE to improve low-contrast object detection and conspicuity depends on the radiation dose level. At low radiation doses, low-contrast objects are invisible, regardless of reconstruction technique.

Objective: The purpose of our study was to determine whether the MDCT enterography dose can be reduced by changing automatic exposure control (AEC) setting and quality reference milliampere-seconds (mAs) without altering subjective image quality or efficacy in active inflammatory Crohn's disease., Subjects and Methods: This is a prospective study of 2,310 MDCT enterography procedures performed using 16- and 64-MDCT in three cohorts (original, intermediate, and final dose levels). For 16-MDCT, the original and intermediate dose level quality reference mAs was 200, and weight-based (1 pound [0.45 kg] = 1 mAs) for the final dose level. For 64-MDCT, the original dose level quality reference mAs was 260; the mAs was 220 for intermediate and weight-based for the final dose level. For the intermediate and final dose levels, AEC was changed from strong to weak increase for obese and weak to strong decrease for slim patients. Demographic data and volume CT dose index (CTDI(vol)) were analyzed. Three readers evaluated the cases for image quality and efficacy differentiating normal from active inflammatory Crohn's disease., Results: For 16-MDCT, CTDI(vol) decreased from 12.82 to 10.14 mGy and 10.14 to 8.7 mGy between original to intermediate and intermediate to final dose levels. For 64-MDCT, the CTDI(vol) decreased from 15.72 to 11.42 mGy and 11.42 to 9.25 mGy between original to intermediate and intermediate to final dose levels. Images were rated suboptimal or nondiagnostic more often in the intermediate dose level (p < 0.05) but not in the final. There was no reduction in diagnostic efficacy as measured by area under the ROC curve (p > 0.1443 except for one comparison with one reader)., Conclusion: Substantial dose reduction can be achieved using weight-based quality reference mAs and altering AEC settings without affecting diagnostic efficacy in active inflammatory Crohn's disease of the terminal ileum. However, subjective image quality can be compromised at these dose settings, depending on radiologist preference.

Objective: The purpose of our study was to measure relative and absolute wall attenuations and wall thickness in normal small bowel on contrast-enhanced CT enterography and to study the efficacy of relative attenuation, absolute attenuation, and wall thickness in distinguishing normal from active inflammatory Crohn's disease of the terminal ileum., Materials and Methods: Using a case-control study design, we reviewed 630 CT enterography examinations, of which 191 were normal and 36 had active inflammatory Crohn's disease in the terminal ileum. In healthy individuals, wall thickness and attenuation in distended and collapsed loops were measured in the duodenum and four abdominal quadrants. Wall thickness and attenuation were also measured in the terminal ileum. All measurements of intraarterial attenuation were taken at the same slice level. In the examinations of patients with Crohn's disease, only terminal ileum wall thickness and attenuation as well as arterial attenuation at the same slice level were measured. Normal segments were compared with a linear model. Terminal ileum data were fit to a multivariate logistic regression model., Results: Relative attenuation and absolute attenuation in the normal distended and collapsed duodenum and left upper quadrant were significantly greater than in all other segments (p < 0.001 and < 0.048 for relative attenuation and p < 0.001 and < 0.032 for absolute attenuation, respectively). Relative attenuation and wall thickness models and absolute attenuation and wall thickness models discriminated normal from active terminal ileum Crohn's disease significantly better than the same measurements without wall thickness (p = 0.017 and 0.001, respectively). When the bowel wall is > 3 mm, a relative attenuation cutoff of 0.5 is 89% sensitive and 81% specific., Conclusion: In normal small bowel, when wall measurement is taken into account, the duodenum and jejunum have a greater relative attenuation and absolute attenuation than other segments. Relative attenuation and absolute attenuation with wall thickness models discriminate normal from active terminal ileum Crohn's disease better than the same measurements without wall thickness.

Unlabelled: The trend toward focused surgical parathyroidectomy requires precise preoperative localization of parathyroid lesions in patients with hyperparathyroidism. The purpose of this study was to directly compare the diagnostic accuracy of (99m)Tc-sestamibi/(123)I subtraction SPECT with SPECT/CT for the localization of abnormal parathyroid glands in patients with primary hyperparathyroidism., Methods: A total of 61 consecutive surgical patients with primary hyperparathyroidism underwent both (123)I/(99m)Tc-sestamibi subtraction SPECT and SPECT/CT scans preoperatively, using a hybrid SPECT/CT instrument that combined a dual-detector SPECT camera with a 6-slice multidetector spiral CT scanner. Four hours after being given (123)I-sodium iodide orally, each patient received (99m)Tc-sestamibi intravenously, followed immediately by a simultaneous, dual-isotope SPECT scan of the neck and upper chest. Then, without moving the patient, we performed a non-contrast-enhanced CT scan of the same body region. Normalization and subtraction of the (123)I SPECT images from the (99m)Tc SPECT images were performed. The subtraction SPECT and the coregistered fused SPECT/CT studies were interpreted separately, with images scored on a 5-point scale. Surgical and histopathologic findings were used as the standard of comparison., Results: Surgery was successful in 57 patients (solitary parathyroid adenoma in 48 patients, double parathyroid adenomas in 6 patients, and 10 hyperplastic parathyroid glands in 3 patients). The sensitivities of SPECT (50/70 = 71%) and SPECT/CT (49/70 = 70%) were similar (P = 0.779). The specificity of SPECT/CT (26/27 = 96%) was significantly greater than that of SPECT (13/27 = 48%; P = 0.006). The receiver-operating-characteristic area under the curve of SPECT/CT (0.833) was significantly greater than that of SPECT (0.632; P < 0.001)., Conclusion: SPECT/CT is significantly more specific than dual-isotope subtraction SPECT for the preoperative identification of parathyroid lesions in patients with primary hyperparathyroidism.

Background: Dual-source computed tomography (DSCT) was introduced with significant hardware and software changes compared with single-source CT (SSCT), resulting in improved temporal resolution (83 ms) and the potential for improved image quality. The effect of these changes on radiation dose requirements for coronary CT angiography in clinical practice has not been investigated., Objective: We evaluated patient radiation dose and image quality of electrocardiogram (ECG)-gated helical techniques, using DSCT compared with SSCT for clinical imaging of the coronary arteries., Methods: DSCT data from 160 patients were evaluated; 82 patients (DSCT group 1) were imaged with early software, and 78 patients (DSCT group 2) were imaged with a later software version. Patients imaged with SSCT (n = 124) were the control group. Effective radiation dose values were estimated for all patients. Image noise was measured, and image quality was evaluated on a 5-point scale., Results: Effective dose values for DSCT group 2 (11.7 +/- 4.0 mSv) were not different from those for SSCT group (10.9 +/- 2.9 mSv); the highest doses, 13.2 +/- 3.2 mSv, were recorded for DSCT group 1 (P < 0.001). Background: Noninvasive characterization of coronary atherosclerotic plaque is limited with current computed tomography (CT) techniques. Dual-energy CT (DECT) has the potential to provide additional attenuation data for better differentiation of plaque components., Objective: We attempted to characterize coronary atherosclerotic plaque with DECT., Methods: Seven human coronary arteries acquired at autopsy were scanned consecutively at 80 and 140 kVp with CT. Vessels were perfused with saline, and data were acquired before and after contrast agent injection. Lesions were identified, and attenuation measurements were made from CT image quadrants. CT quadrants were classified as densely calcified, fibrocalcific, fibrous, lipid-rich, or normal vessel wall, corresponding to matched histology images. Attenuation values at each peak tube voltage were compared within plaque types for both noncontrast and contrast scans. Further, dual-energy index (DEI) values computed from attenuation were analyzed for classification of plaque., Results: In 14 lesions, a total of 56 quadrants were identified. Histology results classified 8 (14%) as densely calcified, 8 (14%) as fibrocalcific, 9 (16%) as fibrous, 5 (9%) as lipid-rich, and 25 (45%) as normal vessel wall. Calcified lesions attenuated significantly more at 80 kVp in both contrast and noncontrast scans, whereas fibrous plaque attenuated more at 80 kVp only for contrast-enhanced scans. No differences were found for lipid-rich plaques. Using DEI values, only densely calcified plaques could be distinguished from other plaque types except fibrocalcific plaques in contrast images., Conclusions: Only densely calcified and fibrocalcific plaques showed a true change in attenuation at 80 versus 140 kVp. Therefore, calcified plaques could be distinguished from noncalcified plaques with DECT, but further classification of plaque types was not possible.

Purpose: To determine whether using estimated glomerular filtration rate (eGFR) values rather than serum creatinine levels to identify patients with renal insufficiency facilitates any substantial change in the number of outpatients scheduled for computed tomography (CT) who are considered at increased risk for contrast medium-induced nephropathy., Materials and Methods: The study was HIPAA compliant and institutional review board approved for medical chart review; the requirement for informed patient consent was waived. Patients (n = 5138; 2569 women, 2569 men, 753 African Americans, 4385 non-African Americans) examined during a 2-year period formed the final study group after exclusion of patients undergoing dialysis (n = 49), for whom no age data were recorded (n = 9), and younger than 18 years (n = 113). Patient age, sex, and race and the blood urea nitrogen, albumin, and serum creatinine levels most recently measured within 6 months before CT were obtained from the electronic medical records. The number of patients with creatinine levels higher than 1.4 mg/dL was directly compared with the number of patients with eGFR values (calculated with four- and six-variable Modification of Diet in Renal Disease [MDRD] equations) lower than 60 mL/min/1.73 m(2) by using the two-tailed McNemar test. For 2689 patients, data to calculate the eGFR by using the four-variable equation were available, and for 2005 patients, data to calculate the eGFR by using the six-variable equation were available., Results: Among the outpatients scheduled to undergo CT, the percentage of patients with an eGFR lower than 60 mL/min/1.73 m(2) was significantly greater than the percentage of patients with a creatinine level higher than 1.4 mg/dL for both the four-variable (412 [15.3%] vs 166 [6.2%] of 2689 patients) and the six-variable (346 [17.3%] vs 117 [5.8%] of 2005 patients) MDRD equation groups (P < .001)., Conclusion: A significantly higher number of outpatients scheduled for contrast medium-enhanced CT met the National Kidney Foundation criteria for renal insufficiency when the MDRD equations were used to estimate the glomerular filtration rate compared with the number of outpatients who met the criteria on the basis of elevated creatinine levels., ((c) RSNA, 2008.)

Purpose: The authors performed a pilot randomized controlled trial of total-body screening to assess the feasibility of a full-scale study., Materials and Methods: After informed consent, 50 asymptomatic people were randomized to either the intervention arm (total-body screening with multidetector computed tomography) or the control arm (no screening for 3 years). The study was approved by our institutional review board and was compliant with the Health Insurance Portability and Accountability Act. Images were interpreted independently by 6 radiologists from 2 institutions. Subjects in both study arms completed periodic health questionnaires and medical utilization forms over 2 years. Key outcome variables were the incidence of symptomatic disease, medical costs, and patient-reported health., Results: Sixteen screened subjects (64%) had abnormal findings on screening. A second interpretation of the images yielded a similar overall rate but with considerable variability at the subject level. No cancers were detected. Ninety percent of subjects were compliant at 2 years. Medical costs were twice as high for screened subjects, with considerable between-subject variability. Screened subjects reported fewer physical limitations than unscreened subjects., Conclusion: A full-scale randomized controlled trial of total-body screening will need to account for the large interreader variability in interpreting the images, the high rate of incidental findings, and the low prevalence of cancers. A full-scale study using mortality as the endpoint does not seem feasible at this time.

Objective: The purpose of this study was to determine the prevalence of an abnormal colon on CT in adult inpatients with Clostridium difficile colitis, compare the clinical presentation of these patients, and determine whether CT findings predicted the need for surgical treatment., Materials and Methods: Over a 21-month period, 152 of 572 inpatients with C. difficile colitis were identified and had CT scans performed within 2 weeks of the diagnosis. These were independently and retrospectively reviewed by two reviewers. Those with colonic wall thickness greater than 4 mm were considered positive (CT-positive patients) and were further reviewed for specific findings in the colon. All 152 patients with CT scans were also retrospectively reviewed using the hospital information system for certain clinical parameters, admitting diagnoses, and reasons for scanning. The following were compared using several statistical tests: clinical parameters in CT-positive and CT-negative patients and surgical and nonsurgical groups to determine if positive scans or surgical treatment could be clinically predicted; specific CT findings in CT-positive patients to see if an association was found with clinical parameters or surgical treatment; and admitting diagnoses and reasons for scanning in scanned and unscanned populations to see which patients were more likely to undergo CT., Results: Seventy-six (50%) of 152 scanned hospitalized patients with C. difficile colitis were CT-positive. These patients most often had segmental involvement (50 [66%] of 76 patients), with the rectum (60 [82%] of 73 patients) and sigmoid colon (61 [82%] of 74 patients) most often affected. Positive scans were associated with increased WBC, abdominal pain, and diarrhea. Patients with signs and symptoms of infection or abdominal complaints were more likely to be scanned. No statistical correlation was found between specific CT findings and clinical parameters or clinical parameters and patients requiring surgery. There was no predictive value of specific CT findings for surgical treatment., Conclusion: Half of the patients scanned had an abnormal CT, with segmental colonic disease more common than diffuse. Positive scans were more likely in patients with leukocytosis, abdominal pain, and fever. Specific CT findings did not correlate with clinical parameters and could not predict surgical treatment.

Engineering advances in CT have produced multi-slice instruments that can scan large areas of the body in short periods of time, and such instruments now permit high resolution examination of entire anatomic regions (eg, the chest) in a single breath hold. Alternatively, these instruments can quickly scan small areas (such as the heart) with very high resolution in a very short period of time (eg, diastole). Using such CT scanners, there is no question that coronary artery calcium can be detected in small quantities and scored accurately. However, coronary calcium screening, like all screening procedures, poses a significant dilemma: early detection in a few is almost always accompanied by negative consequences for others (eg, false positives causing anxiety and unnecessary work-up, and false negatives causing delayed treatment and false reassurance). How do we balance the benefits to a few against the negative effects to others? That is the subject of this paper. A starting point for resolving the screening dilemma is to count the number of patients needed to be screened to benefit one patient (the NNS), and conversely, to determine the number of patients screened before harming one patient (NSH). Another approach is to apply published criteria suggested for the evaluation of a screening program targeted at early disease detection. In this review article, we propose 10 criteria for evaluating the effectiveness of a screening test designed to detect a risk factor for disease (ie, calcium scoring as a risk factor for coronary artery disease). We discuss how these criteria can be used to estimate NNS and NSH. Although this work focuses on coronary calcification screening, reference is made as well to other areas, such as lung and colon cancer screening.
