Effects were studied of hemosorption on the system of homeostasis in patients with chronic glomerulonephritis presenting with nephrotic syndrome. An optimum graphic express method of control of the homeostasis system state was found, permitting predicting thrombohemorrhagic complications after single-session hemosorption (significance 99.9%). Strategies were identified for correction of changes in coagulogrammes in such patients along with devising measures to prevent the above complications.

The article discusses the results of using the activated coagulation time in hemosorption for the first time to determine the level of heparinization in 60 patients with obstructive jaundice and myasthenia++. The method made it possible to control the blood heparinization level during hemosorption, reduce the dose of heparin administered before the perfusion to 1.2-1.5 mg/kg, prevent the development of thrombosis in the mass exchanger and, simultaneously, avoid the neutralization of heparin with protamine sulphate.

The authors used a method of graded heparinization with special reference to the individual reaction of the patients to heparin. The initial (starting) dose of heparin was injected intravenously, the sustaining dose of heparin was infused through the tube for blood taking. The method described prevented thrombosis of the HMS apparatus and considerably reduced the quantity of heparin used. No complications of hemosorption resulting from heparinization were noted.
