This is a pictorial essay in which we review and illustrate a variety of thoracic injuries related to blunt trauma. Non-aortic blunt thoracic trauma can be divided anatomically into injuries of the chest wall, lungs, pleura, mediastinum, and diaphragm. Some injuries involve more than one anatomic compartment, and multiple injuries commonly coexist. This article provides common imaging findings and discussion of both common and uncommon but critical thoracic injuries encountered., (Published by Elsevier Inc.)

The treatment results of the 286 patients with pleural empyem after thoracic injuries (n=107) and closed trauma of the pleural cavity (n=179) were retrospectively analyzed. The frequency of pleural empyem was 1.39% by injuries and 1.34% by the closed thoracic trauma. 15 (14%) patients of the first group developed the bronchopleural fistula, whereas the complication was observed in 32 (17.9%) patients of the second group. The adequate pleural drainage with intrapleural enzyme therapy in acute inflammation period allowed recovery in 78% and 71.9% of patients, respectively. Early videothoracoscopic sanation of the pleural cavity shortened the recovery time in more then 1.5 times. The chronization of the empyem was more often observed after the closed thoracic trauma - 14.5% rather than 6.5% after the open thoracic injury. The lethality rate by pleural empyem was 14% after the open injuries and 15.6% after the closed trauma.

Thoracic traumas are frequent and potentially fatal, because of the associated neurological and abdominal lesions. They are observed in car crashes, combat environments and urban terrorist bombings. The mechanisms of the traumatic injury are complex and account for the diversity of the lesions. The management of a chest trauma patient is a model of multidisciplinary collaboration where the chest physician can make a significant contribution., (Copyright © 2010 Elsevier Masson SAS. Introduction: The most frequent incidences of blunt chest injuries occur due to motorvehicle accidents, falls and work-related traumas. Chest computed tomography is usually associated with examination of the subsequent regions with the aim to enable a more efficient diagnostic procedure. The purpose of this research study is to define the contribution of the chest CT in blunt injury patients., Material and Methods: This retrospective study encompasses the results of CT examination of 36 patients with one or more injuries of the thoracic wall, pleura, lungs and spinal column suspected to suffer chest organs traumas. Chest CT survey was performed in 30 patients, while thoracic spine CT survey was performed in 11 patients., Results and Discussion: The most frequent incidences of injuries were due to motor vehicle accidents (22 patients--61.11%). Falls were the reason of injuries in 11 patients--17.46%. Chest injuries are usually associated with injuries of the surrounding regions, but occurred in isolated form, too (27.78%). Rib fractures and lungs parenchyma traumas were evidenced by CT examination in more than 60% of the patients. Thoracic spine fractures were most frequently reported at the 10-th, 11-th and 12-th vertebra and made more than 70% of the thoracic spine fractures., Conclusion: The findings of this study coincide to a great extent with the findings presented in world literature.

Background: Tension pneumothorax is a potential cause of death in victims of penetrating chest trauma, but little is known about its actual prevalence., Methods: Data that are part of the Vietnam Wound Data and Munitions Effectiveness Team study were analyzed to address this question. Radiographs of 978 casualties were examined for evidence of tension pneumothorax using standard radiologic criteria such as pleural separation, displacement of the mediastinum and diaphragm, trachea deviation, and compression of the contralateral lung., Results: Some or all of the radiographic changes were found in 198 casualties. Autopsy evidence indicated that 79 of these casualties died solely due to a chest wound. The fatal chest injury involved only the lungs in 55 casualties and caused a tension pneumothorax in 26. Fifteen of the 26 lived long enough to receive first aid from a medic or corpsman., Conclusion: Tension pneumothorax was the cause of death in 3 to 4% of fatally wounded combat casualties. Some may be temporarily helped by battlefield thoracentesis.

This article discusses the role of imaging in evaluating patients who are admitted with penetrating injuries to the chest. Emphasis is placed on the role of multidetector row CT, which has been introduced in the past 5 years into the arena of care for trauma victims. It is important to take full advantage of this new CT technology with its capability to produce high-resolution multiplanar and volumetric images to diagnose penetrating chest injuries. This article emphasizes detection of active bleeding and assessment of the mediastinum for penetrating injury.

Post traumatic pleuro-pulmonary lesions can be divided into 3 phases: Acute phase I--several days to one week--with superficial pleuro-pulmonary defects as a main cause for pneumo-hematothorax, intrapulmonary hematoma from rhexis bleeding of parenchymal vessels, tearing of bronchi and vessels. Latency phase II--1-4 weeks with complications caused by inflammatory overlap, contusion pneumonia, development of fistulae, internal aspiration. Late phase III--1 month to years--special variant of rounded hematoma, pleural inflammation for example in fistulae up to scarring fibrosis. Inflammatory complications and development of ARDS with subsequent complications in the traumatically pre-damaged pulmonary organ.

Purpose: CT is a valuable tool in assessing thoracic gunshot wounds. CT is also the method of choice in emergency, because it permits rapid depiction of bullet damage to the chest and to other body districts. This in turn permits correct assessment of the main thoracic injuries, plus adequate and prompt planning of surgical treatment or support intensive care. We report on the role of CT in diagnosing the complex pleuropulmonary, cardiovascular and thoracic wall injuries caused by gunshot wounds, with their specific and acute signs which differ greatly from those of other types of chest trauma., Material and Methods: In the last 4 years, we observed 76 cases of gunshot injury, twenty-six of them involved the chest. The patients, 25 men and 1 woman (mean age: 32 years, range: 17-48), were all submitted to emergency CT with i.v. contrast agent injection and the CT-angiography technique. The reanimator was always present to monitor the patients' vital functions and shock state. CT of the chest was integrated with CT of the abdomen and pelvis in 4 cases and with CT of the skull in 3 cases, to detect associated bullet wounds if any., Results: The most frequent CT finding was lung parenchyma tear and bruise (25 cases), followed by hemothorax (18 cases) and subcutaneous chest wall emphysema (9 cases). Pneumothorax was seen in 5 cases, associated with hemothorax in 6; rib injuries were found in 7 cases; pneumomediastinum was found in 4 cases and areas of pulmonary atelectasis in 3; the diaphragm was ruptured in 4 cases. CT showed spinal involvement in 11 patients, with injury of D3 and D5 in 4 and 3 cases, respectively; signs of interrupted spinal marrow were found in 7 cases. Damage from gunshot wounds was detected in the liver, spleen, skull and limbs in 3, 2, 3 and 10 cases, respectively., Discussion and Conclusions: Chest radiography shows major gunshot wound damage to the chest and lungs, except for heart injuries and minimal pneumothorax. When abdominal and skull injuries are associated, CT should be the method of choice because it permits prompt and panoramic assessment of the severity of pulmonary and extrathoracic damage. This results in prompt and targeted treatment, avoiding unnecessary delays which may damage the patient further.

Purpose: Computed quantification of the extent of pleuropulmonary trauma by CT and comparison with conventional chest x-ray--Impact on therapy and correlation with mechanical ventilation support and clinical outcome., Method: In a prospective trial, 50 patients with clinically suspicious blunt chest trauma were evaluated using CT and conventional chest x-ray. The computed quantification of ventilated lung provided by CT volumetry was correlated with the consecutive artificial respiration parameters and the clinical outcome., Results: We found a high correlation between CT volumetry and artificial ventilation concerning maximal pressures and inspiratory oxygen concentration (FiO2, Goris-Score) (r = 0.89, Pearson). The graduation of thoracic trauma correlated highly with the duration of mechanical ventilation (r = 0.98, Pearson). Especially with regard to atelectases and lung contusions CT is superior compared to conventional chest x-ray; only 32% and 43%, respectively, were identified by conventional chest x-ray., Conclusion: CT allows rapid classification and quantification of pulmonary lesions after thoracic trauma and provides higher sensitivity and reliability. Because of the great correlation with the extent of artificial respiration in respect of duration and pressure, prognosis of the individual patient, as well as a differential therapy, appear possible.

Objectives: To investigate the contribution of computed tomography scan (CTS) to the initial évaluation of chest trauma., Patients and Methods: We carried out a two-year prospective study in all the chest trauma patients admitted to ICU. They underwent both Chest X-ray (CXR) and CTS within 24 jours of admission. CXR and CTS images were read by achieving a consensus between two radiologists who were unaware of the results of the other investigation. Then we compared these findings with the treatment received by the patients in the ICU., Results: From July 1, 1991 through July 1, 1993, 56 patients were included (21 with conventional CTS and 35 with helicoidal CTS). CTS demonstrated a significant number of pleural (p < 0.001), parenchymatous (p < 0.001), and mediastinal (p < 0.01) lesions that escaped detection on CXR (CTS+/CXR- lesions). Thoracic drainage was done in 16 patients because of abnormalities CTS+/CXR- (p < 0.01); six patients with CTS+/CXR- pulmonary contusions received Pressure Positive Ventilation (p < 0.01); and four CTS+/CXR- lesions were treated surgically (p < 0.05)., Conclusion: CTS adds significantly to the evaluation of chest trauma by allowing prompt, accurate assessment of lesions. In this study, over 50% of these lesions required specific treatment during the ICU stay.

Trauma to the chest may cause a wide range of injuries including fractures of the thoracic skeleton, contusion or laceration of pulmonary parenchyma, damage to the tracheobronchial tree, diaphragmatic rupture or cardiac contusion. Conditions affecting primarily extrathoracic sites may have indirect effects on the lungs causing adult respiratory distress syndrome or fat embolism. Laceration of the aorta is the typical and likewise most life threatening complication of massive blunt chest trauma necessitating immediate diagnosis and repair. Conventional radiography rather than cross-sectional imaging is the mainstay in diagnosing thoracic trauma. During the critical phase with often concomitant shock, pelvic and spinal injuries tailored radiographic views or even upright chest radiographs are impractical. The severely traumatized patient is usually radiographed in the supine position and suboptimal roentgenograms may have to be accepted for several reasons. It is well documented that many abnormalities detected on CT were not apparent on conventional radiographs, but CT is reserved for hemodynamical stable patients. Nevertheless certain situations like aortic rupture require further evaluation by CT and aortography. The value of conventional radiography, CT, MRI and aortography in chest trauma is reviewed and typical radiographic findings are presented.

179 patients with penetrating chest wall and thoracic injuries were treated at the University Hospital "Rudolf Virchow" in Berlin between 1982 and 1992. Most patients (72.4%) were successfully treated with closed tube thoracostomy alone. 33 (18.4%) patients required thoracotomy, 14 (7.8%) due to injury of the great vessels and 15 (8.3%) due to cardiac injuries. In 10 (5.5%) patients also abdominal organs were compromised, so that also a laparotomy had to be done. Seven (3.9%) patients died. All but one patients who died, died in the OR or within 24 h of admission. So lethality in the group of cardiac wounds was 44.6% (7 out of 15). This underlines the necessity of most rapid transport to the hospital in case of thoracic injury.

Complete avulsion of the azygos venous arch associated with a subtotal longitudinal laceration of the posterior mediastinal pleura resulting from blunt motor vehicular trauma is reported. A sudden forceful anteromedial rotation of the decelerating cardiac mass may be the etiologic mechanism of the rare injury.

Trauma is the leading cause of death in people less than 40 years of age. Blunt and penetrating chest trauma mechanisms and the resultant injuries and complications are reviewed in respect to the chest wall, pleura, lungs, mediastinum, and diaphragm. Recent literature reviewed includes a useful sign of flail chest, the costal "hook sign." New evidence suggests that traumatic aortic rupture may be more common in the pediatric population than was previously reported. The diagnostic evaluation of traumatic aortic rupture continues to be controversial, with imaging modalities ranging from chest radiographs and angiography to CT, MR imaging, and transesophageal echocardiography. Recent integration of these methods has brought to the literature a newly proposed algorithm for the work-up of suspected aortic rupture. Survival and modifying factors in adult respiratory distress syndrome and a comprehensive review of esophageal disease, including trauma, are also presented.

Computed tomography (CT) and radiography were used to study 20 patients with chest injuries. Excluding rib fractures, CT revealed 50 traumatic lesions ranging from extrapleural hematomas to pericardial fluid collections. In contrast, only 12 lesions were detected on the corresponding radiographs. While many of the processes demonstrated on CT were not life-threatening, substantial abnormalities such as pericardial effusion and diaphragmatic rupture were detected.

In patients with chest injuries, serial rib fractures and intrathoracic lacerations the indication for primary thoracotomy is usually limited to life-threatening organ ruptures. In our own experience with 18 patients primary thoracotomy offers the chance of immediate closure of lung lacerations, suture of pleural and pericardial tears, ligature of torn intercostal vessels and stabilization of the chest wall by elastic rib clamps "Essener Model". Postoperative respiration, freedom of pain, is restored immediately and therefore complications and duration of high-risk respirator treatment can be reduced.

A prospective study designed specifically to analyze errors in management and iatrogenic complications was conducted on 234 consecutive patients with penetrating chest trauma. Eleven percent of penetrating pleural injuries were incorrectly diagnosed on initial physical and radiologic examination, but this led to wrong management decisions in only 4 percent. Decisions regarding intercostal tube drainage were particularly inaccurate when emergency intubation was required prior to chest radiography. Atypical clinical features of penetrating cardiac injuries caused delay in diagnosis and thoracotomy in 3 of 14 patients. Only two of four mediastinal vascular injuries were recognized on initial examination. Transdiaphragmatic injuries were initially missed in 5 of 14 patients. This resulted in management errors and delay in these patients. Awareness of atypical presentations and circumstances in which misjudgments and wrong decisions are prone to occur should help to avoid pitfalls and reduce mortality from penetrating chest trauma.

The decision to insert an intercostal drain in chest injury must occasionally be made without a chest radiograph. A prospective analysis of the reliability of physical examination in penetrating pleural injuries was undertaken. A total of 51 consecutive patients were examined before obtaining a chest radiograph. The presumptive diagnosis and decision to institute intercostal drainage were compared with the radiological diagnosis and the actual decision in each patient. A policy of selective drainage of large pleural collections was employed. The series consisted mainly of stab injuries in young men. Physical examination accurately diagnosed 13 of the 14 large pneumo- or haemothoraces. This reliability combined with the selective drainage policy showed that physical examination accurately predicted the need for tube thoracostomy with a sensitivity of 96 per cent and a specificity of 93 per cent. This study suggests that experienced clinicians should not hesitate to institute immediate life-saving intercostal drainage when needed, before a chest radiograph is obtained.

The authors observed 298 patients with myasthenia who had been subjected to thymectomy. Complications during operation were as follows: injuries of the mediastinal pleura, pericardium, humerocephalic vein and hemothorax. The improvement of preoperative preparation, methods of thymectomy have decreased complications from 19% to 7%.
