Abdominal aortic aneurysm (AAA) and aortoiliac occlusive disease (AIOD) are multifactorial vascular disorders caused by complex genetic and environmental factors. The purpose of this study was to define risk factors of AAA and AIOD in the Polish population and indicate differences between diseases. [ABSTRACT FROM AUTHOR]

Background. Percutaneous angioplasty of recurrent stenosis of the carotid arteries with simultaneous stent implantation is an alternative to surgical intervention. The observations made and the long-term effects of the above-mentioned treatment are the subject of this paper. Material and methods. In 2001-2004, in 20 patients with recurrent stenosis of the carotid arteries, angiography with stent implantation was performed after reboting. In the control imaging (DSA and Dopplerduplex), the technical effect of the intravascular procedure was assessed. Special attention was paid to stent dilatation, as well as to the spectrum and speed of blood flow in the carotid arteries. Nineteen patients were put under observation for 18-24 months, with control tests every 90 days. Results. One patient died shortly after the intravascular procedure. During the long-term observation period, a haemodynamically significant recurrence of stenosis was observed in 3 of 19 patients. In 2 of the patients, an additional stent was implanted. Treatment of the third patient was limited to expansion of the constricted stent. The rest of the patients displayed neither clinical nor radiological signs of carotid artery stenosis. Conclusions. Introvoscular procedures are on effective and safe method of treating recurrent stenosis of the internal carotid artery. After angioplasty and stent implantation, patients require periodic ultrasound assessment due to the possibility of stenosis recurrence, which most often affects the area of the common carotid artery neighbouring the proximal segment of the implanted stent. Penetrating atherosclerotic ulcers are a rare condition characterized by the ulceration of an aortic atherosclerotic plaque penetrating through the internal elastic lamina into the aortic media. Despite the lack of consensus on optimal management of this disorder, we successfully treated two patients with penetrating atherosclerotic ulcers using an endovascular method. We present our observations and experiences regarding endovascular treatment of penetrating atherosclerotic ulcers of the thoracic aorta. [ABSTRACT FROM AUTHOR]

Background: Severe perioperative bleedings related to major surgeries pose a serious medical problem due to surgical treatment. NovoSeven (recombinant activated factor VII -- rFVIIa, Nova Nordisk, Denmark) accelerates local haemostasis by an exogenous coagulation system, directly stimulating factor X. Material i methods: Three cases of the successful use of rFVIIa in the Department of General and Vascular Surgery of Poznan University of Medical Sciences have been described. The first case refers to the controlling of bleeding from a surgical wound in a female patient with idiopathic thrombocytopenia. The two others refer to the application of rFVIIa in female patients, who previously did not present any signs of haemostatic disorders. Results: In all the cases presented, shortly after the drug's administration, bleeding symptoms abated as confirmed by normalisation of laboratory results and haemodynamic parameters, which standard treatments had failed to produce before. Conclusions: It is important to note that ail the patients were administered a lower dose of rFVIIa than recommended by the manufacturer and described by other authors, which may indicate that lower doses of rFVIIa are also efficient. As well as that, the concomitant use of FFP and rFVIIa allows the reduction of rFVIIa dosage. [ABSTRACT FROM AUTHOR]

Wstęp. Zakażenie protez naczyniowych należy do najgrożniejszych powikłań w chirurgii naczyniowej i wiąże się z dużym ryzykiem zgonu lub utraty kończyny. Podstawowym sposobem leczenia tego powikłania jest całkowite usunięcie zakażonej protezy zjednoczasową rekonstrukcją łożyska naczyniowego. Ponieważ zabiegi te wiążą się z dużym ryzykiem, zwłaszcza w odniesieniu do osób starszych lub w złym stanie ogólnym, coraz częściej pojawiają się opinie o korzyściach związanych z zastosowaniem ograniczonych zabiegów miejscowych w leczeniu wybranych przypadków zakażenia protez. Materiał i metody. W latach 1996-2003 u 64 chorych z objawami zakażenia protez przeprowadzono 80 ograniczonych zabiegów miejscowych. Wyniki. W okresie obserwacyjnym zmarło 5% chorych, a u 7,5% konieczna była amputacja kończyn. U 92,4% pacjentów obserwowano pierwotne wygojenie ran i dorażne ustąpienie objawów zakażenia. Jednak tylko 38,7% osób, u których zastosowano terapię, trwale wyleczono. U pozostałych 53,7% w okresie 1-36 miesięcy odnotowano nawrót objawów zakażenia. Najmniejszy odsetek nawrotów dotyczył pacjentów po częściowym usunięciu protezy, bez rekonstrukcji ciągłości łożyska naczyniowego. Oprócz nielicznych przypadków nawrotu objawów zakażenia zlokalizowanego w pachwinach zauważono znaczący postęp rozległości zakażenia. Wnioski. Miejscowe zabiegi chirurgiczne należy stosować tylko u osób z ujemnym wynikiem badania bakteriologicznego lub w wypadku braku zgody na przeprowadzenie bardziej radykalnego leczenia. W pozostałych przypadkach leczenie ograniczonych infekcji w obrębie pachwin powinno obejmować usunięcie zakażonego fragmentu protezy. Niezależnie od zastosowanej techniki konieczne jest poddanie chorych regularnym badaniom klinicznym i radiologicznym w celu możliwie wczesnego wykrycia nawrotu i podjęcia rozleglejszej interwencji. Background. Vascular prosthesis infection is one of the most severe complications in vascular surgery and is connected with high risk of limb amputation or death. The most common method of treatment is the total removal of the infected prosthesis and at the same time reconstruction of blood vessels using extra-anatomical or allogenic vascular grafts. The extent of these operations makes them of high risk in patients in severe general condition. Due to this problem there is a limitation in operation extent on behalf of local therapy The aim of this work was to analyze the result of local surgical treatment of vascular graft limited infections. Material and methods. In the years 1996-2003 sixty-four patients with prosthetic infection were treated by local therapy or partial removal of the graft. Results. During the observation period, 5% of the patients died and in 7.5% limb amputation was necessary. 92.4% of the patients were primarily cured of their infection but at follow up only 38.7% patients had healed satisfactorily without symptoms of reinfection. Symptoms of reinfection appeared in 53.7% patients within 1-36 months. The infection symptoms recurrence was observed most seldom in patients group after partial removal of the prosthesis without blood vessel reconstruction. Despite 5 cases of the recurrence limited to the grain responding to local therapy techniques, all other cases resulted in the progress of infection. Conclusions. Local treatment conies a high risk of reinfection, and should only be used for culture-negative grafts or in individuals who do not accept more radical treatment. Irrespective of the treatment method used the patients should be kept under regular clinical and radiological review for the early detection of reinfection diagnosis and the application of a more radical treatment approach. Abstract: Fast and reproducible Capillary Zone Electrophoresis (CZE) method for the quantification of (+)-S clopidogrel carboxylic acid metabolite in human fluids was elaborated for the first time. Optimal buffer and CZE conditions were established to obtain the complete separation of clopidogrel, its metabolite and piroxicam (internal standard), during one analytical run. Finally, resolution of the analytes was obtained in an uncoated silica capillary filled with a phosphate buffer of pH 2.5. The analytes were isolated from plasma and urine samples using solid phase extraction (SPE). Validation of the CZE method was carried out. The calibration curve of clopidogrel was linear in the range of 0.5–10.0mg/L in plasma and urine, whereas for (+)-S carboxylic acid metabolite linearity was confirmed in the range of 0.25–20.0mg/L in plasma and 0.25–10.0mg/L in urine. Intra- and inter-day precision and accuracy were repeatable. LOD and LOQ were also estimated. SPE recovery of the analytes from plasma and urine was comparable and greater than 80%. The validated method was successfully applied in pharmacokinetic investigations of (+)-S carboxylic acid metabolite of clopidogrel following the oral administration of clopidogrel to patients prior to percutaneous coronary intervention.
