Background: Myogenic temporomandibular disorders (M‐TMD) commonly involve occlusal splint (OS) therapy and musculoskeletal physiotherapy (MPT). Objectives: To compare the effects of combining OS with MPT and education (EG) against OS and education (CG), in chronic M‐TMD patients. Methods: In this double‐blind randomised controlled trial, 62 participants were assigned to either EG or CG. The primary outcomes, pain levels at rest (VAS rest), maximum oral opening (VAS open) and during chewing (VAS chew), were measured by Visual Analogue Scale (VAS) in cm. The secondary outcome was the range of motion (ROM) for maximum oral opening. Both interventions lasted 3 months, with outcomes assessed at baseline (T0), post‐treatment (T1) and 3 months post‐treatment (T2). Results: Intention‐to‐treat analysis revealed significant improvements favouring EG (VAS rest = −1.50 cm [CI95%: −2.67, −0.32], p =.04; VAS open = −2.00 cm [CI95%: −3.23, −0.75], p <.01; VAS chew = −1.71 cm [CI95%: −2.90, −0.52], p =.01; ROM = 4.61 [CI95%: 0.93, 8.30], p =.04). Additionally, VAS measures were influenced by follow‐up times (VAS rest = −0.73 cm [CI95%: −1.30, −0.17], p = 0.03; VAS open = −0.97 cm [CI95%: −1.57, −0.37], p <.01; VAS chew = −1.15 cm [CI95%: −1.73, −0.58], p <.01). At T1, EG demonstrated higher number of responders compared to CG for VAS open (χ2(1) = 4.39, p =.04) and VAS chew (χ2(1) = 11.58, p <.01). To compare the short-term clinical results between disc repositioning by open surgery (DROS) and stabilization splint (SS) for the treatment of temporomandibular joint (TMJ) anterior disc displacement without reduction (ADDwoR). Patients diagnosed with ADDwoR by magnetic resonance imaging (MRI) and treated by either DROS or SS from June 2020 to January 2022 were reviewed. They were selected to match the baseline data before each treatment and then to compare the clinical results including maximum interincisal opening (MIO), protrusion and lateral excursion distances, visual analogue scale (VAS) scores of pain, diet, and quality of life (QoL) and their effective rates, total satisfaction rate after 3 and 6 months of the treatment, and between the 2 treatment methods. SPSS software was used to analyze the statistical differences. A total of 175 patients were included in the study. There were 95 patients in the DROS group and 80 patients in the SS group. MIO and VAS scores were significantly improved after each treatment at 3 and 6 months (p < 0.05). DROS improved more at 6 months than 3 months (p < 0.001), whereas SS had no significant changes (p > 0.05). DROS had significantly better MIO, VAS score and effect rates than SS (p < 0.001). The total satisfaction rate was 94.73% in DROS and 86.25% in SS. The protrusion and lateral excursions decreased after DROS at 3 months (p < 0.001) but improved at 6 months to better than before the surgery (p = 0.03, p = 0.10). There were no significant changes in protrusion and lateral excursions after SS (p > 0.05, p > 0.05). Background: Disc displacement with reduction (DDwR) is among the common disc disorders of temporomandibular joint (TMJ), which can be managed conservatively by splint therapy. Anterior repositioning splint (ARS) is the most commonly prescribed splint by dental practitioners, but not getting a normal disc– condyle relationship always and other side effects lead to need of comparing with other occlusal splints. This review will help in informed decision-making by clinicians in choosing an appropriate splint type for patients. Aim: The aim is to compare the effectiveness of ARS in the management of DDwR with other occlusal splints for TMJ and muscle pain, TMJ noise, any adverse effects, regaining normal disc–condyle relationship. Materials and Methods: We followed published protocol in the International prospective register of systematic reviews. Databases were searched till May 2023 using different search strategies as per the database. Title and abstract screening, followed by full-text screening and data extraction with risk of bias, was done by two independent reviewers in Covidence. Outcomes were reported as risk ratio (RR) or mean difference (MD) for dichotomous or continuous outcomes, respectively, using RevMan 5.4 (Review Manager 5.4) software. We used a random effect model for statistical analysis. Certainty of evidence was assessed using the Grading of Recommendation, Assessment, Development, and Evaluation Guideline Development Tool (GRADEpro GDT) software. Results: A total of 1145 reports were found from a database search. After screening, four studies were included for systematic reviews. Other occlusal splints reported were sagittal vertical extrusion device and mandibular ARS, full hard stabilization splint of canine or centric stabilization type. Data of only two studies could be used for meta-analysis having 30 participants received ARS and 40 received other occlusal splints. We did not find evidence of any difference between ARS and other occlusal splints for TMJ clicking in short term (RR 1.25, 95% confidence interval [CI] 0.91-1.72) but a small difference in favor of other occlusal splint in long term (RR 2.40, 95% CI 1.04–5.55). No evidence of any difference was found between both treatments for TMJ pain in short term (MD-5.68, 95% CI-17.31–5.95) and long term (MD 0.00, 95% CI-2.86–2.86) and muscle pain in short term. The certainty of evidence for comparison of two treatments for different outcomes was of low or very low level. Conclusion: Evidence is uncertain that other occlusal splints reduced TMJ clicking slightly in comparison to ARS. For the remaining outcomes, no evidence of any difference was found between the two splints and it may be biased due to selection bias, inadequate blinding of participants, and outcome assessor. Background: The term "temporomandibular disorders" (TMDs) refers to a group of painful conditions that affect both the hard and soft orofacial anatomical structures. The primary objectives of TMD treatment are to lessen pain and enhance mandibular movements, re-establishing proper functioning. One of the widely used treatment options for TMDs pathology is the occlusal splint. This non-invasive intraoral device, determines both the muscle relaxation and reversibly repositions the temporomandibular joint structures. The study aimed to compare the efficacy of different occlusal splint designs or materials in managing TMD symptoms. Methods: Twenty-six patients, nineteen of whom were female and seven of whom were male, ranging in age from twenty to seventy, met the criteria needed participate in this study. Two different questionnaires were formulated: a patient-focused and a clinician-focused questionnaire. The patient's data encompassed general health status and TMD signs and symptoms, whereas the clinician's questions addressed assessment and treatment alternatives, including the particularities of occlusal splint therapy. Results: The majority of the patients were diagnosed with disc displacement with reduction (61.5%), followed by myalgia (26.9%). The only form of treatment preferred by physicians was the Michigan occlusal splint, which was mainly used on the maxillary dental arch (84,6%). After the splint therapy was completed and the intended results were obtained, the treatment plan was finished using selective grinding, prosthetic dental restorations, orthodontic therapy, or a combination of these procedures. The patient was required to continue physical therapy (7,7%), wear a full-arch night guard (42.3%), and undergo rigorous periodic control sessions (34.6%) to maintain the final aesthetic and functional results. Conclusions: Occlusal splints are an efficient and reliable initial therapy preference for patients with temporomandibular disorders. In addition to improving symptoms in the temporomandibular joint, occlusal splints may determine mandibular repositioning, which also implies a variety of treatment approaches, ranging from the minimally invasive to the most complex, to achieve a functional occlusal status. Background: Anterior repositioning splint (ARS) is used to treat temporomandibular joint (TMJ) disc displacement with reduction (DDwR). However, high recurrence rate remains a problem especially in patients with unstable occlusions. Objective: This study optimised standard ARS therapy and proposed a step‐back ARS retraction (SAR) method in adult patients with DDwR. Methods: Dental examinations and magnetic resonance imaging of TMJ were obtained before treatment (T0), 1 to 3 months (T1), 3 to 6 months (T2) and 6 to 12 months (T3) during treatment in 48 adults (average age 27.1 ± 5.7 years). After 3 months of basic ARS wearing, personalised treatment for patients with normal disc–condyle relationship was prescribed depending on bilaminar zone adaptations and severity of molar openbite. SAR which required sequential ARS wearing was designed for patients with deep overbite/overjet until retrodiscal tissue adaptations and stable occlusions were achieved. Results: The maximum interincisal opening was increased from 44.3 ± 6.9 to 45.3 ± 6.3 mm (p <.01), and joint pain was alleviated after ARS treatment. The overall success rate of ARS wearing was 92.1% (58/63) featured by a recaptured disc. Fifteen patients who underwent SAR therapy all showed bilaminar zone adaptations in the end, and one patient had positive condylar bone remodelling. Conclusions: ARS treatment could improve mouth opening and joint symptoms in adult DDwR patients. The objective was to compare different dental splint models and materials for inducing abnormal loading on the gross morphology and histological appearance of the mandibular condylar processes of Sprague Dawley rats. Three different types of dental splints (resin molar, aluminum incisor, stainless-steel incisor) were placed unilaterally to induce occlusal perturbation for 4 weeks. At that time, mandibular condylar processes were assessed by gross appearance and histology. Quantitative measurements were also conducted on the hematoxylin & eosin images for condyle shape. The results showed that although the condylar cartilage was affected by all splint types, the resin molar splint was associated with the most extensive mandibular condylar process remodeling, which was primarily a slant (skewness) of the lateral aspect of the condylar process. Additionally, quantitative measurements on the histological specimens demonstrated that the split and tilt angle of the left (ipsilateral) condylar processes in the resin molar group (124.8 ± 12.7° and 104.1 ± 12.7°, respectively) increased significantly (p < 0.05) when compared to right (contralateral) condylar processes (104.7 ± 5.8°and 91.6 ± 4.4°, respectively). However, no changes were noted on the thickness of the fibrocartilage layer at medial, central, and lateral regions of the condylar process. Another major finding is the high variability of morphology of the naïve animals. Future studies will assess the impact of longer durations of splinting, age, and sex on the remodeling of the mandibular condylar process, allowing for the development of diagnostics and therapies.

Objective: Conservative treatment modalities are recommended for managing masticatory myalgia in individuals with temporomandibular disorders. The aim of this study was to retrospectively review and compare the effectiveness of four conservative treatments: counseling and occlusal splint therapy, counseling and manipulation integrated with electrophysiotherapy, the combination of the two treatments, and counseling only. Method and materials: One hundred and sixty-eight patients who had myalgia with limited jaw movement were retrospectively observed in this study. Between January 2015 and December 2017, 63 patients received counseling and stabilization occlusal splint therapy (Group 1), 35 patients received counseling and manipulation integrated with electrophysiotherapy (Group 2), 33 patients received the combination of counseling, splint therapy, and manipulation integrated with electrophysiotherapy (Group 3), and 37 patients received counseling only (Group 4). All subjects were followed up for 12 weeks. The intensity of spontaneous pain, palpation pain, chewing pain in the masticatory muscles, and range of pain-free maximal mouth opening were recorded in the clinical assessments. Intragroup and intergroup differences were examined by using analysis of variance (ANOVA) and the Kruskal-Wallis test. Results: Spontaneous pain in the masticatory muscles was relieved significantly in all groups at the 6-week visit (P < .05), and no significant difference was found among the groups (P > .05). Palpation pain was relieved significantly at the 9-week visit in the counseling + occlusal splint therapy group, counseling + manipulation + electrophysiotherapy group, and counseling + occlusal splint + manipulation + electrophysiotherapy group (P < .05). In the treatment group with counseling alone, significant palpation pain relief occurred at 12 weeks. Chewing pain was relieved significantly at the 6-week visit in the counseling + occlusal splint therapy group, counseling + manipulation + electrophysiotherapy group, and counseling + occlusal splint + manipulation + electrophysiotherapy group (P < .05), yet no significant difference compared to the baseline was observed in the counseling-only group (P > .05). A significant increase in the maximal range of pain-free mouth opening was observed at the 9-week visit in the counseling + occlusal splint therapy group, and at the 3-week visit in the counseling + manipulation + electrophysiotherapy group and counseling + occlusal splint + manipulation + electrophysiotherapy group (P < .05). Nevertheless, no significant change in the range of mouth opening was found throughout the follow-up period in the counseling-only group (P > .05). Conclusion: Each of the included treatment modalities relieved spontaneous pain and tenderness to palpation of the masticatory muscles during the follow-up intervals. Counseling alone did not help patients with chewing pain and limited mouth opening in the short term. Arthrocentesis for arthralgia of the temporomandibular joint (TMJ) is often only indicated when conservative, non-surgical interventions have failed. However, performing arthrocentesis as initial therapy may facilitate earlier and better recuperation of the joint. The aim of this study was to assess the efficacy of this therapy with a long-term follow-up. Eighty-four patients were randomly allocated to receive either arthrocentesis as initial treatment (n = 41) or non-surgical intervention (n = 43). Pain (100-mm visual analogue scale, VAS) and mandibular function impairment questionnaire scores (MFIQ, 0–100) were recorded at 3, 12, and 26 weeks, and ≥ 5 years (median 6.2, interquartile range 5.6–7.4 years). Univariable analyses were performed and linear mixed-effect models were constructed. Patients in the arthrocentesis group experienced significantly lower TMJ arthralgia compared to those treated non-surgically (pain during movement: −10.23 mm (95% confidence interval −17.86; −2.60); pain at rest: − 8.39 mm (95% confidence interval −13.70; −3.08)), while mandibular function remained similar in the two groups (MFIQ −2.41 (95% confidence interval −8.61; 3.78)). Of the final sample, 10 patients (10/39, 26%) in the non-surgical intervention group and two patients (2/34, 6%) in the arthrocentesis group received additional treatment during follow-up. Thus, initial treatment with arthrocentesis reduced TMJ arthralgia more efficaciously than non-surgical intervention in the long term, while maintaining similar mandibular function. Aims and Objectives: The study was carried out to evaluate the efficacy of four conservative therapeutic modalities on the mandibular range of motion (MRM) in subjects with anterior disc displacement with reduction (ADDwR) of the temporomandibular joint (TMJ). Materials and Methods: One hundred patients (64 women and 36 men) were selected, and randomly distributed into four groups. Group I: Subjects receiving behavioral therapy (BT). Group II: Subjects receiving low-level laser therapy (LLLT). Group III: Subjects receiving maxillary anterior repositioning splint (MARS). Group IV: Subjects receiving stabilization splint (SS). The MRM was evaluated for each patient before treatment and after 6 months. Paired t test and one-way analysis of variance (ANOVA) tests were used for statistical analysis followed by a post hoc Tukey test (P ≤ 0.05). Results: All groups showed significant improvement in MRM after 6 months of treatment (P ≤ 0.05) except for BT. There was a significant improvement for SS and MARS on the different movements of MRM, more than for LLLT and BT (P ≤ 0.05). Occlusal splints are commonly used in temporomandibular joint disorder in conjunction with other treatment modalities. Their use in the postoperative setting has not been extensively researched. This study was carried out by creating a computer stereolithography (STL) model to measure stresses on the condyle during clenching and comparing the results between permissive and nonpermissive splints, in addition to no splint use. The results showed maximal stress at the condyle for both splints, but a decreased stress of 14.7 MPa for the nonpermissive splint versus 20.2 MPa for the permissive splint and 23.4 MPa when no splint was used with a bite force of 457 N. These results indicate that the nonpermissive splint may be beneficial in the postoperative setting to decrease stress on the condyle. [ABSTRACT FROM AUTHOR]

Occlusal splints are used as a non-invasive treatment for patients diagnosed with temporomandibular disorders (TMD). Another non-invasive treatment option for TMD patients is kinesiotherapy, which may be used alone or in conjunction with occlusal splint therapy. The aim of this study was to evaluate the changes in the intra-articular spaces of the temporomandibular joint (TMJ) after occlusal splint therapy combined with cranio-temporomandibular kinesiotherapy in TMD patients. Twenty-four patients (N = 24) diagnosed with TMD according to Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) were included. Cone beam computed tomography (CBCT) images were taken before and after treatment. The anterior, superior, posterior, and medial joint spaces were measured on the CBCT images, and the condylar ratio was calculated using Pullinger and Hollender's formula. Additionally, the thickness of the glenoid fossa (GFT), condylar medio-lateral widths (MLW), and condylar height (HCo) were measured. The condylar ratio was significantly higher after treatment (p = 0.049). The changes in dimensions of the anterior, superior, posterior and medial joint spaces were not statistically significant after treatment. No statistically significant differences were found in the dimensions of the GFT, MLW, and HCo post treatment. The condylar position did not shift anteriorly in a statistically significant way after occlusal splint therapy combined with cranio-temporomandibular kinesiotherapy in TMD patients. Poly‐(etheretherketone) (PEEK) began to be used in the field of odontology more than 10 years ago, especially in relation to the creation of removable partial dentures. Here we report the case of a 62 years old woman diagnosed with histamine intolerance (or histaminosis), who presented a very particular set of oral symptoms. She described a certain tingling, burning, and swelling of the mucous membranes. These symptoms seem to be linked with the wearing of a resin occlusal splint which was initially prescribed to compensate for the absence of a meniscus in the left temporomandibular joint of the patient. After a multidisciplinary concertation, it was decided to create a new splint with a resin‐free material. For this reason, the production of a PEEK prosthesis was considered. Following the installation of the occlusal splint in the patient, and after clinical adjustments, she described the occlusion positioning as correct and a disappearance of the symptomatology. Mechanically, PEEK seems to have higher mechanical resistance than PMMA. Despite these characteristics, the use of PEEK still presents some limitations, especially concerning the overall aesthetic. Additionally, the prescription of the occlusal splint seems to be limited to patients who need special care. This article presents the case of a 32-year-old woman with two distinct temporomandibular joint pathologies: right temporomandibular joint arthralgia, headache, disc displacement with reduction and intermittent locking, and limited opening, whereas the left temporomandibular joint was showing arthralgia and subluxation. A neurologist was regularly consulted and treated the patient for migraine. A detailed clinical assessment was used in the therapeutic approach. Cone beam computed tomography (CBCT) was used in the paraclinical assessment to evaluate the temporomandibular joint (TMJ); CBCT imaging revealed condylar bone changes that were not correlated with the clinical symptoms. To alleviate TMJ symptoms, the treatment plan included anti-inflammatory drugs, physiotherapy and an occlusal splint with lateral guiding ramps. After three months of anti-inflammatory medication, physiotherapy and splint wear, an improvement in the migraine symptoms and enhanced life quality was reported. The variation in pathology between the right and left joints and the extended history of temporomandibular disorder onset with no definitive diagnosis or therapy make this case unique. Temporomandibular joint disorder is a term for a group of musculoskeletal disorders and neuromuscular conditions that includes several clinical signs and symptoms involving the mastication muscles, temporomandibular joint, and related structures. The reported prevalence of temporomandibular joint disorders in infants, children, and adolescents varies widely in the literature. Occlusal splint is one type of oral splint that is often used to treat temporomandibular joint disorders. The purpose of this literature review is to analyze the effectiveness of occlusal splints for treating temporomandibular joint disorders in children and adolescents. Data collection was carried out by searching and analyzing literature from the electronic data site Pubmed, Wiley Online Library, Science Direct and manual search on the publication period from 2011 to 2021. Systematic data search using keywords: Temporomandibular joint disorders, children, adolescents, occlusal splints, treatment of temporomandibular joint disorders. The occlusal splint is an effective device for treating temporomandibular joint disorders with symptoms of myofascial pain and TMD with a history of trauma. Occlusal splints are not effective for disc displacement of the temporomandibular joint due to Class II malocclusion in adolescents. [ABSTRACT FROM AUTHOR]

Objectives. This systematic review aimed at assessing the therapeutic efficacy of computer-assisted or digitally constructed occlusal splints in comparison to conventional splint treatment for temporomandibular disorders or bruxism. Material and methods. The study was prospectively registered in the Open Science Framework. Four electronic databases, PubMed, Embase, Web of Science, and Scopus, were searched comprehensively. The following keywords were employed: “3D-printed”, “additive manufacture”, “computer-aided design/computer-aided manufacturing”, “temporomandibular joint”, “temporomandibular joint dysfunction”, “bruxism”, “disc displacement”, “temporomandibular disorder”, “splint’, “oral splint”, “occlusal splint”, “occlusal device”, “bite splint”, “occlusal appliance”. Two risk of bias evaluation instruments were used to assess the quality of the included studies. Outcomes. Following the application of the search strategy, a total of 557 publications were identified in the electronic databases. Seven eligible articles were finally included in the analysis. Six publications (85.7%) compared digitally manufactured occlusal splints to conventionally created splints, while one examined if the use of a facebow influences the performance of digital splints. Visual assessment scores or numerical rating scales of pain, optical axiography, tooth wear, and bruxism frequency were reported as outcomes. Conclusions. Computer aided design occlusal splints provide equivalent outcomes to traditional splints. Background/Purpose: This study aimed to compare the effect of four approaches in the treatment of TMJ disc displacement without reduction (DDwoR). Materials and methods: Thirty-two patients (40 joints) with mean age 28.025 ± 7.18 (23 female patients and nine male patients) were assigned randomly into four groups (10 joints in each group). Group I, patients were treated by centric splint. Group II, patients were treated by distraction splint. Group III, patients were treated by arthrocentesis and centric splint, while group IV patients were treated by arthrocentesis and distraction splint. The groups were compared in terms of joint function (mouth opening), joint pain through joint palpation, and use of visual analog scale (VAS). These records were taken preoperatively, two weeks, one month, three, and six months postoperatively. Also, the presence of disc recapture was evaluated in all patients on MRI at the end of the treatment period. Results: Significant improvements in all parameters were recorded in all groups. At two weeks postoperatively, there was a significant improvement in all parameters in group III and group IV than group I and group II, while there was no statistical difference between group III and group IV. Regarding mouth opening and joint palpation, there was a significant improvement in group III than group I and group II. Also, there was a significant improvement in group IV than group II at the subsequent follow-up periods. Regarding VAS, at one and three months postoperatively, there was a significant improvement in group III than other groups. Conclusion: However, both types of splints provide better results without a statistical difference; the simultaneous application of arthrocentesis and occlusal splint decreases pain and improving the function effectively and more rapidly.

The aim of the study was to investigate and compare short and long-term effects of occlusal splints (OS), ultrasound (US), and high-intensity laser therapy (HILT) in patients with painful temporomandibular joint (TMJ) disc displacement with reduction (DDWR). This prospective, randomised, single-blinded, controlled clinical study was conducted on patients with DDWR at a university oral and maxillofacial surgery clinic. A total of 140 patients were allocated randomly to four groups (OS, US, HILT, and control), with 35 patients in each. Patients were evaluated for pain, range of motion of the jaw, disability, and quality of life. A total of 132 patients completed the study. In all treatment groups (OS, US, and HILT), a significant improvement was observed in terms of pain, function, disability, and quality of life, at both weeks four and 12 compared with the control group (p < 0.001). Improvements in VAS pain and maximum mouth opening were not significantly different between the treatment groups. However, compared with the OS group, there was a significant improvement in the HILT and US groups in terms of total Oral Health Impact Profile (OHIP-14) and Jaw Functional Limitation Scale-20 (JFLS-20) scores at week four, but no difference between the groups at week 12. The results of this study show that OS, US, and HILT are effective treatments for pain and functional jaw movements in patients with DDWR. Our goal is to show that temporomandibular disorder (TMD) patients with orthopaedic instability can be effectively treated by the combination of occlusal splint therapy and molar intrusion. Diagnostic records of 18 patients reporting previous TMD and treated with splint therapy were evaluated. Postsplint anterior open bite was treated by skeletally anchored molar intrusion. Changes in overjet (OJ), overbite (OB) were measured on articulator mounted models: initially in maximal intercuspidation (MI), centric "de jour", postsplint centric relation (CR) and postintrusion CR. Changes in ANB (A point-Nasion-B point) angle, mandibular plane–palatal plane angle and facial axis angle were assessed on lateral cephalograms. Morphological changes of the condyle were detected on pre-and posttreatment CBCT images. When compared screening mountings to MI models, significant differences were found in OJ and OB. Following splint wear, there was a significant increase in lower facial height and significant decrease in facial axis angle, which in turn increased ANB angle. OB and OJ showed a significant change on the postsplint mountings when compared to MI. After intrusion, mandible exhibited counterclockwise rotation, which decreased lower facial height, increased OB and facial axis angle and decreased ANB and OJ. Posttreatment CBCTs confirmed improved condylar morphology.Occlusal splint therapy followed by orthodontic molar intrusion provides MI-CR harmony, therefore, it seems to be an effective method for treating TMD patients. The purpose of this study was to evaluate a modified method of interpositional arthroplasty for post-traumatic temporomandibular joint ankylosis. All patients were treated with a modified interpositional arthroplasty that included: navigation-assisted accurate bone dissection with minimal removal of only 5 mm of the ankylosed bony mass, novel application of bone wax and porcine acellular dermal matrix to prevent re-ankylosis, and a unique 3D-printed splint for occlusal stabilization and gap maintaining. The pre- and post-operative physical and radiological examinations of patients were recorded during routine follow-up visits. Postoperative follow-up visits lasted at least 12 months. Twelve patients, seven males and five females, ranging from 21 years to 59 years, were enrolled in this retrospective case series. All of the twelve patients with eighteen bony ankylosed temporomandibular joints were treated by our new method. The post-operative follow-up periods ranged from 1 year to 4 years. During the follow-up visits within at least 1 year, no one manifested re-ankylosis. The mean maximum incisor opening changed from 7.4 ± 5.3 mm (p < 0.001, before surgery) to 37.6 ± 3.9 mm (p < 0.001, last follow-up visit). No sign of post-operative infection or foreign body rejection was observed during the follow-up visits. The post-operative occlusal relationship was sound and stable. This study aimed to compare the effect of four approaches in the treatment of TMJ disc displacement without reduction (DDwoR). Thirty-two patients (40 joints) with mean age 28.025 ± 7.18 (23 female patients and nine male patients) were assigned randomly into four groups (10 joints in each group). Group I, patients were treated by centric splint. Group II, patients were treated by distraction splint. Group III, patients were treated by arthrocentesis and centric splint, while group IV patients were treated by arthrocentesis and distraction splint. The groups were compared in terms of joint function (mouth opening), joint pain through joint palpation, and use of visual analog scale (VAS). These records were taken preoperatively, two weeks, one month, three, and six months postoperatively. Also, the presence of disc recapture was evaluated in all patients on MRI at the end of the treatment period. Significant improvements in all parameters were recorded in all groups. At two weeks postoperatively, there was a significant improvement in all parameters in group III and group IV than group I and group II, while there was no statistical difference between group III and group IV. Regarding mouth opening and joint palpation, there was a significant improvement in group III than group I and group II. Also, there was a significant improvement in group IV than group II at the subsequent follow-up periods. Regarding VAS, at one and three months postoperatively, there was a significant improvement in group III than other groups. However, both types of splints provide better results without a statistical difference; the simultaneous application of arthrocentesis and occlusal splint decreases pain and improving the function effectively and more rapidly. Background: Posterior disc displacement (PDD) of the temporomandibular joint (TMJ) is quite rare. The normal position of a disc is slightly anterior to the condyle, so the chances of disc displacement to the posterior direction are low.Case Presentation: A 71-year-old Japanese female reported with a chief complaint of lack of occlusion on the right-side molar teeth. Clinical examination revealed lack of occlusal contact in the right posterior region, nearly normal range of mouth opening (41.0 mm), masseter muscle tenderness, and deviation to the left on opening. MRI of the right TMJ showed PDD in both the open- and closed-mouth position and joint effusion. Conservative treatment with a non-steroidal anti-inflammatory drug, stretch exercise with fingers, and stabilization splint improved symptoms for 9 months.Conclusion: Conservative treatment was effective in managing the patient. Aim: To review the accuracy of thickness of occlusal splints, used for particular temporomandibular joint derangement based on MRI findings. The occlusal splint is one of the methods of treatment of discrepancies between the centric relation and maximal intercuspation (CR/MI), and other temporomandibular joint (TMJ) disorders. It is also a method of reducing the effects of bruxism. Designing an occlusal splint for a given relation between the maxilla and the mandible involves: creating partial surfaces, integrating them, and producing the splint on a 3D printer. The paper presents and compares some techniques used to design splint surfaces under a required therapeutic maxilla-mandible relation. Objective: This study treated patients with temporomandibular disc displacement with reduction (with pain, limited mandibular movement, and clicking sound symptoms) using a combination of analgesics, injection, mandibular exercise, and occlusal splints. Method and materials: Twenty-five patients with temporomandibular joint (TMJ) disorders were evaluated for pretreatment complaints and clinical findings, such as TMJ sounds, mandibular deviation, limited mouth opening, and bilateral magnetic resonance imaging results. Diagnostic treatment was then planned; all patients received occlusal splints, and 10 patients received injections. Results: Evaluations were conducted 1 year after the initial diagnosis and treatment. Complaints (especially of pain} by the patients who received injections had reduced significantly. After 6 months of occlusal splinting, clinical findings of patients with TMJ disc displacement had greatly improved. To manage parafunctional habits of the patients, night plate usage was continued. Therefore, clinical symptom reduction was maintained. Conclusion: The use of mandibular manipulation technique can decrease the anterior disc displacement of the TMJ. The aim of the study was to improve occlusal parameters, normalization of masticatory muscles function by correcting re-prosthetic treatment of patients with partial and total ceramic prostheses using different occlusal splints. The algorithm of re-prosthetic approach to occlusal adjustment and reconstruction of mandible position in patients with iatrogenic occlusal disorders was proposed. The clinical groups included 45 patients with iatrogenic occlusal disorders who used removable and fixed ceramic dental prostheses with simplified or detailed design of occlusal surface. The time of treatment using different occlusal splints was from 2 to 3.5 months what had been mentioned in patients' past case histories. Symptomatic and pathogenetic therapy was provided for additional correction of temporomandibular joint (TMJ) functions by anti-inflammatory drugs, vitamins C, D and calcium food supplements, kinesiotherapy and magnetotherapy. It was 73.6% rate of satisfactory results of treatment. Such algorithm of re-prosthetic treatment allows to normalize bioelectric activity of masticatory muscles, functional status of TMJ and occlusal relations of the teeth. Occlusal splints, also known as bite guards, oral orthotics, and oral appliances, are utilized frequently in dental practices. They are commonly used to relax jaw muscles, prevent temporomandibular jaw trauma, protect dentition, and control headaches. How well occlusal splints work is another matter. Numerous studies, when taken as a whole, suggest only a modest indication that they are useful. This review applies principles of neurophysiology to analyze the usefulness of several types of occlusal splint in relation to their design and function. Background: Patient suffering from occlusal abnormality is usually detected months or even years when the acute patient visits a dentist, and generally the patient does not receive direct treatment upon his complaints since minimum information is available on this type of treatment. In general, the dentist provides medication only or conducts incorrect selective grinding where in fact, the patient does not feel better from the previous conditions. Purpose: The objective of this study is to discuss the treatment on the dysfunctional temporomandibular joint followed by orofacial pain caused by occlusal disorder using occlusal splint. Case: In this case, a forty three years old male having trouble with the joint on the left jaw followed by orofacial pain caused by occlusal disorder. Case Management: Initial treatment with occlusal splint makes the patient comfortable and recovers from his complaints since the patient could restructure the chewing muscles. This treatment will be more successful if the dentist has the knowledge to use and choose occlusal splint method properly. Occlusal Splint could be used as a supporting therapy and consideration as one of the therapies to avoid the unwanted side effects. The use of occlusal splint is meant as an alternative of the main therapy in overcoming the problem of occlusal splint. Conclusion: Finally, therapy with occlusal splint is very effective as an alternative treatment to handle the dysfunction of temporomandibular joint caused by occlusion.

This study evaluated the effect of auricular acupuncture (AA) on physical (PA) and emotional (EA) aspects of patients with temporomandibular disorders (TMDs) and compared the effect of AA with those of Sham and occlusal splints (OS). In accordance with the CONSORT guidelines, patients with TMDs were evaluated by axes I and II of the RDC-TMD. The patients were allocated among three groups: OS, Sham and AA; and followed for 8 weeks. Both intragroup and intergroup evaluation for quantitative variables were analyzed with ANOVA, one-way, followed by the Tukey test. Qualitative variables were analyzed with the Kruskal–Wallis, and Dunn tests (intergroup evaluation) and for intragroup analyses, the Friedman and Wilcoxon tests were applied. At the end of the experiment, the chi-squared test was applied to compare the three groups with respect to the number of variables that had shown improved scores. The adopted level of significance for all tests was α = 5%. Intragroup analysis showed that, in the Sham group, no improvement in performance was seen in either EA and significant was improvements in 5 variables for 9 for PA, while in the AA group, significant improvements were observed in 2 variables for EA and in 9 for PA. In the OS group at the end of this study, significant improvements were observed in 8 and 8 variables for EA and PA, respectively. The analysis of therapies used revealed that the number of variables exhibiting significantly improved outcomes after treatment was similar between the AA and OS groups, whereas the sham group showed statistically significant differences when compared with the other two groups. In conclusion, this study demonstrated that AA applied at a specific TMJ ear point was able to bring about improvements in patients with TMDs, with outcomes similar to those in patients treated with an OS. INTRODUCTION: Due to the physical stresses to which they are subjected, military pilots may experience bruxism, an "oral parafunction." Parafunction can cause masticatory muscle suffering and serious dental, periodontal, and temporomandibular joint damage. The aim of this pilot study was to analyze the temperature distribution in masticatory and upper trapezius muscles in a sample of bruxist air force pilots, to evaluate whether an occlusal splint would be able to induce skin temperature variations in the stomatognathic apparatus using the technology of infrared thermography. METHODS: A total of 11 male Italian Air Force pilots of high performance aircraft, ages from 27 to 40 yr (mean 34.91 ± 2.15 yr) with 1000-3000 flight hours, were enrolled in the study and analyzed using an infrared camera in order to evaluate the temperature of the masticatory muscles. All the recordings were taken on each subject using the same protocol with and without a temporary occlusal splint. RESULTS: The occlusal splint statistically increased each muscle temperature (0.10-0.20°C) on both the sides of the body. No statistically significant differences were found between the left and right muscles (asymmetries) before or after the wearing of the splint except for the anterior temporalis muscle. No significant improvement or variations in temperature symmetry of this muscle was found after the application of the splint. Background: The mechanism of action of occlusal splints used for the successful treatment of temporomandibular disorders (TMD) remains unclear and controversial. Aim: The aim of this study was to observe the mandibular response during sleep bruxism (SB) on the elimination of occlusal influences by using a flat anterior and lateral guidance splint (FGS). Material and method: Any changes in mandibular movement patterns and condylar position with the introduction of this tool were measured. Current SB activity on the natural dentition was evaluated using a Brux Checker® (BC) and compared with the activity after insertion of an FGS in 153 subjects. Result: The spatial mandibular position changed individually with a tendency toward forward and downward movement. The insertion of an FGS led to a change in the topographical condyle-fossa relationship and seemed to create an “unloading” condition for the temporomandibular joint. It was found that increased angulation of the maxillar incisors was responsible for altered muscular activity during sleep. Conclusion: The masticatory organ appears to self-regulate and to provide an oral behavior modification, which may be more physiological using the FGS as a compensating factor. In this context, it is assumed that sleep bruxism in terms of parafunctional activity is a physiological function of the masticatory organ. The research question was: do the mandibular condyles change their position within glenoid fossae after treatment combining occlusal splint therapy and physiotherapy in patients diagnosed with temporomandibular disorders (TMD)? Forty patients with TMD were included into the study. They underwent initial physiotherapy, and a six-month treatment of occlusal splint therapy with physiotherapy. Cone-beam computed tomography images of temporomandibular joints (TMJs) were taken before and after the treatment. The control group consisted of 15 asymptomatic patients, who did not receive any type of occlusal treatment. The changes in the dimension of anterior, superior, posterior, and medial joint spaces after the end of the treatment in patients with TMD were statistically insignificant. The average value of condylar ratio was significantly higher after the end of the treatment (p = 0.025). The changes in the condylar sagittal position were statistically insignificant. Occlusal splint therapy with physiotherapy did not change significantly the dimension of joint spaces, nor placed the mandibular condyles into the centric relation. Treatment of patients with TMD should not aim at gnathological concept of placing the mandibular condyles into centric relation, because centric relation appears not to be mandatory to achieve successful results of treatment in patients with TMD. Aim: To evaluate the effect of hard occlusal splint for treatment of the patients of temporo-mandibular joint symptomatology - pain and mouth opening. Methods: On evaluation and examination, patients with temporo-mandibular disorders were selected whose impression was taken using Biostar material which was hard in consistency with 0.5mm x 125mm thickness was used to prepare occlusal splint. A total of 10 patients who were included in the study were assessed for pain using visual analog scale and improvement in mouth opening before and after splint therapy for 3 months. The mouth opening and pain were measured before using the splint. The prepared splint was delivered to patients. The patients were advised to use the splint regularly for at least 12 hours a day particularly during night. The mouth opening and pain assessment were done at one month, two month and three month follow up and were compared with base line values. Results: The mean mouth opening at baseline was 35.8±2.39 mm which increased gradually and significantly at each follow-up visit. There was a significant increase in mouth opening by 4.2 mm and 4.0 mm at 3rd month when compared to baseline and 1st follow up respectively. There were no subjects free of pain at baseline while at the final visit, most (80%) of the subjects were free of pain and only 20% patients had mild pain. Brain functional magnetic resonance imaging (fMRI) was used to evaluate the effect of malocclusion therapy on the brain activity of a patient with malocclusion. A 29-year-old female patient had difficulty opening the mouth with occasional pain in the left temporomandibular joint (TMJ) and a maximum opening of only 29 mm. The treatment objectives included repositioning the displaced mandible and establishing occlusal support. Orthodontic treatment took 14 months. Treatment results were satisfactory in terms of the occlusion and function of the TMJ as well as aesthetics. Analyses of brain activity using fMRI before and after orthodontic treatment indicated clenching at the habitual intercuspal position (ICP) before treatment resulted in increased blood oxygen level dependent (BOLD) signals in the motor area of the oral region abnormally; however, this activity converged after treatment. The purpose of this study was to test the hypothesis that (1) the presence of a clinical temporomandibular joint-related disorder has effects on short-term changes in temporomandibular joint pain, and that (2) clinical variables of time since pain onset predict treatment outcomes of occlusal splint therapy. The study comprised 76 patients with unilateral temporomandibular joint pain. The clinical disorder subgroup included 47 patients with a clinical pain side-related diagnosis of internal derangement type I (n = 16), internal derangement type III (n = 19), and degenerative joint disease (n = 12). The clinical non-disorder subgroup consisted of 29 patients without a temporomandibular joint disorder. A logistic regression analysis was used to compute the odds ratio for the clinical variables of time since pain onset, adjusted for age, gender, pretreatment pain level, and clinical subgroup. For the temporomandibular joint pain measurements there was no significant ‘session’/‘clinical subgroup’ interaction (P = 0·470). Significant increase in benefit of a successful outcome of ‘pain reduction >70%’ occurred with a time since pain onset of ≤6 months (15·1 odds ratio) (P = 0·003). The odds ratio that a patient with a time since pain onset of >2 years might belong to the unsuccessful treatment group of ‘pain reduction <30%’ was strong (6·0) and significant (P = 0·026). Diagnosis of temporomandibular joint disorder proved not to be linked to changes in therapeutic outcome measures of temporomandibular joint pain. A prospective randomized study was carried out to compare the therapeutic success of two different types of splint in patients with painful anterior disc displacement of the temporomandibular joint. The patients in Group I ( n = 20) received stabilization splint therapy and the patients in Group II ( n = 20) pivot splint therapy. Clinical investigation of the craniomandibular system was performed before and 1, 2 and 3 months after therapy and this was accompanied by subjective evaluation by the patients of their symptoms, using a validated questionnaire with visual analogue scales (VAS). There was a significant increase in maximum jaw opening and a significant reduction in subjective pain in both groups during the course of therapy (Wilcoxon test, P < 0·05). Active jaw opening increased by a mean of 8·05 mm in the group of patients treated with a stabilization splint (Group I). The comparable figure with pivot splint therapy (Group II) was 8·26 mm. The VAS scale value in Group I was reduced by 30·54 units and in Group II by 39·36 scale units. However, neither of these differences between the groups was statistically significant ( Mann–Whitney U-test, P > 0·05). In order to evaluate and compare the effects of biofeedback and occlusal splint therapy on mandibular dysfunction, 30 patients were randomly divided into two treatment groups. The patients were women. aged 20-40 years without any obvious organic reason for their symptoms. There were no significant differences between the two groups before the start of treatment in respect of signs and symptoms of mandibular dysfunction. One group used full coverage splints at night for 6 weeks. The other group received biofeedback training up to six times, 30 min per session. One month after completion of the therapy the patients were re-examined. Both groups showed a significant reduction in symptoms, both subjectively and clinically. No significant differences between the groups were found. The two treatments were thus equally effective in the short-term perspective in patients with signs and symptoms of mandibular dysfunction.
