Cysticercosis is a neglected tropical disease caused by the larval stage of the zoonotic tapeworm (Taenia solium). While there is a clear spatial component in the occurrence of the parasite, no geostatistical analysis of active human cysticercosis has been conducted yet, nor has such an analysis been conducted for Sub-Saharan Africa, albeit relevant for guiding prevention and control strategies. The goal of this study was to conduct a geostatistical analysis of active human cysticercosis, using data from the baseline cross-sectional component of a large-scale study in 60 villages in Burkina Faso. The outcome was the prevalence of active human cysticercosis (hCC), determined using the B158/B60 Ag-ELISA, while various environmental variables linked with the transmission and spread of the disease were explored as potential explanatory variables for the spatial distribution of T. solium. A generalized linear geostatistical model (GLGM) was run, and prediction maps were generated. Analyses were conducted using data generated at two levels: individual participant data and grouped village data. The best model was selected using a backward variable selection procedure and models were compared using likelihood ratio testing. The best individual-level GLGM included precipitation (increasing values were associated with an increased odds of positive test result), distance to the nearest river (decreased odds) and night land temperature (decreased odds) as predictors for active hCC, whereas the village-level GLGM only retained precipitation and distance to the nearest river. The range of spatial correlation was estimated at 45.0 [95%CI: 34.3; 57.8] meters and 28.2 [95%CI: 14.0; 56.2] km for the individual- and village-level datasets, respectively. Individual- and village-level GLGM unravelled large areas with active hCC predicted prevalence estimates of at least 4% in the south-east, the extreme south, and north-west of the study area, while patches of prevalence estimates below 2% were seen in the north and west. More research designed to analyse the spatial characteristics of hCC is needed with sampling strategies ensuring appropriate characterisation of spatial variability, and incorporating the uncertainty linked to the measurement of outcome and environmental variables in the geostatistical analysis. Background: Taenia solium cysticercosis is a public health and agricultural problem in many low and middle-income countries where health education, sanitation, pig management practices and meat inspection infrastructure are insufficient. Cysticercosis affects both human and animal health and has important economic consequences. Very few studies have been conducted to evaluate the monetary burden of cysticercosis. This study aimed at estimating the 2015 costs associated with cysticercosis in humans and pigs in Mexico., Methods: The monetary burden of human cysticercosis was estimated based on costs incurred by living with and treating epilepsy and severe chronic headaches associated with neurocysticercosis (NCC). The estimated cost of porcine cysticercosis took into consideration losses due to the reduction in the price of cysticercosis-infected animals. Epidemiologic and economic data were obtained from the published literature, government reports, and setting-specific questionnaires. Latin hypercube sampling methods were employed to sample the distributions of uncertain parameters and to estimate 95% credible regions (95% CRs). All results are reported in 2015 U.S.$., Findings: The overall monetary burden associated with NCC morbidity was estimated at U.S.$215,775,056 (95% CR U.S.$109,309,560 -U.S.$361,924,224), with U.S.$436 (95% CR: U.S.$296 -U.S.$604) lost per patient. If loss of future years of income and productivity due to NCC-associated deaths was included, this value increased by U.S.$54.26 million, assuming that these individuals earned Mexico's median wage salary. An additional U.S.$19,507,171 (95% CR U.S.$5,734,782 -U.S.$35,913,487) was estimated to be lost due to porcine cysticercosis., Conclusions: This study suggests that T. solium cysticercosis results in considerable monetary losses to Mexico., Competing Interests: The authors have declared that no competing interests exist.

Taenia hydatigena is a non-zoonotic cestode that has canines as definitive hosts and ruminants and pigs as intermediate hosts. In pigs, its presence causes cross-reactivity in serological testing for Taenia solium cysticercosis. Therefore, knowledge on the occurrence of T. hydatigena is paramount for validly estimating the seroprevalence of T. solium cysticercosis in pigs. In a cross-sectional abattoir study, we estimated the prevalence of T. hydatigena in pigs slaughtered in Koudougou, Burkina Faso. Carcasses of 452 pigs were examined by investigators for perceived and suspected T. hydatigena cysticercus lesions in the abdominal cavity or on the surface of abdominal organs. Routine meat inspection was performed by local inspectors to identify T. solium cysticerci. All lesions were subjected to PCR-RFLP analysis in order to differentiate Taenia spp. Additionally, individual blood samples were examined for the presence of circulating cysticercus antigens using the B158/B60 Ag-ELISA. Perceived T. hydatigena cysticerci were found in 13 pigs, whereas meat inspectors found seven carcasses infected with T. solium cysticerci. All were confirmed by molecular analysis. Of pigs with other suspected lesions, mostly located in the liver, 27 and six were found to harbour T. hydatigena and T. solium cysticerci, respectively. Overall, 8.8% of pigs (40/452) were found infected with T. hydatigena and 2.9% (13/452) with T. solium. Of these positive pigs, one was found infected with both Taenia spp. (0.2%, 1/452). Blood samples of 48.5% of pigs (219/452) were positive in the Ag-ELISA. Pigs with confirmed cysts of T. hydatigena and T. solium had a positive Ag-ELISA result in 57.5% (23/40) and 61.5% (8/13) of cases, respectively. The observed T. hydatigena prevalence in this study is relatively high in comparison to other studies in Africa. Estimates of the occurrence of active porcine T. solium infection using the B158/B60 Ag-ELISA should therefore be adjusted for the presence of T. hydatigena. The low level of T. solium infection detected upon meat inspection in this study is likely an underestimation of the true prevalence since routine meat inspection shows poor sensitivity and pigs perceived to be infected based on tongue palpation are rarely sent to official abattoirs., (Copyright © 2016 Elsevier B.V. Background: little is known about porcine cysticercosis in Burkina Faso. We conducted a pilot study to estimate the prevalence of antigens of Taenia solium cysticercosis and to identify associated factors in pigs of three villages in Burkina Faso, selected to represent different pig management practices: one village where pigs are allowed to roam freely (Batondo), one village where pigs are penned part of the time (Pabré) and one village with limited pig farming (Nyonyogo)., Methods/principal Findings: a clustered random sampling design was used. Data on socio-demographic characteristics (source of drinking water, presence of latrines in the household, type and number of breeding animals) and pig management practices were collected using a standardized questionnaire. Blood samples were collected from one pig per household to determine the presence of antigens of the larval stages of T. solium by the B158/B60 Ag-ELISA. The associations between seropositivity and socio-demographic and pig management practices were estimated using logistic regression. Proportions of 32.5% (95% CI 25.4-40.3), 39.6% (31.9-47.8), and 0% of pigs, were found positive for the presence of circulating antigens of T. solium in Batondo, Pabré, and Nyonyogo, respectively. The results of the logistic regression analyses suggested that people acquire knowledge on porcine cysticercosis following the contamination of their animals. The presence of antigens in the pigs' sera was not associated with the absence of latrines in the household, the source of drinking water or the status of infection in humans but was associated with pig rearing practices during the rainy season., Conclusions/significance: the results suggest that education of pig farmers is urgently needed to reduce the prevalence of this infection.

Background: Porcine cysticercosis is caused by a zoonotic tapeworm, Taenia solium, which causes serious disease syndromes in human. Effective control of the parasite requires knowledge on the burden and pattern of the infections in order to properly direct limited resources. The objective of this study was to establish the spatial distribution of porcine cysticercosis in Mbulu district, northern Tanzania, to guide control strategies., Methodology/principal Findings: This study is a secondary analysis of data collected during the baseline and follow-up periods of a randomized community trial aiming at reducing the incidence rate of porcine cysticercosis through an educational program. At baseline, 784 randomly selected pig-keeping households located in 42 villages in 14 wards were included. Lingual examination of indigenous pigs aged 2-12 (median 8) months, one randomly selected from each household, were conducted. Data from the control group of the randomized trial that included 21 of the 42 villages were used for the incidence study. A total of 295 pig-keeping households were provided with sentinel pigs (one each) and reassessed for cysticercosis incidence once or twice for 2-9 (median 4) months using lingual examination and antigen ELISA. Prevalence of porcine cysticercosis was computed in Epi Info 3.5. The prevalence and incidence of porcine cysticercosis were mapped at household level using ArcView 3.2. K functions were computed in R software to assess general clustering of porcine cysticercosis. Spatial scan statistics were computed in SatScan to identify local clusters of the infection. The overall prevalence of porcine cysticercosis was 7.3% (95% CI: 5.6, 9.4; n = 784). The K functions revealed a significant overall clustering of porcine cysticercosis incidence for all distances between 600 m and 5 km from a randomly chosen case household based on Ag-ELISA. Lingual examination revealed clustering from 650 m to 6 km and between 7.5 and 10 km. The prevalence study did not reveal any significant clustering by this method. Spatial scan statistics found one significant cluster of porcine cysticercosis prevalence (P = 0.0036; n = 370). In addition, the analysis found one large cluster of porcine cysticercosis incidence based on Ag-ELISA (P = 0.0010; n = 236) and two relatively small clusters of incidence based on lingual examination (P = 0.0012 and P = 0.0026; n = 241). These clusters had similar spatial location and included six wards, four of which were identified as high risk areas of porcine cysticercosis., Conclusion/significance: This study has identified local clusters of porcine cysticercosis in Mbulu district, northern Tanzania, where limited resources for control of T. solium could be directed. Further studies are needed to establish causes of clustering to institute appropriate interventions.

We conducted a randomised community controlled trial to estimate the effectiveness of health and pig-management education intervention in reducing the incidence rate of porcine cysticercosis caused by Taenia solium in Mbulu District, northern Tanzania, between April 2002 and July 2004. We included a random sample of 827 pig-keeping households from 42 randomly selected villages in our 27-month study. We collected baseline data on the prevalence of porcine cysticercosis based on lingual examination of live pigs, and pig-management and sanitation knowledge and practices based on questionnaire interviews and observations. We allocated 21 of the villages to the health-education intervention, developed with community participation; we used stratified randomisation to balance the village-level baseline prevalence of porcine cysticercosis. From July 2003 to March 2004 following the intervention, we gave each participant household a sentinel piglet to raise (the timing was balanced between the intervention groups). Knowledge about the transmission and prevention of porcine cysticercosis was improved >42% in both groups when measured 10-12 months post-intervention. There was no improvement in observed practices in either group throughout the study period. However, the intervention had a significant reduction in the reported cases of household consumption of infected pork (a reduction by 20%). The intervention was associated with a considerable decrease in the incidence rate of porcine cysticercosis (incidence rate ratio 0.57) as measured by antigen-ELISA in sentinel pigs. Public education could lead to a reduction of the risk of infection in humans.

The transmission dynamics of the human-pig zoonotic cestode Taenia solium are explored with both deterministic and stochastic versions of a modified Reed-Frost model. This model, originally developed for microparasitic infections (i.e. bacteria, viruses and protozoa), assumes that random contacts occur between hosts and that hosts can be either susceptible, infected or 'recovered and presumed immune'. Transmission between humans and pigs is modelled as susceptible roaming pigs scavenging on human faeces infected with T. solium eggs. Transmission from pigs to humans is modelled as susceptible humans eating under-cooked pork meat harbouring T. solium metacestodes. Deterministic models of each scenario were first run, followed by stochastic versions of the models to assess the likelihood of infection elimination in the small population modelled. The effects of three groups of interventions were investigated using the model: (i) interventions affecting the transmission parameters such as use of latrines, meat inspection, and cooking habits; (ii) routine interventions including rapid detection and treatment of human carriers or pig vaccination; and (iii) treatment interventions of either humans or pigs. It is concluded that mass-treatment can result in a short term dramatic reduction in prevalence, whereas interventions targeting interruption of the life cycle lead to long-term reduction in prevalence.

To estimate prevalence of and risk factors for the prevalence of porcine cysticercosis in Mbulu District, Tanzania, 770 live pigs were examined by lingual examination in 21 villages. Structured observations and questionnaire interviews were used to assess pig rearing practices and household use of latrines. Associations between factors were analyzed using a Bayesian hierarchical model to obtain prevalence odds ratio (OR) and 95% Bayesian Credible Intervals (95% BCI). Prevalence was 17.4% (village-specific range 3.2-46.7%). Prevalence of porcine cysticercosis was considerably higher in pigs reared in households lacking latrines than in those reared in households that were using latrines (OR = 2.04; 95% BCI = 1.25, 3.45). About 96% of the pigs were kept under free-range conditions. This study suggests the need for further studies in order to design and implement effective prevention and control measures for porcine cysticercosis in Mbulu District, Tanzania.
