Objectives: To determine prevalence of dental erosion among competitive swimmers in Kaunas, the second largest city in Lithuania. Material and Methods: The study was designed as a cross-sectional survey, with a questionnaire and clinical examination protocols. The participants were 12 - 25 year-old swimmers regularly practicing in the swimming pools of Kaunas. Of the total of 132 participants there were 76 (12 - 17 year-old) and 56 (18 - 25 year-old) individuals; in Groups 1 and 2, respectively. Participants were examined for dental erosion, using a portable dental unit equipped with fibre-optic light, compressed air and suction, and standard dental instruments for oral inspection. Lussi index was applied for recording dental erosion. The completed questionnaires focused on the common erosion risk factors were returned by all participants. Results: Dental erosion was found in 25% of the 12 - 17 year-olds, and in 50% of 18 - 25 years-olds. Mean value of the surfaces with erosion was 6.31 (SD 4.37). All eroded surfaces were evaluated as grade 1. Swimming training duration and the participants' age correlated positively (Kendall correlation, r = 0.65, P < 0.001), meaning that older swimmers had practiced for longer period. No significant correlation between occurrence of dental erosion and the analyzed risk factors (gastroesophageal reflux disease, frequent vomiting, dry mouth, regular intake of acidic medicines, carbonated drinks) was found in both study groups. Conclusions: Prevalence of dental erosion of very low degree was high among the regular swimmers in Kaunas, and was significantly related to swimmers' age. [ABSTRACT FROM AUTHOR]

BACKGROUND: Exposure to disinfection by-products (DBPs) in drinking water has been associated with cancer risk. A recent study (Villanueva et al. 2007; Am J Epidemiol 165:148-156) found an increased bladder cancer risk among subjects attending swimming pools relative to those not attending. OBJECTIVES: We evaluated adults who swam in chlorinated pools to determine whether exposure to DBPs in pool water is associated with biomarkers of genotoxicity. Methods: We collected blood, urine, and exhaled air samples from 49 nonsmoking adult volunteers before and after they swam for 40 min in an indoor chlorinated pool. We estimated associations between the concentrations of four trihalomethanes (THMs) in exhaled breath and changes in micronuclei (MN) and DNA damage (comet assay) in peripheral blood lymphocytes before and 1 hr after swimming; urine mutagenicity (Ames assay) before and 2 hr after swimming; and MN in exfoliated urothelial cells before and 2 weeks after swimming. We also estimated associations and interactions with polymorphisms in genes related to DNA repair or to DBP metabolism. RESULTS: After swimming, the total concentration of the four THMs in exhaled breath was seven times higher than before swimming. The change in the frequency of micronucleated lymphocytes after swimming increased in association with higher exhaled concentrations of the brominated THMs (p = 0.03 for bromodichloromethane, p = 0.05 for chlorodibromomethane, p = 0.01 for bromoform) but not chloroform. Swimming was not associated with DNA damage detectable by the comet assay. Urine mutagenicity increased significantly after swimming, in association with the higher concentration of exhaled bromoform (p = 0.004). We found no significant associations with changes in micronucleated urothelial cells. CONCLUSIONS: Our findings support potential genotoxic effects of exposure to DBPs from swimming pools. Abstract: Chlorination of indoor swimming pools is practiced for disinfection and oxidation of reduced compounds that are introduced to water by swimmers. However, there is growing concern associated with formation for chlorinated disinfection by-products (DBPs) in these settings. Volatile DBPs are of particular concern because they may promote respiratory ailments and other adverse health effects among swimmers and patrons of indoor pool facilities. To examine the scope of this issue, water samples were collected from 11 pools over a 6month period and analyzed for free chlorine and their volatile DBP content. Eleven volatile DBPs were identified: monochloramine (NH2Cl), dichloramine (NHCl2), trichloramine (NCl3), chloroform (CHCl3), bromoform (CHBr3), dichlorobromomethane (CHBrCl2), dibromochloromethane (CHBr2Cl), cyanogen chloride (CNCl), cyanogen bromide (CNBr), dichloroacetonitrile (CNCHCl2), and dichloromethylamine (CH3NCl2). Of these 11 DBPs, 10 were identified as regularly occurring, with CHBrCl2 only appearing sporadically. Pool water samples were analyzed for residual chlorine compounds using the DPD colorimetric method and by membrane introduction mass spectrometry (MIMS). These two methods were chosen as complementary measures of residual chlorine, and to allow for comparisons between the methods. The DPD method was demonstrated to consistently overestimate inorganic chloramine content in swimming pools. Pairwise correlations among the measured volatile DBPs allowed identification of dichloromethylamine and dichloroacetonitrile as potential swimming pool water quality indicator compounds. Swimming pools are considered to be the prime source for public exposure to trihalomethanes (THMs), mainly chloroform, because of their use for hygienic purposes. High doses of chlorine are easily perceptible by smell. The aim of this study was to measure simultaneously the THM levels in urine and blood of swimmers and in pool water itself (from city and university swimming pool), in order to investigate further the relationship between substances in different matrices at known environmental levels with uptake of these substances in indoor swimming pools. Volatile analytes were isolated and preconcentrated from the samples by thin layer headspace analysis (TLHS) with autogenous generation of the liquid sorbent. Chloroform, bromodichloromethane and dibromochloromethane were detected in the urine of all persons attending a swimming pool. Concentrations of THM compounds varied in the case of each swimmer. On Saturday June 1, 2013, the Galveston County Health District's (GCHD) epidemiology program received a report that 50 members of an area swim team "A" had developed a gastrointestinal illness. However, after further investigation we learned that pool users who became ill were residents of both Galveston County and Harris County. The swimming pool these swimmers used is located in, Galveston County, Texas, in city "A" and is inspected and issued its permit by the GCHD. However, part of city "A's" boundaries are within the southern part of Harris County. The pool is an eight-lane pool with a maximum pool depth of 5 feet and pool capacity of 235 patrons; this pool also has a baby pool attached. [ABSTRACT FROM AUTHOR]

The analysis of free chlorine concentrations in swimming pool water makes it possible to assess the antiseptic effect of the disinfectant. The concentration of combined chlorine determines the comfort of swimming and indicates if there is a threat from DBP (disinfection by-products). The distribution of free and combined chlorine concentration was analyzed in four basins differing in seasonality of use and in the applied water flow systems. After considering the distribution of free and combined chlorine content in characteristic points of pools, an attempt was made to determine the most reliable point for assessing the quality of water and its suitability for swimming. Such searches should aim to identify the places with the worst water quality. The most uniform distribution of the concentrations of both free and combined chlorine was observed at the middle point of swimming pools, while at points near the corners and walls of swimming pools a varied distribution was observed. Such a control strategy, based on the least favorable test results at a point considered as characteristic, would make it possible to verify the parameters of the swimming pool water treatment system and thus minimize the risk to swimmers' health. Several reports indicate an increased prevalence of dental erosion among intensive swimmers due to low pH gas-chlorinated pool water. Contrary to other extrinsic factors which induce erosion located on the facial aspect, low pH pool water results in general dental erosion. Additionally, a case report is presented which describes the very rapid occurrence of excessive general dental erosion of a competitive swimmer due to gas-chlorinated pool water within 27 days. The observation of several authors as well as this case underscore the significance of a regular pH monitoring of chlorinated swimming pool water. The high incidence indicates that dental erosion due to frequent swimming is of considerable diagnostic and therapeutic significance.
