Purpose: To evaluate the efficacy and safety of the EGIS esophageal stent for treating malignant and benign esophageal strictures.Materials and Methods: Data of 73 patients (mean age 63.0 ± 11.9 years; 66 males) with malignant esophageal stricture and 16 patients (mean age 63.7 ± 9.5 years; 13 males) with benign esophageal stricture who received the EGIS esophageal stent (S&G Biotech, Seongnam, Korea) between October 2010 and April 2016 were obtained from a prospectively maintained electronic database.Results: Technical and clinical success rates were 100% (89/89). Stent malfunction (i.e., tumor/tissue overgrowth, stent migration, and food impaction) occurred in 20.5% (15/73) and 37.5% (6/16) of patients with malignant and benign esophageal strictures, respectively. Stent migration occurred in five (6.8%) and four (25%) patients with malignant and benign esophageal strictures, respectively. The median follow-up durations in patients with malignant and benign esophageal strictures were 130 [interquartile range (IQR) 76-322] days and 486 (IQR 315-736) days, respectively. Recurrent dysphagia occurred in 14.1% (10/73) and 87.5% (14/16) of patients with malignant and benign esophageal strictures, respectively. The median recurrence-free durations in patients with malignant and benign esophageal strictures were 126 (IQR 69-259) days and 100 (IQR 40-182) days, respectively.Conclusion: The EGIS esophageal stent appears to be effective for malignant esophageal strictures, with relatively low rate of stent migration, whereas, for benign esophageal strictures, it seems to be associated with a high rate of recurrent dysphagia, mainly due to stent migration. Background Fluoroscopic or endoscopic placement of metallic stents is used as a safe, non-surgical, palliative treatment option for malignant gastroduodenal obstructions. However, endoscopic stent placement is not always feasible, due to the tight or tortuous stricture of the obstructed area and insufficient visualization of the stomach due to remnant food material. Purpose To assess the technical feasibility and clinical outcomes of fluoroscopic-guided stent placement in malignant gastroduodenal obstructions following the technical failure of endoscopic stent placement. Material and Methods In all, 19 patients (14 men, 5 women; age range, 36-85 years) were referred due to technical failures of tentative endoscopic stent placement. Indications were failure to pass the guide wire through the obstruction (n = 13), failure to reach the stricture lesion due to short endoscope (n = 5), and acute angulation of the stricture lesion (n = 1). Data were retrospectively collected regarding the technical and clinical success of the fluoroscopy, the dysphagia score before and after stent placement, and major complications and their management. Results Fluoroscopic stent placement was technically successful in 15/19 patients (79%). Failures were due to complete obstruction (n = 3) and acute angulation of the stricture lesion (n = 1). Clinical success was achieved in all 15 patients with successful stent placement. The dysphagia score significantly improved after stent placement ( P < 0.001). Complications occurred in 4/15 patients (27%), including tumor overgrowth (n = 2), stent collapse (n = 1), and jaundice (n = 1). Conclusion Fluoroscopic stent placement in malignant gastroduodenal obstructions following a technical failure of endoscopic stent placement is technically feasible and has an acceptable clinical outcome. Objectives: To compare the outcomes between stent placement and surgical gastrojejunostomy (GJ) for the palliation of gastric outlet obstruction (GOO) in patients with unresectable gastric cancer.Methods: A retrospective study was performed in a single university hospital in 224 patients with GOO, and who were treated either by stent placement (n = 124) or surgical GJ (n = 100). The outcomes were assessed with reference to the following variables with the use of propensity-score matching: success rates; complications; dysphagia scores, albumin, and body mass index; survival; symptom-free duration; and hospitalization.Results: We identified a well-balanced cohort of 74 pairs of patients, matched on the basis of propensity score. The dysphagia score 7 days after treatment was significantly better in the stent group (P < 0.001). Albumin level 1 month after treatment was significantly lower in the stent group (P < 0.001). Symptom-free duration and hospitalization were significantly longer in the surgery group (P = 0.002, P < 0.001, respectively). The recurrence rate was significantly higher in the stent group (P = 0.032).Conclusions: Stent placement can provide faster symptom relief and shorter hospitalization, while surgical GJ can provide longer symptom-free duration, less recurrent obstruction symptoms and better nutritional status.Key Points: • The two methods are equally effective in palliating gastric outlet obstruction symptoms • The stent group showed rapid and efficient palliation of symptoms • Recurrent symptoms were more frequent in the stent group • Surgical gastrojejunostomy provides a longer symptom-free duration and better nutritional status. The article presents a study which evaluates the effectiveness, feasibility, and complications of additional gastroduodenal stent placement after primary gastroduodenal stenting in patients with recurrent obstructive symptoms. The study uses the gastric outlet obstruction (GOO) score improvement and clinical success rates for the analysis of the data from 68 patients. Results show that placing additional stent for the treatment of patients with recurrent GOO is feasible and effective.

To investigate the clinical outcomes of fully covered self-expanding metal stent (FCSEMS) placement in patients with malignant esophageal obstruction who survived longer than 6 months. From January 2002 to January 2018, 88 FCSEMS were placed in 64 patients (mean age 62.9 ± 11.6 years; 58 males) with inoperable malignant esophageal obstruction with or without esophago-respiratory fistula. Only patients who survived more than 6 months with FCSEMS in place were included. Data regarding technical and clinical success, complications, reinterventions, stent patency, and patient survival were obtained from a prospectively maintained hospital database. The technical and clinical success rates were 100 % (64/64). During follow-up, the median dysphagia score significantly improved (3.09 ± 0.68 to 1.05 ± 0.60, p < 0.001). The complication rate was 48.8 %. Multivariate analysis revealed that only longer stenting duration was associated with complications [hazard ratio = 1.220, 95 % confidence interval (CI) (1.074–2.760), p = 0.039]. The median follow-up duration was 257 days (range, 181–969). The median stent patency duration was 289 days [95% CI (209.9–368.1)]. The median survival was 254 days [95% CI (219.7–288.3)]. Our data suggest that esophageal FCSEMS placement is an effective option for patients with malignant dysphagia when survival longer than 6 months is expected. The rate of complications increases with time, and SEMS development is needed to keep up with the advancement in oncological treatment. Fully covered esophageal self-expandable stent placement is effective in patients surviving more than 6 months, however, the rate of complications also increases.
