Objective: This study aimed to determine whether or not transfusion of fresh red blood cells (RBCs) reduced the incidence of new or progressive multiple organ dysfunction syndrome compared with standard-issue RBCs in pediatric patients undergoing cardiac surgery. Methods: Preplanned secondary analysis of the Age of Blood in Children in Pediatric Intensive Care Unit study, an international randomized controlled trial. This study included children enrolled in the Age of Blood in Children in Pediatric Intensive Care Unit trial and admitted to a pediatric intensive care unit after cardiac surgery with cardiopulmonary bypass. Patients were randomized to receive either fresh (stored ≤7 days) or standard-issue RBCs. The primary outcome measure was new or progressive multiple organ dysfunction syndrome, measured up to 28 days postrandomization or at pediatric intensive care unit discharge, or death. Results: One hundred seventy-eight patients (median age, 0.6 years; interquartile range, 0.3-2.6 years) were included with 89 patients randomized to the fresh RBCs group (median length of storage, 5 days; interquartile range, 4-6 days) and 89 to the standard-issue RBCs group (median length of storage, 18 days; interquartile range, 13-22 days). There were no statistically significant differences in new or progressive multiple organ dysfunction syndrome between fresh (43 out of 89 [48.3%]) and standard-issue RBCs groups (38 out of 88 [43.2%]), with a relative risk of 1.12 (95% CI, 0.81 to 1.54; P = .49) and an unadjusted absolute risk difference of 5.1% (95% CI, −9.5% to 19.8%; P = .49). Conclusions: In neonates and children undergoing cardiac surgery with cardiopulmonary bypass, the use of fresh RBCs did not reduce the incidence of new or progressive multiple organ dysfunction syndrome compared with the standard-issue RBCs. A larger trial is needed to confirm these results.

An 80-year-old man with a chronic penetrating atherosclerotic ulcer was not a candidate for open surgical repair owing to the presence of diffuse vascular atherosclerosis and a deep ulcerative lesion originating at the level of the aortic arch concavity. No appropriate endovascular landing zone was present in arch zones 1 or 2. However, a totally endovascular branched arch repair involving transapical delivery of the three branches was successful.

Background: The best operative approach to large hiatal hernias still remains controversial between suture cruroplasty and prosthetic hiatal herniorrhaphy. This study aims at analysing results from a single institution in Italy in terms of subjective and objective outcomes. Methods: Retrospectively collected data of all patients that underwent laparoscopic hiatal hernia repair since 2011 were considered. Sixty-five patients were included overall; 17 of them fit the criteria of large hiatal hernia. Follow-up (FU) was assessed by visit, questionnaires and X-ray imaging. Results: No major complications occurred in the post-operative course. No patient was lost during the FU period. Out of all the patients included, 13 agreed to have an X-ray with water-soluble contrast. The questionnaires showed a 76.5% rate of satisfaction (13 patients), and the recurrence rate demonstrated by radiology was 29.4% (five patients). There were no major mesh-related complications. Conclusion: The best operative approach for large hiatal hernias remains far from standardised: There is a lack of evidence on the use of a mesh for this kind of surgery as well as substantial controversy over the definition of what a giant hiatal hernia is. Nevertheless, the results from this study and the main studies in the literature seem to be encouraging in improving giant hiatal hernia repair outcomes.

Introduction: Partial nephrectomy (PN), has become the gold standard for the surgical management of small renal masses, due to excellent oncologic control with concomitant preservation of nephron units. However, data regarding the association of obesity with perioperative outcomes following PN are mixed. Therefore, the association between obesity (using BMI) and post-operative complications (POC) rate following Robotic assisted laparoscopic PN (RPNx) was tested.Methods: Two hundred and fifty-one adult patients who underwent RPNx from 1/2011 to 5/2017 at a single institution, with at least 90 days follow-up were identified and included. No patients were excluded. Electronic medical records were reviewed to record all POC within 90 days of surgery. A piecewise generalized linear model for binary outcomes (logistic) was used to model the proportion of subjects with POC by their BMI. The slope of the line is adjusted to a BMI of 30 Kg/m2.Results: BMI is significantly associated with POC rate. POC rate decreased with increasing BMI below the inflection point of 30 Kg/m2 (0.848[0.756, 0.952]) (OR [95% CI], p = 0.005). POC rate was found to increase with increasing BMI above the BMI inflection of 30 Kg/m2 (1.102 [1.027, 1.182], p = 0.0071).Conclusions: In this cohort study, BMI showed an association with PC. It may be important to take BMI into account in surgical and clinical management considerations of RPNx, since higher rates of POC are associated with patients who are underweight, morbidly obese, and even with normal BMI. Further research is required on larger cohorts of RPNx patients to provide better description of this phenomenon and elucidate the role of BMI in development of POC.

OBJECTIVE: To evaluate treatment outcomes in Wilms' tumor (WT). MATERIALS AND METHODS: We studied 53 children with median age of 2 years with WT, stages I-19, II-14, III-12, IV-6 and V-2. Treatment consisted of surgical excision plus adjuvant (40 children) or neoadjuvant and adjuvant chemotherapy (unresectable tumor, n = 8, or caval tumor extension, n = 5). Chemotherapy and radiotherapy followed protocols of Brazilian Wilms' Tumor Study Group excepting 16 cases with stage I disease that received a short duration postoperative treatment with vincristine (VCR - 11 doses) and dactinomycin (AMD - 4 doses). Relapsed WT was treated with multiagent regimens including cisplatin/carboplatin, cyclophosphamide, ifosfamide and etoposide. One patient with resistant relapsed WT was treated by high-dose conditioning chemotherapy with stem cell rescue. RESULTS: Overall and disease-free survival rates at 5 years were respectively 88.2 ± 5.0% and 76.7 ± 6.6%. Short duration therapy for stage I tumor showed a disease-free survival rate of 100% in a median time of 101 months (range 14 to 248 months). Overall and disease-free survival of 10 patients with recurrent WT at 5 years was 42.8%. The child treated with high-dose chemotherapy plus stem cell transplant is alive without evidence of disease 84 months from relapse. CONCLUSION: The postoperative chemotherapy in stage I disease can be reduced without compromising the cure rate. The treatment of unfavorable stage III and IV disease or relapsed tumor remains a challenge.

We describe a case of spontaneous gastric rupture in a child of 5 years old. The patient reached us in a serious condition; the anamnesis was negative for traumatic events or gastrointestinal disorders. An abdominal X-ray and CT scan revealed free air and fluid in the abdominal cavity, leading to the diagnosis of gastro-intestinal perforation. Submitted to urgent surgery, a rupture of the posterior wall of the stomach was found that was treated with gastrectomy “à la demande”. The surgery follow-up was regular. Morphological and immunohistochemical study showed some muscular abnormalities of the muscular gastric wall.

Aim: Assessment of hematologic improvement, survival and peri-operative morbidity after first-line splenectomy for splenic marginal zone lymphoma (SMZL).Methods: Forty-three patients undergoing open splenectomy were prospectively analyzed. Perioperative clinical course, overall and progression-free survival (OS-PFS) were evaluated. Risk factors analyzed were gender, age, ASA-grade, ECOG performance status, presence of B-symptoms, body mass index, steroidal treatment, serum albumin concentration, IIL-score, operative time, spleen size and weight.Results: The median follow-up was 31 months (IQR 15-76; range 24-154). Anemia and thrombocytopenia resolved in 80% of patients at 6 months; in 60% at 2 years. The 5-year and 10-year PFS were 35% and 13% respectively, with a median of 35 months (shorter in patients with ECOG performance status ≥2 and B-symptoms). Nineteen cases (44.2%) had a progression of disease within 2 years. Of these, 14 (32.6%) received adjuvant chemotherapy (mainly R-FC or R-CVP). Progression was attributed to high-grade B lymphoma in 7 (16.3%) patients. The median time between diagnosis and progression to aggressive lymphoma was 25.5 months (range 18.8-81.8). The median time to next treatment was 83.5 months (95% CI 49-118). The 5-year and 10-year OS were 75% and 53% respectively. Mortality was due to disease progression and histological transformation in high-grade B lymphoma in 50% of cases, myelodisplastic syndrome in 15%, recurrence of hemolytic anemia in 15%, Hodgkin lymphoma in 7% and to infections (mainly pulmonary) in the remaining 13% of cases. Post-operative morbidity was 2.3% (1 patient with grade-3 complication). Overall grade ≥2 complication rate was 32.5% (mainly hemorrhagic and pulmonary complications). Spleen weight was the only independent risk factor for morbidity. Mortality was nil.Conclusion: Splenectomy is safe and effective as regards cytopenia resolution and OS, although disease progression is frequently observed at follow-up. Such results are strictly linked to accurate pre- and post-operative clinical management and optimal anesthesiologic approach. [ABSTRACT FROM AUTHOR]

Radiotherapy and/or pelvic exenteration represent the treatment of vaginal recurrence, but the prognosis remains unsatisfactory and with long-term complications. We investigated the possible role of vaginectomy for isolated vaginal relapse (IVR) in cervical cancer (CC).Patients with vaginal CC recurrence were evaluated for surgical treatment consisting in vaginectomy. Data were prospectively collected and analyzed to identify independent prognostic factors.Twenty-nine patients with IVR from CC were enrolled. Early and late complications were observed in 7 (24%) and 6 (21%) patients, respectively. After a median follow-up of 57.5 months (range, 8-100 months), 16 patients (55%) were disease-free. The 5-year overall survival and progression-free survival rates were 70.5% and 59.4%, respectively.In carefully selected patients, vaginectomy may be considered a therapeutic option for IVR. Older patients with long disease-free interval and small recurrences benefit the most from this bladder-sparing surgical technique. [ABSTRACT FROM AUTHOR]

Summary: A case of malignant epithelioid schwannoma in the skin is reported. This was a rare variant of a malignant tumour that arose on the back of a 35-year-old male without neurofibromatosis. Clinically the nodule appeared to be a benign cyst but as it was painful it was decided to remove it. Ultrastructural and immunohistochemical features of the lesion were consistent with those of malignant epithelioid schwannoma so a radical excision was performed. Most ordinary malignant schwannoma are located in the deep soft tissue of the proximal portions of the upper and lower extremities and trunk; to the best of our knowledge only 26 cases of malignant epithelioid schwannoma in the skin and subcutis have been described in the literature. For the 14 male and 12 female patients reviewed, the median age was 43 years (range 19–84). Upper extremities were the most common sites (10 of 26). Seven patients developed local recurrences and four developed metastases: two to lung, one to lung and lymph node, and one to lung, liver and brain. All patients with local recurrence and all except one who developed metastases did not undergo wide local excision. We can conclude that malignant epithelioid schwannoma in the skin and subcutis is eminently curable if treated with wide local excision. [Copyright &y& Elsevier]

Abstract Purpose To evaluate the effects of alcohol exposure and diabetes on apoptotic process in the corpus cavernosum. Methods Forty eight male Wistar rats were divided into four groups: control, diabetic, alcoholic and diabetic-alcoholic. Samples of the corpus cavernosum were prepared to study protein expression of apoptotic genes (Caspases-3 and 9) by immunohistochemistry and Real-Time PCR. Results The immunoreactivity of Caspases-3 and -9 was diffuse and higher in the treated groups though there was no significant difference between the experimental groups, only when compared with the control group. An increase was observed in the gene expression of Caspases-9 in the diabetic and ethanol-diabetic groups when compared with control and ethanol groups. Conclusions The association of these factors (ethanol and diabetes) probably can affect the apoptosis mechanism in lesions of the cavernous tissue in the rat penis. Both gene and protein expression of Caspase-9 in diabetic and ethanol-diabetic groups suggest the involvement of the apoptosis cascade from this study model.

Abstract Purpose To evaluate the effect of chronic alcoholism on morphometry and apoptosis mechanism and correlate with miRNA-21 expression in the corpus cavernosum of rats. Methods Twenty-four rats were divided into two experimental groups: Control (C) and Alcoholic group (A). After two weeks of an adaptive phase, rats from group A received only ethanol solution (20%) during 7 weeks. The morphometric and caspase-3 immunohistochemistry analysis were performed in the corpus cavernosum. The miRNA-21 expression was analyzed in blood and cavernous tissue. Results Chronic ethanol consumption decreased cavernosal smooth muscle area of alcoholic rats. The protein expression of caspase 3 in the corpus cavernosum was higher in A compared to the C group. There was no difference in the expression of miRNA-21 in serum and cavernous tissue between the groups. Conclusion Chronic ethanol consumption reduced smooth muscle area and increased caspase 3 in the corpus cavernosum of rats, without altered serum and cavernosal miR-21 gene expression.

Abstract Purpose: To evaluate the expression of endothelial and inducible NOS in addition to the miRNA-27b in the corpus cavernosum and peripheral blood of healthy rats, diabetic rats, alcoholic rats and rats with both pathologies. Methods: Forty eight Wistar rats were divided into four groups: control (C), alcoholic (A), diabetic (D) and alcoholic-diabetic (AD). Samples of the corpus cavernosum were prepared to study protein expressions of eNOS and iNOS by immunohistochemistry and expression of miRNA-27b in the corpus cavernosum and peripheral blood. Results: Immunohistochemistry for eNOS and iNOS showed an increase in cavernosal smooth muscle cells in the alcoholic, diabetic and alcoholic-diabetic groups when compared with the control group. Similarly, the mRNA levels for eNOS were increased in cavernosal smooth muscle (CSM) in the alcoholic, diabetic and alcoholic-diabetic groups and miRNA-27b were decreased in CSM in the alcoholic, diabetic and alcoholic-diabetic groups. Conclusion: The major new finding of our study was an impairment of relaxation of cavernosal smooth muscle in alcoholic, diabetic, and alcoholic-diabetic rats that involved a decrease in the nitric oxide pathway by endothelium-dependent mechanisms accompanied by a change in the corpus cavernosum contractile sensitivity.

PURPOSE : Bladder augmentation is an effective surgical procedure for increasing bladder capacity and reducing pressure on the urinary system. It is indicated for patients with anomalies such as spina bifida, myelomeningocele, urethral valve and bladder exstrophy, who progress with low tolerance of medication. CASES : This was a retrospective study conducted on pediatric patients submitted to bladder augmentation from 2000 to 2011. RESULTS : 34 patients aged 4 to 17 years were submitted to bladder augmentation, 30 of them with an ileal loop and 4 with a ureter.A continent urinary shunt was performed in 16 patients, the Mitrofanoff conduit was associated in 15, and the Macedo technique was used in one. Mean follow-up was 34.35 months (1 to 122 months). Mean creatinine was 1.5 ng/ml (0.4 to 7.5 ng/ml) preoperatively and 1.78 ng/ml postoperatively. Three patients required a renal transplant during follow-up. There was improvement or resolution of vesicoureteral reflux in 83.5% of the kidneys on the right and in 75% on the left. Bladder capacity increased, on average, from 152.5 ml to 410 ml. The main complications were vesical lithiasis in 3 patients and conduit perforation in one. CONCLUSION : Bladder augmentation showed good results in this series, preserving renal function in most of the patients.

PURPOSE: To determine the effect of a single dose of adriamycin (ADR) to induce anorectal malformations (ARMs) and determine the effect of folic acid (FA) in this model. METHODS: Ten female Wistar rats were divided randomly in two groups. Group A - ADR; Group B - FA+ADR. Dams from group B received daily, since two weeks before the pregnancy to the end of pregnancy, FA (50mg/kg) by gavage. Dams from both groups received ADR (6mk/kg) by intraperitoneal injection on gestational day (GD) 8. Their fetuses were harvested by cesarean section on GD21 and were examined looking for ARMs. The thickness of anal stratified squamous epithelium (ASSE) and intestinal epithelium (IE) were analyzed. p≤0.05*. RESULTS: 81 fetuses were harvested. The number of fetuses; number of ARMs; mean (∆%) (± SD) were determined to be, respectively: ADR - 41[29;65%(±37%)] versus FA+ADR - 40[04;16%(±36%)] (p=0.05). AMRs were significantly lower in FA+ADR group than in ADR group (p=0.05). Objective: An unclear issue is whether gender may influence at cardiac remodeling after myocardial infarction (MI). We evaluated left ventricle remodeling in female and male rats post-MI. Methods: Rats were submitted to anterior descending coronary occlusion. Echocardiographic evaluations were performed on the first and sixth week post-occlusion to determine myocardial infarction size and left ventricle systolic function (FAC, fractional area change). Pulsed Doppler was applied to analyze left ventricle diastolic function using the following parameters: E wave, A wave, E/A ratio. Two-way ANOVA was applied for comparisons, complemented by the Bonferroni test. A P≤=0.05 was considered significant. Results: There were no significant differences between genders for morphometric parameters on first (MI [Female (FE): 44.0±5.0 vs. Male (MA): 42.0±3.0%]; diastolic [FE: 0.04±0.003 vs. MA: 0.037±0.005, mm/g] and systolic [FE: 0.03±0.0004 vs. MA: 0.028±0.005, mm/g] diameters of left ventricle) and sixth (MI [FE: 44.0±5.0 vs. MA: 42.0±3.0, %]; diastolic [FE: 0.043±0.01 vs. MA: 0.034±0.005, mm/g] and systolic [FE: 0.035±0.01 vs. MA: 0.027±0.005, mm/g] of LV) week. Similar findings were reported for left ventricle functional parameters on first (FAC [FE: 34.0±6.0 vs. MA: 32.0±4.0, %]; wave E [FE: 70.0±18.0 vs. MA: 73.0±14.0, cm/s]; wave A [FE: 20.0±12.0 vs. MA: 28.0±13.0, cm/s]; E/A [FE: 4.9±3.4 vs. MA: 3.3±1.8]) and sixth (FAC [FE: 29.0±7.0 vs. MA: 31.0±7.0, %]; wave E [FE: 85.0±18.0 vs. MA: 87.0±20.0, cm/s]; wave A [FE: 20.0±11.0 vs. MA: 28.0±17.0, cm/s]; E/A [FE: 6.2±4.0 vs. MA: 4.6±3.4]) week. Conclusion: Gender does not influence left ventricle remodeling post-MI in rats.

PURPOSE:To evaluate the effect of short and long term alloxan-induced diabetes on bladder and urethral function of female rats, and also describing its correlated morphological alterations.METHODS:Thirty five female rats were divided into three groups: G1 (n=9), control group; G2 (n=17), six weeks alloxan-induced diabetic rats; G3 (n=9), 20 weeks alloxan-induced diabetic rats. Functional evaluation was performed by cystometry and simultaneous measurements of the urethral pressure during bladder filling and voiding. Morphological evaluation was also performed with measurement of bladder and urethral fibrosis and collagen content and thickness of lamina propria and smooth muscle layers.RESULTS: The peak bladder pressures and contraction amplitudes were decreased in 100% and 47% of the G3 and G2 groups respectively, when compared to control. Bladder overactivity was observed in 53% of the G2 group.CONCLUSION:Alloxan-induced diabetes urethropathty in female rat was associated to bladder morphological alterations as higher thicknesses of it lamina propria, detrusor and adventicea.

PURPOSE: To evaluate the effect of diabetes mellitus and of sildenafil citrate on female urethral function.METHODS: Twenty nine female rats were divided into four groups: G1 - (n=9), normal rats; G2 - (n=6), normal rats treated with sildenafil citrate; G3 - (n=9) rats with alloxan-induced diabetes; G4 - (n=5) rats with alloxan-induced diabetes treated with sildenafil citrate. Under anesthesia, urodynamic evaluation was performed by cystometry and urethral pressure simultaneously.RESULTS: A significant increase in urethral pressure was observed during micturition.CONCLUSION: Sildenafil citrate can partially reduced urethral pressure in diabetic female rats.

PURPOSE: Fetal hydronephrosis is a frequent finding due to advances in prenatal ultrasonography. The definition of fetal and neonatal urinary tract obstruction is a very difficult task requiring confirmation of reduced renal function and hydronephrosis. In this study we followed a series of consecutive patients with intrauterine hydronephrosis that persisted during post-natal life. METHODS: 116 newborns with antenatal hydronephrosis diagnosed by ultrasound and submitted to a specific post-natal evaluative protocol with a follow-up period of 6 years. RESULTS: In 45 (38.8%) of 116 patients, ureteropelvic junction (UPJ) obstruction was confirmed and surgical correction of the UPJ obstruction was done in 19 patients. From 26 children who were initially submitted to non-surgical treatment, only 6 (23%) needed a surgical approach during follow up. Overall analysis showed that surgery was performed in 25 patients with UPJ obstruction, and the others 20 patients were kept under clinical observation, since normal renal function was confirmed by scintigraphy scans. CONCLUSION: Fetal hydronephrosis due to UPJ obstruction deserves careful postnatal evaluation. UPJ obstruction is the most frequent anomaly and its surgical treatment has very precise indications. The evaluative protocol was useful in identify patients that could be followed-up with a non-surgical approach.OBJETIVO: Com a ampla utilização dos exames ultrassonográficos na avaliação pré-natal, é frequente o diagnóstico da hidronefrose fetal. A definição de obstrução do trato urinário no periodo pós-natal necessita da confirmação de redução da função renal além da hidronefrose. Neste estudo, acompanhamos uma série de pacientes consecutivos com hidronefrose intra-útero que persistiu no periodo pós-natal. MÉTODOS: 116 recém-nascidos com hidronefrose pré-natal diagnosticada pela ultrassonografia foram submetidos a protocolo específico de avaliação e companhados pelo periodo de 06 anos. RESULTADOS: Em 46 (38,8%) dos 116 pacientes foi confirmado o diagnóstico de estenose da junção ureteropiélica (JUP). Conforme os resultados do protocolo aplicado a correção cirúrgica da estenose da JUP foi realizada em 19 pacientes. Das 26 crianças encaminhadas inicialmente para observação clínica, apenas 6 (23%) necessitaram cirurgia durante o seguimento ambulatorial. Na análise geral, o procedimento cirúrgico para correção da estenose da JUP foi indicado em 25 pacientes. Nas outras 20 crianças não houve necessidade da realização da cirurgia. CONCLUSÃO: a hidronefrose fetal requer cuidadosa avaliação pós-natal. A estenose da junção pielo-ureteral é a anomalia mais frequente como causa da hidronefrose, e sua correção cirúrgica tem indicações precisas. O protocolo aplicado foi útil em diferenciar pacientes que não necessitaram cirurgia para tratamento da estenose da JUP.

PURPOSE: To investigate the effect of lovastatin on renal ischemia followed by reperfusion. METHODS: Thirty one Wistar rats submitted to left renal ischemia for 60 minutes followed by contralateral nephrectomy were divided into two groups: A (n =17, control, no treatment), and B (n=14, lovastatin 15 mg/kg/day p.o. ten days before ischemia). The animals were sacrificed at the end of ischemia, after 24 hours and at seven days after reperfusion. Survival, serum urea and creatinine levels and renal mitochondrial function were evaluated. RESULTS: Mortality was 29.4% in group A and 0.7% in group B. Urea and creatinine levels were increased in both groups, but the values were significantly lower in group B. Mitochondrial function showed decoupling in 83.4% of group A, as opposed to 38.4/% of group B. CONCLUSIONS: The result shows a protective action of renal function by lovastatin administered before ischemia/reperfusion. Since most of the mitochondrial fraction presented membranes with the ability to maintain ATP production in group B, stabilization of the mitochondrial membrane should be considered as part of the protective action of lovastatin on renal function in ischemia/reperfusion.OBJETIVO: Investigar a ação da lovastatina na isquemia renal seguida de reperfusão. MÉTODOS: Trinta e um ratos Wistar submetidos à isquemia renal esquerda durante 60 minutos, seguida da nefrectomia contralateral, foram distribuídos em dois grupos: A (n=17, controle, sem tratamento) e B (n=14, recebendo 15 mg/Kg/dia de lovastatina via oral), durante os dez dias que antecederam a isquemia. Os animais foram mortos ao final da isquemia, e com 24 horas e sete dias após a reperfusão. Foram avaliadas a sobrevida, os valores séricos de uréia e creatinina e a função mitocondrial renal. RESULTADOS: A mortalidade foi 29,4% no grupo A e 0,7% no grupo B. Os níveis de uréia e creatinina elevaram-se nos dois grupos, mas foram significativamente menores no grupo B. No grupo A a função mitocondrial renal ficou desacoplada em 83,4% dos ensaios, enquanto que no grupo B isto ocorreu em apenas 38,4% dos ensaios. CONCLUSÕES: Os resultados mostram que a administração de lovastatina antes do episódio de isquemia protege a função renal. No grupo B, como a maior parte da fração mitocondrial isolada apresentou função acoplada à produção de ATP, deve-se também considerar a estabilização da membrana mitocondrial como parte da ação protetora da lovastatina na função renal durante isquemia e reperfusão.

Purpose: To quantify keloid fibroblasts after irradiation with 470nm blue LED, in vitro. Methods: Fibroblasts from keloid and adjacent skin have been obtained from 6 patients. Cells have been cultivated and maintained in DMEM culture medium. In Petri dishes, they were irradiated with energy doses of 6J, 12J and 18J. After 24 h, counting was done by the average of the triplicates for each sample. Results: There were no significant differences in the number of irradiated keloid fibroblasts at the studied doses (p=0.261). In adjacent skin fibroblasts, differences were observed (p=0.025) concerning the doses of 18 J and 6 J (p=0.03). Conclusions: There was a reduction in the number of adjacent skin fibroblasts irradiated with 470nm blue LED at the energy dose of 18 J compared to the ones irradiated at the energy dose of 6 J. There were no changes in keloid fibroblasts counting at any of the doses applied, 24 h after irradiation.Objetivo: Quantificar fibroblastos de quelóide após irradiação com LED azul de 470nm, in vitro. Métodos: Foram obtidos fibroblastos de quelóide e pele adjacente, de seis pacientes. As células foram cultivadas e mantidas em meio de cultura DMEM. Em placas de Petri, receberam irradiação com doses de energia de 6J, 12J e 18J. Após 24 horas a contagem foi feita pela média da triplicata para cada amostra. Resultados: Não houve diferença na quantidade de fibroblastos de quelóide irradiados nas doses estudadas (p=0,261). Observou-se diferença nos fibroblastos de pele adjacente (p=0,025), com relação às doses de 18 J e 6 J (p=0,03). Conclusões: Houve redução dos fibroblastos de pele adjacente irradiados com LED azul de 470 nm na dose de energia de 18 J em relação à dose de 6 J. Não houve alteração na quantidade de fibroblastos de quelóide nas doses aplicadas após 24 horas da irradiação.

PURPOSE: To develop an experimental model of infravesical urinary obstruction in female rats. METHODS: After median caudal laparotomy, the urethra of 14 female rats was delicately separated from the vagina and loosely wrapped with cellophane tape measuring 0.4 x 1.0 cm. The animals were evaluated 4 (n=7) and 8 (n=7) weeks later. Five additional control animals were only subjected to separation of the urethra and vagina and monitored for 12 weeks. RESULTS: After four weeks, three rats presented vesical dilation associated with discrete ureteral ectasis in 2 animals, with the third presenting discrete hydronephrosis in one kidney. After eight weeks, five rats (71.4%) presented vesical distension with bilateral ureterohydronephrosis. No significant changes (p>0.05) in serum urea or creatinine occurred in any group compared to preoperative values. CONCLUSION: We present here an inexpensive and simple method for the slow induction of urethral obstruction in adult female rats, with the development of progressive vesical hypertrophy and ureterohydronephrosis, which may be used as an experimental model for the study of different aspects of obstructive nephropathy.OBJETIVO: Desenvolver modelo experimental de obstrução urinária infravesical em ratas. MÉTODOS: Após laparotomia caudal mediana, as uretras de 14 ratas foram delicadamente separadas da vagina e frouxamente envoltas com fita de celofane medindo 0.4 x 1.0 cm. Os animais foram avaliados 4 (n=7) e 8 (n=7) semanas depois. Cinco animais adicionais (controle) foram submetidos apenas à separação da uretra e da vagina e monitoradas por 12 semanas. RESULTADOS: Após quatro semanas, três ratas apresentaram dilatação vesical associada a discreta ectasia ureteral em 2 animais, com o terceiro apresentando discreta hidronefrose em um rim. Após oito semanas, cinco ratas (71.4%) apresentaram distensão vesical com ureterohidronefrose bilateral. Não ocorreram alterações significativas (p>0.05) nos valores de uréia ou creatinina em qualquer grupo, comparado com valores pré-operatórios. CONCLUSÃO: Apresentamos aqui um método barato e simples para a indução lenta de obstrução uretral em ratas adultas, com desenvolvimento progressivo de hipertrofia vesical e ureterohidronefrose, podendo ser utilizado como modelo experimental para estudo de diferentes aspectos da nefropatia obstrutiva.

PURPOSE: To evaluate the influence of ischemia/reperfusion injury on renal compensatory growth (CGR) and mitochondrial function. METHODS: Forty five Wistar rats were divided in 3 groups: Control Group (GC) - 21 rats were submitted to a sham laparotomy and sacrificed at 1st (6 rats) and 7th (15 rats) postoperative days to evaluate the dry weight of both kidneys and their growth during 1 week (6 rats) and to quantify mitochondrial respiration (9 rats); Group 1 (G1) - 12 rats underwent right nephrectomy and were sacrificed 7 days later for analysis of renal mitochondrial function (6 rats) and dry weight (6 rats). Group 2 (G2) - renal warm ischemia for 60 minutes followed by right nephrectomy was performed in 12 rats; they were sacrificed 7 days later to evaluate renal mitochondrial function (6 rats) and dry weight (6 rats). RESULTS: Dry weight (mg) of left kidneys at 7th day: GC - 219±18, G1 - 281±23 and G2 - 338±39 (GCxG1 p0.05). PURPOSE: to evaluate structural and functional effects of Alloxan- induced diabetes and aging on bladder of rats. METHODS: evaluations were performed in three groups: A - 8 weeks of age, B - 44 weeks of age, C - 44 weeks of age with alloxan-induced diabetes. Muscle layer thickness, extracellular matrix fibrosis and collagen were quantified on digital images of bladder samples. Cystometric evaluations before surgical vesical denervation (SVD), included maximum cystometric capacity (MCC), maximum bladder pressure (MBP), bladder contraction frequency (VCF), duration of bladder contraction (DC), threshold pressure (TP) and bladder compliance (BC). After SVD, maximum cystometric capacity (MCC), BC and maximum urethral closing pressure (MUCP) were also measured. RESULTS: Reduced extracellular matrix fibrosis concentration and contraction strength were found in the bladders of group C. Before SVD, bladder compliance was not different between groups. Alterations were observed in MCC after SVD. CONCLUSIONS: We did not notice smooth muscle hypertrophy in Alloxan-induced diabetic rats after 44 weeks. There was alteration in the total and relative amount of fibrosis and collagen. The cystometric studies support the idea that this morphological alterations are important to determine the different bladder functional patterns found in the aging and the Alloxan-induced diabetic animals.OBJETIVOS: avaliar alterações estruturais e funcionais da bexiga de ratos machos, associadas ao diabetes induzido por aloxano e ao envelhecimento. MÉTODOS: três grupos de animais: A - 8 semanas de idade; B- 44 semanas de idade; C - 44 semanas de idade com diabetes induzido por aloxano, foram avaliados. Realizadas medidas de espessura da camada muscular, fibrose de matriz extracelular e quantidade de colágeno, através de análise de imagem digital dos tecidos. Realizados também testes cistométricos, antes da desnervação vesical cirúrgica (DVC), para avaliar capacidade vesical (CV), intensidade máxima de contração vesical (IMCV) e complacência vesical. Após a DVC, foram avaliadas capacidade vesical após a desnervação (CVAD), complacência vesical (CV) e pressão de perda uretral (PPU). RESULTADOS: não foi observada hipertrofia da camada muscular nas bexigas; houve diminuição da concentração de fibrose da matriz extracelular e diminuição da força contrátil, e aumento da capacidade vesical no grupo C. CONCLUSÕES: a atrofia da camadas muscular da bexiga esta relacionada ao diabetes induzido por aloxano. O envelhecimento, como fenômeno isolado, provoca alterações nos parâmetros funcionais, porém associado ao diabetes, gera alterações na IMCV, CV e CVAD. Existe correlação entre alterações estruturais e funcionais nos animais diabéticos após a desnervação.

PURPOSE: Urodynamic studies in small animals can be performed through urethral sounding or cystostomy. OBJECTIVE: To compare the two methods of urodynamic evaluation in female rats. METHODS: Ten female rats weighing on average 250g, under anesthesia with urethane (1,25 mg/kg) were submitted in three repeats to an urethal catheter of 0,64 mm in external diameter for cystometric measurements of vesicle pressure(VP1) and contraction time (CT1). The catheter was extracted at a constant velocity of 0.05 cm/minute until complete exteriorization and determinations of maximal urethral pressure (UP1) and functional urethral length (FUL1). This was followed by a cystostomy with catheter PE50 and a new determination of the vesical pressure (VP2). After bladder denervation, a new cystometric record indirectly infered the maximum urethral closure pressure (UP2). The peak urethal pressure (UP3) and the functional urethral length (FUL2) were determined in another urethral sounding. The pressure registration system consisted of a continuous infusion pump regulated to a flow of 0.1 ml/minute connected both to the cystostomy catheter (PE-50) or the urethal catheter (0.64mm) and the polygraph Narco-Biosystem. Statistical analysis employed the Wilcoxon non-parametric test RESULTS: Mean VP1= 48,2 mmHg (11,8 SD); Mean VP2 = 38,2 mmHg (9,0 SD) "p" (VP1 X VP2) = 0,0039. Surgical denervation up to the bladder neck level does not compromise urethral function.INTRODUÇÃO: O estudo urodinâmico em ratas pode ser realizado através de sondagem vesical por via uretral ou por cistostomia. O objetivo deste estudo foi comparar estes dois métodos. MÉTODOS: Foram utilizadas 10 ratas da raça Wistar, peso médio de 250 gramas, anestesiadas com uretana (1,25 mg/kg). Inicialmente foi realizado estudo por sonda uretral (0,64 mm de diâmetro externo) para determinação da pressão vesical (PV1) e tempo de contração (TC1), após isto a sonda foi tracionada a velocidade constante (0,05 cm/m) até sua exteriorização pelo meato uretral, avaliando-se a pressão uretral máxima (PU1) e o comprimento funcional uretral (CFU1). Fez-se, então, a cistostomia (sonda PE50) para determinação da pressão vesical (PV2). A seguir, realizou-se desnervação cirúrgica da bexiga e realizou-se novo registro cistométrico para se inferir a pressão uretral indireta (PU2). Logo após, foi passada sonda uretral para determinação da pressão uretral máxima (PU3) e do comprimento funcional uretral (CFU2). O sistema de registro das pressões foi constituído de uma bomba de infusão contínua regulada para 0,1 ml/minuto conectada em Y com o cateter de cistostomia (PE-50) ou cateter uretral (0,64mm) a um polígrafo Narco-Bioystem. A análise estatística foi realizada através do método não paramétrico de Wilcoxon. RESULTADOS: Média PV1= 48,2 mmHg (11,8 SD); Média PV2 = 38,2 mmHg (9,0 SD). "p" (PV1 X PV2) = 0,0039. Média TC1=30,2 s (21,5 SD); Média TC2=20,0 (7 SD) p (TC1 X TC2) = 1,28. A presença do cateter na uretra pode ser um fator obstrutivo. A desnervação cirúrgica, até o nível do colo vesical, não compromete a função uretral.

PURPOSE: To evaluate the influence of chlorpromazine (CPZ) on renal function and lipid peroxidation in a rat model of kidney ischemia/reperfusion injury. METHODS: Forty eight Wistar rats underwent a laparotomy for hilar clamping of left kidney with a bulldog clamp for 60 minutes followed by organ reperfusion and contralateral nephrectomy. Of these, 26 received 3mg/kg of CPZ intravenously 15 minutes before renal ischemia (G-E) while the remaining 22 were used as ischemic control group (G-C). Eleven rats of G-E and 8 of G-C were followed for blood urea nitrogen and creatinine determinations before renal ischemia and at 1st, 4th and 7th postoperative days. Samplings of left renal tissue were obtained at 5 minutes (5 rats from each group) and 24 hours (9 G-C and 10 of G-E) of reperfusion for malondialdehy (MDA) content determination. Controls of renal MDA content were determined in kidneys harvested from 6 additional normal rats. RESULTS: Acute renal failure occurred in all animals but levels of BUN and creatinine were significantly lower in G-E (p0.05) and returned near to normal levels 24 hours later. CONCLUSION: CPZ conferred partial protection of renal function to kidneys submitted to ischemia/reperfusion injury that seems to be not dependent on inhibition of lipid peroxidation.OBJETIVO: avaliar a influência da clorpromazina (CPZ) na função renal e na peroxidação lipídica num modelo de lesão de isquemia/reperfusão renal em ratos. MÉTODOS: 48 ratos Wistar foram submetidos à laparotomia para clampamento da artéria renal esquerda durante 60 minutos, seguido da reperfusão e nefrectomia contralateral. Destes animais, 26 receberam 3 mg/kg de CPZ intravenosa 15 minutos antes da isquemia renal (G-E), sendo os 22 animais restantes utilizados como grupo controle isquêmico (G-C). Em 11 ratos do G-E e 8 do G-C foi feita a dosagem de uréia e creatinina sérica antes da isquemia renal e no 1º, 4º e 7º dia pós-operatório. Amostras de tecido do rim esquerdo foram obtidas aos 5 minutos (5 ratos de cada grupo) e 24 horas após reperfusão (9 G-C e 10 G-E) para dosagem de malondialdeído (MDA). Valores controle para níveis de MDA foram obtidos em rins retirados de 6 ratos normais. PURPOSE: To verify if rat kidneys lesioned by ischaemia followed by reperfusion are affected by cyclosporine A (CsA). METHODS: Male Wistar rats were randomly divided into three groups, control (GS) and experimental (G1 and G2). G1 was subdivided in two: G1A composed of animals submitted to 60 minutes ischaemia and G1C with the same ischaemic procedure associated to 20 mg/kg/day CsA. Group G2 was subdivided and treated in the same way as G1 except that ischaemia was applied only for 40 minutes. Clamping the left renal artery followed by right side nephrectomy induced kidney ischaemia. Serum urea and creatinine were quantified on the day of surgery (D0) and in the following day (D1). Twenty four hours after reperfusion the left kidney was removed and histologically analyzed. RESULTS: Group GS had normal values for urea and creatinine both on D0 and D1 and did not show structural alterations. Renal function was not significantly different when G2C was compared to GS (p>0.05). Tissue lesions were smaller in G2C than in the other groups. CONCLUSIONS: Renal function was protected by CsA, which also reduced tissue lesions in the kidneys of rats submitted to 40 minutes ischaemia.OBJETIVO: Verificar se a ciclosporina A (CsA) afeta a lesão provocada pela isquemia seguida de reperfusão em rins de ratos. MÉTODOS: Ratos Wistar machos foram separados em grupos: 1 grupo controle (GS) e 2 grupos experimentais (G1 e G2). O G1 foi dividido em G1A - isquemia de 60 minutos; e G1C - isquemia de 60 minutos associada a 20 mg/kg/dia de CsA. O G2 foi dividido em G2C semelhante ao G1, exceto pelo tempo de isquemia de 40 minutos. A isquemia renal foi provocada pelo pinçamento da artéria renal esquerda, após o procedimento, foi realizada a nefrectomia direita. Dosagem de uréia e creatinina séricos foram feitos no dia da cirurgia (D0) e no dia seguinte (D1). Após 24 horas de reperfusão o rim esquerdo foi removido para análise histológica. RESULTADOS: No GS não foram observados alterações de uréia e creatinina em D0 e D1, tampouco alterações estruturais. A comparação entre GS e G2C não mostrou diferença na função renal (p>0,05); o grau de lesão tecidual foi menor em G2C do que nos demais grupos experimentais. CONCLUSÃO: A CsA protegeu a função renal e reduziu a lesão tecidual em rins de ratos submetidos a 40 minutos de isquemia.

INTRODUÇÃO: A oxibutinina atua como agente anticolinérgico que tem ação anti-muscarínica e, principalmente, ação antiespasmódica na musculatura lisa vesical. Assim, ela causa aumento da capacidade vesical e diminui a frequência miccional e bloqueia o estímulo inicial da micção. OBJETIVO: Verificar se a oxibutinina atua sobre a hiperatividade vesical causada pela cistite hemorrágica, dependente do óxido nítrico. MÉTODOS: Foram estudados dois grupos de animais. O controle com 5 ratas e o experimental com 10 ratas, cujos pesos variaram entre 200g a 250g. A cistite hemorrágica foi provocada pela injeção intraperitoneal de ciclofosfamida 200mg/kg, na véspera do experimento. Após 24 horas, as ratas foram anestesiadas com uretana 1,25mg/kg. A seguir, foi feita cistostomia com cateter P-50. Esse cateter foi conectado em Y a uma bomba de infusão contínua com fluxo de água de 0,3ml/min e a um polígrafo para o registro da cistometria. O registro cistométrico foi feito com a velocidade do papel de 0,05cm/seg, com sensibilidade de 20 e calibração para um curso de 60mm para uma pressão de 100mmHg. Os parâmetros estudados foram: freqüência de contração (Fc), intensidade das contrações (Ic), tempo de enchimento vesical (Te), tempo de contração vesical (Tc) e capacidade vesical (Cv), que foi determinado pelo Te x Fluxo. Esses parâmetros foram determinados por suas médias durante o período de observação de 10 min. Após o registro, foi infundido por gavagem 71 mg/kg de cloridato de oxibutinina. Uma hora depois foi feita nova cistometria. A análise estatística foi feita pelo método de Kruskal-Wallis que comparou os valores do grupo controle com o experimental. O p foi considerado significante quando menor que 0,05. RESULTADOS: A comparação entre os dois grupos dos parâmetros estudados antes da infusão do cloridrato de oxibutinina mostrou: Fc - p=0,007; Ic - p=0,0002; Te - p=0,768; Tc - p=0,492; Cv - p=0,056 A comparação dos parâmetros estudados depois da droga mostrou: Fc - p= 0,055; Ic - p=0,0002; Te - p=0,957; Tc - p=0,181; Cv - p=0,206. CONCLUSÕES: O cloridrato de oxibutinina nesse modelo experimental atuou de forma a alterar somente a freqüência das miccções, controlando a hiperatividade e não promovendo alterações nos demais parâmetros estudados.

A prevalência da incontinência urinária no idoso varia de 8 a 34% segundo o critério ou método de avaliação. A principais causas são: alterações teciduais da senilidade que comprometem o trato urinário inferior e o assoalho pélvico, do sistema nervoso central e periférico, alterações hormonais como a menopausa, poliúria noturna, alterações psicológicas, hiperplasia prostática benigna, doenças concomitantes e efeitos colaterais de medicamentos. A incontinência pode ser transitória ou permanente. Além da anamnese cuidadosa para caracterização das perdas urinárias, a busca de causas associadas ou concomitantes e o diário miccional, recorre-se com freqüência a exames especializados como a urodinâmica. O diagnóstico preciso é importante para o manejo adequado que pode requerer apenas medidas conservadoras baseadas em orientações e mudanças de hábitos, como o uso de medicamentos, ou então métodos invasivos que incluem procedimentos cirúrgicos específicos.

PURPOSE: Urinary tract infections (UTI) are one of the most common infectious diseases diagnosed. UTI account for a large proportion of antibacterial drug consumption and have large socio-economic impacts. Since the majority of the treatments begins or is done completely empirically, the knowledge of the organisms, their epidemiological characteristics and their antibacterial susceptibility that may vary with time is mandatory. OBJECTIVE: The aim of this study was to report the prevalence of uropathogens and their antibiotic susceptibility of the community acquired UTI diagnosed in our institution and to provide a national data. METHODS: We analyzed retrospectively the results of urine cultures of 402 patients that had community acquired urinary tract infection in the year of 2003. RESULTS: The mean age of the patients in this study was 45.34 ± 23.56 (SD) years. There were 242 (60.2%) females and 160 (39.8%) males. The most commonly isolated organism was Escherichia coli (58%). Klebsiella sp. (8.4%) and Enterococcus sp.(7.9%) were reported as the next most common organisms. Of all bacteria isolated from community acquired UTI, only 37% were sensitive to ampicillin, 51% to cefalothin and 52% to trimethoprim/sulfamethoxazole. The highest levels of susceptibility were to imipenem (96%), ceftriaxone (90%), amikacin (90%), gentamicin (88%), levofloxacin (86%), ciprofloxacin (73%), nitrofurantoin (77%) and norfloxacin (75%). CONCLUSION: Gram-negative agents are the most common cause of UTI. Fluoroquinolones remains the choice among the orally administered antibiotics, followed by nitrofurantoin, second and third generation cephalosporins. For severe disease that require parenteral antibiotics the choice should be aminoglycosides, third generation cephalosporins, fluoroquinolones or imipenem, which were the most effective.

Background: In 1990, Ramirez introduced a new procedure to close abdominal wall hernia (AWH), called "components separation technique (CST)". Thanks to endoscopy, surgical repair possibilities have risen, reducing the operative trauma and preserving vascular and neuronal anatomical structures. This report aims to describe a single port endoscopic approach for CST to repair the abdominal wall of a patient undergoing surgery for abdominal aneurysm and already subject to placement of a mesh for AWH. Methods: We performed endoscopic-assisted CST, using a single-port access with a gasless technique. Conclusion: CST is a useful procedure to close large abdominal wall incisional hernia avoiding the use of mesh, notably under contamination, when prosthetic material use is contraindicated. The endoscopic-assisted CST produces same results than the conventional open separation technique and also minimised tissue trauma that ensures blood supply and prevents postoperative wounds complications. The described single port method was found to be safe and effective to close large midline abdominal hernias when a primary open or laparoscopic closure is not feasible or when patients have been previously treated with abdominal meshes.

Acute tubular necrosis (ATN) is a major complication of kidney transplantation, so as of urologic and vascular surgeries. In transplantation, although organ perfusion with proper solution are feasible and at least partially effective, new approaches remains needed to avoid lost of graft function due to ischemic insults, and by this way, chlorpromazine may play this hole. Sixteen male rats were evaluated by scintigraphy (dynamic renal scan with Tc-99m-MAG3), before and after surgically promote ischemia of left kidney. OBJETIVO: Investigar alguns dados epidemiológicos do câncer vesical bem como fatores de risco. MÉTODOS: Foram selecionados 125 pacientes atendidos no período 1980-2002, com idade média de 63,6±11,3 anos, sendo 97 (77,6%) homens e 28 (22,4%) mulheres. As proporções de tumores G1, G2 e G3 foram: 48%, 35,2% e 16,8%. As proporções dos estádios da lesão primária foram: pTa-1 - 85 ( 68%) e pT2-4 - 40 ( 32%). Entre os pacientes com tumores superficiais houve recidiva em 41 (48,2%). RESULTADOS: O grau da lesão primária mostrou relação com o estádio T (pOBJECTIVE: To analyse some epidemiological data of patients with transitional cell carcinoma of the bladder (TCC) and the predictive value of conventional risk factors. INTRODUÇÃO: A rejeição imunológica é uma das principais causas da perda de órgãos transplantados. A tentativa do controle da reação imunológica é clinicamente feita através da imunossupressão inespecífica e experimentalmente também por bloqueio específico. O alotransplante cardíaco em ratos pela técnica de ONO,K é um bom método para avaliação clínica da rejeição e de estudos voltados para o controle da rejeição. Objetivo : estudar o efeito de um anti-antisoro linfocitário, anti-linfócitos do doador sobre a rejeição do alotransplante cardíaco de ratos Wistar para ratos Holtzman. MÉTODOS: o soro anti-linfocitário (SAL) foi obtido através da imunização de coelhos com linfócitos obtidos de gânglios linfáticos da cadeia mesentérica de ratos Wistar, em solução de Tyrode, contendo 3x10(9) células/ ml. A inoculação de 3 coelhos foi feita com 1 ml da suspensão celular e 1 ml de adjuvante completo de Freund. Duas semanas após a primeira inoculação fez-se 4 doses semanais de reforço. Os coelhos foram sangrados na 5ª semana, quando então foram separados os soros. A titulação dos soros foi realizada pelo teste de citotoxicidade, sendo verificado que ambos apresentaram título de 1:1024. A dosagem de proteínas mostrou albumina com 3,1 e 2,7 g% e globulinas com 3,5 e 2,9 g%, sendo o normal 3,7 e 2,2 g% respectivamente. Os dois SAL foram misturados. Duas cabras foram inoculados, com 3 ml da mistura desses SAL, associados a 2 ml de adjuvante de Freund. As doses de reforço com 5 ml do SAL foram iniciadas 2 semanas após. A cabra A recebeu 8 doses (1,4 g de globulinas). A cabra B recebeu 4 doses de reforço (0,7 g de globulinas). Uma semana após a última inoculação retirou-se 125 ml de sangue de cada cabra, fazendo a separação dos anti-soro anti-SAL (ASAL). Uma terceira cabra C foi imunizada com soro normal de coelho. A determinação de precipitinas foi feita pelo método de OUCHTERLONY. O ASAL A teve título de 1:64 e B e C título de 1:128. Os ASAL A e B foram capazes de bloquear "in vitro" a atividade citotóxica do SAL até a diluição de 1:2 do SAL. O soro de cabra anti-soro normal de coelho (SCANC) não foi capaz de bloquear a citotoxicidade do SAL. Os animais submetidos a transplante cardíaco foram divididos em 2 grupos controles um normal com 10 ratos (C1) e outro (C2) com 5 ratos que recebeu 1,0 ml endovenoso de SCANC. O grupo de ratos testes A foi composto por 19 ratos distribuídos em 3 subgrupos. Subgrupo A1 com 5 ratos recebeu 0,5 ml do ASAL A, via endovenosa, logo após a cirurgia,o subgrupo A2 com 7 ratos recebeu 1.0 ml do ASAL A nas mesmas condições e o subgrupo A3 também com 7 ratos recebeu 1,0 ml no dia da cirurgia e 1,0 ml nos outros 2 dias consecutivos. O grupo de ratos testes B que recebeu o ASAL B foi igual ao grupo A. A avaliação dos corações transplantados foi diária através da palpação abdominal. O tempo máximo de seguimento foi de 243 dias. Os corações considerados rejeitados foram retirados e feito estudos histológicos. RESULTADOS: o período de rejeição dos grupos foi : controles C1 e C2 foram 11,9 e 14,6 dias, respectivamente; no subgrupo A1 apenas um rato teve sobrevida cardíaca significante (153 dias), nos demais ela variou de 9 a 15 dias; no subgrupo A2 a sobrevida do coração foi significante e variou de 23 a 230 dias; no subgrupo A3 apenas 5 corações tiveram sobrevida significante que variou de 29 a 190 dias. A sobrevida dos corações transplantados do grupo B foi significante para um animal de cada subgrupo (120,132 e 129 dias). Os corações com sobrevida longa foram retirados batendo. Os demais corações foram rejeitados dentro do período de variação dos grupos controles. CONCLUSÕES: O soro de cabra anti-soro anti-linfócitos do doador, com maior período de imunização, foi capaz de bloquear a resposta imune de rejeição dos corações transplantados nas doses de 1,0 e 3,0 ml. Os ratos que não promoveram a rejeição aguda dos corações transplantados não apresentaram anticorpos citotóxicos circulantes. O fator causador do bloqueio parace n��o estar vinculado aos bloqueios de citotoxicidade "in vitro" e do teor de precepitinas do SAL.OBJECTIVE: To study the immunosupression efficacy an specific anti-antilymphocytic serum prepared in goats in a model of cardiac allografts in rats. METHODS: Three rabbits were immunized with lymphoid cells obtained from mesenteric lymphatic nodes of Wistar rats. Each one received subcutaneously 3x10(9) cells mixed with Freund's adjuvant. After 2 weeks, they were injected with the same amount of cells at weekly intervals for 4 additional times. In the 5th week they were bled and their serum were mixed. This serum, which had a cytotoxic titer of 1:1024, was used to immunize 2 goats that gave rise to the anti-antilymphocytic serum (AAS-1 and AAS-2). As control we immunized 1 additional goat with normal rabbit serum (ANS). The gel diffusion technique (AAS x rabbit serum) showed precipitation bands against till the following dilution: AAS-1 - 1/64, AAS-2 - 1/128 and ANS 1/124. Both AAS were able to block the in vitro lymphocytotoxity of goat antilymphocytic serum till dilution of 1:2 while ANS did not. The hearts from Wistar rats (donors) were transplanted in Holtzman rats. The transplanted rats were divide in groups: C1 - 11 animals (control that received no serum); C2 - 5 animals (control that received 1ml of goat normal serum); A- 19 animals - A1 with 5 rats injected intravenously in the day of surgery with 0.5ml of AAS-1, A2 with 7 rats injected with 1ml of AAS-1 only in the of surgery, and A3 with 7 rats that received 1ml of AAS-1 in days 0, 1 and 2 postoperatively; and group B with 19 rats (B1, B2 and B3) treated as group A except with the AAS-2 serum. RESULTS: Mean heart survival in groups C1 and C2 was respectively 11.9 and 14.6 days Survival range in the subgroups A1 and A2 were respectively 9 to 230 days and 23 to 230 days. In subgroup A3 heart survival was prolonged till 29 to 190 days in 5 animals. In group B only 3 animals had prolonged (120, 130 and 129 days) heart survival in comparison with the control groups. CONCLUSION: Anti-antilymphocytic serum against donor antigen is able to suppress rejection of cardiac allograft in rats.

OBJETIVO: Investigar a expressão imuno-histoquímica da p53 com fator de risco em carcinoma de células transicionais da bexiga (CCT). MÉTODOS: Foram estudados restrospectivamente 90 pacientes com CCT com idade média de 71 anos: G1 - 45, G2 - 29, G3 - 16, pTa-1 - 62 e pT2-4 - 28. Entre os pacientes com tumores não invasivos houve recidiva vesical em 35 casos (55,5%). Os tumores superficiais foram tratados por ressecção trans-uretral associados ao BCG (>G1), e os invasivos por cistectomia radical. O tempo médio de seguimento dos pacientes foi de 55 meses e 25 deles faleceram da doença. A expressão imuno-histoquímica foi estudada em peças preservadas em formol 10% e blocos de parafina pelo método da avidina-biotina-imunoperoxidase. Considerou-se p53 positivo o tumor com índice de marcação nuclear superior a 10%. RESULTADOS: A expressão da p53 mostrou associação com o grau do tumor e com o estádio da lesão primária (p=0,01), mas não com o tamanho do tumor vesical (p=0,25) ou com a taxa de recidiva dos tumores superficiais (p=0,81). Houve forte correlação entre o padrão de marcação da p53 com metástases (p=0,002) e com a sobrevida dos pacientes (p=0,003). CONCLUSÃO: A expressão da p53 mostrou valor preditivo para grau tumoral, estádio, incidência de metástases e sobrevida dos pacientes, mas não para recidiva vesical dos tumores superficiais.

OBJETIVO: Investigar as características e a evolução de homens adultos portadores de tumores germinativos do testículo. MÉTODOS: Estudamos as características e a evolução 29 pacientes tratados (14 seminomas e 15 não seminomas). O tempo médio de seguimento foi de 56 meses para os seminomas e de 40 meses para os não seminomatosos. Todos foram submetidos a orquiectomia. Nos estádios II e III associou-se radioterapia para os seminomas, e quimioterapia e linfadenectomia para os não seminomatosos. RESULTADOS: As queixas mais freqüentes foram aumento de volume testicular (57%) e dor (30%). Nos seminomas a idade média foi de 41,2 anos e nos não seminomas foi de 29,2 anos. Antecedente de criptorquidia foi assinalada em 28,5% dos seminomas e em 15,5% dos não seminomatosos. As proporções respectivas de estádios I, II e III foram de 79%, 14% e 7% em seminomas, e 40%, 27% e 33% em não seminomas. Os seminomas não provocaram elevação dos marcadores AFP ou b-HCG enquanto os não seminomatosos elevaram esses marcadores respectivamente em 46,6% e 33,3% dos casos. Morte pela doença ocorreu em 1 caso de seminoma e 3 de não seminomas, mas não houve diferença na sobrevida entre os grupos. CONCLUSÃO: A criptorquidia continua sendo um fator predisponente importante na etiologia dos tumores germinativos. Apesar dos tumores não seminomatosos se apresentarem em estádios mais avançados a sobrevida dos pacientes não difere da apresentada pelos portadores de seminomas.

OBJETIVO: Pesquisar a viabilidade da alça ileal invertida como substituto ureteral. MÉTODOS: Quarenta cães foram divididos em 5 grandes grupos de 8 animais, cada um subdividido em 2 subgrupos de acordo com a data do sacrifício (I - sacrifício em 12-15 dias de pós-operatório, e II - 60 dias de pós-operatório). Nos grupos A e B fez-se a substituição ureteral parcial com selo de íleo invertido, após abertura lateral do ureter direito com 2 (Grupo A) ou 4 cm (Grupo B) de extensão. Nos demais grupos fez-se a substituição do segmento completo do ureter usando-se um tubo de íleo invertido de comprimento equivalente ao segmento do ureter ressecado com 2cm (Grupo C) e 6cm (Grupos D e E) de extensão. A mucosa ileal foi removida por raspagem ou diérese. A permeabilidade ureteral foi testada por urografia excretora e por exploração anatômica com cateter após o sacrifício. As peças foram estudas histologicamente após preparação e coloração com hematoxilina-eosina. RESULTADOS: O número de animais com urografia normal após o seguimento foi: A-2, B-2, C-3, D-2 e E- 2. A histologia revelou que a serosa ileal se reveste com urotélio, não como decorrência de metaplasia, mas do crescimento a partir da borda da mucosa ureteral. CONCLUSÃO: Conclui-se que o íleo invertido não é um bom substituto ureteral.

O objetivo desse estudo foi investigar a validade de um índice de risco (IR) no prognóstico de metástases ganglionares no carcinoma epidermóide do pênis (CEP). Foram analisados 53 pacientes com CEP atendidos no Hospital das Clínicas da FMRP-USP, de janeiro de 1978 a dezembro de 1995. A idade variou de 32 a 97 anos. Os tumores foram graduados de acordo com a classificação de Broders e estadiados retrospectivamente (TNM 1999). O IR foi determinado pela soma de TeG (IR = T+G). A linfadenectomia inguinal bilateral (LD) foi indicada naqueles que apresentaram gânglios suspeitos após a antibioticoterapia ou que vieram apresentar alterações durante o período de observação. Faleceram 17 pacientes; A LD foi realizada em 14 pacientes e os 39 restantes permaneceram em observação clínica, sendo que em 8 deles foi feito LD após tempo mediano de 9 meses. Dos pacientes com índice de risco 2 e 3 92% e 80% respectivamente não apresentavam metastase ganglionar, ao passo que a totalidade daqueles com IR=6 tinha gânglios comprometidos. A associação dos grupos 2 e 3, comparada ao conjunto 4, 5 e 6, mostrou percentual maior de comprometimento nestes últimos (p = 0,0046). Dos 36 pacientes inicialmente No, 27 apresentavam IR 2 ou 3, dos quais 5 evoluíram para N+ (19%); os 9 restantes eram IR > 4 e 3 também evoluíram para N+ (33%). Apesar da diferença, não houve significância estatística entre eles (p = 0,3837). Embora tenha ocorrido maior comprometimento naqueles com IR > 4, o índice de risco não foi capaz de identificar previamente pacientes que evoluíram para N+.

O antígeno nuclear de proliferação celular (PCNA) foi descrito como marcador da atividade proliferativa. Correlacionamos a marcação do PCNA com a evolução dos pacientes com carcinoma de células transicionais de bexiga. Revisamos 90 pacientes do HC-FMRP-USP de 1980-2000; com idade variando de 29 a 93 anos e média de 71 anos; sendo 77,8% homens e 22,2% mulheres; seguimento médio de 55 meses (2-231 meses). Constatamos 50% de tumores grau I, (32,2%) grau II e 17,8% grau III; estadiados em pTA-60%, pT1-8,9% e >pT1 31,1%. Utilizamos o anticorpo primário Monoclonal Mouse Anti-Proliferating Cell Nuclear Antigen (PCNA) Clone PC10 (DAKO). Foram contadas 500 células (X400), e utilizado ponto de corte de 50%. A análise estatística foi realizada com o teste de Mann-Whitney. Kaplan-Meier e Logrank Oitenta e sete tumores (96,66%) expressaram PCNA (mediana 72.5%, média 60,16%). Observamos diferença não significativa (P=0,39) na imunomarcação do PCNA entre tumores pT2-4 e pTA-1. A comparação de medias entre G1 e G2 (P=0,087), G1 e G3 (P=0,11) e entre G2 e G3 (P=0,66) não mostraram significado estatístico,assim como entre tumores com recidiva e sem recidiva (P=0,84). Os indivíduos PCNA positivos não tiveram intervalo livre de doença significativamente diferente (P=0,86); entretanto foi significativa a diferença nas curvas de sobrevida (P=0,003) e nas curvas de intervalos livres de metástase (P=0,01). A expressão imunohistoquímica do PCNA não foi efetiva para diferenciar os tumores mais avançados e agressivos, assim como para predizer recidivas. Desta forma, apesar da correlação com o tempo livre de metástase e a sobrevida, não mostrou utilidade prática.

A expressão do MIB-1 é um excelente marcador da atividade proliferativa e correlaciona-se com a agressividade biológica do carcinoma de células transicionais da bexiga.Correlacionamos a expressão do MIB-1 com a evolução dos pacientes. Revisamos 90 pacientes do HC-FMRP-USP entre 1980-2000, com idade entre 29 a 93 anos (média 71 anos);sendo 70 (77,8%) homens e 20 (22,2%) mulheres; e seguidos em média por 55 (2-231) meses. 45 (50%) tumores tinham grau I, 29 (32,2%) grau II e 16 (17,8%) grau III. Os tumores foram estadiados em pTA: 54 (60%), pT1: 8 (8,9%) e pT2-4: 28 (31,1%). Foi utilizado o anticorpo monoclonal anti-MIB-1 (Immunotech). Emprega-se o limite de 10% de núcleos corados como nível de corte para o MIB-1. Utilizamos para análise estatística os testes Mann-Whitney, Kaplan-Meier, e log rank, e nível de significância 5%. A grande procura de órgãos para transplante tornou necessária a aceitação de potenciais doadores pediátricos a fim de aumentar a oferta. Trabalhos anteriores relatando menor sobrevida do enxerto e taxas maiores de complicação fizeram com que cirurgiões considerassem o assunto com cautela. Esse é um estudo retrospectivo com o objetivo de analisar os resultados de transplantes renais em bloco de doadores infantis (< 2 anos) no HCFMRP-USP entre 1998 e 2000. Um total de cinco pacientes foi submetido a transplante renal em bloco de doadores com idades entre 9 meses e 2 anos de vida, pesando entre 8 e 14 kg. Apenas um paciente perdeu o enxerto no oitavo dia após transplante devido à trombose. Dois pacientes tiveram complicações ureterais, um com necrose do ureter e fístula urinária e outro com ruptura completa da anastomose ureterovesical. Ambos foram reparados com sucesso. Complicação intra-operatória foi observada em um paciente, com ruptura da anastomose arterial. Corrigiu-se com reperfusão in loco e reanastomose. Outro paciente apresentou ruptura do pólo renal inferior, tratada com nefrectomia polar no quarto dia pós-operatório. Seguimento médio de 20,7 meses mostrou, apesar de tais problemas, creatinina sérica variando entre 0,9 e 2 mg/100 ml, sem complicações sérias. Nossos achados sustentam o uso de rins infantis, em bloco, mesmo de doadores com menos de 2 anos de idade, desde que a necessidade do órgão seja maior que o risco oferecido pelo alto índice de complicações.

Introdução e objetivo - em transplante renal com doador cadáver, a função do enxerto depende da manutenção da integridade celular e subcelular, principalmente mitocondrial. Neste estudo o objetivo foi analisar a função mitocondrial do rins submetidos a período prolongado de isquemia fria, seguido de reperfusão por uma hora, empregando-se, ou não, a clorpromazina previamente à isquemia. Métodos - utilizando autotransplante renal em cães, subdivididos em dois grupos, foram extraidas mitocôndrias de rins submetidos à isquemia fria de 48 horas, seguida de 1 hora de reperfusão pós-transplante. Um grupo recebeu clorpromazina antes da nefrectomia. A análise da fosforilação oxidativa e do intumescimento osmótico ("swelling") mitocôndrial foi comparada com dados obtidos de rins normais, sem isquemia. Resultados - Os dados obtidos para o estado III e IV da respiração não mostraram diferença significativa entre os grupos experimentais. A primeira fase do "swelling" ocorreu em tempo semelhante em todos os grupos experimentais. Durante a reversão, os grupos I e II se comportaram de maneira estatisticamente semelhante, com frações de reversão de 57%, e 68%, respectivamente, valores significativamente menores que os obtidos para o grupo normal (99%) (grupo I: p = 0,0374 e grupo II: p = 0,0221). Discussão - é conhecida a ação protetora da clorpromazina na isquemia renal normotérmica. Entretanto, os dados aqui obtidos mostram que após 48 horas de isquemia fria, o grupo II (clorpromazina) comportou-se de maneira semelhante ao grupo I (hipotermia isolada) tanto no estudo da fosforilação oxidativa, quanto no "swelling", embora os valores apresentem tendência a serem maiores no grupo II. Isto pode ser devido a alguns fatores, como: 1) a clorpromazina possui efeito protetor mínimo quando o tempo de isquemia é prolongado; 2) seu efeito pode ser afetado ou sua ação protetora sobreposta àquela imposta pela hipotermia; 3) tempo de reperfusão curto para manifestação de seus efeitos.

A marcação imunohistoquímica da proteina p53 foi estudada em tumores da adrenal conservados em formol ou em blocos de parafina, pelo método da avidina-biotina-peroxidase com recuperação antigênica. Foram estudados 24 carcinomas e 26 adenomas com o objetivo de verificar se o marcador mostrava capacidade de distinção entre eles. Em 62,5% dos carcinomas a marcação foi positiva enquanto que nos adenomas foi de 15,4%, diferença essa estatisticamente significante (p=0,0003). A sensibilidade, especificidade e valor preditivo positivo desse marcador para o diagnóstico do câncer foram, respectivamente: 83,3%, 71,8% e 62,5%. Não houve relação entre o índice de marcação e outros parâmetros clínicos, como peso do tumor, estádio local, recidiva e metástases. Os autores concluem que o marcador é útil no diagnóstico diferencial de massas da adrenal, mas não tem relação com a agressividade biológica da neoplasia maligna.

Apresentar os resultados obtidos com técnica de implante ureterovesical em ratos após obstrução ureteral ipsilateral. Treze ratos foram submetidos à obstrução ureteral distal à direita e, após uma semana, realizou-se o implante desse ureter na bexiga seguido da nefrectomia contralateral. No período de observação de 4 semanas foram estudados os níveis séricos semanais de uréia e creatinina e, na quarta semana, foi realizada cistografia para pesquisa de refluxo vésico-ureteral. Nove animais sobreviveram ao período de obstrução ureteral. Observou-se aumento significativo dos valores de uréia e creatinina sérica no segundo dia pós-operatório, que regrediram para níveis semelhantes aos basais já na terceira semana de seguimento. Não foi observado refluxo vésico-ureteral à cistografia. Após o sacrifício, a avaliação da junção uretero-vesical não mostrava sinais de obstrução, havendo regressão da hidronefrose presente antes do implante ureteral. A técnica de reimplante ureteral utilizada é eficaz em proporcionar adequada drenagem do trato urinário, evitando refluxovésico-ureteral, sendo adequada para ser utilizada no estudo da recuperação da função renal após período de obstrução ureteral.

Estudou-se a marcação imunohistoquímica para o MIB-1 (Ki-67) em 24 carcinomas da córtex adrenal. A idade dos pacientes variou de 1 a 53 anos, com mediana de 7,5. Excetuando 1 paciente com doença avançada, todos foram submetidos à adrenalectomia. O tempo de seguimento variou de 1 a 13 anos. No momento do diagnóstico 7/24 pacientes já apresentavam metástases e no decorrer do seguimento 3 pacientes adicionais as desenvolveram. A média do peso tumoral dos sobreviventes livres da doença foi de 400± 687 g enquanto que nos falecidos ou com neoplasia após a cirurgia foi de 508± 335 g, diferença essa sem significado estatístico. Tumores maiores que 200 g mostraram pior prognóstico do que aqueles com peso menor (p = 0,01). Os valores médios do índice mitótico (IM) para tumores em estádio I, II, III e IV foram respectivamente: 17,4± 10,0, 12,4± 11,2, 18,3± 14,0 e 28,3± 21,0. A comparação das médias revelou p>0,2. Os valores médios do índice de proliferação(IP) determinado pelo MIB-1 nos estádios I, II, III e IV foram respectivamente: 201,4± 131,0, 179,6± 98,7, 129,1± 170,2 e 124,0± 99,4. A comparação estatística dessas médias mostrou p>0,3. Valores do IP maiores que 20 estão associados a pior prognóstico. O IP não mostrou relação com o prognóstico.

Fez-se um estudo imunohistoquímico do antígeno nuclear de proliferação celular (PCNA) em 26 adenomas e 24 carcinomas de adrenal através da técnica da avidina-biotina-peroxidase. Foram estudados 35 pacientes tratados com nefrectomia radical e quimioterapia, em 17 (48,5%) dos quais se associou também a radioterapia. A idade média dos pacientes foi de 38± 29 meses e o tempo de seguimento pós-operatório de 69± 66 meses. Óbito pela neoplasia ocorreu em 5 pacientes, todos com histologia favorável. As peças conservadas em formol ou blocos de parafina para imunhistoquímica com anticorpo MIB-1, método da avidina-biotina-peroxidase. Em 13/35 (37,1%) dos tumores a marcação para o MIB-1 foi positiva. As proporções respectivas de marcação para blastema, epitélio e estroma foram: 34,6%, 18,7% e 0%. Não houve diferença estatística entre a marcação imunohistoquímica e com o tipo histológico, estádio e sobrevida dos pacientes.

As infecções recorrentes do trato urinário inferior (ITU) em meninas apresentam um grande número de causas etiológicas. Entretanto tem situações em que não se consegue detectar uma causa. Uma dessas seria o nível de estrógeno baixo. Assim foi objetivo do presente trabalho verificar a ação do estrógeno conjugado tópico em 12 crianças com ITUs de repetição. Dessas 12 meninas 10 tinham a urografia excretora e a uretrocistografia miccional normais e 2 apresentavam refluxo vésico-ureteral e uma delas pielonefrite crônica com disfunção vésico-uretral. O estrógeno tópico foi usado na vulva durante 30 dias, tendo sido medidos os níveis plasmáticos pré e pós-tratamento. Os exames de urina cultura e sedimento urinário também foram solicitados pré e pós-tratamento. Todas as crianças com ITU foram tratadas com antimicrobianos antes do uso do estrógeno conjugado. As crianças foram seguidas por um período de até 4 anos com culturas de urina semanais por 3 meses e depois mensais. Resultados: Onze das 12 meninas ficaram livres de infecções e tiveram suas culturas de urina negativas. Uma persistiu com bacteriúria assintomática por todo o período de seguimento. Nenhuma delas necessitou de antimicrobiano profilático. Ocorreu aumento do calibre anatômico da uretra após o tratamento e a avaliação estatística pelo teste t mostrou nível de significância com p< 0,001 Conclusões: Esses achados sugerem que o estrógeno conjugado de uso tópico pode agir de forma a aumentar a resistência uretral às bactérias, como também promover o aumento do calibre uretral e melhorar a dinâmica miccional Esses resultados necessitam de confirmação através de estudos com número maior de crianças.

PURPOSE: Urinary tract infection is the most common nosocomially acquired infection. It is important to know the etiology and antibiotic susceptibility infectious agents to guide the initial empirical treatment. OBJECTIVE: To determine the prevalence of bacterial strains and their antibiotic susceptibility in nosocomially acquired urinary tract infection in a university hospital between January and June 2003. METHODS: We analyzed the data of 188 patients with positive urine culture (= 10(5) colony-forming units/mL) following a period of 48 hours after admission. RESULTS: Half of patients were male. Mean age was 50.26 ± 22.7 (SD), range 3 months to 88 years. Gram-negative bacteria were the agent in approximately 80% of cases. The most common pathogens were E. coli (26%), Klebsiella sp. (15%), P. aeruginosa (15%) and Enterococcus sp. (11%). The overall bacteria susceptibility showed that the pathogens were more sensible to imipenem (83%), second or third generation cephalosporin and aminoglycosides; and were highly resistant to ampicillin (27%) and cefalothin (30%). It is important to note the low susceptibility to ciprofloxacin (42%) and norfloxacin (43%). CONCLUSION: This study suggests that if one can not wait the results of urine culture, the best choices to begin empiric treatment are imipenem, second or third generation cephalosporin and aminoglycosides. Cefalothin and ampicillin are quite ineffective to treat these infections.
