Background: Children with speech sound disorders (SSD) form a heterogeneous group, with respect to severity, etiology, proximal causes, speech error characteristics, and response to treatment. Infants develop speech and language in interaction with neurological maturation and general perceptual, motoric, and cognitive skills in a social-emotional context. Purpose: After a brief introduction into psycholinguistic models of speech production and levels of causation, in this review article, we present an in-depth overview of mechanisms and processes, and the dynamics thereof, which are crucial in typical speech development. These basic mechanisms and processes are: (a) neurophysiological motor refinement, that is, the maturational articulatory mechanisms that drive babbling and the more differentiated production of larger speech patterns; (b) sensorimotor integration, which forms the steering function from phonetics to phonology; and (c) motor hierarchy and articulatory phonology describing the gestural organization of syllables, which underlie fluent speech production. These dynamics have consequences for the diagnosis and further analysis of SSD in children. We argue that current diagnostic classification systems do not do justice to the multilevel, multifactorial, and interactive character of the underlying mechanisms and processes. This is illustrated by a recent Dutch study yielding distinct performance profiles among children with SSD, which allows for a dimensional interpretation of underlying processing deficits. Conclusions: Analyses of mainstream treatments with respect to the treatment goals and the speech mechanisms addressed show that treatment programs are quite transparent in their aims and approach and how they contribute to remediating specific deficits or mechanisms. Recent studies into clinical reasoning reveal that the clinical challenge for speech-language pathologists is how to select the most appropriate treatment at the most appropriate time for each individual child with SSD. We argue that a process-oriented approach has merits as compared to categorical diagnostics as a toolbox to aid in the interpretation of the speech profile in terms of underlying deficits and to connect these to a specific intervention approach and treatment target. Purpose: Diversity considerations are of paramount significance in the clinical assessment and treatment of speech disorders in bilingual children and adults who speak a less common language (and/or its dialects). While comprehensive assessment is essential for culturally and linguistically diverse (CLD) populations, this is clinically challenging due to the indeterminacy entailed in the process and incumbrances in access to resources. This clinical tutorial outlines key considerations necessary for holistic, ecological, and dynamic assessments of speech disorders internationally, focusing on Greek. Method: Pertinent literature is reviewed to provide the relevant theoretical backdrop for the ecosystemic protocol, both conceptually and schematically. Greek is utilized as an example language, aiming to underscore knowledge, tools, and other resources on clinically relevant aspects of dialectal, interlanguage (adult second language [L2]), and child multilingual (L2/third language) variation in Greek-dominant speakers. Results: The tutorial highlights facets of Greek speech and scripts a primary assessment model for clinical use mostly targeting clinicians that are not speakers or cognizant of the language, also viewing speech in linguistic diversity as a sociolinguistic construct. Recommendations for clinical practices are outlined proposing a specific action plan, exemplified by a case scenario. Conclusions: The demonstrated ecosystemic paradigm in holistic, ecological protocols for clinical assessment of speech disorder in CLD speakers addresses the need for nonlinear, multilevel assessment of variable language exposure and use; being sensitive to individual speaker specificities; the sociolinguistic environment; and employing people-first, culturally sensitive, and dynamic strategies in clinical assessment. This paper presents an operational annotation system for (dis)fluencies in typical and atypical speech, based on existing standard annotation schemes previously established in the literature. Grounded in a functional approach to (dis)fluency, we address some of the conceptual and technical limitations found in previous annotation models, and offer an integrated and inclusive system which is compatible with different multi-layered annotation software such as Praat or ELAN. Our aim is twofold: to create comparable annotated corpora both in typical and atypical speech, and to provide natural language processing and the health sector with applications for diagnostic and therapy in speech disorders. This case report explores clinical treatment efficacy in a Cantonese-speaking child with 22q11.2 Deletion Syndrome where diagnosis and management of velopharyngeal dysfunction can be considered late. All treatment sessions were undertaken via telepractice during the peak of the COVID-19 pandemic in Hong Kong. A hybrid of specialized cleft palate speech treatment techniques and traditional treatment approaches in Speech Sound Disorders were utilized. • Aging affects comprehension of focus sentences in quiet and noise. • Younger adults perform similarly in focus conditions, not in noises. • Older adults show varied performance in focus conditions, not in noises. • Younger adults rarely confuse focus conditions, while older adults do. Older adults experiencing normal aging make up most patients seeking services at audiology clinics. While research acknowledges that the speech perception abilities of aging adults can be diminished in lower-level speech identification or discrimination, there is less concern about how aging affects higher-level speech understanding, particularly in tonal languages. This study aimed to explore the effects of aging on the comprehension of implied intentions conveyed through prosodic features in Mandarin focus sentences, both in quiet and noisy environments. Twenty-seven younger listeners (aged 17 to 26) and 27 older listeners (aged 58 to 77) participated in a focus comprehension task. Their task was to interpret SAVO (subject-adverbial-verb-object) sentences with five focus conditions (initial subject-focus, medial adverbial-focus, medial verb-focus, final object-focus, and neutral non-focus) across five background conditions: quiet, white noise (at 0 and −10-dB signal-to-noise ratios, SNRs), and competing speech (at 0 and −10-dB SNRs). Comprehension performances were analyzed based on accuracy rates, and underlying processing patterns were evaluated using confusion matrices. Younger listeners consistently excelled across focus conditions in quiet settings, but their scores declined in white noise at the SNR of −10-dB. Older adults exhibited variability in scores across focus conditions but not in background conditions. They scored lower than their younger counterparts, with the highest scores observed in the comprehension of sentences featuring a medial adverbial-focus. Analysis of confusion matrices revealed that younger adults seldom mistook focus conditions, whereas older adults tended to comprehend the other focused items as medial adverbials. Older listeners' performance reflects their over-reliance on top-down language knowledge, while their bottom-up acoustic processing decreases when interpreting Mandarin focus sentences. These findings provide evidence of active cognitive processing in prosody comprehension among aging adults and offer insights for diagnosing and intervening with speech disorders in clinical settings. Purpose: The purpose of this study was to explore the communication and social interaction experiences of adolescents with congenital motor speech disorders due to cerebral palsy or Down syndrome, with the aim of identifying clinical and research needs to support the development and implementation of speech-language interventions. Method: Five male youths (ages 14–18 years) with congenital motor speech disorders and one of their parents participated in face-to-face, semistructured interviews designed to understand communication and social experiences in daily life. Interviews were audio-recorded and orthographically transcribed offline. Content was coded according to topic areas emerging in the data. Themes were developed to illustrate the most salient and representative aspects of participants’ experiences according to the phenomenological tradition that recognizes that participants are experts in their “lived experience.” Results: Participants described the youths’ day-to-day communication experiences, including facilitators and barriers to successful social interactions. Thematic analysis revealed three main themes: (a) strong core relationships amidst sparse, superficial interactions in daily life; (b) the complicated picture of why; and (c) how speech-language pathologists can help. Conclusions: Participants reported that the impact of congenital motor speech disorders on social interactions and experiences became more apparent in adolescence than in earlier childhood. Addressing communication challenges to meet the unique social demands of this period requires tailored interventions that target multiple contributing factors beyond speech impairment, such as social communication skills, negative communication partner attitudes, and participation opportunities. Shifting practice toward a life participation approach to communication intervention stands to substantially improve the long-term social outcomes of adolescents with motor speech disorders. Purpose: Functional speech disorders (FSDs), a subtype of functional neurological disorders, are distinguishable from neurogenic motor speech disorders based on their clinical features, clinical course, and response to treatment. However, their differential diagnosis and management can be challenging. FSDs are not well understood, but growing evidence suggests a biopsychosocial basis distinct from structural lesions that cause neurogenic motor speech disorders. Method and Results: Following an overview of FSDs, four patients are described to illustrate the range of clinical manifestations, biopsychosocial contexts, and responses to treatment of FSDs. The path to differential diagnosis is discussed, with particular attention to positive features that led to the FSD diagnosis. Approaches to education, counseling, and management are discussed. Conclusions: This case series demonstrates that FSDs can present with a variety of manifestations including dysfluencies, articulation errors, dysphonia, rate and prosodic abnormalities, and combinations of disruptions in speech subsystems. FSDs may present in the context of known recent or remote physical or psychosocial trauma or, as in many cases, in the absence of an identifiable triggering event. FSDs are recognizable by positive clinical features and should not be considered a diagnosis of exclusion. With appropriate identification, counseling, and treatment, FSDs may resolve, sometimes rapidly; in some cases, treatment may be prolonged or ineffective. [ABSTRACT FROM AUTHOR]

Background: Acquired brain injury (ABI), especially to the right hemisphere, can result in difficulty using or understanding prosodic contours in speech. Prosody is used to convey emotional connotation or linguistic intent and includes pitch, loudness, rate, and voice quality. A disorder in the comprehension or production of prosody is known as aprosodia; despite the communication disability caused by prosodic disorders, the assessment and treatment of aprosodia following ABI has received scant attention. Aims: The aim of this scoping review is to gather and synthesise useful knowledge on aprosodia and provide therapists with an exhaustive document in order to guide clinical decision‐making encouraging active identification and treatment of this disorder. Methods & Procedures: This scoping review, conducted in accordance with PRISMA‐ScR guidelines, investigated the existing literature concerning the assessment and treatments of linguistic and affective aprosodia in adult patients after ABI. A systematic search in four electronic databases (PubMed, CINAHL, Web of Science, ScienceDirect) was conducted for articles written in English, French, or Italian published between 1970 and 2020. After all evaluative criteria were applied, 15 articles were included for final review. Outcomes & Results: Results show the presence of six assessment tools for affective aprosodia and five evaluation tools targeting affective and linguistic prosody. Assessment of aprosodia is generally accomplished through acoustic and perceptual approaches. Current treatments for prosodic disorders focus on expressive aprosodia and have applied mostly two different approaches: imitative and cognitive–linguistic methods. Conclusion & Implications: Findings suggest that aprosodia can be assessed by therapists through various techniques and may be amenable to behavioural treatments. Nevertheless, although there are several assessment tools available, no one currently offers a comprehensive assessment that incorporates an ecological dimension. It therefore seems necessary to continue research in this direction. The rehabilitation of receptive prosody abilities also remains to be explored. What this paper adds: What is already known on the subject: Prosody has a fundamental role in communication and conveys speakers' intentions and emotions. Therefore, a deficit of prosody (aprosodia) after acquired brain injury can reduce social participation and engagement. Assessment tools and rehabilitation treatments are necessary in order to improve this disorder and patients' quality of life. What this paper adds to existing knowledge: The evaluation tools currently available focuses mostly on affective aprosodia, whereas the linguistic prosody is less assessed. There exist two treatments for expressive aprosodia: motoric–imitative and cognitive–linguistic treatments; however, their efficacy is tested on small groups of patients. No treatments targeting receptive aprosodia were found. What are the potential or actual clinical implications of this work?: We need more sensitive and reliable tools and systematic evaluations of all the components of prosody (affective and linguistic, receptive and expressive prosody). We need researches who analyse bigger samples of patients after right hemisphere brain injury and we identified the need of more well‐designed studies and better understanding of the pathophysiology of aprosodia. Purpose: This study investigated the practice patterns of private community speech–language pathologists (SLPs) when treating children with a repaired cleft of the palate with or without a cleft of the lip (CP ± L). Practices were explored in terms of diagnostics and treatment focus, treatment dosage and experienced difficulties when treating children with a CP ± L. Method: Eleven female private community SLPs, who lived in Flanders (i.e. the northern part of Belgium) and were aged between 23 and 62 years participated in this study. Data were collected from semi-structured face-to-face interviews. The qualitative software program NVivo 12 was used for data analysis. The interviews were analysed using an inductive thematic approach. Result: SLPs reported a lack of available information on speech-related cleft care. SLPs expressed the need to receive a referral letter from the hospital in order to make an adequate speech diagnosis. Most therapists reported that they performed an articulatory assessment combined with a language assessment. Most SLPs used a hybrid treatment model including a variety of intervention techniques. These techniques were not always in line with available scientific evidence. SLPs reported the desire to receive practical step-by-step guidelines on how to provide speech intervention to children with a CP ± L. In contrast, there was a strong consensus among the therapists that an individualised treatment plan is necessary. Conclusion: The results of this study have revealed gaps in the dissemination and implementation of scientific evidence relevant to speech services for children with a CP ± L (i.e. a research–practice gap) in Flanders. This study investigated the occurrence of speech-language disorders during the acute phase of recovery in children with acquired brain injury (ABI) with an age between 0 and 16 years. A retrospective chart analysis was performed including 228 children (n = 118 boys, n = 110 girls) who consecutively presented with ABI over a 10-year period (2006–2016) at the children's rehabilitation center at Ghent University Hospital. Descriptive statistical analyses were applied. 71.1% (162/228) of the children who were admitted to the rehabilitation center presented with a speech-language disorder. Within this sample (n = 162), results demonstrated the occurrence of acquired disorders in language (48.9%), speech (35.1%), learning (33.3%), swallowing (21.5%), and early communicative functions (17.4%). The proportion of children presenting with disturbances in early communicative functions differed by ABI cause. More than half (10/18, 58.8%) of the children who presented with ABI following inflammatory processes demonstrated disorders in early communicative functions. Especially in young children who present with inflammatory processes as the ABI cause, speech-language pathologists (SLPs) must be aware of disorders in early speech-language development. There are four classification levels for speech disorders namely dysphonia, dysarthria, dysprosody and dysphasia. In general, speech examination mainly focuses on three main components that are spontaneous speech, auditory comprehension, and oral motor examination. Quick bedside assessment on speech in Bahasa Malaysia is essential to assist the speech language therapist (SLT) and other physicians to determine the disorders. Speech therapy is also essential in monitoring and continuous assessment for patients with speech and language disorders such as dysphasia and dysarthria. Speech clinicians in Hospital Universiti Sains Malaysia (HUSM) have been adapting two most widely used batteries of speech assessment tools namely Western aphasia battery-revised (WAB-R) by Andrew Kertesz and Boston diagnostic aphasia examination (BDAE). These tools have been modified into simple and validated speech assessments in Bahasa Malaysia. This video manuscript will demonstrate the use of both tools in performing bedside speech assessment for patients with speech disorders. The Bahasa Malaysia speech examination should not be difficult when WAB-R and BDAE speech assessment tools are applied. The aim of this simple approach using the adapted version of BDAE and WAB-R is to assist the clinician to achieve quick and accurate diagnosis with a validated scoring system. [ABSTRACT FROM AUTHOR]

Purpose: Functional brain imaging is playing an increasingly important role in the diagnosis and treatment of communication disorders, yet many populations and settings are incompatible with functional magnetic resonance imaging and other commonly used techniques. We conducted a systematic review of neuroimaging studies using functional nearinfrared spectroscopy (fNIRS) with individuals with speech or language impairment across the life span. We aimed to answer the following question: To what extent has fNIRS been used to investigate the neural correlates of speechlanguage impairment? Method: This systematic review was preregistered with PROSPERO, the international prospective register of systematic reviews (CRD42019136464). We followed the Preferred Reporting Items for Systematic Reviews and Meta- Analyses (PRISMA) protocol for preferred reporting items for systematic reviews. The database searches were conducted between February and March of 2019 with the following search terms: (a) fNIRS or functional near-infrared spectroscopy or NIRS or near-infrared spectroscopy, (b) speech or language, and (c) disorder or impairment or delay. Results: We found 34 fNIRS studies that involved individuals with speech or language impairment across nine categories: (a) autism spectrum disorders; (b) developmental speech and language disorders; (c) cochlear implantation and deafness; (d) dementia, dementia of the Alzheimer's type, and mild cognitive impairment; (e) locked-in syndrome; (f) neurologic speech disorders/dysarthria; (g) stroke/aphasia; (h) stuttering; and (i) traumatic brain injury. Conclusions: Though it is not without inherent challenges, fNIRS may have advantages over other neuroimaging techniques in the areas of speech and language impairment. fNIRS has clinical applications that may lead to improved early and differential diagnosis, increase our understanding of response to treatment, improve neuroprosthetic functioning, and advance neurofeedback. Purpose: Limited evidence exists to guide the assessment and treatment of cognitive-communication disorders associated with right hemisphere stroke. The purpose of this study was to obtain information about speech-language pathologists' (SLPs') clinical practices and decision making for this population to understand what practices are being used and identify gaps in clinical practice. Method: A survey was distributed via online ASHA Communities for the Special Interest Groups and other social media platforms. Respondents included 143 SLPs from across the United States representing 3-50 years of experience and a wide range of practice settings. Survey questions probed assessment practices including how tests are selected, what tests are used to diagnose specific deficits, and how confident SLPs were in their diagnoses. Treatment decisions were queried for a small set of disorders. Results: SLPs routinely assess cognitive disorders using standardized tests. Communication disorders are less likely to be formally assessed. Three core right cerebral hemisphere deficits--anosognosia, aprosodia, and pragmatic deficits--are either not assessed or assessed only through observation by 80% of SLPs. Evidence-based treatments are commonly used for disorders of attention, awareness, and aprosodia. Conclusion: Communication disorders are less likely to be formally assessed than cognitive disorders, creating a critical gap in care that cannot be filled by other allied health professionals. Suggestions for free or low-cost resources for evaluating pragmatics, prosody, and awareness are provided to aid SLPs in filling this gap. [ABSTRACT FROM AUTHOR]

Background: Huntington's disease (HD) is a neurological disorder that causes severe motor symptoms that adversely impact health-related quality of life. Patient-reported physical function outcome measures in HD have shown cross-sectional evidence of validity, but responsiveness has not yet been assessed.Objectives: This study evaluates the responsiveness of the Huntington Disease Health-Related Quality of Life (HDQLIFE) and the Quality of Life in Neurological Disorders (Neuro-QoL) physical function measures in persons with HD.Methods: A total of 347 participants completed baseline and at least 1 follow-up (12-month and 24-month) measure (HDQLIFE Chorea, HDQLIFE Swallowing Difficulties, HDQLIFE Speech Difficulties, Neuro-QoL Upper Extremity Function, and/or Neuro-QoL Lower Extremity Function). Of the participants that completed the baseline assessment, 338 (90.9%) completed the 12-month assessment, and 293 (78.8%) completed the 24-month assessment. Standardized response means and general linear models evaluated whether the physical function measures were responsive to self-reported and clinician-rated change over time.Results: Small to moderate effect sizes for the standardized response means supported 12-month and 24-month responsiveness of the HDQLIFE and Neuro-QoL measures for those with either self-reported or clinician-rated declines in function. General linear models supported 12-month and 24-month responsiveness for all HRQOL measures relative to self-reported declines in health, but generally only 24-month responsiveness was supported relative to clinician-rated declines in function.Conclusions: Longitudinal analyses indicate that the HDQLIFE and the Neuro-QoL physical function measures are sensitive to change over time in individuals with HD. Objective: To determine whether timing of palatoplasty (early, standard, or late) is associated with speech and language outcomes in children with cleft palate. Design: Retrospective case series. Setting: Tertiary care children's hospital. Participants: Records from 733 children born between 2005 and 2015 and treated at the Cleft Craniofacial Clinic of a tertiary children's hospital were retrospectively reviewed. Exclusion criteria were cleft repair at an outside hospital, intact secondary palate, absence of postpalatoplasty speech evaluation, syndromes, staged palatoplasty, and introduction to clinic after 12 months of age. Data from 232 children with cleft palate ± cleft lip were analyzed. Interventions: Palatoplasty. Main Outcome Measures: Speech/language delays and disorders at 20 months and 5 years of age based on formal hospital or community-based testing or screening evaluation in the Cleft Craniofacial Clinic; additional speech surgery. Results: Median age at palatoplasty was 12.6 months (range: 8.8-21.9 months). Age at palatoplasty was classified as early (<11 months, n = 28), standard (11-13 months, n = 158), or late (>13 months, n = 46). Late palatoplasty was associated with increased odds of speech/language delays and speech therapy at 20 months, and language delays at 5 years, compared with standard or early palatoplasty (P <.05 for all comparisons). However, speech sound production disorders, velopharyngeal incompetence, tube replacement, and hearing loss were not significantly associated with age at palatoplasty. Conclusions: Late palatoplasty may be associated with short- and long-term delays in speech/language development. Future studies with standardized surgical technique/timing and outcome measures are required to more definitively describe the impact of age at palatoplasty on speech/language development. Background: Differential diagnosis and treatment planning of developmental speech disorders (DSD) remains a major challenge in paediatric speech-language pathology. Different classification systems exist, in which subtypes are differentiated based on their theoretical cause and in which the definitions generally refer to speech production processes. Accordingly, various intervention methods have been developed aiming at different parts of the speech production process. Diagnostic classification in these systems, however, is primarily based on a description of behavioural speech symptoms rather than on underlying deficits. Purpose: In this paper, we present a process-oriented approach to diagnosis and treatment planning of DSD. Our framework comprises two general diagnostic categories: developmental delay and developmental disorder. Within these categories, treatment goals/targets and treatment methods are formulated at the level of processes and rules/representations. Conclusion: A process-oriented approach to diagnosis and treatment planning holds important advantages, offering direct leads for treatment aimed at the underlying impairment, tailored to the specific needs of the individual and adjusted to the developmental trajectory. Purpose: Speech and language disorders are hallmark features of 22q11.2 deletion syndrome (22qDS). Learning disabilities, cognitive deficits, palate abnormalities, velopharyngeal dysfunction, behavioral differences, and various medical and psychiatric conditions are also major features of this syndrome. The goal of this document is to summarize the state of the art of current clinical and scientific knowledge regarding 22qDS for speech-language pathologists (SLPs) and provide recommendations for clinical management. Method: Best practices for management of individuals with 22qDS were developed by consensus of an expert international group of SLPs and researchers with expertise in 22qDS. These care recommendations are based on the authors' research, clinical experience, and literature review. Results: This document describes the features of 22qDS as well as evaluation procedures, treatment protocols, and associated management recommendations for SLPs for the often complex communication disorders present in this population. Conclusion: Early diagnosis and appropriate management of speech-language disorders in 22qDS is essential to optimize outcomes and to minimize the long-term effects of communication impairments. Intervention with children with speech and language difficulties has been proven beneficial compared with no treatment yet, knowing what type of intervention to provide remains a challenge. Studies of English-speaking children indicate that intervention targeting the production of morphological targets may have a positive effect on phonological aspects and vice versa. However, studies have not reported on generalization effects to untreated morphemes and little is yet known about morphological intervention in the context of a highly inflected language. The purpose of the current intervention case study was to investigate the effect of intervention in relation to phonological and morphological targets in Greek, a language characterized by complex inflectional morphology. A single subject research design was used with pre- and post-intervention assessment carried out. The participant was a four-year-old Greek-speaking boy with speech difficulties. The production of /s/, a phoneme used in multiple phonological and morphological contexts was targeted with alternating focus of intervention between phonological and morphological targets. Assessment took place at two levels: macro-assessment to monitor broad changes in speech; micro-assessment to measure therapy-specific changes in the production of treated targets and generalization to untreated targets and control items. There were four phases of intervention with a total of 24 hours of therapy. Significant improvement in performance accuracy was found between assessment scores immediately pre- and post-intervention. Intervention targeting the production of a phoneme in the word stem was not sufficient to accomplish the accurate production of morphemes requiring the same phoneme; intervention directly targeting morphemes was successful. Within-domain generalization was observed in both domains. Improved naming accuracy was observed post-intervention that was maintained at follow-up. Background: Cleft palate is a birth defect when the roof of the mouth contains an opening into the nose. It occurs in about 1:2 per 1000 births. These disorders can result in speech disorders, feeding problems and frequent ear infections. It is the result of tissues of the face not joining properly during development. Speech therapy and dental care may also be needed. Approximately 30 percent of patients having undergone cleft palate repair require secondary surgery for velopharyngeal dysfunction. Magnetic resonance imaging (MRI) is one of the common used tests in variable fields of medicine and surgery. Now trials are done for use of MRI in the postpalatoplasty velopharyngeal dysfunction patients. Objective: This study aims to detect static craniofacial and velopharyngeal measures of repaired cleft palate patients specially repaired muscle state (levator veli palatini muscle) by using MRI and correlate these data to speech state. Patients and Methods: this prospective study conducted in AlAzhar and Beni-suef University Hospitals in the period of February 2018 till November 2018. It is conducted on twenty children who complained of postpalatoplasty velopharyngeal dysfunction and examined by nasendoscopy and speech analysis then MRI palate was done using 1.5 T MRI; to correlate MRI data to speech data. Results: our statistical results revealed significant decrease in the velopharyngeal variants than normal ranges. That prove that static MRI is a valid procedure for diagnosis of anatomical abnormality. But after correlation with speech data it showed that it should be accompanied with dynamic procedure (e.g. Nasendoscopy) especially in patients with normal ranged anatomy. Conclusion: this study was done in variant range of children who suffer from post-palatoplasty speech abnormality, scanned by MRI palate and correlate these data by speech data. Purpose: Individuals with velopharyngeal dysfunction (VPD) often present with hypernasality and airflow distortions during speech that negatively affect communication skills. Direct imaging allows for visualization of the velopharyngeal (VP) mechanism and is used to determine whether surgical intervention can improve VP closure and thus decrease the severity of VPD symptoms. Each technique has advantages and disadvantages. Their utility also depends on findings obtained from the speech-language pathologist's (SLP) speech tasks during the perceptual evaluation and the speech sample elicited during the procedure. This article will review 3 VP imaging techniques within the context of the role of the SLP, articulation analysis, and speech sampling considerations. Two case studies are included to illustrate how an SLP and a plastic surgeon used speech information from the perceptual and imaging evaluations to collaborate throughout the VPD assessment process and make treatment recommendations. Conclusion: Articulation components of the VPD evaluation are referenced frequently but are not commonly incorporated within a discussion of VP imaging techniques themselves. Discussing how different speech characteristics reflect VP function, influence VP function, and ultimately influence the choice and utility of different VP imaging modalities can help promote quality care and best outcomes for patients at all stages of the VPD assessment process. Objective: This study investigates the use of a parental questionnaire to determine its effectiveness as a screening tool in identifying speech-language and/or auditory impairments in children. Patients and Methods: Parents completed speech-language (n = 36) or audiology (n = 23) questionnaires prior to their child’s speech-language and/or auditory evaluations. The speech-language and audiology evaluations were conducted by clinicians at three different universities not familiar with the administration and scoring of the parental questionnaires. A research assistant coded the results of the speech-language and audiology evaluations. Statistical analysis was used to determine if any aspects of the parents’ evaluation of their child’s speech, language, or hearing correlated with the speech-language pathologists’/audiologists’ assessment. The results of the analyses would indicate whether parental reports are a reliable alternative to professionals’ assessment. Results: The speech-language questionnaires for children aged 2–5 years revealed the parents’ perception of their children speaking 3-word sentences significantly (p < 0.05) corresponded to the children’s comprehension and expression skills. The results from the auditory questionnaires revealed that reports of a child’s ear pain were (p < 0.05) correlated with auditory dysfunction. Conclusions: These questionnaires can potentially be used in low-income countries where professional resources are scarce and there are barriers to identifying children with speech-language and/or auditory impairment. Purpose: This study aimed to test whether an approach to distinguishing errors arising in phonological processing from those arising in motor planning also predicts the extent to which repetition-based training can lead to improved production of difficult sound sequences. Method: Four individuals with acquired speech production impairment who produced consonant cluster errors involving deletion were examined using a repetition task. We compared the acoustic details of productions with deletion errors in target consonant clusters to singleton consonants. Changes in accuracy over the course of the study were also compared. Results: Two individuals produced deletion errors consistent with a phonological locus of the errors, and 2 individuals produced errors consistent with a motoric locus of the errors. The 2 individuals who made phonologically driven errors showed no change in performance on a repetition training task, whereas the 2 individuals with motoric errors improved in their production of both trained and untrained items. Conclusions: The results extend previous findings about a metric for identifying the source of sound production errors in individuals with both apraxia of speech and aphasia. Background Oral-diadochokinesis (oral-DDK) tasks are extensively used in the evaluation of motor speech abilities. Currently, validated normative data for older adults (aged 65 years and older) are missing in Hebrew. The effect of task stimuli (non-word versus real-word repetition) is also non-clear in the population of older adult Hebrew speakers. Aims (1) To establish a norm for oral-DDK rate for older adult (aged 65 years and older) Hebrew speakers, and to investigate the possible effect of age and gender on performance rate; and (2) to examine the effects of stimuli (non-word versus real word) on oral-DDK rates. Methods & Procedures In experiment 1, 88 healthy older Hebrew speakers (60-95 years, 48 females and 40 males) were audio-recorded while performing an oral-DDK task (repetition of /pataka/), and repetition rates (syllables/s) were coded. In experiment 2, the effect of real-word repetition was evaluated. Sixty-eight older Hebrew speakers (aged 66-95 years, 43 females and 25 males) were asked to repeat ' pataka' (non-word) and ' bodeket' (Hebrew real word). Outcomes & Results Experiment 1: Oral-DDK performance for older adult Hebrew speakers was 5.07 syllables/s (SD = 1.16 syllables/s), across age groups and gender. Comparison of this data with Hebrew norms for younger adults (and equivalent data in English) shows the following gradient of oral-DDK rates: ages 15-45 > 65-74 > 75-86 years. Gender was not a significant factor in our data. Experiment 2: Repetition of real words was faster than that of non-words, by 13.5%. Conclusions & Implications The paper provides normative values for oral-DDK rates for older Hebrew speakers. The data show the large impact of ageing on oro-motor functions. The analysis further indicates that speech and language pathologists should consider separate norms for clients of 65-74 years and those of 75-86 years. Hebrew rates were found to be different from English norms for the oldest group, shedding light on the impact of language on these norms. Finally, the data support using a dual-protocol (real- and non-word repetition) with older adults to improve differential diagnosis of normal and pathological ageing in this task. In recent years, many methods have been introduced for supporting the diagnosis of stuttering for automatic detection of prolongation in the speech of people who stutter. However, less attention has been paid to treatment processes in which clients learn to speak more slowly. The aim of this study was to develop a method to help speech-language pathologists (SLPs) during diagnosis and treatment sessions. To this end, speech signals were initially parameterized to perceptual linear predictive (PLP) features. To detect the prolonged segments, the similarities between successive frames of speech signals were calculated based on correlation similarity measures. The segments were labeled as prolongation when the duration of highly similar successive frames exceeded a threshold specified by the speaking rate. The proposed method was evaluated by UCLASS and self-recorded Persian speech databases. The results were also compared with three high-performance studies in automatic prolongation detection. The best accuracies of prolongation detection were 99 and 97.1% for UCLASS and Persian databases, respectively. Purpose: Magnetic resonance imaging (MRI), an influential experimental approach, provides valuable information about clinical disorders that can be used to select and/or refine speech and language interventions. Functional MRI (fMRI) in particular is becoming a widespread methodological tool for investigating speech and language. However, because MRI is relatively new and complex, potential consumers need to be able to critically assess the methods used in order to appraise results and conclusions. The authors offer a tutorial that (a) relays foundational knowledge related to the collection and analysis of MRI data in general and fMRI data specifically and (b) presents strategies for evaluating studies that utilize fMRI methods. Method: This tutorial outlines methodological considerations that should be addressed by fMRI researchers and noted by consumers of the research, including clinicians and behavioral researchers who work with neurogenic communication disorders. Results: Readers will be able to evaluate a neuroimaging publication and identify the methodological strengths and weaknesses that potentially influence the integrity of reported findings and interpretations. Purpose: Children with speech sound disorders (SSD) find polysyllables difficult; however, routine sampling and measurement of speech accuracy are insufficient to describe polysyllable accuracy and maturity. This study had two aims: (1) compare two speech production tasks and (2) describe polysyllable errors within the Framework of Polysyllable Maturity. Method: Ninety-three preschool children with SSD from the Sound Start Study (4;0–5;5 years) completed the Polysyllable Preschool Test (POP) and the Diagnostic Evaluation of Articulation and Phonology (DEAP-Phonology). Result: Vowel accuracy was significantly different between the POP and the DEAP-Phonology. Polysyllables were analysed using the seven Word-level Analysis of Polysyllables (WAP) error categories: (1) substitution of consonants or vowels (97.8% of children demonstrated common use), (2) deletion of syllables, consonants or vowels (65.6%), (3) distortion of consonants or vowels (0.0%), (4) addition of consonants or vowels (0.0%), (5) alteration of phonotactics (77.4%), (6) alteration of timing (63.4%) and (7) assimilation or alteration of sequence (0.0%). The Framework of Polysyllable Maturity described five levels of maturity based on children's errors. Conclusions: Polysyllable productions of preschool children with SSD can be analysed and categorised using the WAP and interpreted using the Framework of Polysyllable Maturity. Purpose: Speech and other oral functions such as swallowing have been compared and contrasted with oral behaviors variously labeled quasispeech, paraspeech, speechlike, and nonspeech, all of which overlap to some degree in neural control, muscles deployed, and movements performed. Efforts to understand the relationships among these behaviors are hindered by the lack of explicit and widely accepted definitions. This review article offers definitions and taxonomies for nonspeech oral movements and for diverse speaking tasks, both overt and covert. Method: Review of the literature included searches of Medline, Google Scholar, HighWire Press, and various online sources. Search terms pertained to speech, quasispeech, paraspeech, speechlike, and nonspeech oral movements. Searches also were carried out for associated terms in oral biology, craniofacial physiology, and motor control. Results and Conclusions: Nonspeech movements have a broad spectrum of clinical applications, including developmental speech and language disorders, motor speech disorders, feeding and swallowing difficulties, obstructive sleep apnea syndrome, trismus, and tardive stereotypies. The role and benefit of nonspeech oral movements are controversial in many oral motor disorders. The aim of this cross-sectional study was to determine pediatric dentists' attitudes about speech evaluation in the dental setting and assess their knowledge of speech development and pathology. In October 2013, members of the American Academy of Pediatric Dentistry were invited to participate in an electronic questionnaire. Categories of questions were demographics, attitudes and confidence in speech pathology, and theoretical and practical knowledge of speech development and speech pathology. Theoretical knowledge was assessed using questions about phonetics and speech milestones. Practical knowledge was determined with three 30-second interview-style video clips. A total of 539 responses were received for a response rate of 10.4%. The majority of respondents reported feeling that speech evaluation should be part of the pediatric dental visit (72.8%) and felt confident in their ability to detect speech issues (73.2%). However, they did poorly on the theoretical knowledge questions (41.9%) as well as the practical knowledge questions (8.5%). There was a statistically significant difference in theoretical score between gender and type of occupation (p<0.05). This difference was not observed when examining practical knowledge. This study suggests that although pediatric dentists are in an ideal position to aid in the detection of speech issues, they currently have insufficient training and knowledge to do so. In this article, setting-specific challenges and recent ethical concerns relating to speech-language pathology service provision in skilled nursing/long-term care are discussed in the context of implementing evidence-based practice (EBP). The American Speech-Language-Hearing Association's four-step EBP process is reviewed, and barriers to and limitations of EBP are considered. Resources established by experts in various domains for accessing and efficiently evaluating evidence are provided. Guiding principles of medical ethics are reviewed as foundational elements of patient-centered care, and systematic treatment design practices are offered as means of generating practice-based evidence, which allows for assessment of intervention efficacy and complements the EBP process. Last, the integration of all steps of the EBP model is illustrated in a case study of a patient with moderate to severe hypokinetic dysarthria and comorbidities. It is hoped that these resources, principles, and practices will be of use to clinicians specifically in the skilled nursing facility/ long-term care setting in the provision of high-quality speech-language pathology interventions. Background and Aim: Some children with speech sound disorder (SSD) have difficulty with phonological awareness skills; therefore, the purpose of this study was to survey the correlation between phonological processes and phonological awareness. Methods: Twenty-one children with speech sound disorder, aged between 5 and 6, participated in this cross-sectional study. They were recruited from speech therapy clinics at the Tehran University of Medical Sciences. They were selected using the convenience sampling method. Language, speech sound, and phonological awareness skills were investigated by the test of language development-third edition (TOLD-3), the Persian diagnostic evaluation articulation and phonology test, and the phonological awareness test. Both Pearson’s and Spearman’s correlations were used to analyze the data. Results: There was a significant correlation between the atypical phonological processes and alliteration awareness (p=0.005), rhyme awareness (p=0.009), blending phonemes (p=0.006), identification of words with the same initial phoneme (p=0.007), and identification of words with the same final phoneme (p=0.007). Analyzing the correlation on the basis of the phoneme and syllable structure separately showed there was a significant correlation between the atypical phoneme structure and alliteration awareness (p=0.001), rhyme awareness (p=0.008), blending phonemes (p=0.029), identification of words with the same initial phoneme (p=0.007), and identification of words with the same final phoneme (p=0.003). Conclusion: Results revealed a relationship between phonological processes and phonological awareness in children with speech sound disorder. A survey of 231 Australian speech-language pathologists (SLPs) was undertaken to describe practices regarding assessment, analysis, target selection, intervention, and service delivery for children with speech sound disorders (SSD). The participants typically worked in private practice, education, or community health settings and 67.6% had a waiting list for services. For each child, most of the SLPs spent 10-40 min in pre-assessment activities, 30-60 min undertaking face-to-face assessments, and 30-60 min completing paperwork after assessments. During an assessment SLPs typically conducted a parent interview, single-word speech sampling, collected a connected speech sample, and used informal tests. They also determined children's stimulability and estimated intelligibility. With multilingual children, informal assessment procedures and English-only tests were commonly used and SLPs relied on family members or interpreters to assist. Common analysis techniques included determination of phonological processes, substitutions-omissions-distortions-additions (SODA), and phonetic inventory. Participants placed high priority on selecting target sounds that were stimulable, early developing, and in error across all word positions and 60.3% felt very confident or confident selecting an appropriate intervention approach. Eight intervention approaches were frequently used: auditory discrimination, minimal pairs, cued articulation, phonological awareness, traditional articulation therapy, auditory bombardment, Nuffield Centre Dyspraxia Programme, and core vocabulary. Children typically received individual therapy with an SLP in a clinic setting. Parents often observed and participated in sessions and SLPs typically included siblings and grandparents in intervention sessions. Parent training and home programs were more frequently used than the group therapy. Two-thirds kept up-to-date by reading journal articles monthly or every 6 months. There were many similarities with previously reported practices for children with SSD in the US, UK, and the Netherlands, with some (but not all) practices aligning with current research evidence. Purpose: to determine the clinical applicability of the proposed classification of the lingual frenulum by index and percentage. Methods: the sample of this study consisted of 287 subjects of both sexes aged between 23 and 28 years. The lingual frenulums were classified based on two measures defined with aid of a compass and a digital caliper and it were analyzed in the formulas and proposed pattern from a research of corpses which resulted in formulas for index and percentage calculations. Results: it was observed that the pattern established in the classification of lingual frenulum by the proposal is reliable by percentage. At the index classification a new pattern was found in the studied population. Conclusion: after clinical application, it is suggested that the new pattern proposed by the index be adopted. In percentage proposal, the initial pattern should be maintained, providing effectiveness to both ways of classification of the lingual frenulum in speech-language practice. [ABSTRACT FROM AUTHOR]

Over the past decade speech and language therapy has taken an interesting turn towards the use of information communication technologies (ICTs) for diagnosis of disorders and delivery of therapy. In many cases ICTs have worked as assistive tools to therapists, while in others as sole providers of therapy, especially in remote areas. In this report we provide a brief overview of the most representative articles for applications and assistive technologies used for assessment and intervention purposes in Speech Therapy according to the type of disorders. Good quality normative data are essential for clinical practice in speech-language pathology but are largely lacking for French-speaking children. We investigated speech production accuracy by French-speaking children attending kindergarten ( maternelle) and first grade ( première année). The study aimed to provide normative data for a new screening test - the Test de Dépistage Francophone de Phonologie. Sixty-one children named 30 pictures depicting words selected to be representative of the distribution of phonemes, syllable shapes and word lengths characteristic of Québec French. Percent consonants' correct was approximately 90% and did not change significantly with age although younger children produced significantly more syllable structure errors than older children. Given that the word set reflects the segmental and prosodic characteristics of spoken Québec French, and that ceiling effects were not observed, these results further indicate that phonological development is not complete by the age of seven years in French-speaking children. Purpose: Applying surface electromyography (sEMG) to the study of voice, speech, and swallowing is becoming increasingly popular. An improved understanding of sEMG and building a consensus as to appropriate methodology will improve future research and clinical applications. Method: An updated review of the theory behind recording sEMG for the speech and swallowing systems is provided. Several factors that are known to affect the content of the sEMG signal are discussed, and practical guidelines for sEMG recording and analysis are presented, focusing on special considerations within the context of the speech and swallowing anatomy. Results: Unique challenges are seen in application of sEMG to the speech and swallowing musculature owing to the small size of the muscles in relation to the sEMG detection volume and the present lack of knowledge about innervation zone locations. Conclusions: Despite the challenges discussed, application of sEMG to speech and swallowing has potential as a clinical and research tool when used correctly and is specifically suited to noninvasive clinical studies using between-condition or between-group comparisons for which detection of specific isolated muscle activities is not necessary. Objective: The study was aimed at evaluating the effectiveness of a systematic population-based screening programme for specific language impairment (SLI) in preschool children in Germany. Methods: The study question was divided into a review of (1) evidence from studies evaluating screening programmes, (2) diagnostic instruments in the German language, and (3) studies evaluating speech and language interventions. A systematic database search was conducted between June and October 2007 and was updated in January and again in May 2008. Relevant studies were identified by 2 independent reviewers based on screened titles/abstracts and full texts. Results: 4,806 studies were screened. The only existing controlled screening study did not provide data for SLI. No diagnostic study met the inclusion criteria. Sixteen randomized intervention studies were included, 3 studies contributed to the appraisal of earlier against later initiation of treatment. Most studies were of limited quality. We found indications of short-term positive effects from language therapies in children with SLI. Long-term outcomes were not investigated. No evidence supporting the advantage of earlier treatment initiation was identified. Conclusions: The benefit of population-based language screening of preschool children with SLI is not proven. Controlled screening studies are therefore necessary. For Germany, the accuracy of existing diagnostic instruments has not yet been sufficiently examined. Copyright © 2011 S. EBP requires reasoned evaluation and the integration of different sources of evidence, such as clinical expertise and client perspectives. The authors explain how craft and science can be balanced for evidence-based assessment, along with a description of the psychometric properties of assessment tasks. They also discuss the relevance of EBP to speech therapy.

Little is documented about contemporary management of speech and swallowing disorders associated with paediatric acquired brain injury (ABI). It is therefore challenging for clinicians in this field to benchmark their clinical management against current evidence or practices undertaken in other centres. To address this issue, we aimed to provide much-needed baseline data on speech and language pathology management of speech and swallowing disorders associated with childhood ABI. Key objectives were to: (i) determine whether clinicians use formalized referral criteria, clinical guidelines, protocols or care pathways; and (ii) to document the specific assessment and treatment approaches used. Speech and language pathology managers and clinicians at 31 major paediatric rehabilitation centres across Australia, New Zealand, the UK and Ireland were invited to participate in an online survey. Fifty-one speech and language pathologists responded representing 26 centres (84% response rate). Routine referrals of ABI patients to speech and language pathology occurred relatively infrequently in these centres (12%). Centres utilized assessment protocols (23%) and guidelines (35%) more frequently than treatment guidelines (8%). Multidisciplinary care pathways were applied by 31%. Most centres used adult-based motor speech assessments and informal ('in-house developed') swallowing assessment tools. The limited use of referral criteria, protocols, care pathways and guidelines invites the possibility of unequal care, and less than optimal outcomes. Reliance on adult-based or in-house assessments is inappropriate, yet frequently a necessity due to an absence of paediatric-specific tools in this field. Further research is required in parallel with the formation of consensus groups to support the development of: (i) paediatric-specific assessment tools and management approaches; and (ii) clinical protocols and guidelines. Purpose: to compare subjects with cleft palate who underwent surgical correction at different ages and determine which factors are most important in the speech alterations Methods: the sample is composed of 29 subjects who were born with cleft plate and were submitted to surgical correction at different ages, without no other associated commitments(neurological, physical, psychological, et.). These patients, 13 of masculine gender, and 16 of the feminine gender have ages varying from six to thirteen year old. The patients are of the clinic of Speech Therapy and the clinic of Plastic Surgery of the São Paulo Hospital that had still not entered in speech therapy. The speech, these patients were evaluated by test Phonology (Chapter 1 of ABFW, Test Language Playground) and were examined by nominating the speech of the subjects. The appointment was mad through simple figures, which were shown to them. Results: the results of this study regarding gender and type of cleft in the sample showed no statistically significant differences. Meanwhile, the late time when the restorative surgery occurred interferes in a negative way in the speech of these patients. Conclusions: based on the results obtained from the analysis of speech in subjects with cleft palate, we can conclude that one intervening factor important for their speech is the age in which the surgery had been carried through.
