Prior research has established associations between neighbourhood poverty and cumulative biological risk (CBR). CBR is conceptualized as indicative of the effects of stress on biological functioning, and is linked with increased morbidity and mortality. Studies suggest that supportive social relationships may be health protective, and may erode under conditions of poverty. This study examines whether social relationships are inversely associated with CBR and whether associations between neighbourhood poverty and CBR are mediated through social relationships. Data were from a stratified probability sample community survey (n=919) of residents of Detroit, Michigan, USA (2002-2003) and from the 2000 US Census. The outcome variable, CBR, included anthropometric and clinical measures. Independent variables included four indicators of social relationships: social support, neighbourhood satisfaction, social cohesion and neighbourhood participation. Multilevel models were used to test both research questions, with neighbourhood poverty and social relationships included at the block group level, and social relationships also included at the individual level, to disentangle individual from neighbourhood effects. Findings suggest some associations between social relationships and CBR after accounting for neighbourhood poverty and individual characteristics. In models that accounted for all indicators of social relationships, individual-level social support was associated with greater CBR (β=0.12, p=0.04), while neighbourhood-level social support was marginally significantly protective of CBR (within-neighbourhood: β=-0.36, p=0.06; between-neighbourhood: β=-0.24, p=0.06). In contrast, individual-level neighbourhood satisfaction was protective of CBR (β=-0.10, p=0.02), with no within-neighbourhood (β=0.06, p=0.54) or between-neighbourhood association (β=-0.04, p=0.38). Results indicate no significant association between either social cohesion or neighbourhood participation and CBR. Associations between neighbourhood poverty and CBR were not mediated by social relationships. These findings suggest that neighbourhood-level social support and individual-level neighbourhood satisfaction may be health protective and that neighbourhood poverty, social support and neighbourhood satisfaction are associated with CBR through independent pathways.

Patterns of mental health are clearly associated with life circumstances, including educational and economic opportunities, access to safe and supportive neighborhoods, socially structured exposures to stressors and to supportive relationships. In this article, we examine the social and economic correlates of depressive symptoms among African American women residing within a predominantly African American urban neighborhood in Detroit, USA, with relatively few economic resources. We identify distinct stressors associated with financial strain, neighborhood social disorder (concern about police responsiveness, safety stress), and experiences of discrimination. We test the extent to which each of these stressors mediates relationships between household income, length of residence in the neighborhood, social support and depressive symptoms. Our results suggest that for women in this racially segregated area with a high concentration of poverty, relationships between household income and symptoms of depression are partially mediated by financial stress and social support, but that stressors associated with neighborhood disorder and discrimination influence depressive symptoms independent of household income. Furthermore, we find that length of residence in the neighborhood is negatively associated with financial stress and positively associated with police stress and social support, with no significant net effect on symptoms of depression. We conclude that higher household income may help reduce symptoms of depression by reducing financial stress and strengthening social support even within neighborhoods with high concentrations of poverty. However, increased household income does not protect African American women residing in a high poverty community from distress associated with neighborhood disorder or experiences of discrimination.

A conceptual model of the stress process has been useful in examining relationships between numerous social determinants (e.g., chronic stress), protective factors (e.g., social support), and health status. In this article, the authors examine multiple sources of chronic stress, instrumental and emotional support, and health outcomes among a random sample (N = 679) of predominantly low-income African American women who reside on Detroit's east side. The findings suggest that a number of chronic stressors have an impact on depressive symptoms and general health and that even though instrumental and emotional support each have a significant effect over and above the effects of the stressors, when both are included in the model, instrumental support, and not emotional support, remains as a significant predictor of health outcomes. These findings suggest the need for health education interventions and policy strategies that strengthen social support and aim at macro-level changes necessary to reduce chronic stressful conditions.

Individuals with chronic renal disease who receive dialysis treatment are continually faced with major adjustments. These may include dealing with changes in work and economic status, social roles, activity levels, self-image, health status, and normal routines, as well as learning to live with uncertainty and loss. The individual's social network plays a key role as the individual experiences and moves through various stages of adjustment. Networks with certain characteristics (e.g. provision of affective support, reciprocal ties) may be more effective than others lacking these characteristics in meeting the individual's changing needs during the process of adjusting to chronic renal disease. This paper examines this relationship between the characteristics of an individual's social network and adjustment to chronic renal illness. The discussion focuses on the impact of chronic renal disease on the individual, the composition and characteristics of the social network, and on the relationships between network members. How the social network affects a person's adjustment to stages of adaptation to chronic renal disease is also addressed. Finally, suggestions are presented for how health care professionals can intervene at the individual, network, and organizational level to strengthen and enlarge social networks in order to enhance social support.

Social support and social networks are among the psychosocial influences of physical and mental wellbeing. This literature review attempts to link the theory, research, and practice relevant to social networks and their association with health status. It is necessary that we extend our intervention strategies to build upon and collaborate with the individual and collective expertise of social networks.

The convincing evidence of the relationship between social support, social networks, and health status has influenced the development of program strategies which are relevant to health education. This article focuses on the linkage between social support and social networks and health education programs which involve interventions at the network and community level. Two broad strategies are addressed: programs enhancing entire networks through natural helpers; and programs strengthening overlapping networks/communities through key opinion and informal leaders who are engaged in the process of community wide problem-solving. Following a brief overview of definitions, this article highlights several network characteristics which are often found to be related to physical and mental health status. Suggestions are made for how these network characteristics can be applied to the two program strategies. Principles of practice for the health educator, and some of the limitations of a social network approach are delineated. The article concludes with a recommendation for engaging in action research--a perspective highly consistent with both the strategies discussed and the concepts of social networks and social support. This approach not only recognizes, but also acts to strengthen indigenous skills and resources.
