Abstract

The aim of this study was to investigate the effect of enhancing environmental factors on the duration and quality of sleep among patients in a cardiovascular surgery intensive care unit (CSICU). This quasi-experimental study was conducted upon 100 patients. The experimental group received nursing interventions to control the environmental factors that negatively affect nocturnal sleep. Data also were collected using a form to assess the environmental factors and the Richards–Campbell Sleep Questionnaire in the morning of the second day after the operation. The difference between the groups was statistically significant in favor of the experimental group for eight of the environmental factors. Compared with that in the control group, the duration of sleep was longer (experimental, 6.30 h; control, 3.98 h) and the quality of sleep was higher (experimental, 66.57 units; control, 44.43 units) in the experimental group. We conclude that nursing interventions should be used to control the environmental factors that negatively affect nocturnal sleep in CSICUs. [ABSTRACT FROM PUBLISHER]

Abstract

Objective: To evaluate the reliability and validity of Richard-Campbell Sleep Questionnaire Turkish version. Materials and Methods: The scale was administered in 50 patients to evaluate the sleep quality and in 50 patients to evaluate the testretest reliability. Percentage distribution and average evaluation of the data, factor analysis, Pearson's correlation test, Cronbach's α reliability coefficient were used for analysis. Results: The majority of study participants were male (70.0%), married (96.0%), literate-primary school graduates (68.0%) and the mean age was 58.26±11.36 years. Factor analysis of the scale was the only factor, item-total correlation scores were determined between 0.48 and 0.96 points. The Cronbach alpha reliability coefficient was 0.91. Test-retest analysis of the scale derived from preliminary and final application there was no difference between the mean scores. Conclusion: The psychometric properties of the scale was found as good and acceptable. [ABSTRACT FROM AUTHOR]

Abstract

Aim: This study aimed to determine the effects of an early mobilization protocol performed in patients who underwent cardiac surgery on post‐operative outcomes. Background: Post‐operative complications are common in patients undergoing cardiac surgery. Early mobilization is recommended for patients who undergo cardiac surgery to prevent complications and achieve successful outcomes in post‐operative care. Design The study design was quasi‐experimental with a control group. Methods: Participants were patients who underwent cardiac surgery between January and October 2015. The study included 102 patients (51 patients each in the experimental and control groups). The introductory characteristics form, the Richards‐Campbell Sleep Questionnaire (RCSQ), duration of hospital stay (post‐operatively), and development of a post‐operative late complications form were used to collect data. Results: The study results revealed that patients in the experimental group had better improvement in RCSQ scores, shorter duration of hospitalization, and fewer late complications after surgery than patients in the control group. Conclusion: Early mobilization is feasible in adult cardiac surgery patients and has significant benefits. More research is recommended into the effectiveness of early mobilization in different patient groups. SUMMARY STATEMENT: What is already known about this topic? Cardiac surgery stands out from other forms of treatment due to the advances in techniques and materials that have resulted in safer procedures and lower perioperative risks. Despite these advances, post‐operative complications are frequent and are a determinant of length of hospital stay and functional recovery.Early mobilization is one of the evidence‐based, effective nursing interventions that improve patient care outcomes and prevent immobilization‐related complications.However, standard guidelines, evidence‐based practices, and early mobilization protocols are underused by nurses during post‐operative care. Bazı Çevresel Faktörleri Kontrol Altına Almanın Kalp Damar Cerrahisi Yoğun Bakım Hastalarının Gece Uyku Kalitesine EtkisiBu çalışmanın amacı, Kalp Damar Cerrahi Yoğun Bakım Ünitesinde ışık, sıcaklık, koku, gürültü, yatak ve yastıktan kaynaklanan rahatsızlık gibi bazı çevresel faktörleri kontrol altına almanın hastaların gece uyku durumlarına etkisini incelemekti.Araştırma, Mart 2009- Ekim 2010 tarihleri arasında yapıldı. Atatürk Üniversitesi Aziziye Araştırma Hastanesi Kalp Damar Cerrahisi Yoğun Bakım Ünitesinde yürütüldü.Araştırma iki aşamada gerçekleştirildi. Araştırmanın birinci aşamasında metodolojik araştırma modeli, ikinci aşamasında kontrol gruplu yarı deneme modeli kullanıldı. Birinci aşamada, Richard?Campbell Uyku Ölçeği'nin (RCUÖ) Türkçe geçerlik ve güvenirliliği; ikinci aşamada, bazı çevresel faktörleri kontrol altına almanın hastaların gece uyku durumlarına etkisi değerlendirildi.Araştırmanın örneklemini, araştırmaya katılma ölçütlerine uyan 100 hasta oluşturdu (50 deney, 50 kontrol). Veri toplamada anket formu, hastaların uyku durumlarını olumsuz etkileyen durumlar formu ve RCUÖ kullanıldı.Birinci aşamanın verilerinin değerlendirilmesinde, ölçeğin geçerlilik - güvenirliliği için faktör analizi; madde toplam korelasyonu için Pearson korelasyon tekniği ve iç tutarlılığı için Cronbach's ? güvenirlik katsayısı analizi kullanıldı. İkinci aşamanın verilerinin değerlendirilmesinde, sayımla belirlenen verilerde yüzdelik ve X2 testi; ölçümle belirlenen verilerde bağımsız gruplarda t testi, varyans analizi, Kruskal Wallis varyans analizi ve Mann-Whitney U testi kullanıldı.RCUÖ'nün puan ortalamaları deney grubunda 66.57 (SS= 7.12), kontrol grubunda 44.43 (SS= 12.86) idi ve grupların puan ortalamaları arasındaki fark istatistiksel olarak anlamlıydı (p= .000). Grupların gece uyku saati ortalaması deney grubundaki hastalarda, kontrol grubundaki hastalara oranla istatistiksel yönden fark oluşturacak şekilde anlamlı bulundu (Deney uyku saati ortalaması= 6.30; Kontrol uyku saati ortalaması= 3.98), (p= .000). Grupların RCUÖ puan ortalamaları deney grubunda 66.57, kontrol grubunda 44.43 olarak saptandı ve gruplar arasında istatistiksel olarak anlamı bir fark vardı (p= .000). Yatağın rahatsız edici, yastığın rahatsız edici, ortamın; havasız, fazla ışıklı, fazla gürültülü, çalışan ziyaretçilerinin fazla olması, YBÜ'de çalışan sohbetleri, sık sık yapılan telefon görüşmeleri ve hemşirenin bakım uygulaması faktörlerinde istatistiksel olarak anlamlı bir fark saptandı (p0.05).Sonuç olarak, ışık, sıcaklık, koku, gürültü, yatak ve yastıktan kaynaklanan rahatsızlık gibi bazı çevresel faktörleri kontrol altına almak, hastaların gece uyku durumlarını olumlu yönde etkiledi. The aim of this study was to investigate the effect of getting some environmental factors (discomfort stem from illumination, heat, odor, noise, bed and pillow etc) under control on night?s sleep state of patients in Cardiovascular Surgery Intensive Care Unit.The study was conducted between March 2009 and October 2010 in Ataturk University Aziziye Research Hospital Cardiovascular Surgery Intensive Care Unit.This study was performed in two stages; a methodological model in the first stage and a quasi-experimental model with a control group in the second stage were used. Turkish validity and reliability of the Richard?Campbell Sleep Questionnaire ?RCSQ were assessed in the first stage, whereas the effect of getting some environmental factors under control on patients? night?s sleep states was evaluated in the second stage.The sample of the study was composed of 100 patients (50 experimental, 50 control) who complied with the criteria of inclusion for the study. A questionnaire form, a form of conditions affecting the patients? sleeping states negatively and the Richard?Campbell Sleep Questionnaire were utilized as data collection tools.In evaluation of the first stage?s data, the factor analysis was utilized for the validity-reliability of the questionnaire, whereas the Pearson correlation and the Cronbach?s ? reliability coefficient analysis were used for item-total correlation and internal consistency, respectively. In evaluating the data of the second stage, percentage and X2 test were applied for the data determined with counting; on the other hand, for the data determined with measurements t-test, variance analysis, the Kruskal Wallis variance analysis and the Mann-Whitney U test were carried out in independent groups.The score averages of the RCSQ were 66.57 (SS= 7.12) in the experimental group, and 44.43 (SS= 12.86) in the control group, and the difference between the score averages of the groups was statistically significant (p= .000). The average night?s sleep hour of the patients was found to be statistically significant among the patients in the experimental group than among the patients in the control group (the average of sleep hour in the experiment =6.30 ; the average of sleep hour in the control group=3.98, (p=.000). The RCUQ score averages of the groups were determined to be 66.57 in the experimental group and 44.43 in the control group and there was a statistically significant difference between the groups (p=.000). A statistically significant difference was determined in the factors such as having an uncomfortable bed and pillow, a badly ventilated, over illuminated, and noisier environment, the staff?s having too many visitors, and the ICU staff?s having conversations, frequent phone calls and the nurse?s applying care (p0.05).As a conclusion, getting some environmental factors (discomfort stem from illumination, heat, odor, noise, bed and pillow etc) under control had positive effect on night?s sleep states of the patients. In accordance with this result, it is possible to suggest that nursing interventions should be applied in order to get environmental factors, which affect the sleep state negatively, under control in the ICUs and that related studies should be continued.Key words: Intensive care unit, Sleep, Environmental factors, Nurse 98
