The article contains a literature review of clinical cases describing sessile serrated adenomas in the appendix from 1998 to 2018. Currently, it is still diffi cult to assess the true incidence of this pathology due to the fact that in most cases serrated adenomas are accidental fi ndings diagnosed after removing an inflamed appendix, since often, at small sizes, they do not cause clinical symptoms, do not bother patients and are inaccessible for endoscopic examination (if located behind its orifi ce). In addition, we present 2 clinical cases of sessile serrated adenoma of the appendix in patients who were admitted to the City Clinical Hospital No. 31 of the Moscow City Health Department in 2018 due to acute appendicitis, with a clinical and morphological characteristic, including a molecular genetics study.

In the course of the serrated pathway of carcinogenesis, there are changes in the expression of mucins with a characteristic immunophenotypic sign, such as a late loss of intestinal differentiation and an increase in gastric differentiation.To comparatively assess the expression of Muc 2, Muc 5AC, and Muc 6 in hyperplastic polyps (HPs), sessile serrated adenomas (SSAs) and traditional serrated adenomas (TSAs) of the colon for determination of their role in differential diagnosis.Sixty-five serrated masses from 52 patients were examined. Among them, there were 26 SSAs, 26 HPs, and 13 TSAs. A histological examination was done using hematoxylin and eosin staining; periodic acid-Schiff reaction in combination with alcian blue, as well as immunohistochemistry with anti-Muc 2, anti-Muc 5AC, and anti-Muc 6 antibodies were used. Genetic testing of the specimens for KRAS and BRAF mutations was also carried out.All the serrated neoplasms of the colon exhibited a pronounced expression of Muc 2. A marked Muc 6 expression in the dilated crypt bases was found in 76.9% of SSAs, while no reaction was seen in 92.3% of HPs and in 100% of TSAs. SSAs were characterized by an intense Muc 5AC expression in the whole length of the crypts and in the surface epithelium in contrast with HPs and TSAs, where the expression of the marker was focal. Comparison of the response of the markers and the presence of gene mutations identified that the SSAs with BRAF mutation intensely expressed along the length of the crypt for Muc 5AC and Muc 6; and the TSAs with KRAS mutation had a moderate focal Muc 5AC expression in the crypt bases in 100% of cases.For differential diagnosis of the types of serrated adenomas of the colon, it is useful for a pathologist to apply the immunohistochemical markers Muc 2, Muc 5AC, and Muc 6 in his/her practice.В ходе зубчатого пути канцерогенеза происходят изменения экспрессии муцинов с характерным иммунофенотипическим признаком поздней потери кишечной и усиления желудочной дифференцировки. Цель исследования - провести сравнительную оценку экспрессии Muc 2, Muc 5AC и Muc 6 в гиперпластических полипах (ГП), сидячих и традиционных зубчатых аденомах (соответственно СЗА и ТЗА) толстой кишки для определения их роли в дифференциальной диагностике. Материал и методы. Исследовано 65 зубчатых образований от 52 пациентов. Среди них было 26 СЗА, 26 ГП, 13 ТЗА. Проведено гистологическое исследование с окраской гематоксилином и эозином, использована ШИК-реакция в сочетании с альциановым синим, иммуногистохимическое исследование с антителами к Muc 2, Muc 5AC и Muc 6. Также выполнено генетическое исследование образцов на наличие мутаций гена KRAS и BRAF. Результаты. Выраженная экспрессия Muc 2 оказалась характерна для всех зубчатых новообразований толстой кишки. В 76,9% СЗА выявлена выраженная экспрессия Muc 6 в дилатированных основаниях крипт, в то время как в 92,3% ГП и в 100% случаев ТЗА реакция отсутствовала. Для СЗА оказалась характерна выраженная экспрессия Muc 5AC вдоль всей длины крипты и в поверхностном эпителии в отличие от ГП и ТЗА, где экспрессия маркера была очаговой. При сравнении реакции маркеров и наличия мутаций в гене определено, что СЗА с мутацией гена BRAF интенсивно экспрессировали вдоль длины крипты Muc 5AC и Muc 6, а ТЗА с мутацией гена KRAS в 100% случаев имели умеренную очаговую экспрессию Muc 5AC в основаниях крипт. Заключение. С целью дифференциальной диагностики типов зубчатых аденом толстой кишки полезно применять иммуногистохимические маркеры Muc 2, Muc 5AC и Muc 6 в практике врача-патологоанатома.
