Background The symmetrical Lyon brace is a brace, usually used to maintain correction after a plaster cast reduction in the Cotrel's EDF (Elongation-Derotation-Flexion) frame. The new Lyon brace or ARTbrace is an immediate corrective brace based on some of the principles of the plaster cast which are improved due to advances in CAD/CAM technology. The aim of this paper is to describe concepts of this new brace to be not only a replacement of the plaster cast, but also a definitive brace. Methods Instead of a plaster cast, three segmental CAD/CAM moulds are made with the instantaneous full 3D raster stereography digitizer (Orten): 1. In self axial elongation 2. In shift and lumbar lordosis 3. In shift and thoracic kyphosis A specific software (OrtenShape) makes up the overlay of the three moulds. Mould 1 is used for the pelvis and the shoulders mould 2 for the lumbar segment and mould 3 for the thoracic segment. The mathematical basis of the ARTbrace is the torso column which is a circled helicoid with horizontal circle generator. A torso column is reproduced in the opposite direction of the scoliosis. Like the plaster cast, the ARTbrace is worn for a "total time" of 24 hours 7 days a week without modifying the standard protocol of the Lyon brace reduction. The prospective controlled cohort observational study of the 225 first patients treated since May 2013 is reported below. Results The in-brace immediate reduction is: 0.7, i.e. 40% better with the ARTbrace than with a plaster cast. The correction of flat back is 9° (from 18°.4 to 28°.5 kyphosis Cobb angle). The improved aesthetic appearance is equal for rib hump and ATR. Conclusion This first paper is an introduction with very short results and does not prejudge the final outcome. The ARTbrace can be used not only to replace the plaster cast, but also as a definitive brace. The new segmental moulding with final detorsion is even more efficient and to this day the ARTbrace is the most effective to reduce the Cobb angle of scoliosis. [ABSTRACT FROM AUTHOR]

Purpose: To report an original technique for the surgical correction of adolescent idiopathic scoliosis (AIS) based on hybrid construct using polyester bands placed under the thoracic transverse processes. Methods: We reviewed 105 patients operated between 2013 to 2017 for AIS with a minimum of 2 years follow-up. Clinical and radiological data, including 2D measurements of scoliosis parameters preoperatively, and at 3- and 24-months postoperatively were analyzed. Radiological analysis was performed by two independent observers using KEOPS (Smaio, Lyon-France, CE 2014) database. Results: Mean age was 14.6 ± 2 years. Mean follow-up was 23.7 ± 1.2 months. Most curves were classified as Lenke-1 (78%). The average number of vertebra involved in the construct was 10.9 ± 1.3 (range 7–13). A mean of 12.8 implants were used per construct (mean implant density 0.59). The number of subtransverse bands placed averaged 3 (range 1–5 bands). The mean frontal Cobb angle decreased from 57.9 to 21.2 ° (p < 0.05) postoperatively. The average T4-T12 kyphosis increased from 20.2°to 32.4° (p < 0.05) postoperatively and no loss of correction was observed at 2 years follow-up. We encountered one case of transverse process fracture intraoperativley. No neurological complications were observed. In the postoperative period, 4 cases were diagnosed with distal junctional failure, three of which required an extension of the construct, while the fourth case was treated by bracing. Conclusions: The use of subtransverse band is an efficient and safe method for the surgical correction of AIS. This technique incorporates technical ease (minimal risk of neurological injury), provides good frontal correction and restores thoracic kyphosis. Results are maintained over a two-year period. Purpose: Aim of the study was to verify the accuracy of rasterstereography (RST), as radiation-free alternative to plain radiography (RAD) in the monitoring of spine deformity and scoliosis progression in juvenile and adolescent subjects with idiopathic scoliosis.Methods: 192 subjects underwent RST (by Formetric 4D device) and low-dose RAD (EOS Imaging, France) in the same session. A sub-group of 30 subjects, selected for conservative treatment with corrective bracing, was assessed at 6-months follow-up. The Cobb angles (CA) obtained by the 3D spine reconstruction from RAD were compared with those provided by RST. Thoracic kyphosis (TK) and lumbar lordosis (LL) were compared as well.Results: RST provided lower CA compared to RAD (15° vs. 33°, mean values). The average difference in measuring CA was 18°, and the correlation coefficient was 0.55. Comparable TK was observed, whereas LL resulted underestimated by RST compared to RAD (34° vs. 43°, average values). The within-subjects correlation, measuring the accuracy of RST in monitoring the scoliosis progression, was 0.3. Accuracy of RST in identifying increased or decreased CA was 67%. Sensitivity and specificity were 64% and 69%.Conclusions: RST demonstrated moderate accuracy in measuring the scoliosis degree and low accuracy in monitoring the curve progression. Accordingly, it cannot be considered as a valid alternative to radiographic evaluation. However, since demonstrated capable of revealing the presence of spine deformity, it could be in principle considered for the early screening in large adolescent populations, but after accounting for a cost-benefit analysis with respect to other traditional approaches.
