Background Social cognitive impairment is a recognized feature of psychotic disorders. However, potential age-related differences in social cognitive impairment have rarely been studied. Study Design Data came from 905 individuals with a psychotic disorder, 966 unaffected siblings, and 544 never-psychotic controls aged 18–55 who participated in the Genetic Risk and Outcome of Psychosis (GROUP) study. Multilevel linear models were fitted to study group main effects and the interaction between group and age on emotion perception and processing (EPP; degraded facial affect recognition) and theory of mind (ToM; hinting task) performance. Age-related differences in the association between socio-demographic and clinical factors, and EPP and ToM were also explored. Study Results Across groups, EPP performance was associated with age (β = −0.02, z = −7.60, 95% CI: −0.02, −0.01, P <.001), with older participants performing worse than younger ones. A significant group-by-age interaction on ToM (X 2(2) = 13.15, P =.001) indicated that older patients performed better than younger ones, while no age-related difference in performance was apparent among siblings and controls. In patients, the association between negative symptoms and ToM was stronger for younger than older patients (z = 2.16, P =.03). Conclusions The findings point to different age-related performance patterns on tests of 2 key social cognitive domains. ToM performance was better in older individuals, although this effect was only observed for patients. EPP was less accurate in older compared with younger individuals. Cluster studies identified a subgroup of patients with psychosis whose premorbid adjustment deteriorates before the onset, which may reflect variation in genetic influence. However, other studies reported a complex relationship between distinctive patterns of cannabis use and cognitive and premorbid impairment that is worthy of consideration. We examined whether: (1) premorbid social functioning (PSF) and premorbid academic functioning (PAF) in childhood and adolescence and current intellectual quotient (IQ) define different clusters in 802 first-episode of psychosis (FEP) patients; resulting clusters vary in (2) polygenic risk scores (PRSs) for schizophrenia (SCZ_PRS), bipolar disorder (BD_PRS), major depression (MD_PRS), and IQ (IQ_PRS), and (3) patterns of cannabis use, compared to 1,263 population-based controls. Four transdiagnostic clusters emerged (BIC = 2268.5): (1) high-cognitive-functioning (n  = 205), with the highest IQ (Mean = 106.1, 95% CI: 104.3, 107.9) and PAF, but low PSF. (2) Low-cognitive-functioning (n  = 223), with the lowest IQ (Mean = 73.9, 95% CI: 72.2, 75.7) and PAF, but normal PSF. (3) Intermediate (n  = 224) (Mean_IQ = 80.8, 95% CI: 79.1, 82.5) with low-improving PAF and PSF. 4) Deteriorating (n  = 150) (Mean_IQ = 80.6, 95% CI: 78.5, 82.7), with normal-deteriorating PAF and PSF. The PRSs explained 7.9% of between-group membership. FEP had higher SCZ_PRS than controls [ F (4,1319) = 20.4, P  < .001]. Among the clusters, the deteriorating group had lower SCZ_PRS and was likelier to have used high-potency cannabis daily. Patients with FEP clustered according to their premorbid and cognitive abilities. Pronounced premorbid deterioration was not typical of most FEP, including those more strongly predisposed to schizophrenia, but appeared in a cluster with a history of high-potency cannabis use. Background and hypothesis Recovery from psychosis is a complex phenomenon determined by an array of variables mutually impacting each other in a manner that is not fully understood. The aim of this study is to perform an approximated replication of a previous network analysis study investigating how different clinical aspects—covering psychopathology, cognition, personal resources, functional capacity, and real-life functioning—are interrelated in the context of schizophrenia-spectrum disorders. Study design A sample of 843 subjects from a multisite cohort study, with the diagnosis of a schizophrenia-spectrum disorder, was used to estimate a network comprising 27 variables. The connectivity and relative importance of the variables was examined through network analysis. We used a quantitative and qualitative approach to infer replication quality. Study results Functional capacity and real-life functioning were central and bridged different domains of the network, in line with the replicated study. Neurocognition, interpersonal relationships, and avolition were also key elements of the network, in close relation to aspects of functioning. Despite significant methodological differences, the current study could substantially replicate previous findings. Conclusions Results solidify the network analysis approach in the context of mental disorders and further inform future studies about key variables in the context of recovery from psychotic disorders. Psychotic and autistic symptoms are related to social functioning in individuals with psychotic disorders (PD). The present study used a network approach to (1) evaluate the interactions between autistic symptoms, psychotic symptoms, and social functioning, and (2) investigate whether relations are similar in individuals with and without PD. We estimated an undirected network model in a sample of 504 PD, 572 familial risk for psychosis (FR), and 337 typical comparisons (TC), with a mean age of 34.9 years. Symptoms were assessed with the Autism Spectrum Quotient (AQ; 5 nodes) and the Community Assessment of Psychic Experiences (CAPE; 9 nodes). Social functioning was measured with the Social Functioning Scale (SFS; 7 nodes). We identified statistically significant differences between the FR and PD samples in global strength (P <.001) and network structure (P <.001). Our results show autistic symptoms (social interaction nodes) are negatively and more closely related to social functioning (withdrawal, interpersonal behavior) than psychotic symptoms. More and stronger connections between nodes were observed for the PD network than for FR and TC networks, while the latter 2 were similar in density (P =.11) and network structure (P =.19). The most central items in strength for PD were bizarre experiences , social skills , and paranoia. In conclusion, specific autistic symptoms are negatively associated with social functioning across the psychosis spectrum, but in the PD network symptoms may reinforce each other more easily. Understanding longitudinal cognitive performance in individuals at ultra-high risk for psychosis (UHR) is important for informing theoretical models and treatment. A vital step in this endeavor is to determine whether there are UHR subgroups that have similar patterns of cognitive change over time. The aims were to: i) identify latent class trajectories of cognitive performance over 12-months in UHR individuals, ii) identify baseline demographic and clinical predictors of the resulting classes, and iii) determine whether trajectory classes were associated with transition to psychosis or functional outcomes. Cognition was assessed using the Brief Assessment of Cognition in Schizophrenia (BACS) at baseline, 6- and 12-months (N = 288). Using Growth Mixture Modeling, a single unimpaired improving trajectory class was observed for motor function, speed of processing, verbal fluency, and BACS composite. A two-class solution was observed for executive function and working memory, showing one unimpaired and a second impaired class. A three-class solution was found for verbal learning and memory: unimpaired, mildly impaired, and initially extremely impaired, but improved ("caught up") to the level of the mildly impaired. IQ, omega-3 index, and premorbid adjustment were associated with class membership, whereas clinical variables (symptoms, substance use), including transition to psychosis, were not. Working memory and verbal learning and memory trajectory class membership was associated with functioning outcomes. These findings suggest there is no short-term progressive cognitive decline in help-seeking UHR individuals, including those who transition to psychosis. Duration of untreated psychosis (DUP) is associated with clinical outcomes in people with a diagnosis of first-episode psychosis (FEP), but factors associated with length of DUP are still poorly understood. Aiming to obtain insights into the possible biological impact on DUP, we report genetic analyses of a large multi-center phenotypically well-defined sample encompassing individuals with a diagnosis of FEP recruited from 6 countries spanning 17 research sites, as part of the European Network of National Schizophrenia Networks Studying Gene-Environment Interactions (EU-GEI) study. Genetic propensity was measured using polygenic scores for schizophrenia (SZ-PGS), bipolar disorder (BD-PGS), major depressive disorder (MDD-PGS), and intelligence (IQ-PGS), which were calculated based on the results from the most recent genome-wide association meta-analyses. Following imputation for missing data and log transformation of DUP to handle skewedness, the association between DUP and polygenic scores (PGS), adjusting for important confounders, was investigated with multivariable linear regression models. The sample comprised 619 individuals with a diagnosis of FEP disorders with a median age at first contact of 29.0 years (interquartile range [IQR] = 22.0–38.0). The median length of DUP in the sample was 10.1 weeks (IQR = 3.8–30.8). One SD increases in SZ-PGS, BD-PGS, MDD-PGS or IQ-PGS were not significantly associated with the length of DUP. Psychotic patients with a lifetime history of cannabis use generally show better cognitive functioning than other psychotic patients. Some authors suggest that cannabis-using patients may have been less cognitively impaired and less socially withdrawn in their premorbid life. Using a dataset comprising 948 patients with first-episode psychosis (FEP) and 1313 population controls across 6 countries, we examined the extent to which IQ and both early academic (Academic Factor [AF]) and social adjustment (Social Factor [SF]) are related to the lifetime frequency of cannabis use in both patients and controls. We expected a higher IQ and a better premorbid social adjustment in psychotic patients who had ever used cannabis compared to patients without any history of use. We did not expect such differences in controls. In both patients and controls, IQ was 3 points higher among occasional-users than in never-users (mean difference [Mdiff] = 2.9, 95% CI = [1.2, 4.7]). Both cases and control daily-users had lower AF compared to occasional (Mdiff = −0.3, 95% CI = [−0.5; −0.2]) and never-users (Mdiff = −0.4, 95% CI = [−0.6; −0.2]). Finally, patient occasional (Mdiff = 0.3, 95% CI = [0.1; 0.5]) and daily-users (Mdiff = 0.4, 95% CI = [0.2; 0.6]) had better SF than their never-using counterparts. This difference was not present in controls (F group*frequency(2, 2205) = 4.995, P =.007). Our findings suggest that the better premorbid social functioning of FEP with a history of cannabis use may have contributed to their likelihood to begin using cannabis, exposing them to its reported risk-increasing effects for Psychotic Disorders. Introduction Patients with schizophrenia have an elevated mortality risk compared to the general population, with cardiovascular-related deaths being the leading cause. The role of clozapine use in the long-term mortality risk is unclear. While clozapine treatment may increase the risk for cardiovascular mortality, it may have protective effects regarding suicidal behavior. Methods We systematically searched EMBASE, MEDLINE, and PsycINFO and reviewed studies that used a long-term follow-up (ie, >52 weeks) and reported on mortality in adults diagnosed with schizophrenia-spectrum disorders who had received clozapine treatment. Results Altogether, 24 studies reported on 1327 deaths from any causes during 217691 patient years in patients treated with clozapine. The unadjusted mortality rate in 22 unique samples during a follow-up of 1.1–12.5 (median = 5.4) years was 6.7 (95% confidence interval [CI] = 5.4–7.9) per 1000 patient years. Long-term, crude mortality rate ratios were not significantly lower in patients ever treated with clozapine during follow-up, but significantly lower in patients continuously treated with clozapine compared to patients with other antipsychotics (mortality rate ratio = 0.56, 95% CI = 0.36–0.85, P -value =.007). Few studies reported on rates of long-term cause-specific mortality (suicide and ischemic heart disease), which showed no significant difference in patients using clozapine compared to patients using other antipsychotics. Statistical heterogeneity was high in all analyses. Discussion Continuous clozapine treatment in schizophrenia patients was associated with a significantly lower long-term all-cause mortality rate compared to other antipsychotic use. These findings, combined with the known efficacy of clozapine, give reason to re-evaluate the hesitancy to prescribe clozapine in regular care settings. Trial registration PROSPERO CRD42017069390. Deficits in motivational behavior and psychotic symptoms often observed in schizophrenia (SZ) may be driven by dysfunctional reward processing (RP). RP can be divided in two different stages; reward anticipation and reward consumption. Aberrant processing during reward anticipation seems to be related to SZ. Studies in patients with SZ have found less activation in the ventral striatum (VS) during anticipation of reward, but these findings do not provide information on effect of the genetic load on reward processing. Therefore, this study investigated RP in healthy first-degree relatives of SZ patients. The sample consisted of 94 healthy siblings of SZ patients and 57 healthy controls. Participants completed a classic RP task, the Monetary Incentive Delay task, during functional magnetic resonance imaging (fMRI). As expected, there were no behavioral differences between groups. In contrast to our expectations, we found no differences in any of the anticipatory reward related brain areas (region of interest analyses). Whole-brain analyses did reveal group differences during both reward anticipation and reward consumption; during reward anticipation siblings showed less deactivation in the insula, posterior cingulate cortex (PCC) and medial frontal gyrus (MFG) than controls. During reward consumption siblings showed less deactivation in the PCC and the right MFG compared to controls and activation in contrast to deactivation in controls in the precuneus and the left MFG. Exclusively in siblings, MFG activity correlated positively with subclinical negative symptoms. These regions are typically associated with the default mode network (DMN), which normally shows decreases in activation during task-related cognitive processes. Thus, in contrast to prior literature in patients with SZ, the results do not point to altered brain activity in classical RP brain areas, such as the VS. However, the weaker deactivation found outside the reward-related network in siblings could indicate reduced task-related suppression (i.e. hyperactivation) of the DMN. The presence of DMN hyperactivation during reward anticipation and reward consumption might indicate that siblings of patients with SZ have a higher baseline level of DMN activation and possible abnormal network functioning. Background Traditionally, schizophrenia and major depressive disorder (MDD) are seen as separate disorders based on their distinct clinical presentation. In clinical practice however, overlapping symptoms are noticeable such as psychotic symptoms, apathy and cognitive decline. Schizophrenia and MDD are both associated with increased risks of immune disorders and metabolic syndrome. However, it is unclear whether dysfunctions in growth factors, the immune and metabolic system underlie the psychopathology of both schizophrenia and MDD without the influence of chronic illness and treatment. In this meta-analysis we aimed to answer the following question: which peripheral growth, immune and glucose metabolism proteins are similarly altered in drug-naïve first-episode patients with either schizophrenia or MDD compared with healthy controls? Methods We conducted a meta-analysis of studies providing serum or plasma measurements (in mean and standard deviation) of peripheral protein markers regarding growth, immune and glucose metabolism proteins in drug-naïve first-episode patients with schizophrenia or MDD and healthy controls. We used a random-effects meta-analysis for which mean differences were quantified with Hedges' g (g). Results A total of 65 case-control studies were retrieved comprising of 3156 drug-naïve and first-episode schizophrenia patients and 3687 controls, and a total of 29 studies were retrieved comprising of 1077 drug-naïve and first-episode MDD patients and 1405 controls. In schizophrenia patients brain-derived neurotrophic factor (BDNF) (g = -0.80, 95% CI -1.00 to -0.61; P < 0.01) and nerve growth factor (NGF) (g = -3.28, 95% CI -6.35 to -0.21; P = 0.04) were significantly decreased, and interleukin 6 (IL-6) (g = 1.03, 95% CI 0.61 to 1.46; P < 0.01), interleukin 8 (IL-8) (g = 0.65, 95% CI 0.11 to 1.18; P = 0.02), tumor necrosis factor alpha (TNFα) (g = 0.51, 95% CI 0.19 to 0.83; P < 0.01), fasting glucose concentration (g = 0.26, 95% CI 0.08 to 0.44; P < 0.01) and fasting insulin concentration (g = 0.40, 95% CI 0.13 to 0.68; P < 0.01) were all significantly elevated compared with controls. In MDD patients C-reactive protein (CRP) (g = 0.45, 95% CI 0.18 to 0.71; P < 0.01), interleukin 1 beta (IL-1β) (g = 1.52, 95% CI 0.38 to 2.66; P < 0.01), interleukin 2 (IL-2) (g = 4.41, 95% CI 0.13 to 8.69; P = 0.04), IL-6 (g = 0.62, 95% CI 0.17 to 1.06; P < 0.01) and TNFα (g = 1.21, 95% CI 0.57 to 1.85; P < 0.01) were all significantly elevated compared with controls. IL-8 was significantly decreased in MDD patients compared with controls (g = -0.84, 95% CI -1.48 to -0.20; P < 0.01). CRP, IFNγ, IL-1β, IL-2, IL-4, interleukin 10 (IL-10) and monocyte chemoattractant protein-1 (MCP-1) were not significantly altered in schizophrenia patients compared with controls. However, for IL-4 we observed a trend of increased levels in schizophrenia. BDNF, NGF, IFNγ, IL-10, fasting insulin concentration and MCP-1 were not significantly altered in MDD patients compared with controls. However, a similar trend of increased levels of CRP was seen for schizophrenia when compared to MDD, and decreased levels of BDNF and NGF and elevated levels of fasting glucose concentration were seen for MDD when compared to schizophrenia. Discussion This meta-analysis suggests that dysfunctions in growth, immune and glucose metabolism markers are already present in patients with schizophrenia and MDD from disease onset without the influence of psychotropic treatment. Strikingly, of the 14 proteins identified in this meta-analysis, 10 (71%) are changed in the same direction (trends included) or unchanged in both schizophrenia and MDD. Background The concept of a psychosis continuum implies that the same symptoms seen in patients with psychotic disorders can be measured in a healthy population. The assumption is that experiencing symptoms of psychosis such as delusions and hallucinations is not inevitably associated with the presence of the disorder. Surveys of unselected general population samples have consistently shown that a large number of individuals report experiences that strongly resemble the symptoms seen in psychotic patients. Only a fraction of these individuals met criteria for a clinical disorder such as schizophrenia. Alterations in the metabolic system in antipsychotic-naïve first-episode schizophrenia have been described, posing the question whether these metabolic markers are altered in persons with subclinical psychotic experiences as well. Methods We used cross-sectional data of 1890 participants of a prospective population-based cohort study, 'the Rotterdam Study (ERGO)'. The ages ranged from 45 to 54 years and the participants filled in a self-report questionnaire, 'The Community Assessment of Psychic Experience'. To assess subclinical psychotic symptoms, the frequency and distress ratings of the positive symptom dimension from this questionnaire were used. These items were offset against metabolic measured parameters. The metabolic parameters used were body-mass index (BMI; body weight in kg/m2), fasting glucose levels, low-density lipoproteins (LDL), high-density lipoproteins (HDL), triglycerides and cholesterol. Linear regression analyses were used with adjustment for age, gender, ethnicity, education level, employment status and marital status. The influences of psychopharmacology use, anxiety disorder, depressive symptoms and use of lipid reducing agents on the subclinical psychotic symptoms were analysed in sensitivity analyses. Results We observed a strong positive association between BMI and the frequency of subpsychotic symptoms, and also with the perceived distress of these subpsychotic symptoms. In participants with a BMI ≥ 30 we observed an even stronger association with the frequency score and distress rating of subpsychotic symptoms. In a restricted sample to non-lipid reducing agent users only, we observed that LDL is negatively associated with the frequency of subpsychotic symptoms and perceived distress of these subpsychotic symptoms. For HDL, we observed a negative association with only the frequency of subpsychotic symptoms. The other metabolic markers, total cholesterol, triglycerides and fasting glucose, did not show any significant associations with subpsychotic symptoms. The use of psychopharmaceuticals, the presence of an anxiety disorder or experiencing depressive symptoms did not alter our results. Discussion In this study of a large community sample we found that increased BMI and metabolic dysfunctions are associated with subclinical psychotic experiences. Our findings add to the evidence that psychotic phenomena are related to alterations in the metabolism and obesity even without antipsychotic medication use. Background: Patients are often treated with high doses or combinations of antipsychotics. High doses are associated with more severe side effects and reduction of motivation and drive, which may hamper recovery. Nevertheless, dose-reduction (DR) or discontinuation of antipsychotic medication in chronic patients, carries the risk of psychotic relapse. In order to identify risk factors of psychotic relapse after DR or discontinuation, we performed a meta-analysis, aimed (i) to determine the rate of relapse after DR or discontinuation in patients with chronic schizophrenia, and (ii) to assess risk factors for psychotic relapse. Methods: We searched PubMed, EMBASE and PsycINFO for studies on dose-reduction of antipsychotics from January 1950 through June 2019. We extracted data and calculated event rates (ER=relapse rate) per person-years including 95% confidence intervals (95%CI). The following data were extracted: (1) patient characteristics (age, percentage of male subjects, setting, duration of illness), (2) dose-reduction/discontinuation characteristics (start-dose before dose-reduction, end-dose after dose-reduction, dose-reduction in milligrams, dose-reduction as percentage of start-dose, time period of dose-reduction), (3) follow-up characteristics (time after dose-reduction), and (4) study characteristics (blinding, year of publication and relapse definition). To account for sample variation we pooled the results following the Dersimonian and Laird random effects method (CMA; Borenstein et al 2009). Between-study heterogeneity was assessed with Cochran’s I2-statistic. We examined the risk of bias in the included studies based on five aspects that could affect the association between exposure and outcome from the Newcastle-Ottawa scale (NOS). Results: 46 unique cohorts, presenting 1677 patients in which doses were reduced/discontinued were included in meta-analysis. Most included patients were man at middle age, and with a mean duration of illness of 15 years. There was a considerable risk of bias in studies (48% of studies with NOS≤3). We found an overall event rate (ER) per person-years on psychotic relapse of 0.55 (CI95% 0.46–0.65; p<0.0001; I2 =79). We present various variables that influence event rates. Highest rates were found for inpatients with a short duration of illness. Most robust event rates for psychotic relapse were seen for discontinuing antipsychotics, and if not discontinuing, dose-reduction till under 5mg haloperidol equivalents daily (HE). Abrupt reduction yielded higher rates than gradual reduction. During short follow-up time more relapses occurred than in studies with long follow-up time. Older studies and studies in which relapse was defined as a clinical decision (without applying a psychometric scale) also yielded high event rates, explained by the fact that older studies mostly reduced antipsychotics abruptly till zero or at least doses under 5mgHE, while more recent studies did not, and used rating scales for relapse. Discussion: In patients with chronic schizophrenia discontinuing, and to a lesser extent DR till end-dose <5mgHE, patients who reduce doses abrupt, inpatients, and patients with a short duration of illness carry highest relapse risk. Abstract: The catechol-O-methyltransferase (COMT) gene is a candidate gene for schizophrenia because of its role in the breakdown of dopamine in the prefrontal cortex. The COMT gene contains a functional polymorphism changing enzyme activity that has been associated with some neuropsychiatric (endo)phenotypes, e.g. cognitive performance and anxiety. In this study we investigated the association between the COMT Val158Met polymorphism and obsessive–compulsive symptoms in patients with schizophrenia. Severity of obsessive–compulsive symptoms in 77 male patients with recent-onset schizophrenia was assessed using the Yale-Brown Obsessive–Compulsive Scale (Y-BOCS), and the COMT Val158Met polymorphism was genotyped for these patients. We found a significant effect of the COMT genotype on Y-BOCS scores: the Val/Val genotype was associated with the highest Y-BOCS scores, whereas patients with the Met/Met genotype had the lowest Y-BOCS scores. Our data suggest that the COMT high-activity Val allele is associated with more obsessive–compulsive symptoms in young patients with schizophrenia. These results support the hypothesis that the COMT Val158Met polymorphism may be a modifier gene for the symptomatology of schizophrenia. [Copyright &y& Elsevier]
