Aim: To investigate the effects of ultrasonic activation file type, lateral canal location and irrigant on the removal of a biofilm-mimicking hydrogel from a fabricated lateral canal. Additionally, the amount of cavitation and streaming was quantified for these parameters., Methodology: An intracanal sonochemical dosimetry method was used to quantify the cavitation generated by an IrriSafe 25 mm length, size 25 file inside a root canal model filled with filtered degassed/saturated water or three different concentrations of NaOCl. Removal of a hydrogel, demonstrated previously to be an appropriate biofilm mimic, was recorded to measure the lateral canal cleaning rate from two different instruments (IrriSafe 25 mm length, size 25 and K 21 mm length, size 15) activated with a P5 Suprasson (Satelec) at power P8.5 in degassed/saturated water or NaOCl. Removal rates were compared for significant differences using nonparametric Kruskal-Wallis and/or Mann-Whitney U-tests. Streaming was measured using high-speed particle imaging velocimetry at 250 kfps, analysing both the oscillatory and steady flow inside the lateral canals., Results: There was no significant difference in amount of cavitation between tap water and oversaturated water (P = 0.538), although more cavitation was observed than in degassed water. The highest cavitation signal was generated with NaOCl solutions (1.0%, 4.5%, 9.0%) (P < 0.007) and increased with concentration (P < 0.014). The IrriSafe file outperformed significantly the K-file in removing hydrogel (P < 0.05). Up to 64% of the total hydrogel volume was removed after 20 s. The IrriSafe file typically outperformed the K-file in generating streaming. The oscillatory velocities were higher inside the lateral canal 3 mm compared to 6 mm from WL and were higher for NaOCl than for saturated water, which in turn was higher than for degassed water., Conclusions: Measurements of cavitation and acoustic streaming have provided insight into their contribution to cleaning. Significant differences in cleaning, cavitation and streaming were found depending on the file type and size, lateral canal location and irrigant used. In general, the IrriSafe file outperformed the K-file, and NaOCl performed better than the other irrigants tested. Aim: To introduce and characterize a reproducible hydrogel as a suitable biofilm mimic in endodontic research. To monitor and visualize the removal of hydrogel from a simulated lateral canal and isthmus for the following: I) Ultrasonic-Activated Irrigation (UAI) with water, ii) UAI with NaOCl and iii) NaOCl without UAI., Methodology: A rheometer was used to characterize the viscoelastic properties and cohesive strength of the hydrogel for suitability as a biofilm mimic. The removal rate of the hydrogel from a simulated lateral canal or isthmus was measured by high-speed imaging operating at frame rates from 50 to 30,000 fps., Results: The hydrogel demonstrated viscoelastic behaviour with mechanical properties comparable to real biofilms. UAI enhanced the cleaning effect of NaOCl in isthmi (P < 0.001) and both NaOCl and water in lateral canals (P < 0.001). A greater depth of cleaning was achieved from an isthmus (P = 0.009) than from a lateral canal with UAI and also at a faster rate for the first 20 s. NaOCl without UAI resulted in a greater depth of hydrogel removal from a lateral canal than an isthmus (P < 0.001). The effect of UAI was reduced when stable bubbles were formed and trapped in the lateral canal. Different removal characteristics were observed in the isthmus and the lateral canal, with initial highly unstable behaviour followed by slower viscous removal inside the isthmus., Conclusions: The biofilm-mimicking hydrogel is reproducible, homogenous and can be easily applied and modified. Visualization of its removal from lateral canal anatomy provides insights into the cleaning mechanisms of UAI for a biofilm-like material. Initial results showed that UAI improves hydrogel removal from the accessory canal anatomy, but the creation of stable bubbles on the hydrogel-liquid interface may reduce the cleaning rate., (© 2014 International Endodontic Journal. During a root canal treatment, an antimicrobial fluid is injected into the root canal to eradicate all bacteria from the root canal system. Agitation of the fluid using an ultrasonically vibrating miniature file results in a significant improvement in the cleaning efficacy over conventional syringe irrigation. Numerical analysis of the oscillation characteristics of the file, modeled as a tapered, driven rod, shows a sinusoidal wave pattern with an increase in amplitude and decrease in wavelength toward the free end of the file. Measurements of the file oscillation with a scanning laser vibrometer show good agreement with the numerical simulation. The numerical model of endodontic file oscillation has the potential for predicting the oscillation pattern and fracture likeliness of various file types and the acoustic streaming they induce during passive ultrasonic irrigation.

Endosonic files vary in their oscillation when operated in air with the greatest displacement at the unconstrained tip. It was the aim of this study to investigate how files of different lengths and sizes varied in their ability to cut dentine. Single rooted teeth were sectioned to produce 1 mm dentine discs. These were positioned on a plastic block connected to a load transducer. Endosonic K-files (#15 to 25) were used to cut the disc for 10 s using operator assisted movement. The files contacted the disc at the following points along their length; apical (3 mm from tip), middle (8 mm from tip) and coronal (13 mm from tip) under applied loads of 30 and 50 g. The depth of cut produced by each file was measured using light microscopy. The #15 and #25 were similar in their pattern of dentine removal. The #20 file produced a large variation in its cutting ability and was also susceptible to fracture. The tip of the files was prone to constraint at higher loading with the thicker part of the file producing more cutting. It was concluded that the pattern of dentine removal of endosonic files was affected by the amount of load applied. The displacement amplitudes of endosonic files in air would not appear to be a good indicator of file cutting ability.

This study investigated the effect of air inlet ring opening on the oscillatory pattern of the MM 1500 sonic handpiece and files. The oscillatory pattern of the driver head and files in air and under load was determined microscopically. The results showed that a critical opening of the air inlet ring was necessary to elicit oscillation of the driver head and sonic file. The driver oscillated in an elliptical manner, the displacement of which varied with air inlet ring opening. When operated in air the file exhibited an elliptical movement which was influenced by different design, length and thickness. This action was constrained under load and replaced by a longitudinal action of the same dimension for all files which appeared to have an efficient cutting action. This work shows that air inlet ring opening increases oscillation of the driver head once a critical opening has been reached and that clinical loading does not constrain the cutting action of the file.

The aim of this study was to develop a model system to demonstrate the pattern of load and how much is applied. A lower molar was mounted on a beam and attached to a mandibular phantom head jaw. A strain gauge fitted to the beam allowed loading of the molar to be recorded, during root canal preparation. Ten operators were asked to instrument the distal root canal; after preparation the operators were shown a graphical representation of the loads applied. This formed the basis of discussion and instruction in filing technique. The operators then repeated the filing. The results showed that prior to instruction operators used a push-pull motion, applying a constant lateral force to the canal wall. After instruction and discussion of the graphical representation of the loads applied the stroke rates used were slower and loads applied were higher (two-sample t-test, n = 10, P < 0.01). The experiment was repeated 1 year later and this showed that the change in technique had been retained. In conclusion, this model system allows visual feedback to the operator and can be used as an aid to teaching filing technique.

This study investigated the effect of direction of file oscillation on the amount of debris and smear remaining within oval canals. Sixty-five lower premolars were allocated to six experimental groups (n = 10) and one control group (n = 5). Canals were prepared using Gates Glidden burs and hand instruments. Preparations were completed using either an ultrasonic size 15 K file or a sonic size 15 shaper file. Three preparation techniques were used: (a) file oscillation toward oval recesses within the canal, (b) file oscillation at right angles to the recesses, and (c) rotational movement of handpiece. Canals were examined under the scanning electron microscope and blindly scored for debris and smear layer. Log linear analysis revealed no significant difference (p > 0.05) between sonic or ultrasonic instrumentation as to debris or smear layer remaining. File oscillation directed toward oval recesses left the least amount of debris (p < 0.05). Smear layer remained unaffected by all techniques.

An in vitro model system was developed to investigate the stroke rate and loading during hand, sonic, or ultrasonic instrumentation. The model system consisted of a mandibular molar tooth attached to a strain gauge and interface to a computer. Ten clinicians instrumented the distal canal of lower molar teeth. Stroke rates used and loads applied were measured using different sized files with the three different preparation techniques. There was no significant difference in stroke rates between different file sizes (p > 0.05) within the same generator, however, stroke rate for hand was significantly slower than for sonic or ultrasonic (p < 0.01). Loading of the canal wall rose with increase in file size which was significant for hand (p < 0.01) and sonic (p < 0.05) but not ultrasonic (p > 0.05). There was a tendency for loading to be higher with hand instrumentation, this however was not significant (p > 0.05).

Endosonic files are prone to constraint; this reduces their efficiency, particularly in the apical third and in curved root canals. Precurving the file may reduce constraint, thereby improving the file's ability to debride. This investigation was undertaken to compare both straight and precurved endosonic files as to debris and smear layer removal. Thirty-six curved root canals were randomly allocated into six groups. Each group was prepared using a step-down technique, then instrumented with either a straight or precurved endosonic file of size 15, 20, or 25. The tooth roots were split and examined under the scanning electron microscope. Blind examinations were made for debris and smear layer removal and evaluation scores were analyzed using a log linear approach. The root canals instrumented with the precurved files had a significantly lower debris score than those prepared with straight files. Precurving did not affect smear layer removal. In conclusion, precurving of files decreased the amount of debris but did not affect smear layer removal.

This study compared the performance of precurved and straight endosonic files. Size 15, 20, and 25 endosonic K files were precurved to different degrees (20 to 90 degrees) and the resultant oscillatory pattern showed no significant difference compared with corresponding straight files. Endovue blocks were prepared with either a #15, 20, or 25 endosonic file which was either straight or precurved. Those blocks prepared with precurved files had a continuous taper while blocks prepared with straight files had constrictions along their lengths. Finally, a curved root of a human natural tooth had windows prepared along its length so that the oscillatory pattern of the file could be observed. The precurved file oscillated more freely than a straight file as observed by the presence of antinodes along the file and the accumulation of dentin chips within the canal. The results of this study suggest that it is advantageous to precurve endosonic files before using them in curved canals.

Ultrasound in the kHz frequency range is used widely in clinical dentistry. The most common uses are in the fields of periodontology and endodontics. The ultrasonic scaler works by the vibratory chipping action of the oscillating tip and is assisted by the presence of cavitational activity in the associated cooling water. When assessing clinical studies it is often difficult to interpret results from different workers due to the lack of standardization of the ultrasonic scaler. Operators should be aware of the oscillatory pattern of different instruments. Endosonics utilizes an ultrasonically oscillating endodontic file to clean and shape the root canal prior to obturation. The cleaning ability of such files is assisted by the occurrence of acoustic microstreaming forces. The endosonic file is prone to constraint when it contacts the canal wall which alters its oscillatory pattern. Clinical techniques should be modified to reduce this problem.

Acoustic microstreaming is known to be an important phenomenon associated with the use of clinically available endosonic systems. However, it has proved difficult to calibrate its pattern and effectiveness around the oscillating file. An in-vitro model system consisting of slow setting plaster was developed to demonstrate the activity of streaming around both ultrasonically and sonically activated files. For ultrasonic files, streaming occurred mainly in front of and behind the file, in contrast to the sonic file, where the plaster was disturbed evenly around it. With both types of file most activity occurred around the file tip, and became reduced towards the driver. Streaming patterns associated with the ultrasonic device were found to be dependent on the power setting of the instrument and whether the side of the file was constrained or lightly touched. The sonic device produced a large disturbance around the freely oscillating tip. Under load this streaming occurred along the whole length of the file, and was unaffected by constraint. This investigation demonstrated that streaming does occur with the instruments currently available, although it is dependent on their operating conditions. Clinical users of such equipment should be aware of the characteristic three-dimensional patterns of streaming that occur around these endosonic files, in order to maximize their efficiency.

The displacement amplitude of endosonic files was measured with the aid of light microscopy in order to determine the power output of different ultrasonic units. The results showed that there was a substantial difference in the displacement amplitudes when the same size of file was used in different ultrasonic units made by the same or different manufacturers. File fracture was found to be a problem when files were driven at high displacement amplitudes.

It has been recognised that ultrasonic files leave a diagonal pattern within the canal following endosonic instrumentation. There is confusion on the causative factor on the production of these marks. Furthermore there is little information about the marks produced by sonic files. An in vitro system consisting of highly polished dentin discs demonstrated that the ultrasonic K-file made a diagonal pattern that was an imprint of the cutting edges of the file. The sonic files produced different markings which were the result of their longitudinal and transverse motion. Root canals of extracted teeth were prepared with these instruments and similar grooves were observed. The ultrasonic K-file had diagonal grooves in the coronal and middle third of the root canal but longitudinal marks in the apical third. The rispisonic and shaper files produced debris even in the presence of NaOCl. This debris was formed into discrete parallel bands. Both longitudinal and transverse grooves were present and the canal had many other scratches similar to those seen with the dentin discs. There were differences between the marks produced by either ultrasonic or sonic endosonic instruments but it is unknown whether this influences the long term prognosis of the root canal therapy.

The step-back method of root canal preparation may not be suitable for endosonic files as they may be prevented from working efficiently within the apical region. This study determined the debris present in the apical portion of the canal after using three different endosonic instruments and compared the step-back versus the step-down technique; the latter involves initial preparation of the upper regions of the root canal which may reduce file constraint. Thirty-six teeth were prepared by either the step-back or step-down method using endosonic generators (magnetostrictive, piexoelectric, and sonic). Preparation times were standardized so that files were used for the same period. From scanning electron micrographs, debris removal was scored on a four-point scale by four clinicians. A log linear analysis showed that there was significantly less debris present with the step-down method using piezoelectric and sonic devices. More debris was present when files driven by a magnetostrictive instrument were used with the step-back method. This study showed that the step-down method may be useful when using certain types of endosonic generators.

When operated in air, the sonic endodontic instrument produced a movement at the driver head which resulted in a large elliptical oscillation of the file. This motion had similar properties to ultrasonically operated files in that there was a distinct variability in the magnitude of the vibratory motion between files of different design, length and thickness. When the sonic file was loaded, the elliptical motion was eliminated leaving a pure longitudinal file oscillation. This mode of vibration was particularly efficient as it was largely unaffected by loading, with the displacement amplitude being similar for different types of file used. The oscillatory pattern of the sonic file may offer a useful mode of mechanically assisted root canal preparation.

When operated in air, the transverse oscillation of the endosonic file exhibits a series of nodes and antinodes along its length with the greatest displacement amplitude occurring at the unconstrained tip. Endosonic files of varying design, length, and thickness exhibit differences in their oscillation. Preliminary work has revealed that the endosonic file is susceptible to constraint when working within the root canal and a model system was designed to investigate this problem. The constraining influence was found to be greatest when it was applied at those antinodes nearest the tip. This effect was further enhanced if the file was angled. Furthermore, constraint applied at the nodes also influenced the file oscillation. Such results may explain in situ observations on the occasional inefficiency of the system, especially when negotiating the apical third of a curved root canal. Clinical techniques utilizing an endosonic file may require reappraisal in order to minimize such constraint.
