Today's business systems are faced with an increased intensity of changes from the environment, growing complexity and increased competition, which affects the increase in the level of risk in all areas of management and execution of business processes. Therefore, Risk Management (RM) discipline and practice is increasingly applied in almost all types of business systems. RM is the systematic approach of identifying, estimating and reducing threats or uncertainties that may influence organizations. This article presents the proposed model for monitoring and managing business risks, including strategic and operational goals, organizational responsibility and management, business performance, internal audit and internal control, application of information systems and the success of business processes. The research was conducted on a sample of 387 respondents, employed in manufacturing and service companies. Introduction: Needlestick and sharps injuries (NSSIs) represent an existential occupational hazard risk to orthopaedic surgeons during their career due to the interaction with various devices, instruments and bone fragments. Consequently, NSSIs have the potential to transmit infections such as Hepatitis B (HBV), Hepatitis C (HCV) and Human Immunodeficiency Virus (HIV) leading to serious illness. The purpose of this cross-sectional study was to identify the clinical settings predisposing orthopaedic surgeons to NSSIs and assess their adherence to safety protocols in the Indian context. Materials and Methods: An online cross-sectional survey of 618 orthopaedic surgeons in India, stratified by experience into two groups: under five years and with 5 years or more was undertaken. The data were collected via an expert-validated online questionnaire to evaluate demographic distribution, injury characteristics, knowledge of safety protocols, and adherence to these protocols. Descriptive statistics summarized the data, Chi-square tests assessed variable associations, and odds ratios were computed for significant variables. Ethical integrity was maintained via electronic informed consent and for confidentiality assurances. Results: The study revealed that orthopaedic surgeons with less than 5 years of clinical experience had higher risks for NSSIs as compared to those with 5 or more years of clinical practice. Conversely, the latter group was more susceptible to bone spike injuries and viral positive needlestick incidents. The analysis shows that whilst the more experienced practitioners displayed greater proficiency in the application of universal precautions and NSSI prevention, they were also less likely to report injuries, often due to discomfiture. Risk profiles were consistent across different practice settings and affiliations, regardless of experience level. Conclusion: This cross-sectional study reveals less experienced orthopaedic surgeons face higher risks of NSSIs, possibly due to inadequate education or awareness. More experienced practitioners encounter distinct risks, likely owing to long-term exposure and traditional practices. There is an immediate need to raise awareness of the potential risks of NSSIs, enhanced education, appropriate training, collaboration with the hospital risk management team and developing a culture of transparent reporting to mitigate these risks. Purpose: This paper aims to present a comprehensive assessment of the literature about the research agenda for future studies on risk management in small and medium-sized businesses (SMEs) with a research agenda for bibliometric analysis. The author's goals are to point out inconsistencies and gaps in the literature and to suggest directions for future study. Design/methodology/approach: A total of 147 papers were analyzed in terms of bibliographic information, research design, and findings. These included publication by year, most cited documents, citation by year, publication by authors, publication by subject area, publication by country, publication by affiliation, funding sponsor, network representation of keyword co-occurrence, and cluster visualization of keyword co-occurrence. This study used bibliometric analysis methods, keyword searches, and suitability assessment for bibliometric analysis. Findings: The bibliometric research revealed a number of potential risk categories for small and medium enterprises (SMEs). In addition, an analysis of the risk-related elements aimed to avoid or reduce their adverse effects, the published study highlights how crucial a risk management procedure is for SMEs. Risk factors were also shown to be unavoidable; as a result, everyone needs to practice social responsibility in order to reduce the detrimental effects on the economy. Research limitations/implications: Future studies are required on risk identification, analysis, plan implementation, and control in the context of small and medium-sized enterprise risk management. Understanding the risks in the months following hospital discharge is crucial for healthcare professionals to ensure the need for assistance is met. However, this may be challenging in the case of patients living with a major neurocognitive disorder (PLMNCD). Thus, it is important to incorporate patients' and caregivers' experiences of the transition from hospital to home in the risk assessment. This multiple case study comprised 7 PLMNCD, their caregivers, and occupational therapists. Fifty-four interviews, conducted just before, as well as 3 weeks and 3 to 6 months after hospital discharge, were qualitatively analyzed. Results revealed that risk management during the hospital-to-home transition is a dynamic process aimed at establishing a satisfactory routine while avoiding adverse events. This risk management process, which identifies challenges over time and between stakeholders, involves (a) determining the seriousness and acceptability of risks, (b) reflecting on ways to manage risks, and (c) taking steps to manage risks. Identifying ways to ensure resident safety is increasingly becoming a priority in residential settings and nursing homes. The aim of this qualitative systematic review was to identify, describe, and assess research evidence on managers' perceptions regarding the barriers and facilitators of daily resident and patient safety work in residential settings and nursing homes. A qualitative systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta‐Analysis checklist. Published studies were sought through academic databases: Academic Search Premier, CINAHL, PubMed (MEDLINE), Scopus, SocINDEX, and Web of Science Core Collection in April 2023. Finally, 12 studies were included. The results of the included studies were synthesized using thematic synthesis after data extraction. According to the results, (1) competent staff and material resources; (2) management and culture; (3) communication, networks, optimal use of expertise; and (4) effective use of guidelines, rules, and regulations play a significant role in the success of resident and patient safety work. The findings revealed that promoting resident safety should not be seen solely as the responsibility of individual residential or nursing home personnel, as it requires multiprofessional cooperation and access to wider networks. Staff and managers must be receptive to learning, changing, and improving safety. Falls are among the top 10 causes of years lived with disability in people aged 75 and over. Preventive programs like case management (CM) are crucial. Objectives: To evaluate the effects of a multifactorial fall prevention program based on CM on physical performance, the presence of pain, and the risk of falls and fractures in older people who have suffered falls. Methods: This randomized, single-blind clinical trial with parallel groups, Intervention Group (IG) and Control Group (CG), was composed of 55 older people with a history of falling, living in the community. All participants underwent an initial assessment via video call (containing anamnesis, timed up-and-go test, falls risk score, short physical performance battery, and clinical frax). The IG underwent CM, the physical exercise protocol, and the cognitive stimulation protocol. The CG was monitored through telephone calls and received general health and fall guidance. Results: No significant results were found in the physical capacity, the presence of pain, the risk of falls, or the fractures between the Intervention and Control Groups and between assessments. Conclusion: This program was not effective in improving functional performance, but it was important for characterizing pain and the probability of fracture in the next 10 years in this population. Introduction: Digitalization in medicine is steadily increasing. Complex treatments, scarce personnel resources and a high level of documentation are a constant burden on healthcare systems. The balancing between correct manual documentation in the digital records and limited staff resources is rarely successful. The aim of this study is to evaluate the adherence between documentation and lack of documentation in the treatment of critical care patients. Material and Methods: For the evaluation, data from the hospital information system (HIS) of several intensive care units (ICU) were examined in conjunction with data collected from a checklist. All boluses of sedatives, analgesics and catecholamines were documented paper based across all shifts and all weekdays and compared with corresponding digital data from the HIS (2019–2022) of previous years. Results: 939 complete digital patient records revealed a massive under-documentation of the medication administration compared to that applied according to the checklist. Only 12% of all administered catecholamines, 11% of α2-agonists, 33% of propofol, 92% of midazolam and 46% of opioids were found in the digital recordings. In comparison, the effect was more pronounced on weekdays compared to weekends. In addition, the highest documentation gap was found in the comparison of early shifts. Comparing neurosurgical vs. internal vs. anesthesiologic ICUs there was a highly significant difference between anesthesiologic ICUs compared with other disciplines (p < 0.0001). Discussion: Our data shows that there is a remarkable documentation gap and incongruence in the area of applied boli. Automated documentation by connecting syringe pumps that enter data directly into the HIS can not only reduce the workload, but also lead to comprehensive and legally required documentation of all administered medication. Domestic violence (DV) impacts the safety and well-being of adult victims and their children who are exposed to the violence. Police play a critical role in providing intervention strategies to protect the welfare of victims of DV. Police training, policy and procedures on intervention strategies are often adult-centric leaving little direct police interaction with children. Objective: The objective of the current study is to identify the challenges police officers perceive in providing effective safety planning and risk management strategies for domestic violence victims and their children. Participants, Setting & Methods: Interviews with domestic violence police specialists (n = 12) in Ontario, Canada were used to gather information on police officers' perspective on the challenges they face in executing safety planning and risk management strategies. A dual deductive/inductive approach to a thematic analysis at the sematic level was used (Braun and Clarke, Qualitative Research in Psychology 3:77–101, 2006) to explore themes. Results: The major themes that emerged from the interviews were: (a) challenges with the broader economic/social and judicial system; (b) challenges created by organizational factors; and (c) individual challenges affiliated with police officers' perception of barriers they encountered. These challenges all impact the police response to providing intervention strategies to DV victims and their child(ren). Conclusions: This study highlights the need for police organizations to formalize training, policies, and directives specific to safety planning and risk management for adult victims of DV and their children. Introduction: Workplace violence is endemic, destructive, and escalating in frequency and severity in healthcare. There is a paucity of research on workplace violence in regional and remote hospital emergency departments (EDs). Objective: The aim of this study was to identify the perpetrator and situational characteristics associated with violent incidents in the ED across five regional and remote Australian sites. Method: This study audited hospital summary data, incident reports, and medical records for a 12-month period in 2018 to examine the perpetrator and situational characteristics of workplace violence incidents in five regional and remote Australian EDs. Results: Violent incidents were evenly spread throughout the week and across shifts. Most incidents were triaged as urgent, occurred within the first 4 hr, and had multidisciplinary involvement. Almost one in every six incidents resulted in an injury. Perpetrators of violence were predominantly young and middle-aged males and almost always patients, with most presenting with mental and behavioral disorders, or psychoactive substance use. Conclusions: Understanding the characteristics of perpetrators of violence can help in seeking to tailor interventions to reduce further violent behaviors. This study looks into the effectiveness of the authorisation procedure as a regulatory instrument within the framework of the European REACH regulation. It highlights its impact on enhancing occupational safety and health for both applicants and companies utilising the substances. This procedure encompasses manufacturers, importers, and downstream users of substances, as well as representatives of foreign manufacturers who are also eligible to seek authorisation. When applying for authorisation, the ECHA Risk Assessment Committee (RAC) assesses the risks associated with the intended uses of the substance, including the appropriateness and effectiveness of the Occupational Conditions (OCs) and Risk Management Measures (RMMs) described in the application and the risks posed by potential alternatives. If the RAC determines that the OCs/RMMs are inadequate for managing or controlling the risk, or if the measures to protect workers are deemed insufficient, it may recommend additional measures to enhance occupational safety and health or environmental protection. The 398 processed Applications for Authorisation (AfA) that have been submitted to date were examined to determine these recommended measures, categorised as Conditions for use, Monitoring arrangements, and Recommendations for Review Reports. Overall, a significant improvement concerning occupational safety and health seems necessary, as indicated by the large number of measures recommended by the ECHA Committee for Risk Assessment (RAC) and ECHA Committee for Socio-economic Analysis (SEAC) or supplemented by the European Commission. In addition to the proposed measures, a short assessment provided by the committees as to whether the operational conditions and risk management measures are adequate in controlling the risks is also included in the study. This study investigates the effectiveness of internal controls governing green credit operations in Vietnamese commercial banks. We employ regression analysis to identify key factors influencing control effectiveness. Six key areas emerge: Risk Management, Macroeconomic Factors, Monitoring Activities, Group Benefits, Work Motivation, and Information and Communication. Independent sample T-tests and one-way ANOVA tests reveal significant variations in control effectiveness based on employee age groups and positions within the banks. Further analysis categorizes these factors into strong and weak impact groups based on their Beta coefficients. Risk Management and Macroeconomic Factors emerge as strong influencers, highlighting the critical role of robust risk management practices and favorable economic conditions. Conversely, Monitoring Activities, Group Benefits, Information and Communication, and Work Motivation have weaker but still notable impacts, suggesting areas for improvement in internal supervisory practices, communication systems, and employee motivation strategies. The findings underscore the critical role of risk management and macroeconomic factors while highlighting opportunities for enhancing supervisory practices, internal communication, and employee motivation within Vietnamese commercial banks. Enterprise risk management (ERM) was introduced in the 1990s and has since become expected by boards of directors and regulators as a sign of good management and good corporate governance. However, many organizations struggle to implement ERM, and still seek practical advice on ERM implementation. This article explains many of the reasons why organizations are unsuccessful in their efforts at implementation and provides practical solutions provided by an experienced risk manager and consultant, an ex-Chief Risk Officer, and an academic, all of whom have written extensively on the subject. This article should be of interest to practitioners involved in implementing ERM, to consultants in ERM, and to academics teaching courses on ERM, risk management, and related topics. Nowadays, occupational risk management presents a significant challenge for enterprises and occupational health and safety (OHS) services. Economic changes drive efforts to verify working conditions and optimize production processes involving human participation. Employers are obligated to provide safe and hygienic working conditions using all available technical and organizational solutions. However, many enterprises still marginalize the correlation between reliable occupational risk assessments and employee absenteeism. Employers often fear high economic costs with potentially low returns. Such actions often stem from the lack of precise analyses clearly determining the costs of inadequate employee safety and ignorance of the possible consequences of workplace accidents. A well-prepared occupational risk assessment can beneficially impact employee safety, company profits, and the reduction or elimination of material and financial losses. This article discusses appropriate remedial measures for accurately determining occupational risk levels and the elements of its control. This research was done with the aim of designing a customer validation system model based on risk management with the approach of realizing fee income in the National Bank of Iran. The current research is applied in terms of purpose and in terms of method, it is a mixed (mixed)-exploratory design, which is based on grounded theory in the qualitative stage and descriptive-survey method in the quantitative stage. Based on the theme analysis method, after the interviews with the experts, which were conducted after the theoretical saturation of 15 interviews, all the text of the interviews was entered into the Max QDA software, and then the primary codes and sub- and main categories were extracted. Then, based on the Strauss and Corbin model, the central, causal, strategic, consequences, interventionists and contextual factors categories were identified. Finally, the research model was designed. In order to ensure the coordination of the data with the factor structure, and the quality indicators of the model, a questionnaire consisting of 95 items was distributed to a quantitatively wide population including 384 employees of National Bank. The data were analyzed using the structural equation modelling approach. The results of the qualitative part showed that the central phenomenon includes a customer validation system, causal conditions including risk management, background conditions including customer orientation, strategies and measures including banking facilities, and intervening conditions including cultural factors. The consequences include fee income and profitability. According to the results presented in the research model review, it can be stated that causal conditions (risk management) have a positive and significant effect on the central category of the customer validation system. The central category of the customer validation system is on strategies (banking facilities) which have a positive and significant effect. Background conditions (customer-oriented) have a positive and significant effect on strategies. Intervening conditions (cultural factors) have a positive and significant effect on strategies. Over the past decade, healthcare systems have started to establish control centres to manage patient flow, with a view to removing delays and increasing the quality of care. Such centres—here dubbed Healthcare Capacity Command/Coordination Centres (HCCCs)—are a challenge to design and operate. Broad-ranging surveys of HCCCs have been lacking, and design for their human users is only starting to be addressed. In this review we identified 73 papers describing different kinds of HCCCs, classifying them according to whether they describe virtual or physical control centres, the kinds of situations they handle, and the different levels of Rasmussen's [1] risk management framework that they integrate. Most of the papers (71%) describe physical HCCCs established as control centres, whereas 29% of the papers describe virtual HCCCs staffed by stakeholders in separate locations. Principal functions of the HCCCs described are categorised as business as usual (BAU) (48%), surge management (15%), emergency response (18%), and mass casualty management (19%). The organisation layers that the HCCCs incorporate are classified according to the risk management framework; HCCCs managing BAU involve lower levels of the framework, whereas HCCCs handling the more emergent functions involve all levels. Major challenges confronting HCCCs include the dissemination of information about healthcare system status, and the management of perspectives and goals from different parts of the healthcare system. HCCCs that take the form of physical control centres are just starting to be analysed using human factors principles that will make staff more effective and productive at managing patient flow. Background: The improvement of patient safety requires management of clinical risks in hospitals. However, implementation status of clinical risk management (CRM) varies substantially across hospitals. Investigations into facilitators and barriers for effective implementation are thus necessary. Using an in-depth interview study with CRM experts in hospitals, this study aimed to investigate factors accountable for effective development and implementation status of CRM measures in Germany. Methods: As part of a nationwide survey, we conducted semi-standardized interviews with n = 10 selected clinical risk managers from general hospitals. We used qualitative content analysis to identify barriers and facilitators to the implementation of CRM. Results: A broad range of determinants influencing CRM implementation were identified. Determinants were categorized into five thematic categories: general knowledge of CRM, safety culture, availability of resources, multi-level engagement and organizational structures. Discussion: While numerous barriers to CRM implementation were identified, interviewees reported only a few supporting factors in comparison. This study aims to provide empirical evidence regarding risk management disclosure through the perspective of shareholders, company size and board of commissioners. The measurement of the dependent variable, namely risk management disclosure in this study, is calculated using the total score of disclosure items based on the ISO:31000 dimension, including 5 dimensions, namely mandate and commitment, framework planning, risk management implementation, monitoring and continuous improvement according to ISO:31000 component standards. The sample used in this study is mining companies listed on the Indonesia Stock Exchange in 2019-2022. The sample was chosen using the purposive sampling technique, which took into account information availability and compliance with predetermined criteria. 21 companies were chosen after 84 observation data points were analyzed. The test on the hypothesis uses Multiple Linear Regression The study of this sample's data reveals that the degree of risk management disclosure is influenced by the factors of management ownership, firm size, and board of commissioners. This qualitative study explores the opportunities and challenges of collaboration experienced by social workers and police officers when dealing with cases of intimate partner violence (IPV) and stalking. The study aims to examine their collaborative approaches in risk assessment and risk management by identifying the structures, supports, and foundations crucial for effective collaboration. Our data, collected from twelve interviews and one focus group interview with social workers and police officers, reveals that collaboration was facilitated by assigning specific roles to involved parties, proximity, structure and professionalism. These key factors emerged as crucial and contributing to the effectiveness of the collaborative efforts. Practitioners should consider integrating these key elements into their practices to enhance and improve collaboration when addressing cases of IPV and stalking. The initial APEAS study, conducted in June 2007, examined adverse events (AEs) in Spanish Primary Healthcare (PHC). Since then, significant changes have occurred in healthcare systems. To evaluate these changes, a study was conducted in the Camp de Tarragona PHC region (CTPHC) in June 2019. This cross-sectional study aimed to identify AEs in 20 PHC centres in Camp de Tarragona. Data collection used an online questionnaire adapted from APEAS–2007, and a comparative statistical analysis between APEAS–2007 and CTPHC–2019 was performed. The results revealed an increase in nursing notifications and a decrease in notifications from family doctors. Furthermore, fewer AEs were reported overall, particularly in medication-related incidents and healthcare-associated infections, with an increase noted in no-harm incidents. However, AEs related to worsened clinical outcomes, communication issues, care management, and administrative errors increased. Concerning severity, there was a decrease in severe AEs, coupled with an increase in moderate AEs. Despite family doctors perceiving a reduction in medication-related incidents, the overall preventability of AEs remained unchanged. In conclusion, the reporting patterns, nature, and causal factors of AEs in Spanish PHC have evolved over time. While there has been a decrease in medication-related incidents and severe AEs, challenges persist in communication, care management, and clinical outcomes. Background: The Global Patient Safety Action Plan, an initiative of the World Health Organization (WHO), draws attention to patient safety as being an issue of utmost importance in healthcare. In response, the World Federation of Chiropractic (WFC) has established a Global Patient Safety Task Force to advance a patient safety culture across all facets of the chiropractic profession. This commentary aims to introduce principles and call upon the chiropractic profession to actively engage with the Global Patient Safety Action Plan beginning immediately and over the coming decade. Main text: This commentary addresses why the chiropractic profession should pay attention to the WHO Global Patient Safety Action Plan, and what actions the chiropractic profession should take to advance these objectives. Each strategic objective identified by WHO serves as a focal point for reflection and action. Objective 1 emphasizes the need to view each clinical interaction as a chance to improve patient safety through learning. Objective 2 urges the implementation of frameworks that dismantle systemic obstacles, minimizing human errors and strengthening patient safety procedures. Objective 3 supports the optimization of clinical process safety. Objective 4 recognizes the need for patient and family engagement. Objective 5 describes the need for integrated patient safety competencies in training programs. Objective 6 explains the need for foundational data infrastructure, ecosystem, and culture. Objective 7 emphasizes that patient safety is optimized when healthcare professionals cultivate synergy and partnerships. Conclusions: The WFC Global Patient Safety Task Force provides a structured framework for aligning essential considerations for patient safety in chiropractic care with WHO strategic objectives. Embracing the prescribed action steps offers a roadmap for the chiropractic profession to nurture an inclusive and dedicated culture, placing patient safety at its core. This commentary advocates for a concerted effort within the chiropractic community to commit to and implement these principles for the collective advancement of patient safety. Background: The risk management program will run well with commitment and good management. This study aims to describe the experience of nurse managers in implementing risk management in hospitals. Research methodology: this study used a descriptive qualitative approach. The data collection technique used Focus Group Interviews with 12 nurse managers consisting of 1 Head of Sub Installation, 7 Head nurses, and 4 Team Leaders. Interviews were carried out three times, and each implementation consisted of four participants. Analysis using thematic content and consolidation for reporting qualitative research (COREQ) was used as a guide to research reporting. The research found three inhibiting factors in implementing risk management: 1) Busyness, 2) Lack of knowledge, and 3) Lack of structured monitoring and evaluation. There is a ubiquitous problem with medical errors and the concomitant costs it brings in terms of human suffering and financial loss for patients, families, and caregivers. Professional caregivers, including physicians, nurses, and others who have made clinical errors normally will fall under the risk management and quality improvement policies of the organization at which they are employed and subsequent investigation and response occurs internally. Sometimes further consequences can entail the caregiver being named as a defendant or codefendant in a civil lawsuit, and sometimes the caregiver can have professional licensure restricted or even revoked. More rarely, a caregiver can be prosecuted in a criminal legal action. When criminal prosecution occurred, it was usually for purposeful wrongdoing such as fraud, diversion of drugs, or even the intentional or reckless killing of elderly or other vulnerable people. The recent criminal prosecution of a Tennessee nurse for the reckless series of mistakes that led to the death of a single patient opens new considerations for nurses, physicians, and all caregivers, along with hospitals and healthcare systems that employ and/or work with them. The "dynamic tension" of encouraging all caregivers to own up to mistakes with patients as quickly as possible in healthcare organizations seems to be especially challenged now by the Vaught decision. The climate crisis significantly impacts the health and well-being of older adults, both directly and indirectly. This issue is of growing concern in Canada due to the country's rapidly accelerating warming trend and expanding elderly population. This article serves a threefold purpose: (i) outlining the impacts of the climate crisis on older adults, (ii) providing a descriptive review of existing policies with a specific focus on the Canadian context, and (iii) promoting actionable recommendations. Our review reveals the application of current strategies, including early warning systems, enhanced infrastructure, sustainable urban planning, healthcare access, social support systems, and community engagement, in enhancing resilience and reducing health consequences among older adults. Within the Canadian context, we then emphasize the importance of establishing robust risk metrics and evaluation methods to prepare for and manage the impacts of the climate crisis efficiently. We underscore the value of vulnerability mapping, utilizing geographic information to identify regions where older adults are most at risk. This allows for targeted interventions and resource allocation. We recommend employing a root cause analysis approach to tailor risk response strategies, along with a focus on promoting awareness, readiness, physician training, and fostering collaboration and benchmarking. BACKGROUND: Police Tactical Groups (PTGs) are specialist police units tasked with rigorous physical and psychosocial duties. Consequently, selection courses (SCs) for service in these units must also be rigorous. Given the intensity of SCs, holistic monitoring for potential overstress may be beneficial. Heart Rate Variability (HRV) is one holistic stress measure that can be obtained in austere environments. OBJECTIVE: The purpose of this study was to profile HRV during a PTG SC. Six (n = 6) qualified male police officers attempted a 36-hour PTG selection course held at an Australian state facility. METHODS: HRV was obtained from Equivitaltrademark EQ02 + LifeMonitor bioharnesses. The selection course consisted of physically demanding events with minimal sleep (approx. 45 mins). Only one candidate completed the full selection course; whose results are reported here. RESULTS: A visual time-series of 384 consecutive 5 min HRV analyses was generated. Contextual analysis was applied to appreciate HRV changes between SC serials. HRV decline occurred during the planning of a navigation exercise and a pack march. Increases in HRV were observed throughout the pack march exercise and rest period. CONCLUSION: This case study demonstrates the potential utility for selection personnel to obtain additional insight into candidate responses to various occupational challenges throughout an SC. Information provided by HRV monitoring may support leadership decisions when evaluating personnel holistically. For example, the ability to continue occupational task execution even while experiencing potential overstress (as measured by HRV) and after food and sleep deprivation is desirable. This qualitative study is a systematic literature review (draws on literature primarily published within the last five years) addresses a comprehensive approach to a crucial but often overlooked aspect of cybersecurity: the human factors underlying insider threats. Attention is focused on the so-called "organizational arsonists" -- individuals who willfully seek to adversely impact the organization by inducing anarchy aligned with their own motivations, insiders who purposefully damage their companies using digital methods, someone intentionally causing mayhem within a company, which can be criminal in cyber environments. The purpose of the research is to identify how cybersecurity leadership can effectively detect and mitigate the risks associated with insiders, particularly those exhibiting arsonist -like behaviors. Review uncovering that organizational arsonists can escalate cybersecurity risks substantially, with insider incidents costing organizations an average of $16.2 million per incident. These incidents now represent a persistent challenge, increasing in frequency by 68% over the past year according to the 2022 Insider Threat Report. The findings highlight the necessity of leadership strategies that preemptively recognize and neutralize potential insider threats to improve organizational resilience and security posture. This approach not only informs current cybersecurity practices but also aids in the development of targeted policies and refined regulatory measures. By integrating insights from psychology, criminology, and cybersecurity, the study provides a comprehensive understanding of the human elements influencing insider threats, essential for enhancing both academic knowledge and practical applications in risk management. The results showed a parallel between the motivations of arsonists who set physical fires to the characteristics and motivations of insider threats who exploit organizational vulnerabilities. The impact of this research can be helpful in assisting cybersecurity professionals, leaders who strategize against cyber threats, and risk managers and analysts who understand and mitigate human factors and insider threats. Leaders and executives may use these insights to improve security resource allocation and culture. Policymakers and regulators may use the study's results to create more nuanced cybersecurity legislation, while academics and students in related disciplines can use it for future research. This study aims to analyze the urgency of policies, regulators, and risk management in maintaining business at one of the 16th largest banks in the USA, namely Silicon Valley Bank . The method used is descriptive quantitative method. The data used is secondary data in the form of financial reports from 2020 to 2022 and is enriched by qualitative data sourced from articles from 2019 to 2023. The results show that SVB's financial management has experienced an impairment in the ROA, NIM and CAR ratios. SVB has an urge or tendency to take very high risks in an effort to achieve large profits which result in the use of high leverage and unstable sources of funding. The urgency of policies, regulators and risk management is considered very high in the case of SVB because their overall role is in maintaining a sustainable business. [ABSTRACT FROM AUTHOR]

Aim: One of the most important, unpredictable and stressful areas in hospitals is the emergency department (ED) where seconds are crucial for providing immediate care and saving the patients' lives. Therefore, the present study aimed to identify the factors which impact the patient safety management as perceived by the ED nurses in Southern Iran. Design: This is a qualitative, descriptive study. Methods: The participants were 23 ED nurses selected via purposeful sampling who were asked to take part in an interview. Data were collected using semi‐structured, individual, in‐depth interviews and analysed via content analysis. Results: Analysis of the qualitative data yielded 4 themes and 12 subthemes. The four main themes were: negligence of safety standards and standard precautions, disregard of ethical principles, professional challenges and inefficient organizational management. The research aims to describe the state of enterprise risk management (ERM) in the insurance sector. It highlights emerging trends in the application of risk management in the insurance sector and thereby reports the prominent research gaps and new avenues for research in ERM. The research adopts a systematic literature review (SLR) approach, using 187 research papers spanning 44 years (1977–2021). The paper identifies the fact that most ERM and insurance sector research is performed in North America and Europe, while developing economies in Asia and Africa lag. The paper establishes a three-way relationship between ERM, risk management (RM) and risk-based capital (RBC) where RM is a subset of ERM and RBC is a driver of ERM. The research shows that very few studies are conducted on risk culture, three lines of defence and the role of chief risk officers. The determinants of ERM identified are board, firm size, audit and risk management committee and corporate governance. The determinants identified for firm value are return on assets, return on equity, profit, Tobin's Q, among others. EPC project performance is affected by the relation between work processes. E-procurement implemented as a tool to mitigate procurement process risk gives a positive effect and residual risk that needs to be managed further. This research aims to analyze the e-procurement process risk and identify the root causes of critical risks. Data is collected using Focus Group Discussion (FGD) and a questionnaire by 41 e-procurement practitioners from 5 (five) Indonesian construction state-owned enterprises (BUMN). There are 116 risks identified from 22 e-procurement steps, of which three risks are determined as critical risks based on Risk Priority Number analysis, with delay of material requisition document as the highest RPN risk. Fault Tree Analysis identifies that several failure modes, such as miscalculation, misallocation of workforce, and payment issues cause the critical risk. Preparing an in-house engineering team to support the calculation process can mitigate two critical risks at the same time. Cyber threats are one of the main issues in the digital era. Cyberattacks can result in financial, reputational and even operational disruption for organizations. In an effort to overcome this, organizations need to have an effective risk management system to identify, assess and manage cyber risks. The aim of this research is to determine the effectiveness of a risk management system that can help organizations reduce the impact of cyber threats. This study used qualitative research methods. The data collection technique in this research is literature study. The data that has been collected is then analyzed in three stages, namely data reduction, data presentation and drawing conclusions. The research results show that developing an effective risk management system is an important step for organizations in reducing the impact of cyber threats. In this study, we investigated social workers' use of risk assessments and risk management in cases concerning intimate partner violence. The study examined social workers' experiences of work performance, organizational conditions, internal and external collaboration and challenges and opportunities at work. We interviewed twelve respondents in Swedish social work offices about what structures, supports and foundations they found essential for work sustainability and resilience. Support from colleagues and managers, experience, openness and trust were critical factors for work satisfaction. The study showed that work experience created trust and security, partly because the social worker became better at translating laws and regulations into practical work. Concerning the severity of the cases, collaboration was essential for building workplace confidence and well-being. A well-functioning internal and external collaboration was described as trust, reducing stress and access to open and straightforward communication with others. Sustainable routines and access to guidelines facilitated the work. An unsupportive work climate included a culture of silence, a lack of trust and generated feelings of stress. Feelings of vulnerability in the professional role arose when the workgroup and/or the management did not take a supportive approach to operational work. [ABSTRACT FROM AUTHOR]

Objective: To (a) compare characteristics of patients who fall with those of patients who did not fall; and (b) characterise falls (time, injury severity and location) through three fall reporting methods (incident system reports, medical notes and clinician reports). Methods: A substudy design within a stepped‐wedge clinical trial was used: 3239 trial participants were recruited from two inpatient Geriatric Evaluation and Management Units and one general medicine ward in two Australian states. To compare the characteristics of patients who had fallen with those who had not, descriptive tests were used. To characterise falls through three reporting methods, bivariate logistic regressions were used. Results: Patients who had fallen were more likely than patients who had not fallen to be cognitively impaired (51% vs. 29%, p < 0.01), admitted with falls (38% vs. 28%, p = 0.01) and have poor health outcomes such as prolonged length of stay (24 [16–34] vs. 12 [8–19] days [IQR], p < 0.01) and less likely to be discharged directly to the community (62% vs. 47%, p < 0.01). Most falls were captured from medical notes (93%), with clinician (71%) and incident reports (68%) missing 21%–25% of falls. The proportion of injurious falls identified through incident reports was higher than medical records or clinician reports (40% vs. 34% vs. 37%). Conclusions: This study reaffirms the need to improve reporting falls in incident systems and at clinical handover to the team leader. Research should continue to use more than one method of identifying falls, but include data from medical records. Many falls cause injury, resulting in poor health outcomes. [ABSTRACT FROM AUTHOR]

Enterprise risk management holds significant importance in fostering sustainable growth of businesses and in serving as a critical element for regulatory bodies to uphold market order. Amidst the challenges posed by intricate and unpredictable risk factors, knowledge graph technology is effectively driving risk management, leveraging its ability to associate and infer knowledge from diverse sources. This review aims to comprehensively summarize the construction techniques of enterprise risk knowledge graphs and their prominent applications across various business scenarios. Firstly, employing bibliometric methods, the aim is to uncover the developmental trends and current research hotspots within the domain of enterprise risk knowledge graphs. In the succeeding section, systematically delineate the technical methods for knowledge extraction and fusion in the standardized construction process of enterprise risk knowledge graphs. Objectively comparing and summarizing the strengths and weaknesses of each method, we provide recommendations for addressing the existing challenges in the construction process. Subsequently, categorizing the applied research of enterprise risk knowledge graphs based on research hotspots and risk category standards, and furnishing a detailed exposition on the applicability of technical routes and methods. Finally, the future research directions that still need to be explored in enterprise risk knowledge graphs were discussed, and relevant improvement suggestions were proposed. Practitioners and researchers can gain insights into the construction of technical theories and practical guidance of enterprise risk knowledge graphs based on this foundation. Objectives: A patient safety near miss is a safety event that had the potential to cause harm, but did not reach the patient. For over 20 years healthcare has been exhorted to learn from patient safety near misses to support improvements in patient safety. The belief is that, by addressing the factors that contribute to patient safety near misses, harmful incidents will be avoided. However, there seems to have been little progress made to learn from patient safety near misses. This study aimed to explore why there has been limited progress, and how best patient safety near misses may be learned from. Methods: A qualitative case study was undertaken to explore the learning from patient safety near misses in different National Health Service contexts. Semi-structured interviews were conducted with patient safety leads in secondary care, primary care, and regional/national bodies. Interviews were recorded, transcribed, and thematically analysed. Results: Seventeen interviews were undertaken across the National Health Service, with further data collected from policy, guidance, field notes, and research memos. Thematic analysis identified the following: variations in safety event schema; limited processes for patient safety near misses; unsupportive reporting contexts; and assumed, but non-evidenced improvements in patient safety. Participants also shared their thoughts on how learning from patient safety near misses could be improved. Conclusion: A lack of progress has been made to learn from patient safety near misses in the National Health Service. This is contributed to by a lack of agreement about what is and how best to learn from a patient safety near miss. Purpose: This descriptive research study attempted to determine how general hospital nurses’ awareness of the importance of patient safety management and patient safety competency affected patient safety management activities. Methods: From September 13 to 26, 2022, a survey was administered to 230 ward nurses who provided direct care to patients at five non-accredited general hospitals being evaluated for accreditation located in metropolitan cities. The collected data were analyzed with descriptive statistics, the t-test, one-way analysis, the Scheffé test, Pearson correlation coefficients, and hierarchical regression using SPSS for Windows version 26.0. Results: In total, 221 (96.1%) respondents were female. The average age was 32.2 years, and the average clinical experience was 3.5 years; 196 participants (85.2%) were general nurses. Patient safety competency (β = .44, p < .001), awareness of the importance of safety management (β = .31, p < .001), and medication error experience (β = -.15, p = .002) all had statistically significant associations with patient safety management activities. The explanatory power of these variables for patient safety management activities was 50.7%. Conclusion: This study confirmed that patient safety competency, awareness of the importance of patient safety management, and experience with medication errors significantly influenced patient safety management activities. Working with a suicidal patient is a high-risk enterprise for the patient who might die from suicide, the patient's family who might lose a loved one, and the psychologist who is likely to feel extreme grief or fear of legal liability after the suicide of a patient. To minimize the likelihood of such patient deaths, psychologists must ensure that they know and follow the relevant laws dealing with suicidal patients, rely on risk management strategies that anticipate and address problems in treatment early, and use overarching ethical principles to guide their clinical decisions. This article looks at the roles of laws, risk management strategies, and ethical principles; how they interact; and how a proper understanding of them can improve the quality of patient care while protecting psychologists from legal liability. An understanding of clinical governance and risk management is essential for a career in obstetrics and gynaecology. In a stressed and struggling system, knowing where and how to improve our maternity services is crucial in order to keep our patients safe. This article outlines the principles of clinical governance with a focus on risk management; it provides insight as to how this is managed and implemented within NHS trusts. We will look at how just culture and systems-based learning determine the effectiveness of both, and we will demonstrate how this is used by external governing bodies to judge the safety of maternity units. Only through robust clinical governance structures is the NHS able to ensure that it learns from mistakes and evolves to safeguard future patients and its workforce. Knowledge of the inner workings of these structures is important for examination preparation, any job interview, and for providing responsible and safe care to patients as a clinician. Since the advent of South Africa's democracy in 1994, there has been an increasing demand by stakeholders for several public services due to a variation of complex and challenging events that have taken place and reported both in social and academic circles. In the context of the Northern Cape, there has been a continued increase in unemployment and poor service delivery coupled with an inability of the Department of Economic Development and Tourism (DEDaT) to achieve clean audit reports from the Auditor-General of South Africa (AGSA). This development raises questions about the implementation of ERM processes. Thus, this study aimed to assess the implementation of ERM processes using DEDaT as a case study. The study sought to determine the ERM systems currently deployed and the extent to which the ERM components are utilized by DEDaT. Data was gathered through in-depth interviews using a qualitative approach. A sample of eighteen participants who are ERM role players at DEDaT described their knowledge and experiences on the implementation of ERM systems and how the components are applied. From the findings, two main themes, namely, ‘ERM systems deployed at DEDaT’ and ‘perceptions on ERM components that are utilized,' emerged with related subthemes. The study revealed that DEDaT has ERM systems in place, however there were gaps in key areas that need improvement to enhance the ERM instrument. Furthermore, the research found that a set of eight components were utilized to implement the ERM process. Objective: The purpose of the study is to determine the disaster preparedness perceptions, psychological resilences and empathy levels of nurses after 2023 Great Turkiye Earthquake. Method: This descriptive study was conducted in 2023 using with E‐Anket. This study was carried out with voluntary participation of 464 nurses living in different geographical regions of Turkiye. The data of the study were obtained utilizing Nurses' Disasters Prepardeness Perception Scale (NDPPS), Pyschological Resileince Scale (PRS) and Empathy Scale (ES). Results: In this study, the disaster preparedness perception of the nurses was found to be at medium level. More than half of the nurses (65.1%) reported that they did not received any disaster training before. A meaningful was found between the nurses' disaster preparedness perceptions and their empathy levels and psychological resilence. Conclusions: As in many countries, it is possible to state that also in Turkiye, the training of nurses for disasters are inadequate and that the studies on this subject are limited. This study will provide a theoretical platform to develop nurses' awareness of disaster preparedness and pyschological resilience and empathic approach programs to increase disaster resilience, and to conduct future research on disaster nursing. Introduction to The Problem: Risk management is critical for Islamic banks, which must navigate financial and operational risks while adhering to Sharia principles. Understanding how risk governance structures influence risk management's effectiveness in enhancing financial and social performance is essential. Purpose/Objective Study: This study explores the moderation effect of risk governance structures on the relationship between risk management, specifically insolvency risk, financing risk, and operational risk, and Islamic banks' financial and social performance in Indonesia. Design/Methodology/Approach: The research utilized a dynamic panel data regression method to analyze data extracted from the annual reports of 11 Islamic commercial banks spanning from 2012 to 2021. Findings: The study finds that specific risk governance structures, including the size of the audit committee, its independence, the expertise of its members, and the frequency of its meetings, as well as the quality of external audits, significantly enhance the impact of risk management on both the financial and social performance of Islamic banks.
