1. Frequency of RPE65 Gene Mutation in Patients with Hereditary Retinal Dystrophy.

Abstract

Objectives: Hereditary retinal dystrophies are a rare group of diseases which are heterogeneous in genotype and phenotype and result in total blindness. One of the genetic defects that cause hereditary retinal dystrophy is mutation of the RPE65 gene. Genetic therapy studies in hereditary retinal dystrophies have increased in number recently, and important developments have been reported in these studies. Voretigene neparvovec-rzyl (Luxturna, Spark Therapeutics), a gene therapy drug for retinal dystrophy associated with RPE65 mutation, received Food and Drug Administration approval in 2017. This study aimed to investigate the frequency and clinical findings of patients with RPE65 gene defects, which may be amenable to genetic treatment. Materials and Methods: The data of patients diagnosed with hereditary retinal dystrophy who were followed up between 2017 and 2021 were retrospectively reviewed. Of these, 460 patients with genetic analysis results were included in the study. The clinical findings of patients with homozygous (biallelic) RPE65 mutation were screened. Results: RPE65 homozygous gene mutation was detected in only 11 of 460 cases (2.39%). Genetic results of the cases were presented in detail. The inheritance patterns of the cases were autosomal recessive. The demographic data and clinical findings were defined. Conclusion: RPE65 gene mutation is a very rare disorder. Genetic screening has gained importance with the emergence of gene therapy alternatives. New treatment methods are promising in cases for which there was no chance of a cure to date. Abstract

Use of stem cells in the treatment of retinal diseases is a new and popular topic in ophthalmology. Embryonic and bone marrow derived stem cells can be used for treatment. Age-related macular degeneration, Stargardt’s macular dystrophy and retinitis pigmentosa are common encountered retinal diseases causing progressive vision loss. The researches therefore mostly focus on these diseases which have no curative treatment modality in order to evaluate the efficacy of stem cell therapy. In this review, we aimed to present the results of the phase 1/2 clinical studies about stem cell treatments in eye diseases. Stem cell therapies are the rising trends in treatment of retinal diseases. Further clinical studies are required for standardization of the therapy and obtaining long-term data about the results and complications Retina hastalıklarının tedavisinde kök hücrelerin kullanımı oftalmolojide yeni ve popüler bir konudur. Embriyonik ve kemik iliği kaynaklı kök hücreler tedavi için kullanılabilir. Yaşla bağlı maküla dejenerasyonu, Stargardt'ın maküla distrofisi ve retinitis pigmentosa, progresif görme kaybına neden olan yaygın görülen retina hastalıklarıdır. Bu nedenle araştırmalar kök hücre tedavisinin etkinliğini değerlendirmek için herhangi bir küratif tedavi modalitesine sahip olmayan bu hastalıklara odaklanmaktadır. Bu yayında göz hastalıklarında kök hücre tedavisi ile ilgili faz 1/2 klinik çalışmaların sonuçlarını incelemeyi amaçladık. Kök hücre terapileri, retinal hastalıkların tedavisinde yükselen bir trenddir ve tedavinin standardizasyonu, sonuçlar hakkında uzun vadeli veriler elde etmek ve komplikasyonlara ilişkin daha ileri klinik çalışmalara ihtiyaç vardır.

Abstract

Stem cells are undifferentiated cells which have the ability to self-renew and differentiate into mature cells. They are highly proliferative, implying that an unlimited number of mature cells can be generated from a given stem cell source. On this basis, stem cell replacement therapy has been evaluated in recent years as an alternative for various pathologies. Degenerative retinal diseases cause progressive visual decline which originates from continuing loss of photoreceptor cells and outer nuclear layers. Theoretically, this therapy will enable the generation of new retinal cells from stem cells to replace the damaged cells in the diseased retina. In addition, stem cells are able to perform multiple functions, such as immunoregulation, anti-apoptosis of neurons, and neurotrophin secretion. With recent progress in experimental stem cell applications, phase I/II clinical trials have been approved. These latest stem cell transplantation studies showed that this therapy is a promising approach to restore visual function in eyes with degenerative retinal diseases such as retinitis pigmentosa, Stargardts' macular dystrophy, and age-related macular degeneration. Abstract

Hereditary retinal dystrophies (HRDs) are degenerative diseases of the retina which have marked clinical and genetic heterogeneity. Common presentations among these disorders include night or colour blindness, tunnel vision, and subsequent progression to complete blindness. The known causative disease genes have a variety of developmental and functional roles, with mutations in more than 120 genes shown to be responsible for the phenotypes. In addition, mutations within the same gene have been shown to cause different disease phenotypes, even amongst affected individuals within the same family, highlighting further levels of complexity. The known disease genes encode proteins involved in retinal cellular structures, phototransduction, the visual cycle, and photoreceptor structure or gene regulation. Significant advancements have been made in understanding the genetic pathogenesis of ocular diseases, and gene replacement and gene silencing have been proposed as potentially efficacious therapies. Because of its favorable anatomical and immunological characteristics, the eye has been at the forefront of translational gene therapy. Recent improvements have been made in the safety and specificity of vector-based ocular gene transfer methods. Dozens of promising proofs of concept have been obtained in animal models of HRDs and some of them have been relayed to the clinic. The results from the first clinical trials for a congenital form of blindness have generated great interest and have demonstrated the safety and efficacy of intraocular administrations of viral vectors in humans. Abstract

Retinopathy of prematurity (ROP), primarily affects premature infants and it can cause severe visual disability in childhood. It is characterized by anormal retinal neovascularizations and the main reason of blindness in children in developing countries. The well known risk factors are premature birth, low birth weight, high oxygen therapy ; the others are asidozis, blood transfusion, intracranial hemorrhage, multiple birhts and etnicity. The pathogenezis of ROP occurs in two phases. In the first phase there is relative hyperoxia and obliteration in vessels, in the second phase there is neovascularization which is induced by hypoxia. Neovascularization, occured by interruption of normal angiogenesis, reaches vitreus. Anormal fibrovasküler doku develops and it causes retinal traction and detachment. Abstract

Amaç: Kronik santral seröz koryoretinopatili (SSKR) olgularda fotodinamik tedavi (FDT) öncesi ve sonrası optik kohorens tomografi (OKT) ve multifokal elektroretinografi (mfERG) bulgularının değerlendirilmesi amaçlanmıştır.Hastalar ve Yöntem: Çalışmaya kliniğimizde takip edilen 31 kronik SSKR hastasının 31 gözü dahil edildi. Kontrol grubu rutin polikliniğe başvuran aynı yaş grubundan 15 sağlıklı gönüllüden oluşturuldu. Çalışma grubunda bulunan tüm olgulara FDT öncesi ve sonrasında 1. hafta, 1. Ay ve 3. ay takiplerinde rutin oftalmolojik muayene ile birlikte OKT, mf ERG testleri yapıldı. Bulgular: Tüm olgular 6 aylık takiplerini tamamladı. Takiplerde hiçbir olguda sistemik ve okuler komplikasyon gelişmedi. Kontrol grubu ile çalışma grubunun tedavi öncesi değerleri ile tedaviden 3 ay sonra elde edilen görme keskinliği, santral fovea ve koroid kalınlıkları karşılaştırıldı. Ortalam görme keskinliği değeri çalışma grubunda tedavi öncesinde kontrol grubuna göre anlamlı olarak düşüktü. Tedaviden 3 ay sonra görme keskinliği düzeyleri anlamlı olarak iyileşmesine rağmen kontrol grubu değerlerine ulaşamadı. FDT öncesi ve sonrası OKT ölçümleri karşılaştırıldığında santral koroidal kalınlığı (SKK) ve santral makula kalınlığında (SMK) anlamlı azalma görüldü. mfERG değerlendirmesinde çalışma grubunun FDT öncesindeki santral 5 derecede N1 ve P1 dalgalarının amplitüdleri kontrol grubuna göre anlamlı olarak düşük bulunurken, latansları da anlamlı olarak uzun bulundu. FDT sonrasında mfERG değerlerinde anlamlı iyileşmeler gözlenirken, santral 5 derecede ölçülen değerler kontrol grubunun değerlerine ulaşamadı.mfERG de santral 5 derecede N1 ve P1 amplitüd ve latans değerleri ile SMK arasında anlamlı negatif korelasyon izlendi. Ayrıca görme keskinliği değerleri ile mfERG santral 5 derecede mfERG parametreleri arasında anlamlı pozitif korelasyon saptandı.Sonuç: SSKR hastalarının anatomik ve fonksiyonel değerlendirilmesinde mf-ERG ve OKT birbirlerini tamamlayıcı tetkikler olarak kullanabilir. FDT sonrasında mfERG ile değerlendirilen fonksiyonel düzelme, OKT ile değerlendirilen anatomik düzelmeden daha geç görülebilmektedir. mfERG'nin santral parametreleri ile SMK arasındaki korelasyon makulada elektriksel cevap iletimindeki bozukluğun makulada biriken sıvı ile orantılı olduğunu düşündürmektedir. Purpose: The aim was to compare optical coherence tomography (OCT) and multifocal electroretinogram (mfERG) findings of chronic central serous chorioretinopathy (CSCR) cases before and after photodynamic therapy (PDT.Patients and method: We included 31 eyes of 31 patients with chronic CSCR. The control group was formed of 15 healthy individuals who admitted to our routine clinic. OCT and mfERG tests were performed along with normal opthalmological examination before PDT and at first week, first month and third month after PDT. Results: All cases completed the 6 month follow up period. There were no systemic or ocular complications during this period. Visual acuity, central foveal and choroidal thickness measurements were compared between the control and the patient group before and 3 months after treatment. The mean visual acuity of the patient group before treatment was significantly lower than the mean visual activity of the control group. The mean visual acuity of the patients 3 months after the treatment showed significant recovery however could not reach control group levels. The comparison of OCT measurements before and after PDT showed significant decrease in central choroidal thickness (CCT) and central macular thickness (CMT) after PDT.When we evaluate mfERG analysis of the patient group before PDT, N1 and P1 wave amplitudes of central 5 degrees were significantly lower than the control group and latencies were found to be significantly delayed. Significant improvements were seen in mfERG findings after FDT, however values at cental 5 degrees could not reach the control group levels. N1 and P1 wave amplitude and latency values of the mfERG, at the central 5 degrees showed negatif correlation with CMT. There was also a positive correlation between visual acuity levels and mfERG parameters at central 5 degrees showed. Conclusion: In the anatomical and functional analysis of CSCR patients, mfERG and OCT can be used as complementary techniques. Functional improvement evaluated with mfERG after FDT could be observed later than the anatomical improvement evaluated with the OCT. Negatif correlation between central parameters of the mfERG and central macular thickness may show that electrical impulse transmission defects can correlate with the amount of fluid collected between the layers of the macula.Key words: Central serous chorioretinopathy (CSCR), Photodynamic therapy (PDT), Multifocal electroretinography (mfERG), Optical cohorence tomograhy (OCT) 75

Abstract

Amaç: Prematüre retinopatisi (PR) nedeniyle takip edilen hastaların birinci yıl kontrollerindeki göz bulgularını değerlendirmekGereç ve Yöntem: Ağustos 2010 ile Ağustos 2013 tarihleri arasında Erciyes Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Polikliniğine bir yaş kontrolüne gelen gestasyonel yaşı 32 hafta ve altında olan veya doğum ağırlığı 1500 gram ve altında doğan toplam 70 bebek çalışmaya dahil edildi. Hastalar lazer fotokoagülasyon yapılanlar (n=27) ve yapılmayanlar (n=43) olmak üzere iki gruba ayrıldı. İki grup arasında risk faktörleri, şaşılık insidansı, sferik ekivalan (SE) değerleri, F-VEP testi sonuçları açısından farklılıklar değerlendirildi.Bulgular: Lazer yapılan ve yapılmayan iki grup arasında cinsiyet, F-VEP test sonuçları bakımından istatistiksel olarak anlamlı fark bulunmadı (p>0.05). Gestasyonel yaş, doğum ağırlığı, sistemik risk faktörleri, hastaların takiplerinde görülen PR evresi, ezotropya ya da ekzotropya bulunması, SE değerleri yönlerinden iki grup arasında istatistiksel olarak anlamlı fark tespit edildi (p0.05).
