Purpose: The aim of this study was to determine if self-threading dental implants placed using stopper drills to bicortically engage both the alveolar crest and sinus floor (bicortical fixation) achieved primary and/or secondary stability comparable to that of short implants only engaging alveolar crest cortical bone (unicortical fixation) or implants engaging both the crest and sinus floor but via greenstick fracture and grafting (indirect sinus elevation). Materials and Methods: Thirty-eight patients exhibiting 7 to 11 mm of bone coronal to the sinus floor as confirmed by preoperative CBCT were recruited. Forty-five implants were randomly assigned to one of the placement techniques. No patient received more than two implants, which were placed in opposite sides of the maxilla while using different surgical techniques. An Osstell ISQ was employed immediately after implant placement to measure stability six times in a buccolingual dimension. Secondary stability was measured at stage-two surgery after a 3- to 6-month healing period. Results: The greatest primary implant stability was achieved via indirect sinus elevation. However, no statistically significant difference was found among the three surgical techniques (P = .13; bicortical fixation: 71.4 [standard error = 2.1]), unicortical fixation: 69.6 [2.1], indirect sinus elevation: 75.9 [2.3]). The three techniques had similar secondary stability (P > .999; 79.9 [1.2], 80.0 [1.2], and 80.0 [1.3], respectively). Baseline residual ridge height measured on CBCT was similar (P = .1; 8.8, 9.9, and 9.4 mm, respectively), but implant diameter and length placed in the maxilla differed (P =,03/P < .001; 4.7/11.4 mm, 4.3/8.1 mm, and 4.7/11.8 mm, respectively). Primary implant stability was significantly correlated to CBCT bone density (r = 0.37). Conclusion: Primary and secondary implant stabilities of bicortical fixation did not differ significantly from those of unicortical fixation and indirect sinus elevation. However, use of the bicortical fixation technique is warranted since it is simpler and more economical than indirect sinus elevation; plus, it allows for longer implants than the unicortical fixation while yielding similar secondary implant stability. Objective: Compare effectiveness of two differently formatted training programs in educating night-time postural care implementers. Design: Mixed-methods parallel-group double-blind design with random assignment. Setting: United States academic institution. Participants: Thirty-eight adult caregivers/providers of children with cerebral palsy. Interventions: Both 2-hour online programs included content on night-time postural care evidence, risk-factor monitoring, sleep-system types, positioning methods, and assessments. Group A used interactive videos, Group B summary information with web-links. Main Measures: We measured self-perceived competence via questionnaires (baseline, post-training, post-simulation) containing 4-point rating-scales of knowledge, ability, and confidence and measured positioning ability via a simulation observation instrument comprising 16 positioning-task ratings with space for describing performance. We recorded participant actions/statements using fieldnotes. Results: Thirty-eight completed training (19 per group). Group A (vs B) showed significantly greater self-perceived competence changes post-training (0.46 points (SE 0.17), P = 0.008). Thirty-seven positioned a standardized "client," with groups not differing significantly on total tasks completed correctly (F (1, 92.32) = 1.91, P = 0.17) averaging 11.85 (SE 0.83) and 12.60 (SE 0.84) of 16 tasks correct. Group A's post-positioning/simulation self-ratings were significantly associated with actual ability (r = 0.53, P = 0.019). In both groups ⩾47% of caregivers incorrectly completed the tasks of placing head and neck in neutral and snugging up all [positioning] parts. Conclusion: The sleep care positioning training program (interactive video-based format) is effective in building caregivers' self-perceived competence for night-time postural care. While the lesson was well-received by caregivers and considered a "match [to their] learning style," the lesson did not lead to greater improvement in actual ability to position the "client" compared to control training. Bayesian analyses with the arm-based (AB) network meta-analysis (NMA) model require researchers to specify a prior distribution for the covariance matrix of the treatment-specific event rates in a transformed scale, for example, the treatment-specific log-odds when a logit transformation is used. The commonly used conjugate prior for the covariance matrix, the inverse-Wishart (IW) distribution, has several limitations. For example, although the IW distribution is often described as noninformative or weakly informative, it may in fact provide strong information when some variance components are small (eg, when the standard deviation of study-specific log-odds of a treatment is smaller than 1/2), as is common in NMAs with binary outcomes. In addition, the IW prior generally leads to underestimation of correlations between treatment-specific log-odds, which are critical for borrowing strength across treatment arms to estimate treatment effects efficiently and to reduce potential bias. Alternatively, several separation strategies (ie, separate priors on variances and correlations) can be considered. To study the IW prior's impact on NMA results and compare it with separation strategies, we did simulation studies under different missing-treatment mechanisms. A separation strategy with appropriate priors for the correlation matrix and variances performs better than the IW prior, and should be recommended as the default vague prior in the AB NMA approach. Finally, we reanalyzed three case studies and illustrated the importance, when performing AB-NMA, of sensitivity analyses with different prior specifications on variances. [ABSTRACT FROM AUTHOR]

Objective: In contrast to intentionally restricting energy intake, restricting the eating window may be an option for treating obesity. By comparing time-restricted eating (TRE) with an unrestricted (non-TRE) control, it was hypothesized that TRE facilitates weight loss, alters body composition, and improves metabolic measures.Methods: Participants (17 women and 3 men; mean [SD]: 45.5 [12.1] years; BMI 34.1 [7.5] kg/m2 ) with a prolonged eating window (15.4 [0.9] hours) were randomized to TRE (n = 11: 8-hour window, unrestricted eating within window) versus non-TRE (n = 9: unrestricted eating) for 12 weeks. Weight, body composition (dual x-ray absorptiometry), lipids, blood pressure, 2-hour oral glucose tolerance, 2-week continuous glucose monitoring, and 2-week physical activity (actigraphy assessed) were measured during the pre- and end-intervention periods.Results: The TRE group significantly reduced the eating window (end-intervention window: 9.9 [2.0] hours) compared with the non-TRE group (end-intervention window: 15.1 [1.1] hours) (P < 0.01). Compared with non-TRE, TRE decreased the number of eating occasions, weight, lean mass, and visceral fat (all P ≤ 0.05). Compared with preintervention measures, the TRE group reduced the number of eating occasions (-21.9% [30.1%]) and reduced weight (-3.7% [1.8%]), fat mass (-4% [2.9%]), lean mass (-3.0% [2.7%]), and visceral fat (-11.1% [13.4%]) (all P ≤ 0.05). Physical activity and metabolic measures remained unchanged.Conclusions: In the setting of a randomized trial, TRE presents a simplified view of food intake that reduces weight. Purpose: Dental implant bone cement (DIBC) was developed to rescue unstable implants at the timeof placement. The purpose of this study was to compare the mechanical properties of bone- dementimplant interfaces of cemented implants that were unstable initially and bone-implant interfaces of self-threaded implants placed in rabbit femurs after various healing periods. Interfaces and failuremodes were also characterized using histology and scanning electron microscopy (SEM). Materials and Methods: Eighty dental implants were placed in 20 rabbits. In each rabbit, two experimental and two control implants were placed in the right or left femur; one was in the distal epiphysis and the other in the cortical shaft. The experimental implants were cemented in loosely prepared bony sockets, while the control implants were self-threaded. The rabbits were sacrificed after varying healing periods. Magnetic pulse stability and push-in yield tests on ex vivo specimens measured secondary implant stability, 0.2% yield load, displacement, interface stiffness, and load at 100 ?m. After loading tests, interfaces were evaluated with histology and SEM. Most analyses used mixed linear models. Results: The 0.2% yield load, interface stiffness, load at 100 ?m, and secondary stability were significantly higher for bone-cement-implant interfaces than for bone-implant interfaces. Mechanical properties of bone-cement-implant interfaces plateaued at 1 week, with minimal change over the following 12 weeks, whereas bone-implant interfaces improved gradually. SEM and histology showed intimate bone-cement-implant interfaces without soft tissue intervention and mainly cohesive failures within DIBC. Secondary stability was significantly correlated with interface stiffness and load at 100 ?m. Conclusion: The results suggest that DIBC can provide early implant stability and mechanical properties superior to those of self-threaded implants while maintaining intimate interfaces without signs of osteonecrosis or soft tissue intervention. In observational studies, misclassification of exposure is ubiquitous and can substantially bias the estimated association between an outcome and an exposure. Although misclassification in a single observational study has been well studied, few papers have considered it in a meta-analysis. Meta-analyses of observational studies provide important evidence for health policy decisions, especially when large randomized controlled trials are unethical or unavailable. It is imperative to account properly for misclassification in a meta-analysis to obtain valid point and interval estimates. In this paper, we propose a novel Bayesian approach to filling this methodological gap. We simultaneously synthesize two (or more) meta-analyses, with one on the association between a misclassified exposure and an outcome (main studies), and the other on the association between the misclassified exposure and the true exposure (validation studies). We extend the current scope for using external validation data by relaxing the "transportability" assumption by means of random effects models. Our model accounts for heterogeneity between studies and can be extended to allow different studies to have different exposure measurements. The proposed model is evaluated through simulations and illustrated using real data from a meta-analysis of the effect of cigarette smoking on diabetic peripheral neuropathy. [ABSTRACT FROM AUTHOR]

Dual X-ray absorptiometry (DXA) remains the most common mode of bone mineral density (BMD) evaluation. In adults, presence of a lumbar spine (LS) BMD T-score discrepancy (>1 SD difference between adjacent vertebrae) can indicate a vertebral fracture. In children, however, the clinical significance of such discrepancies is unknown. We conducted a retrospective study to evaluate the association between LS DXA and LS morphology to elucidate the clinical significance of an LS BMD Z-score discrepancy. We identified 360 DXA scans performed between September 2014 and May 2016 in patients 5-18 years of age. DXA scans were cross-referenced against available LS radiographs and vertebral fracture assessment (VFA) within the 6 months preceding or following a DXA scan. After excluding 44 DXA scans because of spinal hardware, incomplete DXA, or repeat scans, 316 DXA scans were included; 81 (25.6%) had either an LS radiograph or a VFA. Twenty-five of 81 patients (30.9%) had >1 SD difference between adjacent vertebrae in LS BMD Z-score. Two of these 25 patients (8%) had a lumbar vertebral fracture documented by a spine radiograph. Of the remaining 56 patients who did not have a discrepancy >1 SD, 6 patients (11%) had a lumbar vertebral fracture. Discrepancies in LS BMD Z-scores were not associated with lumbar vertebral fractures and, in the absence of fractures, likely represented vertebral developmental variants in children whose skeletons are still growing. Importance: Biotinylated antibodies and analogues, with their strong binding to streptavidin, are used in many clinical laboratory tests. Excess biotin in blood due to supplemental biotin ingestion may affect biotin-streptavidin binding, leading to potential clinical misinterpretation. However, the degree of interference remains undefined in healthy adults.Objective: To assess performance of specific biotinylated immunoassays after 7 days of ingesting 10 mg/d of biotin, a dose common in over-the-counter supplements for healthy adults.Design, Setting, and Participants: Nonrandomized crossover trial involving 6 healthy adults who were treated at an academic medical center research laboratory.Exposure: Administration of 10 mg/d of biotin supplementation for 7 days.Main Outcomes and Measures: Analyte concentrations were compared with baseline (day 0) measures on the seventh day of biotin treatment and 7 days after treatment had stopped (day 14). The 11 analytes included 9 hormones (ie, thyroid-stimulating hormone, total thyroxine, total triiodothyronine, free thyroxine, free triiodothyronine, parathyroid hormone, prolactin, N-terminal pro-brain natriuretic peptide, 25-hydroxyvitamin D) and 2 nonhormones (prostate-specific antigen and ferritin). A total of 37 immunoassays for the 11 analytes were evaluated on 4 diagnostic systems, including 23 assays that incorporated biotin and streptavidin components and 14 assays that did not include biotin and streptavidin components and served as negative controls.Results: Among the 2 women and 4 men (mean age, 38 years [range, 31-45 years]) who took 10 mg/d of biotin for 7 days, biotin ingestion-associated interference was found in 9 of the 23 (39%) biotinylated assays compared with none of the 14 nonbiotinylated assays (P = .007). Results from 5 of 8 biotinylated (63%) competitive immunoassays tested falsely high and results from 4 out of 15 (27%) biotinylated sandwich immunoassays tested falsely low.Conclusions and Relevance: In this preliminary study of 6 healthy adult participants and 11 hormone and nonhormone analytes measured by 37 immunoassays, ingesting 10 mg/d of biotin for 1 week was associated with potentially clinically important assay interference in some but not all biotinylated assays studied. BACKGROUND: Severe malaria in children is associated with long-term neurocognitive impairment, but it is unclear whether it is associated with long-term behavioral problems. METHODS: Children <5 years old with cerebral malaria (CM) or severe malarial anemia (SMA) treated at Mulago Hospital, Kampala, Uganda were assessed for behavioral outcomes at 0, 6,12, and 24 months using the Child Behavior Checklist. Sample sizes at 0,12, and 24 months were 122,100, and 80 in the CM group, 130, 98, and 81 in the SMA group, and 149, 123, and 90 in healthy community control (CC) children, respectively. Age adjusted z-scores for behavioral outcomes were computed using scores for the CC group. Study groups were compared using regression models adjusted for age, nutritional status, preschool education, and socioeconomic status. RESULTS: At 12 months, children with SMA had higher z-scores than CC children for internalizing (mean difference, 0.49; SE, 0.14; P = .001), externalizing (mean difference, 0.49; SE, 0.15; P = .001), and total problems (mean difference, 0.51; SE, 0.15; P < .001). Children with CM had higher adjusted z-scores than CC children for externalizing problems (mean difference, 0.39; SE, 0.15; P = .009) but not internalizing or total problems. At 24 months, children with CM or SMA both had increased internalizing and externalizing behavioral problems compared with CC children (P ≤ .05 for all). CONCLUSIONS: CM and SMA are associated with long-term internalizing and externalizing behavioral problems in children. Network meta-analysis of randomized controlled trials is increasingly used to combine both direct evidence comparing treatments within trials and indirect evidence comparing treatments across different trials. When the outcome is binary, the commonly used contrast-based network meta-analysis methods focus on relative treatment effects such as odds ratios comparing two treatments. As shown in a recent report, when using contrast-based network meta-analysis, the impact of excluding a treatment in the network can be substantial, suggesting a methodological limitation. In addition, relative treatment effects are sometimes not sufficient for patients to make decisions. For example, it can be challenging for patients to trade off efficacy and safety for two drugs if they only know the relative effects, not the absolute effects. A recently proposed arm-based network meta-analysis, based on a missing-data framework, provides an alternative approach. It focuses on estimating population-averaged treatment-specific absolute effects. This article examines the influence of treatment exclusion empirically using 14 published network meta-analyses, for both arm- and contrast-based approaches. The difference between these two approaches is substantial, and it is almost entirely due to single-arm trials. When a treatment is removed from a contrast-based network meta-analysis, it is necessary to exclude other treatments in two-arm studies that investigated the excluded treatment; such exclusions are not necessary in arm-based network meta-analysis, leading to substantial gain in performance. A history of trauma is common in refugee populations and appropriate treatment is frequently avoided. Using a convenience sample of 64 patients in a Somali primary care clinic, a culture and trauma specific intervention was developed to address retention into appropriate treatment. One goal of the intervention was to improve the rate of engagement in psychotherapy after a mental health referral and to test the effect of psychotherapy on health care utilization using a staged primary care clinical tool. Forty-eight percent of patients given a mental health referral engaged in psychotherapy. Patients engaging in psychotherapy had higher baseline utilization and over 12 months trended towards less emergency room use and more primary care. Background: Past studies of health-related quality of life (HRQL) in aortic stenosis (AS) have focused on valve replacement, using generic or heart failure measures because no disease-specific measure exists. The literature is lacking in both performance of these measures among patients with AS and HRQL outcomes in the nonsurgical elderly AS population. Objective: The aims of this study were to measure HRQL and test the reliability of the Minnesota Living With Heart Failure Questionnaire (MLHFQ), Geriatric Depression Scale (GDS), and Functional Assessment of Chronic IIIness Therapy-Spiritual Well-being Scale (FACIT-Sp) in persons older than 70 years with AS receiving nonsurgical treatment. Methods: The prospective, descriptive design in this study included baseline administration of questionnaires to a consecutive convenience sample of patients aged 75 to 97 years (mean, 85 years) enrolled in a clinical trial for AS (n = 25). Results: Mean aortic valve area was 0.54 cm² (range, 0.37 0.96 cm²). Patients reported angina (52%) and light-headedness (72%); these 2 items were not included on the MLHFQ but were added as investigator-developed items. Scores for MLHFQ varied widely (median, 52; range, 7-101). Although the median GDS was 4 (range, 1-13), almost half (48%) scored higher than 5, indicating a positive depression screen. Scores for FACIT-Sp were moderately high (median, 37.5; range, 18-45), indicating strong spiritual well-being among many participants. A significant inverse relationship (r = -0.73, P < .0001; 95% confidence interval, -0.87 to -0.48) was found between depression and spiritual well-being. Cronbach α was 0.91, 0.83, and 0.81 for the MLHFQ, GDS, and FACIT, respectively. Conclusions: The HRQL measures selected had good internal consistency reliability, but use of the MLHFQ alone would have missed common disease-specific concerns (eg, angina, light-headedness); studies for minimally invasive aortic valve replacement should include these items. Because higher spiritual well-being was associated with less depressive symptoms, both should receive further study in HRQL assessment. Larger samples may clarify appropriate education and interventions for depressive symptoms, spiritual well-being, as well as safe physical activity and fall prevention for those with light-headedness. Purpose: To elucidate long-term sequelae of radiation therapy (RT) in head and neck cancer (HNC) patients, a multicenter, prospective study, Clinical Registry of Dental Outcomes in Head and Neck Cancer Patients (OraRad), was established with tooth failure as its primary outcome. We report tooth failure and associated risk factors.Methods and Materials: Demographics and cancer and dental disease characteristics were documented in 572 HNC patients at baseline and 6, 12, 18, and 24 months after RT. Eligible patients were aged 18 or older, diagnosed with HNC, and receiving RT to treat HNC. Tooth failure during follow-up was defined as losing a tooth or having a tooth deemed hopeless. Analyses of time to first tooth-failure event and number of teeth that failed used Kaplan-Meier estimators, Cox regression, and generalized linear models.Results: At 2 years, the estimated fraction of tooth failure was 17.8% (95% confidence interval, 14.3%-21.3%). The number of teeth that failed was higher for those with fewer teeth at baseline (P < .0001), greater reduction in salivary flow rate (P = .013), and noncompliance with daily oral hygiene (P = .03). Patients with dental caries at baseline had a higher risk of tooth failure with decreased salivary flow. Patients who were oral-hygiene noncompliant at baseline but compliant at all follow-up visits had the fewest teeth that failed; greatest tooth failure occurred in participants who were noncompliant at baseline and follow-up.Conclusions: Despite pre-RT dental management, substantial tooth failure occurs within 2 years after RT for HNC. Background: Hallucinogen use is rising in the US, yet little is known regarding the prevalence and psychosocial/behavioral correlates of driving under the influence of these drugs.Methods: This study examined data from 4447 individuals ages 16-64 who reported past-year hallucinogen use in the 2016-2018 National Survey on Drug Use and Health. Prevalence estimates (with 95% confidence intervals [CIs]) were calculated for driving under the influence of hallucinogens (DUIH). Logistic regression examined psychosocial/behavioral correlates of DUIH, and latent class analysis (LCA) characterized subgroups of individuals reporting DUIH.Results: The prevalence of past-year hallucinogen use was 2.42% (CI = 2.30-2.54) and, in the general population, the prevalence of DUIH was 0.21%. Among past-year hallucinogen users, 8.94% (CI = 7.74-10.31) reported DUIH on at least one occasion over the previous 12 months. The probability of DUIH increased significantly with more frequent use. Compared to individuals who used hallucinogens and did not report DUIH, individuals reporting DUIH were significantly more likely to report mental health problems; use of other illicit drugs; selling drugs; a past-year arrest; or driving under the influence of alcohol, cannabis, or other illicit drugs. LCA identified three classes of individuals reporting DUIH, characterized by: use of and driving under the influence of cannabis; use of and driving under the influence of cannabis and other illicit drugs; and mental health concerns.Conclusions: Nearly one in ten individuals who report using hallucinogens also report driving under the influence of hallucinogens-drugs that affect perception and risk-taking, with alarming implications for driving safety.
