Background: With the increase in immunosuppressed patients and antimicrobial misuse, Non-diphtheriae Corynebacterium species (NDC spp.) have risen as opportunistic pathogens in hospitalized patients. The current work aimed at comparing Vitek 2C automated identification cards versus MALDI-TOF (Matrix-associated laser-induced desorption ionization-time of flight) as the gold standard method for the identification of different NDC spp. and determining the antimicrobial susceptibility of NDC spp. by the disc diffusion method for proper management of patients. Methods: Thirty NDC spp. isolates were subjected to identification by MALDI-TOF MS and Vitek 2C and antibiotic susceptibility testing by the disc diffusion method. Results: Our study showed that Corynebacterium (C.) striatum was the most commonly isolated species (83.3%), followed by C. amycolatum (10%). 36.7 % of the isolates were recovered from blood culture while 20 % were recovered from deep wounds. As regard isolate identification by Vitek-2C ANC ID CARD compared to MALDI-TOF as the gold standard method, the agreement was 86.7%. Regarding the antimicrobial susceptibility by disc diffusion method, all the isolates showed increased resistance against penicillin (100%), ciprofloxacin (70%), gentamicin (93.3%), tetracycline (76.6%), and clindamycin (70%). On the other hand, vancomycin and linezolid showed promising results where 100 % of the isolates were susceptible. Conclusion: We spotlight that NDC spp. is not viewed as a contaminant organism anymore, particularly in immunosuppressed patients. Timely identification, proper clinical correlation, and appropriate therapeutic intervention can lead to favorable outcomes.

Background: Malaria is a severe disease, so delay in treatment could increase morbidity and mortality. World Health Organization recommends confirmation of the diagnosis by laboratory test before initiation of therapy. Smear microscopy and polymerase chain reaction are methods approved by WHO for diagnosing malaria, but they are time-consuming, operator-dependent, and require laboratory staff training. WHO has recommended the Blood-based Rapid Diagnostic Test (RDT) as an acceptable method for diagnosing malaria. It provides rapid results and can be performed with limited resources. Methods: Four hundred and fourteen febrile cases admitted to Khartoum North General Hospital, Khartoum, Sudan during January 2021 - June 2021 with clinical suspicion of malaria were examined for the presence of asexual forms of Plasmodium falciparum in peripheral blood smears. Blood smear microscopy-positive patients who matched the acceptance criteria were included, while negative patients were selected as control cases. Blood and urine samples were examined with the same RDT kits designed for blood. Results: Fifty-eight blood smear-positive cases and 50 febrile blood smear-negative cases were enrolled in the study. The sensitivity and specificity of urine-based RDT were 82.76% and 92.00%, respectively, while blood-based RDT was 96.55% and 96.00%. Conclusion: This study aimed to evaluate the performance of urine-based RDT through comparison with blood-based RDT and the use of blood smear microscopy as the reference method. Although the results showed acceptable performance of both tests, more extensive sample size studies should be conducted to consider urine samples as an alternative sample for diagnosing Plasmodium falciparum malaria.

Background: The prevalence of fungal keratitis has risen in recent years significantly. To avoid additional complications, diagnosing and treating fungal keratitis is crucial. This study aims to measure the efficacy of a calcofluor white (CFW) stain for the quick diagnosis of fungal keratitis and to contrast the positive rates, sensitivity, and specificity with a 10% potassium hydroxide (KOH)-based smear and culture technique. Methods:From individuals with clinically suspected corneal ulcers, 30 corneal scrapings had been collected. Data on demographics had been analyzed. Results: Of the 30 patients, 40% were women and 60% were men. There was a 1.5:1 man-to-woman ratio. The age of patients ranged from 29 to 71 years (mean 46.67 ± 10.90). The age presentation of those between the ages of 41 and 50 years was the most frequent (36.7%). The majority of cases were farmers (43.3%). Trauma was the most common predisposing factor (46.6%). Twenty-four (80%) cases were culture positive. Eleven (36.7%) were fungal, 13 (43.3%) were bacterial and 6 (20%) showed no growth. Fusarium was the most common fungal isolate (36.4%), followed by Aspergillus (27.3%). While Staphylococcus aureus was the most common bacterial isolate (46.2%), followed by Pseudomonas (38.4%). The sensitivity of KOH wet mount and CFW stain was 72.7% and 90.9%, respectively. The specificity of both KOH wet mount and CFW stain was 100%. Conclusion:The early diagnosis of fungal keratitis can be made rapidly by direct microscopic examination of fungal elements using CFW stain. When diagnosing fungal keratitis, CFW has higher sensitivity to KOH.
