Dosimetric benchmarking at four hospitals was performed to investigate incident entrance dose and dose rate on a phantom, and entrance detector dose and dose rate for protocols that are used in routine clinical practice for complex neuroradiological treatment of arteriovenous malformations (AVMs). Measurements were performed with a head phantom that simulates the attenuation and scattering of the human head for the lateral and posteroanterior (PA) views. For fluoroscopy, the measured incident entrance dose rate and entrance detector dose rate were in the range of 44-172 and 0.3-1.3 μGy s(-1), respectively. The pulse rate in fluoroscopy varied between 6.3 and 15 frames per second (fps). For digital subtraction angiography (DSA), incident entrance dose per frame and entrance detector dose per frame were in the range of 744-2800 and 2.6-8.1 μGy/frame, respectively. Optimisation of acquisition parameters such as pulse rate in fluoroscopy, dose per frame in DSA, beam filtration and tube voltage may further improve imaging protocols and lower the patient dose in very complex X-ray-guided embolisations of AVMs in the brain. However, differences in these acquisition parameters observed in this study were relatively small, suggesting that a relatively high degree of optimisation has already been achieved., (© The Author 2016. Objective: The purpose of this study was to estimate the patient dose from perfusion CT examinations of the brain, lung tumors, and the liver on a cone-beam 320-MDCT scanner using a Monte Carlo simulation and the recommendations of the International Commission on Radiological Protection (ICRP)., Materials and Methods: A Monte Carlo simulation based on the Electron Gamma Shower Version 4 package code was used to calculate organ doses and the effective dose in the reference computational phantoms for an adult man and adult woman as published by the ICRP. Three perfusion CT acquisition protocols--brain, lung tumor, and liver perfusion--were evaluated. Additionally, dose assessments were performed for the skin and for the eye lens. Conversion factors were obtained to estimate effective doses and organ doses from the volume CT dose index and dose-length product., Results: The sex-averaged effective doses were approximately 4 mSv for perfusion CT of the brain and were between 23 and 26 mSv for the perfusion CT body protocols. The eye lens dose from the brain perfusion CT examination was approximately 153 mGy. The sex-averaged peak entrance skin dose (ESD) was 255 mGy for the brain perfusion CT studies, 157 mGy for the lung tumor perfusion CT studies, and 172 mGy for the liver perfusion CT studies., Conclusion: The perfusion CT protocols for imaging the brain, lung tumors, and the liver performed on a 320-MDCT scanner yielded patient doses that are safely below the threshold doses for deterministic effects. The eye lens dose, peak ESD, and effective doses can be estimated for other clinical perfusion CT examinations from the conversion factors that were derived in this study.

Objective: To quantify radiation exposure and mortality risk from computed tomography (CT) and positron emission tomography (PET) imaging with (18)F-fluorodeoxyglucose ((18)F-FDG) in patients with malignant lymphoma (Hodgkin's disease [HD] or non-Hodgkin's lymphoma [NHL])., Methods: First, organ doses were assessed for a typical diagnostic work-up in children with HD and adults with NHL. Subsequently, life tables were constructed for assessment of radiation risks, also taking into account the disease-related mortality., Results: In children with HD, cumulative effective dose from medical imaging ranged from 66 mSv (newborn) to 113 mSv (15 years old). In adults with NHL the cumulative effective dose from medical imaging was 97 mSv. Average fractions of radiation-induced deaths for children with HD [without correction for disease-related mortality in brackets] were 0.4% [0.6%] for boys and 0.7% [1.1%] for girls, and for adults with NHL 0.07% [0.28%] for men and 0.09% [0.37%] for women., Conclusion: Taking into account the disease-related reduction in life expectancy of patients with malignant lymphoma results in a higher overall mortality but substantial lower incidence of radiation induced deaths. The modest radiation risk that results from imaging with CT and (18)F-FDG PET can be considered as justified, but imaging should be performed with care, especially in children.

Computed tomography (CT) dosimetry should be adapted to the rapid developments in CT technology. Recently a 160 mm wide, 320 detector row, cone beam CT scanner that challenges the existing Computed Tomography Dose Index (CTDI) dosimetry paradigm was introduced. The purpose of this study was to assess dosimetric characteristics of this cone beam scanner, to study the appropriateness of existing CT dose metrics and to suggest a pragmatic approach for CT dosimetry for cone beam scanners. Dose measurements with a small Farmer-type ionization chamber and with 100 mm and 300 mm long pencil ionization chambers were performed free in air to characterize the cone beam. According to the most common dose metric in CT, namely CTDI, measurements were also performed in 150 mm and 350 mm long CT head and CT body dose phantoms with 100 mm and 300 mm long pencil ionization chambers, respectively. To explore effects that cannot be measured with ionization chambers, Monte Carlo (MC) simulations of the dose distribution in 150 mm, 350 mm and 700 mm long CT head and CT body phantoms were performed. To overcome inconsistencies in the definition of CTDI100 for the 160 mm wide cone beam CT scanner, doses were also expressed as the average absorbed dose within the pencil chamber (D100). Measurements free in air revealed excellent correspondence between CTDI300air and D100air, while CTDI100air substantially underestimates CTDI300air. Results of measurements in CT dose phantoms and corresponding MC simulations at centre and peripheral positions were weighted and revealed good agreement between CTDI300w, D100w and CTDI600w, while CTDI100w substantially underestimates CTDI300w. D100w provides a pragmatic metric for characterizing the dose of the 160 mm wide cone beam CT scanner. This quantity can be measured with the widely available 100 mm pencil ionization chamber within 150 mm long CT dose phantoms. CTDI300w measured in 350 mm long CT dose phantoms serves as an appropriate standard of reference for characterizing the dose of this CT scanner. A CT dose descriptor that is based on an integration length smaller than the actual beam width is preferably expressed as an (average) dose, such as D100 for the 160 mm wide cone beam CT scanner, and not as CTDI100.

Objective: This study aimed to assess patient dose and occupational dose in established and new applications of MDCT fluoroscopy., Materials and Methods: Electronic personal dosimeters were used to measure occupational dose equivalent. Effective patient dose was derived from the recorded dose-length product. Acquisition parameters that were observed during CT fluoroscopy (CTF) provided the basis for the estimation of an entrance skin dose profile. Two hundred ten CT-guided interventional procedures were included in the study., Results: The median effective patient dose was 10 mSv (range, 0.1-235 mSv; 107 procedures). The median peak entrance skin dose was 0.4 Sv (0.1-2.1 Sv; 27 procedures). From 547 measurements of occupational dose equivalent, a median occupational effective dose of 3 muSv per procedure was derived for the interventional radiologists and 0.4 muSv per procedure for the assisting radiologists and radiology technologists. The estimated maximum occupational effective dose reached 0.4 mSv., Conclusion: The study revealed high effective patient doses, up to 235 mSv, mainly for relatively new applications such as CTF-guided radiofrequency ablations using MDCT, vertebroplasty, and percutaneous ethanol injections of tumors. Entrance doses were occasionally in the range of the warning level for deterministic skin effects but were always below the threshold for serious deterministic effects. The complexity of the procedure, expected benefits of the treatment, and general health state of the patient contribute to the justification of observed high effective patient doses.

Patient dose is usually estimated for a single radiographic projection or computed tomography (CT) series. In this study, patient dose was calculated for predefined clinical indications (24 radiography, 11 CT). Members of the radiology staff of each of 11 hospitals were trained in dose measurement and calculation techniques. Based on clinical indications participants decided on imaging protocols and calculated cumulative effective dose for a complete examination. Effective dose ranged from <1 microSv to 0.6 mSv for examinations with radiographs and from 0.2 to 12 mSv for CT scans. Differences in the imaging protocols contributedd to a substantial variation in patient dose. For mammography, average glandular dose (AGD) was estimated for 32-, 53- and 90-mm compressed breast thicknesses, with a median value of 0.74, 1.74 and 3.40 mGy, respectively. The results presented here demonstrate that a pragmatic choice of dosimetry methods enables local staff to estimate effective dose. The inclusion of imaging protocols in the dose surveys provided a broader view on the variations in patient dose between hospitals.

Purpose: To quantify the number of overrange rotations and to assess their relative contribution to organ and effective doses at 16-section body computed tomography (CT)., Materials and Methods: Overranging was quantified for four 16-section scanners by means of free-in-air dose measurements at different scan lengths. Overrange rotations and lengths at a certain section width were derived for all collimations and clinically used pitches by extrapolation. The effect of reconstructed section width on overranging was analyzed separately. Results were applied to clinical protocols for the chest and abdomen. Thyroid and testicular dose and effective dose were established, and relative dose contributions from overranging were calculated. Statistical analysis was performed by using Pearson correlation and paired t tests. P<.05 indicated a significant difference., Results: The number of overrange rotations showed considerable differences between scanners, with a range of 1.99-4.04 at the lowest and 0.93-2.59 at the highest pitch. Number of rotations correlated negatively with pitch, while overrange length correlated positively with collimation and pitch. The effect of section width was variable. In the protocols, overrange length ranged from 3.2 to 5.8 cm for chest and from 3.2 to 5.2 cm for abdominal CT. When the contribution of overranging was not taken into account, significantly lower values for thyroid (P=.012) and testicular (P=.025) doses and effective doses for chest (P=.005) and abdominal (P=.011) CT resulted., Conclusion: Overranging is reconstruction-algorithm specific, and its length generally increases with collimation and pitch, while the effect of section width is variable. A wide variety of radiation-induced deterministic skin effects have been observed after X-ray guided interventions ranging from mild effects, such as transient erythema or temporary epilation, to severe effects, such as desquamation and necrosis. Radiation biologists have identified, in addition to absorbed dose to the skin, other factors that strongly influence the type and severity of a skin reaction, including exposure-related factors (dose rate, fractionation, the size of the exposed area and its site), biological factors (age, oxygen status, capillary density, hormonal status and genetic factors) and ethnic differences. A peak entrance skin dose of 2 Gy is an arbitrary, but pragmatic, threshold for radiation-induced skin effects after X-ray guided interventions. Transient skin injury originating in the epidermis is not expected in the average patient population at peak entrance skin doses up to 6 Gy. Serious skin effects are not likely to occur in clinical practice when optimised X-ray equipment is used in combination with good techniques for fluoroscopy and imaging. However, this might not be true for patients with biological factors that are associated with an increased sensitivity for radiation-induced skin reactions.

The purpose of this study was to determine the effects of various protective measures on patient and operator radiation dose levels in catheter ablation procedures. Catheter ablation procedures are associated with significant radiation levels. The patient's skin and operator radiation levels were measured (1) at baseline, (2) after primary beam filtration by 0.3-mm copper sheet and 2-mm aluminium plate and implementation of the LocaLisa system, and (3) after reduction of the left anterior oblique fluoroscopic pulse rate and installation of a lead glass screen. Additionally, a comparative analysis of radiation exposure levels was performed in the seven Dutch catheter ablation centers. Filtration of both primary beams resulted in a more than two-fold reduction in patient skin dose. Together with the LocaLisa system, this resulted in a six-fold reduction in patient and operator dose. As expected, lowering of the left anterior oblique pulse rate from 25 to 12.5 Hz reduced the corresponding patient skin dose with a factor 2 while the lead-glass protection caused an extra factor 2 reduction for the operator. Large differences were observed between fluoroscopy systems used for catheter ablation in the Netherlands. Depending on patient body mass and fluoroscopy system, patient skin dose varied between 0.2 and 8.4 Gy/hour. Proper measures may allow for a significant reduction of patient and operator radiation exposure in catheter ablation procedures. The large influence of body mass and equipment on patient's skin dose requires a more direct monitoring of skin dose than total fluoroscopy time.

The complex conditions of irradiation in computed tomography (CT), involving highly-collimated X-ray beams, necessitate the use of specially-defined dose descriptors such as the computed tomography dose index (CTDI). When used in a weighted form (CTDIW), this concept can describe the absorbed dose from a single slice in standard head and body phantoms. The model can easily be extended to characterise patient exposure for a complete examination by means of the reference dose quantity dose-length product (DLP). Effective dose can also be estimated from DLP, when required.

General ion recombination has been studied under irradiation conditions relevant for diagnostic radiology and with four different ionization chambers. When the exposure time is appreciably shorter than the ion transit time, the exposure can be designated as pulsed irradiation. On the contrary, for relatively long irradiation times, the term continuous irradiation can be applied. Recombination was estimated by measuring the collected charge at various collecting potentials of the ionization chamber. This is a well-known method in radiotherapy, but unfortunately it cannot be used in diagnostic radiology with typical exposure meters, since they do not offer the option of varying the collecting potential. For exposures with diagnostic x-ray units, an alternative approach is to vary the exposure or exposure rate over a wide range at a constant collecting potential. Experimental and theoretical estimates of ion recombination did not yield similar values. This might be due to several causes, such as differences between the actual and the nominal dimensions and volumes of the ionization chambers, due to errors and uncertainties in the physical parameters used in the theoretical models or due to deviations of the shape of the ionization chambers from the perfect cylindrical or parallel plate geometry. For better accuracy, corrections for recombination losses should therefore be based on experimental verification rather than on theoretical models.
