Following the publication of the joint The International Commissions on Radiation Units and Measurements (ICRU) and on Radiological Protection (ICRP) report on new operational quantities for radiation protection, the European Dosimetry Group (EURADOS) have carried out an initial evaluation. The EURADOS report analyses the impact that the new quantities will have on: radiation protection practice; calibration and reference fields; European and national regulation; international standards and, especially, dosemeter and instrument design. Occupational radiation doses from interventional procedures have the potential to be relatively high. The requirement to optimise these doses encourages the use of electronic or active personal dosimeters (APDs) which are now increasingly used in hospitals. They are typically used in tandem with a routine passive dosimetry monitoring programme, with APDs used for real-time readings, for training purposes and when new imaging technology is introduced. However, there are limitations when using APDs. A survey in hospitals to identify issues related to the use of APDs was recently completed, along with an extensive series of APD tests by the EURADOS Working Group 12 on Dosimetry for Medical Imaging. The aim of this review paper is to summarise the state of the art regarding the use of APDs. We also used the results of our survey and our tests to develop a set of recommendations for the use of APDs in the clinical interventional radiology/cardiology settings, and draw attention to some of the current challenges., (Copyright © 2021 Associazione Italiana di Fisica Medica. Since 2012, the European Radiation Dosimetry Group (EURADOS) has developed its Strategic Research Agenda (SRA), which contributes to the identification of future research needs in radiation dosimetry in Europe. Continued scientific developments in this field necessitate regular updates and, consequently, this paper summarises the latest revision of the SRA, with input regarding the state of the art and vision for the future contributed by EURADOS Working Groups and through a stakeholder workshop. Five visions define key issues in dosimetry research that are considered important over at least the next decade. They include scientific objectives and developments in (i) updated fundamental dose concepts and quantities, (ii) improved radiation risk estimates deduced from epidemiological cohorts, (iii) efficient dose assessment for radiological emergencies, (iv) integrated personalised dosimetry in medical applications and (v) improved radiation protection of workers and the public. This SRA will be used as a guideline for future activities of EURADOS Working Groups but can also be used as guidance for research in radiation dosimetry by the wider community. It will also be used as input for a general European research roadmap for radiation protection, following similar previous contributions to the European Joint Programme for the Integration of Radiation Protection Research, under the Horizon 2020 programme (CONCERT). Radiation overexposure accidents are rare but can have severe health consequences. Evaluating the dose received by the patient is a crucial step in the medical management. For that purpose, for more than 15 years, IRSN has been developing an in-house tool named SESAME for the numerical reconstruction of radiological accidents due to external sources. Recently, two new functionalities were implemented in SESAME to allow accurate reconstructions of interventional radiology (IR) overexposures. The experimental validation of SESAME for the reconstruction of overexposures in IR is presented. First, an anthropomorphic dummy equipped with dosemeters was irradiated following conditions similar to a fluoroscopically guided interventional procedure. Then the procedure was simulated using SESAME. Finally measured doses were compared to calculated doses. Even with a limited amount of data available, SESAME can provide valuable dose information for the medical team in charge of the patient, such as skin dose mapping and dose distribution in depth., (© The Author(s) 2019. In the context of a new annual eye lens dose limit for occupational exposure equal to 20 mSv, European Radiation Dosimetry Group (EURADOS) organized an intercomparison dedicated to eye lens dosemeters, including photon and beta radiations. The objective was to complete the first intercomparison recently organized by EURADOS for photons and to update the overview of eye lens dosemeters available in Europe. The dosemeters provided by the 22 participants coming from 12 countries were all composed of thermoluminescent detectors. The dosemeters were irradiated with photon and beta fields defined in relevant standards. The results, provided by participants in terms of Hp(3), were compared to the reference delivered doses. Results are globally satisfactory for photons since 90% of the data are in accordance to the ISO 14146 standard requirements. The respective values for betas stress the fact that dosemeters designed for Hp(0.07) are not suitable to monitor the eye lens dose in case of betas.

In the context of the decrease in the eye lens dose limit for occupational exposure to 20 mSv per year stated by the recent revision of the European Basic Safety Standards Directive 2013/59/EURATOM, the European Radiation Dosimetry Group (EURADOS) has organised in 2014, for the first time, an intercomparison exercise for eye lens dosemeters. The main objective was to assess the capabilities of the passive eye lens dosemeters currently in use in Europe for occupational monitoring in medical fields. A total of 20 European individual monitoring services from 15 different countries have participated. The dosemeters provided by the participants were all composed of thermoluminescent detectors, of various types and designs. The irradiations were carried out with several photon fields chosen to cover the energy and angle ranges encountered in medical workplace. Participants were asked to report the doses in terms of Hp(3) using their routine protocol. The results provided by each participant were compared with the reference delivered doses. All the results were anonymously analysed. Results are globally satisfactory since, among the 20 participants, 17 were able to provide 90 % of their response in accordance with the ISO 14146 standard requirements., (© The Author 2015. The European Radiation Dosimetry Group (EURADOS) has been organising dosimetry intercomparisons for many years in response to an identified requirement from individual monitoring services (IMS) for independent performance tests for dosimetry systems. The participation in intercomparisons gives IMS the opportunity to show compliance with their own quality management system, compare results with other participants and develop plans for improving their dosimetry systems. In response to growing demand, EURADOS has increased the number of intercomparisons for external radiation dosimetry. Most of these fit into the programme of self-financing intercomparisons for dosemeters routinely used by IMS. This programme is being coordinated by EURADOS working group 2 (WG2). Up to now, this programme has included four intercomparisons for whole-body dosemeters in photon fields, one for extremity dosemeters in photon and beta fields, and one for whole-body dosemeters in neutron fields. Other EURADOS working groups have organised additional intercomparisons including events in 2014 for eye-lens dosemeters and passive area dosemeters for environmental monitoring. The work package 3 of the ORAMED project, Collaborative Project (2008-11) supported by the European Commission within its seventh Framework Programme, is focused on the optimisation of the use of active personal dosemeters (APDs) in interventional radiology and cardiology (IR/IC). Indeed, a lack of appropriate APD devices is identified for these specific fields. Few devices can detect low-energy X rays (20-100 keV), and none of them are specifically designed for working in pulsed radiation fields. The work presented in this paper consists in studying the behaviour of some selected APDs deemed suitable for application in IR/IC. For this purpose, measurements under laboratory conditions, both with continuous and pulsed X-ray beams, and tests in real conditions on site in different European hospitals were performed. This study highlights the limitations of APDs for this application and the need of improving the APD technology so as to fulfil all needs in the IR/IC field.

The Laboratory of Ionizing Radiation Dosimetry of the IRSN (France) is developing an instrumented anthropomorphic phantom in order to measure the effective dose for photon fields at workplaces. This anthropomorphic phantom will be equipped with small active detectors located inside at chosen positions. The aim of this paper is to present the development of these new detectors showing the results of the characterisation of the prototype under metrological conditions. New evaluations of the effective dose for standard and non-homogenous irradiation configurations taking into account the real constraints of the project have been done validating the feasibility and utility of the instrument.

This paper describes the results of the simulation of a radiophotoluminescent (RPL) dosemeter with the Monte Carlo transport code MCNPX. The aim of this study is to calculate the response with MCNPX of the RPL dosemeter in terms of equivalent doses H(p) (0.07) and H(p)(10) using X-ray photon radiation qualities N series, together with S-Cs and S-Co nuclide radiation qualities, specified in ISO 4037-1. After comparison with reference values versus experimental results, the deviation of the theoretical responses of the RPL dosemeter proved to be lower than 5 % for reference values and lower than 10 % for experimental results. This good correlation validates the model over the energy range studied.

From feedback experience from recent radiation accident cases, in addition to biological dosimetry and physical dosimetry based on Monte Carlo calculations or experimental means, there is a need for complementary methods of dosimetry for radiation accident. Electron paramagnetic resonance (EPR) spectrometry on bones or teeth is considered as efficient but is limited by the invasive character of the sampling. Since 2005, Institute for Radiological Protection and Nuclear Safety (IRSN) develops some new approaches and methodologies based on the EPR and luminescence techniques. This article presents the overview of the different studies currently in progress in IRSN.

The use of active personal dosemeters (APD) in interventional radiology was evaluated by Working Group 9 (Radiation protection dosimetry of medical staff) of the CONRAD project, which is a Coordination Action supported by the European Commission within its sixth Framework Programme. Interventional radiology procedures can be very complex and they can lead to relatively high doses to personnel who stand close to the primary radiation field and are mostly exposed to radiation scattered by the patient. For the adequate dosimetry of the scattered photons, APDs must be able to respond to low-energy [10-100 keV] and pulsed radiation with relatively high instantaneous dose rates. An intercomparison of five APD models deemed suitable for application in interventional radiology was organised in March 2007. The intercomparison used pulsed and continuous radiation beams, at CEA-LIST (Saclay, France) and IRSN (Fontenay-aux-Roses, France), respectively. A specific configuration, close to the clinical practice, was considered. The reference dose, in terms of Hp(10), was derived from air kerma measurements and from the measured and calculated energy distributions of the scattered radiation field. Additional Monte Carlo calculations were performed to investigate the energy spectra for different experimental conditions of the intercomparison. The results of this intercomparison are presented in this work and indicate which APDs are able to provide a correct response when used in the specific low-energy spectra and dose rates of pulsed X-rays encountered in interventional radiology.

An overview of the use of active personal dosemeters (APD) in interventional radiology is presented. It is based on the work done by the working package 7 of the CONRAD coordinated action supported by the EC within the frame of the 6th FP. This study was done in collaboration with the working package 4 of CONRAD to deal with the calculations required for studying the new calibration facility. The main requirements of the standard for the APD and the difficulties caused by the use of pulsed radiations are presented through the results of an intercomparison organised in a realistic calibration facility similar to the workplace situation in interventional radiology. The main characteristics of this facility are presented.

In the case of a radiological accident due to external exposure, the assessment of the organs and whole body dose received by the victim is fundamental information for therapeutic strategy. Two complementary dosimetric techniques based on physical means are used in routine practice in the laboratory: Monte Carlo (MC) calculations and ESR spectroscopy performed on materials removed from the victim. The aim of the present study is to provide data relevant for a quick assessment of the dose distribution in case of accidental overexposure using dose ESR measurements performed on one or several points of the body. The chosen configurations were, on the one hand, standard homogeneous exposures (ICPR 74) and, on the other hand, exposures typical of accidental situations (source at 1m, in a pocket, in a hand and contaminated floor). The study was performed for monoenergetic photons and neutrons, and several sources (60Co, 137Cs, 192Ir, 252Cf and AmBe).

The study of naturally occurring radiation and its associated risk is one of the preoccupations of bodies responsible for radiation protection. Cosmic particle flux is significantly higher on-board the aircraft that at ground level. Furthermore, its intensity depends on solar activity and eruptions. Due to their professional activity, flight crews and frequent flyers may receive an annual dose of some millisieverts. This is why the European directive adopted in 1996 requires the aircraft operators to assess the dose and to inform their flight crews about the risk. The effective dose is to be estimated using various experimental and calculation means. In France, the computerised system for flight assessment of exposure to cosmic radiation in air transport (SIEVERT) is delivered to airlines for assisting them in the application of the European directive. This professional service is available on an Internet server accessible to companies with a public section. The system provides doses that consider the routes flown by aircraft. Various results obtained are presented.

The paper presents the OEDIPE (French acronym that stands for tool for personalised internal dose assessment) and SESAME (for simulation of external source accident with medical images) computational tools, dedicated to internal and external dose assessment, respectively, and currently being developed at the Institute for Radiological Protection and Nuclear Safety. The originality of OEDIPE and SESAME, by using voxel phantoms in association with Monte Carlo codes, lies in their ability to construct personalised voxel phantoms from medical images and automatically generate the Monte Carlo input file and visualise the expected results. OEDIPE simulates in vivo measurements to improve their calibration, and calculates the dose distribution taking both internal contamination and internal radiotherapy cases into account. SESAME enables radiological overexposure doses to be reconstructed, as also victim, source and accident environment modelling. The paper presents the principles on which these tools function and an overview of specificities and results linked to their fields of application.
