Abstract Background The coronavirus 2019 (COVID-19) pandemic impacted cancer health care in several countries, with delays in the detection and treatment of breast and cervical cancer. The objective of this study is to analyze and compare the screening, diagnosis and treatment of breast and cervical cancer in the pre-COVID period and during the COVID-19 period. Methods Cross-sectional study with secondary data collected from the Mortality Information System (SIM), Hospital Information System (SIH), Ambulatory Information System (SIA) and the Oncology Panel (PO) of breast cancer notifications with ICD C50.0 to C50.9 and cervix ICD C53.0 to C53.9, The analyzed period before the pandemic was from March 1 to October 1, 2019, and during the pandemic from March 1 to October 1, 2020. The period from 2013 to 2022 was also analyzed with the same information, including the number of diagnoses, treatments, and deaths from breast cancer and cervical cancer. The study population consisted of Brazilian women aged 25 to 70 years. In order to compare categorical variables between periods, the Chi-Square or Fisher’s Exact tests, and Mann-Whitney U tests were applied, and the Poisson Regression model was applied to model the number of reported cases of COVID-19 and the amount of procedures. Results There was a decrease in the number of mammograms and cytopathological exams during COVID-19, as well as a decrease in cases of breast and cervical cancer. The Poisson regression showed that the increase in the number of COVID-19 cases caused a decrease in the number of breast cytopathological examinations, cervical-vaginal cytopathological examinations/microflora and screening, diagnosis, initiation of treatment for breast cancer and deaths from this disease. Meanwhile, in some regions of Brazil, as the number of Covid-19 increased, there was a significantly increase in the number of mammograms performed and cervical cancer diagnoses. Conclusions The COVID-19 period in 2020 significantly impacted screening, diagnosis, treatment for breast and cervical cancer.

Objetivo. Apresentar a experiência de um serviço de resposta rápida para apoiar a tomada de decisão em saúde. Método São descritos os processos e resultados de um serviço de produção de revisões rápidas e mapas de evidências para apoiar a tomada de decisão no âmbito da Política Nacional de Promoção da Saúde, bem como a percepção dos autores sobre o processo de trabalho. Resultados O serviço de resposta rápida teve início em 2020. Até dezembro de 2023, foram produzidas 54 revisões rápidas e cinco mapas de evidências, abrangendo nove temáticas de Promoção da Saúde. Estes produtos foram desenvolvidos em quatorze etapas por uma equipe composta por coordenador, supervisoras, revisores e bibliotecária. O desenvolvimento das respostas rápidas se configurou um processo de tradução do conhecimento e envolveu a interação contínua entre as equipes demandantes e de produção. O estabelecimento de comunicação efetiva foi um fator crítico para que os produtos fossem entregues em tempo oportuno e alinhados às necessidades dos tomadores de decisão e seus apoiadores. Conclusão Os serviços de resposta rápida podem contribuir para melhorar o uso de evidências na tomada de decisão nas políticas e sistemas de saúde.

Objetivo. Descrever o padrão temporal e espacial e identificar os fatores associados a incidência de HIV/AIDS entre jovens no Brasil. Método. Estudo ecológico que incluiu jovens brasileiros de 15 a 24 anos notificados com HIV/AIDS de 2001 a 2021. Utilizou-se o método joinpoint para a análise temporal. Aglomerados espaciais foram detectados pelos métodos Bayesiano, autocorrelação espacial, Getis-Ord Gi* e Varredura Scan. Quatro modelos de regressão não espacial e espacial foram usados para identificar fatores associados ao desfecho. Todas as análises estatísticas consideraram p < 0,05. Resultados. No Brasil, a incidência média foi de 12,29 por 100 000 habitantes, com aumento de 7,3% ao ano no período 2007-2014 e posterior decréscimo de 3,4% em 2014-2021. Observou-se padrão Alto/Alto e hotspots, principalmente em municípios do Sul, Sudeste, Centro-Oeste e Norte. O cluster primário localizou-se em 572 municípios do Rio Grande do Sul e Santa Catarina e os maiores riscos relativos em Manaus (Amazonas) e Rondonópolis (Mato Grosso). A taxa de analfabetismo (β = -0,08), Índice de GINI (β = -3,74) e Cobertura da Estratégia de Saúde da Família (β = -0,70) apresentaram relação negativa com o desfecho. Em contrapartida, o Índice Firjan de Desenvolvimento Municipal (β = 2,37), o Índice de Vulnerabilidade Social (β = 6,30), o percentual de pessoas que recebem o Bolsa Família (β = 0,04) e renda per capita (β = 0,008) apresentaram associação positiva. Conclusão. Houve tendência de aumento da incidência de HIV/AIDS até 2014 com posterior declínio até 2021. Aglomerados de altas taxas concentraram-se, especialmente, em municípios das regiões Norte, Sul, Sudeste e Centro-Oeste. Indicadores de vulnerabilidade socioeconômica influenciam o desfecho positivamente ou negativamente, dependendo do território investigado.

American tegumentary leishmaniasis (ATL) is highly endemic in the Amazon basin and occurs in all South American countries, except Chile and Uruguay. Most Brazilian ATL cases are due to Leishmania (Viannia) braziliensis, however other neglected Amazonian species are being increasingly reported. They belong to the subgenus L. (Viannia) and information on suitable models to understand immunopathology are scarce. Here, we explored the use of the golden hamster Mesocricetus auratus and its macrophages as a model for L. (Viannia) species. We also studied the interaction of parasite glycoconjugates (LPGs and GIPLs) in murine macrophages. The following strains were used: L. (V.) braziliensis (MHOM/BR/2001/BA788), L. (V.) guyanensis (MHOM/BR/85/M9945), L. (V.) shawi (MHOM/BR/96/M15789), L. (V.) lindenbergi (MHOM/BR/98/M15733) and L. (V.) naiffi (MDAS/BR/79/M5533). In vivo infections were initiated by injecting parasites into the footpad and were followed up at 20- and 40-days PI. Parasites were mixed with salivary gland extract (SGE) from wild-captured Nyssomyia neivai prior to in vivo infections. Animals were euthanized for histopathological evaluation of the footpads, spleen, and liver. The parasite burden was evaluated in the skin and draining lymph nodes. In vitro infections used resident peritoneal macrophages and THP-1 monocytes infected with all species using a MOI (1:10). For biochemical studies, glycoconjugates (LPGs and GIPLs) were extracted, purified, and biochemically characterized using fluorophore-assisted carbohydrate electrophoresis (FACE). They were functionally evaluated after incubation with macrophages from C57BL/6 mice and knockouts (TLR2-/- and TLR4-/-) for nitric oxide (NO) and cytokine/chemokine production. All species, except L. (V.) guyanensis, failed to generate evident macroscopic lesions 40 days PI. The L. (V.) guyanensis lesions were swollen but did not ulcerate and microscopically were characterized by an intense inflammatory exudate. Despite the fact the other species did not produce visible skin lesions there was no or mild pro-inflammatory infiltration at the inoculation site and parasites survived in the hamster skin/lymph nodes and even visceralized. Although none of the species caused severe disease in the hamster, they differentially infected peritoneal macrophages in vitro. LPGs and GIPLs were able to differentially trigger NO and cytokine production via TLR2/TLR4 and TLR4, respectively. The presence of a sidechain in L. (V.) lainsoni LPG (type II) may be responsible for its higher proinflammatory activity. After Principal Component analyses using all phenotypic features, the clustering of L. (V.) lainsoni was separated from all the other L. (Viannia) species. We conclude that M. auratus was a suitable in vivo model for at least four dermotropic L. (Viannia) species. However, in vitro studies using peritoneal cells are a suitable alternative for understanding interactions of the six L. (Viannia) species used here. LRV1 presence was found in L. (V.) guyanensis and L. (V.) shawi with no apparent correlation with virulence in vitro and in vivo. Finally, parasite glycoconjugates were able to functionally trigger various innate immune responses in murine macrophages via TLRs consistent with their inflammatory profile in vivo.

Introdução: A lombalgia é uma condição prevalente e que apresenta importante impacto na capacidade funcional e na qualidade de vida, sendo a sua correta abordagem na Atenção Primária à Saúde fundamental para a identificação e o estabelecimento de um diagnóstico etiológico precoce de possíveis patologias que possam estar relacionadas a desfechos mórbidos e a graves limitações funcionais. Apresentação do caso: Paciente de 56 anos, sexo masculino, hipertenso, foi encaminhado para serviço especializado de reumatologia com histórico de lombalgia havia mais de 20 anos. Ao exame físico foi constatada presença de deformidades da coluna vertebral e extensa limitação de movimentos. Exames radiográficos mostravam esclerose de articulações sacroilíacas, osteopenia difusa e coluna vertebral em aspecto de “bambu”. Conclusões: Constata-se a importância de que na abordagem das lombalgias na atenção primária se busque o reconhecimento de possíveis etiologias graves e potencialmente incapacitantes que possam estar subjacentes à queixa de dor lombar. Com esse objetivo, é fundamental o reconhecimento das chamadas red flags relacionadas às lombalgias, além de sua caracterização como mecânica ou inflamatória. Perante a atuação da atenção primária no oferecimento de um cuidado pautado na integralidade e na prevenção de agravos, reafirma-se a importância de uma avaliação clínica pormenorizada das lombalgias nesse nível de atenção à saúde.

Abstract Background Cholestatic pruritus and fatigue are debilitating conditions associated with primary biliary cholangitis (PBC) and can significantly impact patients’ quality of life. Pruritus in PBC often worsens at night and patients frequently report sleep disturbance, which contributes to cognitive symptoms and fatigue. Linerixibat is an ileal bile acid transporter inhibitor in clinical development for the treatment of pruritus associated with PBC and was recently assessed versus placebo in the Phase 2b GLIMMER trial. This post-hoc analysis assesses the relationship between pruritus severity and sleep disturbance in participants of GLIMMER regardless of treatment group. Methods GLIMMER (NCT02966834), a multicenter, double-blind, randomized, placebo-controlled trial, recruited 147 patients with PBC and moderate-to-severe pruritus. Following 4 weeks single-blind placebo, patients (randomized 3:1) received linerixibat or placebo for 12 weeks (to Week 16). Participants graded their itch (twice daily) and its interference with sleep (once daily) in an electronic diary using a 0–10 numerical rating scale (NRS). Weekly and monthly itch scores were calculated as the mean of the worst daily itch score over the respective time period. At study visits, participants completed the 5-D itch scale and the PBC-40 quality of life questionnaire, both of which contain an item specific to itch-related sleep disturbance. The impact of pruritus on sleep was assessed post hoc through correlations between the changes in NRS, 5-D itch, and PBC-40. Results Strong correlations were found between change from baseline in weekly itch and sleep NRS scores (r = 0.88 [95% confidence interval (CI): 0.83; 0.91]) at the end of treatment (Week 16), as well as in monthly itch and sleep NRS scores (r = 0.84 [95% CI: 0.80; 0.87]). Patients with improved weekly pruritus score severity category demonstrated reduced perceived sleep interference on average. Itch responders (≥2-point improvement in weekly itch score from baseline) displayed larger improvements in weekly sleep NRS score, 5-D itch, and PBC-40 sleep items, than itch non-responders (

Schistosoma mansoni worms cause a waterborne parasitic disease called schistosomiasis. It commonly affects individuals in lack of sanitation structure. In Brazil, Pará state has Belém as one of the worst sanitation-ranking places in 2023, where schistosomiasis transmission was already documented. This study reports the occurrence of schistosomiasis in residents of Ilha das Onças, an island next to Belém. Stool samples were obtained from participants over 2 years old, all residents from Furo do Rio Grande, one of the rivers on the island. The Kato-Katz technique was performed for parasite investigation in the stool samples. Each participant responded to a sociodemographic and clinical questionnaire. The residences were georeferenced for map designing. Three out of 263 participants were S. mansoni positive, all men, ages ranging from 19 to 41 years old, with low parasitic load. Malacological surveys were carried out, but no Biomphalaria snails were found. Risk factors for schistosomiasis establishment are present on the island, and the lack of sanitation makes it a potential risk area. Malacological surveys are highly encouraged as preventive measures, as well as health surveillance for riverside populations, generating data that will help health authorities in the management and planning of preventive control actions. HIGHLIGHTS The study revealed the presence of Schistosoma mansoni infection in residents of Ilha das Onças.; The area had no previous reports of the disease.; Only male individuals tested positive for the infection.; No Biomphalaria snails were found in the peridomestic area of infected individuals.; Environmental conditions might enable schistosomiasis establishment on the island.;

Abstract Diabetes poses a substantial disease burden, prompting preventive interventions. Physical inactivity, a major risk factor for type 2 diabetes, can potentially be mitigated by enhancing area-level walkability. Despite this, limited population-based studies have investigated the link between walkability and objective diabetes measures. Our study aims to estimate the association between area-level walkability and individual glycated haemoglobin levels in the Portuguese adult population without the diagnosis of diabetes. Data from the 2011 census and an updated street map were obtained to construct a walkability index based on residential density, land-use mix, and street connectivity. Individual health data were sourced from The National Health Examination Survey (INSEF) 2015, a representative survey of the Portuguese adult population. Gamma regression was employed for estimation of the main associations, revealing that residing in moderately walkable areas significantly reduced average glycated haemoglobin levels (Exp(β) = 0.906; 95% CI: 0.821, 0.999) compared to the least walkable areas. The association was less pronounced and not statistically significant for the third tertile of walkability (Exp(β) = 0.919; 95% CI: 0.822, 1.028). Our findings highlight a nonlinear protective association between walkability and glycated haemoglobin, emphasizing the potential policy implications for urban planning, diabetes prevention, and health promotion.

Abstract Background Men who have sex with Men (MSM) are known to contribute to increased HIV prevalence as an integral part of key populations with high vulnerability to HIV/AIDS due to their sexual behaviours. Mozambique conducted two rounds of bio-behavioral surveys (BBS) in this population with the main objective of estimating HIV prevalence and associated risk behaviors among MSM in Mozambique. The present study aims to estimate the trend of HIV prevalence and determine the correlations of HIV infection among MSM. Methods A repeated cross-sectional analytical study was conducted from secondary data from the first and second rounds of BBS in Mozambique conducted in 2011 and 2020–2021 respectively. Each round used a similar methodology which allowed for comparison between the two surveys. Chi-square test and logistic regression was used to compare the HIV prevalence between the BBS rounds, identify factors associated with HIV, and assess changes in HIV prevalence across selected risk factors. Results There was a significant increase in HIV prevalence among MSM (7.1–14.9%), living in Maputo (9.3–14.7%), uncircumcised (11.7–25.1%), and those who reported two sexual partners in the last year (5.2–14.4%). In contrast, there was a decrease in prevalence among adult MSM aged between 25 and 29 years (24.7–13.9%), aged 30 years or older (45.7–27.7%), married (29.1–16.8%), with higher education (16.7–5.9%) and moderate perception of HIV risk (10.9–3.4%). Multivariable analysis showed that factors such as age, marital status, religion, sexual identity, circumcision, and the use of lubrication during anal sex are significantly associated with the risk of HIV infection. Conclusions This study underscores the continuing need for HIV prevention and education efforts. The rise in prevalence among specific population segments and the sustained presence of risk factors emphasize the requirement for holistic strategies tailored to the unique requirements of each subgroup. Understanding trends and risk factors is crucial to guiding public health policies and designing efficacious prevention programs that aim to curtail HIV transmission while enhancing the well-being of those impacted by the condition.

Background: Infective endocarditis continues to be a significant concern and may be undergoing an epidemiological transition. Methods: Were studied 1804 consecutive episodes of infective endocarditis between 1978 and 2022. The mean age was 48 ( ± 19), and 1162 (64%) patients were male. Temporal trends in demographic data, comorbidities, predisposing conditions, microorganisms, complications and in-hospital death have been studied over the decades (1978–1988, 1989–1999, 2000–2010 and 2011–2022). The outcomes and clinical characteristics were modeled using nonlinear cubic spline functions. Findings: Valve surgery was performed in 50% of the patients and overall in-hospital mortality was 30%. O artigo tem como objetivo refletir sobre os efeitos da violência armada sobre a saúde mental da população moradora e trabalhadora de territórios de favela, tendo como foco o estado do Rio de Janeiro, Brasil. Trata-se de um estudo teórico e crítico, com subsídio de pesquisas empíricas desenvolvidas pelas autoras a partir de uma abordagem pós-colonial, tomando como elemento central o fenômeno do racismo. Discute-se a violência armada no Rio de Janeiro, seus impactos sobre a saúde mental e algumas ações de seu enfrentamento, identificando como territórios favelizados e sua população, em sua maioria negra, estão no centro do problema, sofrendo efeitos que matam e fazem adoecer. Ainda que insuficientes, ações de enfrentamento angariadas pelo setor público e pela sociedade civil buscam dar visibilidade ao problema e ser formas de denúncia e resistência quanto aos efeitos deletérios da violência armada na saúde.

IntroductionDengue is a public health challenge worldwide. Brazil registered about 70% of cases in Latin America in 2023; in 2024, the country is experiencing an unprecedented increase in the number of infected individuals. By May 2024, more than 4 million people were infected. Our goal was to: (1) determine the epidemiology of dengue cases and their spatiotemporal distribution and (2) carry out a survey of the storm drains and through a geospatial analysis to determine their possible correlation with cases of dengue in Presidente Prudente, São Paulo, Brazil.MethodsCases and information on the habitat of mosquito in the storm drain underground drainage system from 2020 to 2021 were obtained from public agencies. Larvae, pupae, and Ae. aegypti were identified according to species and described in taxonomic keys. Kernel density maps were constructed.ResultsFrom 1996 to 2023, the prevalence of cases peaked in 2016 and 2019, and in 2023 reached alarming levels, and the city was considered hyperendemic. In 2021, 2,609 cases were registered with 2 clusters of high density. Of 5,492 storm drains analyzed, 18.0% were found to have water, 9.0% had larvae or pupae of Aedes aegypti and 91.0% were classified as dirty or damaged. A direct correlation between the kernel layer of cases in 2021 with the kernel layer of storm drains containing water (r = 0.651) and larvae and pupae (r = 0.576) was found, suggesting that storm drains are risk factors and have an impact on the maintenance of dengue endemicity. The high number of damaged units found demonstrated the lack of storm drain management, compromising the urban drainage system and possibly contributing to dengue outbreaks.ConclusionPolicymakers may use these findings to improve existing dengue control strategies focusing on the control of storm drains and increase local and global perspectives on reducing dengue outbreaks.

Summary: Background: Ethno-racial inequalities are critical determinants of health outcomes. We quantified ethnic-racial inequalities on adverse birth outcomes and early neonatal mortality in Brazil. Methods: We conducted a cohort study in Brazil using administrative linked data between 2012 and 2019. Estimated the attributable fractions for the entire population (PAF) and specific groups (AF), as the proportion of each adverse outcome that would have been avoided if all women had the same baseline conditions as White women, both unadjusted and adjusted for socioeconomics and maternal risk factors. AF was also calculated by comparing women from each maternal race/skin colour group in different groups of mothers’ schooling, with White women with 8 or more years of education as the reference group and by year. Findings: 21,261,936 newborns were studied. If all women experienced the same rate as White women, 1.7% of preterm births, 7.2% of low birth weight (LBW), 10.8% of small for gestational age (SGA) and 11.8% of early neonatal deaths would have been prevented. Percentages preventable were higher among Indigenous (22.2% of preterm births, 17.9% of LBW, 20.5% of SGA and 19.6% of early neonatal deaths) and Black women (6% of preterm births, 21.4% of LBW, 22.8% of SGA births and 20.1% of early neonatal deaths). AF was higher in groups with fewer years of education among Indigenous, Black and Parda for all outcomes. AF increased over time, especially among Indigenous populations. Interpretation: A considerable portion of adverse birth outcomes and neonatal deaths could be avoided if ethnic-racial inequalities were non-existent in Brazil. Resumo: Este ensaio traz uma reflexão teórica sobre os desafios para alcançar as metas dos Objetivos do Desenvolvimento Sustentável da Agenda 2030, considerando seu lema de “não deixar ninguém para trás”. Para exemplificar esses desafios, apresenta-se como pano de fundo as doenças crônicas não transmissíveis (DCNT), um dos principais temas da agenda da saúde global antes da pandemia de COVID-19, discutindo as dimensões políticas e econômicas que determinam sua presença e avanço global. Após um breve panorama sobre as DCNT, busca-se responder a três perguntas: em “Sem deixar ninguém para trás?”, elencamos alguns temas para refletir sobre como e quem tem ficado historicamente para trás, aprofundando um pouco mais os exemplos ao adentrar em “Quem tem ficado para trás no mundo?” e “Quem tem ficado para trás no Brasil?”. A partir de dados da literatura mais relevante e recente sobre o tema, apresentamos os desafios e alguns caminhos para não deixar ninguém para trás em um mundo em que o modo de produção tem historicamente vulnerabilizado alguns grupos sociais, com destaque para a população negra e a população indígena. Trazemos nas considerações finais a inspiração do ideograma Sankofa para lembrar que as respostas para o desenvolvimento sustentável que buscamos podem estar em algum lugar de nosso passado mais originário e tradicional, e que é preciso apostar na construção de novos caminhos a partir de outras epistemologias e cosmovisões presentes do outro lado da linha abissal.

Abstract: To analyze the temporal trend of the late maternal mortality ratio (LMMR) in Brazil and its geographic regions in the period from 2010 to 2019, an ecological time series study was conducted. Data related to late maternal mortality from information systems of the Brazilian Ministry of Health were used. Statistical analysis used Prais-Winsten autoregressive models. A total of 1,470 late maternal deaths were reported in Brazil, resulting in an LMMR of 5 deaths per 100,000 live births. The late maternal mortality records revealed regional disparities, with the lowest index in the North (3.5/100,000 live births) and the highest in the South (8.3/100,000 live births). The LMMR showed an increasing trend in the country, with a general increase in the LMMR in the period and a mean annual percentage variation of 9.79% (95%CI: 4.32; 15.54). The Central-West region led this increase, with a mean annual percentage change of 26.06% (95%CI: 16.36; 36.56), followed by the North and Northeast regions, with 23.5% (95%CI: 13.93; 33.88). About 83% of the reported late maternal deaths were investigated, and 65.6% were corrected by the Maternal Mortality Committees. These findings highlight the relevance of late maternal mortality as an important indicator for maternal health, which is often invisible. The increase in the LMMR result from the improvement in the quality of the registration of these deaths in recent years in Brazil, and especially from the work of investigating deaths. The fragility of reporting with regional disparities points to the need for a more comprehensive approach that promotes equity and prevention of avoidable late maternal mortality.

ABSTRACT Objective To describe vaccination coverage and hesitation for the basic children’s schedule in Belo Horizonte and Sete Lagoas, Minas Gerais state, Brazil. Methods Population-based epidemiological surveys performed from 2020 to 2022, which estimated vaccine coverage by type of immunobiological product and full schedule (valid and ministered doses), according to socioeconomic strata; and reasons for vaccination hesitancy. Results Overall coverage with valid doses and vaccination hesitancy for at least one vaccine were, respectively, 50.2% (95%CI 44.1;56.2) and 1.6% (95%CI 0.9;2.7), in Belo Horizonte (n = 1,866), and 64.9% (95%CI 56.9;72.1) and 1.0% (95%CI 0.3;2.8), in Sete Lagoas (n = 451), with differences between socioeconomic strata. Fear of severe reactions was the main reason for vaccination hesitancy. Conclusion Coverage was identified as being below recommended levels for most vaccines. Disinformation should be combated in order to avoid vaccination hesitancy. There is a pressing need to recover coverages, considering public health service access and socioeconomic disparities.

ABSTRACT Objective To describe timely vaccination completion and obstacles in the first 24 months of life in Brazil, examining associations with maternal race/skin color. Methods Study participants were 37,801 children born in 2017 and 2018 included in the National Immunization Coverage Survey. We calculated prevalence and 95% confidence intervals for timely vaccine completeness and obstacles at 5, 12 and 24 months of life, according to maternal race/skin color. Associations were analyzed using logistic regression. Results 7.2% (95%CI 6.3;8.2) of mothers faced difficulties in taking their children to be vaccinated, and 23.4% (95%CI 21.7;25.1) were not vaccinated when taken. These proportions were 75% (95%CI 1.25;2.45) and 97% (95%CI 1.57;2.48) higher, respectively, among Black mothers. At least one vaccination was delayed among 49.9% (95%CI 47.8;51.9) and 61.1% (95%CI 59.2;63.0) of children by 5 and 12 months, respectively. These rates were higher among Black/mixed race mothers. Conclusion There are racial inequalities in both the obstacles faced and in vaccination rates in Brazil.

Este ensaio teórico visou trazer uma reflexão crítica sobre as aproximações entre as ideias de bell hooks e Paulo Freire em contribuição para a construção de uma Educação Alimentar e Nutricional (EAN) popular e engajada. A metodologia adotada foi a análise das obras desses autores, buscando compreender suas interseções e suas especificidades, sem a pretensão de esgotá-las. A pedagogia engajada proposta por hooks, aliada à pedagogia libertadora de Freire, buscam transformar não só a educação, mas a sociedade, tendo os sujeitos como protagonistas de seu próprio processo de aprendizagem e emancipação. Essas abordagens, ao serem incorporadas na EAN, podem ser promotoras de uma alimentação mais saudável, levando em consideração as questões de raça, gênero, cultura e poder presentes nas desigualdades alimentares nos mais diversos territórios. A EAN popular e engajada também se relaciona com a promoção da segurança alimentar e nutricional, respeitando a diversidade cultural e fomentando a participação política das pessoas e sua autonomia no processo de tomada de decisões relacionadas à alimentação e à saúde. Além disso, a formação de profissionais de saúde comprometidos com essa abordagem é fundamental para promover mudanças significativas nos paradigmas educacionais e na compreensão de lacunas importantes existentes na formação dos profissionais de saúde, no que se refere aos conteúdos do campo das ciências sociais e humanas. No contexto atual, marcado pelo fortalecimento do agronegócio e pela perda da diversidade alimentar, a EAN popular e engajada se mostra como um caminho para defender as culturas alimentares, lutar pela soberania alimentar e por uma alimentação mais diversa e sustentável.

Introdução: Documentos orientadores de alimentação adequada e saudável destacam a importância da culinária doméstica para sua promoção. Vídeos educativos vêm sendo utilizados como estratégias de educação alimentar e nutricional e são promissores para a abordagem da culinária doméstica. Objetivo: Apresentar o percurso metodológico de construção da websérie “Cidinha dá Jeito” e analisar a convergência dos conteúdos dos episódios com os guias alimentares brasileiros e com o Modelo Conceitual de Autonomia Culinária (MCAC). Métodos: Na primeira fase, elaboração da websérie, foram conduzidas seis etapas: escolha das bases conceituais; definição do público de interesse; elaboração preliminar de temas e conteúdos dos vídeos; oficinas de escuta; sistematização das contribuições, versão final dos temas e conteúdos e elaboração dos roteiros; e definição da plataforma virtual para divulgação e produção dos vídeos. Na segunda fase, analisou-se a convergência dos conteúdos dos episódios com os guias alimentares e o MCAC. Resultados: Após quatro oficinas de escuta com o público de interesse e especialistas, foi desenvolvida uma websérie composta por 21 episódios com recursos para acessibilidade comunicacional, cada um com duração de até sete minutos, tendo como audiência prioritária famílias de baixa renda e usuárias do Sistema Único de Saúde. Em abril de 2024, a websérie registrou 20.773 visualizações no canal do YouTube®. Os conteúdos dos guias alimentares brasileiros e do MCAC permeiam todos os episódios. Conclusão: A websérie, com potencial de amplo alcance e avanços na acessibilidade, busca reconhecer a complexidade da culinária e favorecer a implementação das recomendações sobre alimentação adequada e saudável.

Introduction Access to COVID-19-related care during the pandemic deserve attention and study to inform future strategies to deal with similar health emergencies in the future. We aimed to describe access to care for COVID-19-related reasons during the first 9 weeks of the pandemic in Portugal, to quantify the frequency of remote and in-person contacts with healthcare for COVID-19-related reasons and to assess the association between individual and context characteristics and contacts with health services.Methods We conducted an internet-based open cohort study with a non-probabilistic sample of 12 006 persons aged 16 years or more. One questionnaire was sent daily from 23 March 2020 for 9 weeks.Results General practitioners (GPs) were the main points of contact of patients with health services for COVID-19-related reasons. In our population, the main drivers found for any difference in the probability of contact with health services for COVID-19-related reasons were perceived high-risk contacts, followed by the existence of COVID-19 main symptoms. There were 17% more (0.17, 95% CI 0.15 to 0.18) contacts with health services among participants who reported personal contact with a confirmed case, 7% more (0.07, 95%CI 0.06 to 0.07) contacts if the contact had been with a suspected case and 6% more (0.06, 95%CI 0.05 to 0.07) contacts among participants who reported COVID-19 main symptoms in the previous 24 hours. Sociodemographic and household factors were not associated with major differences in healthcare contacts. The probability of contact with any health service for COVID-19-related reasons was highest at the beginning of the pandemic.Conclusion Most contacts with health services for COVID-19-related reasons during the first wave of the pandemic were with GPs, highlighting the role of first-contact care with these clinicians in Portuguese Primary Care, particularly in the context of the COVID-19 pandemic. Sociodemographic and household factors were not associated with major risk differences, suggesting that universal coverage and equity worked in our study sample at this pandemic stage.

Abstract: This study aimed to analyze the incidence of work-related musculoskeletal disorders (WMSD) in Brazil from 2007 to 2019, examining the spatial, temporal, and spatiotemporal patterns of their occurrence. An ecological time series study was conducted using spatial analysis techniques. WMSD morbidity data from 2007 to 2019 were collected from the Brazilian Information System for Notificable Diseases of the Brazilian Health Informatics Department. Incidence rates were standardized and smoothed using the local empirical Bayes’ theorem. Time trends were analyzed by segmented linear regression. Spatial analysis was performed using Moran’s univariate global (I) and local (LISA) indexes. The spatiotemporal scan statistic was used to identify high-risk spatiotemporal clusters for WMSD. A total of 93,387 cases of WMSD were recorded in Brazil. Temporal trends showed an increase in all regions except the Northeast, which remained stable. The incidence of WMSD showed a spatial dependence, with spatial and space-time clusters identified, especially in the Southeast region, overlapping the largest economic-industrial center of the country. The spatiotemporal clustering observed in one region suggests the highest level of industrial and economic development. Our findings highlight the need to implement intersectoral surveillance policies, inspect working conditions, and invest in the prevention and promotion of workers’ health.

ABSTRACT Objective To identify the epidemiological, spatial, and temporal pattern of TB-HIV coinfection in Brazil during the period from 2001 to 2020. Methods: Ecological study using space-time analysis techniques. It included cases of TB-HIV coinfection registered in Brazil from 2001 to 2020. The temporal trend analysis was performed using segmented regression by Joinpoint regression. For spatial analysis, Moran indices were calculated and choropleth maps were produced using TerraView and QGIS software. Results: A stable temporal trend was observed in the incidence rates of TB-HIV coinfection in Brazil during the analyzed period. In addition, high-risk areas for coinfection located in states in the North, Southeast, South, and Midwest regions were identified. Conclusion: There was stability in the incidence of TB-HIV coinfection in Brazil over the last 20 years and heterogeneous geographic distribution of risk areas for the condition.

Enquadramento: o potencial para o uso da Acupuntura na saúde das populações e na gestão dos cuidados tem vindo a ser reconhecido pelas mais variadas entidades, apoiada num crescimento da evidência da sua efetividade. Num momento em que o desafio atual se prende com a integração dos profissionais de Acupuntura no Serviço Nacional de Saúde, Portugal carece de estudos epidemiológicos representativos da população que recorre a esta terapêutica. Objetivos: conhecer e avaliar o perfil de saúde do utente de Acupuntura. Metodologia: realizou-se um estudo transversal a uma amostra de utentes de Acupuntura do litoral-norte de Portugal. A recolha de dados foi realizada através de um questionário, composto por questões sociodemográficas e pela aplicação da escala do Perfil de Saúde de Nottingham (PSN). Resultados: a amostra foi maioritariamente constituída por utentes do sexo feminino, de meia idade e profissionalmente ativos. As queixas musculoesqueléticas foram as mais frequentemente reportadas. As medianas das diferentes dimensões do PSN não ultrapassam os 12,57. Conclusão: no geral, a autoperceção do estado de saúde é boa, contudo, as queixas tendem a afetar em particular as dimensões de saúde “dor”, “energia”, “reações emocionais” e “sono”.

Introdução: A gravidez é um período de reajustamentos físicos, psicológicos e sociais, levando muitas vezes a situações de crise quer pessoal, quer familiar. O aconselhamento é uma intervenção psicoterapêutica que permite empoderar e capacitar a mulher para lidar com as alterações na gravidez. O programa de aconselhamento pretende promover a saúde mental da mulher no período perinatal aumentando o autoconhecimento e estratégias. Objetivo: Avaliar a efetividade de um programa de aconselhamento de enfermagem de saúde mental perinatal. Métodos: Estudo quantitativo, descritivo, quasi experimental, através de um estudo piloto. Amostra não probabilística por conveniência com 10 mulheres grávidas. O programa consiste em 3 sessões ao longo das semanas gestacionais, com utilização dos princípios do processo de enfermagem e do processo de aconselhamento psicoeducacional e em grupo. Como instrumentos utilizados o questionário sociodemográfico e clínico, a escala de Empowerment na grávida e a escala DASS-21 (ansiedade, stresse e depressão). Utilizou-se o Teste T para amostras emparelhadas. Resultados: Após a intervenção houve um aumento do empowerment (Me=86,6) em quase todas as dimensões da escala e uma diminuição na dimensão Stresse. Através do Teste-T infere-se que o programa provocou alterações no empowerment e na DASS-21 (dimensão Stresse). Da correlação entre as duas variáveis, extrai-se que quanto mais empoderadas as mulheres estiverem menos ansiedade, depressão e stresse sentirem. Conclusão: O estudo mostra a efetividade de programas de aconselhamento em enfermagem facilitando o processo de transição para a maternidade.

Introduction: Premature infant breastfeeding depends on the maternal perspective, which is influenced by positive and negative factors. Objective: To map factors that influence the breastfeeding of PN from the maternal perspective. Methods: Scoping Review following the Joanna Briggs Institute (JBI) and PRISMA ScR methodology. Data search for articles was carried out on Web of Science, Core Collection via Web of Science, b-On with full text, and PubMed between December 2019 and January 2021, available in full text in Spanish, Portuguese, and English. Of the 5.769 articles identified, scrutinized by title, abstract, and inclusion and exclusion criteria, the sample comprises 10 articles that meet the inclusion criteria. Results: Mothers' most reported difficulties are fears, stress, anxiety, insecurity, prematurity, Neonatal Intensive Care Unit environment, baby separation, breastfeeding, and milk extraction difficulties. As facilitators, mothers point out the nurse´s role in building the mother-child bonds. Conclusion: The paper informs about positive and negative factors in breastfeeding premature infants from the maternal point of view. Mothers need active support and professional guidance to promote and engage in good breastfeeding practices. From the maternal perspective, nurses are active players in promoting, protecting, and educating families about breastfeeding, mother-centered care, and mother-child bonding.

RESUMO O objetivo do estudo foi atualizar e avaliar a validade de conteúdo do Questionário Palmore-Neri-Cachioni de Conhecimentos Básicos sobre a Velhice. A atualização contou com três especialistas em gerontologia. Realizou-se duas rodadas de validação de conteúdo. Utilizou-se o Índice de Validade de Conteúdo a nível de item, traduzido pelo Kappa modificado e a Razão de Validade de Conteúdo, adotando pontuações ≥ 0,78, 0,74 e 1,00, respectivamente. A validade de conteúdo global foi avaliada pelo Índice de Validade de Conteúdo a nível de escala de acordo com a média, assumindo ponto de corte de 0,90. Uma reunião com a autora da versão brasileira foi realizada para esclarecimentos. O estudo piloto foi conduzido com indivíduos da população alvo e avaliado pela taxa de concordância, com ponto de corte 80%. Atualizou-se as respostas de sete itens, 14 itens foram revisados e após a segunda rodada, a validade geral do instrumento foi de 0,96. Quatro itens e a instrução foram discutidos com a autora. O estudo piloto contou com 34 respondentes e alcançou satisfatoriedade. Foi possível obter o Questionário Palmore-Neri-Cachioni | Atualizado e Revisado (2022) que evidenciou validade de conteúdo e de face quando aplicado a uma amostra de profissionais e gestores de saúde.

Summary: Background: Dengue virus (DENV) is an arbovirus transmitted by Aedes aegypti mosquitoes, which can cause severe conditions such as hemorrhagic fever and dengue shock syndrome. These conditions are associated with adverse social, clinical, and economic consequences in Brazil. Herein, the Wolbachia mosquito replacement method is a promising dengue control strategy. Methods: We estimated the economic impact of implementing the Wolbachia mosquito replacement method in seven Brazilian cities. A mathematical microsimulation model tracked nearly 23 million inhabitants over a 20-year period, considering the transitions between five different health states (susceptible, inapparent, outpatient, hospitalised and death). Direct costs included local dengue control programs, Wolbachia implementation and dengue care. Indirect costs related to death and productivity loss, as well as disability-adjusted life-years (DALY) averted were also considered. Findings: Without Wolbachia, the model projected 1,762,688 reported dengue cases over 20 years. Implementing the Wolbachia method would avert at least 1,295,566 dengue cases, resulting in lower costs and greater effectiveness in all simulated cities. On average, for every 1000 inhabitants followed for 20 years, the Wolbachia method yielded a cost difference of USD 538,233.68 (BRL 2,691,168.40) and averted 5.56 DALYs. Net monetary benefits (NMB) were positive in all seven cities, ranging from USD 110.72 (BRL 553.59) to USD 1399.19 (BRL 6995.95) per inhabitant. Alternative scenarios have also shown a favourable return on investment with a positive benefit-cost ratio (BCR). Interpretation: Wolbachia is likely a cost-effective strategy in the Brazilian context, consistent with international studies. Summary: Background: CYD-TDV (Dengvaxia®) was the first dengue vaccine approved, launched in Brazil in 2015 for individuals aged 9–44 years. We aimed to estimate the effectiveness of CYD-TDV in preventing symptomatic dengue cases during a campaign targeting individuals aged 15–27 years in selected municipalities in Paraná, Brazil. Additionally, we examined whether a history of dengue, as recorded by the surveillance system, modified the vaccine's effectiveness. Methods: We conducted a case-cohort analysis comparing the frequency of vaccination, with at least one dose of CYD-TDV, in individuals with dengue confirmed by RT-PCR, identified by the surveillance system during 2019 and 2020, with the vaccination coverage in the target population. Moreover, in a case-control design using weighted controls, we assessed the documented history of dengue as a modifier of the vaccine's effectiveness. We used a logistic random-effects regression model, with data clustered in municipalities and incorporating covariates such as the incidence of dengue before the campaign, age, and sex. We calculated vaccine effectiveness (VE) as (1-relative risk) x 100%. Findings: 1869 dengue cases were identified, which had a vaccination frequency significantly lower than the overall vaccination coverage in the target population (50.3% vs. 57.2%, respectively; overall VE: 21.3%; 95% confidence interval [CI]: 13.4%–28.4%). In individuals with a documented history of dengue, vaccination had a VE of 71% (95% CI: 58%–80%) in reducing the incidence of dengue. However, vaccination was not associated with a significant reduction in the overall dengue case risk in individuals without a documented history of dengue (VE: 12%; 95% CI: −21% to 36%). In this last stratum, vaccination was associated with reduced cases due to DENV-1 and DENV-4, but an excess of DENV-2 cases. Interpretation: Vaccination led to a significant reduction in reported dengue cases within the target population. The case-control design suggested that this reduction was primarily driven by the benefits observed in individuals with a documented history of dengue. In endemic regions with limited serological testing facilities, a previous history of dengue diagnosis recorded by epidemiological surveillance could be used to triage candidates for CYD-TDV vaccination. BackgroundThe COVID-19 pandemic represented a great stimulus for the adoption of telehealth and many initiatives in this field have emerged worldwide. However, despite this massive growth, data addressing the effectiveness of telehealth with respect to clinical outcomes remain scarce. ObjectiveThe aim of this study was to evaluate the impact of the adoption of a structured multilevel telehealth service on hospital admissions during the acute illness course and the mortality of adult patients with flu syndrome in the context of the COVID-19 pandemic. MethodsA retrospective cohort study was performed in two Brazilian cities where a public COVID-19 telehealth service (TeleCOVID-MG) was deployed. TeleCOVID-MG was a structured multilevel telehealth service, including (1) first response and risk stratification through a chatbot software or phone call center, (2) teleconsultations with nurses and medical doctors, and (3) a telemonitoring system. For this analysis, we included data of adult patients registered in the Flu Syndrome notification databases who were diagnosed with flu syndrome between June 1, 2020, and May 31, 2021. The exposed group comprised patients with flu syndrome who used TeleCOVID-MG at least once during the illness course and the control group comprised patients who did not use this telehealth service during the respiratory illness course. Sociodemographic characteristics, comorbidities, and clinical outcomes data were extracted from the Brazilian official databases for flu syndrome, Severe Acute Respiratory Syndrome (due to any respiratory virus), and mortality. Models for the clinical outcomes were estimated by logistic regression. ResultsThe final study population comprised 82,182 adult patients with a valid registry in the Flu Syndrome notification system. When compared to patients who did not use the service (n=67,689, 82.4%), patients supported by TeleCOVID-MG (n=14,493, 17.6%) had a lower chance of hospitalization during the acute respiratory illness course, even after adjusting for sociodemographic characteristics and underlying medical conditions (odds ratio [OR] 0.82, 95% CI 0.71-0.94; P=.005). No difference in mortality was observed between groups (OR 0.99, 95% CI 0.86-1.12; P=.83). ConclusionsA telehealth service applied on a large scale in a limited-resource region to tackle COVID-19 was related to reduced hospitalizations without increasing the mortality rate. Quality health care using inexpensive and readily available telehealth and digital health tools may be delivered in areas with limited resources and should be considered as a potential and valuable health care strategy. The success of a telehealth initiative relies on a partnership between the involved stakeholders to define the roles and responsibilities; set an alignment between the different modalities and levels of health care; and address the usual drawbacks related to the implementation process, such as infrastructure and accessibility issues.

The Volunteer Functions Inventory (VFI) is an instrument widely used to assess volunteers’ motivation based on the Functionalist Model of Omoto and Snyder. It assesses six factors that reflect several motivational functions. The VFI has been translated into various languages and validated in different cultural contexts, but some studies have reported different factor structures (e.g., five or four factors). In the Portuguese context, previous studies have also shown inconsistent results. The aim of this study was to adapt and validate the VFI for Portuguese volunteers, testing several alternative models (nine models) using confirmatory factor analysis. The sample comprised 468 volunteers (76.3% women), aged from 13 to 81 years (M = 36.66, SD = 14.93). The results support the original interrelated six-factor model as the best-fitting one. The VFI showed good internal consistency and convergent validity. Significant correlations were found between the VFI factors, organizational commitment, and volunteers’ satisfaction. Overall, the six-factor VFI is a valid and reliable tool for measuring the motivational functions of Portuguese volunteers, with implications for practice and research in the volunteering field.

Abstract Background Hazardous drinking and drug consumption are associated with an increased risk of HIV due to the complex interplay of factors influencing decision-making capability, stigma and social marginalization. In this study, we explore the patterns of hazardous alcohol and drug use and correlates of risk factors among female sex workers (FSW) and men who as sex with men (MSM) in Mozambique. Methods We conducted a secondary data analysis of bio-behavioral surveys (BBS) among FSW and MSM using a respondent-driven sampling methodology conducted in five main urban areas of Mozambique from 2019 to 20. The survey included a standardized questionnaire, where hazardous drinking was assessed (using AUDIT-C scores ≥ 4 for men, ≥ 3 for women) and drug use in the last year (FSW). Chi-squared test was used to analyze the association between socio-demographic and behavioral variables, and multivariate logistic regression measured the impact of the associated factors. Results The prevalence of hazardous alcohol drinking was 47.1% (95% CI:44.8–49.5) for FSW and 46.5 (95% CI: 44.0–49.0) for MSM. Current drug use was reported in 13.3% of FSW. Abstract Background Preterm births increase mortality and morbidity during childhood and later life, which is closely associated with poverty and the quality of prenatal care. Therefore, income redistribution and poverty reduction initiatives may be valuable in preventing this outcome. We assessed whether receipt of the Brazilian conditional cash transfer programme - Bolsa Familia Programme, the largest in the world - reduces the occurrence of preterm births, including their severity categories, and explored how this association differs according to prenatal care and the quality of Bolsa Familia Programme management. Methods A retrospective cohort study was performed involving the first live singleton births to mothersenrolled in the 100 Million Brazilian Cohort from 2004 to 2015, who had at least one child before cohort enrollment. Only the first birth during the cohort period was included, but born from 2012 onward. A deterministic linkage with the Bolsa Familia Programme payroll dataset and a similarity linkage with the Brazilian Live Birth Information System were performed. Objective: To investigate the factors associated with urinary incontinence in older adults living in nursing homes. Methods: This exploratory, cross-sectional, observational, and quantitative study used exploratory and path analysis (PA). Eighty-six older adults living in nursing homes in the city of Salvador, Brazil, and the city of Brasília, Brazil, were included. Data were collected from January to March 2020, before the Covid-19 pandemic. The following variables were evaluated: sex, age group, functional performance, global cognitive function, comorbidities, and health conditions. We considered urinary incontinence for older adults who answered affirmatively to the "In the last 12 months, have you had an involuntary loss of urine?". Data were initially analyzed by descriptive analysis and nonparametric statistical tests. A PA was performed using generalized structural equation modeling toinvestigate the relationship between the variables of interest and the direct and indirect effects of the independent variables and UI according to the theoretical model. Subsequently, the statistical distributions of the variables and the connections were defined for the regression estimation. After the analysis, the model was respecified, excluding the variables and paths that were not statistically significant to determine the best model. The final model was designed after statistical adjustments, corroborating or not with the theoreticalmodel. Results: Urinary incontinence was associated with educational level, marital status, hypertension,one or more difficulties in basic activities of daily living, mood, insomnia, lossof appetite,fecal incontinence, and difficulty swallowing. In the PA, depression and difficulty swallowing were directly associated with urinary incontinence, and urinary incontinence wasdirectly and significantly associated with insomnia and fecal incontinence. Conclusion: Given the variety of social and health components associated with urinary incontinence, it is necessary to assess, prevent, treat, and rehabilitate this condition in Brazilian nursing homes. Interventions in urinary incontinence demand integrated actions in functional, clinical, and mental healthaspects to promote the well-being of older adults living in nursing homes.

Abstract Globally, People Who Inject Drugs (PWID) have limited healthcare, treatment, and prevention services, and they frequently experience stigma and negative attitudes toward healthcare providers when accessing services. Mozambique, with a general population HIV prevalence of 12.5%, has one of the highest rates in the world, and the PWID population has the highest HIV prevalence among key populations, estimated at nearly 50%. Less than half of HIV positives who inject drugs are linked to HIV treatment and are retained in care. One of the main reasons is that HIV treatment is mainly provided in a public health facility and PWID delayed accessing healthcare since they anticipated mistreatment from multiple levels of healthcare providers. To improve the health outcomes in this group, we need to treat them where they feel comfortable and respected. In this commentary, we outline the importance of innovative approaches to enhance the management of HIV-positive PWID. As a country gets close to controlling the HIV epidemic, refocusing and targeting responses to the highest-risk groups becomes even more essential for shaping more effective HIV interventions and achieving epidemic control.

Transgender women (TGW) and men who have sex with other men (MSM) often encounter disparities in accessing HIV testing, leading to delayed diagnoses and worse prognoses. We analysed barriers and facilitators for accessing HIV rapid testing by TGW and MSM in Brazil, 2004-2023. Citations were included whether the study population consisted of individuals aged ≥18y old, and studies addressed HIV testing and have been conducted in Brazil. The study protocol was based on Joanna Briggs’ recommendations for scoping reviews. We included 11 studies on TGW and 17 on MSM. The belief that one is not at risk of contracting HIV infection, fear expressed in different ways (e.g. lack of confidentiality) and younger age were the main barriers. Feeling at risk for HIV infection, curiosity, and favourable characteristics of the setting where the testing takes place were cited as the main facilitators. Barriers and facilitators specifically for HIV self-testing included, respectively, concerns about conducting the test alone vs. autonomy/flexibility. Brazil is unlikely to achieve the UN’ 95-95-95 goal without minimising testing disparities. Combating prejudice against TGW and MSM in testing settings, along with educational campaigns and transparent protocols to ensure confidentiality, can help increase HIV testing among these populations.

Summary: Background: Chagas disease (CD), endemic in 21 Latin American countries, has gradually spread beyond its traditional borders due to migratory movements and emerging as a global health concern. We conducted a systematic review and meta-analysis of available data to establish updated prevalence estimates of CD in Latin American migrants residing in non-endemic countries. Methods: A systematic search was conducted in MEDLINE/PubMed, Embase, Cochrane Library, Scopus, Web of Science, and LILACS via Virtual Health Library (Biblioteca Virtual em Saúde - BVS), including references published until November 1st, 2023. Pooled prevalence estimates and 95% confidence intervals (CI) were calculated using random effect models. Heterogeneity was assessed by the chi-square test and the I2 statistic. Subgroup analyses were performed to explore potential sources of heterogeneity among studies. The study was registered in the PROSPERO database (CRD42022354237). Findings: From a total of 1474 articles screened, 51 studies were included. Studies were conducted in eight non-endemic countries (most in Spain), between 2006 and 2023, and involving 82,369 screened individuals. The estimated pooled prevalence of CD in Latin American migrants living in non-endemic countries was 3.5% (95% CI: 2.5–4.7; I2: 97.7%), considering studies in which screening was indicated simply because the person was Latin American. Per subgroups, the pooled CD prevalence was 11.0% (95% CI: 7.7–15.5) in non-targeted screening (unselected population in reference centers) (27 studies); in blood donors (4 studies), the pooled prevalence was 0.8% (95% CI: 0.2–3.4); among people living with HIV Latin American immigrants (4 studies) 2.4% (95% CI: 1.4–4.3) and for Latin American pregnant and postpartum women (14 studies) 3.7% (95 CI: 2.4–5.6). The pooled proportion of congenital transmission was 4.4% (95% CI: 3.3–5.8). Regarding the participants’ country of origin, 7964 were from Bolivia, of which 1715 (21,5%) were diagnosed with CD, and 21,304 were from other Latin American countries of which 154 (0,72%) were affected. Interpretation: CD poses a significant burden of disease in Latin American immigrants in non-endemic countries, suggesting that CD is no longer a problem limited to the American continent and must be considered as a global health challenge. Objetivo: Avaliar as reações imunomediadas dermatológicas em pacientes tratados com inibidores de checkpoint em um centro oncológico de Salvador-BA. Método: Foi realizado um estudo observacional, longitudinal, retrospectivo, descritivo e não controlado, em pacientes submetidos ao tratamento com inibidores de checkpoint (n = 72), durante o período de jan/2021 até dez/2021. Após os critérios de exclusão, o tamanho amostral do estudo resultou em 69 pacientes. Planilhas eletrônicas da ferramenta Excel (Microsoft®) foram utilizadas para tratamento dos dados e análise estatística. A identificação e mensuração da gravidade das toxicidades seguiram os Critérios Comuns de Toxicidade, conforme definidos pelo Common Terminology Criteria for Adverse Events (CTCAE), versão 5.0. Resultados: A investigação constatou 84 reações imunomediadas dermatológicas ocorridas em 44 pacientes (63,77%), com maior frequência para pele seca (37%), rash maculopapular (26%) e prurido (20%). O regime com maior incidência de reações foi aquele que incluía o pembrolizumabe, com 47 ocorrências. A gravidade das toxicidades imunomediadas dermatológicas variou de grau 1 a grau 2, indicando um bom perfil de segurança para esses medicamentos. As principais estratégias de gerenciamento incluíram o uso de emolientes, aumento da ingestão de líquidos e a administração de anti-histamínicos e corticosteroides. Conclusão: Os achados deste estudo são consonantes com as evidências da literatura clínica e destacam a importância da compreensão aprofundada dos fatores relacionados à toxicidade da imunoterapia, de maneira a detectar prematuramente essas reações, otimizando o manejo e prevenindo complicações mais graves.

Discorre sobre relações entre a Ciência da Informação e o movimento da Ciência Aberta, sob a ótica de artigos científicos identificados na Base de Dados Referenciais de Artigos de Periódicos em Ciência da Informação. Objetiva determinar as dimensões que campo e movimento se relacionam, estabelecendo um panorama dessa relação com as pesquisas brasileiras do campo no período entre 2015 e 2019, analisando como essa relação perpassa e se estabelece no domínio da comunicação científica. A metodologia empregada é a Revisão Narrativa de Literatura, na análise dos títulos, resumos e palavras-chave dos artigos selecionados. O campo empírico é composto pelos resultados obtidos em busca na base, totalizando 36 resultados. Conclui-se que a Ciência da Informação está se relacionando com a Ciência Aberta, observando-se a prevalência de estudos sobre temáticas de Dados de Pesquisa Abertos e Repositórios como maneiras de aperfeiçoar os fazeres científicos observando os preceitos do movimento.

Objective: To evaluate dermatological immune-mediated reactions in patients treated with checkpoint inhibitors at an oncology center in Salvador-BA. Method: An observational, longitudinal, retrospective, descriptive, and uncontrolled study was carried out in patients undergoing treatment with checkpoint inhibitors, during the period from Jan/2021 to Dec/2021. After applying the exclusion criteria, the study’s sample size resulted in 69 patients. Electronic spreadsheets from the Excel tool (Microsoft®) were used for data processing and statistical analysis. The identification and measurement of the severity of toxicities followed the Common Toxicity Criteria, as defined by the Common Terminology Criteria for Adverse Events (CTCAE), version 5.0. Results: The investigation found 84 immunemediated dermatological reactions occurring in 44 patients (63.77%), with the most frequent being dry skin (37%), maculopapular rash (26%), and itching (20%).The regimen with the highest incidence of reactions was the one that included pembrolizumab, with 47 occurrences. The severity of dermatological immune-mediated toxicities ranged from grade 1 to grade 2, indicating a good safety profile for these medications. Key management strategies included the use of emollients, increased fluid intake, and administration of antihistamines and corticosteroids. Conclusion: The findings of this study are aligned with the evidence from the clinical literature and highlight the importance of in-depth understanding of factors related to immunotherapy toxicity, in order to detect these reactions prematurely, optimizing management and preventing more serious complications.

The economic, social, and health crisis in Venezuela has resulted in the largest forced migration in recent Latin American history. The general scenario in host countries influence migrants’ self-perception of quality of life, which can be understood as an indicator of their level of integration. The COVID-19 pandemic has exacerbated socioeconomic and health vulnerabilities, especially for forced migrants. We hypothesized that the adverse circumstances faced by Venezuelan migrants during the pandemic have deepened their vulnerability, which may have influenced their perception of quality of life. This study aims to evaluate the quality of life of Venezuelan migrants in Brazil during the COVID-19 pandemic. We assessed the quality of life of 312 adult Venezuelan migrants living in Brazil using the World Health Organization WHOQOL-BREF quality of life assessment, which was self-administered online from October 20, 2020, to May 10, 2021. The associations of quality of life and its domains with participants’ characteristics were analyzed via multiple linear regression models. Mean quality of life score was 44.7 (±21.8) on a scale of 0 to 100. The best recorded mean was in the physical domain (66.2±17.8) and the worst in the environmental domain (51.1±14.6). The worst quality of life was associated with being a woman, not living with a partner, lower household income, and discrimination based on nationality. Factors associated with overall quality of life and respective domains, especially income and discrimination, were also observed in other studies as obstacles to Venezuelan migrants. The unsatisfactory quality of life among Venezuelans living in Brazil may have been worsened by the pandemic during the study period.

Introdução: O sequilho é um biscoito popular no Brasil, geralmente produzido artesanalmente por agroindústrias familiares. Entretanto, muitas vezes, devido à falta de boas práticas de fabricação (BPF), os alimentos assim produzidos apresentam risco de contaminação por agentes patogênicos, tornando-se potenciais veiculadores de Doenças de Transmissão Hídrica e Alimentar (DTHA). Objetivo: Avaliar a qualidade microbiológica de biscoitos produzidos por agroindústrias familiares de Empreendimentos Econômicos Solidários da Bahia. Método: Entre agosto e outubro de 2022, avaliou-se 14 lotes de biscoito, compostos por 5 amostras, totalizando 70 amostras, advindos de 5 agroindústrias familiares situadas nos municípios de Santo Antônio de Jesus, São Felipe, Mutuípe, Teolândia e Valença, no estado da Bahia, Brasil. Realizou-se uma análise microbiológica por meio do método rápido de contagem em placas Petrifilm™ (3M Company) de Salmonella sp. (AOAC 2014.01), Escherichia coli (AOAC 991.14), bolores e leveduras (AOAC 997.02) e Estafilococos coagulase positiva (AOAC 2003.07) e pelo método tradicional para quantificação de Bacillus cereus (AOAC 980.31). Resultados: 13 lotes (92,9%) revelaram qualidade satisfatória. Desses, nove (69,2%) apresentaram resultado satisfatório com qualidade aceitável e quatro (30,8%) resultaram satisfatórios com qualidade intermediária. Ademais, um lote (7,1%) foi considerado insatisfatório por apresentar contagem de B. cereus acima do limite permitido pela legislação sanitária. Conclusões: Os biscoitos avaliados apresentaram baixo índice de contaminação, não representando risco para a veiculação de DTHA, sendo, assim, adequados ao consumo humano. Entretanto, sugere-se a elaboração de políticas públicas que envolvam a educação sanitária dos manipuladores, incremento da produção e valorização da cultura regional.

O uso de ferramentas da saúde digital tem sido intensificado na Atenção Primária à Saúde (APS) e nas práticas de agentes comunitários de saúde (ACS). Este artigo tem como objetivo analisar os desafios impostos pelas condições de trabalho dos ACS no contexto da saúde digital. Trata-se de uma pesquisa qualitativa com triangulação de métodos envolvendo lideranças sindicais, gestores e profissionais da saúde. Os resultados apontam uma reedição de velhos desafios em torno do trabalho dos ACS, como maior burocratização, controle, divisão social e técnica. Porém, novos desafios emergem em torno da manutenção, da qualidade dos instrumentos e da formação profissional. Conclui-se demarcando a necessidade de uma garantia logística, financeira e política para a implementação da saúde digital no trabalho dos ACS.

Introdução: Existe uma relação entre inflamação e doenças crônicas não transmissíveis, que geralmente se manifestam na fase adulta. Entretanto, já é possível perceber alterações inflamatórias no público infanto-juvenil.Sendo assim, torna-se necessário o estudo dos fatores associados à inflamação ainda nessa fase, a fim de que sejam feitas intervenções cabíveis para impedir que taisalterações se prologuem até a vida adulta. Objetivo: Avaliar a associação entre marcadores inflamatórios, indicadores de excesso de peso e marcadores de consumo alimentar em crianças e adolescentes. Métodos: Estudo transversal com indivíduos de 8 a 14 anos, de escolas públicas de Vitória/ES. Para avaliar inflamação, utilizaram-se leucócitos, proteína C reativa e ácido úrico. Os indicadores de excesso de peso utilizados foram perímetro da cintura, razão cintura/estatura, IMC-para-idade e percentual de gordura corporal. Para estimar as probabilidades de associação, calcularam-se razões de prevalência e seus respectivos intervalos de confiança de 95%, por meio de modelos de regressão de Poisson, ajustados para características sociodemográficas. Resultados: Avaliaram-se 296 indivíduos, com média de 10,7 anos, sendo 54,4% do sexo feminino e 53,7% adolescentes. Todas as variáveis com risco dos indicadores de excesso de peso mostraram-se significativas com o risco de ácido úrico elevado, além do risco de elevação da razão cintura/estatura (RP = 1,14), do IMC-para-idade (RP = 1,08) e da gordura corporal (RP = 1,10) serem significativos com o risco de PCR elevado. Conclusão: Conclui-se que o risco de alterações nos indicadores de excesso de peso esteve relacionado com o risco de elevação dos marcadores inflamatório sem escolares.

ABSTRACT Objective: To evaluate the impact of the state action-research project on vaccination coverage in children under two years of age in the state of Minas Gerais, according to the size of the municipalities, comparing the years 2021 and 2022. Methods: This is a study nested within the state action-research project, a before-after community clinical trial carried out in 212 municipalities in the state of Minas Gerais. This study used secondary data on Vaccination Coverage (VC), Homogeneity of Vaccines (HVC) and Abandonment rate of multi-dose vaccines. After classifying municipalities by size and vaccination coverage rates were equitably classified, an analysis of secondary data on 12 immunobiologicals indicated for the age group in question and their abandonment rate of multi-dose vaccines was carried out. Results: There was an increase in the proportion of municipalities classified as small that reached the vaccination coverage target set by the National Immunization Program (PNI) after the action-research project was carried out. There was an increase in the proportion of small municipalities classified as having a low abandonment rate for the rotavirus vaccine, in the adequate homogeneity of vaccination coverage and in the classification of risk as very low risk and low and medium risk, all with a statistically significant difference. Conclusion: There was an influence of municipal size on the effectiveness of the actions applied to increase vaccination coverage, explaining that proposing individualized actions for each municipality is essential to improve vaccination coverage.

Resumo: A detecção precoce é uma das estratégias para o controle do câncer de mama e, para tanto, é fundamental garantir o acesso à investigação dos casos suspeitos para continuidade do cuidado e tratamento oportuno. Este estudo tem por objetivo estimar a necessidade de procedimentos para detecção precoce dessa neoplasia e avaliar a sua adequação no atendimento às mulheres rastreadas e sintomáticas no Sistema Único de Saúde (SUS), no ano de 2019. Foi realizado um estudo descritivo transversal para analisar a oferta de exames de detecção precoce do câncer de mama, comparando a necessidade estimada com os procedimentos realizados no SUS. Foram utilizados os parâmetros disponibilizados pelo Instituto Nacional de Câncer para estimar a população e a necessidade de exames para a detecção precoce. No Sistema de Informações Ambulatoriais do SUS, obteve-se o número de procedimentos realizados em 2019. Observou-se um déficit de mamografias de rastreamento no país (-45,1%), variando entre -31,4% na Região Sul a -70,5% na Região Norte. Se a oferta desse exame fosse direcionada para a população-alvo do rastreamento, o déficit no país reduziria para -14,8% e haveria sobreoferta no Sul (6,2%). Os procedimentos de investigação diagnóstica apresentaram variações entre as regiões, com maiores déficits de punção por agulha grossa (-90,8%) e biópsia/exérese de nódulo da mama (-80,6%) observados no Centro-oeste, e o maior déficit de exames anatomopatológicos no Norte (-88,5%). A comparação entre a produção e a necessidade de procedimentos para detecção precoce do câncer de mama no Brasil identificou déficits e inadequações que devem ser melhor conhecidos e equacionados em nível estadual e municipal.
