2. [The maturity model for the public health service-a tool for assessing and improving the digital maturity level of German public health agencies].

The COVID 19 crisis has highlighted the key role of the public health service (PHS), with its approximately 375 municipal health offices involved in the pandemic response. Here, in addition to a lack of human resources, the insufficient digital maturity of many public health departments posed a hurdle to effective and scalable infection reporting and contact tracing. In this article, we present the maturity model (MM) for the digitization of health offices, the development of which took place between January 2021 and February 2022 and was funded by the German Federal Ministry of Health. It has been applied since the beginning of 2022 with the aim of strengthening the digitization of the PHS. The MM aims to guide public health departments step by step to increase their digital maturity to be prepared for future challenges. The MM was developed and evaluated based on qualitative interviews with employees of public health departments and other experts in the public health sector as well as in workshops and with a quantitative survey. The MM allows the measurement of digital maturity in eight dimensions, each of which is subdivided into two to five subdimensions. Within the subdimensions a classification is made on five different maturity levels. Currently, in addition to recording the digital maturity of individual health departments, the MM also serves as a management tool for planning digitization projects. - Abstract
Objectives This report aims to present the community assessment model developed by the Committee on Public Health Nursing (6th term) of the Japanese Association of Public Health. This new model was designed such that it could be applied to a broad range of public health activities. It aims at theorizing public health nurses' practice-based knowledge and sharing it among other public health professionals.Methods The model was developed during seven committee meetings held from October 2014 to September 2017. In the first step, we brainstormed the definition and methods of community assessment and constructed a framework for a literature review. In the second step, information on theories, research, and practice relevant to community assessment was reviewed based on this framework. In the third step, the community assessment model was developed based on the results of the literature review and the practice experience of the committee members. In the last step, we examined the applicability of this model to the practice of occupational health and public health administration.Project activities We defined community assessment as the "skills and methods based on applied science that drive Plan-Do-Check-Action (PDCA) cycles in every activity that aims at achieving a better quality of life (QOL)." We further classified community assessment into two types; comprehensive assessment and targeted assessment. The model underlined that community assessment was a continuous and developmental process that occurs throughout every stage of the PDCA cycle, and that it was oriented toward improving the QOL of community residents. This model also purported that the empirical and scientific intuition, and ethical sensitivity of assessors were among the key determinants of assessment quality.Conclusion The model on community assessment developed in the present study based on the empirical knowledge of public health nurses could be applied to all types of public health activities in communities.

The article presents the history of the establishment of the State School of Hygiene (PSH) in Poland after the First World War. The difficulties faced by the public health service in a country destroyed by war and created after the reunification of the lands, which for over 150 years remained under the control of three powers, were pointed out. It discusses how the foundations of modern teaching in the field of public health were created in the National Institute of Hygiene (PZH) in Warsaw, an institution to which the Ministry of Public Health entrusted tasks related to health education in the country. The State School of Hygiene was built by the Polish Government with a significant financial contribution from the John Davison Rockefeller Foundation. The official opening ceremony took place on 20 April 1926. The State School of Hygiene in Warsaw was the first such school in Europe. It educated professional staff for the health service in Poland, especially sanitary physicians, sanitary inspectors, nurses and staff to work in health offices. The importance and scope of influence of the State School of Hygiene as the Department of Education in the National Institute of Hygiene was constantly increasing, as evidenced by the number of students (about 800 per year) participating in courses, especially in the first years after its establishment. By the end of 1935, 6,389 students had completed the courses, including 1,900 physicians. Apart from the teaching activities, the State School of Hygiene also carried out research work. The State School of Hygiene was supported by the Rockefeller Foundation, which funded scholarships for the employees of the National Institute of Hygiene at university centers in the USA, (National Institute of Public Health - National Institute of Hygiene)

5. Strengthening the public health workforce: An estimation of the long-term requirements for public health specialists in Serbia.

- Abstract
At the beginning of the 21st century, planning the public health workforce requirements came into the focus of policy makers. The need for improved provision of essential public health services, driven by a challenging non-communicable disease and causes of death and disability within Serbia, calls for a much needed estimation of the requirements of the public health professionals. Mid and long-term public health specialists' supply and demand estimations out to 2025were developed based on national staffing standards and regional distribution of the workforce in public health institutes of Serbia. By 2025, the supply of specialists, taking into account attrition rate of -1% reaches the staffing standard. However, a slight increase in attrition rates has the impact of revealing supply shortage risks. Demand side projections show that public health institutes require an annual input of 10 specialists or 2.1% annual growth rate in order for the four public health fields to achieve a headcount of 487 by 2025 as well as counteract workforce attrition rates. Shortage and poor distribution of public health specialists underline the urgent need for workforce recruitment and retention in public health institutes in order to ensure the coordination, management, surveillance and provision of essential public health services over the next decade., (Copyright © 2018 Elsevier B.V. Background: Achieving the Sustainable Development Goal of 'ensuring healthy lives and promoting well-being for all at all ages' necessitates regular monitoring of inequality in the availability of health-related infrastructure and access to services, and in health risks and outcomes., Objectives: To quantify subnational regional inequality in Indonesia using a composite index of public health infrastructure, services, behavioural risk factors and health outcomes: the Public Health Development Index (PHDI)., Methods: PHDI is a composite index of 30 public health indicators from across the life course and along the continuum of care. An overall index and seven topic-specific sub-indices were calculated using data from the 2013 Indonesian Basic Health Survey (RISKESDAS) and the 2011 - Village Potential Survey (PODES). These indices were analysed at the national, province and district levels. Within-province inequality was calculated using the Weighted Index of Disparity (IDISW)., Results: National average PHDI overall index was 54.0 (out of a possible 100); scores differed between provinces, ranging from 43.9 in Papua to 65.0 in Bali. Provinces in western regions of Indonesia tended to have higher overall PHDI scores compared to eastern regions. Large variations in province averages were observed for the non-communicable diseases sub-index, environmental health sub-index and infectious diseases sub-index. Provinces with a similar number of districts and with similar overall scores on the PHDI index showed different levels of relative within-province inequality. Greater within-province relative inequalities were seen in the environmental health and health services provisions sub-indices as compared to other indices., Conclusions: Achieving the goal of ensuring healthy lives and promoting well-being for all at all ages in Indonesia necessitates having a more focused understanding of district-level inequalities across a wide range of public health infrastructure, service, risk factor and health outcomes indicators, which can enable geographical comparison while also revealing areas for intervention to address health inequalities.

Background: Nation-wide studies on the health care situation of asylum-seekers in Germany are lacking, but decision-makers increasingly need such information., Objectives: The aim of the study was to assess structures, processes and needs related to the health care provision for asylum-seekers along the continuum of reception centres to community dwellings from the perspective of the German public health authorities., Materials and Methods: A nation-wide cross-sectional mixed-methods survey was carried out. All heads of public health authorities in Germany (N = 389) were invited to complete a standardized questionnaire related to: (1) medical procedures and screening; (2) prevention and health promotion; (3) communication; (4) documentation and information; (5) coordination; (6) structural resources and needs. The quantitative survey was complemented by qualitative semi-structured interviews., Results: In total, 123 heads of public health authorities (response rate: 31,6 %) completed the questionnaire, and 29 were interviewed. Priority areas to improve the health care situation were better coordination and standardisation of care, enhancing vaccination capacities, standardised documentation, better health information exchange (in line with data protection laws), and a stronger focus on a few relevant infectious diseases in the scope of compulsory health entry examinations., Conclusion: The instruments proved useful to assess the health care situation of asylum-seekers in a decentralized health care system. Repeated surveys with a focus on selected domains of the questionnaire could help monitor the health care situation on a regular basis.

8. Individual rights vs the public health: the problem of the Asian hepatitis B carriers in America.

The influx of Asians into the United States (both as refugees and immigrants) presented American public health authorities with a significant challenge because many of them were chronic hepatitis B carriers. The carriers posed a real, but undetermined level of risk of spreading the infection to the larger community. Of special importance was the danger that susceptible children would be infected in day-care centers, because the earlier the age of exposure, the greater the risk of developing chronic carriership; which in turn is associated with cirrhosis and cancer of the liver in later life. The Asian carriers presented both a health problem, and a moral dilemma. If the carrier situation became a subject of public debate, there was danger of a racist and discriminatory reaction to the newcomers. Faced with this problem, American public health authorities opted to protect the Asian carriers by assuring the medical profession and concerned members of the public that there was either no health risk, or the risk was easily controllable, despite the lack of evidence to support such positions. In their attempt to protect a vulnerable group from discrimination, they functioned as humanitarians, but abrogated their duty as scientists and thereby undermined the long-term credibility of those charged with protecting the public health. Thus, despite achieving some immediate benefits for a defenseless group, the price they paid for that protection was too high.

Information and Communication Technology (ICT) progression has notably impacted the shift from traditional public services to digital alternatives. Among the various digital services, m-government services, provided by smartphone technology, have gained widespread popularity. Unfortunately, the broader adoption of digital technology encounters several challenges, including insufficient user interest and acceptance, as well as concerns regarding security and user privacy. The primary goal of the research is to address the existing gap in the literature by examining the factors that contribute to the effective implementation of m-government services. A mix of key components is employed, incorporating the Information Systems (IS) Success model and Technology Acceptance Model (TAM) as research variables. The research applies a quantitative approach in the form of an online survey. Furthermore, a Partial Least Square-Structure Equational Modeling (PLS-SEM) analytic approach is performed to evaluate 230 data points. The research findings support five hypotheses while rejecting three hypotheses. Significantly, the findings suggest that perceived usefulness and ease of use influence behavioral intention considerably. Additionally, constructions related to service quality significantly impact behavioral intention. Meanwhile, both system quality and information quality do not contribute to affecting behavioral intention. Furthermore, information quality exerts a substantial impact on perceived usefulness, but it does not influence perceived ease of use. The Republic of Korea has established an institutional framework to expedite the provision of rehabilitation sports public services to individuals with disabilities post-hospital discharge (Act on Guarantee of Right to Health and Access to Health Services for People with Disabilities in December 2017). Regrettably, this service remains non-operational to date. This study employs a service design approach to identify and develop essential elements for the effective implementation of public rehabilitation sports services in Korea. Adopting the service design method, including the empathize–define–ideate–prototype phases, co-creation activities were conducted with three teams comprising people with disabilities, caregivers, rehabilitation physicians, specialized sports instructors, facility managers, and government officials, emphasizing equitable distribution. By leveraging the experiences of people with disabilities, these teams collaboratively engaged in creative activities to formulate strategies for delivering prompt and user-friendly rehabilitation sports public services post-hospital discharge. Contributions from each team were meticulously collected and organized, incorporating diverse perspectives into the development of the Korean Rehabilitation Sports Public Service Information System (KRSPSIS). Additionally, we presented a scenario illustrating the practical application of the KRSPSIS. Through this system, we anticipate providing more efficient and convenient rehabilitation sports public services to individuals with disabilities during the critical early stages following hospital discharge. Background: Basic public health services for diabetes play an essential role in controlling glycemia in patients with diabetes. This study was conducted to understand the urban-rural disparities in the utilization of basic public health services for people with diabetes and the factors influencing them. Methods: The data were obtained from the 2018 China Health and Retirement Longitudinal Study (CHARLS) with 2976 diabetes patients. Chi-square tests were used to examine the disparities in the utilization of diabetes physical examination and health education between urban and rural areas. Logistic regression was performed to explore the factors associated with the utilization of diabetes public health services. Results: Among all participants, 8.4% used diabetes physical examination in the past year, and 28.4% used diabetes health education services. A significant association with age (OR = 0.64, 95% CI:0.49–0.85; P < 0.05) was found between patients' use of health education services. Compared with diabetes patients living in an urban area, diabetes patients living in a rural area used less diabetes health education. (χ2= 92.39, P < 0.05). Patients' self-reported health status (OR = 2.04, CI:1.24–3.35; P < 0.05) and the use of glucose control (OR = 9.33, CI:6.61–13.16; P < 0.05) were significantly positively associated with the utilization of diabetes physical examination. Patients with higher education levels were more likely to use various kinds of health education services than their peers with lower education levels (OR = 1.64, CI:1.21–2.22; P < 0.05). Conclusion: Overall, urban-rural disparities in the utilization of public health services existed. Vulnerable with diabetes, such as those in rural areas, are less available to use diabetes public health services. Providing convenient health service infrastructure facilitates the utilization of basic public health services for diabetes in older patients with diabetes, especially in rural areas. There is growing interest in evaluating the quality of services provided. This has allowed government program and policy makers to identify bottlenecks and highlight requirements that lead to the optimization of public resources. Furthermore, the supply of public health services tends to be precarious in developing countries due to increased demand and diminished resources. Thus, this paper proposes an integrated approach to the Kano Model and the Balanced Scorecard perspectives for improving public hospital services. The investigation was carried out in a Brazilian public hospital. The hospital is known as a regional reference center in several medical specialties and one of the pioneers in total quality programs in public hospitals in the country. This study is original because it developed a new approach that integrates the BSC and the Kano Model for the context of health services. The association between these methodologies is useful for strategic planning and a better understanding by managers of the aspects that most impact patient satisfaction in the public hospital. This approach allows hospital public managers to assess the program's internal efforts to combat neglected diseases and improve patient adherence indicators for treatment. Char land is well-known as a hard-to-reach community in Bangladesh where the basic health services are very pitiable. Drawing an example from two char land communities in Bangladesh, this study explored the status of the public health services and service gaps. The study collected data from 222 respondents (heads of the households) and used a quantitative research approach where a face-to-face structured interview schedule was employed for data collection. Results showed that the char land family members were suffering different types of diseases, but they did not have adequate scientific treatment facilities; manpower and institutional facilities for health services were lacking against needs; and the quality of health services was awfully poor. All of the respondents mentioned a high service gap between demand and supply on health tests, specialized services and emergency services. Allergic rhinitis has been identified as a major respiratory disease that places a significant burden on patients and the healthcare system. Nevertheless, the management of allergic rhinitis is challenging for both patients and practitioners. Pharmacists have been recognised as strategic in providing advice for allergic avoidance, disease information, and pharmacological care for allergic rhinitis management. This role has been underutilised in the public health service sector in Malaysia due to variation in practice, regulation, and health system structures when compared to the international guidelines. This article proposed a PhaRmacISt-led Education Model (AR-PRISE) that includes explicit patient education materials and an algorithm for structured counselling by pharmacists in the management of patients with allergic rhinitis. Objectives: Collaboration and cooperation are critical for public health management. Nevertheless, collaboration and cooperation between government departments, as well as other entities, are still in a weak condition in China. This article analyzes the status of collaboration and cooperation in the management of public health services in China and explores the problems and gives strategic suggestions for strengthening collaboration and cooperation in the development of public health service systems in China, in order to provide a reference for improvement of public health management in the future. Methods: This study uses a qualitative case study approach, including documents review, in-depth interview, and focus group. Results: The main problems of collaboration and cooperation in public health service management in China include problems of effective collaboration and cooperation between institutions and relevant departments, public information platform, and implementation of public health and health promotion. On this basis, several relevant policy recommendations are put forward. Conclusions: Collaboration and cooperation are critical for the overall coordination and sustainable development of public health in China, and there is still work to be done in order to achieve appropriate cooperation and collaboration between different entities in the provision of public health services. 37. A Randomized-Controlled Trial of Mentalization-Based Treatment Compared With Structured Case Management for Borderline Personality Disorder in a Mainstream Public Health Service.

Background: Treatment of borderline personality disorder (BPD) in publicly funded mental health services generally use approaches based on medical interventions and generic case management. Specific psychological therapies developed for BPD may be more effective but have rarely been evaluated in routine clinical practice. Aim: To examine the effectiveness of Mentalization Based Treatment (MBT) in adults with an established diagnosis of BPD under the care of a publicly funded Mental Health Service (MHS), on rates of non-suicidal self-harm (NSSH) and attempted suicide (SA). Methods: A randomized, controlled trial (RCT) comparing 18 months of MBT with Enhanced Therapeutic Case Management (ETCM), a form of Structured Clinical Case Management (ICTRP: ACTRN12612000951853). Participants were adults recruited from a patient population under the care of a publicly funded mental health service (MHS) with a confirmed diagnosis of BPD. The primary outcome measures were the incidence of non-suicidal self-harm or suicide attempt over 18 months of treatment. Results: 72 participants (71 females, 1 male) were randomized to MBT (n = 38) or ETCM (n = 34). Both groups showed a significant reduction in the overall incident rate of SA and NSSH. Between groups, SA rates were higher in the MBT group and conversely NSSH rates were higher in the ECTM group. Conclusions: The introduction of a structured service that delivered a structured psychotherapy (MBT) and an effective case management approach (ETCM) both resulted in a reduction in SA and NSSH. Background: The implementation of evidence-based interventions for borderline personality disorder in community settings is important given that individuals with this diagnosis are often extensive users of both inpatient and outpatient mental health services. Although work in this area is limited, previous studies have identified facilitators and barriers to successful DBT implementation. This study seeks to expand on previous work by evaluating a coordinated implementation of DBT in community settings at a national level. The Consolidated Framework for Implementation Research (CFIR) (Damschroder et al., Implementation Sci. 4:50, 2009) provided structural guidance for this national level coordinated implementation. Methods: A mixed methods approach was utilised to explore the national multisite implementation of DBT from the perspective of team leaders and therapists who participated in the coordinated training and subsequent implementation of DBT. Qualitative interviews with DBT team leaders (n = 8) explored their experiences of implementing DBT in their local service and was analysed using content analysis. Quantitative surveys from DBT therapists (n = 74) examined their experience of multiple aspects of the implementation process including orienting the system, and preparations and support for implementation. Frequencies of responses were calculated. Written qualitative feedback was analysed using content analysis. Results: Five themes were identified from the interview data: team formation, implementation preparation, client selection, service level challenges and team leader role. Participants identified team size and support for the team leader as key points for consideration in DBT implementation. Key challenges encountered were the lack of system support to facilitate phone coaching and a lack of allocated time to focus on DBT. Implementation facilitators included having dedicated team members and support from management. Conclusions: The barriers and facilitators identified in this study are broadly similar to those reported in previous research. Barriers and facilitators were identified across several domains of the CFIR and are consistent with a recently published DBT implementation Framework (Toms et al., Borderline Personal Disord Emot Dysregul. 6: 2, 2019). Future research should pay particular attention to the domain of characteristics of individuals involved in DBT implementation. The results highlight the importance of a mandated service plan for the coordinated implementation of an evidence-based treatment in a public health service. Introduction: The purpose of the cross-sectional study was to explore the relationships between occupational and general-self efficacy, and perceived preparedness among Commissioned Corps officers in the United States Public Health Service (Commissioned Corps). Commissioned Corps officers fight to protect the United States from diseases and care for the survivors of natural disasters and terrorist attacks. Commissioned Corps officers play a vital role in the fight to protect the United States from diseases and care for the survivors of natural disasters and terrorist attacks. The Commissioned Corps provided healthcare services in Liberia during the 2014 Ebola crisis that underscored the challenges of emerging diseases in a globalized community. It is imperative that these health professionals maintain a high level of self-efficacy and feel confident in their overall preparedness training as they respond to public health emergencies.Materials and Methods: This study used assessment instruments derived from Albert Bandura's concept of self-efficacy to analyze the occupational and general self-efficacy, and perceived preparedness levels of health services officers in the Commissioned Corps. 82 Commissioned Corps officers completed the assessment survey. To date, no study has examined the relationship between these constructs in this population.Results: There was a statistically significant relationship between feeling confident in one's Commissioned Corps training and perceived preparedness (rs = 0.55, p < 0.001).Conclusion: This study reflects the training perceptions and self-beliefs of Commissioned Corps officers, fills an important gap in the empirical research in this population, and advances previous investigations, which suffered from an underrepresentation of female service members. Purposes: (1) Cluster analysis of psychiatric disorders and partitioning the youth; (2) determining socio-demographic correlates and parental histories for each one of the clusters; and (3) comparing clusters based on the extent and type of using psychotherapeutic services.Methods: The current cross-sectional study is a part of the first phase of PERSIAN Youth Cohort. The sample of the study includes 2991 participants aged 15-34 (27 ± 5.1 years, 55.6% female) from Ravansar district in western Iran. Enrollment and data collection for this phase were performed from October, 2014 to January, 2017. The data were collected through structured interviews, including the Composite International Diagnostic Interview (CIDI; version 2.1), mental health-related Sheehan Disability Scale, and Service Use Questionnaire. The obtained data were analyzed using two-step cluster analysis, multinomial logistic regression, and Chi-square test.Results: Our model proposed three clusters: a clinical cluster with significant mental disability; a healthy cluster with significant disability; and a healthy cluster with mild disability. There is a direct relationship between widow/divorced marital status and psychiatric maternal history with the clinical cluster (P < 0.05). Clinical and non-clinical clusters with medium to severe disability used services for mental health more often that the healthy cluster with mild functional disability (P < 0.05).Conclusions: The results of the study show that 28.7% of the youth in the general population of western Iran are suffering from psychiatric disorders and nearly two-thirds of the total population reported a medium-severe functional disability. Considering the wide range of mental disorders and the functional disability levels created by these disorders, cluster analysis could provide invaluable information regarding the partitioning of the youth population. China initiated the equalisation of an essential public health service programme in 2009 with the goal of developing a more equitable and effective public health system for all people. This study employs data from the China Migrants Dynamic Survey to examine regional-level and household-level income-related inequalities in public health service utilisation and its determinants. Wagstaff concentration indices indicate that essential public health services and family doctor contract services are concentrated among less developed prefectures and poorer households. Decomposition analysis based on recentered influence function regression shows that education contributes to pro-poor inequality in health records and health education utilisation. China's policies of essential public health services and family doctor contract services reduce income-related inequalities in health service utilisation, which has important implications for developing countries striving to achieve universal health service coverage and equal health outcomes. • First study measures and decomposes inequalities in public health services utilisation among migrants in China. • Pro-poor inequities in essential public health services and family doctor contract services. • The coverage of public health services for the migrants remains quite low. • Targeted interventions are required for specific public health services. BACKGROUND The conduct of research by clinicians is vital to maintain evidence-based practice and currency of clinician knowledge. However, the ability to conduct valid research is contingent on several factors including the knowledge of research ethics guidelines applicable within the researcher's location. AIMS To compare the actual and perceived knowledge of research ethics guidelines in a sample of healthcare clinicians in a public health service. METHODS A prospective cross-sectional design utilising an anonymous online survey. RESULTS Of the 432 valid responses, 8% were totally well calibrated on perceived and actual knowledge. A further 9% were totally miscalibrated on all items. The remaining 83% of respondents were variously miscalibrated on 1-4 items. The majority of miscalibrated responses represented overconfidence. CONCLUSIONS Clinicians are not as knowledgeable as they think about research ethics. This has implications for help-seeking behaviours within this population and highlights challenges for stakeholders in the provision of research support and education.

El Servicio Nacional de Asistencia Pública se creó en el Perú en el año 1912 como Servicio Nacional de Primeros Auxilios con cuatro secciones. En 1945 se inauguró el Puesto Central como Servicio Médico Quirúrgico de Emergencias y Primeros Auxilios. En 1951, se incorpora a médicos especialistas y estudiantes del último año de Medicina, ambulancias para atención domiciliaria y apertura puestos periféricos en varios distritos de Lima, El Callao y algunas capitales de departamento. La atención se efectuaba mediante equipos en turnos continuos de 24 horas incluyendo los domingos y feriados. En 1980 se desinstala el Servicio Nacional de Salud Pública y se migra a un modelo centralizado en el Hospital de Emergencias José Casimiro Ulloa. El artículo fundamenta la utilidad de recobrar el modelo del Sistema Nacional de Asistencia Pública, entendido como un componente funcional del Sistema de Salud, que fortalecerá el primer nivel de atención en salud.

Umweltbezogene Gesundheitsvorsorge wird zunehmend ein Thema des öffentlichen Gesundheitsdienstes. Um die gesundheitlichen Auswirkungen des Klimawandels angehen zu können, bedarf es ämterübergreifender Zusammenarbeit. Dies benötigt Voraussetzungen sowohl auf Seiten der Beschäftigten als auch auf Seiten der Verwaltungsstruktur. Das theoretische Konzept des Health in all Policies bildet die Grundlage für das Handeln. Am Beispiel der Stadt Dresden wird beschrieben, wie dies gelingen kann. Environmental health care will increasingly become a public health service issue. In order to tackle the health effects of climate change, intersectoral cooperation is needed. This requires certain conditions both on the employee level and on the level of the structure. The theoretical approach of health in all policies forms the basis for action. Indian health sector is a complex admixture of public and private providers, but several lacunae of public health service delivery system in terms of human resource, access, and quality have resulted in unprecedented growth of private sector. Although seamless delivery of services at rural and urban areas, timely approach, and improved information technology system are the strength of the private sector, this package comes at the cost of high out-of-pocket expenditure. Mobilization of private sector health workforce by their capacity building in terms of orientation toward public health services is one of the available solutions to move toward wider coverage of public health services.
