While the concepts of race and ethnicity have been abused historically, they are potentially invaluable in epidemiology and public health. Epidemiology relies upon variables that help differentiate populations by health status, thereby refining public health and health care policy, and offering insights for medical science. Race and ethnicity are powerful tools for doing this. The prerequisite for their responsible use is a society committed to reducing inequalities and inequities in health status. When this condition is met, it is irresponsible not to utilize these concepts.

Several UK medical schools have reorganized academic public health in 'schools of public health' or 'health sciences'. This paper considers the implications for the United Kingdom of the US concept of schools of public health independent of medical schools. These schools provide multidisciplinary research environments with scientists from laboratory and population-based disciplines competing successfully for federal research funds on projects ranging from basic science (e.g. biochemistry or genetics) to public health policy. Few public health academics see the expenditure of a trillion dollars per year on health care as a priority for public health. These schools have excelled in research, but not in leadership of service public health, so academic and service public health goals have diverged. In seeking the research advantages of the US system, the strengths of the UK one, including the relatively close links between public health and medicine and academia and service, together with a focus on applied work, should not be lost.

Objectives: Smokeless tobacco (SLT) is an addiction resulting in serious health problems including cancers. The social context around SLT use among South Asians was reviewed to help inform interventions for its prevention and cessation. Study design: Systematic review. Methods: Electronic databases were searched to identify studies examining the social context of SLT use. As heterogeneous qualitative, quantitative and mixed method studies were included, meta-analysis was not appropriate. Results: Of 428 studies identified, 17 were reviewed. These studies were conducted in India, Nepal, Pakistan and the UK between 1994 and 2009. SLT use among South Asians was culturally widely acceptable due to its association with socializing, sharing and family tradition (100% in Anwar et al.'s study). Other reasons for use were addiction, easy accessibility, low cost and lack of prohibitive legislation. SLT users had limited awareness of its association with oral cancer (29.3% in Ahmed et al.'s study); however, there was a distinct lack of knowledge regarding other health effects, such as cardiovascular disease (0.85%). Users attempted to quit (32.7% in Prabhu et al.'s study) but success was low (8.2%). Conclusions: Cessation programmes for South Asians should address cultural acceptance, limited knowledge of health effects, inadequate legislation and controls, scarce social support and insufficient SLT cessation services. The 3-yearly World Congress of Epidemiology is the premier, international, scientific conference organised under the auspices of the International Epidemiological Association (in open competition). This paper explores the justification for seeking to host the Congress and reflects on the structures and processes adopted in making the XIXth Congress in Scotland happen. Preparing the bid was invaluable for forming collaborations, generating scientific ideas, and garnering opinion. After the bid was accepted, we formed a local organising committee, named the Management Executive Committee to signal its decision making authority; and scientific, fundraising, marketing, international and social subcommittees. There was uncertainty about critical matters such as delegate numbers, costs and the total budget. Early decisions had to be made on, for example, the fee and fundraising target (~250,000), despite financial risks. Development of the scientific programme was a critical step that under-pinned fundraising and marketing and permitted involvement of the international committee. Overall the 2011 WCE succeeded. The key ingredients to success were: a large collaboration of institutions and individuals; early pledges of financial support mostly from the UK; the valuable and relevant experience of the professional conference organisers; unstinting support and advice from IEA; and the effectiveness of the committee structure. The educational and professional development benefits of this WCE will reach a worldwide community and not just delegates, because of video, PowerPoint and textual accounts being open access on the Internet. This reach is unprecedented for IEA's World Congresses. We anticipate that the Congress will translate into better public health practice, better future Congresses, advances in epidemiology and improved population health. [ABSTRACT FROM AUTHOR]

This paper briefly recaps the role of migration in creating multi-ethnic societies, the main types of migration and/or ethnicity studies, and the power and potential of such studies. Next, the paper examines whether opportunities are being grasped globally, and whether information systems exist to grasp these opportunities. The paper concludes that there are massive global inequalities in the use of migration status and ethnicity as epidemiological variables, to the detriment of global health. Background: European public health systems are converging, particularly in relation to communicable disease control. This process requires mutual learning through comparison; this was undertaken for Scotland (population 5.1 million) and Hungary (population 10.5 million). Methods: Using the official web- and paper-based publications, the practice of communicable disease control was compared between the two countries in three specific fields: seasonal influenza surveillance; human immunodeficiency virus (HIV) surveillance; and the childhood vaccination system. Results: The organization structure for communicable disease control was very similar, comprising of government, national, regional and sub-regional tiers in Hungary, and government, national and local (sub-regional) tiers in Scotland. The influenza surveillance system in both countries was mainly based on the 'fluspotter system'. In the 2005/6, 2006/7 and 2007/8 seasons, there was no exceptional influenza activity in either country. Although the data collection and surveillance system of HIV is similar, there was a massive difference in the number of reported cases. In 2007, the cumulative incidence of reported HIV cases was 14.74/ 100,000 in Hungary and 105.21/100,000 in Scotland. The routine childhood vaccination schedule is similar in the two countries. However, while the vaccine uptake rates were nearly 100% in Hungary, these rates were lower in Scotland. The numbers of reported pertussis (98 vs 48), mumps (2741 vs 16), rubella (146 vs 0) and measles (168 vs zero) cases were significantly higher in Scotland than in Hungary. There were no differences for polio and chickenpox. Conclusions: The economic difference between the two countries not reflected in the efficiency of communicable disease control and in communicable disease patterns. Definition of Chinese. For the purposes of this study, Chinese refers to residents of the UK who either on the basis of name (face-to-face contact not made), or self-definition and appearance (contact made) had origins in China and included those born in the UK and others migrating to the UK via a third country (for example Vietnam, Singapore, Hong Kong etc). This is a pragmatic definition. There is a paucity of research on health in the UK Chinese community partly due to the difficulties of identifying and accessing study populations. For a survey of cardiovascular disease we aimed to identify and recruit all Chinese adults aged 25–64 y living in Newcastle-upon-Tyne, UK. One thousand, eight hundred and sixty-five potential subjects were identified using a variety of methods. Of the 1702 potential subjects identified from a name analysis of the 1991 FHSA register (FHSA group), 638 students in halls of residence were excluded and the remaining 1064 were invited to participate. Non-respondents were followed up. Of the 1064, 658 (65.5%) addresses were no longer valid, 21 (2%) were reclassified as non-Chinese and no contact was made with 18 individuals (1.6%). A further 163 subjects (non-FHSA group) came forward in response to publicity, giving a total of 530 Chinese actually identified in Newcastle. Three hundred and eighty subjects took part in the study. Compared to the 1991 Census, the recruitment procedure underestimated the total population size, particularly for men and younger ages. In the FHSA group, men were significantly more likely to be current drinkers, and women were more likely to smoke and have a lower educational attainment that the non-FHSA group. There were no other important differences in the distribution of CHD risk markers in the two groups. Our experience indicates that the FHSA register is suitable for identifying Chinese but should be used alongside other complementary methods to augment samples for ethnicity and health research.
