Virtual Learning Environments have become innovative tools in health professionals education. Through Massive Open Online Courses, they enable different ways of connecting with knowledge, facilitating study autonomy, interaction, and closer alignment with professional practices and the context of course participants. MOOCs comprise an educational strategy for many fields, including health. As they educate health professionals about a variety of practices, MOOCs play a crucial role in interculturality by enabling professionals to approach cultural diversity in work settings. This study provides a review of the literature investigating the element of interculturality in the production of healthcare-related MOOCs intended for a variety of audiences, including practicing healthcare professionals, healthcare professional trainees, and the general public. Based on a systematic review protocol, we searched for peer-reviewed studies published between 2016 and 2023 in Science Direct, PubMed, ERIC, and Scopus. Fifteen studies were selected for final analysis, which highlighted MOOC development, its underlying processes, and its importance in promoting health and social well-being. MOOCs have (1) provided new approaches to technology-mediated learning in distance health education, (2) aided training, (3) disseminated knowledge, and (4) promoted interculturality. Continuous collaboration and innovation in MOOC development are essential to ensure their effectiveness and relevance in the contemporary educational scenario.

Abstract Introduction Interpersonal violence is a phenomenon that can occur with different people and conditions. However, people with intellectual disabilities have increased vulnerability to this problem, with potential risks to their health and well-being. The aim of this study was to identify the sociodemographic characteristics of people with disabilities who have been victims of interpersonal violence, the profile of the perpetrators and the measures taken after the victims have been cared for. Methods This is an exploratory, descriptive, cross-sectional study using the Interpersonal Violence Notification Forms entered into the Brazilian Ministry of Health’s Notifiable Diseases Information System. The city of São Paulo was chosen as the setting because it is the largest city in Latin America and has a faster data processing system than other cities. The period covered notifications made between 2016 and 2022. The information was collected between October and November 2023 and a univariate statistical analysis was carried out. Fisher’s exact test was used, with a significance level of 5% (α = 0.05). Results There were 4,603 notifications against people with intellectual disabilities in the period. The forms of physical violence, neglect/abandonment and psychological/moral violence were more frequent in the 15–19 age group, while sexual violence was more frequent in the 10–14 age group (p

Introduction: Literacy of health professionals on patient safety aims to prevent and reduce risks and adverse events in healthcare. Objective: The objective of this study was to translate, adapt, and validate for Portugal of the World Health Organization Questionnaire to Assess the Implementation of the Multiprofessional Guide. Methodology: Following the cultural adaptation process, we carried out a psychometric analysis on a sample of 300 health professionals. The scale was tested for apparent and content validity. Psychometric properties were assessed using Cronbach’s alpha coefficient (α) through exploratory and confirmatory factor analysis, convergent validity, and discriminant validity. Results: Exploratory factor analysis led to a tetra factor structure that accounted for 43.0% of the variance and had an overall alpha of 0.759. The hypothesized structure was submitted to confirmatory factor analysis, and the following items were eliminated from factors 1, 2, and 3 and the respective items: 7; 5; and 3, 4, 5, and 6, due to multicollinearity problems. Overall good-of-fit indexes are reliable. Conclusions: The “KIT TOOL-S2 TEXT” scale presents a factor structure with satisfactory validity and reliability results, adequately representing the constructs in question.

Summary: Background: Nutri-Score is a scientifically validated 5-color front-of-pack nutrition label reflecting the nutrient profile of foods. It has been implemented in several European countries on a voluntary basis, pending the revision of the European labeling regulation. Hence, scientific evidence is needed regarding the ability of the nutrient profile underlying the Nutri-Score (uNS-NPS, 2023-updated version) to characterize healthier foods. Our objective was therefore to study the prospective association between the nutritional quality of diet characterized by the uNS-NPS and the risk of cardiovascular diseases in a large European population. Methods: Our analyses included 345,533 participants from the European Prospective Investigation into Cancer and Nutrition study (EPIC, 1992–2010, 7 European countries). Food intakes were assessed at baseline using country-specific dietary questionnaires. The uNS-NPS was calculated as a continuous scale for each food, based on its 100 g content of energy, sugars, saturated fatty acids, salt, fibre, and protein and percentage content of fruit, vegetables, and pulses. A dietary index was derived at the individual level (uNS-NPS DI: energy-weighted mean of uNS-NPS scores of all foods consumed by a participant). Cardiovascular events during follow-up were retrieved using country-specific methods (self-report, registry data). Multi-adjusted Cox models were computed. Findings: Overall, 16,214 first cardiovascular events were reported (median follow-up: 12.3 years; 4,103,133 person-years). The consumption of foods with a higher uNS-NPS score (reflecting a lower overall nutritional quality of diet) was associated with higher risks of total cardiovascular events (Hazards Ratio (HR) for an increment of 1 standard deviation: 1.03 (95% Confidence Interval 1.01–1.05)), especially myocardial infarction (HR = 1.03 (1.01–1.07)), and stroke (HR = 1.04 (1.01–1.07)). Interpretation: In this large prospective study among European adults, a higher risk of cardiovascular diseases (total and several subtypes) was observed in individuals consuming a diet with a lower nutritional value, as graded by the uNS-NPS score. This brings new evidence on the relevance of the updated nutrient profile underlying the Nutri-Score to characterize foods with a healthier nutrient profile. Summary: Background: This study addresses the rising burden of type 2 diabetes mellitus, and explores the potential of teleconsultation, as an alternative for diabetes management. The primary objective was to test the hypothesis that teleconsultation is non-inferior to face-to-face consultation in terms of glycaemic control measured as glycated haemoglobin (HbA1c) (non-inferiority margin for the upper confidence interval for the difference between groups of 0,5% in HbA1c) for type 2 diabetes mellitus patients referred from Primary Healthcare to Specialized Care within the SUS. Methods: TELECONSULTA, is a randomized, pragmatic, phase 2, single-centre, open-label, non-inferiority trial conducted in Joinville, Brazil. A total of 278 participants diagnosed with type 2 diabetes were randomized through mandatory teleconsulting services from primary care health units. The randomization was 1:1 to teleconsultation or face to face consultation. The study was registered at the Brazilian Clinical Trial Register—REBEC, under the code RBR-8gpgyd. Study status is “Completed”. Findings: This study included 278 participants in the intention-to-treat (ITT) analysis. The median age was 61 (54–68) years, 167 (60%) were women. The between-groups comparative average reduction in HbA1c was −0.6% (90% CI −1.0; −0.1) at 3-months and −0.5% (90% CI −0.9; 0.0) at 6-months in Modified Intention-to-Treat (mITT) population with imputed data, showing the non-inferiority of teleconsultation. Results with no missing data imputation and in the per protocol population were similar. The frequency of hypoglycaemia and other adverse events was well balanced between groups. Interpretation: The results underscore the transformative potential of telemedicine in addressing the complexities of diabetes management within the framework of a universal healthcare system, contributing with valuable insights for healthcare policymakers and practitioners seeking innovative solutions to tackle the growing diabetes epidemic. Abstract Introduction Climate change has a disproportionate impact on women in comparison to men, and women have a key role to play in climate adaptation. However, evidence is lacking on how gender inequalities may be associated with climate vulnerability and ability to respond at country level. Methods This ecological study investigated the association between climate adaptation, measured by the Notre Dame Global Adaptation Initiative Country Index (ND-GAIN), and gender equality, measured by the Global Gender Gap Index (GGGI) developed by the World Economic Forum and the Gender Inequality Index (GII) developed by the United Nations. Simple linear regression was used to estimate the associations between the indices and their subdomains for 146 countries. Results There was an approximately linear association between the GGGI and climate adaptation. Each 1% increase in gender equality was associated with a 0.6% increase in the ND-GAIN score (the slope was 0.59, with a 95% confidence interval [0.33 to 0.84]). This was driven by a negative association between gender equality and vulnerability (-0.41 [-0.62 to -0.20]), and a positive association between gender equality and readiness (0.77 [0.44 to 1.10]). The strongest associations between gender equality and climate adaptation were observed for the education domain of the GGGI. There was a strong negative linear association between the GII and climate adaptation, which explained most (86%) of the between-country variation in climate adaptation. Each 1% increase in gender inequality was associated with a 0.5% decrease in the ND-GAIN score (-0.54 [-0.57 to -0.50]). The association between gender inequality and readiness was stronger than the association with vulnerability (0.41 [0.37 to 0.44] for vulnerability versus − 0.67 [-0.72 to -0.61] for readiness). Conclusions Gender inequality, measured broadly across different domains of life, is associated with climate adaptation at country level, both in terms of vulnerability to impact and readiness to respond.

Objective: To compare the frequency of underweight and obesity among previously hospitalized older adults and analyze their association with malnutrition, sarcopenia, frailty, inflammatory markers, and adverse outcomes both during hospitalization and after discharge. Methods: This secondary analysis of a prospective study, conducted at Hospital das Clínicas da Universidade Federal de Pernambuco, Brazil, included hospitalized older patients (age ≥ 60 y). Nutritional status, body composition, sarcopenia, frailty, and outcomeswere assessed. Cox regression was performed to evaluate the impact of the body mass phenotypes on clinical outcomes. Results: This secondary analysis included one hundred patients. The prevalence of obesity was 22.10%, while that of underweight was 34.60%. Individuals with underweight had a higher frequency of weaker immune response, worse inflammatory profile, higher nutritional risk, higher frequency of sarcopenia and malnutrition, longer hospital stay, and a higher incidence of mortality when compared to those with obesity. Being underweight was independently associated with higher mortality rates, even after adjustment for age, sex, muscle mass, malnutrition, and diagnosis of malignancy [adjusted HR = 2.82 (95% confidence interval 1.03 – 7.72), p = 0.044]. Conclusions: The underweight phenotype represented a worst-case scenario in hospitalized older patients.

Objective: To evaluate frailty and its relationship with prognostic markers in hospitalized patients with acute coronary syndrome. Methods: This cross-sectional study with a prospective variable analysis (prognostic markers) involved adults of both sexes aged ≥ 50 years with acute coronary syndrome. Patients with ≥ 3 of the following criteria wereconsidered frail: 1) unintentional weight loss; 2) exhaustion (assessed by self-reported fatigue); 3) low handgrip strength; 4) low physical activity level; and 5) low gait speed. The included prognostic markers were: metabolic changes (lipid and glycemic profile), changes in inflammatory status (C-reactive protein), thrombolysis in myocardial infarction risk score, troponin level, angioplasty or surgery, hospitalization in the intensive care unit, length of hospital stay, and hospital outcome. Results: The sample consisted of 125 patients, whose mean agewas 65.5 (SD, 8.7) years. The prevalence of frailty was 48.00%, which was higher in women (PR = 1.55; 95%CI 1.08-2.22; p = 0.018) and patients with systemic arterial hypertension (PR = 2.18; 95%CI 1.01-5.24; p = 0.030). Frailty was not associated with age, cardiac diagnosis, or prognostic markers (p > 0.05). Conclusions: Frailty was highly prevalent in patients with acutecoronary syndrome, affecting almost half of the sample, particularly women and patients with hypertension, irrespective of age. However, despite its high prevalence, frailty was not associated with markers of metabolic change or poor prognosis.

Resumo: Trata-se de uma pesquisa documental, exploratória, descritiva, partindo de um estudo multicêntrico e internacional entre Brasil, Espanha, Itália e Portugal sobre sistemas nacionais de saúde com modelo de atenção baseado na atenção primária à saúde e financiado pelo Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq) do Brasil. Tem como objetivo identificar as legislações de base da saúde, o direito à saúde e os princípios doutrinários e organizativos de cada país selecionado com ênfase no impacto dos determinantes sociais de saúde sobre os sistemas nacionais de saúde. Os resultados revelaram países com legislações e princípios doutrinários semelhantes, com direito à saúde constitucional, ancorados na atenção primária à saúde, e com modelo assistencial de acesso do tipo saúde da família. Os desafios encontrados foram a baixa natalidade e elevada expectativa de vida ao nascer em países europeus e critérios para acesso a medicamentos e financiamento assistencial. Com base nos nossos achados, os países que tiveram maior investimento em base estrutural, perpassando por assegurar condições socioeconômicas e sanitárias mais dignas, sólidas e vigilantes, garantiram importante diferenciação na capacidade de resposta e sustentabilidade do sistema nacional de saúde e no impacto direto na qualidade de vida das pessoas.

Resumo Introdução: Introduzido nas interpretações sobre as perspectivas de expansão da epidemia de HIV/aids na década de 1990, o conceito de vulnerabilidade procura articular a dimensão individual de vulnerabilidade ao HIV aos contextos sociais e programáticos, muitas vezes marcados pela desigualdade e injustiça social. Objetivo: Analisar os usos da concepção multifatorial (individual, social, programática) da vulnerabilidade nos estudos sobre mulheres e HIV/aids, de 1990 a 2018, publicados no Scielo. Método: A partir da revisão de escopo da literatura nacional, 39 estudos selecionados foram classificados quanto ao uso do conceito de vulnerabilidade em: uso consistente, parcial ou inconsistente. Resultados: Os principais aspectos associados à vulnerabilidade das mulheres à aids foram: dificuldade de negociação do uso do preservativo, dimensão social e simbólica do casamento e da maternidade, significados diferenciados da sexualidade entre os gêneros e fragilidade dos direitos sexuais e reprodutivos. Observou-se lacunas quanto à articulação dos três componentes do conceito e a escassez de avaliações de intervenções orientadas pelo referencial da vulnerabilidade. Conclusão: Os achados indicam o potencial de contribuição do conceito na análise da sindemia entre HIV, desigualdades sociais, raciais, de gênero e demais problemas de saúde, como a pandemia de Covid-19.

ABSTRACT Objective: To investigate the association between the dimensions of the Health Belief Model (HBM) and complete vaccination for hepatitis B among healthcare workers (HCW). Methods: Cross-sectional epidemiological study with HCW in Primary Health and Medium Complexity Care. Univariate and bivariate analyses were performed to test the association between the outcome variable (complete vaccination for hepatitis B based on self-report) and the variables of the HBM dimensions. Prevalence ratio (PR) and its respective 95% confidence intervals (95%CI) were calculated. Results: 453 HCW participated. The prevalence of complete vaccination for hepatitis B was 56.9%. In the final analysis model, the following variables were associated with complete vaccination for hepatitis B: chances of having hepatitis B (PR=1.73) – related to the susceptibility dimension; disease severity (PR=0.74) – related to severity; reduced risk of absenteeism (PR=1.29) – related to benefits; not spending time to get vaccinated (PR=1.41) and not worrying about Events Supposedly Attributable to Vaccination or Immunization (PR=1.43) – related to barriers. Conclusions: The completeness of the hepatitis B vaccination schedule, reported by the investigated HCW, reveals the prevalence is below the target established by the Ministry of Health, which follows the national scenario of low coverage presented for other age groups. Understanding the risk perception and severity of hepatitis B can contribute to increasing the prevalence of vaccination for this infection.

Objetivo: Avaliar as potencialidades e desafios existentes no cuidado a usuários de substâncias psicoativas em uma Estratégia de Saúde da Família através da percepção dos profissionais. Método: Estudo de caso qualitativo, realizado por meio da Avaliação de Quarta Geração, desenvolvido em 2023, numa Estratégia de Saúde da Família de um município do Rio Grande do Sul, Brasil, com 19 profissionais. Foram utilizadas técnicas de etnografia prévia, entrevistas com aplicação do círculo hermenêutico-dialético, para análise dos dados utilizou-se o método comparativo constante. Resultados: São potencialidades o comprometimento da equipe e matriciamento. Os desafios são as dificuldades dos vínculos com os usuários, os medos, os estigmas, os despreparos da equipe, a falta de estrutura física e profissionais. Conclusão: Este estudo contribuiu para o desvelamento dos desafios encontrados no cuidado à usuários de drogas e pretende fomentar um atendimento voltado para o acolhimento e integralidade do sujeito.

Objetivo: Evaluar las potencialidades y desafíos en la atención a usuarios de sustancias psicoactivas en una Estrategia de Salud Familiar a través de la percepción de los profesionales. Método: Estudio de caso cualitativo, realizado utilizando la Evaluación de Cuarta Generación, desarrollada en 2023, en una Estrategia de Salud Familiar en un municipio de Rio Grande do Sul, Brasil, con 19 profesionales. Se utilizaron técnicas etnográficas previas, así como entrevistas con la aplicación del círculo hermenéutico-dialéctico, y se empleó el método comparativo constante para analizar los datos. Resultados: El compromiso del equipo y el apoyo de la matriz son potencialidades. Los retos son las dificultades para establecer vínculos con los usuarios, los miedos, los estigmas, la falta de preparación por parte del equipo, la falta de estructura física y de profesionales. Conclusión: Este estudio ha contribuido a desvelar los retos a los que se enfrenta la atención a los consumidores de drogas y pretende promover una atención orientada a acoger e integrar al individuo.

Objective: To assess the potential and challenges in caring for users of psychoactive substances in a Family Health Strategy through the perception of professionals. Method: A qualitative case study was carried out using the Fourth Generation Evaluation, developed in 2023, in a Family Health Strategy in a municipality in Rio Grande do Sul, Brazil, with 19 professionals. Previous ethnographic techniques were used, interviews were conducted using the hermeneutic-dialectic circle, and the constant comparative method was used to analyze the data. Results: Team commitment and matrix support are potentialities. The challenges are difficulties establishing bonds with users, fears, and stigmas, the team's lack of preparation, and the lack of physical structure and professionals. Conclusion: This study has helped to uncover the challenges encountered in caring for drug users and aims to promote care that embraces and integrates the individual.

BackgroundThe COVID-19 pandemic represented a great stimulus for the adoption of telehealth and many initiatives in this field have emerged worldwide. However, despite this massive growth, data addressing the effectiveness of telehealth with respect to clinical outcomes remain scarce. ObjectiveThe aim of this study was to evaluate the impact of the adoption of a structured multilevel telehealth service on hospital admissions during the acute illness course and the mortality of adult patients with flu syndrome in the context of the COVID-19 pandemic. MethodsA retrospective cohort study was performed in two Brazilian cities where a public COVID-19 telehealth service (TeleCOVID-MG) was deployed. TeleCOVID-MG was a structured multilevel telehealth service, including (1) first response and risk stratification through a chatbot software or phone call center, (2) teleconsultations with nurses and medical doctors, and (3) a telemonitoring system. For this analysis, we included data of adult patients registered in the Flu Syndrome notification databases who were diagnosed with flu syndrome between June 1, 2020, and May 31, 2021. The exposed group comprised patients with flu syndrome who used TeleCOVID-MG at least once during the illness course and the control group comprised patients who did not use this telehealth service during the respiratory illness course. Sociodemographic characteristics, comorbidities, and clinical outcomes data were extracted from the Brazilian official databases for flu syndrome, Severe Acute Respiratory Syndrome (due to any respiratory virus), and mortality. Models for the clinical outcomes were estimated by logistic regression. ResultsThe final study population comprised 82,182 adult patients with a valid registry in the Flu Syndrome notification system. When compared to patients who did not use the service (n=67,689, 82.4%), patients supported by TeleCOVID-MG (n=14,493, 17.6%) had a lower chance of hospitalization during the acute respiratory illness course, even after adjusting for sociodemographic characteristics and underlying medical conditions (odds ratio [OR] 0.82, 95% CI 0.71-0.94; P=.005). No difference in mortality was observed between groups (OR 0.99, 95% CI 0.86-1.12; P=.83). ConclusionsA telehealth service applied on a large scale in a limited-resource region to tackle COVID-19 was related to reduced hospitalizations without increasing the mortality rate. Quality health care using inexpensive and readily available telehealth and digital health tools may be delivered in areas with limited resources and should be considered as a potential and valuable health care strategy. The success of a telehealth initiative relies on a partnership between the involved stakeholders to define the roles and responsibilities; set an alignment between the different modalities and levels of health care; and address the usual drawbacks related to the implementation process, such as infrastructure and accessibility issues.

Introdução: A segurança do doente é uma questão séria de saúde pública global, focada num sistema de prestação de cuidados que previne erros e é construído sobre uma cultura de segurança que envolve todos os profissionais de saúde, organizações e doentes. Objetivo: Avaliar o nível de literacia dos profissionais de saúde em relação à segurança do doente num centro hospitalar na região central de Portugal. Métodos: Foi realizado um estudo quantitativo, descritivo-correlacional e transversal para avaliar o nível de literacia dos profissionais de saúde em relação à segurança do doente. Uma amostra de 300 profissionais de um Centro Hospitalar na região central de Portugal respondeu ao "Questionário para Avaliar a Implementação do Guia Multiprofissional" da Organização Mundial de Saúde. A maioria dos profissionais de saúde incluídos no estudo são do sexo feminino (76,3%), encontram-se na faixa etária (dos 36-50 anos) 50,3% e são maioritariamente enfermeiros (90%). Resultados: 44,0% dos inquiridos demonstraram baixa literacia na dimensão "Erro e segurança do doente". De forma semelhante, 44,7% apresentaram baixa literacia na dimensão "Segurança do Sistema de Saúde". Quanto à dimensão "Influência Pessoal na Segurança", mais de metade da amostra (52,7%) é altamente letrada. Por último, a maioria dos inquiridos mostrou baixa literacia (54,7%) na dimensão "Atitudes Pessoais em relação à Segurança do Doente". Conclusão: Os resultados apontam existir lacunas por parte dos profissionais de saúde sobre literacia na segurança do doente, com predominância na dimensão "Atitudes Pessoais em Relação à Segurança do Doente". Assim, é essencial que os profissionais de saúde recebam mais formação em segurança do doente para garantir a confiança dos cidadãos nas suas práticas profissionais e na confiança no sistema de saúde.

Abstract Introduction Socioeconomic disparities have been shown to correlate with perinatal mortality and the incidence of type 2 diabetes. Few studies have explored the relationship between deprivation and the incidence of gestational diabetes (GDM). We aimed to identify the relationship between deprivation and incidence of GDM, after adjusting for age, BMI, and ethnicity. We also examined for relationships between deprivation and perinatal outcomes. Methods A retrospective cohort analysis of 23,490 pregnancies from a major National Health Service Trust in Northwest London was conducted. The 2019 English Indices of Multiple Deprivation was used to identify the deprivation rank and decile for each postcode. Birthweight centile was calculated from absolute birthweight after adjusting for ethnicity, maternal height, maternal weight, parity, sex and outcome (live birth/stillbirth). Logistic regression and Kendall’s Tau were used to identify relationships between variables. Results After controlling for age, BMI & ethnicity, Index of Multiple Deprivation postcode decile was not associated with an increased risk of developing gestational diabetes. Abstract Objective: We examined whether associations between the food environment, frequency of home cooking, diet quality and BMI were modified by the level of cooking skills. Design: Cross-sectional study using linear and modified Poisson regression models adjusted for age, sex, energy intake, education, income, household size and urbanisation. The frequency of home cooking was categorised into 0·1). Conclusions: Exposure to food stores was associated with a higher diet quality and exposure to restaurants with a lower BMI. Better cooking skills were associated with a higher frequency of home cooking and better diet quality but did not modify associations with the food environment. Future studies should explore different approaches to understand how individuals interact with their food environment.

ABSTRACT OBJECTIVE To present the results of a cost analysis of remote consultations (teleconsultations) compared to in-person consultations for patients with type 2 diabetes, in the Brazilian public healthcare system (SUS) in the city of Joinville, Santa Catarina (SC). In addition to the costs from the local manager’s perspective, the article also presents estimates from the patient’s perspective, based on the transportation costs associated with each type of consultation. METHOD Data were collected from 246 consultations, both remote and in-person, between 2021 and 2023, in the context of a randomized clinical trial on the impact of teleconsultation carried out in the city of Joinville, SC. Teleconsultations were carried out at Primary Health Units (PHU) and in-person consultations at the Specialized Health Center. The consultation costs were calculate by the method time and activity-based costing (TDABC), and for the estimate of transportation costs data was collected directly from the research participants . The mean costs and time required to carry out each type of consultation in different scenarios and perspectives were analyzed and compared descriptively. RESULTS Considering only the local SUS manager’s perspective, the costs for carrying out a teleconsultation were 4.5% higher than for an in-person consultation. However, when considering the transportation costs associated with each patient, the estimated value of the in-person consultation becomes 7.7% higher and, in the case of consultations in other municipalities, 15% higher than the teleconsultation. CONCLUSION The results demonstrate that the incorporation of teleconsultation within the SUS can bring economic advantages depending on the perspective and scenario considered, in addition to being a strategy with the potential to increase access to specialized care in the public network.

BackgroundThe rapidly increasing availability of medical data in electronic health records (EHRs) may contribute to the concept of learning health systems, allowing for better personalized care. Type 2 diabetes mellitus was chosen as the use case in this study. ObjectiveThis study aims to explore the applicability of a recently developed patient similarity–based analytics approach based on EHRs as a candidate data analytical decision support tool. MethodsA previously published precision cohort analytics workflow was adapted for the Dutch primary care setting using EHR data from the Nivel Primary Care Database. The workflow consisted of extracting patient data from the Nivel Primary Care Database to retrospectively generate decision points for treatment change, training a similarity model, generating a precision cohort of the most similar patients, and analyzing treatment options. This analysis showed the treatment options that led to a better outcome for the precision cohort in terms of clinical readouts for glycemic control. ResultsData from 11,490 registered patients diagnosed with type 2 diabetes mellitus were extracted from the database. Treatment-specific filter cohorts of patient groups were generated, and the effect of past treatment choices in these cohorts was assessed separately for glycated hemoglobin and fasting glucose as clinical outcome variables. Precision cohorts were generated for several individual patients from the filter cohorts. Treatment options and outcome analyses were technically well feasible but in general had a lack of statistical power to demonstrate statistical significance for treatment options with better outcomes. ConclusionsThe precision cohort analytics workflow was successfully adapted for the Dutch primary care setting, proving its potential for use as a learning health system component. Although the approach proved technically well feasible, data size limitations need to be overcome before application for clinical decision support becomes realistically possible.

Objective: To understand the meaning of the crisis phenomenon and the motivations and expectations of workers in the care of people in crisis in a Psychosocial Care Center for Alcohol and Drugs (CAPSad). Method: qualitative research, based on Alfred Schutz's social phenomenology, was carried out with 14 workers in January and February 2022 in the municipality of Porto Alegre, RS, Brazil. The interviews were subjected to phenomenological analysis. Results: the crisis is reported as a time of suffering, with a potential for transformation, covering subjective, social, and family aspects. Effective crisis care involves verbal management, bonding, and teamwork. The expectations following the care of a person in crisis are aimed at the well-being, reduction of suffering, and protection of the user. Conclusion: It is essential to break away from the biological model in the approach to users, overcoming stigmas and strengthening comprehensive care.

Objetivo: Comprender el significado del fenómeno de la crisis, las motivaciones y expectativas de los trabajadores en la atención a personas en crisis en un Centro de Atención Psicosocial de Alcohol y Drogas (CAPSad). Método: investigación cualitativa, basada en la fenomenología social de Alfred Schutz, realizada con 14 trabajadores en enero y febrero de 2022 en el municipio de Porto Alegre, RS-BR. Las entrevistas se sometieron a un análisis fenomenológico. Resultados: la crisis se describe como un momento de sufrimiento, con un potencial de transformación, que abarca aspectos subjetivos, sociales y familiares. Una atención eficaz en caso de crisis implica la gestión verbal, la vinculación y el trabajo en equipo. Las expectativas tras la atención a una persona en crisis tienen como objetivo el bienestar, la reducción del sufrimiento y la protección del usuario. Conclusión: es imprescindible romper con el modelo biologicista en el abordaje de los usuarios, superando estigmas y reforzando la atención integral.

Método: pesquisa qualitativa, sob o referencial da fenomenologia social de Alfred Schutz, realizado com 14 trabalhadores no período de janeiro e fevereiro de 2022 no município de Porto Alegre, RS-BR. As entrevistas foram submetidas à análise fenomenológica. Resultados: a crise é relatada como um momento de sofrimento, com um potencial de transformação, que abrange aspectos subjetivos, sociais e familiares. O cuidado eficaz da pessoa em crise envolve o manejo verbal, o vínculo e o trabalho em equipe. As expectativas após o atendimento de uma pessoa em crise visam o bem-estar, a redução do sofrimento e a proteção do usuário. Conclusão: mostra-se essencial a ruptura do modelo biologicista na abordagem ao usuário com superação de estigmas e fortalecimento do cuidado integral.

Introduction Scaling up interventions targeting non-communicable diseases (NCDs) is a global health priority, and implementation research can contribute to that effort. In 2019, the Global Alliance for Chronic Diseases funded 27 implementation research studies to improve evidence for scaling up interventions targeting prevention and/or control of hypertension and/or diabetes in low-resource settings. We examined these studies to improve the understanding of the implementation factors, including challenges and facilitators, that influence the early implementation phase of scale-up research projects targeting NCDs.Methods This qualitative study was undertaken between August 2020 and July 2021. 43 semi-structured interviews were conducted with project investigators, implementers and policymakers, across 19 diverse scale-up projects, being implemented in 20 countries. The transcripts were inductively, open-coded using thematic analysis. Generated themes were mapped systematically to four out of five domain categorisations of the Consolidated Framework for Implementation Research (CFIR); the innovation domain fell outside the scope of this study.Results Highlighted findings using CFIR are: (i) outer setting: influence of politics, lack of coordination between government departments and differing agendas towards NCDs hindered implementation while reliable and trustworthy government connections proved useful; (ii) inner setting: commitment of resources for implementation was a challenge while research capacity, work culture and trustworthy networks facilitated implementation; (iii) individuals: high-level stakeholder support and leadership was essential; (iv) process: extensive time and efforts required for stakeholder engagement towards local contextualisation was challenging, while collaborating, joint reflection, effective communication and adaptation facilitated. COVID-19 provided both challenges and opportunities and these varied depending on the intervention characteristics and study objectives.Conclusion Researchers supporting the scale-up of complex interventions targeting NCDs need to leverage on existing trusting relationships and foster equitable stakeholder partnerships through research. Interpersonal skills and good communication are essential complements to research expertise and must be considered during capacity building.

RESUMO A Covid-19 é uma doença multissistêmica e consequências funcionais e tardias estão em estudo. Sequelas psicológicas e neurocognitivas podem comprometer a Capacidade para o Trabalho (CT) dos trabalhadores. Objetivou-se investigar a CT de pessoas previamente infectadas pelo Sars-CoV-2, correlacionando-a com avaliação da sonolência, ansiedade, depressão e fadiga. Estudo transversal, com trabalhadores diagnosticados com Covid-19 e em acompanhamento no Serviço de Neurologia da Universidade Estadual de Campinas (Unicamp). Aplicou-se o instrumento Índice de Capacidade para o Trabalho (ICT), um formulário com dados sociodemográficos e ocupacionais, bem como escalas de sonolência, ansiedade, depressão e fadiga. Dos 119 trabalhadores que participaram do estudo, mais da metade apresentaram comprometimento da CT (52,9%). Distúrbio emocional foi o agravo relatado mais frequente (31,9%). A regressão logística múltipla mostrou que a interação entre ansiedade e sonolência esteve associada ao comprometimento da CT (OR=4,50 com p=0,002). Ansiedade e sonolência foram alterações tardias da Covid-19 e associadas ao comprometimento da CT dos trabalhadores avaliados. Este estudo demonstra a necessidade de que todos os trabalhadores com teste positivo por Covid-19 tenham sua CT avaliada por ocasião do retorno ao trabalho. Ações de promoção à saúde, reabilitação funcional e adaptação do trabalho de acordo com as sequelas apresentadas pelos trabalhadores.

Abstract Background Worsening environmental conditions may amplify people’s emotional responses to an environmental crisis (eco-anxiety). In Portugal, young people seem to be especially concerned about climate change. However, this phenomenon needs to be interpreted using accurate instruments. Thus, this study aimed to validate the Portuguese version of the Hogg Eco-Anxiety Scale (HEAS) in young adults and examine the associations among eco-anxiety, sociodemographic characteristics, and pro-environmental behaviours. Methods A survey was administered to 623 Portuguese university students aged between 18 and 25 years. The survey included our Portuguese translation of the HEAS (obtained through a back-translation and pretesting process), a sociodemographic assessment, and questions related to pro-environmental behaviours. Confirmatory factor analysis was conducted to assess the construct validity of the Portuguese version of the HEAS, and global fit indices were used to assess whether the original four-dimensional structure of the scale was reproduced. The reliability of the Portuguese version of the HEAS was evaluated by Cronbach’s alpha and the intraclass correlation coefficient. Measurement invariance examined sex differences in scale interpretation. Linear regressions were used to detect whether sociodemographic variables predict eco-anxiety and whether eco-anxiety predicts pro-environmental behaviours. Results The factorial structure of the original scale was replicated in the Portuguese version of the HEAS, showing good internal consistency, reliability over time and strict invariance between men and women. A higher paternal education level predicted greater eco-anxiety in children. Two dimensions of eco-anxiety—namely, rumination and anxiety about personal impacts on the environment—predicted higher engagement in pro-environmental behaviours. Conclusions The translated scale is an appropriate tool to measure eco-anxiety in the Portuguese context and should be used to collect evidence to drive environmental and health policies. An individual’s education level should be considered a determinant of their emotional response to environmental conditions. Importantly, eco-anxiety can act as a protective emotional response to preserving the planet.

Abstract Objective: The purpose of this study is to determine if healthier neighbourhood food environments are associated with healthier diet quality. Design: This was a cross-sectional study using linear regression models to analyse data from the Maastricht Study. Diet quality was assessed using data collected with a FFQ to calculate the Dutch Healthy Diet (DHD). A buffer zone encompassing a 1000 m radius was created around each participant home address. The Food Environment Healthiness Index (FEHI) was calculated using a Kernel density analysis within the buffers of available food outlets. The association between the FEHI and the DHD score was analysed and adjusted for socio-economic variables. Setting: The region of Maastricht including the surrounding food retailers in the Netherlands. Participants: 7367 subjects aged 40–75 years in the south of the Netherlands. Results: No relationship was identified between either the FEHI (B = 0·62; 95 % CI = –2·54, 3·78) or individual food outlets, such as fast food (B = –0·07; 95 % CI = –0·20, 0·07) and diet quality. Similar null findings using the FEHI were identified at the 500 m (B = 0·95; 95 % CI = –0·85, 2·75) and 1500 m (B = 1·57; 95 % CI = –3·30, 6·44) buffer. There was also no association between the food environment and individual items of the DHD including fruits, vegetables and sugar-sweetened beverages. Conclusion: The food environment in the Maastricht area appeared marginally unhealthy, but the differences in the food environment were not related to the quality of food that participants reported as intake.

Enquadramento: a comunicação em cuidados paliativos é um contínuo repto para profissionais. Os enfermeiros são responsáveis pelo desenvolvimento destas intervenções e será destas que advém melhores resultados junto dos doentes e famílias. Objetivo: mapear as intervenções sobre a comunicação, dos enfermeiros em contexto de cuidados paliativos. Metodologia: estudo exploratório de revisão seguindo a metodologia da Joana Briggs Institute. Perante a questão “Que intervenções de comunicação são desenvolvidas pelos enfermeiros nas equipas de cuidados paliativos?”, realizou-se uma pesquisa nas bases de dados, com descritores MESH, “palliative care”, “nursing Care”, “comunication” com critérios de elegibilidade. Resultados: dos 88 estudos, selecionamos 11, apresentando critérios de seleção. Referem a existência de escassos estudos na área da comunicação em cuidados paliativos; os médicos encontram-se menos representados; a condição clínica no momento do encontro, é condicionadora e foca-se num momento único. É importante a comunicação até com ferramentas informáticas associada à tomada de decisão e realçando o seu indiscutível benefício. A comunicação é um instrumento muito pertinente entre o enfermeiro e o doente. Conclusão: a humanização da morte e o sentir do enfermeiro na passagem de informação sustentam “um sentido para a vida”. É possível mapear as intervenções dos enfermeiros e identificar limitações/lacunas dos saberes na comunicação em cuidados paliativos.

IntroductionHIV late presentation (LP) remains excessive in Europe. We aimed to analyze the factors associated with late presentation in the MSM population newly diagnosed with HIV in Portugal between 2014 and 2019.MethodsWe included 391 newly HIV-1 diagnosed Men who have Sex with Men (MSM), from the BESTHOPE project, in 17 countrywide Portuguese hospitals. The data included clinical and socio-behavioral questionnaires and the viral genomic sequence obtained in the drug resistance test before starting antiretrovirals (ARVs). HIV-1 subtypes and epidemiological surveillance mutations were determined using different bioinformatics tools. Logistic regression was used to estimate the association between predictor variables and late presentation (LP).ResultsThe median age was 31 years, 51% had a current income between 501–1,000 euros, 28% were migrants. 21% had never been tested for HIV before diagnosis, with 42.3% of MSM presenting LP. 60% were infected with subtype B strains. In the multivariate regression, increased age at diagnosis, higher income, lower frequency of screening, STI ever diagnosed and higher viral load were associated with LP.ConclusionOur study suggests that specific subgroups of the MSM population, such older MSM, with higher income and lower HIV testing frequency, are not being targeted by community and clinical screening services. Overall, targeted public health measures should be strengthened toward these subgroups, through strengthened primary care testing, expanded access to PrEP, information and promotion of HIV self-testing and more inclusive and accessible health services.

IntroductionMultisectoral action is a central component of the global response to the rising prevalence of non-communicable diseases (NCDs). In this paper we aimed to unpack the definition of multisectoral action and provide an overview of the historical context, challenges, and recommendations alongside three country case studies: salt reduction in the UK, tobacco legislation in Nigeria, and regulation of edible oils in Iran.MethodsWe used an iterative review process to select three country case studies from a list of 20 potential cases previously identified by WHO. At our third round of review we unanimously agreed to focus on salt reduction in the UK, tobacco regulation in Nigeria, and edible oil regulation in Iran as these represented rich cases on diverse risk factors from three different world regions that we felt offered important lessons. We conducted literature reviews to identify further data for each case study.ResultsAcross the three studies a number of important themes emerged. We found that multisectoral approaches demand the often difficult reconciliation of competing and conflicting values and priorities. Across our three chosen cases, commercial interests and free trade agreements were the most common obstacles to successful multisectoral strategies. We found that early consultative stakeholder engagement and strong political and bureaucratic leadership were necessary for success.DiscussionThe complex multi-rooted nature of NCDs requires a multisectoral approach, but the inevitable conflicts that this entails requires careful navigation.

Summary: Background: There is a lack of up-to-date estimates about the prevalence of Chagas disease (ChD) clinical presentations and, therefore, we aimed to assess the prevalence of clinical forms of ChD among seropositive adults, pooling available data. Methods: A systematic review was conducted in Medline, Embase, Biblioteca Virtual em Saúde and Cochrane databases looking for studies published from 1990 to August 2023, which investigated the prevalence of ChD clinical forms among seropositive adults, including: (i) indeterminate phase, (ii) chronic Chagas cardiomyopathy (CCM), (iii) digestive and (iv) mixed (CCM + digestive) forms. Pooled estimates and 95% confidence intervals (CI) were calculated using random-effects models. Studies quality and risk of bias was assessed with the Leboeuf-Yde and Lauritsen tool. Heterogeneity was assessed with the I2 statistic. The study was registered in the PROSPERO database (CRD42022354237). Findings: 1246 articles were selected for screening and 73 studies were included in the final analysis (17,132 patients, 44% men). Most studies were conducted with outpatients (n = 50), followed by population-based studies (n = 15). The pooled prevalence of the ChD clinical forms was: indeterminate 42.6% (95% CI: 36.9–48.6), CCM 42.7% (95% CI: 37.3–48.3), digestive 17.7% (95% CI: 14.9–20.9), and mixed 10.2% (95% CI: 7.9–13.2). In population-based studies, prevalence was lower for CCM (31.2%, 95% CI: 24.4–38.9) and higher for indeterminate (47.2%, 95% CI: 39.0–55.5) form. Resumo O trabalho docente nas universidades tem sido marcado por sobrecarga exaustiva com a lógica do trabalho intensificado e precarizado. Além disso, estudos relacionando gênero, saúde e trabalho docente ainda são escassos. Neste artigo, objetivou-se identificar e avaliar criticamente as evidências científicas relacionadas ao estresse ocupacional e aos transtornos mentais entre docentes do ensino superior na perspectiva de gênero. Realizou-se revisão integrativa nas bases de dados PubMed, SciELO e Lilacs, entre os meses de maio e junho de 2019, a partir das combinações dos descritores indexados: occupational stress; mental disorder; mental health; higher education; faculty; universities e das palavras-chave: faculty teacher; faculty teachers; university teacher; university teachers; academic setting; academic settings. Os resultados sinalizam que o estresse ocupacional é uma realidade nas universidades, com docentes cada vez mais insatisfeitos(as) e apresentando altas prevalências de transtornos mentais e sintomas depressivos. Ficou evidente a necessidade de responsabilização por parte da gestão educacional. A perspectiva de gênero manteve-se ausente ou superficial nas produções acadêmicas revisadas. É preciso concentrar-se em medidas que possam melhorar a qualidade do trabalho nas universidades, tornando-as um espaço prazeroso para homens e mulheres de forma igualitária.

Introdução: A adolescência representa um período crítico na formação de hábitos que perdurarão na idade adulta. A prática de atividade física constante resulta em um bom desenvolvimento escolar, homeostase psíquica e manutenção do peso corporal ideal. Objetivo: Analisar a prevalência da prática de atividade física de adolescentes no município de Montes Claros (MG). Métodos: Trata-se de um estudo quantitativo, transversal e analítico, realizado em 13 instituições de ensino de Montes Claros (MG) no ano de 2017, utilizando o Questionário sobre Atividade Física Regular (PAQ-C) para avaliar a prática de atividade física, relacionada às variáveis: sexo, faixa etária e série escolar. Participaram alunos de 11 a 14 anos, devidamente matriculados nas últimas quatro séries do ensino fundamental. Realizaram-se análises descritivas e bivariadas com nível de significância de 5%. Resultados: Foram entrevistados 897 alunos, com média de idade de 12,64 (±0,98) anos. Dos entrevistados, 48,0% foram considerados moderadamente ativos, seguidos de 29,9% de sedentários e 22,0% de ativos. Resumo Objetivo: analisar a prevalência do aumento da automedicação durante a pandemia de COVID-19 entre professores da educação básica pública e fatores associados. Métodos: inquérito epidemiológico do tipo websurvey, realizado com professores da educação básica pública de Minas Gerais. A coleta de dados ocorreu de agosto a setembro de 2020 via formulário online. A variável dependente foi a automedicação durante a pandemia. Foi utilizada a Regressão de Poisson, com variância robusta. Resultados: participaram do estudo 15.641 professores de 795 cidades mineiras, 81,9% eram do sexo feminino. Observou-se que 14,5% dos professores aumentaram a automedicação durante a pandemia, entre eles houve maior prevalência entre as mulheres (Razão de Prevalência [RP]=1,25; Intervalo de Confiança de 95% [IC95%]=1,12;1,40), professores com menos de 60 anos (RP=1,48; IC95%=1,12;1,95), residentes da área urbana (RP=1,15; IC95%=1,02;1,29), insatisfeitos com o trabalho docente (RP=1,60; IC95%=1,41;1,82), com pior padrão alimentar (RP=1,44; IC95%=1,33;1,56), parte do grupo de risco para a COVID-19 (RP=1,18; IC95%=1,10;1,27), com problemas de sono (RP=2,10; IC95%=1,85;2,38), tristes ou regularmente deprimidos (RP=6,29; IC95%=4,08;9,69) e aqueles que apresentaram muito medo da COVID-19 (RP=1,39; IC95%=1,23;1,57). Conclusão: a automedicação esteve associada a características sociodemográficas, hábitos de vida, condições de saúde e indiferença ou insatisfação com o trabalho, configurando-se como um problema de saúde entre docentes.

Introdução: No processo de educação em saúde para que o indivíduo mude seus hábitos de saúde, é necessário que tenha conhecimentos, apresente crenças de autoeficácia positivas e compartilhe a eficácia coletiva. Objetivo: Avaliar o conhecimento e a percepção de autoeficácia e eficácia coletiva de professores sobre o Guia Alimentar para a População Brasileira. Método: Realizou-se estudo transversal no período de dezembro de 2019 a dezembro de 2020. A população do estudo foi composta por 1.491 professores da Educação Básica da rede pública municipal de ensino de Montes Claros, Minas Gerais. Para a amostra, estimou-se o tamanho mínimo de 511 professores e foi utilizado um questionário on-line para coleta de dados. Quanto a avaliação do conhecimento, percepção de autoeficácia e eficácia coletivados professores sobre o Guia Alimentar, utilizaram-se escalas autoadministradas e previamente validadas. Os dados coletados foram categorizados e processados eletronicamente através do software Statistical Package for the Social Science (SPSS), versão 22.0. Resultados: Na análise do desempenho dos professores quanto ao Guia Alimentar, observou-se que 79,7% (474) dos profissionais apresentaram baixo conhecimento; 70,4% (419), baixa autoeficácia; e 69,1% (411), baixa eficácia coletiva. O alto conhecimento entre os docentes foi associado à maior renda percapita (p= 0,004). A alta autoeficácia foi associada ao maior grau de escolaridade (p= 0,031). A alta autoeficácia coletiva foi associada às variáveis renda (p=0,035) e escolaridade (p=0,004). Conclusão: Os professores do presente estudo demonstraram baixo conhecimento e baixa percepção de autoeficácia e eficácia coletiva sobre o Guia Alimentar.

Wastewater-based epidemiology (WBE) is a valuable tool for investigating the existence, prevalence, and spread of pathogens, such as SARS-CoV-2, in a given population. WBE, proposed as part of the SARS-CoV-2 surveillance strategy for monitoring virus circulation, may complement clinical data and contribute to reducing the spread of the disease through early detection. In developing countries such as Brazil, where clinical data are scarce, information obtained from wastewater monitoring can be crucial for designing public health interventions. In the United States, the country with the largest number of confirmed SARS-CoV-2 cases worldwide, WBE programs have begun to be carried out to investigate correlations with coronavirus disease 2019 (COVID-19) clinical data and support health agencies in decision-making to prevent the spread of the disease. This systematic review aimed to assess the contribution of WBE to SARS-CoV-2 screening in Brazil and the United States and compare studies conducted in a developed and developing country. Studies in Brazil and the United States showed WBE to be an important epidemiological surveillance strategy in the context of the COVID-19 pandemic. WBE approaches are useful for early detection of COVID-19 outbreaks, estimation of clinical cases, and assessment of the effectiveness of vaccination program. BackgroundChagas disease (CD) is a neglected disease affecting millions worldwide, yet little is known about its economic burden. This systematic review is part of RAISE project, a broader study that aims to estimate the global prevalence, mortality, and health and economic burden attributable to chronic CD and Chronic Chagas cardiomyopathy. The objective of this study was to assess the main costs associated with the treatment of CD in both endemic and non-endemic countries.MethodsAn electronic search of the Medline, Lilacs, and Embase databases was conducted until 31st, 2022, to identify and select economic studies that evaluated treatment costs of CD. No restrictions on place or language were made. Complete or partial economic analyses were included.ResultsFifteen studies were included, with two-thirds referring to endemic countries. The most commonly investigated cost components were inpatient care, exams, surgeries, consultation, drugs, and pacemakers. However, significant heterogeneity in the estimation methods and presentation of data was observed, highlighting the absence of standardization in the measurement methods and cost components. The most common component analyzed using the same metric was hospitalization. The mean annual hospital cost per patient ranges from $25.47 purchasing power parity US dollars (PPP-USD) to $18,823.74 PPP-USD, and the median value was $324.44 PPP-USD. The lifetime hospital cost per patient varies from $209,44 PPP-USD for general care to $14,351.68 PPP-USD for patients with heart failure.DiscussionDespite the limitations of the included studies, this study is the first systematic review of the costs of CD treatment. The findings underscore the importance of standardizing the measurement methods and cost components for estimating the economic burden of CD and improving the comparability of cost components magnitude and cost composition analysis. Finally, assessing the economic burden is essential for public policies designed to eliminate CD, given the continued neglect of this disease.

Resumo Objetivo: analisar a associação entre infecção prévia por Chikungunya e sintomas depressivos em trabalhadores da saúde. Métodos: estudo transversal com amostra probabilística de trabalhadores atuantes na atenção primária e na média complexidade de um município do estado da Bahia, Brasil. Utilizou-se o teste rápido DPP-ZDC-IgM/IgG para identificar infecção recente (IgM) ou prévia (IgG) pelo vírus Chikungunya. O Patient Health Questionnaire-PHQ-9 foi usado para avaliar sintomas depressivos. Regressão de Poisson com variância robusta foi usada para estimar razões de prevalência e intervalos de confiança 95%. Resultados: participaram da pesquisa 392 trabalhadores, sendo 83,2% do sexo feminino. A frequência de infecção pelo vírus Chikungunya foi de 8,9%. A prevalência de sintomas depressivos foi de 22,7%. Na análise ajustada, a infecção por Chikungunya associou-se positivamente aos sintomas depressivos (RP=2,00; IC95%:1,29;3,07). As análises estratificadas apontaram associação de maior magnitude no sexo masculino (RP=7,57;1,15;50,06), em comparação ao feminino (RP=1,68;1,03;2,74). Conclusão: os achados corroboram a hipótese de associação positiva entre Chikungunya e sintomas depressivos. Mecanismos fisiopatológicos decorrentes de ação viral, bem como fatores emocionais, comportamentais e psicossociais associados à doença podem explicar os achados. Reitera-se a importância do cuidado em saúde mental para os trabalhadores da saúde.

Introdução: Fórmulas infantis podem ser coadjuvantes ou medida terapêutica para recuperação das crianças hospitalizadas, quando a amamentação está impossibilitada ou contraindicada. Assim, as atividades no lactário devem ser realizadas sob rigorosas técnicas assépticas, objetivando oferecer à criança alimentação com menor risco de contaminação. Conhecimento e controle dos processos desenvolvidos no preparo das fórmulas infantis são fundamentais para um monitoramento eficaz da qualidade do produto produzido e imprescindíveis para a melhoria na gestão da qualidade de um lactário hospitalar. Ferramentas podem apoiar a gestão da qualidade, sendo o mapeamento dos processos uma delas. Para a coleta de informações dos processos a serem mapeados, a matriz SIPOC, utilizada mundialmente, surge como uma alternativa, pois permite a visualização de todas as inter-relações dentro de um processo. Objetivo: Mapear processos identificados na manipulação de fórmulas infantis acondicionadas em mamadeiras e propor oportunidades de melhorias ao lactário de um Instituto Nacional de Saúde. Método: Para identificação dos processos envolvidos na manipulação das fórmulas infantis foi realizada observação durante dois dias. Para o mapeamento dos processos foi produzida a matriz SIPOC, em seguida, os processos foram desenhados utilizando o software Bizagi®. Resultados: Foram identificados 27 processos na manipulação das fórmulas infantis. Com o mapeamento, foram percebidas quatro oportunidades de melhorias: plano de contingência para falta de água, para o resfriamento, utilização de mamadeiras estéreis e desenvolvimento de cronograma de reuniões com equipes multiprofissionais. Conclusões: Decerto o mapeamento dos processos, utilizando a ferramenta Bizagi®, foi um desafio enfrentado neste trabalho, pois se constituiu uma novidade no âmbito da Nutrição. A principal contribuição deste estudo foi mostrar que o mapeamento possibilitou a observação detalhada dos processos envolvidos na manipulação das fórmulas infantis, permitindo a identificação das oportunidades de melhorias, destacando que esta metodologia pode perfeitamente ser utilizada em outros serviços assistenciais e produtivos.

Enquadramento: a insuficiência respiratória é uma síndrome com grande impacto nos internamentos, na morbilidade e mortalidade. A aplicabilidade do oxigénio nasal de alto fluxo tem sido alvo de interesse no doente crítico. Objetivo: conhecer a efetividade do oxigénio nasal de alto fluxo no tratamento da insuficiência respiratória no adulto em unidades de cuidados intensivos. Métodos: revisão sistemática de efetividade que utiliza a estratégia PICO e recomendações do Joanna Briggs Institute. A pesquisa foi realizada em agosto de 2021 com recurso às plataformas de acesso PubMed e EBSCOhost. Resultados: identificaram-se 583 resultados. Foram analisados seis ensaios clínicos randomizados. A seleção foi feita após eliminação de duplicados; leitura do título, resumos e textos integrais de acordo com o diagrama Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Conclusão: o oxigénio nasal de alto fluxo revelou-se confortável, tolerável e eficaz no tratamento da insuficiência respiratória hipoxémica e hipercápnica. Foi eficaz quando comparado com oxigenoterapia convencional na pós-extubação de doentes hipoxémicos e na redução da pressão parcial de dióxido de carbono quando comparado com a Ventilação Não Invasiva nos doentes hipercápnicos.

Abstract Objectives: To investigate changes in mean sugar content of non-alcoholic beverages (overall and sugar-sweetened beverages (SSB)) available for purchase in Australia and to compare signatories v. non-signatories of the Australian Beverages Council voluntary pledge from 2018 Design: Retrospective observational study. Setting: Australia. Participants: About 1500 non-alcoholic beverages per year included in the FoodSwitch Monitoring Datasets for 2015–2019. Results: Overall, mean sugar content fell by 1·3 g/100 ml (17·1 %) from 7·5 g/100 ml in 2015 to 6·2 g/100 ml in 2019. SSB have accounted for about 56 % of all beverages available for purchase since 2015. Between 2015 and 2019, the sugar content of SSB dropped by about 10 % (0·8 g/100 ml). Soft drinks and milk-based drinks were the categories with the largest decrease in sugar content. The greater reduction in sugar observed for beverages overall than SSB suggests at least some of the overall decrease in sugar content is due to the appearance of new products with low or no sugar rather than reformulation. Over the same period, beverages with added non-nutritive sweeteners increased from 41 % to 44 %. The decrease in sugar content for all beverages and SSB was, in general, larger for non-signatories than signatories of the voluntary industry pledge. Conclusions: Between 2015 and 2019, the small reduction in sugar content of non-alcoholic beverages in Australia resulted from the combined effects of introducing low- or no-sugar products and reformulation of some categories of SSB. Further policy and regulatory measures are required to reap the most benefit that sugar reduction among non-alcoholic beverages can bring to population health.

IntroductionThe strong association between age and the increasing prevalence of chronic diseases, makes it imperative to promote self-care throughout life. Systematic knowledge on the health findings of person-centered care models may contribute to designing effective healthcare strategies to promote empowerment for self-care in long-term care.ObjectiveTo assess the association between the implementation of person-centered care models that promote self-care training in long-term care and health-related outcomes, among adults with chronic illness.MethodsA rapid review of the literature was performed following the Cochrane rapid review methodology. The electronic databases CINAHL, MedicLatina, MEDLINE, and Psychology and Behavioral Sciences Collection were searched for randomized experimental studies, published between 2017 and 2022, that implemented interventions based on person-centered models to promote self-care in adults aged ≥18 years with chronic diseases and needing long-term health care. Verification of the eligibility of the articles and the extraction of data were performed by two independent investigators. Quantitative data on the health-related variables assessed were collected and, through narrative synthesis, health outcomes were grouped into individual, institutional and societal levels.ResultsEight studies, mostly conducted in European countries, were included. All satisfied more than 60% of the methodological quality score. A large variability among studies was found regarding the number of participants, the data collection period and duration of the intervention, the samples selected and the care model implemented. A high number of health-related outcomes (n = 17) were analyzed in the studies, using 52 different instruments. The main health-related outcomes were multidimensional, with implications at the individual, institutional and societal levels. The promotion of overall health and wellbeing (n = 4), the implementation of patient-centered care models (n = 1), the positive and more frequent interactions with health professionals (2), the decrease on staff psychosocial distress (n = 1), and the absence of added costs (n = 1), while improving family caregivers’ skills (n = 1) were the main health-related outcomes described.ConclusionThere is a need to develop robust experimental studies focused on the views and experiences of all stakeholders and conducted in different countries and cultures. Short-, medium- and long-term health outcomes should be measured using internationally accepted and validated scales for chronic patients.

IntroductionAfter three years since the beginning of the pandemic, the new coronavirus continues to raise several questions regarding its infectious process and host response. Several mutations occurred in different regions of the SARS-CoV-2 genome, such as in the spike gene, causing the emergence of variants of concern and interest (VOCs and VOIs), of which some present higher transmissibility and virulence, especially among patients with previous comorbidities. It is essential to understand its spread dynamics to prevent and control new biological threats that may occur in the future. In this population_based retrospective observational study, we generated data and used public databases to understand SARS-CoV-2 dynamics.MethodsWe sequenced 1,003 SARS-CoV-2 genomes from naso-oropharyngeal swabs and saliva samples from Pará from May 2020 to October 2022. To gather epidemiological data from Brazil and the world, we used FIOCRUZ and GISAID databases.ResultsRegarding our samples, 496 (49.45%) were derived from female participants and 507 (50.55%) from male participants, and the average age was 43 years old. The Gamma variant presented the highest number of cases, with 290 (28.91%) cases, followed by delta with 53 (5.28%). Moreover, we found seven (0.69%) Omicron cases and 651 (64.9%) non-VOC cases. A significant association was observed between sex and the clinical condition (female, p = 8.65e-08; male, p = 0.008961) and age (p = 3.6e-10).DiscussionAlthough gamma had been officially identified only in December 2020/January 2021, we identified a gamma case from Belém (capital of Pará State) dated May 2020 and three other cases in October 2020. This indicates that this variant was circulating in the North region of Brazil several months before its formal identification and that Gamma demonstrated its actual transmission capacity only at the end of 2020. Furthermore, the public data analysis showed that SARS-CoV-2 dispersion dynamics differed in Brazil as Gamma played an important role here, while most other countries reported a new infection caused by the Delta variant. The genetic and epidemiological information of this study reinforces the relevance of having a robust genomic surveillance service that allows better management of the pandemic and that provides efficient solutions to possible new disease-causing agents.

ABSTRACT Objective: To analyze the spatiotemporal distribution of mortality in older people living with the human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) in the state of São Paulo, Brazil. Methods: This is an ecological study with temporal and spatial approaches to analyze mortality from HIV/AIDS in the older adult population in the 2010-2020 period in the state of São Paulo, Brazil. Analysis of temporal trends was performed using the joinpoint regression, and spatial analysis was carried out using the Moran's index and the local empirical Bayesian model. Results: We identified a total of 3,070 deaths from HIV/AIDS among older adults and a mortality rate of 51.71 per 100 thousand inhabitants during the study period. The joinpoint method showed a growing trend for the age groups from 70 to 79 years (annual percent change [APC]=3.45; p=0.01) and ≥80 years (APC=6.60; p=0.006) and stability for the general older adult population (APC=0.99; p=0.226). The spatial distribution of the crude mortality rate was diffuse throughout the state. After smoothing by the Bayesian estimator, we observed greater concentration in the eastern mesoregions. In Moran's analysis, we observed clusters of lower mortality rates in more central regions; and of higher rates in the southern and northern regions of the state. Conclusions: We found a major growing trend in mortality from HIV/AIDS in the age group of older adults over 69 years during the 2010-2020 period. Clusters of high mortality rates were located in regions further to the south and north of the state, where places of greater social inequalities are concentrated.

Resumo Objetivo: estimar soroprevalências de arboviroses, sífilis, HIV e Hepatite B (VHB) em trabalhadores de saúde (TS). Métodos: inquérito entre TS selecionados por amostragem aleatória, em uma cidade da Bahia - Brasil. Pesquisa realizada em 2019 por meio de questionário estruturado, que coletou informações sociodemográficas, ocupacionais e de saúde. Testes imunocromatográficos rápidos foram utilizados para rastrear as infecções; dessa forma, resultados positivos para arboviroses, sífilis, HIV e VHB indicaram soropositividade para a respectiva infecção. Além disso, detecção simultânea de anticorpos para zika (ZIKV) e dengue (DENV) indicou soropositividade para infecção por flavivírus. Resultados: 453 TS foram incluídos, sendo 82,8% do sexo feminino. A maioria (55,1%) relatou contato com material biológico; 5,2% referiram acidentes com material biológico. Encontrou-se soroprevalência de 34,7% (IC95%:30,1-39,4) para dengue, 1,7% (IC95%:0,7-3,4) para zika, 9,9% (IC95%:7,2-13,2) para chikungunya e 39,9% (95%CI:35,2-44,7) para flavivírus (ZIKV+DENV); 21,9% (IC95%:18,1-26,2) foram negativos para todos os arbovírus. Soropositividade para arbovírus aumentou com a idade e foi maior entre agentes de combate a endemias. Três TS testaram positivo para VHB (HBsAg); nenhum para HIV. Conclusão: elevado percentual de TS estava exposto a agentes infecciosos (contato com material biológico e/ou exposição a arbovírus). Entre as condições necessárias à prevenção de infecções no ambiente de trabalho, destacam-se: monitoramento de infecções entre TS, vigilância dos ambientes laborais e medidas de controle de exposições ocupacionais, como disponibilidade de repelentes.

Introduction: Suicide is one of the main causes of death in adolescents. In assessing suicide risk, technology is gaining ground. Adolescents have experienced users of technology, justifying interventions based on new technologies in suicide prevention. Objective: To identify the benefits of using new technologies to prevent suicide risk in adolescents. Methods: Systematic literature review. The research was performed in EBSCOhost and PubMed databases. Included articles according to PRISMA recommendations. Boolean conjugation with descriptors in English was used: technology* AND (APP OR application OR mobile application) AND (suicide*) AND (adolescent OR youth OR puberty). Results: 11 articles met the inclusion and eligibility criteria. Mobile applications or APPs and language detection were the typologies found, with Smartphone Applications/APPs present in a greater number of studies. The results prove the importance of using new technologies to prevent the risk of suicide in adolescents. Conclusion: Evidence demonstrated an elevated adhesion to the use of new technologies, useful in preventing the risk of suicide by adolescents. The new technologies were considered to be well-accepted and have beneficial effects. Presently, there is insufficient data to support the use of these interventions in clinical practice. The number of studies on this topic is limited, requiring further investigation.

BackgroundThe natural history of Chagas disease (ChD) in older ages is largely unknown, and it is a matter of controversy if the disease continues to progress in the elderly.ObjectiveTo investigate the evolution of electrocardiographic abnormalities in T. cruzi chronically infected community-dwelling elderly compared to non-infected (NChD) subjects and how it affects this population's survival in a follow-up of 14 years.Methods and resultsA 12-lead ECG of each individual of the Bambui Cohort Study of Aging was obtained in 1997, 2002, and 2008, and the abnormalities were classified using the Minnesota Code. The influence of ChD on the ECG evolution was assessed by semi-competing risks considering a new ECG abnormality as the primary event and death as the terminal event. A Cox regression model to evaluate the population survival was conducted at a landmark point of 5.5 years. The individuals of both groups were compared according to the following categories: Normal, Maintained, New, and More by the development of ECG major abnormalities between 1997 and 2002. Among the participants, the ChD group had 557 individuals (median age: 68 years) and NChD group had 905 individuals (median age: 67 years). ChD was associated with a higher risk of development of a new ECG abnormality [HR: 2.89 (95% CI 2.28-3.67)]. The development of a new major ECG abnormality increases the risk of death ChD patients compared to those that maintain a normal ECG [HR: 1.93 (95% CI 1.02-3.65)].ConclusionChD is still associated with a higher risk of progression to cardiomyopathy in the elderly. The occurrence of a new major ECG abnormality in ChD patients predicts a higher risk of death.

ABSTRACT OBJECTIVE Evaluating characteristics of unpaid domestic work and its association with mental disorders, exploring gender differences. METHODS We analyzed cross-sectional data from the second wave of an urban population cohort (n = 2,841) aged 15 and older from a medium-sized city in Bahia (BA). The representative population sample was randomly selected in subsequent multiple steps. We interviewed the survey participants at their homes. This study analyzed sociodemographic, occupational, unpaid domestic work and mental illness data, stratified by sex (gender). We investigated the association between the work-family-personal time conflict, the effort-reward imbalance in domestic and family work and the occurrence of common mental disorders, such as generalized anxiety disorder and depression. We estimated prevalence, prevalence ratios and their respective 95% confidence intervals. RESULTS Among the participants, the unpaid domestic activities were performed by 71.3% of men and 95.2% of women, who were responsible for the investigated activities, except for minor repairs. The percentages of paid work were higher among men (68.1% versus 47.2% among women). The distribution of stressors and conflict experiences showed an inverse situation between genders: men depicted the highest high percentage of low work-family-personal time conflict (39.0%), while among women, the highest percentage was of high conflict (40.0%); 45.8% of the men reported low effort-reward imbalance in domestic and family work, while only 28.8% of women reported low imbalance. The investigated mental disorders were more prevalent among women, who showed a significant association between work-family-personal time conflict and common mental disorders, as well as depression; among men, conflict was positively associated with common mental disorders. The effort-reward imbalance, in turn, was strongly related to CMD (Common Mental Disorders), generalized anxiety disorder and depression among women. Amid men, this discrepancy was only associated to depression. CONCLUSIONS Domestic work persists as a mostly feminine assigned activity. The stressful situations of unpaid domestic work and the work-family-personal time conflict were more strongly associated with adverse effects on the female mental health.

Chagas disease (ChD), a Neglected Tropical Disease, has witnessed a transformative epidemiological landscape characterized by a trend of reduction in prevalence, shifting modes of transmission, urbanization, and globalization. Historically a vector-borne disease in rural areas of Latin America, effective control measures have reduced the incidence in many countries, leading to a demographic shift where most affected individuals are now adults. However, challenges persist in regions like the Gran Chaco, and emerging oral transmission in the Amazon basin adds complexity. Urbanization and migration from rural to urban areas and to non-endemic countries, especially in Europe and the US, have redefined the disease’s reach. These changing patterns contribute to uncertainties in estimating ChD prevalence, exacerbated by the lack of recent data, scarcity of surveys, and reliance on outdated models. Besides, ChD’s lifelong natural history, marked by acute and chronic phases, introduces complexities in diagnosis, particularly in non-endemic regions where healthcare provider awareness is low. The temporal dissociation of infection and clinical manifestations, coupled with underreporting, has rendered ChD invisible in health statistics. Deaths attributed to ChD cardiomyopathy often go unrecognized, camouflaged under alternative causes. Understanding these challenges, the RAISE project aims to reassess the burden of ChD and ChD cardiomyopathy. The project is a collaborative effort of the World Heart Federation, Novartis Global Health, the University of Washington’s Institute for Health Metrics and Evaluation, and a team of specialists coordinated by Brazil’s Federal University of Minas Gerais. Employing a multidimensional strategy, the project seeks to refine estimates of ChD-related deaths, conduct systematic reviews on seroprevalence and prevalence of clinical forms, enhance existing modeling frameworks, and calculate the global economic burden, considering healthcare expenditures and service access. The RAISE project aspires to bridge knowledge gaps, raise awareness, and inform evidence-based health policies and research initiatives, positioning ChD prominently on the global health agenda.
