BACKGROUND: Recent changes in psychiatric care and teaching that limit patient contact for medical students can be overcome in part by simulation-based education. Understanding the learning processes of medical students involved in psychiatric simulation-based programmes could usefully inform efforts to improve this teaching. This study explored the learning processes of medical students the first time they role-play in psychiatry.METHODS: We used constructivist grounded theory to analyse semi-structured interviews of 13 purposively sampled medical students and the six psychiatrists who trained them. To improve the triangulation process, the results of this analysis were compared with those of the analyses of the role-play video and the debriefing audio-tapes.RESULTS: Five organising themes emerged: improving the students' immediate perception of patients with mental disorders; cultivating clinical reasoning; managing affect; enhancing skills and attitudes and fostering involvement in learning psychiatry.CONCLUSION: Results suggest that psychiatric role-playing can improve students' progressive understanding of psychiatry through the development of intuition and by allaying affects. Emotional elaboration and student involvement appear to be key features.

International audience; Purpose: Despite the emergence of a growing qualitative literature about the personal recovery process in mental disorders, this topic remains little understood in anorexia nervosa (AN), especially severe AN during adolescence. This cases series is a first step that aims to understand recovery after severe AN among adolescents in France, from a first-person perspective. Methods: This cases series applied the interpretative phenomenological analysis (IPA) method to data collected in semi-structured face-to-face interviews about the recovery process of five young women who had been hospitalized with severe AN 10 years earlier during adolescence. Results: A model of recovery in four stages (corseted, vulnerable, plastic, and playful) crossing seven dimensions (struggle and path of initiation; work on oneself; self-determination and help; body; family; connectedness; and timeline) emerged from the analysis. New features of the AN personal recovery process were characterized: bodily well-being and pleasure of body; stigmatization; the role of the group; relation to time; and importance of narratives. We suggest a new shape to model the AN recovery process, one that suggests several tipping points. Recruitment must now be widened to different AN contexts. Conclusions: The personal recovery paradigm may provide a new approach to care, complementary to medical paradigm. Registration of clinical trial: No. NCT03712384. Our study was purely observational, without assignment of medical intervention. As a consequence, this clinical trial was registered retrospectively. Level of evidence: Level V, descriptive study.

Resume Le concept de retablissement en sante mentale, et en particulier dans la schizophrenie, est ne de la rencontre entre plusieurs facteurs d’ordre societaux, scientifiques et therapeutiques. Dans son acception experientielle, le retablissement ne designe pas tant une modalite evolutive du trouble qu’un processus personnel et singulier permettant de retrouver une vie satisfaisante et utile, de sortir de « l’identite de malade », malgre la persistance eventuelle de symptomes et limitations inherentes au trouble. Un certain nombre d’etudes qualitatives decrivent le retablissement comme un processus de transformation du sens de soi. Or, la plupart des approches theoriques de la schizophrenie rapportent cette psychopathologie a un trouble fondamental, voire irreversible, de l’identite. Nous soutenons des lors que le paradigme du retablissement vient rompre avec ces conceptions, en mettant a l’honneur l’experience subjective des patients, et en attestant de la possibilite de reamenagements identitaires signifiants dans la schizophrenie. Les transformations du sens de soi seront abordees sous l’angle de la narrativite. Nous illustrerons ces remaniements identitaires a partir de propos de patients issus d’une recherche qualitative, qui nous permettrons de degager certaines implications sur le plan psychotherapeutique.

Les avancees medicales et psychosociales dans la prise en charge de la schizophrenie ainsi que la place grandissante accordee aux mouvements d’usagers ont profondement modifie les conceptions evolutives du trouble schizophrenique. La notion de retablissement en constitue le paradigme le plus recent. Nous proposons de developper une reflexion theoricoclinique autour de la conception processuelle du retablissement qui met en lumiere, au travers d’etudes qualitatives, le vecu subjectif de la maladie et de son evolution. Nous tentons de degager une meilleure comprehension des processus de remaniements identitaires propres au retablissement dans la schizophrenie. Nous en declinons deux modalites : le rapport au trouble et le rapport a soi que nous illustrons par les propos de patients issus d’entretiens cliniques. Enfin, nous envisageons les implications cliniques de ces elements de comprehension.

Resume La prevention des rechutes est devenue l’un des enjeux majeurs de toute prise en charge a long terme des patients schizophrenes. Les strategies psychosociales de detection des signes avant-coureurs de rechute s’averent pertinentes car elles permettent d’intervenir precocement sur le processus de rechute afin de le stopper ou d’en limiter les consequences. L’objectif de cette recherche etait d’etudier les contributions apportees par chacun des acteurs inclus dans ce type de strategie. Nous avons ainsi realise une etude qualitative et exploratoire aupres de dix parents de sujets schizophrenes, dix patients schizophrenes et dix soignants dans l’objectif de mieux comprendre la maniere dont ces trois groupes definissent les elements de la rechute. L’analyse de contenu realisee par le logiciel Alceste a degage differentes classes de discours mettant en exergue les conceptions variees d’une rechute, notamment la nature des signes precoces identifies. Nos resultats etayent l’importance d’une alliance therapeutique entre la triade patients/parents/soignants, en mettant en avant la complementarite de leur role et de leur expertise dans l’identification de la « signature de rechute » des patients. L’application de notre etude concerne la mise en place d’un dispositif de prevention des rechutes respectant les specificites de chacun.

Resume Les notions de remission et de retablissement dans le champ de la schizophrenie semblent renvoyer a deux formes d’evolution favorable du trouble : l’une psychiatrique, l’autre davantage psychologique. Au sein de cet article, nous souhaitons mieux comprendre et caracteriser les liens entre ces continuums evolutifs. Nous presentons ainsi le discours de deux sujets atteints de schizophrenie, presentant des profils cliniques differents du point de vue de la remission symptomatique et fonctionnelle. Nos resultats mettent en evidence un rapport au trouble different chez ces deux patients, renvoyant au processus de retablissement. Cette evolution psychologique apparait relativement independante du critere psychiatrique de la remission symptomatique. Au regard de notre analyse qualitative, les etapes du processus de « retablissement du moi » peuvent se decliner en la reconnaissance de la maladie, d’une part, et l’integration de son vecu experientiel a la personnalite, d’autre part.
