Background: Kidney function impairment in pregnancy is challenging, with incidence and prognosis only partially known. Studies concerning maternal and fetal outcomes in pregnancies occurring in patients with renal injury and the therapeutic strategies for improving the prognosis of these patients are scarce due to the limited number of cases reported. Objectives: We aimed to establish correlations between the main maternal and fetal outcomes in patients with severe CKD or AKI in pregnancy to improve the prognosis, referring to a control group of patients with mild kidney impairment. Methods: For this purpose, we conducted a retrospective study, at University Emergency Hospital in Bucharest, Romania, from January 2019 until December 2021, selecting 38 patients with AKI and 12 patients diagnosed with advanced CKD, compared to 42 patients displaying borderline values of serum creatinine (0.8–1 mg/dL), reflecting the presence of milder kidney impairment. Results: The probability of having a child that is premature and small for gestational age, with a lower Apgar score and more frequent neonatal intensive care unit admissions, delivered by cesarean section, is higher in patients with highly reduced kidney function. Objectives. The prevention of preterm birth is one of the highest priorities for international obstetric research. This study aimed to evaluate the maternal factors that may influence the pregnancy outcomes in order to assess the fetal outcomes. The main parameters analyzed were the variables newborn’s Apgar score at 1 minute and Apgar score at 5 minutes. Materials and method. This is designed to be a retrospective, observational and non-randomized study on a sample of 30 pregnant women with ruptured membranes, diagnosed and treated in the Department of Obstetrics and Gynecology of the Bucharest University Emergency Hospital, Romania, between May 2021 and October 2021. We used two simple and multiple univariate linear regressions to achieve the main objective. The dependent variable used the APGAR score at 1 minute and the APGAR score at 5 minutes. Results. We obtained significant results regarding the impact of CRP, leukocytes count at admission, mode of delivery, presence of chorioamnionitis, funisitis, and male sex of the newborn on neonatal outcome. Conclusions. These results support further research in developing feasible multivariable prediction models in women with preterm rupture of membranes below 34 weeks of gestation. Total placenta praevia is used to describe a placenta that completely covers the internal os. Placenta increta is one of the most important complications of placenta praevia and requiring usually a hysterectomy. Objectives Pregnancy is considered a real metabolic challenge and a key moment for the development of subsequent maternal overweight or obesity-related complications such as gestational diabetes mellitus (GDM). Literature studies constantly report certain risk factors for GDM: family history of diabetes, previous GDM, advanced maternal age, being overweight or obese, fetal macrosomia, smoking. Among these factors, diet and regular physical exercise are modifiable, therefore educating patients could help prevent GDM and its related maternal and fetal complications. The aim of this study is to present our experience in the prevention of GDM in a group of 204 pregnant patients with detected risk factors. Methods A group of 204 pregnant patients with detected risk factors for GDM was enrolled in the study between April 2016 and April 2017. All patients were admitted in the Emergency University Hospital in Bucharest and followed-up at regular visits during pregnancy. A questionnaire was given to record patients' family history of cardiovascular disease, obesity and diabetes, lifestyle, diet, smoking and self-medication. Data about patients' pre-pregnancy BMI and gestational weight-gain was also recorded. At 24-28 weeks of gestation, all patients performed a 75g oral glucose tolerance test and pregnant patients with impaired glucose tolerance (IGT) were selected. These patients were put on a prudent diet, with a high intake of vegetables and white meat and were advised to quit smoking. Results The prudent diet had a positive impact on the gestational weight gain in the third trimester of the pregnant woman found to have an IGT, lowering the risk for obesity. Interestingly, the patients who had an active life before pregnancy and a moderate physical activity during pregnancy, gained less weight. Those who quit smoking and who supplemented their diet with vitamin D, C and iron also had better pregnancy outcomes. Conclusions Changes in modifiable risk factors (healthy diet, exercise) among pregnant women, especially overweight or obese, may reduce the risk of GDM and subsequent complications for both mother and fetus.
