Objective: This article describes the development of an interpersonal psychotherapy group which has been adapted to address the mother–child relationship in the context of postnatal depression (PND). Background: When PND develops, the child of the sufferer is also at risk for deleterious outcomes. It is thought that this is because the mother–baby bonding process is interrupted, affected, or reduced in quality by the existence of depression in addition to genetics and biological effects of exposure to illness in utero. Past approaches to mitigating this risk have focused on treating the depression as the primary issue and the mother–baby relationship as secondary. This article makes the argument that interpersonal psychotherapy has neglected this relationship despite the evidence that this is a key precipitating/perpetuating factor in PND, and that targeting this relationship has benefits for both mother and baby. Method: An interpersonal psychotherapy protocol was developed, modified to incorporate psychoeducation and practice of maternally sensitive interactions. A case study from a version of this group intervention is provided. Results: The case study outcomes on self-report scales of depression suggest the modified protocol is as effective in treating PND as the original protocol. Self-report of maternal attachment and videotape measures of maternal sensitivity also improved, suggesting that the modified protocol can address the mother–baby relationship. Conclusion: The suitability of adapting interpersonal psychotherapy to address the mother–baby relationship appears promising. Abstract: Background: Long-term follow-up studies are required to better understand the extent of the effectiveness of early detection programs for perinatal depression. We followed up participants in such a program to investigate the long-term depression, treatment and relationship outcomes of mothers originally identified as ‘probably depressed’ (screened positive). Methods: At 2years postpartum all participants who had ‘screened positive’ (N=159) and a random sample of participants who had ‘screened negative’ were invited to participate in a mailed survey. Measures included: current mood; coping; access to treatment; quality of partner relationship; and mother–infant bonding. Results: Mothers originally detected as probably depressed (n=98) fared significantly worse than ‘screened negative’ mothers (n=101) both in terms of their higher mean depression scores (EPDS: Ms=11.0 vs. 6.4) and greater proportions categorised as probably depressed at 2years postpartum (40% vs. 11% respectively, p<.001, phi=.33). Elevated depression symptoms at 2years postpartum were associated with poorer partner relationships and mother–infant bonding. Moreover, there appears to be a double dose effect for women who screen positive on two occasions. Thirty-seven percent of depressed mothers did not take up treatment, frequently citing a preference for using their own resources. Limitations: Limitations include the use of self-report measures to assess depression symptoms and mother–infant bonding. Treatment data was collected retrospectively. Conclusions: Despite being offered treatment options, a substantial proportion of women who screened positive had poor long-term mental health and relationship outcomes. The aim of this article is to review the published evidence on the effectiveness of strategies to engage men in psychosocial services during the perinatal period, and to consider anecdotal information from unpublished Australian programs. Thirteen published studies were found that provided some information on the enrolment numbers of men, together with the strategies used in each study. In addition, anecdotal information from nine Australian programs has been reported. Engaging men is more likely to succeed if they are, at least initially, offered a single-session program, rather than multiple sessions, they receive a personal invitation to attend, the initial emphasis is on practical things men can do rather than emotional issues, programs are not scheduled to coincide with televised major sporting fixtures, their partner is already attending the service, and the sessions are scheduled to occur several weeks into a program, not at the start. After-hours services may in some circumstances result in better enrolment by men, and use of plain language by program facilitators is better than counselling 'jargon' when engaging and working with men. Perinatal psychosocial services aimed at including men should consider carefully the strategies they employ if they wish to optimise the attendance of fathers or expectant fathers. Objective: To outline the processes involved in establishing an integrated perinatal mental health service in a public mental health setting. Conclusions: The Australian Capital Territory (ACT) Mental Health Services established a perinatal mental health service based on coordinated, evidence-based treatments. Objective: Although the efficacy of a number of psychotherapeutic interventions has been well established in tightly controlled, randomized trials, there remains a paucity of literature examining the effectiveness of these interventions in community practice settings. In light of this, the Australian Capital Territory Mental Health Services (Canberra, ACT) set out to investigate the effectiveness of an empirically supported psychotherapeutic intervention, interpersonal psychotherapy (IPT). The present study describes a pilot evaluation of the training programme for health professionals and the IPT treatment programme. Methods: Forty community mental health professionals participated in intensive IPT training. Clinicians who completed a course of supervision were asked to apply the treatment with non-psychotic acutely depressed patients. Measures of patients’ health outcomes were taken before and after treatment using a standardized outcome measure. Results: A total of 17 out of 21 patients who were selected completed a course of 12-16 weeks of IPT. The majority of the patients had a depression originating in the post-partum period. A comparison of pre- and post-treatment scores of treatment completers revealed a significant decrease in mean depression scores. Clinicians who completed a course of training and supervision found that they were able to confidently apply IPT in a clinical setting. Conclusions: Although there were a number of barriers and obstacles to the introduction of an evidenced-based treatment, the results are promising and demonstrate that IPT can be readily taught to experienced mental health professionals. Further study is required to determine the feasibility of IPT in other non-academic settings using larger sample sizes and homogenous groups of patients. [ABSTRACT FROM AUTHOR]
