Purpose: To investigate whether potential differences in staging between a traditional staging imaging algorithm and 18 F-FDG PET/MR lead to a change in patient management in breast carcinoma and to compare the diagnostic accuracy between the traditional staging algorithm and 18 F-FDG PET/MR for the TNM classification., Method: In this prospective cohort study from two university hospitals 56 women with newly diagnosed, therapy-naive breast cancer and increased pre-test probability for distant metastases were included. All patients were examined by a traditional staging imaging algorithm (X-ray mammography, breast ultrasonography, chest plain radiography, bone scintigraphy, and ultrasonography of the liver and axillary fossa) and whole-body 18 F-FDG PET/MR including dedicated 18 F-FDG PET/MR breast examinations. Each patient was discussed two times in a separate tumor board session to determine a total of three therapy recommendations based on histopathological data of the primary tumor and (1) traditional algorithm only, (2) traditional algorithm and 18 F-FDG PET/MR, and (3) 18 F-FDG PET/MR only. Major changes in therapy recommendations and differences between the traditional staging algorithm and 18 F-FDG PET/MR for the TNM classification were evaluated., Results: Staging by 18 F-FDG PET/MR led to a difference in treatment compared the traditional staging algorithm in 8/56 cases (14%). Therapy changes included therapy of the breast, locoregional nodes and systemic therapy. A trend to staging superiority was found for 18 F-FDG PET/MRI without statistical significance (p = 0.3827)., Conclusion: In conclusion, for breast cancer patients with elevated pre-test probability for distant metastases a change of the therapy regiment occurs in 14 % of patients when staged by 18 F-FDG PET/MR and confirmed by histopathology compared to a traditional staging algorithm. In particular with regard to the amendment of the guideline further assessment of 18 F-FDG-PET/MR in this setting is necessary to assess the true value of this modality., (Copyright © 2020 Elsevier B.V. Objectives: To investigate the diagnostic value of different MR sequences and 18F-FDG PET data for whole-body restaging of breast cancer patients utilizing PET/MRI., Methods: A total of 36 patients with suspected tumor recurrence of breast cancer based on clinical follow-up or abnormal findings in follow-up examinations (e.g. CT, MRI) were prospectively enrolled in this study. All patients underwent a PET/CT and subsequently an additional PET/MR scan. Two readers were instructed to identify the occurrence of a tumor relapse in subsequent MR and PET/MR readings, utilizing different MR sequence constellations for each session. The diagnostic confidence for the determination of a malignant or benign lesion was qualitatively rated (3-point ordinal scale) for each lesion in the different reading sessions and the lesion conspicuity (4-point ordinal scale) for the three different MR sequences was additionally evaluated., Results: Tumor recurrence was present in 25/36 (69%) patients. All three PET/MRI readings showed a significantly higher accuracy as well as higher confidence levels for the detection of recurrent breast cancer lesions when compared to MRI alone (p<0.05). Furthermore, all three PET/MR sequence constellations showed comparable diagnostic accuracy for the identification of a breast cancer recurrence (p>0.05), yet the highest confidence levels were obtained, when all three MR sequences were used for image interpretation. Moreover, contrast-enhanced T1-weighted VIBE imaging showed significantly higher values for the delineation of malignant and benign lesions when compared to T2w HASTE and diffusion-weighted imaging., Conclusion: Integrated PET/MRI provides superior restaging of breast cancer patients over MRI alone. Facing the need for appropriate and efficient whole-body PET/MR protocols, our results show the feasibility of fast and morphologically adequate PET/MR protocols. However, considering an equivalent accuracy for the detection of breast cancer recurrences in the three PET/MR readings, the application of contrast-agent and the inclusion of DWI in the study protocol seems to be debatable., (Copyright © 2017 Elsevier B.V. Purpose: This study aims to develop, implement, and evaluate a 16-channel radiofrequency (RF) coil for integrated positron emission tomography/magnetic resonance (PET/MR) imaging of breast cancer. The RF coil is designed for optimized MR imaging performance and PET transparency and attenuation correction (AC) is applied for accurate PET quantification., Methods: A 16-channel breast array RF coil was designed for integrated PET/MR hybrid imaging of breast cancer lesions. The RF coil features a lightweight rigid design and is positioned with a spacer at a defined position on the patient table of an integrated PET/MR system. Attenuation correction is performed by generating and applying a dedicated 3D CT-based template attenuation map. Reposition accuracy of the RF coil on the system patient table while using the positioning frame was tested in repeated measurements using MR-visible markers. The MR, PET, and PET/MR imaging performances were systematically evaluated using modular breast phantoms. Attenuation correction of the RF coil was evaluated with difference measurements of the active breast phantoms filled with radiotracer in the PET detector with and without the RF coil in place, serving as a standard of reference measurement. The overall PET/MR imaging performance and PET quantification accuracy of the new 16-channel RF coil and its AC were then evaluated in first clinical examinations on ten patients with local breast cancer., Results: The RF breast array coil provides excellent signal-to-noise ratio and signal homogeneity across the volume of the breast phantoms in MR imaging and visualizes small structures in the phantoms down to 0.4 mm in plane. Difference measurements with PET revealed a global loss and thus attenuation of counts by 13% (mean value across the whole phantom volume) when the RF coil is placed in the PET detector. Local attenuation ranging from 0% in the middle of the phantoms up to 24% was detected in the peripheral regions of the phantoms at positions closer to attenuating hardware structures of the RF coil. The position accuracy of the RF coil on the patient table when using the positioning frame was determined well below 1 mm for all three spatial dimensions. This ensures perfect position match between the RF coil and its three-dimensional attenuation template during the PET data reconstruction process. When applying the CT-based AC of the RF coil, the global attenuation bias was mostly compensated to ±0.5% across the entire breast imaging volume. The patient study revealed high quality MR, PET, and combined PET/MR imaging of breast cancer. Quantitative activity measurements in all 11 breast cancer lesions of the ten patients resulted in increased mean difference values of SUVmax 11.8% (minimum 3.2%; maximum 23.2%) between nonAC images and images when AC of the RF breast coil was applied. This supports the quantitative results of the phantom study as well as successful attenuation correction of the RF coil., Conclusions: A 16-channel breast RF coil was designed for optimized MR imaging performance and PET transparency and was successfully integrated with its dedicated attenuation correction template into a whole-body PET/MR system. Systematic PET/MR imaging evaluation with phantoms and an initial study on patients with breast cancer provided excellent MR and PET image quality and accurate PET quantification.

Objectives: To compare the diagnostic performance of (18)F-fluordesoxyglucose positron emission tomography/magnetic resonance imaging ((18)F-FDG PET/MRI) with (18)F-FDG PET/computed tomography ((18)F-FDG PET/CT), MRI, and CT in whole-body staging of recurrent breast cancer., Material and Methods: Twenty-one consecutive patients (age 59.4 ± 11.5 years, range 38.5-76.9 years; 20 female, 1 male) with suspected breast cancer recurrence underwent a clinically indicated (18)F-FDG PET/CT and subsequently a (18)F-FDG PET/MRI examination in a single injection protocol (mean injected activity: 257 ± 44 MBq (18)F-FDG). Each (18)F-FDG PET/MRI, (18)F-FDG PET/CT, as well as the CT component of PET/CT (CTPET/CT) and MR images of PET/MRI (MRIPET/MRI) were separately evaluated by two radiologists regarding lesion count, lesion localization, and lesion categorization (benign/malignant). The reference standard was based on histopathological results as well as prior and follow-up imaging. A Wilcoxon test assessed differences in SUVmax between (18)F-FDG PET/MRI and (18)F-FDG PET/CT. Correlation of SUVmax between (18)F-FDG PET/MRI and (18)F-FDG PET/CT was calculated using Pearson's correlation coefficient. Interobserver agreement on dignity ratings was evaluated using Cohen's kappa., Results: According to the reference standard, 17 patients had breast cancer recurrence. (18)F-FDG PET/MRI, (18)F-FDG PET/CT, and MRIPET/MRI correctly identified each of the 17 patients, whereas CTPET/CT correctly identified 15 of the 17 patients. A total of 134 lesions were described (116 malignant, 18 benign). (18)F-FDG PET/MRI detected all 134 lesions, of which (18)F-FDG PET/CT, MRIPET/MRI, and CTPET/CT detected 97.0%, 96.2%, and 74.6%, respectively. (18)F-FDG PET/MRI yielded the highest proportion of correctly categorized lesions (98.5%) compared with (18)F-FDG PET/CT (94.8%), MRIPET/MRI (88.1%), and CTPET/CT (57.5%). SUVmax was strongly correlated (r=0.72) but measured significantly higher on (18)F-FDG PET/MRI than on (18)F-FDG PET/CT in corresponding PET-positive lesions (SUVmax: 5.6 ± 2.8 vs. 4.9 ± 1.8; p=0.001). Interobserver agreement on lesion dignity was substantial with (18)F-FDG PET/MRI (k=0.65; p<0.001) and (18)F-FDG PET/CT (k=0.65; p<0.001). With MRIPET/MRI interobserver analysis yielded a moderate agreement (k=0.56; p<0.001), whereas there was only fair agreement evaluating the CTPET/CT datasets (k=0.31; p=0.002)., Conclusions: (18)F-FDG PET/MRI offered the highest diagnostic performance compared with (18)F-FDG PET/CT, MRI and CT. Purpose: To implement a protocol for PET/MR enterography for a multimodal assessment of intestinal pathologies., Materials and Methods: 19 patients with bowel malignancies, Crohn's disease or fever of unknown origin (male: n=14, female: n=5, age: 57±13years) underwent PET/MR enterography with either [(18)F]FDG (n=10) or [(68)Ga]-DOTATOC (n=9) using an integrated scanner. For small bowel distension a contrast solution (1500 cm(3) of mannitol and locust bean gum) was ingested. The following sequences were acquired: (a) coronal TrueFISP; (b) coronal T2w HASTE; (c) coronal dynamic T1w VIBE; (d) axial and coronal T1w FLASH post gadolinium. All datasets were reviewed by two readers with regard to co-registration of anatomical structures based on a 3-point ordinal scale as well as overall image quality using a 4-point ordinal scale. Furthermore, visualization of intestinal and extra-intestinal pathologies was assessed., Results: PET/MR enterography was well tolerated by all patients. High overall MR image quality was achieved (mean score: 3.2±0.6) with good co-registration of PET and MRI (mean scores: 2.6 to 3.0). PET/MR enterography allowed for an excellent visualization of both intestinal as well as extra-intestinal pathologies., Conclusion: PET/MR enterography is technically feasible and offers good co-registration of bowel structures. Objectives: To compare the diagnostic competence of FAST-PET/MRI and PET/CT for whole-body staging of female patients suspect for a recurrence of a pelvic malignancy., Methods: 24 female patients with a suspected tumor recurrence underwent a PET/CT and subsequent PET/MRI examination. For PET/MRI readings a whole-body FAST-protocol was implemented. Two readers separately evaluated the PET/CT and FAST PET/MRI datasets regarding identification of all tumor lesions and qualitative assessment of visual lesion-to-background contrast (4-point ordinal scale)., Results: Tumor relapse was present in 21 of the 24 patients. Both, PET/CT and PET/MRI allowed for correct identification of tumor recurrence in 20 of 21 cases. Lesion-based sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy for the detection of malignant lesions were 82%, 91%, 97%, 58% and 84% for PET/CT and 85%, 87%, 96%, 63% and 86% for PET/MRI, lacking significant differences. Furthermore, no significant difference for lesion-to-background contrast of malignant and benign lesions was found., Conclusion: FAST-PET/MRI provides a comparably high diagnostic performance for restaging gynecological cancer patients compared to PET/CT with slightly prolonged scan duration, yet enabling a markedly reduced radiation exposure., (Copyright © 2015 Elsevier Ireland Ltd. Purpose: To assess the diagnostic value of integrated PET/MRI for whole-body staging of cervical cancer patients, as well as to investigate a potential association between PET/MRI derived functional parameters and prognostic factors of cervical cancer., Methods: The present study was approved by the local institutional review board. Twenty-seven patients with histopathologically confirmed cervical cancer were prospectively enrolled in our study. All patients underwent a whole-body PET/MRI examination after written informed consent was obtained. Two radiologists separately evaluated the PET/MRI data sets regarding the determination of local tumor extent of primary cervical cancer lesions, as well as detection of nodal and distant metastases. Furthermore, SUV and ADC values of primary tumor lesions were analyzed and correlated with dedicated prognostic factors of cervical cancer. Results based on histopathology and cross-sectional imaging follow-up served as the reference standard., Results: PET/MRI enabled the detection of all 27 primary tumor lesions of the uterine cervix and allowed for the correct determination of the T-stage in 23 (85 %) out of the 27 patients. Furthermore, the calculated sensitivity, specificity and diagnostic accuracy for the detection of nodal positive patients (n = 11) were 91 %, 94 % and 93 %, respectively. PET/MRI correctly identified regional metastatic disease (N1-stage) in 8/10 (80 %) patients and non-regional lymph node metastases in 5/5 (100 %) patients. In addition, quantitative analysis of PET and MRI derived functional parameters (SUV; ADC values) revealed a significant correlation with pathological grade and tumor size (p < 0.05)., Conclusions: The present study demonstrates the high potential of integrated PET/MRI for the assessment of primary tumor and the detection of lymph node metastases in patients with cervical cancer. Providing additional prognostic information, PET/MRI may serve as a valuable diagnostic tool for cervical cancer patients in a pretreatment setting.

Objectives: This study aimed to assess the diagnostic performance of integrated positron emission tomography (PET)/magnetic resonance imaging (MRI) of the breast for lesion detection and local tumor staging of patients with primary breast cancer in comparison to PET/computed tomography (CT) and MRI., Materials and Methods: The study was approved by the local institutional review board. Forty-nine patients with biopsy-proven invasive breast cancer were prospectively enrolled in our study. All patients underwent a PET/CT, and subsequently, a contrast-enhanced PET/MRI of the breast after written informed consent was obtained before each examination. Two radiologists independently evaluated the corresponding data sets (PET/CT, PET/MRI, and MRI) and were instructed to identify primary tumors lesions as well as multifocal/multicentric and bilateral disease. Furthermore, the occurrence of lymph node metastases was assessed, and the T-stage for each patient was determined. Histopathological verification of the local tumor extent and the axillary lymph node status was available for 30 of 49 and 48 of 49 patients, respectively. For the remaining patients, a consensus characterization was performed for the determination of the T-stage and nodal status, taking into account the results of clinical staging, PET/CT, and PET/MRI examinations. Statistical analysis was performed to test for differences in diagnostic performance between the different imaging procedures. P values less than 0.05 were considered to be statistically significant., Results: Positron emission tomography/MRI and MRI correctly identified 47 (96%) of the 49 patients with primary breast cancer, whereas PET/CT enabled detection of 46 (94%) of 49 breast cancer patients and missed a synchronous carcinoma in the contralateral breast in 1 patient. In a lesion-by-lesion analysis, no significant differences could be obtained between the 3 imaging procedures for the identification of primary breast cancer lesions (P > 0.05). Positron emission tomography/MRI and MRI allowed for a correct identification of multifocal/multicentric disease in 3 additional patients if compared with PET/CT. For the definition of the correct T-stage, PET/MRI and MRI showed identical results and were correct in significantly more cases than PET/CT (PET/MRI and MRI, 82%; PET/CT, 68%; P < 0.05). Furthermore, the calculated sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy for the detection of nodal positive patients (n = 18) were 78%, 94%, 88%, 88%, and 88% for PET/CT; 67%, 87%, 75%, 82%, and 80% for MRI; and 78%, 90%, 82%, 88%, and 86% for PET/MRI, respectively. Differences between the imaging modalities were not statistically significant (P > 0.05)., Conclusions: Integrated PET/MRI does not provide diagnostic advantages for local tumor staging of breast cancer patients in comparison to MRI alone. Positron emission tomography/MRI and MRI enable an improved determination of the local tumor extent in comparison to PET/CT, whereas all 3 imaging modalities offer a comparable diagnostic performance for the identification of axillary disease.

Objectives: The objective of this study was to assess the diagnostic value of integrated positron emission tomography/magnetic resonance imaging (PET/MRI) for whole-body staging of patients with recurrent gynecological pelvic malignancies, in comparison to whole-body MRI alone., Materials and Methods: The study was approved by the local institutional ethics committee. Written informed consent was obtained before each examination. Thirty-four consecutive patients with a suspected recurrence of cervical (n = 18) or ovarian (n = 16) cancer were prospectively enrolled for an integrated PET/MRI examination, which comprised a diagnostic, contrast-enhanced whole-body MRI protocol including dedicated sagittal dynamic imaging of the pelvis. Two radiologists separately evaluated the data sets regarding lesion count, lesion detection, lesion characterization, and diagnostic confidence. Mean and median values were calculated for each rating. Statistical analyses were performed both per-patient and per-lesion bases using a Wilcoxon signed-rank test to indicate potential significant differences among PET/MRI and MRI (alone) data sets., Results: Malignant lesions were present in 25 of the 34 patients. Positron emission tomography/magnetic resonance imaging offered correct and superior identification of all 25 patients with cancer recurrence, compared with MRI alone (23/25). A total of 118 lesions (malignant, 89; benign, 29) were detected. Positron emission tomography/magnetic resonance imaging correctly identified 88 (98.9%) of 89 malignant lesions, whereas MRI alone allowed for correct identification of 79 (88.8%) of the 89 malignant lesions. In addition, PET/MRI provided significantly higher lesion contrast and diagnostic confidence in the detection of malignant lesions (P < 0.001) compared with MRI alone., Conclusions: These first results demonstrate the high diagnostic potential of integrated PET/MRI for the assessment of recurrence of female pelvic malignancies compared with MRI alone.

Objective: To retrospectively evaluate the performance of two water-equivalent oral contrast agents [locust bean gum (LBG)-mannitol and VoLumen] concerning their potential to distend the bowel while avoiding contrast-associated artifacts in PET/computed tomography., Methods: PET/computed tomography examinations of 30 patients with two different oral contrast agents were reviewed. Bowel distension, intraluminal density, and potential contrast-associated artifacts were assessed for stomach, jejunum, and ileum. Statistical significance was tested by Student's t-test., Results: Distension was slightly better in the stomach with VoLumen as compared with LBG-mannitol whereas LBG-mannitol was found to slightly better distend the small bowel. This difference proved to be statistically significant for the jejunum. A statistically significant difference was detected for intraluminal density with higher densities for VoLumen. This difference, however, did not result in a higher incidence of PET artifacts with VoLumen., Conclusion: LBG-mannitol provides excellent bowel distension, thereby avoiding contrast-associated PET artifacts. If this solution is not available, VoLumen provides a satisfactory alternative for bowel distension without relevant PET artifacts.

Background: Colorectal cancer (CRC) is a leading cause of death, and necessitates a conjointly performed staging. Until now, a multi-step-examination including optical colonoscopy, cross-sectional and functional imaging is recommended. However, a single examination for whole-body staging with a dedicated CRC staging protocol is desirable. Thus, we developed and evaluated a combined whole-body PET/CT-colonography protocol for dedicated CRC staging in routine clinical use., Methods: We integrated CT-colonography into a whole-body PET/CT protocol to achieve a specific "all-in-one" examination for patients suspected of having CRC. After oral and rectal bowel distension, PET/CT-colonography has been performed in 55 patients. All patients had optical colonoscopy one day before PET/CT. PET/CT data sets were evaluated concerning detection and evaluation of colorectal tumour sites, lymph nodes and distant metastases; these results were compared to the results of CT-colonography alone. Surgical resection and/or biopsy served as standards of reference in all patients., Results: All examinations were fully diagnostic and well tolerated by the patients. PET/CT-colonography showed highly accurate results for overall TNM-evaluation and was significantly more accurate than CT-colonography alone., Conclusions: Staging patients with whole-body PET/CT-colonography is technically feasible and accurate. Patients with incomplete colonoscopy or potential synchronous bowel lesions might benefit from this approach.
