Introduction: Problems associated with the increasing abuse of plant-derived psychoactive substances have recently attracted attention. This study involved bioanalytical and clinical examinations of intoxication cases suspected to be linked to such plant materials., Methods: Urine samples were collected at emergency wards in Sweden from patients who either admitted or were suspected of ingestion of psychoactive plant materials. The bioanalytical investigation employed a liquid chromatography-tandem mass spectrometry multicomponent method covering 10 plant-derived substances (atropine, dimethyltryptamine, ephedrine, harmaline, harmine, ibogaine, lysergic acid amide, psilocin, scopolamine, and yohimbine) and a gas chromatography-mass spectrometry method for asarone. Routine testing for illicit drugs was also performed., Results: Over a 4-year period, 103 urine samples collected from mainly young people (age range 13-52 years, median 19) were studied. Among 53 cases where ingestion of any of the 11 plant-derived substances covered in this study was admitted or suspected, 41 (77%) could be confirmed bioanalytically. Nine of the 11 substances tested for were detected, the exceptions being ibogaine and yohimbine. Psilocin, originating from ingestion of hallucinogenic mushrooms, was the most frequent drug accounting for 54% of the cases. The most common means of drug acquisition (56%) was purchase over the Internet., Conclusion: The patients using psychoactive plant materials were mainly young and commonly used the Internet for drug acquisition. Having access to bioanalytical methods for detection of plant-derived psychoactives is therefore considered important, when providing clinical toxicology service.

Deaths attributed to ingestion of Nicotiana glauca are extremely rare. We report here a case where a 43-year-old man was found dead after apparently drinking a water extract of Nicotiana glauca. The primary alkaloid in the plant is anabasine. Toxicological analysis by capillary gas chromatography showed the deceased had a blood anabasine concentration of 2.2 mg/L. Clinically, the features of poisoning are nicotine-like and if death occurs it results from respiratory paralysis. The case further supports the view that, in the human, anabasine is considerably more toxic than nicotine.

Datura stramonium and related species are relatively common causes of atropine-like poisoning by ingestion or inhalation. Toxic absorption after mucosal application is evident in 24 h of atropinism sustained by a woman who used a toothpaste mixed with the leaves and flowers of Datura sp., table salt, vinegar and an alcoholic beverage.

A 22-year-old man presented to our emergency department after an intentional overdose of a homemade foxglove extract. Clinical symptoms with symptomatic bradyarrhythmia and ECG changes were consistent with cardiac glycoside poisoning. Treatment with digoxin-specific Fab fragments resulted in transient clinical and ECG improvement. Serum immunoassay demonstrated a digitoxin-like glycoside. The serum levels showed no evidence of altered elimination or distribution with Fab therapy despite temporary improvements in the clinical course. The use of Fab did not result in a shortened clinical course in this episode of foxglove poisoning, as one would expect in the setting of commercial glycoside product poisoning.

The Manchineel tree is an evergreen widely distributed in tropical regions. The toxic nature of Manchineel has been known since the early sixteenth century. Contact with its milky sap (latex) produces bullous dermatitis and acute keratoconjunctivitis. We identified 19 patients who had ocular injuries caused by Manchineel between 1985 and 1990 and were able to review 12. All of these patients had been treated by lavage, cycloplegia, and topical antibiotics. Of 20 episodes of exposure 14 affected both eyes. The cornea was damaged in 16 episodes, the extent varying from large corneal epithelial defects to superficial punctate keratitis. The epithelial changes had resolved in a mean period of 3.75 days (range 1 to 14 days). Two episodes caused stromal infiltration to appear and in one of these a stromal opacity remained 5 years later. The final visual acuity was 6/9 or better in all eyes except in one patient who had visual impairment because of glaucoma. Our results suggest that despite the severity of the acute reaction, the long term visual prognosis is excellent in Manchineel keratoconjunctivitis. The historical and toxicological literature on Manchineel is reviewed.

To characterize the convulsions induced by an aqueous extract of Palicourea marcgravii (Pm), male Wistar rats were injected sc with 3.75 g/kg of the extract, and electroencephalogram and behavior were observed for periods up to 180 min after the Pm injections. Sleep spindles and generalized convulsive seizures were observed following Pm administration. Spiking activity was detected in both cortical and hippocampus recordings. They lasted approximately 2 min and were followed by a period of postical depression and/or death. These results agreed with previous behavioral experiments which suggested the toxic effect of Pm aqueous extract was mainly at the central nervous system level.

Four prisoners drank a decoction of yew (Taxus baccata) needles containing the toxic alkaloid taxine++ B. Two died in prison (one in the cell, the other shortly after admission to prison hospital, death was caused in both cases by cardiac arrest). Two other prisoners were admitted to the Warsaw Poisons Control Centre. One of them was in deep coma, with severe ventricular arrhythmias and had several episodes of ventricular fibrillation controlled with defibrillatory procedures. After an initial improvement (return of consciousness) his general condition deteriorated suddenly, he lost consciousness again, his circulation stopped and he died on the fourth day of hospitalization. The other patient drank a much smaller amount of the decoction. He was conscious, had bradycardia requiring transient endocavitary pacemaking, and had mild ventricular arrhythmia. He left the hospital in a very good condition after ten days of treatment. In both these cases long-standing excessive diuresis and corresponding considerable hypokalemia were observed. A synergism of the toxic effects of taxine++ B, digitalis glycosides and hypokalemia is considered very likely. In both cases atropine was effective for a short time in the control of bradycardia.
