Condyloma acuminatum is a common sexually transmitted disease caused by human papillomavirus. Its prevalence and recurrence rates are high. While there are multiple treatment options for condyloma acuminatum, no single therapy form is effective in achieving complete remission. Photodynamic therapy (PDT) was shown to be very promising as a treatment option for condyloma acuminatum. However, for some patients the therapeutic result after a single PDT session is not satisfactory. Thus, it is necessary to develop an effective treatment method for recalcitrant condyloma acuminatum. We hypothesize that suboptimal therapeutic results after a single PDT may be caused by an insufficient penetration of the photosensitizer into the tissue. Here, we applied a combination of superficial shaving and PDT to address this issue. All patients were satisfied with the effect of the treatment, and their quality of life improved significantly. Mohs micrographic surgery (MMS) is the preferable surgery for difficult -to-treat basal cell carcinoma (BCC) but is an expensive, labor-intensive, and time-consuming technique. The aim of this study is to compare the efficacy and safety of photodynamic therapy combined with surgery(S-PDT) versus Mohs micrographic surgery (MMS) for the treatment of difficult-to-treat BCC. This was a retrospective, comparative study. A total of 32 patients, 16 patients with 48 lesions, were treated with S-PDT, and the other 16 patients with 17 lesions treated by MMS were enrolled in this study. Follow-up was at least 36 months posttreatment. The recurrence rate was no statistical difference between the S-PDT and MMS (p = 1.000, Fishers exact test). The median follow-up was 42.5 months (range 36–63 months). The mean healing time in the S-PDT [17.9 d (SD 9.8)] is longer than in MMS [7.5 d (SD 1.5)] during follow-up (p＜.001, Independent T-test). On the whole, the cosmetic outcome of patients in S-PDT was statistically no significant difference with that in MMS according to a 4-point scale (p =.719, chi-squared test). • Combination superficial shaving with photodynamic therapy is an effective and safe therapy for patients with recalcitrant periungual warts. • Pretreatment of superficial shaving is easy to perform, possibly increasing the topical penetration of PDT photosensitizers in periungual warts, leading to an excellent curative effect. • Frequent or continuous hand activity, such as playing Mah-jong, may be a potential risk factor for periungual warts. Periungual warts are a viral infectious disease that occurs in a particular location. It is difficult to eliminate completely, and recurrence is common. Photodynamic therapy (PDT) as an option that has been widely recommended to treat viral warts. However, there are always a few patients with poor efficacy after PDT treatment. We have considered that the reason is the limitation of PDT penetrating deep into tissue. Thus, we combined superficial shaving with PDT to treat recalcitrant periungual warts. Twenty-three patients had a total of 61 periungual wart lesions. All patients had recalcitrant periungual warts that had failed to respond to various treatments that had poor curative effects. After local injection of anesthesia, the lesions were shaved in situ, and PDT was performed immediately. A total of three sessions of PDT were applied for each patient after only one superficial shaving. The overall clinical response rate, recurrence rates, cosmetic outcomes, adverse events, patient satisfaction and quality of life were assessed. The potential risk factors have also been recorded. We achieved a 96% success rate (defined as more than 50% on clearance) in our 23 patients using combination superficial shaving with PDT after treatment for 3 months. At the 12-month follow-up, 21 patients (91%) had excellent cosmetic outcomes. All patients had satisfactory therapeutic effects and significant improvement in the quality of life. Pain during the illumination process was the main adverse event, but all patients were able to tolerate it. We also found that frequent or continuous hand activity, such as playing Mah-jong, may be a potential risk factor for periungual warts. Our results offer promise for combining superficial shaving with PDT as an effective and safe therapy for patients with periungual warts, especially for those periungual warts that are recurrent, have multiple lesions, and thickness corneum stratum of lesions. Penile precancerous lesions with similar histopathological features but different clinical manifestations, including Bowenoid papulosis (BP), Bowen's disease and erythroplasia of Queyrat, were terminologically characterized as penile intraepithelial neoplasia (PIN). Here, we applied sequential curettage and photodynamic therapy (PDT) to successfully address a problematic case of BP that had failed multiple established approaches to treatment. In conclusion, sequential treatment of BP using curettage and PDT shows improved efficacy and lower relapse and might be considered as an alternative treatment for BP, especially for difficult cases. Angiolymphoid hyperplasia with eosinophilia (ALHE) is an inflamed vascular tumefaction of uncertain pathogenesis and is characterized by higher recurrence rates after surgical excision or other approaches. In the present study, we performed a new approach by combining electrocoagulation with photodynamic therapy (PDT) for the treatment of two ALHE patients. They had multiple treatments in the past and had poor therapeutic effects with relapse each time. After informed consent was obtained, electrocoagulation was used to remove the superficial lesions of ALHE, and the first session of PDT was immediately applied to the lesion. A total of three sessions of PDT were applied to each patient with an interval of 7–10 days. Through more than 12 months of follow-up, the two patients showed complete regression, and no recurrence was observed. Meanwhile, patients were very satisfied with the cost and cosmetic outcomes of the combination treatment. Based on our results, we strongly recommend the combination of electrocoagulation with PDT as a favourable treatment for ALHE, especially in the external ear and other areas that are inconvenient for routine surgery.
