Competing Interests: Competing interests: Sukhy Mahl reports receiving organizational support from the Maternal-Infant Care Research Centre (MiCare) at Mount Sinai Hospital in Toronto, Ontario, Canada. MiCare is supported by a Canadian Institutes of Health Research (CIHR) Team Grant (CTP 87518) and the Ontario Ministry of Health. Joel Lexchin reports receiving royalties from the University of Toronto Press and James Lorimer & Co. Ltd. for books he has written. Dr. Lexchin has also received payments from the CIHR and payment for expert testimony from Goodmans LLP. He is a member of the Foundation Board of Health Action International and the Board of Canadian Doctors for Medicare. Brian Rowe’s research is supported by a Scientific Directors Operating Grant (SOP 168483) from CIHR. Dr. Rowe is the Scientific Director of the Institute of Circulatory and Respiratory Health, CIHR, and Professor and Director at the Emergency Medicine Research Group, Department of Emergency Medicine, Faculty of Medicine and Dentistry, College of Health Sciences.
