Objectives: The purpose of this analysis was to evaluate the prognostic characteristics of peak oxygen consumption (Vo2) and the minute ventilation/carbon dioxide (VE/Vco2) slope of different peak respiratory exchange ratios (RERs) obtained from cardiopulmonary exercise testing in patients with heart failure (HF)., Background: For patients with HF, peak Vo2 and the VE/Vco2 slope are used for assessing prognosis. Peak Vo2 is assessed in association with peak RER ≥1.10, indicating maximal effort and prognostic sensitivity. Conversely, the VE/Vco2 slope provides effort-independent prognostic discrimination., Methods: Patients with HF scheduled to undergo cardiopulmonary exercise testing were enrolled. Patients were subclassified by peak RER (RER <1.00, RER 1.00 to 1.04, RER 1.05 to 1.09, RER ≥1.10) and followed for up to 3 years for major cardiac-related events (death, left ventricular assist device implantation, or cardiac transplantation)., Results: Included were 1,728 patients with HF (75% males; 40% ischemic etiology; age: 55 ± 14 years; left ventricular ejection fraction: 28 ± 10%). Two hundred seventy major events occurred, with no proportional differences across the RER subgroups. Multivariate Cox regression analysis indicated that the VE/Vco2 slope and peak Vo2 remained prognostic within each subgroup; the VE/Vco2 slope remained the strongest predictor. Receiver-operating characteristic analysis demonstrated equitable prognostic cutoffs for the VE/Vco2 slope (range: 34.9 to 35.7; area under the curve [AUC] range: 0.69 to 0.75) and peak Vo2 (range: 13.8 to 14.0 ml·kg(-1)·min(-1); AUC range: 0.68 to 0.75)., Conclusions: Peak Vo2 provided a sensitive assessment of prognosis in patients with HF in all RER subgroups. The VE/Vco2 slope provided greater prognostic discrimination in all RER subgroups. The objective of the study is to assess the role of cardiopulmonary exercise testing (CPX) variables, including peak oxygen consumption (VO(2)), which is the most recognized CPX variable, the minute ventilation/carbon dioxide production (VE/VCO(2)) slope, the oxygen uptake efficiency slope (OUES), and exercise oscillatory ventilation (EOV) in a current meta-analysis investigating the prognostic value of a broader list of CPX-derived variables for major adverse cardiovascular events in patients with HF. A search for relevant CPX articles was performed using standard meta-analysis methods. Of the initial 890 articles found, 30 met our inclusion criteria and were included in the final analysis. The total subject populations included were as follows: peak VO(2) (7,319), VE/VCO(2) slope (5,044), EOV (1,617), and OUES (584). Peak VO(2), the VE/VCO(2) slope and EOV were all highly significant prognostic markers (diagnostic odds ratios ≥ 4.10). The OUES also demonstrated promise as a prognostic marker (diagnostic odds ratio = 8.08) but only in a limited number of studies (n = 2). No other independent variables (including age, ejection fraction, and beta-blockade) had a significant effect on the meta-analysis results for peak VO(2) and the VE/VCO(2) slope. CPX is an important component in the prognostic assessment of patients with HF. The results of this meta-analysis strongly confirm this and support a multivariate approach to the application of CPX in this patient population.

Purpose: While patients with heart failure who achieve a peak oxygen uptake (peak VO2) of 10 mL·kg(-1)·min(-1) or less are often considered for intensive surveillance or intervention, those achieving 14 mL·kg(-1)·min(-1) or more are generally considered to be at lower risk. Among patients in the "intermediate" range of 10.1 to 13.9 mL·kg(-1)·min(-1), optimally stratifying risk remains a challenge., Methods: Patients with heart failure (N = 1167) referred for cardiopulmonary exercise testing were observed for 21 ± 13 months. Patients were classified into 3 groups of peak VO2 (≤10, 10.1-13.9, and ≥14 mL·kg(-1)·min(-1)). The ability of heart rate recovery at 1 minute (HRR1) and the minute ventilation/carbon dioxide output (VE/VCO2) slope to complement peak VO2 in predicting cardiovascular mortality were determined., Results: Peak VO2, HRR1 (<16 beats per minute), and the VE/VCO2 slope (>34) were independent predictors of mortality (hazard ratio 1.6, 95% CI: 1.2-2.29, P = .006; hazard ratio 1.7, 95% CI: 1.1-2.5, P = .008; and hazard ratio 2.4, 95% CI: 1.6-3.4, P < .001, respectively). Compared with those achieving a peak VO2 ≥ 14 mL·kg(-1)·min(-1), patients within the intermediate range with either an abnormal VE/VCO2 slope or HRR1 had a nearly 2-fold higher risk of cardiac mortality. Those with both an abnormal HRR1 and VE/VCO2 slope had a higher mortality risk than those with a peak VO2 ≤ 10 mL·kg(-1)·min(-1). Survival was not different between those with a peak VO2 ≤ 10 mL·kg(-1)·min(-1) and those in the intermediate range with either an abnormal HRR1 or VE/VCO2 slope., Conclusions: HRR1 and the VE/VCO2 slope effectively stratify patients with peak VO2 within the intermediate range into distinct groups at high and low risk.

Cardiopulmonary exercise testing (CPX) might aid in the diagnosis of coronary artery disease. However, a heterogeneous clinical population without previous workup bias has not been studied nor has a more extensive list of CPX variables. A total of 303 subjects (age 49.9 ± 11.6 years, 157 men) with symptoms suggestive of coronary artery disease underwent CPX and a single photon emission computed tomographic myocardial perfusion study (MPS). Ventilatory efficiency was calculated using the oxygen uptake efficiency slope (OUES). The change in the OUES was calculated by subtracting the OUES response during the first 50% of CPX from the OUES obtained during the last 25% of CPX. A negative change in the OUES (< 0) from the first 50% to the last 25% of CPX was predictive of positive MPS findings only in the male subjects. The diagnostic significance of the change in OUES in men was found for any level (including equivocal studies) of positive MPS findings (area under the curve 0.67, 95% confidence interval 0.59 to 0.76, p < 0.0001) and was even stronger in those with a more definitive (excluding equivocal studies) perfusion defect (area under the curve 0.76, 95% confidence interval 0.67 to 0.85; relative risk 5.4, 95% confidence interval 2.1 to 13.8, p < 0.0001). In conclusion, this is the first time that a change in ventilatory efficiency, assessed using the OUES, has been shown to be predictive of positive MPS findings However, the OUES change only provided diagnostic information for men, a finding that warrants additional analysis., (Copyright © 2010 Elsevier Inc. Background: Peak oxygen consumption derived from cardiopulmonary exercise (CPX) testing provides important prognostic information in patients with heart failure (HF). The oxygen consumption at the ventilatory threshold (VT) has also been shown to be prognostic. However, the VT cannot always be detected in patients with HF. Other variables such as the difference between peak oxygen consumption and oxygen consumption at the VT (termed the functional aerobic reserve [FAR]) may also provide prognostic information. The purpose of this study was to determine the prognostic value of an undetectable VT and FAR., Methods: Eight hundred seventy-four patients with chronic, systolic HF (70% male, age 54 ± 14 years, ejection fraction 29% ± 12%) underwent CPX and were tracked for 2 years for major events (death, transplant, and left ventricular assist device implantation)., Results: Patients were divided into 2 subgroups based on whether VT could be detected or not. There were 141 major events during the 2-year follow-up. Kaplan-Meier analysis for the 2 VT subgroups demonstrated worse prognoses for patients with a nondetectable VT versus those with a detectable VT (P < .001). Based on receiver operating characteristic curve analysis (FAR = 0 mlO₂ kg⁻¹ min⁻¹ for patients with undetectable VT), the optimal cut-point for FAR was ≤/>3 mlO₂ kg min⁻¹ (sensitivity/specificity 69%/60%). Cox regression analysis identified the FAR as a significant univariate predictor of risk and was retained in multivariate analysis., Conclusion: In conclusion, these data reveal that an undetectable VT and the FAR during CPX testing can provide useful prognostic information in patients with HF., (Copyright © 2010 Mosby, Inc. Background: Cardiopulmonary exercise testing (CPET) provides powerful information on risk of death in heart failure (HF). We sought to define the relative and additive contribution of the 3 landmark (CPET) prognostic markers--peak oxygen consumption (VO₂), minute ventilation/carbon dioxide production (VE/VCO₂) slope, and exercise periodic breathing (EPB)-to the overall risk of cardiac death and to develop a prognostic score for optimizing risk stratification in HF patients., Methods and Results: A total of 695 stable HF patients (average LVEF: 25 ± 8%) underwent a symptom-limited CPET maximum test after familiarization and were prospectively tracked for cardiac mortality. At multivariable Cox analysis EPB emerged as the strongest prognosticator. Using a statistical bootstrap technique (5000 data resamplings), point estimates, and 95% confidence intervals were obtained. Thirty-two configurations were adopted to classify patients into a given cell, according to EPB presence or absence and values of the 2 other covariates. Configurations without EPB and with VE/VCO₂ slope ≤30 were not significantly different from 0 (reference value). Statistical power of configurations increased with higher VE/VCO₂ slope and lower peak VO₂. This prompted us to formulate a score including EPB as a discriminating variable, the (P)e(R)i(O)dic (B)reathing during (E)xercise (PROBE), which ranges between -1 and 1, with zero as reference configuration, that would help to optimize the prognostic accuracy of CPET-derived variables. The greatest PROBE score impact was provided by EPB, followed by VE/VCO₂ slope, whereas peak VO₂ added minimal prognostic power., Conclusions: EPB with an elevated VE/VCO₂ slope leads to the highest and most precise PROBE score, whereas no additional risk information emerges when EPB is present with a peak VO₂ ≤10 mL O₂·kg⁻¹·min⁻¹. There is scarce information regarding the prognostic utility of peak exercise oxygen pulse (peak O(2) pulse), a surrogate for stroke volume, in patients with heart failure (HF). From May 1994 to November 2007, 998 patients with HF underwent cardiopulmonary exercise testing. The ability of peak oxygen uptake (VO(2)) and peak O(2) pulse to predict cardiac events was examined. Peak O(2) pulse was calculated by dividing peak VO(2) by heart rate at the time peak VO(2) was achieved and was expressed in both milliliters per beat and as a percentage achieved of the age-predicted value. There were 212 cardiac events (176 deaths, 26 transplantations, and 10 left ventricular assist device implantations) over a mean of 28 +/- 26 months of follow-up. Peak VO(2) and age-predicted peak O(2) pulse were demonstrated by univariate and multivariate Cox regression analyses to be independent predictors of mortality (p <0.001). The optimal cut points for peak VO(2) and age-predicted peak O(2) pulse (<14.3 and > or =14.3 [mL/kg(-1)/min(-1)] and <85% and > or =85%, respectively) were established by areas under the receiver-operating characteristic curves. Patients exhibiting abnormalities for both responses had 4.8-fold (95% confidence interval 2.7 to 8.5) and 6.7-fold (95% confidence interval 4.1 to 11.1) higher risks for mortality and cardiac events, respectively, than those whose responses were normal. Age-predicted peak O(2) pulse also predicted mortality in patients in the intermediate range of peak VO(2) (10 to 14 (mL/kg(-1)/min(-1))). The 3-year mortality rate for patients in this range who had age-predicted peak O(2) pulse values <85% was even slightly higher than those with peak VO(2) <10.1 (mL/kg(-1)/min(-1)). In conclusion, age-predicted peak O(2) pulse was a strong and independent predictor of cardiac mortality and complemented peak VO(2) in predicting risk in patients with HF.

Background: Peak oxygen consumption (Vo(2)) is routinely assessed in patients with heart failure undergoing cardiopulmonary exercise testing. The purpose of the present investigation was to determine the prognostic ability of several established peak Vo(2) prediction equations in a large heart failure cohort., Methods and Results: One thousand one hundred sixty-five subjects (70% males; age, 57.0+/-13.8 years; ischemic etiology, 43%) diagnosed with heart failure underwent cardiopulmonary exercise testing. Percent-predicted peak Vo(2) was calculated according to normative values proposed by Wasserman and Hansen (equation), Jones et al (equation), the Cooper Clinic (below low fitness threshold), a Veteran's Administration male referral data set (4 equations), and the St James Take Heart Project for women (equation). The prognostic significance of percent-predicted Vo(2) values derived from the 2 latter, sex-specific equations were assessed collectively. There were 179 major cardiac events (117 deaths, 44 heart transplantations, and 18 left ventricular assist device implantations) during the 2-year tracking period (annual event rate, 10%). Measured peak Vo(2) and all percent-predicted peak Vo(2) calculations were significant univariate predictors of adverse events (chi(2)> or =31.9, P<0.001) and added prognostic value to ventilatory efficiency (VE/Vco(2) slope), the strongest cardiopulmonary exercise testing predictor of adverse events (chi(2)=150.7, P<0.001), in a multivariate regression. The Wasserman/Hansen prediction equation provided optimal prognostic information., Conclusions: Actual peak Vo(2) and the percent-predicted models included in this analysis all were significant predictors of adverse events. It seems that the percent-predicted peak Vo(2) value derived from the Wasserman/Hansen equations may outperform other expressions of this cardiopulmonary exercise testing variable.

Aims: Exercise intolerance occurs in both systolic and diastolic heart failure (HF). Exercise oscillatory breathing (EOB) is a powerful predictor of survival in patients with systolic HF. In diastolic HF, EOB prevalence and prognostic impact are unknown., Methods and Results: A total of 556 HF patients (405 with systolic HF and 151 with diastolic HF) underwent cardiopulmonary exercise testing (CPET). Diastolic HF was defined as signs and symptoms of HF, a left ventricular ejection fraction > or =50%, and a Doppler early (E) mitral to early mitral annulus ratio (E') > or =8. CPET responses, EOB prevalence and its ability to predict cardiac-related events were examined. EOB prevalence in systolic and diastolic HF was similar (35 vs. 31%). Compared with the patients without EOB, patients with EOB and either systolic or diastolic HF had a higher New York Heart Association class, lower peak VO(2) and higher E/E' ratio (all P < 0.01). Univariate Cox regression analysis demonstrated that peak VO(2), VE/VCO(2) slope and EOB all were significant predictors of cardiac events in both systolic and diastolic HF. Multivariable analysis revealed that EOB was retained as a prognostic marker in systolic HF and was the strongest predictor of cardiac events in diastolic HF., Conclusion: EOB occurrence is similar in diastolic and systolic HF and provides relevant clues for the identification of diastolic HF patients at increased risk of adverse events.

Background: Ventilatory efficiency, commonly assessed by the minute ventilation (VE)-carbon dioxide production (VCO2) slope, has proven to be a strong prognostic marker in the heart failure (HF) population. Recently, the oxygen uptake efficiency slope (OUES) has demonstrated prognostic value, but additional comparisons to established cardiopulmonary exercise test (CPET) variables are required., Methods and Results: A total of 341 subjects were diagnosed with HF participated in this analysis. The VE/VCO2 slope and the OUES were calculated using 50% (VE/VCO2 slope(50) or OUES(50)) and 100% (VE/VCO2 slope(100) or OUES(100)) of the exercise data. Peak oxygen consumption (VO2) was also determined. There were 47 major cardiac-related events during the 3-year tracking period. Receiver operating characteristic (ROC) curve analysis demonstrated the classification schemes for both VE/VCO2 slope and OUES calculations as well as peak VO2 were statistically significant (all areas under the ROC curve: > or = 0.74, P < .001). Area under the ROC curve for the VE/VCO2 slope(100) was, however, significantly greater than OUES(50), OUES(100), and peak VO2 (P < .05)., Conclusions: Although the OUES was a significant predictor of mortality, the VE/VCO2 slope maintained optimal prognostic value. An elevated VE/VCO2 slope may be the single best indicator of increased risk for adverse events.

Background: Cardiopulmonary exercise testing (CPET) is considered a standard of care in heart failure (HF). The impact that duration of time post-evaluation has on the prognostic value of CPET, however, has not been explored. The purpose of this investigation was to examine the ability of peak oxygen consumption (VO(2)) and the minute ventilation-carbon dioxide production (VE/VCO(2)) slope to predict cardiac-related events at different time intervals post CPET., Methods: Two hundred fifty-eight subjects diagnosed with HF underwent CPET. The ability of peak VO(2) and VE/VCO(2) slope to predict cardiac-related events without a time limit, one year post CPET and greater than one year post CPET was examined., Results: Both peak VO(2) and VE/VCO(2) slope were significant predictors of cardiac-related mortality and hospitalization during the no time constraint and one year post CPET tracking scenarios (Hazard Ratio Range: 3.5-12.4, p<0.001). Peak VO(2) was no longer a significant predictor of mortality greater than one year post CPET. Generally, as time following CPET surpassed one year, the prognostic sensitivity of both variables slightly increased ( approximately 5%) while specificity sharply decreased ( approximately 20%)., Conclusions: Both peak VO(2) and VE/VCO(2) slope are significant predictors of cardiac-related events in HF. However, the prognostic characteristics of these variables following CPET appear to be time-sensitive.

Background: Peak oxygen consumption (V(O2)) and minute ventilation (V(E))/carbon dioxide production (V(CO2)) slope have been widely demonstrated to have strong prognostic value in patients with heart failure (HF). In the present study, we investigated the effect of HF etiology on the prognostic applications of peak V(O2) and Ve/V(CO2) slope., Methods: Two hundred sixty-eight subjects underwent symptom-limited cardiopulmonary exercise testing (CPX). The population was divided into ischemic (115 men and 22 women) and nonischemic (108 men and 23 women) subgroups. The occurrence of cardiac-related events over the year following CPX was compared between groups using receiver operating characteristic curve (ROC) analysis, Results: Mean age +/- SD was significantly higher (61.0 +/- 10.0 years vs 50.3 +/- 16.2 years) while mean peak V(O2) was significantly lower (15.0 +/- 5.2 mL/kg/min vs 17.5 +/- 6.7 mL/kg/min) in the ischemic HF group (p < 0.05). ROC curve analysis demonstrated that both peak V(O2) and V(E)/V(CO2) slope were significant predictors of cardiac events in both the ischemic group (peak V(O2), 0.74; V(E)/V(CO2) slope, 0.76; p < 0.001) and the nonischemic group (peak V(O2), 0.75; V(E)/V(CO2) slope, 0.86; p < 0.001). Optimal prognostic threshold values for peak V(O2) were 14.1 mL/kg/min and 14.6 mL/kg/min in the ischemic and nonischemic groups, respectively. Optimal prognostic threshold values for the V(E)/V(CO2) slope were 34.2 and 34.5 in the ischemic and nonischemic groups, respectively., Conclusions: Baseline and exercise characteristics were different between ischemic and nonischemic patients with HF. However, the prognostic power of the major CPX variables was strikingly similar. Different prognostic classification schemes based on HF etiology may therefore not be necessary when analyzing CPX responses in clinical practice.

Background: Exercise testing with ventilatory expired gas analysis has proven to be a valuable tool for assessing patients with heart failure (HF). Peak oxygen consumption (peak VO2) continues to be considered the gold standard for assessing prognosis in HF. The minute ventilation--carbon dioxide production relationship (VE/VCO2 slope) has recently demonstrated prognostic significance in patients with HF, and in some studies, it has outperformed peak VO2., Methods: Two hundred thirteen subjects, in whom HF was diagnosed, underwent exercise testing between April 1, 1993, and October 19, 2001. The ability of peak VO2 and VE/VCO2 slope to predict cardiac-related mortality and hospitalization was examined., Results: Peak VO2 and VE/VCO2 slope were demonstrated with univariate Cox regression analysis both to be significant predictors of cardiac-related mortality and hospitalization (P <.01). Multivariate analysis revealed that peak VO2 added additional value to the VE/VCO(2) slope in predicting cardiac-related hospitalization, but not cardiac mortality. The VE/VCO2 slope was demonstrated with receiver operating characteristic curve analysis to be significantly better than peak VO2 in predicting cardiac-related mortality (P <.05). Although area under the receiver operating characteristic curve for the VE/VCO2 slope was greater than peak VO2 in predicting cardiac-related hospitalization (0.77 vs 0.73), the difference was not statistically significant (P =.14)., Conclusions: These results add to the present body of knowledge supporting the use of cardiopulmonary exercise testing in HF. Consideration should be given to revising clinical guidelines to reflect the prognostic importance of the VE/VCO2 slope in addition to peak VO2.

Purpose: A significant discrepancy between measured oxygen consumption (VO(2)) (via ventilatory expired gas analysis) and estimated VO(2) (via the imposed workload) frequently is reported in the heart failure (HF) population during symptom-limited exercise testing. The purpose of this investigation was to examine the difference between measured and estimated VO(2) (VO(2) discrepancy) during a highly conservative ramping protocol., Methods: For this study, 28 subjects with compensated HF (20 men and 8 women; age, 51.1 +/- 14.6 years) and 19 healthy control subjects (age-, gender-, and activity-matched to an HF subgroup) underwent symptom-limited exercise testing (treadmill) with ventilatory expired gas analysis., Results: Peak estimated and measured VO(2) values were significantly higher in the age-, gender-, and activity-matched control group than in the HF group, but the change in measured VO(2) per change in estimated VO(2) (Deltameasured/Deltaestimated VO(2) slope) and the VO(2) discrepancy did not reach statistical significance. Peak estimated VO(2) was a significant predictor of peak measured VO(2) in the overall HF group (R2 = 0.90; P <.001)., Conclusions: Although estimated VO(2) is not considered a replacement for measured VO(2), these results indicate that a highly conservative exercise protocol may allow for a more accurate prediction of peak measured VO(2) via the estimated oxygen cost for a given workload in patients with compensated HF.

Purpose: The analysis of oxygen (O(2)) uptake on-kinetics during steady-rate is gaining interest in the heart failure (HF) population. The rate change in O(2) at the initiation of exercise can be assessed via nonlinear regression time constant (TC) or an algebraic equation (mean response time [MRT]). These calculations are presumed to be interchangeable, but research supporting this claim is limited. This investigation compares and contrasts two of the more commonly used O(2) uptake on-kinetic calculations., Method: Twenty-eight subjects diagnosed with compensated HF and 19 age, sex, and activity-matched controls underwent a symptom-limited exercise test and a steady-rate exercise session (6 min). Peak O(2) uptake, O(2) uptake at ventilatory threshold, the O(2) uptake TC (TC), and the O(2) uptake mean response time (MRT) were calculated for each subject., Results: O(2) uptake on-kinetics was significantly faster for the control group ( < 0.05) regardless of calculation method. There was a significant difference between the O(2) uptake TC and MRT for the HF group. All O(2) uptake on-kinetic calculations were significantly correlated with aerobic capacity., Conclusions: O(2) uptake TC and MRT values may not be interchangeable in the HF population. All O(2) uptake on-kinetic calculations did produce a significant difference between experimental and control groups and correlated with indicators of aerobic capacity. The 10-s O(2) uptake on-kinetic calculations may be preferable secondary to expired gas fluctuations associated with breath-by-breath measures. Further work is, however, needed to determine which averaged O(2) uptake on-kinetic expression is optimal given the significant difference between TC and MRT. A mechanism for this difference may be the oscillatory ventilatory expired gas pattern demonstrated by some patients with HF.

Purpose: This study assessed the relationship between the Minnesota Living With Heart Failure Questionnaire (MLWHFQ) and key ventilatory expired gas measures during a symptom-limited exercise test in the heart failure (HF) population. Specifically, is there evidence to indicate that perceived quality of life (QOL) influences exercise performance independent of physiologic function in the HF population?, Methods: Thirty-one subjects (21 male/10 female), diagnosed with compensated HF, underwent exercise testing and completed the MLWHFQ. Mean age and left ventricular ejection fraction were 52.8 years and 27.2%, respectively. Partial correlation, controlling for age and sex, assessed the relationship between MLWHFQ (overall and subscores) and key ventilatory expired gas measures. Intraclass correlation coefficient (ICC) analysis was used to determine reliability of the MLWHFQ., Results: MLWHQ overall score (mean = 38.9, median = 36.0), physical subscore (mean = 14.8, median = 16.0), and psychosocial/symptomatology subscore (mean = 24.1, median = 19.0), were significantly correlated (P < or =.05) with peak oxygen consumption (VO2). The relationship between MLWHFQ and the minute ventilation-carbon dioxide production (VE/VCO2) slope was, however, not significant. ICC analysis revealed high reliability (0.95) for the MLWHFQ., Conclusions: The MLWHFQ demonstrates a significant relationship with peak VO2, a measure whose validity is dependent upon subject effort. VE/VCO2 slope, which is independent of subject effort and therefore potentially a better predictor of true physiologic function, does not appear to have a relationship with perceived QOL. These findings have implications for how the MLWHFQ is assessed, related to an exercise test, and used during clinical practice.

Cardiopulmonary exercise test (CPX) responses are strong predictors of outcomes in patients with heart failure. We recently developed a CPX score that integrated the additive prognostic information from CPX. The purpose of this study was to validate the score in a larger, independent sample of patients.A total of 2625 patients with heart failure underwent CPX and were followed for cardiovascular (CV) mortality and major CV events (death, transplantation, left ventricular assist device implantation). Net reclassification improvement (NRI) for the score and each of its components were determined at 3 years. The VE/VCO2 slope was the strongest predictor of risk and was attributed a relative weight of 7, with weighted scores for abnormal heart rate recovery, oxygen uptake efficiency slope, end-tidal CO2 pressure, and peak VO2 having scores of 5, 3, 3, and 2, respectively. A summed score of >15 was associated with an annual mortality rate of 12.2% and a relative risk >9 for total events, whereas a score of <5 was associated with an annual mortality rate of 1.2%. The composite score was the most accurate predictor of CV events among all CPX responses considered (C indexes, 0.70 for CV mortality and 0.72 for the composite outcome). Each component of the score provided significant NRI compared with peak VO2 (category-free NRI, 0.61-0.77), and the score provided significant NRI above clinical risk factors for both CV events and mortality (NRI, 0.63 and 0.65 for CPX score compared with clinical variables alone).These results validate the application of a simple, integrated multivariable score based on readily available CPX responses. The article presents a study that determined if ventilatory efficiency and peak oxygen consumption, which provide information on heart patients undergoing cardiopulmonary exercise testing (CPX), can be used for long term prognosis. The study revealed that values taken from the ventilatory efficiency slope is an excellent predictor of adverse effects during a CPX patient's period of follow-ups and that peak oxygen consumption value provides additive prognosis within the first two years of follow-up.

Abstract: Background: Previous investigations have established the prognostic value of variables obtained from both echocardiography with tissue Doppler Imaging (TDI) and cardiopulmonary exercise testing (CPX) in patients with heart failure (HF). Past prognostic comparisons of variables obtained from these evaluation techniques have been limited to the comparison of left ventricular ejection fraction (LVEF) vs. peak oxygen consumption (VO2). The present investigation undertakes a more thorough correlative and prognostic assessment. Methods: Two hundred and forty-three patients with HF (190 male/53 female, age: 62.2±9.7) underwent echocardiography with TDI and CPX to determine the following variables: 1) the ratio between mitral early (E) to mitral annular (E´) velocity, 2) LVEF, 3) LV mass, 4) LV end systolic volume (LVESV), 5) Peak VO2, 6) The minute ventilation/carbon dioxide production (VE/VCO2) slope, and 7) Exercise oscillatory ventilation (EOV). Results: There were 43 cardiac-related deaths during the four-year tracking period. In the multivariate analysis, E/E′ was the strongest prognostic variable (Chi-square: 46.1, p < 0.001). LV mass (Residual chi-square: 16.8, p < 0.001), LVESV (Residual chi-square: 8.0, p =0.005) and the VE/VCO2 slope (Residual chi-square: 4.6, p = 0.03) all added significant predictive value and were retained in the regression. Conclusions: The results of the present study indicate several variables obtained from echocardiography with TDI are prognostically important in HF. The VE/VCO2 slope, which is one of the strongest prognostic markers obtained from CPX, adds prognostic value to these variables. Abstract: Chronic phosphodiesterase-5 inhibition improves peak oxygen consumption, ventilatory efficiency (VE/VCO2 slope) and pulmonary artery pressure (PAP) in heart failure (HF). In 40 male patients, Sildenafil treatment produced a significant (p <0.001) decrease in dyspnea upon exertion (DOE) at maximal exercise. The correlations between the change in systolic PAP and both the change in the VE/VCO2 slope (r =0.57, p <0.001) and DOE at maximal exercise (r s =0.49, p <0.001) were significant. DOE at maximal exercise is significantly reduced and the degree of improvement in PAP is reflected by the degree of improvement in the VE/VCO2 slope and DOE following Sildenafil therapy. Abstract: Background: Previous research has demonstrated the prognostic value of cardiopulmonary exercise testing (CPX) in elderly patients with heart failure (HF). Investigations that have comprehensively examined the value of CPX across different age groups are lacking. The purpose of the present investigation was to evaluate the prognostic value of CPX in young, middle-aged and older patients with HF. Methods: A total of 1605 subjects (age: 59.2±13.7years, 78% male) underwent CPX and were subsequently tracked for major cardiac events. Ventilatory efficiency (VE/VCO2 slope) and peak oxygen consumption (VO2), both absolute and percent-predicted, were determined. The prognostic value of these CPX variables was assessed in ≤45, 46–65 and ≥66year subgroups. Results: The three year event rates for major cardiac events in the ≤45, 46–65 and ≥66year subgroups were 8.8%, 6.0% and 5.7%, respectively. The VE/VCO2 slope (Hazard ratio≥1.07, p<0.001), peak VO2 (Hazard ratio≤0.87, p<0.001) and percent-predicted peak VO2 (Hazard ratio≤0.98, p<0.001) were all significant prognostic markers in each age subgroup. While the VE/VCO2 slope carried the greatest prognostic strength, peak VO2 and percent-predicted peak VO2 were retained in multivariate analyses (Residual Chi-Square≥5.2, p<0.05). With respect to peak VO2, the actual value was the more robust prognostic marker in the ≤45 and ≥66year subgroups while the percent-predicted expression provided better predictive resolution in subjects who were 46–65years old. Several investigations have demonstrated that higher body weight, as assessed by the body mass index, is associated with improved prognosis in patients with heart failure (HF). The purpose of the present investigation was to assess the influence of HF etiology on the prognostic ability of the body mass index in a cohort undergoing cardiopulmonary exercise testing. A total of 1,160 subjects were included in the analysis. All subjects underwent cardiopulmonary exercise testing, at which the minute ventilation/carbon dioxide production slope and peak oxygen consumption were determined. In the overall group, 193 cardiac deaths occurred during a mean follow-up of 30.7 ± 25.6 months (annual event rate 6.0%). The subjects classified as obese consistently had improved survival compared to those classified as normal weight (overall survival rate 88.0% vs ≤81.1%, p <0.001). Differences in survival according to HF etiology were observed for those classified as overweight. In the ischemic subgroup, the survival characteristics for the overweight subjects (75.5%) were similar those for subjects classified as normal weight (81.1%). The converse was true for the nonischemic subgroup, for whom the survival trends for the obese (86.4%) and overweight subjects (88.4%) were similar. The minute ventilation/carbon dioxide production slope was the strongest prognostic marker (chi-square ≥43.4, p <0.001) for both etiologies, and the body mass index added prognostic value (residual chi-square ≥4.7, p <0.05). In conclusion, these results further support the notion that obesity confers improved prognosis in patients with HF, irrespective of the HF etiology. Moreover, the body mass index appears to add predictive value during the cardiopulmonary exercise testing assessment. Abstract: Background: The Weber classification system is a well established method for categorizing patients according to peak oxygen consumption (VO2), but it is unknown whether ventilatory efficiency adds prognostic value. The purpose of the current study was to assess the added prognostic value of the minute ventilation/carbon dioxide production (VE/VCO2) slope to the Weber classification system in patients diagnosed with heart failure (HF). Methods: Five hundred and forty-eight subjects with HF participated in this analysis. Mean left ventricular ejection fraction was 32.9±13.3%. Subjects were tracked for cardiac-related mortality following the exercise test. Results: The numbers of subjects in Weber Classes A–D were 144, 119, 212 and 71, respectively. One hundred and eight major cardiac events (91 deaths, 10 emergent heart transplants and 7 LVAD implantations) occurred during a mean tracking period of 33.8±28.6 months. Receiver operating characteristic (ROC) curve analysis revealed the VE/VCO2 slope prognostic classification schemes were significant in each of the four Weber classes (A: area=0.84, optimal threshold=31.3, 79% sensitivity/90% specificity, p <0.01; B: area=0.66, optimal threshold=33.9, 73% sensitivity/67% specificity, p <0.05; C: area=0.71, optimal threshold=36.0, 65% sensitivity/68% specificity, p <0.01; D: area=0.66, optimal threshold=41.8, 68% sensitivity/58% specificity, p <0.05). Conclusions: These results indicate that the VE/VCO2 slope improves the identification of individuals at higher risk for mortality within each Weber class.
