Study Design: International, multicenter, prospective, longitudinal observational cohort., Objective: To assess how new motor deficits affect patient reported quality of life scores after adult deformity surgery., Summary of Background Data: Adult spinal deformity surgery is associated with high morbidity, including risk of new postoperative motor deficit. It is unclear what effect new motor deficit has on Health-related Quality of Life scores (HRQOL) scores., Methods: Adult spinal deformity patients were enrolled prospectively at 15 sites worldwide. Other inclusion criteria included major Cobb more than 80°, C7-L2 curve apex, and any patient undergoing three column osteotomy. American Spinal Injury Association (ASIA) scores and standard HRQOL scores were recorded pre-op, 6 weeks, 6 months, and 2 years., Results: Two hundred seventy two complex adult spinal deformity (ASD) patients enrolled. HRQOL scores were worse for patients with lower extremity motor score (LEMS). Mean HRQOL changes at 6 weeks and 2 years compared with pre-op for patients with motor worsening were: ODI (+12.4 at 6 weeks and -4.7 at 2 years), SF-36v2 physical (-4.5 at 6 weeks and +2.3 at 2 years), SRS-22r (0.0 at 6 weeks and +0.4 at 2 years). Mean HRQOL changes for motor-neutral patients were: ODI (+0.6 at 6 weeks and -12.1 at 2 years), SF-36v2 physical (-1.6 at 6 weeks and +5.9 at 2 years), and SRS-22r (+0.4 at 6 weeks and +0.7 at 2 years). For patients with LEMS improvement, mean HRQOL changes were: ODI (-0.6 at 6 weeks and -16.3 at 2 years), SF-36v2 physical (+1.0 at 6 weeks and +7.0 at 2 years), and SRS-22r (+0.5 at 6 weeks and +0.9 at 2 years)., Conclusion: In the subgroup of deformity patients who developed a new motor deficit, total HRQOLs and HRQOL changes were negatively impacted. Background Context: Significant variability in neurologic outcomes after surgical correction for adult spinal deformity (ASD) has been reported. Risk factors for decline in neurologic motor outcomes are poorly understood., Purpose: The objective of the present investigation was to identify the risk factors for postoperative neurologic motor decline in patients undergoing complex ASD surgery., Study Design/setting: This is a prospective international multicenter cohort study., Patient Sample: From September 2011 to October 2012, 272 patients undergoing complex ASD surgery were prospectively enrolled in a multicenter, international cohort study in 15 sites., Outcome Measures: Neurologic decline was defined as any postoperative deterioration in American Spinal Injury Association lower extremity motor score (LEMS) compared with preoperative status., Methods: To identify risk factors, 10 candidate variables were selected for univariable analysis from the dataset based on clinical relevance, and a multivariable logistic regression analysis was used with backward stepwise selection., Results: Complete datasets on 265 patients were available for analysis and 61 (23%) patients showed a decline in LEMS at discharge. Univariable analysis showed that the key factors associated with postoperative neurologic deterioration included older age, lumbar-level osteotomy, three-column osteotomy, and larger blood loss. Multivariable analysis revealed that older age (odds ratio [OR]=1.5 per 10 years, 95% confidence interval [CI] 1.1-2.1, p=.005), larger coronal deformity angular ratio [DAR] (OR=1.1 per 1 unit, 95% CI 1.0-1.2, p=.037), and lumbar osteotomy (OR=3.3, 95% CI 1.2-9.2, p=.022) were the three major predictors of neurologic decline., Conclusions: Twenty-three percent of patients undergoing complex ASD surgery experienced a postoperative neurologic decline. Purpose: To determine the incidence of pseudarthrosis at the osteotomy site after three-column spinal osteotomies (3-COs) with posterior column reconstruction., Methods: 82 consecutive adult 3-COs (66 patients) with a minimum of 2-year follow-up were retrospectively reviewed. All cases underwent posterior 3-COs with two-rod constructs. The inferior facets of the proximal level were reduced to the superior facets of the distal level. If that was not possible, a structural piece of bone graft either from the local resection or a local rib was slotted in the posterior column defect to re-establish continual structural posterior bone across the lateral margins of the resection. No interbody cages were used at the level of the osteotomy., Results: There were 34 thoracic osteotomies, 47 lumbar osteotomies and one sacral osteotomy with a mean follow-up of 52 (24-126) months. All cases underwent posterior column reconstructions described above and the addition of interbody support or additional posterior rods was not performed for fusion at the osteotomy level. Among them, 29 patients underwent one or more revision surgeries. There were three definite cases of pseudarthrosis at the osteotomy site (4%). Six revisions were also performed for pseudarthrosis at other levels., Conclusion: Restoration of the structural integrity of the posterior column in three-column posterior-based osteotomies was associated with > 95% fusion rate at the level of the osteotomy. Pseudarthrosis at other levels was the second most common reason for revision following adjacent segment disease in the long-term follow-up.

Study Design: Retrospective case series., Objective: To compare the early results of posterior column (PCO) and three-column (3CO) osteotomies performed in patients with previously fused idiopathic scoliosis and review their abilities to achieve coronal correction of residual deformities., Summary of Background Data: Residual deformity of previously fused AIS can accelerate adjacent segment degeneration secondary to lowest instrumented vertebra (LIV) tilt and rotation. Many of these patients are not satisfied with their cosmetic appearance and would choose revising the deformity when future surgery is indicated., Methods: The data from 29 consecutive patients who underwent PCOs or 3COs for late revisions of idiopathic scoliosis were reviewed. Measurements included Cobb angle, focal osteotomy angle, and coronal balance. Perioperative data, complications, and patient-reported outcomes were also reviewed., Results: Fourteen patients were treated with PCOs and 15 with 3COs. Global coronal correction was equal between the two groups. In the PCO group, where patients underwent a mean of 2.4 osteotomies, 20.2° of correction was obtained compared to 19.5° in the 3CO group (p = .33), which all underwent single osteotomies. The average coronal correction was 9.2°/osteotomy for the PCO group and 14.1°/osteotomy for the 3CO group (p < .01). Estimated blood loss was 1,417.5 mL in the PCO group compared to 3,199.3 in the 3CO group (p < .01). Five patients (36%) had intraoperative complications in the PCO group compared to 12 (80%) in the 3CO group (p < .05). There were no differences in operative times, length of stay, or patient-reported outcomes between groups., Conclusion: PCOs and 3COs performed in patients with previously fused spines for idiopathic scoliosis are effective in achieving residual deformity correction. In cases of posterior fusions, where the patient has a mobile anterior column, PCOs should be considered over 3COs because of their decreased risk of blood loss and complications., (Copyright © 2017 Scoliosis Research Society. Study Design: Retrospective analysis., Objective: To demonstrate the effectiveness of hook-rod constructs in closing thoracic osteotomies safely and effectively., Summary of Background Data: The outcomes of hook-rod instrumentation in osteotomies for the correction of kyphosis at the lumbar region of the spine have been described. Little literature exists on the outcomes at the thoracic level., Methods: The radiographs and clinical scores of 38 patients who underwent pedicle subtraction osteotomy or Smith-Petersen osteotomy in the thoracic spine with the osteotomies closed using a central rod were retrospectively reviewed. Measurements included osteotomy angle, thoracic kyphosis (T2-T12), and maximum kyphosis. Perioperative and long-term complications were reviewed., Results: Thirty-eight patients underwent thoracic level osteotomies. There were 8 males and 30 females with a mean age of 51.9 years (range, 18-76 yr) at the time of surgery. The mean construct length was 13.2 levels (4-25). Kyphosis correction was equal in the 2 groups. In the pedicle subtraction osteotomy group, a mean of 24.7° (4°-47°) correction was obtained through the osteotomies compared with 24.0° (9°-65°) in the Smith-Petersen osteotomy group. Correction per osteotomy was 23.7° (4°-47°) in the pedicle subtraction osteotomy group compared with 11.8° (2.8°-46.0°) in the Smith-Petersen osteotomy group. No difference in the amount of correction achieved at the different regions of the thoracic spine was observed with either type of osteotomy with central rod closure., Conclusion: Central hook-rod constructs provide a safe and effective means of closing thoracic osteotomies and result in good correction of rigid sagittal plane deformities., Level of Evidence: 4.

Study Design: Case report., Objective: To surgically regain shoulder balance in patients with adolescent idiopathic scoliosis after loss of alignment after posterior fusion for Lenke II deformity correction., Summary of Background Data: Shoulder balance is known to have a large effect on patient satisfaction after deformity correction. Previous studies have outlined guidelines for determining levels of instrumentation to prevent postoperative high left shoulder. However, to our knowledge, no study has provided instructions on how to correct coronal imbalance in patients with previously fused scoliosis. We describe a case using a T4 unilateral pedicle subtraction osteotomy and contralateral Smith-Petersen osteotomy to treat high left shoulder in a patient who had previously undergone posterior instrumented fusion for adolescent idiopathic scoliosis., Methods: The radiographs and clinical charts were reviewed for a 17-year-old female patient treated with a revision fusion and modified T4 hemivertebrectomy for a persistently high left shoulder after previous correction of a Lenke II idiopathic scoliosis., Results: A reduction in the T1 tilt angle from 19.2° to 10.1° and an improvement in the coronal Cobb angle of the proximal thoracic curve from 37° to 17° were obtained. Shoulder balance was greatly improved., Conclusion: A proximal thoracic partial vertebrectomy with unilateral pedicle subtraction osteotomy and contralateral Smith-Petersen osteotomy is a technique that can be used to successfully correct fixed shoulder imbalance after posterior instrumented fusion of a double thoracic adolescent idiopathic scoliosis., Level of Evidence: N/A.

Study Design: Retrospective review of prospectively collected data on the neuromonitoring changes recorded during a consecutive series of cord level 3-column posterior spinal osteotomies for the correction of rigid pediatric spinal deformities in children between 2005 and 2012., Objective: To review the neuromonitoring changes observed during the performance of these procedures, to highlight the high-risk steps, and to describe actions taken to avert major neurological injury., Summary of Background Data: Significant motor evoked potentials (MEP) changes are common during the performance of spinal osteotomies in children. The real-time intraoperative information provided by MEPs can provide the necessary information to direct key surgical decisions., Methods: The neuromonitoring changes occurring during the performance of 37 3-column, cord level, posterior spinal osteotomies in 28 patients were recorded. The procedures were divided, for comparative purposes, into 2 groups based on the presence or absence of alerts. A decrease in somatosensory evoked potentials and transcranial MEPs greater than 50% of baseline was considered an alert. Alerts were classified chronologically as type I: prior to decompression, type II: occurring during decompression and bone resection, type III: occurring after osteotomy closure., Results: Somatosensory evoked potential alerts occurred in 3 patients, all of whom had significant MEP alerts. There were 2 type I, 15 type II, and 6 type III MEP alerts. Increasing blood pressure improved MEPs in all with the exception of 8 type II and 4 type III. The unresponsive 8 type II alerts were treated with osteotomy closure with the expectation that spinal shortening would decompress the spinal cord and improve spinal cord perfusion. The unresponsive 4 type III alerts all responded to reopening, manipulation, and subsequent reclosure of the osteotomy either with a cage or less correction. There were 5 immediate postoperative motor deficits. No patient had a permanent deficit., Conclusion: Changes unresponsive to increasing blood pressure occurring during decompression and bone resection (type II) responded well to osteotomy closure. Unresponsive changes during osteotomy closure (type III) were treated successfully with opening the osteotomy, cage adjustment, and less correction.

Study Design: Case report., Objective: To determine the viability and safety of Reverse Smith Petersen Osteotomy (RSPO) to re-established sagittal balance in patients with fixed lumbar hyperlordosis., Summary and Background Data: Lumbar hyperlordosis is seen as a compensatory mechanism in thoracic Scheuermann disease and in sagittal decompensation in severe neuromuscular scoliosis. Hyperlordosis may also be seen after overcorrection with spinal osteotomies, but rarely causes clinically significant negative sagittal imbalance because of the thoracic compensation. We describe a case using a kyphosing osteotomy to treat hyperlordosis in a patient that was treated with a pedicle subtraction osteotomy for post-Harrington kyphosis., Methods: The radiographs and clinical chart were reviewed of a patient treated with a RSPO at L2-L3 to correct the negative sagittal imbalance created by a previous extension of her fusion to the sacrum with a pedicle subtraction osteotomy., Results: A reduction in the lumbar lordosis by 20 degrees at L2-L3 and restoration of the global sagittal balance was achieved with the RSPO., Conclusion: RSPO is a viable surgical technique that can be used to re-establish sagittal balance in patients with fixed lumbar hyperlordosis. Appreciation of a patient's balanced sagittal alignment and available compensatory mechanisms can help ensure appropriate osteotomies are performed.

Study Design: Retrospective review of consecutive case series., Objective: To evaluate the early surgical results and complications of thoracic transdiscal osteotomies and vertebral shortening for the treatment of thoracic discitis/osteomyelitis., Summary of Background Data: Thoracic discitis/osteomyelitis leads to collapse of the disc space and/or vertebral body. We propose a novel technique to achieve the same goals as anterior column reconstruction through an entirely posterior approach. Shortening of the vertebral column provides structural support without the morbidity of an anterior approach., Methods: Following REB approval, retrospective review of the charts of five patients that underwent posterior only thoracic transdiscal osteotomies and vertebral shortening for discitis/osteomyelitis was carried out. Posterior only surgery was performed in these patients with excision of the affected disc space and corresponding posterior elements. Instrumented fusion was performed across the segment spanning multiple vertebral levels. Clinical outcome, radiographic correction, and perioperative complications were analyzed., Results: Three patients had bacterial discitis, and 2 had tuberculosis. Mean age at the time of surgery was 61 years (50-76). Mean follow-up was 45 months (25-63). There was no neurologic deterioration; 2 patients with Frankel grade B improved to grade D and E, respectively. Mean kyphosis corrected from 36 degrees (14-90) to 4 degrees (0-8), and the mean construct spanned 9 levels (6-15). No major complications were encountered during surgery. Two patients underwent revision surgeries, 1 patient died of unrelated causes at 6 months. All patients were treated with a full course of postoperative antibiotic treatment. No cases of recurrent infection were recorded., Conclusion: Thoracic transdiscal osteotomy with vertebral shortening is a safe and effective option for the treatment of infectious discitis/osteomyelitis with associated kyphosis. With adjuvant antibiotics, it effectively eradicates the infection through a posterior only approach, avoiding the need for anterior procedures and long anterior struts.

Study Design: Case report., Objective: To describe a novel technique to remove anterior instrumentation from a posterior approach while performing posterior-based osteotomies for spinal deformities., Summary of Background Data: Posterior-based osteotomies such as pedicle subtraction osteotomies (PSOs) and vertebral column resections are performed to restore sagittal alignment. The removal of previously placed anterior implants at the desired osteotomy level can often be challenging. We propose a technique for the removal of anterior instrumentation through a posterior approach to facilitate osteotomy closure and deformity correction, while avoiding the need for an anterior incision., Methods: A 34-year-old woman presented with a residual deformity after several anterior and posterior procedures. The residual coronal Cobb angle measured 60 degrees between T7 and L2, with a 46 degrees thoracolumbar kyphosis between T10 and L2. The screw head at the desired osteotomy level was in close proximity to the liver after the previous right-sided thoracoabdominal approach. Therefore, the T11 anterior screw was accessed through a posterior costotransversectomy approach and disconnected from the rod proximally and distally with a high-speed side-cutting burr. A portion of the right lateral vertebral body of T11 was removed to expose the neck of the screw, which was separated from the shaft with the same burr. A PSO was performed at T11 and the remaining screw shank was removed with the posterior-based osteotomy., Results: No major complications were encountered during the procedure. The anterior screw at T11 was removed from posteriorly, and the PSO was completed successfully. Postoperative recovery was without incident, and the patient was very satisfied with her results., Conclusion: This technique describes a novel, safe, and effective method to deal with anterior instrumentation from the posterior approach while performing posterior-based osteotomies for rigid spinal deformities.

Study Design: A retrospective analysis of prospectively collected data of 102 consecutive adult patients who underwent intraoperative neurophysiological monitoring (IOM) during spinal deformity corrective surgery., Objective: To report the sensitivity and specificity of combined IOM in this study population using the postoperative neurologic examination as the "gold standard.", Summary of Background Data: IOM is recommended during corrective spinal surgery and has been widely used in the pediatric deformity population. However, there are limited data describing the application of IOM in adults undergoing spinal deformity corrective surgery., Methods: The study group consisted of 102 patients undergoing spinal deformity corrective surgery between 2001 and 2004. Patients were monitored using at least 2 or more electrophysiological methods including somatosensory-evoked potentials (SSEP), motor-evoked potentials (MEP), and electromyography (EMG)., Results: The mean age of patients was 41.5 years (+/-17). The majority of the operative procedures involved instrumented fusion from thoracic to lumbar/sacral spine (n = 55), thoracic-pelvis fusion (n = 26), and a combined total of 32 osteotomies (including 25 pedicle subtraction osteotomies and 7 Smith-Peterson osteotomies). SSEPs were recorded successfully in 101 (99%), EMGs in 89 of 102 (87%), and MEPs in 12 of 16 (75%). Five cases were true positives (4.95%), and these were all detected by combined monitoring (2-SSEP, 2-EMG, 1-MEP). There were no false positives with SSEPs, but EMG resulted in 30 of 89 (34%) false positives. There were 4 false negatives with SSEPs, which reduced its sensitivity to 33%. There was 1 false negative with EMG, and 0 with MEPs. When these results were collated, the overall sensitivity of combined multimodality IOM in this adult deformity series was 100%, specificity 84.3%, PPV 13.9%, and NPV 97%. The combined sensitivity in the osteotomy group (n = 32) was 67%, specificity 98%, PPV 80%, and NPV 96%. In comparison, there were no IOM abnormalities in those patients who had in situ/minor corrective procedures (n = 18; largely adult degenerative scoliosis)., Conclusion: Multimodality IOM of spinal cord sensory and motor function during surgical correction of adult spinal deformity is feasible and provides useful neurophysiological data with an overall sensitivity of 100% and a specificity of 84.3% (67% and 98%, respectively in patients undergoing major deformity correction).

Background: Fixed sagittal imbalance (a syndrome in which the patient is only able to stand with the weight-bearing line in front of the sacrum) has many etiologies. The most commonly reported technique for correction is the Smith-Petersen osteotomy. Few reports on pedicle subtraction procedures (resection of the posterior elements, pedicles, and vertebral body through a posterior approach) are available in the peer-reviewed literature. We are aware of no report involving a substantial number of patients with coexistent scoliosis who underwent pedicle/vertebral body subtraction for the treatment of fixed sagittal imbalance., Methods: Twenty-seven consecutive patients in whom sagittal imbalance was treated with lumbar pedicle subtraction osteotomy at one institution were analyzed. Radiographic analysis included assessment of thoracic kyphosis, lumbar lordosis, lordosis through the pedicle subtraction osteotomy site, and the C7 sagittal plumb line. Outcomes analysis was performed with use of a before-and-after pain scale, items from the Oswestry questionnaire, and the Scoliosis Research Society (SRS) questionnaire after a minimum duration of follow-up of two years. Complications and radiographic findings were also analyzed for the entire group., Results: Overall, the average increase in lordosis was 34.1 degrees and the average improvement in the sagittal plumb line was 13.5 cm. One patient had development of a lumbar pseudarthrosis through the area of pedicle subtraction osteotomy, and six patients had development of a thoracic pseudarthrosis. Two patients had development of increased kyphosis at L5/S1, caudad to the fusion, resulting in some loss of sagittal correction. There were significant improvements in the overall Oswestry score (p < 0.0001) and the pain-scale score (p = 0.0002). Most patients reported improvement in terms of pain and self-image as well as overall satisfaction with the procedure., Conclusions: Pedicle subtraction osteotomy is a useful procedure for patients with fixed sagittal imbalance. A worse clinical result is associated with increasing patient comorbidities, pseudarthrosis in the thoracic spine, and subsequent breakdown caudad to the fusion.

Study Design: Radiographic analysis, outcomes analysis (pain scale, Oswestry, SRS-24), and accumulation of complications. Outcomes and complications collected prospectively. Radiographic analysis performed retrospectively., Objectives: To assess the benefits and stress complications of pedicle subtraction osteotomies for patients with fixed sagittal imbalance., Summary of Background Data: Few reports on pedicle subtraction osteotomies exist in the peer-review literature for conditions other than trauma and ankylosing spondylitis., Materials and Methods: Thirty-three consecutive patients with sagittal imbalance treated with lumbar pedicle subtraction osteotomy at one institution (minimum 2-year follow-up) were analyzed. Complications were also analyzed for the entire group of consecutive pedicle subtraction osteotomies done at our institution to date (n = 66)., Results: For the 33 patients with minimum 2-year follow-up, there were significant improvements in the overall Oswestry score (P 0.0001) and pain score (P = 0.0001). Most patients reported improvement in pain and self-image and reported overall satisfaction based on ultimate SRS-24 questionnaire. There was one pseudarthrosis in the lumbar spine through an area of pedicle subtraction osteotomy (area of previous laminectomy and nonunion), and six patients had thoracic pseudarthroses (levels other than the osteotomy level) and one patient had a pseudarthrosis at L5-S1. Two patients had acute angular kyphosis at the thoracolumbar junction at the proximal end of the construct. Five patients who experienced transient neurologic deficits resolved their deficits after central canal enlargement., Conclusions: The clinical result with pedicle subtraction osteotomy is reduced with pseudarthrosis in the thoracic or lumbar spine and subsequent breakdown adjacent to the fusion. For patients with a degenerative sagittal imbalance etiology the results were worse and the complications were higher. Central canal enlargement is critical.

BackgroundThe reported neurologic complication rate following surgery for complex adult spinal deformity (ASD) is variable due to several factors. Most series have been retrospective with heterogeneous patient populations and use of nonuniform neurologic assessments. The aim of this study was to prospectively document lower extremity motor function by means of the American Spinal Injury Association (ASIA) lower extremity motor score (LEMS) before and through 2 years after surgical correction of complex ASD.MethodsThe Scoli-RISK-1 study enrolled 272 patients with ASD, from 15 centers, who had undergone primary or revision surgery for a major Cobb angle of ≥80°, corrective osteotomy for congenital spinal deformity or as a revision procedure for any type of deformity, and/or a complex 3-column osteotomy.ResultsOne of 272 patients lacked preoperative data and was excluded from the analysis, and 62 (22.9%) of the remaining 271 patients, who were included, lacked a 2-year postoperative assessment. Patients with no preoperative motor impairment (normal LEMS group; n = 203) had a small but significant decline from the mean preoperative LEMS value (50) to that at 2 years postoperatively (49.66 [95% confidence interval = 49.46 to 49.85]; p = 0.002). Patients who did have a motor deficit preoperatively (n = 68; mean LEMS, 43.79) had significant LEMS improvement at 6 months (47.21, p < 0.001) and 2 years (46.12, p = 0.003) postoperatively. The overall percentage of patients (in both groups combined) who had a postoperative LEMS decline, compared with the preoperative value, was 23.0% at discharge, 17.1% at 6 weeks, 9.9% at 6 months, and 10.0% at 2 years.ConclusionsThe percentage of patients who had a LEMS decline (compared with the preoperative score) after undergoing complex spinal reconstructive surgery for ASD was 23.0% at discharge, which improved to 10.0% at 2 years postoperatively. These rates are higher than previously reported, which we concluded was due to the prospective, strict nature of the LEMS testing of patients with these challenging deformities.Level of evidenceTherapeutic Level IV. Study Design: Prospective cohort. Objective: To prospectively evaluate PROs up to 5-years after complex ASD surgery. Methods: The Scoli-RISK-1 study enrolled 272 ASD patients undergoing surgery from 15 centers. Inclusion criteria was Cobb angle of >80°, corrective osteotomy for congenital or revision deformity, and/or 3-column osteotomy. The following PROs were measured prospectively at intervals up to 5-years postoperative: ODI, SF36-PCS/MCS, SRS-22, NRS back/leg. Among patients with 5-year follow-up, comparisons were made from both baseline and 2-years postoperative to 5-years postoperative. PROs were analyzed using mixed models for repeated measures. Results: Seventy-seven patients (28.3%) had 5-year follow-up data. Comparing baseline to 5-year data among these 77 patients, significant improvement was seen in all PROs: ODI (45.2 vs. 29.3, P < 0.001), SF36-PCS (31.5 vs. 38.8, P < 0.001), SF36-MCS (44.9 vs. 49.1, P = 0.009), SRS-22-total (2.78 vs. 3.61, P < 0.001), NRS-back pain (5.70 vs. 2.95, P < 0.001) and NRS leg pain (3.64 vs. 2.62, P = 0.017). In the 2 to 5-year follow-up period, no significant changes were seen in any PROs. The percentage of patients achieving MCID from baseline to 5-years were: ODI (62.0%) and the SRS-22r domains of function (70.4%), pain (63.0%), mental health (37.5%), self-image (60.3%), and total (60.3%). Surprisingly, mean values (P > 0.05) and proportion achieving MCID did not differ significantly in patients with major surgery-related complications compared to those without. Conclusions: After complex ASD surgery, significant improvement in PROs were seen at 5-years postoperative in ODI, SF36-PCS/MCS, SRS-22r, and NRS-back/leg pain. No significant changes in PROs occurred during the 2 to 5-year postoperative period. Study Design. A retrospective analysis. Objective. The purpose of this study was to determine whether the deformity angular ratio (DAR) can reliably assess the neurological risks of patients undergoing deformity correction. Summary of Background Data. Identifying high-risk patients and procedures can help ensure that appropriate measures are taken to minimize neurological complications during spinal deformity corrections. Subjectively, surgeons look at radiographs and evaluate the riskiness of the procedure. However, 2 curves of similar magnitude and location can have significantly different risks of neurological deficit during surgery. Whether the curve spans many levels or just a few can significantly influence surgical strategies. Lenke et al have proposed the DAR, which is a measure of curve magnitude per level of deformity. Methods. The data from 35 pediatric spinal deformity correction procedures with thoracic 3-column osteotomies were reviewed. Measurements from preoperative radiographs were used to calculate the DAR. Binary logistic regression was used to model the relationship between DARs (independent variables) and presence or absence of an intraoperative alert (dependent variable). Results. In patients undergoing 3-column osteotomies, sagittal curve magnitude and total curve magnitude were associated with increased incidence of transcranial motor evoked potential changes. Total DAR greater than 45 ° per level and sagittal DAR greater than 22 ° per level were associated with a 75% incidence of a motor evoked potential alert, with the incidence increasing to 90% with sagittal DAR of 28 ° per level. Conclusion. In patients undergoing 3-column osteotomies for severe spinal deformities, the DAR was predictive of patients developing intraoperative motor evoked potential alerts. Identifying accurate radiographical, patient, and procedural risk factors in the correction of severe deformities can help prepare the surgical team to improve safety and outcomes when carrying out complex spinal corrections.
