A 20-year-old woman with right-sided congenital cranial dysinnervation disorder (exotropia, ophthalmoplegia, and ptosis) presented for strabismus surgery. Exploration of the right eye confirmed abnormal extraocular muscles. Lateral rectus recession with medial rectus plication and inferior oblique recession improved primary position ocular alignment. However, an unusual pattern of inferior chemosis developed postoperatively and persisted during the next month. Excision and histologic examination of this tissue revealed areas of lacrimal gland and cartilage with surrounding inflammation, suggestive of a disrupted orbital choriostoma. Probing of the lacrimal outflow system is commonly performed by ophthalmologists for treating nasolacrimal duct obstruction in infants or for intubating the lacrimal system. Visualization of the probe tip in the inferior meatus during probing insures the surgeon of reaching the nose with the probe. Similarly, visualization of the probe is the key to success in intubation irrespective of the retrieval system used. Rotation of a curved metal probe plus infracturing of the inferior turbinate after shrinking the nasal mucosa with cocaine allows such visualization in most instances.

Most penetrating or lacerating injuries of the eye in children justify examination under anesthesia to avoid further harm to an uncooperative patient. The pediatrician in doubt should merely apply a sterile dressing and have an ophthalmologist examine the injury in hospital. Nonperforating injuries may result in severe bleeding 48 to 72 hours later; this may be averted by bandaging the eyes and maintaining rest for four or five days. Removal of foreign bodies should be followed by application of antibiotic ointment and patching to prevent contamination. Congenital stenosis of the lacrimal duct may clear spontaneously or through application of decongestants and sympathomimetic drops. More severe effects, especially infection, justify probing at six months or earlier. The operation should be done under general anesthesia, preferably in hospital.Acute conjunctivitis is best treated by local application of antibiotics or sulfonamides only. Chronic infections may be better managed with the addition of corticosteroids, which reduce local inflammation and control bacterial reaction. Bacterial study should be done only if empirical antibiotic therapy fails. Bacterial desensitization may be helpful. The same methods are effective in blepharitis, aided by hygienic measures. Corticosteroids are most useful in allergic inflammations.Refractive difference is difficult to test before a child can read, and apparent defects may be due to lack of cooperation. Marked inequality of the eyes may signify organic disorder. Strabismus, on the other hand, can be detected as early as 12 or 15 months and should be treated as early as possible by proper lenses, surgery, or both. Pediatricians and parents should be aware that many children appear to have strabismus because of wide epicanthi and deep-set eyes.

It underscores the advancements in the field, from decellularized grafts and acellular matrices to innovative natural and synthetic polymers, and explores their applications in lacrimal gland tissue engineering, including the promise of 3D bioprinting technologies. This review highlights the importance of multidisciplinary collaboration between material scientists and clinicians in enhancing surgical outcomes and patient quality of life, emphasizing that such cooperation is pivotal for translating benchtop research into bedside applications. To review the literature on the adjuvant use of mitomycin C (MMC) during dacryocystorhinostomy (DCR) in adults with primary nasolacrimal duct obstructions (NLDOs) to determine the efficacy in improving functional and anatomic outcomes with an acceptable level of risk. A literature search conducted in November 2020 and updated in November 2022 yielded 137 articles. Twenty-four articles met the inclusion criteria and were rated for level of evidence by the panel methodologist. Inclusion criteria required controlled studies on the effect of MMC on outcomes of external, endoscopic endonasal, or diode laser-assisted transcanalicular DCR in adults with primary acquired nasolacrimal obstruction with 6 months minimum follow-up and at least 10 participants. Six of the 24 articles were rated level I evidence, 15 level II , and 3 level III. In primary external DCR, MMC significantly improved functional outcomes in 3 of 9 series. In primary endoscopic endonasal DCR, MMC significantly improved functional outcomes in 1 of 9 series. In revision endoscopic endonasal DCR, MMC significantly improved functional success in 1 of 3 series. The use of MMC did not improve outcomes statistically in any diode laser-assisted transcanalicular DCR studies. Concentrations of MMC ranged from 0.05 to 1 mg/ml, with 0.2 mg/ml used most frequently in 12 series, with duration of application ranging from 2 to 30 minutes. Ostium size was significantly larger in MMC groups than in control groups at 6 months after surgery in 4 of 5 reporting studies. However, these larger ostia did not confer higher functional success rates. Reporting of adverse events related to MMC were rare, with delayed cutaneous wound healing reported in 1 of 750 patients. Intraoperative use of MMC in external and endoscopic endonasal DCR has been shown to improve functional and anatomic outcomes compared with controls in some series, but there is no agreement on the recommended concentration or application time for MMC in DCR. The data support that MMC use can result in a larger ostium size, decreased granulation tissue formation, and a decreased number of postoperative nasal debridements compared with controls, but this does not translate into improved functional success. The lacrimal gland is of central interest in ophthalmology both as the source of the aqueous component of tear fluid and as the site of autoimmune pathology in the context of Sjogren's syndrome (SjS). To provide a foundational description of mouse lacrimal gland cell types and their patterns of gene expression, we have analyzed single-cell transcriptomes from wild-type (Balb/c) mice and from two genetically based SjS models, MRL/lpr and NOD (nonobese diabetic).H2b, and defined the localization of multiple cell-type-specific protein and mRNA markers. This analysis has uncovered a previously undescribed cell type, Car6+ cells, which are located at the junction of the acini and the connecting ducts. More than a dozen secreted polypeptides that are likely to be components of tear fluid are expressed by acinar cells and show pronounced sex differences in expression. Additional examples of gene expression heterogeneity within a single cell type were identified, including a gradient of Claudin4 along the length of the ductal system and cell-to-cell heterogeneity in transcription factor expression within acinar and myoepithelial cells. The patterns of expression of channels, transporters, and pumps in acinar, Car6+, and ductal cells make strong predictions regarding the mechanisms of water and electrolyte secretion. In MRL/lpr and NOD.H2b lacrimal glands, distinctive changes in parenchymal gene expression and in immune cell subsets reveal widespread interferon responses, a T cell-dominated infiltrate in the MRL/lpr model, and a mixed B cell and T cell infiltrate in the NOD.H2b model. Objective: To describe the successful use of endoscopy to visualize and place a soft canine ureteral stent to relieve a chronic nasolacrimal duct (NLD) obstruction in a horse. Animal Studied: A 7‐year‐old, Quarter horse gelding. Procedure: Under general anesthesia, retrograde nasolacrimal endoscopy was performed using an 8.5 Fr Storz Flex XC ureteroscope through the nasal punctum (NP). An obstructive web of fibrous tissue was visualized approximately 20 cm proximal to the NP. A 0.035″/150 cm hydrophilic guidewire was passed normograde from the ventral lacrimal punctum and used to puncture the stenotic tissue. Then, a 5.0Fr/70 cm open‐end ureteral catheter was threaded normograde over the guidewire and NLD patency was re‐established. The catheter confirmed a NLD length of 30 cm and was then removed. A 5.0Fr/22–32 cm Universa© Soft Ureteral Stent was threaded normograde over the guidewire until the loops of the stent were exposed at each end. The guidewire was removed and the stent loops were sutured in place. Results: The stent was withdrawn 1 month after the procedure. Telephone follow‐up with the client reported significant improvement in the amount of ocular discharge and decreased sensitivity around the face and ears. Conclusion: Endoscopy is a safe and effective procedure allowing for definitive diagnosis of NLD obstruction and to assist in interventional procedures. Placement of a canine indwelling ureteral stent seems to be an effective alternative treatment option for equine NLD obstruction compared to conventional invasive surgical procedures. We herein report a 76-year-old male who presented to the ophthalmology clinic after complaining from excessive lacrimation for ~6 months prior to presentation. He reports previously undergoing a nasolacrimal recanalization procedure in attempts to resolve his symptoms, but reported no improvement. On examination, a mass on the nasal conjunctiva around the medial canthus of the right eye was noted and subsequently excised. The findings support the diagnosis of low-grade follicular lymphoma. The patient was referred to radiation oncology for radiation therapy and received a total dose of 2400 cGy. Most ocular adnexal lymphomas are B-cell in origin, with follicular lymphomas being one of the rarest forms of such lymphomas. The most common translocation reported in over 85% of follicular lymphomas of the ocular adnexa is t(14; 18) (q32; q21). Purpose: To correlate the clinical data to the histopathological characteristics of lacrimal sac and bone specimens in adult patients undergoing indicated external transcutaneous dacryocystorhinostomy surgery (DCR) for primary acquired nasolacrimal duct obstruction (PANDO) in order to determine the important clinical parameters that may necessitate selective lacrimal biopsy in certain situations. Methods: This is non-comparative, non-controlled descriptive case series study with histopathological correlation. Lacrimal sac and bone specimens for each case were collected during DCR from consecutive patients with PANDO presenting to the outpatient clinic of "Ain Shams University" hospitals, then sent for histopathological examination. Results: Fifty-one lacrimal specimens were obtained from a total of 49 consecutive patients who underwent external DCR for clinical PANDO at the time interval from March 2018 to March 2020 at "Department of Ophthalmology, Ain Shams University" operating theatre. The mean patients' age was 44.82± 13.63 years (range 21-75). Among the 49 patients recruited, 6 (12.2%) were males and 43 (87.8%) were females. Non -specific lacrimal sac pathology was present in all 51 samples (3.9% acute inflammation and 96.1% chronic non-specific inflammation). Of the chronic non-specific inflammation specimens, mild degree of inflammation was seen in 40.8%, 44.9% showed moderate chronic inflammation, whereas only 14.3% showed severe inflammatory changes. Observing the histopathological findings of lacrimal sac and bone specimens in our series showed that the inflammation and capillary proliferation were more evident in lacrimal sac specimens especially in moderate and severe cases (p-value <0.01 and 0.03 respectively). The only two clinical parameters that were strongly correlated with the severity of inflammation are the intraoperative appearance of the sac and the presence of a lacrimal fistula with P-value (<0.01). Conclusions: Lacrimal sac biopsy specimens should not be routinely sent for histopathological assessment after DCR surgery, except for atypical clinical presentation or wary intraoperative appearance. In such situations, lacrimal sac rather than bone specimens should be obtained, being more indicative and revealing of the underlying lacrimal pathology. The most important clinical parameter that strongly correlates with histopathological features is the intraoperative sac appearance. To review the published literature assessing the efficacy and safety of in-office probing compared with facility-based probing to treat congenital nasolacrimal duct obstruction (NLDO). Literature searches were conducted in March 2020 in the PubMed database with no date restrictions and limited to studies published in English and in the Cochrane Library database with no restrictions. The combined searches yielded 281 citations. Of these, 21 articles were deemed appropriate for inclusion in this assessment and assigned a level of evidence rating by the panel methodologist. Four articles were rated level I, 2 articles were rated level II, and 15 articles were rated level III. Treatments consisted of observation, in-office nasolacrimal probing, or facility-based nasolacrimal probing. Success rates and complications or recurrences were recorded from 1 week to 6 months after surgery. Complete resolution of symptoms after surgery ranged from 66% to 95.6% for office-based procedures versus 50% to 97.7% for facility-based procedures. Level I evidence indicated that 66% of cases spontaneously resolved after 6 months of observation in infants between 6 and 10 months of age. Success rates for in-office probing were lower for bilateral than for unilateral NLDO (67% vs. 82%), whereas success rates were high in both unilateral (83%) and bilateral (82%) patients who underwent facility-based probing after 6 months of observation. Cost data did not indicate a definitive cost savings of either treatment method ($562 for in-office vs. $701 for facility-based, depending on cost models predicting spontaneous resolution rates at different ages). No serious adverse events with treatment or anesthesia were reported for either treatment method. Evidence supports the efficacy and safety of both in-office and facility-based surgery for congenital NLDO. However, treating bilateral NLDO in a facility setting may be better. Because a significant percentage of children achieved resolution spontaneously before 12 months of age, deferring treatment until 12 to 18 months of age is a reasonable option. Additional research may address symptom burden on families and the impact of anesthesia and emotional trauma of nonsedated office probings on patients and may explore further the cost of treatment for each treatment method. The coronavirus disease 2019 (COVID-19) pandemic has instigated severe global turmoil both medically and socioeconomically. Research continues to rapidly develop in order to fully comprehend the new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This study focuses on the rare ophthalmologic manifestations of the SARS-CoV-2 disease process in both adults and children. There is evidence to suggest that viral transmission can occur via tears and conjunctival secretions, although it is not a predominant finding. This review considers all the published studies describing ocular findings and SARS-CoV-2 viral transmission through the eye. The review addresses the ongoing debate over the importance of ocular manifestations during this pandemic. The most updated safety guidelines, protocols, timelines of ocular manifestations during the disease course, and treatment recommendations are discussed. The majority of patients with COVID-19 with eye symptoms presented with them initially. It is possible that the virus becomes inoculated at the site of the eye and spreads via the nasolacrimal duct to the respiratory system. There are also some reports which show that ocular findings present later in the disease course, suggestive of a correlation between ocular manifestation and increased disease severity as the infection becomes systemic. We highlight the importance of recognizing conjunctivitis as an early finding of COVID-19, and that testing or appropriate follow-up could be beneficial in both the pediatric and adult populations. Congenital dacryocystocele is a rare clinical condition, more commonly unilateral, secondary to the defective canalization of the nasolacrimal duct. In case of failure of conservative treatment, surgical marsupialization is recommended. We describe the case of a 40‐day‐old male newborn treated by means of microdebrider marsupialization. Purpose: To assess post-operative cosmetic and functional results of external Dacryocystorhinostomy with Subciliary incision in adults with primary naso lacrimal duct obstruction (NLDO). Study Design: Quasi experimental study. Place and Duration of Study: Oculoplasty clinic, Al Ibrahim Eye Hospital, Malir, from July 2016 to September 2017. Material and Methods: Hundred eyes of hundred patients were included. Inclusion criteria was all the patients between the ages of 20 to 70 years having epiphora with diagnosis of NLDO, chronic Dacryocystitis or Mucocele. Patients having ectropion or entropion of inferior punctum were excluded from the study. DCR with intubation was performed through Sub ciliary incision. Inner canthus was photographed with a Nikon D70S digital camera at 1, 3 and 6 months after surgery. Functional success was determined by subjective improvement in watering and patent drainage passage on syringing. Cosmetic result of the scar was assessed by subjective satisfaction of the patient and grading of the subciliary incision scar according to a four level scale by two ophthalmologists. SPSS version 20.0 was used to analyze the data. Result: DCR was done on 100 eyes in 100 patients. Functional and cosmetic success was noted in 90 (90%) eyes. 83 patients attained successful functional outcome, which was confirmed on syringing. On objective grading of the scars by ophthalmologist at the final follow up it was observed that 85 patients had invisible scar. Whereas, 2 had moderately visible scar and 3 patients had minimally visible scar. Subjective grading revealed 85 scars to be invisible, 2 moderately visible and 2 minimally visible. Conclusion: Post-operative cosmetic as well as functional results of subciliary incision in external Dacryocystorhinostomy were found to be highly encouraging. [ABSTRACT FROM AUTHOR]

Eye drops and ointments are the most prescribed methods for ocular drug delivery. However, due to low drug bioavailability, rapid drug elimination, and low patient compliance there is a need for improved ophthalmic drug delivery systems. This study provides insights into the design of a new drug delivery device that consists of an ocular coil filled with ketorolac loaded PMMA microspheres. Nine different ocular coils were created, ranging in wire diameter and coiled outer diameter. Based on its microsphere holding capacity and flexibility, one type of ocular coil was selected and used for further experiments. No escape of microspheres was observed after bending the ocular coil at curvature which reflect the in vivo situation in human upon positioning in the lower conjunctival sac. Shape behavior and tissue contact were investigated by computed tomography imaging after inserting the ocular coil in the lower conjunctival fornix of a human cadaver. Thanks to its high flexibility, the ocular coil bends along the circumference of the eye. Because of its location deep in the fornix, it appears unlikely that in vivo, the ocular coil will interfere with eye movements. In vitro drug release experiments demonstrate the potential of the ocular coil as sustained drug delivery device for the eye. We developed PMMA microspheres with a 26.5 ± 0.3 wt% ketorolac encapsulation efficiency. After 28 days, 69.9% ± 5.6% of the loaded ketorolac was released from the ocular coil when tested in an in vitro lacrimal system. In the first three days high released dose (48.7% ± 5.4%) was observed, followed by a more gradually release of ketorolac. Hence, the ocular coil seems a promising carrier for ophthalmic drugs delivery in the early postoperative time period. [ABSTRACT FROM AUTHOR]

Purpose: The purpose is to identify the various causes of ocular disease and surgical interventions undertaken in children (0-18 years of age) seen during an annual ophthalmic surgical trip to Belize. Methods: A retrospective, cross-sectional review of records from seven annual surgical trips conducted by the World Pediatric Project providing pediatric ophthalmologic clinical and surgical care to Belize from 2013 to 2019 was performed. Results: The charts of 313 patients who received care during the surgical mission trips were examined. From these 313 patients, there were 545 independent clinical encounters, yielding 405 diagnoses. Patients aged 6-10 years were most commonly seen (32.1%) followed by those 2-5 years old (28.1%). The most common pathophysiology seen was strabismus/ocular motility disorder (151 diagnoses; 37.4%), followed by refractive issues (69 diagnoses; 17.1%), eyelids and/or lacrimal system disorders (53 diagnoses; 13.1%), disorders of the retina/vitreous (30 diagnoses; 7.4%), lens abnormalities/cataract (28 diagnoses; 6.9%), amblyopia (23 diagnoses; 5.7%), disorders of the cornea/conjunctiva (23 diagnoses; 5.7%), trauma (13 cases; 3.2%), conditions affecting the globe/orbit (7 diagnoses; 1.7%), other (6 diagnoses; 1.5%), and glaucoma (1 diagnoses, 0.2%). Conclusion: This study identified the most frequently diagnosed ocular conditions encountered during annual surgical mission trips to Belize. The most prevalent pathophysiology encountered was strabismus/ocular motility disorders, followed by refractive issues. The most common refractive issues encountered were myopia and astigmatism.
