La investigación sobre la pandemia de COVID-19, se ha estudiado en tiempo real, ha sido y sigue siendo reveladora. Objetivo. Analizar la morbilidad y la mortalidad por COVID-19, asociadas a factores de riesgo metabólicos en población no indígena e indígena de México. Materiales y métodos. Utilizamos la Base Nacional de Datos COVID-19, durante los años críticos 2020-2021- 2022. Se trabajó con 5.380.247 casos que representaron la población total de positivos al SARS-CoV-2. Se analizaron las discrepancias entre las prevalencias de población no indígena, población indígena, defunción y no defunción. Se definió población indígena, con la clasificación oficial de auto-identificación. Se aplicó el modelo de regresión logística para determinar el riesgo de morir para cada variable: enfermedades cardiovasculares, hipertensión, diabetes, obesidad, sexo, edad y condición indígena. El análisis de multicolinealidad se analizó a través de la prueba de asociación Phi para variables dicotómicas y a través del ajuste de Nagelkerke. Resultados. En los positivos totales 99,2% fue población no indígena y 0,8% indígenas, mientras su porcentaje de letalidad fue de 5,8% y 11,1% respectivamente. En ambos grupos, murieron más hombres (61,5%) que mujeres (38,5%) y las edades de mayor defunción fueron 60 a 79 años. La mortalidad por enfermedades cardiovasculares fue la de mayor incidencia, 26,6% en población general y 32,3% en indígena; por diabetes 22,1% y 27,9%; hipertensión 20,0% y 26,7%y la obesidad 11, 3% y 17,4% respectivamente. Los análisis de regresión logística se ajustaron por sexo, edad y condición indígena. El condicionante de mayor riesgo de muerte, fueron las comorbilidades metabólicas y el de menor riesgo, la condición indígena. Conclusiones. El impacto de la pandemia por COVID-19 fue más grave cuando hubo padecimientos metabólicos tanto en la población no indígena como en la indígena.

BACKGROUND: Visceral fat has greater metabolic activity in obesity. Recent studies in Mexico show that the waist-to-height ratio (WHtR) is the best marker for diagnosing central obesity in children. Obesity is a risk factor for hypertension, which has its origins in childhood. OBJECTIVE: To determine the association between central obesity in children and hypertension in a Family Medicine Unit. MATERIAL AND METHODS: Association study, conducted in July 2021 in children aged 6 to 11 years, in which obesity was determined by WHtR. Two groups of schoolchildren were established: those with central obesity as determined by waist-to-height ratio and participants without obesity. This were selected by nonrandom/non-probabilistic sampling for convenience. Sociodemographic and anthropometric variables were studied. Descriptive statistics, student t, χ² and odds ratio were used. RESULTS: Seventy-five school-age children with obesity were studied and were found to have association was found of central obesity and hypertension in children, with a P<0.001 value, OR of 11.13. Obesity and overweight are defined as: “the abnormal or excessive accumulation of fat that can be detrimental to health”. The increase in the percentage of body fat is directly associated with the appearance of multiple comorbidities and serious consequences, since it predisposes or favors the development of cardiovascular risk factors, such as arterial hypertension, dyslipidemia, glucose intolerance, diabetes, heart disease and other related diseases. This document aims to synthesize information concerning overweight and obesity in adults so that general practitioners, specialists and subspecialists within their clinical practice can provide adequate diagnosis, treatment, primary and secondary prevention based on the best information and scientific evidence for decision making and reduce the economic and epidemiological burden caused by this disease to the Mexican population. Introducción: La Hipertensión arterial representa un problema de salud pública a nivel universal. Objetivo: puntualizar la ocurrencia en la población de la Hipertensión Arterial en el Consultorio Médico Familiar # 9. Métodos: se efectuó un estudio descriptivo y transversal en pacientes precisados con Hipertensión Arterial en el Consultorio Médico familiar # 11 en el período incluido entre enero del 2019 a diciembre 2019. El universo estuvo compuesto por 1375 pacientes, de los cuales se adquirió una muestra de 280 pacientes diagnosticados con la enfermedad. Los datos se obtuvieron a partir de las historias clínicas de los pacientes y el registro de control de consultas. Se manejaron variables como: sexo, edad, factores de riesgo. Resultados: se pudo apreciar que la población objeto de estudio simbolizó el 20, 4 % del total de población del consultorio médico. El sexo femenino tuvo una mayor incidencia para un 57,1 %. Por rango de edad la más afectada fue la de 60-79 años para un 53,9 % y los factores de riesgos que tuvieron una mayor prevalencia fueron los antecedentes patológicos familiares (57,1 %) y la obesidad para un 48,9 %. Conclusiones: El estudio corroboró que el sexo femenino tuvo una mayor afectación y la edad sobre la que más incidió esta enfermedad fue la de 60-79 años de edad; dentro de los factores de riego que favorecieron la aparición de la Hipertensión arterial se encuentran los antecedentes patológicos familiares y la obesidad.

Introducción: Las investigaciones dirigidas a la identificación de los factores de riesgo aterogénico en niños y adolescentes, demuestran que las enfermedades, como consecuencia de la aterosclerosis en el adulto, se generan desde edades tempranas de la vida. Objetivo: Identificar las señales aterogénicas tempranas en adolescentes. Método: Se realizó una investigación observacional, descriptiva, de corte transversal, que incluyó 298 adolescentes de 11 a 16 años. Se identificaron las variables sobrepeso-obesidad, hipertensión arterial, tabaquismo, inactividad física y dieta inadecuada. Resultados: El 20,5 % de los adolescentes resultaron prehipertensos y el 2,3 % hipertensos. El 40,3 % tenía alteraciones del peso corporal y 44,3 % obesidad abdominal. La señal aterogénica temprana más frecuente fue la dieta inadecuada en el 30 %. Conclusiones: Las señales aterogénicas tempranas más frecuentes son la obesidad abdominal, el sobrepeso y el sedentarismo, en ese orden.

Se hace necesario continuar argumentando acerca de las relaciones fisiopatológicas entre el sedentarismo, la hiperreactividad cardiovascular y la obesidad como factores de riesgo de enfermedades cardiovasculares, que se manifiestan de manera interdependiente y cuya prevalencia en la actualidad sigue siendo elevada. El presente trabajo tuvo como objetivo argumentar las relaciones fisiopatológicas entre la hiperreactividad cardiovascular, la obesidad y el sedentarismo. Para ello se consultaron un total de 24 referencias bibliográficas entre fuentes primarias y secundarias, así como otras fuentes de información a las que se accedieron por medio de los principales gestores. Los factores de riesgo de las enfermedades cardiovasculares son aditivos. El sedentarismo y la obesidad constituyen factores estrechamente relacionados entre sí, a su vez ellos se asocian a un aumento de la actividad del eje simpático, con el inicio de una hiperreactividad cardiovascular como fenómeno que aumenta el riesgo de padecer hipertensión arterial y otras enfermedades cardiovasculares.

It is necessary to continue arguing the pathophysiological relationships between a sedentary lifestyle, cardiovascular hyperreactivity and obesity as risk factors for cardiovascular diseases, which are interdependent and whose prevalence currently remains high. The objective of this study was to argue the pathophysiological relationships between cardiovascular hyperreactivity, obesity and sedentary lifestyle. For this, a total of 24 bibliographic references were consulted between primary and secondary sources, as well as other sources of information that were accessed through the main managers. Risk factors for cardiovascular diseases are additive. Sedentary lifestyle and obesity are factors closely related to each other; in turn they are associated with increased activity of the sympathetic axis, with the onset of cardiovascular hyperreactivity as a phenomenon that increases the risk of suffering from high blood pressure and other cardiovascular diseases.

Objective. To identify the clinical and epidemiological characteristics related to lethality in patients hospitalized for COVID-19 at the Simón Bolívar Hospital in Cajamarca, during June-August 2020. Materials and Methods. This was a retrospective cohort, that used information collected from clinical records and official epidemiological surveillance systems (NOTIWEB, SISCOVID and SINADEF), on hospitalized patients with confirmed COVID-19 diagnosis. Information was collected on sociodemographic and clinical factors, considering discharge (death/survival) and days of hospitalization as outcomes. Results. The in-hospital fatality rate was 39.6%, and significant differences were found between hospitalization time and status at discharge in people over 60 years of age (p0.001). Age older than 60 years (HR: 2.87; 95% CI: 1.76-4.68),) unattended ICU bed request (HR: 3.49; 95% CI: 2.02-6.05), oxygen saturation less than 80% at admission (HR: 2.73; 95% CI: 1.35-5.53) and the use of ivermectin (HR: 1.68; 95%CI 1.06-2.68) were factors associated with lethality. The chosen ML model considered that the most important variables were oxygen saturation, age over 60 years, time of hospitalization and time of the onset of symptoms. Conclusion. The factors that could increase lethality in patients hospitalized for COVID-19 were age, oxygen saturation less than 80%, use of ivermectin as part of hospital treatment and unattended request for ICU beds. Future studies with better representativeness could confirm these possible relationships at the regional or national level.

Introducción. Los problemas relacionados con los trastornos metabólicos se han incrementado en la actualidad en población joven. Promover un estilo de vida saludable en estudiantes universitarios puede ayudar a prevenir diabetes, enfermedades cardiovasculares y diferentes tipos de cáncer. Objetivo. Calcular el Índice de Masa Corporal (IMC) de universitarios para identificar la prevalencia de obesidad e hipertensión como factores de riesgo metabólico. Materiales y método. Se utilizó muestreo probabilístico aleatorio simple (n= 282, edad 19,1 ± 1,8, 54,6% mujeres, 48,5% hombres). Se realizaron mediciones de peso y estatura para calcular el IMC, circunferencia de cintura y presión arterial. Resultados. Los resultados promedio para mujeres y hombres respectivamente fueron: IMC 24,0 ±5,5 y 25,2 ±5,2 kg/m2 (p= 0,25); cintura 76,25 ± 11,87 y 82,32 ± 15,85 cm (p= 0,001), indicando un riesgo bajo en 73,1% de mujeres y 82,5% de hombres; y presión arterial 127,73/86,03 y 142,3/94,2 mmHg (p= 0,001). La probabilidad de tener hipertensión arterial fue mayor al tener una relación cintura/estatura (RCE) alterada en mujeres y hombres (OR=3,037; IC95% 1,385-6,657 y OR=3,664; IC95% 1,249-2,179, respectivamente). Conclusiones. La población universitaria presenta factores de riesgo que pueden derivar en enfermedades crónicas no transmisibles en un futuro próximo, por lo cual es necesario modificar sus estilos de vida. La RCE es un buen predictor de riesgo de hipertensión en esta población.

Introduction: childhood obesity is associated with an increased risk of chronic diseases. We aimed to examine the relation between serum levels of 25-hydroxyvitamin D (25[OH]D) and blood pressure in obese schoolchildren. Material and methods: a cross-sectional study in school-age children with obesity. The serum levels of 25(OH)D were measured and classified as sufficient or insufficient/deficient. Blood pressure was measured. Normal values were considered < 90th percentile, elevated blood pressure ≥ 90th to < 95th percentiles, and hypertension ≥ 95th percentile, according to blood pressure reference tables specific for age, sex, and height. A Pearson correlation was performed. Results: a total of 256 obese schoolchildren (123 [48.0 %] females and 133 [51.9 %] males) were evaluated. The prevalence rates of vitamin D deficiency, insufficiency, and sufficiency were 23.4 %, 52.3 %, and 24.2 %, respectively. Normal blood pressure was observed in 101 (39.4 %) children; the frequencies of elevated blood pressure and hypertension were 10.9 % and 49.6 %, respectively. A moderate inverse correlation of 25(OH)D levels with systolic blood pressure levels (r = -0.54, p = 0.03) was observed. When analyzed by sex, a significantly high inverse correlation between 25(OH)D levels and systolic blood pressure was observed in males (r = -0.85 and p = 0.001). No significant correlation was found in females (systolic r = -0.16 and p = 0.67; diastolic r = -0.15 and p = 0.812). When performing the multiple regression analysis, the 25(OH)D levels and BMI were the significant predictors for systolic blood pressure. Conclusions: we identified an inverse correlation between 25[OH]D levels and systolic blood pressure in male schoolchildren with obesity. [ABSTRACT FROM AUTHOR]

Introduction: arterial hypertension at early ages is an important pathology that can lead to disorders in the normal development of infants and if not treated promptly can evolve to chronicity and trigger secondary pathologies such as renal failure and heart disease. Objective: to clinico-epidemiologically characterize pediatric patients diagnosed with arterial hypertension. Methods: an observational, descriptive and retrospective cross-sectional study was conducted on the behavior of arterial hypertension in children and adolescents attended at the "Hermanos Cordové" hospital in the period from January 2015 to December 2018. The sample was selected by non-probabilistic purposive sampling, it was constituted by 48 children and adolescents. Results: the male sex was the most affected for 58.3 %; while by age ranges the most affected were patients aged 15-19 years for 50 %. The white race was the most affected with 62.5 %. The most predominant cardiovascular risk factor and clinical manifestation were obesity and headache (37.5% and 95.8%, respectively). The most predominant type of arterial hypertension was primary hypertension with 91.6 %. Conclusions: the study showed that the most affected sex was male and that the white race was the most affected. By age groups, the most affected were those aged 15-19 years. The cardiovascular risk factor and the most frequent clinical manifestation were obesity and headache.

Antecedentes: el sobrepeso y la obesidad son importantes factores de riesgo de hipertensión y diabetes tipo 2. Además, imponen una carga significativa sobre el sistema nacional de salud. Objetivo: evaluar la correlación entre el índice de masa corporal (IMC) y la circunferencia de la cintura (CC) con la glucemia de glucosa en ayunas por presión arterial en residentes de la fracción Norma Luisa del municipio de Minga Guazú, Paraguay en 2013. Métodos: Estudio transversal que incluyó a 280 participantes entre 18 y 64 años, de la comunidad Norma Luisa del municipio de Minga Guazú, Paraguay, entre septiembre de 2012 y mayo de 2013. Desde una perspectiva sociológica, y sobre la base de datos publicados por la Secretaría de Salud sobre la pandemia de COVID-19 en México, se describe la situación de los servicios médicos, la infraestructura de salud y los efectos de la obesidad, la diabetes y la hipertensión en el significativo número de muertes. Se analiza la probabilidad de infectarse y fallecer en asociación con dichas comorbilidades. Resultados: desde la década de los 80 no hay un cambio significativo en los servicios, ni en la infraestructura para la salud; en ese periodo hubo un incremento exponencial de enfermos de diabetes, de hipertensión y obesidad; del total de muertos por COVID-19 registrados hasta la fecha de análisis, el 65.8% padecía al menos una de las tres co-morbilidades; finalmente, la probabilidad de fallecer por COVID-19 se incrementa en los hombres mayores de 54 años que padecen alguna co-morbilidad.

Antecedentes. Las enfermedades crónicas no transmisibles, en particular la diabetes, hipertensión y obesidad son enfermedades cuya prevalencia a nivel mundial, y también en México, ha tomado dimensiones de pandemia, ya que, pese a los diferentes modelos clínicos de atención aplicados, los cuales se basan principalmente en tratamientos nutricionales, farmacológicos y recomendaciones de cambio de estilo de vida, la prevalencia continúa creciendo. Identificar las barreras de los pacientes y del sistema de salud que afectan la adherencia terapéutica para el control de la enfermedad permitirá desarrollar modelos con mayor efectividad y eficacia. Objetivo. Realizar una revisión sobre las barreras de la adherencia terapéutica de pacientes con diabetes, hipertensión y obesidad en clínicas de atención de primer nivel. Método. Revisión bibliográfica, con búsquedas en las siguientes bases de datos: PubMed, Science Direct, Springer, Biblioteca Virtual en Salud (BVS) y Google Scholar. Se revisaron fuentes primarias de tres tipos: revisiones sistemáticas, ensayos clínicos y artículos de revisión. Para la estrategia de búsqueda se utilizaron estos términos: adherencia terapéutica, barreras de la adherencia terapéutica, enfermedades crónicas no transmisibles, diabetes, obesidad, hipertensión. El periodo seleccionado para la revisión bibliográfica es el comprendido entre el 2005 y el 2020. Resultados. Para el cumplimiento de la adherencia terapéutica existen diferentes barreras que no permiten el control de la enfermedad. Estas barreras pueden ser propias del paciente, pero también propias del sistema de salud que otorga el servicio, enfatizando el papel de los médicos para educar y orientar al paciente con respecto a su enfermedad y coadyuvar a su adherencia. Para identificar estas barreras existen diferentes instrumentos, los cuales han sido diseñados para diferentes enfermedades, por lo que para seleccionar el adecuado se debe considerar la enfermedad y el perfil de la población en la que se aplicará el cuestionario.

Introducción: El Síndrome Metabólico (SM) se considera actualmente una enfermedad multifactorial relacionada con la inflamación asintomática, insidiosa y deletérea que predispone al individuo a la vulnerabilidad al agregar marcadores de riesgo cardiovascular. Objetivo: analizar los factores asociados al síndrome metabólico y calidad de vida en adultos usuarios de una unidad de salud. Materiales y Métodos: estudio transversal realizado con 108 usuarios adultos. La recogida de datos se realizó mediante un cuestionario sociodemográfico, clínico y metabólico, estructurado y mediante el cuestionario The Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36). Para el diagnóstico de Síndrome Metabólico se utilizaron los siguientes criterios: aumento de la circunferencia abdominal e hipertensión arterial, diabetes, hipertrigliceridemia y colesterol HDL bajo. El análisis estadístico se realizó utilizando el software Statistical Package for Social Sciences (SPSS) 21.0. Resultados: Se identificó síndrome metabólico en el 88,0% de los adultos. De este total de usuarios evaluados con el síndrome, el 87,4% de los individuos eran mujeres; 71,7% con diabetes; 87,0% tenía hipertensión arterial; Se identificó sedentarismo en 53,7%. En la valoración del IMC, predominaron el sobrepeso y la obesidad en 68,4% y 24,9%, respectivamente. Los dominios con las puntuaciones más bajas de calidad de vida fueron Salud general y Vitalidad. Conclusiones: el estudio permitió identificar el Síndrome Metabólico en la mayoría de los adultos evaluados. Hubo una baja percepción de la calidad de vida entre los adultos en todos los dominios, excepto en los aspectos físicos y vitalidad. Por tanto, es necesaria la vigilancia y educación sanitaria de la población estudiada y la mejora de su calidad de vida. Introducción: la apnea obstructiva del sueño es una patología respiratoria caracterizada por paradas de la respiración de forma repetida durante el sueño. Esta patología está relacionada con los trastornos hipertensivos, pudiendo agravarlos. Objetivo: determinar el perfil de un paciente hipertenso bajo sospecha clínica de apnea obstructiva del sueño en el Centro de Diagnóstico y Medicina Avanzada y de Conferencias Médicas y Telemedicina (CEDIMAT) durante el periodo enero 2019 – febrero 2020. Material y métodos: se realizó un estudio observacional-descriptivo, transversal con datos retrospectivos. Se estudiaron 38 expedientes clínicos de pacientes hipertensos de la consulta de neumología y que cumplen con criterios de sospecha clínica de apnea obstructiva del sueño. Resultados: el rango de edad fue de 40-64 años con un 55.26 %. El 66 % son masculinos. 36.84 % son hipertensos grado I mientras que otro 34.21% tienen sus niveles tensionales normales. El 68.41 % son obesos y un 26.32 % están en sobrepeso. 63.16 % se clasifican como IV en la escala de Mallampati. Conclusión: el perfil de los pacientes hipertensos bajo sospecha clínica de apnea obstructiva del sueño es: sexo masculino, adultez media, obesidad, escala de mallampati categoría IV y el uso de múltiples fármacos en el control de la hipertensión.

To identify the occurrence of obesity, high blood pressure, and altered blood glucose levels among women with breast cancer; to evaluate the distribution of these comorbidities in relation to age, time since breast cancer diagnosis, waist circumference, hormone therapy, and chemotherapy. Cross-sectional study with 67 women with breast cancer. An instrument with sociodemographic and clinical variables, anthropometric assessments, and blood collection for fasting glucose was used. Fisher's exact test identified the relationship between qualitative variables and the Mann-Whitney and Kruskal-Wallis tests for continuous and categorical measures. Among the participants, 34.3% were pre-obese and 29.9% had parameters of mild to severe obesity, 53.7% were hypertensive and 20.9% were diabetic. An association was found between age and arterial hypertension; waist circumference and arterial hypertension; blood glucose levels and waist circumference; body mass index and waist circumference; age and diabetes mellitus; and age and waist circumference. Introducción: La presión arterial elevada es una categoría de presión arterial, poco estudiado en adultos jóvenes y está asociado a otros factores de riesgo cardiovascular. Objetivos: Estimar la prevalencia de presión arterial elevada y otros factores de riesgo cardiovascular en universitarios de la Universidad Nacional de Asunción-Paraguay.Material y métodos: Estudio transversal en universitarios de primer curso de las carreras relacionadas a la salud de la Universidad Nacional de Asunción. Se consideró presión arterial elevada a una presión arterial sistólica de 120-129 mmHg y/o presión arterial diastólica de ?80 mmHg. Se midieron las variables antropométricas, lipídicas, metabólicas y se determinó el hábito tabáquico y de alcohol.Resultados: Se incluyeron en el análisis 284 universitarios (edad media [±DE] 19,4 ±2,1 años; 73,2% fueron mujeres). La prevalencia global de la presión arterial elevada fue del 25,5%; (IC del 95%: 20,5-30,5%) (hombres 40%; mujeres 29%). La presión arterial elevada se asoció con la circunferencia abdominal aumentada (OR: 1,031; IC del 95%: 1,005-1,058; p= 0,024); el c-HDL baja (OR: 1,355; IC del 95%: 1,054-1,743; p=0,018); la obesidad, OR: 2,124; IC del 95%: 1,334-3,941; p=0,007) y la DM2 (OR: 4,431; IC del 95%: 3,642-7,963; p=Conclusiones: alta prevalencia de la presión arterial elevada y está asociado con otros factores de riesgo cardiovascular.

La Encuesta Demográfica de Salud Familiar (ENDES) es una encuesta nacional de base poblacional con representatividad a nivel departamental y área de residencia, constituyéndose en una fuente de información del estado de salud de la población peruana. Con el objetivo de estandarizar su procesamiento y posterior reutilización por parte de la comunidad académica y otros actores interesados; documentamos el código para la manipulación, análisis y visualización de datos del cuestionario de salud de la ENDES 2017, mediante un ejemplo sobre prevalencia de hipertensión arterial y obesidad, utilizando el entorno y lenguaje de programación estadístico R. Se presenta y detalla secuencialmente el código en R, así como, el sustento teórico de la estructura de la encuesta para la manipulación de las bases de datos, considerando que la compleja estructura de la ENDES podría ser una potencial barrera que enfrentan los investigadores. Finalmente, este ejemplo puede servir de base para que se generen mayores estudios basados en la ENDES que sean relevantes para la toma de decisiones en salud pública.

Introducción: La obesidad tiene consecuencias para el sistema de salud por su asociación con la morbimortalidad y la afectación de la calidad de vida. A nivel individual genera proteinuria y glomeruloesclerosis, que aceleran la disminución de la filtración glomerular. Objetivo: Determinar la asociación entre obesidad y tasa de filtración glomerular en población con hipertensión arterial. Material y método: Estudio descriptivo de corte transversal, que analizó la obesidad y filtración glomerular en 3.197 pacientes con hipertensión arterial de una institución de primer nivel de atención. Se revisaron historias clínicas obteniendo variables sociodemográficas, antropométricas, parámetros clínicos, y clasificación de la enfermedad renal crónica. Resultados: La población tuvo un promedio de edad de 64±12,47 años, y en su mayoría fueron del sexo femenino (70,58%). La prevalencia de obesidad grado I fue (26,11%), grado II (9,63%) y grado III (4,56%). Los obesos tuvieron una mediana de la tasa de filtrado glomerular superior comparado con los no obesos, pero dicha diferencia solo fue observada en los estadios de ERC I y II. Conclusión: Existe asociación entre obesidad e incremento de la tasa de filtrado glomerular solamente en estadios I y II. El perímetro abdominal, la tensión arterial no controlada y el LDL fueron variables que influyen en el promedio de la tasa de filtrado glomerular, pero tienen un comportamiento diferente por grado de enfermedad renal crónica.

Introduction: Metabolic Syndrome (MS) is currently considered a multi-factorial disease related to the asymptomatic, insidious, and deleterious inflammation that predisposes the individual to vulnerability by aggregating cardiovascular risk markers. Objective: to analyze the factors associated with Metabolic Syndrome and Quality of Life (QOL) in adult users of a health unit. Material and Methods: a cross-sectional study carried out with 108 adult users. Data collection was performed using a sociodemographic, clinical, and metabolic structured questionnaire and The Medical Outcomes Study 36-item Short-Form Health Survey (SF-36) questionnaire. For the diagnosis of Metabolic Syndrome, the following criteria were used: increased abdominal circumference and arterial hypertension, diabetes, hypertriglyceridemia, and low HDL-cholesterol. Statistical analysis was performed using the Statistical Package for Social Sciences (SPSS) 21.0 software. Results: Metabolic Syndrome was identified in 88.0% of the adults. Of this total of users evaluated with the syndrome, 87.4% of the individuals were female; 71.7% had diabetes; 87.0% had arterial hypertension; sedentary lifestyle was identified in 53.7%. In the assessment of the BMI, overweight and obesity predominated in 68.4% and 24.9%, respectively. The domains with the lowest quality of life scores were General Health and Vitality. Conclusions: The study made it possible to identify the Metabolic Syndrome in most of the adults evaluated. There was a low perception of quality of life among adults in all domains, except for physical aspects and vitality. Introduction: COVID-19 has caused millions of deaths around the world since its appearance in Wuhan, China in late 2019. It reached Mexico in early 2020 after it caused many casualties in western Europe. Mexico is now one of the countries with the largest death rate due to COVID-19. Mexico City is not only the largest city in Mexico, but one of the largest in the world. Fortunately, its data regarding COVID-19 patients is publicly available and can be analyzed to obtain information that may guide public policy and personal decisions. Method: This work reports the findings of a descriptive study based on data retrieved July 28th, 2020 from the Mexico City government web page that reports suspected COVID-19 patients. Only confirmed cases were analyzed. To calculate the proportion of cases and deaths by age range in Mexico City, the population pyramid of Mexico City was used. To obtain the proportion of cases and deaths relative to population by age range, cases and deaths were adjusted to match those reported by CONACYT. Case and death frequency by gender, age range and previous health condition were calculated using a spreadsheet program. Tables and graphs were obtained using the same program. Results and discussion: Age, gender and previous health conditions have an effect in death increase among confirmed COVID-19 patients, so that: older people have a higher mortality rate than younger people, males have a higher mortality rate than females and people with certain previous health conditions such as hypertension and diabetes have a higher mortality rate than people with no previous health conditions. Some examples are regarding the effect of age, COVID-19 patients 61 to 70 years old are twice as likely to die than those 51 to 60 years old; regarding the effect of gender, male patients 51 to 60 years old are more than twice as likely to die than female patients their same age and, regarding the effect of previous health conditions, female patients with hypertension are six times as likely to die than female patients with no previous health condition. Conclusion: According to the information available and analyzed in this work, age, gender, and previous health condition are important causes for death rate among COVID-19 patients in Mexico City. Introduction: Chronic renal insufficiency is related to multiple vascular risk factors. The prevalence of these entities has increased in Cuba in recent decades, associated with population aging. Objective: To identify the relationship between chronic renal failure, diagnosed according to glomerular filtration stages, with vascular risk factors (obesity, high blood pressure, diabetes mellitus, hyperlipoproteinemia, smoking) and established cardiovascular disease. Methods: A descriptive, cross-sectional study was carried out in 2913 patients. General, anthropometric data, vascular risk factors, and hemochemistry were collected. Carriers of chronic renal insufficiency were considered those with glomerular filtration rate lower than 60 mL / min / 1.73 m2. The relationship between chronic renal insufficiency and vascular risk factors was identified using the Odds Ratio. The mean values of the hemochemical variables were compared with one-way ANOVA. Results: 10.8% of chronic renal insufficiency (15.3% in women) was found, its frequency increased with age and was associated with the selected risk factors (established cardiovascular disease: OR 3,58; hyperlipoproteinemia: OR 2,96; hypertension: OR 2,95; diabetes mellitus: OR 2,70 and obesity: OR 1,33). The frequency of chronic renal insufficiency as well as blood glucose, cholesterol, triglyceride and HbA1c levels increased with the sum of risk factors. Conclusions: chronic renal insufficiency was very frequent in this series. It prevailed in the female sex and in the most advanced age groups. A significant relationship with the selected risk factors was found, which was higher in patients with established cardiovascular disease.

Objetivos. Determinar las características clínicas y personales del síndrome metabólico (SM) en pacientes asegurados, adscritos a la Red Asistencial de ESSALUD (Seguro Social de Salud) en el Tambo, Huancayo. Métodos. Estudio descriptivo, observacional y retrospectivo, realizado desde enero a octubre del año 2015. Participaron 101 pacientes mediante criterios de inclusión: edad entre 18 y 60 años, cumplir 3 de los 5 criterios de SM según Federación Internacional de Diabetes (IDF) 2009 y Asociación Latinoamericana de Diabetes (ALAD), historias clínicas y encuestas completas, y criterios de exclusión: poseer enfermedad previa como diabetes e hipertensión arterial. Resultados. La frecuencia de SM fue de 14,98 %. El 62,4 % fueron mujeres y la media de edad fue entre 47 y 51 años. El 66,4 % proviene de la zona urbana. El 54,5 % fue catalogado con sobrepeso y el 23,6 % con obesidad grado I. La media del perímetro abdominal en mujeres fue de 98,63 cm y para varones 100,66 cm. El 68,3 % tuvo controles glicémicos mayores a 100 m/dL. La media de colesterol y triglicéridos fue de 234,26 mg/dL y 251,94 mg/dL respectivamente. El 51,5 % de los pacientes encuestados no tienen un nivel adecuado de conocimientos de factores de riesgo cardiovascular, 69,3 % tiene estilos de vida inadecuados y el 58,41 % refiere tener un nivel bajo de estrés. Conclusión. La frecuencia del SM es baja, con predomino en el sexo femenino; siendo el sobrepeso y la obesidad los factores más relacionados al estilo de vida inadecuado.

La energía necesaria para la regulación de las funciones fisiológicas depende del equilibrio entre el aporte y el gasto energético. Un disbalance entre estas condiciones, donde se vea favorecido el consumo sobre el gasto, lleva a un incremento de la reserva, el cual, a su vez, favorece la hipertrofia e hiperplasia del tejido adiposo; en condiciones crónicas, este reservorio energético lleva a la obesidad. A través del uso de fórmulas, se ha tratado calcular la tasa metabólica basal para considerar el aporte óptimo energético y, por ende, la individualización en la prescripción de estrategias nutricionales en el paciente sano y con enfermedades crónicas no transmisibles (ECNT). Sin embargo, estas fórmulas no han sido evaluadas ni validadas para todas las poblaciones y menos en obesidad, que es, de forma coincidente, la población donde más se usan en la práctica clínica, además del poco conocimiento en medicina y la mayor aplicación por nutrición. El propósito de este artículo es revisar los elementos que constituyen las fórmulas de cálculo de la tasa metabólica basal (TMB) y la población de estudio para evidenciar la usabilidad de estas fórmulas en la práctica clínica en pacientes sanos y con ECNT, como la obesidad y diabetes mellitus, dada la importancia que esto representa en el contexto del balance energético.

RESUMEN Introducción: La obesidad se relaciona con múltiples consecuencias adversas para la salud, como hipertensión, diabetes, hiperlipoproteinemia, enfermedad cardiovascular y otras. La prevalencia de estas entidades se ha incrementado en Cuba en las últimas décadas, muy asociadas a la ganancia ponderal. Objetivo: Describir la relación de la obesidad con la enfermedad cardiovascular y los factores de riesgo metabólicos como hipertensión arterial, hiperlipoproteinemia y diabetes mellitus. Métodos: Se realizó un estudio descriptivo, de corte transversal, en 2902 pacientes que acudieron a chequeo médico en el Hospital Militar Central “Dr. Carlos J. Finlay”. Se recolectaron datos generales, antropométricos y factores de riesgo metabólico de enfermedad cardiovascular. Se realizó glucemia en ayunas, colesterol, triglicéridos, creatinina y prueba de tolerancia a la glucosa oral en casos indicados. Se calculó el filtrado glomerular. Los pacientes fueron clasificados en bajo peso, normo peso, sobrepeso y obeso, se identificó la relación entre el estado nutricional y los factores de riesgo metabólico y la enfermedad cardiovascular. Resultados: Se encontró 44,5 % de sobrepeso, más frecuente entre los hombres (45,5 %) y 29,2 % de obesidad, más frecuente entre las mujeres (31,6 %). El índice de masa corporal aumentó progresivamente con la edad. Los valores de glucemia, colesterol, triglicéridos, HbA1c y filtrado glomerular aumentaron con el estado nutricional, así como la frecuencia de diabetes, hipertensión, hiperlipoproteinemia y enfermedad cardiovascular. Conclusiones: La obesidad fue muy frecuente en este grupo de pacientes, en los cuales se relacionaron directamente los factores de riesgo metabólico hipertensión arterial, hiperlipoproteinemia, diabetes mellitus y enfermedad cardiovascular.

Objetivo. Determinar la frecuencia y características de una población de pacientes obesos con y sin riesgo cardiometabólico (RCM) y revisar el concepto de obeso cardiometabólico normal (OCMN). Materiales y Métodos. Estudio analítico, transversal, de 105 pacientes obesos, de 30 a 75 años, de ambos géneros. Recolectados de un programa preventivo promocional de mayo de 2015 a abril del año 2016. Previo consentimiento informado, se registró la edad, género, medidas antropométricas, presión arterial (PA), antecedentes personales y familiares de factores de riesgo cardiovascular (FRCV). En ayunas, se determinó en sangre glucosa (G), colesterol total (CT), colesterol HDL, triglicéridos (Tg), creatinina, proteínas plasmáticas, bilirrubinas, transaminasas (AST,ALT), fosfatasa alcalina (FA) y gamaglutamiltranspeptidasa (GGTP), por métodos convencionales; las fracciones VLDL, LDL y NoHDL fueron calculados. Se empleó el programa SPSS.23 para el análisis estadístico Resultados. El 77,4% fueron mujeres, edad promedio 51±0,86 años. El 45,7% tuvo obesidad grado I, 24,7% grado II y 29,5% grado III. Las mujeres tuvieron menor talla, peso y circunferencia abdominal (CA) y cifras más altas de HDL que los varones, no hubo diferencias en otros parámetros. El 89,2 % de mujeres y 87% de varones tuvieron dislipoproteinemia; hipertensión arterial (HTA): mujeres 20,7% y varones 21,4%; tabaquismo: mujeres 5,5% y varones 42,3%; hiperglicemia: mujeres 14,2% y varones 14,2%; macrosomía/bajo peso al nacer: mujeres 6,9%, varones 0; enzimas hepáticas elevadas: mujeres 29,8%, varones 32,1%. El 9,5% fueron OCMN por tener PA, G, cHDL, Tg, cLDL cNoHDL normales. Conclusiones: La mayoría de los pacientes obesos tuvieron RCM, solo 10 (9,5%) fueron OCMN.

Introduction: High blood pressure (HBP), diabetes (DM2), obesity, dyslipidemia and lifestyle habits such as diet, physical activity, alcohol consumption and smoking are risk factors for the development of chronic non-communicable diseases. This study aimed to determine the prevalence of risk factors, which are associated with cardiovascular diseases, in the population aged 18 to 64 years old living in the Ayacucho district and their relationship with modifiable behavioral factors. Materials and Methods: A descriptive cross-sectional correlational study was conducted in which the Ministry of Health’s “Research Model, Risk Factors of Noncommunicable Diseases, and Health-Related Quality of Life Measures” was applied to a sample of 412 people. Laboratory tests were conducted for glycemia and lipid panel, and weight, height (anthropometry) and blood pressure were recorded. The data were analyzed using Generalized Linear Mixed Models and Chi-squared tests with a 95% confidence level. Results: 64.8% of the population showed at least one risk factor for cardiovascular disease. Prevalences of 14.6 and 7.8% were observed for HBP and DM2, respectively, which significantly increase with age (p<0.05) and occur more frequently in men, alcohol abusers and smokers who are poorly fed with low levels of physical activity. Conclusions: HBP, DM2, dyslipidemia and obesity are associated with modifiable behavioral risk factors. If corrective actions are implemented, the risk of acquiring cardiovascular diseases will be reduced. Introduction: Introduction: sedentary behavior (SB) has been independently associated with detrimental health outcomes in different regions worldwide. The aim of this systematic review was to examine whether domain-specific SB is associated with cardiometabolic diseases (CMD) and related biomarkers in South American adults. Methods: nine electronic databases were searched to identify all studies that analyzed the association between SB and CMD-e.g. obesity, diabetes, hypertension, metabolic syndrome (MetS) and clustering of chronic diseases (CCD)-and related biomarkers in South American adults. Two independent reviewers performed the necessary abstract/full-text screening, data abstraction, and quality assessments. The review protocol was registered in the PROSPERO database (CRD42018099319). Results: from the 1,262 articles identified in the search 262 were reviewed in full and 20 were used in the analysis in accordance to the inclusion criteria. High SB (mainly sitting and TV time) was associated with an increased likelihood of obesity (n = 8), diabetes (n = 6), and CCD (n = 3), as well as high values of BMI (n = 8), WC (n = 7), % BF (n = 4), plasma lipids (n = 4), and glycemia (n = 5). Eleven out of 20 studies were of higher quality. Conclusion: long time spent in SB, mainly sitting and TV time, was positively associated with the occurrence of CMD and related biomarkers in South American adults. [ABSTRACT FROM AUTHOR]

In Brazil, the promotion, protection, and recovery of health are among the initiatives provided by the public and universal Unified Health System (SUS). The primary healthcare level (PHC), the preferred point of access to the system, plays the role of coordinating and ordinating health care services and actions in the network. In the context of PHC, feeding and nutrition efforts must be aligned with National Feeding and Nutrition Policy (PNAN) guidelines and may be supported by feeding and nutritional surveillance actions. Data from PHC information systems and population surveys show that excess weight affects more than half the adult population of Brazil, and that consumption of ultra-processed foods is on the rise. This scenario requires that health care teams be prepared to prioritize initiatives for users with chronic diseases, using risk stratification, stabilization of the disease, and enhancement of supported self-care with a focus on diet and physical activity. At the same time, considering the country's epidemiological profile, teams must carry out initiatives to fight undernutrition and prevent anemia and hypovitaminosis A to address the multiple burden of malnutrition. The present article describes the current scenario of feeding and nutrition initiatives implemented at the PHC level in Brazil via the SUS. [ABSTRACT FROM AUTHOR]

Objective. To estimate the cost attributable to arterial hypertension, diabetes and obesity in the Unified Health System of Brazil in 2018. Method. The study estimated the cost attributable to non-communicable chronic diseases based on relative risk and population prevalence of hypertension, diabetes, and obesity, considering the cost of hospitalizations, outpatient procedures, and medications distributed by the SUS to treat these diseases. Cost data were obtained from SUS information systems. The analysis explored the cost of disease according to sex and age in the adult population. Results. The total cost of hypertension, diabetes, and obesity in the SUS reached R$ 3.45 billion (95%CI: 3.15- 3.75) in 2018, that is, more than US$ 890 million. Of this amount, 59% referred to the treatment of hypertension, 30% to diabetes, and 11% to obesity. The age group from 30 to 69 years accounted for 72% of the total costs, and women accounted for 56%. When obesity was considered separately as a risk factor for hypertension and diabetes, the cost attributable to this diseases reached R$ 1.42 billion (95%CI: 0.98-1.87), i.e., 41% of the total cost. Conclusions. The estimates of costs attributable to the main chronic diseases associated with inadequate diet revealed a heavy economic burden of these disorders for the SUS. The data show the need to prioritize integrated and intersectoral policies for the prevention and control of hypertension, diabetes, and obesity, and may support the advocacy for interventions such as fiscal and regulatory measures to ensure that the objectives of the United Nations Decade of Action on Nutrition are met.
